Effects of Subglottic Secretion Drainage on the Morbidity of Ventilator-associated Pneumonia in Mechanically Ventilated Patients

[Objective]To assess the effects of subglottic secretion drainage on the morbidity of ventilator‐associated pneumonia (VAP) in mechanically ventilated patients .[Methods]A total of 84 patients undergoing mechanical ventilation in Emergency Intensive Care Unit (EICU ) were randomly divided into two groups of subglottic secretion drainage (SSD ,n =41) and routine care (non‐SSD ,n=41) .Bacterial culture of samples from oropharynx ,subglottic secretions and lower respiratory tract were obtained periodically .The bacterial a‐mount of subglottic secretion ,ventilation duration ,hospitalization time and count of VAP were recorded .Eti‐ologic diagnosis of VAP was based on the quantitative bacterial culture of secretions obtained by protected specimen brush (PSB) .[Results]Group SSD had a lower rate of early‐onset VAP ,fewer ventilated days ,less hospitalization time and lower mortality rate than those of non‐SDD group ( P <0 .05) .The bacterial amount of subglottic secretion was lower in SDD group than that in non‐SDD group ( P<0 .05) .And the pathogenetic organisms were similar in subglottic secretion and lower respiratory tract .[Conclusion] The presence of sub‐glottic secretion may be an origin of pathogenetic organisms of VAP .SDD may reduce the incidence of VAP and fatality rate ,delay the onset of VAP and shorten mechanical ventilation time .And SSD is a simple and ef‐fective method of preventing VAP .

Effects of Honokiol on Inhibition of Airway Inflammation During Acute Asthma and its Possible Mechanism

[Objective]To explore the effects of honokiol on inhibition of airway inflammation during acute asth‐ma and analyze its possible mechanism .[Methods]A murine model of acute asthma was established by stimulation and induction with VOA .After a treatment of honokiol ,total cells ,neutrophils ,lymphocytes ,macrophages and eosino‐phils cells were counted .And the levels of tumor necrosis factor‐alpha (TNF‐α) ,interleukin‐4 (IL‐4) ,immunoglob‐ulin E (IgE) and Eotaxin were detected by enzyme‐linked immunosorbent assay (ELISA) in bronchoalveolar lavage fluid .Meanwhile ,lung tissue was sectioned for hematoxylin & eosin staining to analyze the histopathological chan‐ges .Moreover ,the expressionx of NF‐κBp65 and I‐κb were detected by Western blot .[Results]Honokiol effectively reduced the total cell number in bronchoalveolar lavage fluid (30976 .00 ± 3865 .54 vs 64861 .67 ± 7613 .98) while de‐creased the counts of neutrophils ,lymphocytes ,macrophages and eosinophils count and the differences were statisti‐cally significant ( P < 0 .05) .The results of ELISA showed that honokiol effectively inhibited the expressions of TNF‐α,IL‐4 ,IgE and Eotaxin .And the differences were statistically significant ( P<0 .01) .The results of pathol‐ogy showed that the area of tissue damage and inflammatory cells were effectively alleviated after a treatment of hono‐kiol as compared with model group .As compared with model group ,the expressions of NF‐κBp65 and I‐κb decreased after a treatment of honokiol and the difference was statistically significant ( P <0 .05) .[Conclusion]Honokiol can significantly inhibit the inflammation and lung tissue damage induced by acute asthma .And its possible mechanism is through an inhibition of NF‐κB activation .

Effects of Geographic Region,Personal Hygiene and Intestinal Functions on the Incidence of Peritonitis in Patients of Continuous Ambulatory Peritoneal Dialysis
[Objective]To explore the effects of geographics ,personal hygiene and intestinal functions on the incidence of peritonitis in patients of continuous ambulatory peritoneal dialysis (CAPD) .[Methods]A total of 133 hospitalized pa‐tients on peritoneal dialysis at our hospital were recruited during 2008 to 2014 according to the criteria of peritonitis .They were divided into peritonitis and non‐peritonitis groups .Their general information (gender ,age ,place of residence ,edu‐cation level & primary disease ,etc .) and personal hygiene ,etc were recorded .And the relationship was analyzed between these factors and peritoneal dialysis‐related peritonitis .[Results]There were significant inter‐group differences in geo‐graphic region ( P<0 .05) .Risk factors associated with the development of peritonitis included region (township& rural areas) [OR=5 .876 ,P=0 .000] ,washing [OR=5 .596 ,P=0 .009] and brushing [OR=4 .865 ,P=0 .003] and diar‐rhea [OR=5 .744 ,P=0 .006] .[Conclusion]Geographical region ,personal hygiene and diarrhea affects the development of peritonitis .We should enhance the awareness of preventing peritonitis in rural patients and pay more attention to hy‐giene education for peritoneal dialysis patients .

Construction of Murine GSK3βLentiviral Expression Vector and its Expression in DC2 .4 Cell Li ne

[Objective]To construct pLVX‐IRES‐TDtomat‐GSK‐3βlentiviral expression vectors and infect murine bone marrow DC2 .4 cell line for establishing DC2 .4 cell line with an over‐expression of murine GSK‐3β.[Methods]Coding sequence in murine GSK‐3β cDNA was amplified by reverse transcription‐polymerase chain reaction (RT‐PCR) assay and recombined into pLVX‐IRES‐TDtomat plasmid .After confirming with re‐striction endonucleases and sequencing ,the recombinant plasmid was transfected into 293T cells with Lipo‐fectamine 2000 and then packaged into lentivirus particles .DC2 .4 cell line was infected by lentivirus particles . There were 3 groups of DC2 .4 cell line control ,non‐virus control and GSK‐3βlentivirus .The expression lev‐els of GSK‐3βamong three groups were detected by real‐time PCR and Western blot .[Results]Restriction en‐donuclease assay and sequence analysis verified the successful construction of recombinant vector pLVX‐IRES‐TDtomat‐GSK‐3β.And it was expressed highly in packaging cell 293T and the titer of recombinant lentivirus particles was 1 .26 × 108 TU/mL .GSK‐3βexpression level increased more in GSK‐3βlentivirus group than that in DC2 .4 cell line control and non‐virus control at the levels of mRNA and protein .[Conclusion]GSK‐3βlenti‐viral recombinant vector is successfully constructed and expressed stably in DC2 .4 cell line .The study may provide rationales for further elucidating the immunological functions of DC2 .4 .

Effect of Ultrasound Training on the Incidence of Birth Defects in Hunan Province

[Objective]To explore the effect of overall tracking training of ultrasound doctors on the inci‐dence of birth defects in Hunan province .[Methods]Doctors who engaged in fetal ultrasound screening in Hu‐nan province received overall tracking training .The monitoring results of birth defects before and after training were analyzed retrospectively .[Results] Total number of perinatal infants in 53 monitored hospital before (2007~2010) and after training(2011~2013) showed an upward trend .The incidence of birth defects with fe‐tuses ≥28 gestational weeks before training showed an upward trend ,but showed an down trend after train‐ing .Compared with 2012 ,the incidence in 2013 was decreased by 7 .49% .[Conclusion]The overall tracking training for ultrasound doctors contributes to improve the level of ultrasound doctors'diagnosis level in primary hospital and reduce the rate of birth defects .

