Effect of One-lung Ventilation with Pressure Control Ventilation-volume Guarantee on Respiratory Mechanics in Elderly Patients Undergoing Radical Esophageal Cancer Surgery

To explore the effect of one-lung ventilation (OLV ) with pressure control ventilation-volume guarantee(PCV-VG) on respiratory mechanics in elderly patients undergoing radical esophageal cancer sur-gery .[Methods] Totally 40 elderly patients(ASA Ⅰ ~ Ⅱ) undergoing radical esophageal cancer surgery were di-vided into two groups with 20 in each .All patients were given sevoflurane inhalation for maintenance anesthesia and intubated with double lumen endotracheal catheter .Two-lung volume controlled ventilation(VCV) with a tidal volume(VT) of 10ml/kg and frequency(f) of 12 breaths per minute were performed before chest opening .After chest was opened ,OLV was performed .Group A received VCV mode with a VT of 8ml/kg and f of 12 breaths per minute .Group B received PCV-VG mode with the same peak airway pressure (Ppeak) as that during two-lung VCV ,VT of 8ml/kg and f of 12 breaths per minute .The VT ,minute volume(MV) ,Ppeak ,mean airway pres-sure(Pmean) ,end-tidal pressure of carbon dioxide (PET CO2 ) and dynamic lung compliance (Cdyn) before OLA (T1 ) ,30min after OLA(T2 ) ,60min after OLA(T3 ) and 30min after two-lung ventilation(TLA) were observed . Lung effective dynamic compliance(Cdyn) was calculated .[Results]Compared with group A ,Ppeak and Pmean in group B were decreased ,but there was no significant difference in VT ,MV and PET CO2 ,and Cdyn was increased .[Conclusion] PCV-VG mode in OLA for elderly patients undergoing radical esophageal cancer surgery can decrease Ppeak ,ensure ventilatory capacity and improve Cdyn ,so it is good for lung protection in elderly patients .

Effect of Autogenous Blood Salvage on Microcirculation of Small Intestine in Patients Undergo-ing Spine Surgery
To observe the effect of autogenous blood salvage (BS ) on blood protection and small intestinal microcirculation in patients undergoing spine surgery .[Methods] A total 40 ASA Ⅰ ~ Ⅱ pa-tients scheduled for spine surgery under general anesthesia were randomly divided into autogenous BS group (group Ⅰ) and xenogenous blood transfusion group(group Ⅱ) .Blood loss ,xenogenous blood transfusion vol-ume and the expressions of intestinal fatty acid binding protein (I-FABP) and D-lactate before operation (T1 ) , 30min after tracheal intubation(T2 ) ,at the end of operation(T3 ) and 24h after operation(T4 ) were recorded in two groups .[Results]Compared with group Ⅰ ,xenogenous blood transfusion volume in group Ⅱ increased and the expressions of I-FABP and D-lactate in group Ⅱ at T3 and T4 time points increased .[Conclusion]Au-togenous BS can effectively reduce xenogenous blood transfusion during spine surgery ,and has less effect on small intestinal microcirculation perfusion .

Study on the Correlation of Inflammatory Markers with Essential Hypertension and Metabolic Syndrome
To explore the changes of serum levels of inflammatory factors human cartilage glycoprotein(YKL-40) and human pentraxins(PTX-3) in patients with essential hypertension(EH) and their correlation with metabolic syndrome(MS) .[Methods]Totally 60 EH patients and 39healthy subjects were en-rolled in this study . Serum levels of YKL-40 and PTX-3 were measured by using enzyme immunoassay (ELISA) .According to hypertension with or without MS ,all patients were divided into hypertension with MS group( n=23) and simple hypertension group( n=37) .Serum levels of YKL-40 and PTX-3 among hyperten-sion with MS group ,simple hypertension group and healthy control group were compared .[Results] Serum levels of YKL-40 in hypertension groups were obviously higher than those in healthy control group [(54 .3 ± 29 .0ng/mL) vs .(27 .2 ± 8 .7ng/mL) ,P<0 .05] ,while there was no significant difference in PTX-3 between two groups( P<0 .05) .Serum levels of YKL-40 in hypertension with MS group were higher than those in simple hypertension group[(69 .2 ± 35 .9μg/L) vs .(45 .1 ± 19 .1μg/L) ,P<0 .05] .YKL-40 was significantly correlated with systolic pressure ,diastolic pressure ,leukocyte number and PTX-3( r = 0 .341 ,0 .268 ,0 .299 and 0 .371 ,P <0 .05) ,and PTX-3 was correlated with heart rate ,serum uric acid and serum creatinine( r =0 .209 ,0 .206 and 0 .242 ,P<0 .05) .[Conclusion]Inflammatory factor YKL-40 in patients with hypertension is increased ,which suggests that YKL-40 may participate in the pathophysiological process of EH .YKL-40 for blood pressure control and prognosis evaluation in patients with hypertension is better than PTX-3 .