Efficacy and Safety of S-1 Monotherapy for Non-small Cell Lung Cancer After Two or More Failed Prior Chemotherapies
[Objective] To explore the efficacy and safety of S‐1 monotherapy for patients with advanced or recurrent non‐small cell lung cancer (NSCLC) after the failure of two or more prior chemotherapy regimens .[Methods] Retrospective reviews were conducted for 43 patients with advanced or recurrent NSCLC on S‐1 monotherapy between August 2011 and October 2014 after failed previous chemotherapy .S‐1 was administered orally twice daily at days 1~14 every 3 weeks .The dose of S‐1 was 80 mg/day [body surface area (BSA) <1 .25 m2 ] ,100 mg/day (BSA ≥1 .25 and <1 .50 m2 ) or 120 mg/day (BSA ≥1 .50 m2 ) .[Results] The medi‐an number of prior chemotherapy regimens was 5 (range ,2~ 5) and the median course 4 (range ,1~ 8) . Treatment response was assessed in 38 patients .None achieved complete response .The outcomes were partial response ( n =5 ,11 .6% ) ,stable disease ( n =11 ,25 .6% ) and progressive disease ( n=22 ,51 .2% ) .The median progression‐free survival was 3 months (95% CI:2 .6 ~ 3 .4) and the median overall survival 5 .5 months (95% CI:4 .9~6 .1) .No grade IV hematological toxicity was noted .The side effects were generally mild and consisted of gastrointestinal reactions and hematological toxicity .No drug‐related death occurred .[Conclusion] S‐1 monotherapy is efficacious with acceptable toxicity as third‐line or subsequent chemotherapy for advanced NSCLC .

Clinical value of Percutaneous Sclerotherapy in Children with Complex Venous Malfunctions
[Objective]To explore the clinical characteristic ,diagnosis and treatment of children with complexvenous malformation .[Methods]Retrospective analyses were conducted for the clinical data of 76 cases ofcomplex venous malformation .And the efficacies and adverse reactions of percutaneous sclerotherapy were evaluated.[Results]A total of 235 sessions were offered at an average of 3 .09 sessions per case .The singledose of anhydrous ethanol was 2 ~ 10 mL .The average hospitalization time was 3 .5 days .During the postoperativefollow‐ups ,52 cases were cured and 17 cases improved markedly with an effective rate of 90 .8% .[Conclusion]Percutaneous sclerotherapy has the advantages of obvious efficacies ,safety ,little trauma ,short hospitalizationtime ,and better prognosis .Therefore it is worthy of further clinical promotions .

Detection of Hs-CRP in Hydatid Fluid,Cerebrospinal Fluid and Serum of Patients with Cranio-pharyngioma and Its Significance
[Objective]To explore the level of high‐sensitivity C‐reactive protein(hs‐CRP) in hydatid fluid , cerebrospinal fluid(CSF) and serum of craniopharyngioma and its significance .[Methods]Turbidimetry immu‐noassay and double‐antibody sandwich ELISA were used to measure the levels of hs‐CRP in hydatid fluid ,CSF and serum of 22 patients with ameloblast craniopharyngioma and 6 patients with squamous papillary cranio‐pharyngioma ,respectively .The results were analyzed by statistical method .[Results]The levels of hs‐CRP in hydatid fluid ,CSF and serum of ameloblast craniopharyngioma were (42 .81 ± 8 .96) × 105 pg/mL ,(0 .35 ± 0 .06)× 105 pg/mL and (17 .23 ± 5 .45)× 105 pg/mL respectively ,and those in squamous papillary craniophar‐yngioma were (34 .25 ± 9 .88)× 105 pg/mL ,(0 .34 ± 0 .06)× 105 pg/mL and (16 .07 ± 9 .47)× 105 pg/mL re‐spectively .The levels of hs‐CRP in hydatid fluid of craniopharyngioma were significantly higher than those in CSF and serum( P <0 .01) .There was no significant difference in hs‐CRP between two pathological types of craniopharyngioma( P >0 .05) .[Conclusion] The significantly higher level of hs‐CRP in hydatid fluid of cra‐niopharyngioma than that in cerebrospinal fluid and serum suggests that stimulation by inflammation sub‐stances in hydatid fluid may be related to the tumor tissue inflammation .There is no significant difference in hs‐CRP level of hydatid fluid among different pathological types of craniopharyngioma .It indicates that the in‐flammatory severity in tumor hydatid fluid can not explain the diversity of craniopharyngioma tissue inflamma‐tion with different pathological types .

Comparison of Curative Effect of Cataract Operation Through Corneoscleral Limbus Versus Clear Corneal Incisions

[Objective]To explore the curative effect of corneoscleral limbus versus clear corneal incisions during phacoemulsification and intraocular lens implantation .[Methods]A total of 120 cataract patients were selected from January 2011 to January 2014 to undergo phacoemulsification and intraocular lens implantation . They were divided into observation and control groups ( n=60 each) .The observation group underwent lim‐bal incision while the control group had clear corneal incision .Both groups were observed with regards to vi‐sion ,astigmatism and complications at Day 1 ,Week 1 ,Month 1 and Month 3 .[Results]The parameters of un‐corrected visual acuity and astigmatism were much better in observation group than those in control group . And the differences were statistically significant ( P <0 .05) .But no significant inter‐group difference existed in complications ( P > 0 .05 ) .[Conclusion]During phacoemulsification and intraocular lens implantation for cataract patients ,use of modified corneoscleral rim incision is significantly superior to that of clear corneal inci‐sion .The postoperative visual acuity is better with a smaller degree of corneal astigmatism .And it is worthy of wider clinical applications .

Clinical Study of Intestinal Mucosal Barrier Function in Gallstones
[Objective] To explore the changes of epithelial cell proliferation and apoptosis in gallstones with different distributions and properties and investigate the role of intestinal barrier function in gallstone for‐mation .[Methods]According to different distributions of gallstones ,there were normal control group (A1 ,n=20) ,gallbladder group (B1 ,n=36) ,bile duct group (C1 ,n=36) and combined group of gallbladders com‐plicated with bile duct (D1 ,n=36) .Also on the basis of different properties of gallstones ,there were normal control group (A2 ,n=20) ,cholesterol stone group (B2 ,n = 40) ,pigment stone group (C2 ,n =52) and combination calculus group of cholesterol stone complicated with pigment stone (D2 ,n =16) .The histology changes of terminal ileum mucosa and the rate of proliferation and apoptosis in intestinal mucosa cells were measured and analyzed .[Results] According to different distributions of gallstones ,no significant difference existed in pathohistology of intestinal mucosa injury and rate of proliferation and apoptosis among A1 ,B1 ,C1 and D1 groups .While with different properties of gallstones ,pathohistology of intestinal mucosa injury and rate of proliferation and apoptosis showed higher levels in C2 group versus B2 and A2 groups ( P≤0 .05) .But no difference existed between C2 and D2 groups ( P > 0 .05) .[Conclusion] Pigment stone has the grading changes of intestinal mucosa injury in pathohistology and proliferation and apoptosis in intestinal mucosa cells . And a probable correlation exists between pigment stone formation and damaged function of intestinal mucosal barrier .It is possible that this kind of intestinal injury play some roles in the pigment stone formation .

Characterization and Expression of Interleukin-6 in Remodeling of Nasal Mucosa in Allergic Rhinitis

[Objective]To explore the nasal mucosa remodeling and determine the changes in expression of interleukin‐6 (IL‐6) in patients with allergic rhinitis (AR) .[Methods]A total of 40 middle turbinate tissues samples were obtained from 15 healthy subjects ,15 AR patients and 10 AR asthmatics .The infiltration of eo‐sinophils in nasal mucosa and the damage of epithelium were examined by hematoxylin and eosin staining (HE) .Goblet cell hyperplasia was counted by alcian blue‐periodic acid‐Schiff (AB‐PAS) double staining .The extracellular matrix (ECM) was stained by masson's trichrome (MT) .And the percentage of stained EMC ar‐ea was processed by the software and transferred into Microsoft Excel for statistical analyses .The level of in‐terleukin 6 (IL‐6) in middle turbinate tissues was measured by enzyme‐linked immunosorbent Assay (ELISA) .[Results]The eosinophil infiltration ,goblet cells ,epithelial damage ,ECM deposition and level of IL‐6 signifi‐cantly increased in AR patients versus control group ( P < 0 .05) .Yet the extent of eosinophil infiltration , ECM deposition and the expression of IL‐6 were much higher in AR asthmatics than those of AR non‐asthmat‐ics ( P<0 .05) .[Conclusion] The remodeling of nasal mucosa is evident in AR patients .And IL‐6 plays some roles during tissue remodeling .Furthermore asthma may influence upper airway remodeling .

Analysis of the Correlation Between Homocysteine and Inflammatory Factors in Diabetic Ne-phropathy

[Objective] To explore the correlation between homocysteine(Hcy) and inflammatory factors in patients with diabetic nephropathy(DN) .[Methods]According to DN progression ,120 patients diagnosed as type 2 diabetes mellitus(T2DM) in our hospital were divided into DM with normal proteinuria group(DMA group) ,early DN group(DMB proup) and clinical DN group(DMC group) with 40 patients in each group .In addition ,40 healthy people were chosen as the control group .The levels of Hcy and inflammatory factors(hs‐CRP ,TNF‐α,IL‐6) in 4 groups were detected .[Results]Compared with the control group ,the levels of Hcy and inflammatory factors in DN group were obviously increased ,and there was significant difference( P <0 .01) .With the increasing of urine proteins in DN ,the levels of Hcy ,hs‐CRP ,TNF‐αand IL‐6 were signifi‐cantly increased by degrees ,and there was significant difference( P<0 .01) .The increasing of Hcy in DN pa‐tients was positively correlated with hs‐CRP ,TNF‐αand IL‐6( r =0 .420 ,0 .461 ,0 .429 ,P <0 .01) .[Con‐clusion]Hyperhomocysteinaemia and inflammatory factors may involve in the occurrence and development of DN .Hcy may promote the occurrence and development of DN through promoting body inflammatory status .

Effects of miRNA-29b on Cell Biological Properties in Prostate Cancer
[Objective]To explore the expression of miRNA‐29b (miR‐29b) ,examine its effects on cell bi‐ological properties in prostate cancer (Pca) and discuss its potential clinical values .[Methods]The expressions of miR‐29b in prostate cancer tissues and matched adjacent non‐tumor tissues were detected by real‐time quan‐titative polymerase chain reaction (QRT‐PCR) .The cellular growth and chemotherapeutic effect of cisplatin were measured by methyl thiazol tetrazolium (MTT) assay .Flow cytometry was used to determine the cell cy‐cle changes .And AV/PI double labeling method was utilized to detect the apoptosis of LNCap .Finally the im‐pact of miR‐29b on the invasiveness of LNCap cell was determined by Transwell invasion assay .[Results]The expression of miR‐29b was down‐regulated in Pca tissues versus matched adjacent non‐tumor tissues .Com‐pared with negative controls ,group of transfected miR‐29b showed lower tumor cell survival rate .Moreover , miR‐29b could increase the inhibition of cisplatin on LNCap with much lower cell survival rate .LNCap with transfected miR‐29b revealed a higher percentage of cells during sub‐G0/G1 phase and significant decreases of apoptosis cell rate and invasive cell rate ( P <0 .05) .[Conclusion]The expression of miR‐29b is down‐regula‐ted in prostate cancer .A higher expression of miR‐29b inhibits cell proliferation and invasion and induces cell apoptosis in prostate cancer .And the chemosensitivity of cisplatin increases in Pca cell .

Clinicopathological Observations of Type AB Thymoma with Liver Metastases
[Objective] To explore the clinicopathological features ,histological types ,diagnosis ,differen‐tial diagnosis ,treatment and prognosis of type AB thymoma with liver metastases .[Methods]Liver metasta‐sis specimens were harvested from one 49‐year‐old female case of type AB thymoma 5 years after operation and examined histologically with light microscopy and immunohistochemistry .Comprehensive analyses were con‐ducted for clinical manifestations ,histopathology ,immunohistochemical features ,differential diagnosis ,treat‐ment and prognosis after initial operation .The pathological diagnosis for liver metastases was type AB thy‐moma .Immunohistochemical staining showed CKpan (+ ) ,CD3(+ ) ,CD2 (+ ) ,TdT (+ ) ,CD5 (+ ) ,p53 (+ ) ,Hep‐1 (-) and Ki67‐positive cells (25% ~30% ) .The expression level of galectin‐3 was higher than that in mediastinal thymoma tissue identified 5 years ago .[Conclusion]Liver metastases of type AB thymoma are extremely rare .Type AB thymoma with low malignant potential may recur and develop distant metastases . Overexpressions of p53 ,galectin‐3 and Ki67 in type AB thymoma might coordinately manipulate the process of development ,progression and malignant transformation .

Relationship Between Family History of Obesity and Hypertriglyceridemic-waist Phenotype in Adolescents
[Objective]To explore the relationship between a family history of obesity and hypertriglyceridemic‐waist (HTWC) phenotype in adolescents .[Methods]A cross‐sectional study was conducted for 3 ,136 Han adoles‐cents aged 13~17 years in Qinhuangdao through stratified and cluster sampling .The HTWC phenotype was defined as serum triglyceride concentrations ≥1 .47 mmol/L and waist circumference ≥90th percentile for age and gender . Based on a family history of obesity ,they were divided into two groups of positive family history ( n =1169) and negative family history ( n =1967) .The triglyceride concentrations ,height ,weight and waist circumference were measured .And body mass index was calculated .[Results]Waist circumference and serum triglyceride were significant‐ly higher in adolescents with a family history of obesity than those in adolescents without a family history of obesity ( P<0 .05) .The prevalence of HTWC phenotype was significantly higher in adolescents with family history than that in those without family history (4 .4% vs 2 .5% ,χ2 =8 .468 ,P =0 .004) .After adjusting for gender and age ,the prevalence of HTWC phenotype among adolescents with family history was 1 .735 (95% CI:1 .167~2 .578 ,P =0 .006) folds of those without family history .[Conclusion]A significant correlation exists between a family history of obesity and HTWC phenotype .And it suggests that a family history of obesity is an independent risk factor for HT‐WC phenotype .

[Objective]To evaluate the expression of EGFL7 and microvessel density (MVD) in renal cell carcinoma (RCC) .[Methods]The expression of EGFL7 was detected by SP immuno‐histochemical technique in 63 RCC and 6 cases of normal renal tissue .Meanwhile the levels of MVD were evaluated for RCC .And dif‐ferent pathologic periods ,grade factors of RCC and the relationship of EGFL7 and MVD were analyzed .[Re‐sults]All normal renal cells were negatively stained .The positive expression of EGFL7 in tumor cells closely correlated with different pathological grades and clinical stages ( P < 0 .05) .The levels of MVD varied be‐tween different pathological grades and clinical stages ( P<0 .05) .[Conclusion]The expression of EGFL7 and MVD value are closely correlated with biological behaviors of RCC and play important roles in the process of invasiveness and metastasis .

Clinical Significance of lh-plasma Glucose Cutoff Value During Oral Glucose Tolerance Test in the Screening of Abnormal Glucose Metabolism in Hypertension Patients
[Objective]To explore the clinical significance of lh‐plasma glucose (1hPG) cutoff value during oral glucose tolerance test (OGTT) in the screening of abnormal glucose metabolism on hypertension patients .[Methods]A total of 112 hypertension patients were recruited for OGTT .The cutoff value of 1hPG during OGTT was analyzed by receiver operating characteristic (ROC) curve .The tangent point value was used to di‐vide them into two groups .And their values of height ,weight ,body mass index (BMI) ,fasting plasma glu‐cose (FPG) ,triglycerides (TG) ,cholesterol (CHO) ,high & low density lipoprotein cholesterol (HDL‐c &LDH‐c) ,blood uric acid (UA) ,HOMA‐IR and HOMA‐β were measured and compared .[Results](1) A‐mong them ,30 cases (26 .8% ) had normal glucose tolerance (NGT) while 46 cases (41 .1% ) impaired glucose regulation (IGR) .There were 36 new cases (32 .1% ) of diabetes ;(2) According to the results of ROC ,for diagnosing abnormal glucose metabolism ,the optimal cutoff value of 1hPG during OGTT was 10 .1 mmol/L with a sensitivity of 89% and a specificity of 90% ;(3) .In group of 1hPG ≥10 .1 mmol/L ,UA ,FPG and HOMA‐IR index were all higher than those in group of <10 .1 mmol/L ( P <0 .05) while HOMA‐β index was lower than group of <10 .1 mmol/L ( P <0 .05) .No statistic difference existed between two groups in BMI ,FTI ,TG ,CHO ,LDL‐c or HDL‐c .[Conclusion]Hypertension patients with 1hPG ≥10 .1 mmol/L are more likely to have abnormal glucose metabolism .And lh PG may reflect insulin resistance and islet β‐cell function .

5-Aza Induces Apoptosis and Autophagy in Gastric Cancer Cells
[Objective]To explore the effects of 5‐Aza‐2‐deoxycytidine (5‐Aza) on apoptosis ,autophagy and in‐sulin signal pathway in GC‐7901 cell line .[Methods]GC‐7901 cells were treated with 5 ,10 and 20μmol/L of 5‐Aza for 3 days .The expressions of autophagy‐related proteins LC3‐Ⅱ and LC3‐Ⅰwere detected by Western blot .And 10μmol/L5‐Aza was used for intervening GC‐7901 .Two groups of control and 5‐Aza were assigned .The expressions of insulin signaling axis associated proteins IGF‐1R and p‐IGF‐1R were detected by Western blot .And the expressions of IGF‐1R downstream signaling pathway related proteins PI3K ,p‐PI3K ,mTOR ,p‐mTOR were measured .Annexin V‐PI flow cytometry was used for detecting apoptotic cells .[Results]5‐Aza dose‐dependently decreased the level of LC3‐Ⅰand increased the level of LC3‐Ⅱ .And a significant increase in LC3‐Ⅱ/LC3‐Ⅰratio suggested that demethylating agent could significantly increase autophagy in gastric cancer cells .Cell autophagy and apoptosis could be induced by 5‐Aza .The expressions of phosphorylated insulin signaling pathway related proteins ,such as p‐IGF‐1R ,p‐PI3K and p‐mTOR ,significantly decreased .But it had no effect on total protein IGF‐1R ,PI3K or mTOR .[Conclusion]5‐Aza can induce cell autophagy and apoptosis and decrease the levels of phospho‐IGF‐1R , phospho‐PI3K and phospho‐mTOR proteins .However ,there is no effect on the total protein levels .

Comparison of Serum Activities of Cathepsin D and Alpha-1 Antitrypsin in Stages Ⅰ ~ Ⅳ Knee Osteoarthritis Patients
[Objective]To explore the serum activity changes of cathepsin D (CATD) and alpha‐1 antitryp‐sin (AAT) in different stages of osteoarthritis patients .[Methods]The serum samples were collected from 20 patients with stagesⅠ ~ Ⅳ knee osteoarthritis according to the Kellgren‐Lawrence (K‐L) criteria .And anoth‐er 20 healthy controls were also recruited .Enzyme‐linked immunosorbent assay (ELISA) was used to detect the serum activities of CATD and AAT .The data were analyzed by SPSS19 .0 software to determine whether the serum activities of CATD and AAT had changed or not for different stages of osteoarthritis and whether the differences were significant compared with healthy controls .[Results]Compared with healthy sera ,the ser‐um activity of CATD increased for stages Ⅰ ~ Ⅱpatients while that of CATD decreased for stages Ⅲ ~ Ⅳ ones .As for serum activity of AAT ,stagesⅠ ~ Ⅱincreased while stages Ⅲ ~ Ⅳhad no statistical difference com‐pared with healthy sera .[Conclusion]The serum activities of CATD and AAT vary during different stages of osteoarthritis .And testing these two serum enzymes may help to determine the degeneration degree of osteoar‐thritis cartilage in patients with osteoarthritis .

Expression and Clinical Significance of miR-26a in Childhood B-cell Acute Lymphoblastic Leu-kemia
[Objective]To explore the expression and clinical significance of miR‐26a in childhood B‐cell a‐cute lymphoblastic leukemia (ALL) .[Methods]Bone marrow samples were collected from 38 children with B‐cell ALL and 18 controls with non‐hematologic diseases .Total RNA was harvested from bone marrow .Real‐time polymerase chain reaction (PCR) was performed to detect the expression level of miR‐26a .[Results]The relative expression level of miR‐26a was remarkably lower in B‐cell ALL group than that in controls ( P <0 .05) .And the expression of miR‐26a in newly diagnosed samples was lower than those in Day 30 and Week 12 samples respectively ( P <0 .05) .Also its expression in Day 30 samples was lower than those in Week 12 samples ( P<0 .05) .And its expression in children aged over 10 years old was significantly lower than those in children aged under 10 years ( P <0 .05) .Its expression was higher in low‐risk group than those in middle and high‐risk groups respectively ( P <0 .05) .There was significant difference between middle and high‐risk groups ( P <0 .05) .[Conclusion]miR‐26a has tumor suppressor effect so that it may become a potential prog‐nostic biomarker for childhood B cell ALL .

Correlation Between Diabetic Nephropathy and the Level of Urinary Mindin in Elder Patients with Type 2 Diabetes Mellitus
[Objective]To evaluate the level of urinary mindin in elder patients with type 2 diabetes melli‐tus (T2DM ) and analyze its association with renal injury .[Methods]From October 2012 to June 2013 ,a total of 168 T2DM patients were recruited and divided into three groups of normoalbuminuria (urinary albumin <30 mg/24h ,n=70) ,microalbuminuria (30 ≤ urinary albumin <300 mg/24h ,n=50) and macroalbuminuria (u‐rinary albumin >300 mg/24h ,n =48) .And 50 age and gender‐matched subjects were selected into normal control group .The levels of urinary mindin ,blood & urinary creatinine ,serum urea nitrogen ,cystatin C , blood glucose ,glycated hemoglobin and C‐peptide ,etc .were measured .[Results]The levels of urinary mindin were significantly higher in the macroalbuminuria ,microalbuminuria and normoalbuminuria groups than those of control group [435(227~905) ,327(179~406) ,163(117~223) & 91(50~120) L?ng/mL? μmol re‐spectively ;F=31 .037 ,P <0 .05] .With rising 24 h‐UAE ,the level of urinary mindin also showed a trend of increase (r=0 .461 ,P<0 .05) .The urinary level of mindin was positively correlated with urea nitrogen ,cre‐atinine ,cystatin C ,blood glucose ,24 h‐urinary albumin and course of T2DM while negatively correlated with estimated glomerular filtration rate .Logistic regression analysis showed that urinary mindin ,diastolic blood pressure and fasting blood‐glucose were independent influencing factors of renal injury in T2DM .[Conclusion]Urinary mindin is probably closely correlated with diabetic nephropathy and participates in the progress of dia‐betic nephropathy .And it may serve as a potential marker for an early diagnosis of diabetic nephropathy .

Correlation Between Serum Calprotectin Level and Short-term Prognosis in Patients with Acute Myocardial Infarction

[Objective]To explore the correlation between serum calprotectin(S100A8/A9) and short‐term prognosis in patients with acute myocardial infarction(AMI) .[Methods]A total of 64 AMI patients and 30 pa‐tients with stable angina pectoris(SAP group) who had intact clinical data and diagnosed in our hospital from March 2012 to March 2013 were collected .Other 30 healthy subjects(NC) were chosen as control group .Gen‐eral condition and biochemical indicators of all subjects were collected .Serum level of fasting S100A8/A9 in each group at admission was detected .Patients with AMI at 3 months after discharge were followed up by phone so as to understand the short‐term prognosis of patients .[Results] Serum level of S100A8/A9 in AMI group was obviously higher than that in SAP group and healthy control group( P <0 .05) ,and that in SAP group was also higher than that in healthy control group( P <0 .05) .Short‐term prognosis(death or rehospi‐talization due to cardiac event) of patients with AMI was affected by S100A8/A9 ,FPG ,smoking and hyper‐cholesterolemia( P <0 .05) .[Conclusion]Serum level of S100A8/A9 in patients with AMI is significantly ele‐vated ,and related with the short‐term prognosis of patients with AMI .

Meta Analysis of the Effect of Motivational Interview on Blood Glucose of Patients with Type 2 Diabetes Mellitus

[Objective]To systematically evaluate the efficacy of motivational interview in patients with type 2 diabetes mellitus(T2DM) .[Methods] Such databases as Cochrane library ,MEDLINE ,EMbase ,EB‐SCO host ,CNKI ,CBM and Wanfang data were searched from Jan .1989 to Dec .2013 to collect the random‐ized controlled trials(RCTs) about the effect of motivational interview on glycemic control in patients with T2DM .The quality of RCTs was critically appraised and data were extracted by 3 reviewers independently . Meta analysis was conducted for the eligible literatures .[Results]A total of 7 RCTs were enrolled in the stud‐y .The experiment group included 412 cases ,while the control group included 384 cases .Compared with rou‐tine diabetic health education ,motivational interview could obviously decrease blood glucose of T2DM patients [hemoglobin A1c(HbA1c)(WMD= -0 .52 ,95% CI= -0 .85 , -0 .19 ,P < 0 .01)] ,fasting blood‐glucose [(WMD= -0 .97 ,95% CI= -1 .33 , -0 .60 ,P < 0 .00001)] ,2h‐postprandial blood glucose[WMD =-2 .03 ,95% CI= -2 .58 , -1 .49 , P < 0 .00001 )] .[Conclusion] Motivational interview can improve HbA1c ,fasting blood‐glucose and 2h‐postprandial blood glucose of patients with T2DM .

Efficacies of Treating Advanced Colorectal Cancer with Cetuximab Plus Different Chemotherapy Regimens
[Objective] To explore the clinical efficacies and adverse reactions of cetuximab plus FOLFOX or FOLFIRI chemotherapy in the treatment of advanced colorectal cancer .[Methods] A total of 80 patients with advanced colorectal cancer patients were randomly divided into A and B groups ( n=40 each) .Group A received cetuximab plus FOLFOX regimen while group B cetuximab plus FOLFIRI chemotherapy .And their immediate outcomes ,quality of life and adverse reactions after 6 weeks were recorded and compared .[Re‐sults] There were insignificant inter‐group differences in efficiency or life quality improvement rate ( P >0 .05) .For both groups ,the diameter and thickness of tumor significantly decreased as compared with pre‐treatment ( P <0 .05) .But the inter‐group difference was insignificant ( P >0 .05) .Occurrences of diarrhea and neurotoxicity had significant inter‐group differences ( P <0 .05) .[Conclusion]Cetuximab plus FOLFOX or FOLFIRI has similar efficacies for advanced colorectal cancer .Only individual differences exist in adverse reactions .

Surgical Experiences for Hoffa Fracture with Hollow Double Head Screw and Seed Nail
[Objective] Hollow double compression screw fixation were retrospectively analyzed the clinical effect of treatment of Hoffa fractures .[Methods] From 2004 to 2013 ,a total of 21 Hoffa fracture patients were recruited .There were 13 males and 8 females .And the causes were traffic accident ( n=15) ,high fall‐ing ( n=4) and crush injury ( n=2) .According to the Letenneur scheme ,the types were Ⅰ( n=11) ,Ⅱ( n=6) and Ⅲ ( n =4) .And the fractures were closed ( n=15) and open ( n=6) .Double hollow compression screw was applied for fracture fixation .And postoperative rehabilitation exercise was implemented in a timely manner .[Results] The average follow‐up period was 21 .5 (6~36) months .During follow‐ups ,osseous heal‐ing was achieved without perioperative vascular or nerve injury ,wound infection ,screw loosening or rupture . Postoperative function assessments were based on Letenneur system .And the outcomes were excellent ( n =18) ,fair ( n =2) and poor ( n =1) .And the excellent rate was 85 .7% .[Conclusion]Hollow double com‐pression screw is efficacious for Hoffa fractures .And it offers the advantages of mini‐invasiveness ,reliable fix‐ation and quick recovery of postoperative functions .

Expression and Clinical Significance of Id-2 and E-cadherin in Rectal Cancer
[Objective] To explore the expression levels of the inhibitor of differentiation 2(Id‐2) and epi‐thelial cadherin(E‐cadherin) in rectal cancer and its relation with clinicopathological characteristics .[Methods]Immunohistochemical method was used to observe the expression levels of Id‐2 and E‐cadherin in rectal cancer tissue and normal tissue adjacent to cancer .Spearman correlation test was used to analyze the correlation be‐tween Id‐2 and E‐cadherin expression .The correlation of positive expression rate of Id‐2 and E‐cadherin with clinicopathological indicators of rectal cancer was analyzed .[Results] The positive expression rates of Id‐2 in rectal cancer tissues and normal tissues adjacent to cancer were 48 .33% and 71 .67% respectively ,and the positive expression rate of E‐cadherin was 71 .67% and 46 .67% ,respectively .Spearman correlation test showed that there was negative correlation between Id‐2 expression and E‐cadherin expression .There was sig‐nificant difference in the expression levels of Id‐2 and E‐cadherin of rectal cancer among different differentia‐tion ,TNM stage and lymph node metastasis( P<0 .05) ,but there was no significant difference among differ‐ent age and sex( P>0 .05) .[Conclusion]The expression levels of Id‐2 and E‐cadherin are closely related with the occurrence and development of rectal cancer .High expression of Id‐2 in rectal cancer may promote tumor invasion and metastasis through E‐cadherin pathway .

Clinical Efficacies of Leuprorelin Acetate Microspheres for Uterine Adenomyosis with Infertility
[Objective] To observe the clinical efficacies of leuprorelin acetate microspheres for uterine ade‐nomyosis with infertility .[Methods] A total of 60 infertile patients with uterine adenomyosis (observation group) undergoing in vitro fertilization embryo transplant (IVF‐ET) ovum induction after using leuprorelin ac‐etate microspheres were studied retrospectively .Another 60 infertile patients with tubal factors were enrolled as control group .The volumes of uterus and outcomes of IVF‐ET were compared between two groups .[Re‐sults] ① Volume of uterus :In observation group after 2~6 courses of therapy (3 .75 mg/28 days) ,the mean uterine volume shrank from (178 ± 75) to (85 ± 66) cm3 ( P <0 .05);② Outcome of IVF‐ET :The rate of embryo implantation ,clinical pregnancy and abortion between observation group (38 .8% ,53 .5% ,4 .3% ) and control group (35 .3% ,52 .8% ,3 .6% ) had no significant difference ( P > 0 .05);③ In observation group ,the rate of fertilization ,two pronuclear and superior embryo in patients with successful pregnancy (76 .1% ,67 .8% ,63 .4% ) after IVF‐ET were higher than those with failed pregnancy (61 .4% ,60 .8% , 50 .5% ) .[Conclusion] Leuprorelin acetate microspheres may shrink the volume of uterine adenomyosis and improve the outcomes of pregnancy in patients undergoing IVF‐ET .

Clinical Study of Olanzapine Combined with Metoclopramide to Treat Explosive Chemotherapy-induced Nausea and Vomiting

[Objective] To retrospectively analyze the efficacy of olanzapine combined with metoclopramide to treat explosive chemotherapy‐induced nausea and vomiting in patients with malignant tumor cancer receiving moderately and highly emetogenic chemotherapy .[Methods] Patients with malignant cancer receiving moder‐ately and highly emetogenic chemotherapy(platinum and anthracyclines) were chosen .Among 68 patients with explosive nausea and vomiting after conventional anti‐vomiting treatment before chemotherapy ,46 patients with highly emetogenic chemotherapy were treated with conventional anti‐vomiting therapy i .e .palonosetron 0 .25mg intravenously at d1 and dexamethasone 10mg intravenously once daily at d1~3 30min after chemothera‐py ,and 22 patients with moderately emetogenic chemotherapy were treated with conventional anti‐vomiting therapy i .e .ondansetron 8mg intravenously at d1~2 and dexamethasone10mg intravenously at d1~3 .When ex‐plosive nausea and vomiting appeared , all patients were given metoclopramide 20mg intramuscularly and olanzapine 10mg orally once daily for 3 days .The remission of nausea and vomiting and other adverse events at 0~24h after mediation were observed .[Results] A total of 68 patients with explosive nausea and vomiting af‐ter chemotherapy were treated .The efficacy was assessed .The complete remission(CR) rate of explosive vomiting and nausea were 80 .9% (55/68) and 75 .0% (51/68) ,respectively .The main side effects were drowsiness(13 .2% ,9/68) ,fatigue(10 .3% ,7/68) and dizziness(7 .4% ,5/68) and so on .No obvious grade 3~4 toxic reaction was found .[Conclusion]Metoclopramide combined with olanzapine for treating explosive nausea and vomiting induced by moderately and highly emetogenic chemotherapy had good efficacy without ob‐vious toxic and side reaction .Therefore ,it is worthy of clinical promotion .

Surgical Treatment of Intertrochanteric Femoral Fracture in Patients with Parkinson's Disease
[Objective]To explore the surgical approaches and preoperative measures of intertrochanteric femoral fracture in patients with Parkinson's disease (PD) patients .[Methods]A retrospective study was per‐formed for 19 intertrochanteric fracture patients with PD .There were 6 males and 13 females with an average age of 76 .4 (64~89) years .According to the classification scheme of Evans‐Jensen ,the fracture types wereⅡ ( n=2) ,Ⅲ ( n =4) ,Ⅳ ( n=7) and V ( n=6) .The treatments were dynamic hip screw (DHS ,n=6) and proximal femoral nail antirotation (PFNA ,n=13) .And the preoperative and postoperative scores of Har‐ris were evaluated .[Results]All patients were safe during perioperative period .The mean follow‐up period was 30 .2 (6~72) months .Operative incision healed without infection ,nerve damage or fracture complica‐tions .The average Harris scores increased from preoperative (25 .7 ± 6) (24~36) to (78 .5 ± 4 .9) (65~86) points at the last follow‐up .In DHS group ,1 case had mild claudication ,coxa vara and nail cutting femoral . And tibial tubercle traction was applied for 3 weeks and there was delayed bone healing .Better fixation was a‐chieved for PFNA group and there was no obvious sequel .[Conclusion] For femoral fracture with Parkinson disease ,a surgeon should pay close attention to treating complications and selecting proper surgical and fixa‐tion approaches to achieve rapid healing .

Requirements of Propofol and Remifentanil with Dexmedetomidine During Breast Cancer Surgery

[Objective]To explore whether dexmedetomidine can reduce the requirement for propofol and remifentanil during breast cancer operation .[Methods]A total of 60 American Society of Anesthesiologists physical status Ⅰ ~ Ⅱ patients received either dexmedetomidine (1 μg/kg over 10 min followed by a continuous infusion of 0 .4 μg/kg/h until tumor removal) or comparable volumes of saline as a placebo .Propofol and remifentanil require‐ments were recorded and analyzed .Changes in mean arterial pressure ,heart rate and bispectral index (BIS) were recorded at operation room (T0 ) ,before anesthesia induction (T1 ) ,1 min pre‐intubation (T2 ) ,30 min during sur‐gery (T3 ) ,end of surgery (T4 ) and 5 min post‐extubation (T5 ) .[Results]All patients completed the study .The propofol dosage required for anesthetic maintenance was 29% (with a 95% confidence interval ,18~40) and it was lower in patients on dexmedetomidine [2 .0 (1 .3~ 3 .4) vs 3 .1(2 .2~4 .7) mg/(kg ? h) ,P = 0 .005] .And remifentanil dosages were also significantly different [0 .16(0 .09~0 .21) vs 0 .10(0 .07~0 .15)μg/(kg?h) ,P=0 .03] .The incidence of adverse events ,including hemodynamic instability and delayed recovery ,was comparable with and without dexmedetomidine .[Conclusion]Dexmedetomidine administration significantly reduces the re‐quirement for both propofol and remifentanil during anesthetic induction and maintenance .

[Objective]To explore the clinical efficacies of OB glue ductal blocking treatment for plasma cell mastitis (PCM ) of fistula period .[Methods]A total of 44 non‐lactating and non‐pregnant female patients with PCM of fistula period were recruited from January 2009 to January 2014 and divided into two groups of OB glue ductal blocking (group A ,n=21) and conventional technique (group A ,n=23) .And the clinical efficacies of two groups were analyzed .[Results]The curative rate was 85 .71% (18/21) in group B versus 56 .52% (13/23) in group A ( P <0 .05) .The wound healing time was significantly shorter in group B than that in group A (28 .2 ± 5 .3 vs 51 .8 ± 6 .7 days) .And there were significant differences ( P<0 .05) .[Conclusion]For patients with PCM of fistula period ,the use of OB glue may cause little trauma ,promote wound healing ,reduce the recurrence and improve quality of life .And it is worthy of wider applications .

Effects of Addition-subtraction Amber Powder on Serum Levels of CA125 and CA199 in Patients with Ovarian Cysts

[Objective]To explore the effects of addition‐subtraction amber powder on serum levels of CA125 and CA199 in patients with ovarian cysts .[Methods]A total of 90 patients with ovarian cysts were di‐vided into control and treatment groups .And another 30 female healthy subjects were selected into normal group .The control group received Dahuang Zhechong pill ,while the treatment group by addition‐subtraction amber powder .All treatments lasted 2 months .The serum levels of CA125 and CA199 were detected by chemiluminescent detection in normal ,control and treatment groups before and after treatment .Type B ultra‐sound was used to detect the diameter of cysts in control and treatment groups before and after treatment . Then clinical efficacies were analyzed .[Results]Before treatment ,the serum levels of CA125 and CA199 were both much higher than those of normal group ( P < 0 .05) .After treatment ,the serum levels of both de‐creased in two groups .However ,the decrease of treatment group was more dramatic ( P<0 .05) .The diam‐eter of cysts in treatment group was much smaller than that of control group ( P<0 .05) .And its overall effi‐cacy was significantly higher than that of control group ( P<0 .05) .[Conclusion]Addition‐subtraction amber powder can decrease significantly the serum levels of CA125 and CA199 to improve clinical symptoms .It is ef‐ficacious for ovarian cysts and worthy of wider applications .

Expression and Significance of c-Jun Activation Domain Binding Protein 1 in Patients with Colorectal Cancer

[Objective]To explore the expression and significance of c‐Jun activation domain binding protein 1 (Jab1) in patients with colorectal cancer .[Methods]From January 2010 to January 2014 ,98 samples of colorectal cancer tissues and adjacent normal tissues were collected and analyzed by immunohistochemistry (SP method) to de‐tect the expression of Jab1 .[Results]Jab1 had a positive expression rate of 96 .9% (95/98) in colorectal cancer tissues versus 17 .3% (17/98) in corresponding adjacent tissues .And the inter‐group differences were statistically significant ( P<0 .05) .Jab1 expression was significantly correlated with tumor cell differentiation ,depth of invasion ,TNM stage and lymph node metastasis ( P<0 .05) .However ,gender ,age or tumor size had no significant correlation ( P>0 .05) .The 1 ,3 ,5‐year survival rates of JAB1‐positive patients were 81 .0% ,49 .6% and 31 .0% versus 100 .0% ,87 .5% and 75 .0% of JAB1‐negative ones .And the differences were statistically significant ( P<0 .05) .[Conclusion]Jab1 is involved in the occurrence of colorectal cancer ,invasion and metastasis .And its expression deter‐mines the prognosis of patients with colorectal cancer .

Clinical Imaging Characteristics of Long-term Use of Antibiotics in Plateau Bronchiectasis Pa-tients
[Objective]To explore the effects of long‐term antibiotics on stable bronchiectasis patients .[Methods]A total of 96 bronchiectasis patients in stable stage were divided into observation and control groups ( n =48 each) .The control group received oral ambroxol hydrochloride 30 mg ,thrice daily while the observa‐tion group took roxithromycin 0 .15 g ,once daily plus oral ambroxol hydrochloride 30 mg ,thrice daily .After 6‐month treatment ,two groups were compared with regards to the scores of quality of life ,dyspnea and chest computed tomography (CT) imaging .[Results]After treatment ,the observation group had lower scores than those of control group .And the differences were significant ( P<0 .05) .[Conclusion]In the treatment of sta‐ble bronchiectasis ,a combination of roxithromycin and ambroxol hydrochloride can significantly reduce the to‐tal scores of quality of life score ,dyspnea and chest CT imaging .Such a regimen is worthy of wider applica‐tions .

Effects of Danhong Injection on the Expression of Glial Fibrillary Acidic Protein After Cerebral Is-chemic Reperfusion in Rats

[Objective]To explore the effects of Danhong injection on the brain tissue expression of glial fibril‐lary acidic protein (GFAP) after cerebral ischemic reperfusion (IR) in rats .[Methods]A cerebral ischemia‐reperfu‐sion rat model was established through an occlusion of middle cerebral artery .A total of 96 healthy male Sprague‐Dawley rats were randomly divided into 4 groups of normal control ( n =8) ,sham operation ( n =8) ,ischemia reperfusion ( n =40) and Danhong injection ( n=40) .The expressions of GFAP in brain tissue were detected by immunohistochemistry at 6h ,24h ,48h ,72h and 7d post‐IR in ischemia‐reperfusion and Danhong intervention groups .Before sacrificing ,the neurological deficit scores were evaluated for all rats .The experimental results were analyzed with Spss17 .0 .[Results]The expressions of GFAP were weak in both normal control and sham op‐eration groups .And the levels of GFAP were evidently up‐regulated at 6h and peaked at 72h ( P <0 .05) .Then there was a gradual decline .Compared with ischemia/reperfusion group ,the levels of GFAP evidently decreased in Danhong intervention group ( P<0 .05) .At 24h ,48h ,72h and 7d ,the neurological deficit score of Danhong in‐tervention group was better than that of ischemia/reperfusion group .And the neurological deficit scores decreased in Danhong group ( P<0 .05) .[Conclusion]After ischemia‐reperfusion injury ,the levels of GFAP were evidently up‐regulated in ischemia‐reperfusion rats .Injury of brain tissue after ischemia‐reperfusion is blunted in Danhong intervention group .And Danhong may down‐regulate the level of GFAP and suppress the excessive proliferation of astrocytes .

Curative Efficacy of Prostaglandine1 Plus Acarbose and its Effects on Oxidative Stress in Pa-tients with Type 2 Diabetic Nephropathy

[Objective]To explore the clinical efficacy of prostaglandin e1 plus acarbose and its effects on oxidative stress in patients with type 2 diabetic nephropathy (DN) .[Methods]A total of 85 cases of type 2 DN fulfilling the inclusion criteria were divided into two groups according to therapeutic protocol .Besides conven‐tional treatments ,the control group treated acarbose 100 mg ,thrice daily while the observation group took prostaglandin e1 10 μg ,once daily and acarbose 100 mg thrice daily .Then their curative efficacy ,renal func‐tion ,oxidative stress and adverse reactions were observed and compared .[Results]The observation group had a therapeutic efficiency ratio (TER) of 88 .9% and it was significantly higher than that of 77 .5% in control group (Z=2 .15 ,P <0 .05) .In comparison with control group after treatment ,the renal function indices of blood urea nitrogen ,serum creatinine and urine total protein were statistically lower in observation group ( P<0 .05) .As to oxidative stress ,compared to control group ,malondialdehyde were statistically lower while superoxide dismutase was statistically higher in observation group ( P <0 .05) .During treatment ,there was on onset of severe adverse reactions .[Conclusion]Combined treatment of prostaglandin e1 and acarbose is effi‐cacious for type 2 DN .It can significantly increase therapeutic efficiency ratio ,improve renal function and alle‐viate oxidative stress with minor adverse reactions .

Expression of Macrophage Migration Inhibition Factor and Related Factors in Cervical Squamous Carcino-ma and Its Significance

[Objective] To explore the expression of macrophage migration inhibition factor(MIF) ,metalloprotease‐9 (MMP‐9) and vascular endothelial growth factor(VEGF) in squamous carcinoma of the cervix(SCC) .[Methods]A total of 51 SCC samples(SCC group) ,41 cervical intraepithelial neoplasia(CIN) samples(CIN group) and 32 normal cervix (NCE) sample(NCE group) were chosen .The expressions of MIF ,MMP‐9 and VEGF were detected by immunohisto‐chemical SP method ,and compared and analyzed .[Results] The positive rates of MIF ,MMP‐9 and VEGF and MVD value in NCE ,CIN and SCC groups were increased by turns ,and there was significant difference( P<0 .05) ,and those in SCC group were markedly higher than those in CIN and NCE groups .The positive expression rates of MIF ,MMP‐9 and VEGF in SCC group were correlated with histological differentiation ,lymph node metastasis and clinical stage( P<0 .05) .The positive expression rates of MIF ,MMP‐9 and VEGF and MVD value were markedly correlated with clinical stage ,differentiation and lymph node metastasis of SCC( P<0 .05) .The expression of MIF in SCC was markedly corre‐lated with the expression of MMP‐9( P<0 .05) ,but the expression of VEGF was not correlated with the expressions of MIF and MMP‐9( P>0 .05) .[Conclusion]MIF ,MMP‐9 and VEGF may play an important role in the occurrence ,de‐velopment ,infiltration and metastasis of SCC .The poorer the histological differentiation of SCC is ,the higher the expres‐sions of MIP ,MMP‐9 and VEGF and MVD value are .It indicates that the combination detection can evaluate the severity of SCC .

Influence of Early Use of Rosuvastatin on Blood Lipids and Inflammatory Factors in Patients with Acute Coronary Syndrome

[Objective]To explore the effect of early use of rosuvastatin on blood lipids and inflammatory factors high sensitive C‐reactive protein(hs‐CRP) and interleukin‐6(IL‐6) in patients with acute coronary syndrome(ACS) .[Meth‐ods]A total of 54 patients confirmed as ACS by clinical diagnosis in our hospital were chosen and randomly divided into routine treatment group(group A ,n=26) and rosuvastatin treatment group(group B ,n =28) by using randomization case control method .Other 20 concurrent healthy subjects were chosen as the control group(group C ,n=20) .Patients in group B were given rosuvastatin(crestor produced by astrazeneca company) 5~10mg within 24~28h after the diagnosis of ACS once per night based on routine treatment .The course was 8 weeks .Double antibody sandwich enzyme‐linked im‐munosorbent assay(ELISA) was used to detect serum IL‐6 of patients in two groups before and after treatment ,and im‐muno‐turbidimetry was used to determine hs‐CRP .[Results]Compared with group C ,blood lipids ,IL‐6 and hs‐CRP in ACS patients before treatment increased significantly( P<0 .01) ,but there was no significant difference between group A and group B( P>0 .05) .Low density lipoprotein cholesterol(LDL‐C) ,IL‐6 and hs‐CRP in group B after treatment de‐creased significantly( P<0 .05) ,and there was significant different between group B and group A( P<0 .05) .[Conclu‐sion]ACS patients have inflammatory response .Early use of rosuvastatin can improve inflammatory status and reduce car‐diovascular events of patients .

Correlation of Uncertainty and Social Support for Patients with Gastric Cancer
[Objective]To explore the correlation of uncertainty and social support for gastric cancer pa‐tients .[Methods]A total of 74 gastric cancer patients were randomly surveyed for Mishel's Uncertainty In Ill‐ness Scale (MUIS) and social support scale (SSS) .[Results]The uncertainty degree was medium for gastric cancer patients .And significant negative correlations existed between objective support ,subjective support ,u‐tilization of social support in perceived social support and uncertainty( P <0 .05) .[Conclusion]Medical staff should pay more attention to uncertainty and social support for gastric cancer patients .

Efficacy and Safety of Domestic Semiconductor Laser Treatment of Moderate-to-severe Acne

[Objective] To explore the efficacy and safety of domestic semiconductor laser treatment of moderate‐to‐severe acne .[Methods]A total of 100 moderate‐to‐severe acne patients were randomly divided in‐to observation and control groups ( n=50 each) .The observation group received domestic semiconductor laser therapy (wavelength:650 and 810nm .power :400~450W ,once daily ,10 min/time for 14 day ;twice daily , 10 min/times for 14 days) .The control group received Roxithromycin Capsules (1 tablet twice daily) ,vitamin B6 tablets (2 tablets thrice daily) and green ointment (twice daily ,topical) .Clinical efficacy ,adverse events during treatment , resolution time and recurrence within 3 months after treatment were compared for two groups .[Results] The efficiency of observation group was lower than that of control group (82% vs 86% ) . The differences were not statistically significant ( P > 0 .05) .The incidence of pigmentation and recurrence rate of rash in observation group were lower than those in control group .And the differences were statistically significant ( P <0 .05) .[Conclusion] For moderate‐to‐severe acne ,the clinical efficacy of domestic semicon‐ductor laser technology is satisfactory .And it is worthy of wider promotions .

Therapeutic Effect of Local Immune Regulation on Nodular Subacute Thyroiditis
[Objective]To evaluate the therapeutic effect of local immune regulation on subacute thyroiditis pa‐tients (SAT ) with nodules .[Methods]A total of 112 nodular SAT patients were randomly divided into two groups .The control group received an oral dose of prednisone while the observation group had an intra‐thyroid nodule injection of triamcinolone acetonide . The effectiveness and recurrence rate were compared between two groups .And the changes of clinical efficacies and indicators were observed .[Results]The withdrawal time of local pain symptoms was significantly shorter in observation group than that in control group(7 .3 ± 2 .7 vs 21 .1 ± 5 .2 days ,P<0 .01) .The same results were observed for thyroid nodule disappearing time (9 .7 ± 3 .4 vs 33 .5 ± 4 .9 days ,P<0 .01) .The effectiveness after 6‐month treatment were 88 .14% (52/59)in observation group versus 47 .17% (25/53) in control group .During a follow‐up period of 12 months ,the recurrence rate was significantly lower in observation group than that in control group [15 .4% (8/52) vs 28 .0% (7/25) ,P<0 .01] .[Conclusion]For nodular SAT patients ,local immune treatment via an intra‐thyroid nodule injection of triamcinolone acetonide is superior to an oral dose of prednisone .

Effects of Dopamine Plus Phentolamine on Infantile Pneumonia Complicated with Heart Failure and Myocardial Enzyme

[Objective]To explore the effects of dopamine plus phentolamine on infantile pneumonia com‐plicated with heart failure and myocardial enzyme so as to provide rationales clinical diagnosis and treatment .[Methods]A total of 114 children with pneumonia complicated with heart failure were randomly divided into two groups of control (A ,n =57) and observation (B ,n =57) .The control group received phentolamine a‐lone for 7 days while the observation group had both phentolamine and dopamine for 7 days .[Results]The o‐verall treatment efficiency was significantly higher in group B than that in group A (92 .89% vs 77 .19% ) and the difference had statistical significance ( P <0 .05);In observation group after treatment ,the serum levels of lactate dehydrogenase and creatine kinase were significantly lower than those before treatment and control group after treatment .And the difference had statistical significance ( P <0 .05) .In observation group after treatment ,dyspnea ,cyanosis and rales disappeared .And failure to control time and hospital stay were signifi‐cantly shorter than those in control group .And the difference had significant statistical significance ( P <0 .05) .Neither group had an onset of marked adverse reactions .[Conclusion]The efficacy of dopamine plus phentolamine is definite for pneumonia in children with heart failure .And it improves patient enzymes and is worthy of wider applications .