Effect of Human Antisense Nucleic Acid of Cyclooxygenase-2 (COX-2) on Expression of COX-2 in Bladder Cancer T24 Cells

To construct the antisense nucleic acid eukaryotic expression vector of human cy-clooxygenase-2(COX-2) ,and to explore its role in inhibiting the expression of COX-2 in human bladder cancer T24 cells .[Methods] PCR method was used to amplify cDNA from COX-2 plasmid .The COX-2 cDNA was cloned into the sites between T vectors EcoR 1 and Xba I through restrictive endonuclease ,and then inversely inserted into pcDNA3 .1 to obtain the recombinant plasmid .The recombinant plasmid was transfected into bladder cancer T24 cells mediated by liposome .After the scanning by G418 ,flow cytometry ,indirect immuno-fluorescence staining and RT-PCR were performed .[Results] Nucleic acid and restrictive endonuclease diges-tion confirmed that COX-2-antisense nucleic acid eukaryotic expression plasmid pcDNA 3 .1/COX-2 was cor-rect .Identification showed that the expression of COX-2 in T24 cells transfected with pcDNA3/COX-2 was in-hibited obviously .[Conclusion] Antisense nucleic acid eukaryotic expression vector of COX-2 is constructed successfully .Bladder cancer cell line T24-AS with persistent downregulation of COX-2 expression is obtained .

Study of Sleep Quality in Patients with Maintenance Hemodialysis
To investigate the sleep quality in patients with maintenance hemodialysis and to ex-plore the influential factors of sleep quality .[Methods]According to Pittsburg sleep quality index (PSQI) score ,99 maintenance hemodialysis patients were divided into good sleep group (PSQI≦5) and poor sleep group(PSQI<5) .The biochemical indicators between good sleep group and poor sleep group were compared . The factors affecting sleep quality was analyzed .Logistic regression method was use to analyze the relationship of clinical general data such as age and sex and biochemical indexes such as hemoglobin (Hb) and immunoreac-tive parathyroid hormone(iPTH) with sleep quality .[Results]Compared with poor sleep group ,Hb ,albumin (ALB) and Kt/V in good sleep group were increased ,while hsCRP ,iPTH ,urea ,creatinine ,sodium bicar-bonate ,calcium-phosphorus product and ferritin before dialysis were decreased .Logistic regression analysis showed that Hb(OR=1 .051 ,P =0 .003) ,ALB(OR=1 .209 ,P =0 .014) ,iPTH(OR=0 .998 ,P =0 .043) and calcium-phosphorus product(OR=0 .94 ,P=0 .003) were associated with sleep quality of patients .[Con-clusion]Sleep quality of hemodialysis patients is commonly decreased .Hb ,ALB ,iPTH and calcium-phos-phorus product may be associated with sleep quality of patients .Recognition and control of sleep quality should be strengthened so as to improve the sleep and increase quality of life of patients .

Impact of Lapatinib on Transplanted Tumor in Nude Mouse from Human Ovarian Cancer Cell line SKOV3
To explore the impact of new targeted therapy drug lapatinib on transplanted tumor of nude mouse from human ovarian cancer cell line SKOV3 .[Methods]Human ovarian cancer cell line SKOV3 was inoculated into subcutaneous tissue of nude mouse's chest wall in order to establish nude mouse model with transplanted tumor of ovarian cancer successfully .Then they were randomly divided into blank control group (group A) and experimental group (group B) ,experimental group were divided into two subgroups by differ-ent drug concentrations of lapatinib :100mg/kg(B1 ) and 200 mg/kg(B2 ) ,the changes of the volume of bearing cancer and the weight of nude mouse before and after lapatinib treatment were examined .[Results] Lapatinib can markedly inhibit the proliferation of transplanted tumor in nude mouse from SKOV 3 in a dose-dependent manner .The tumor inhibition rate of group B2 was markedly higher than that of group B1 ,and there was sig-nificant difference between two groups( P<0 .05) ,but there was no significant difference in mouse weight be-tween before and after medication ( P > 0 .05) .[Conclusion]Lapatinib can markedly reduce the volume of transplanted tumor in nude mouse from human ovarian cancer cell line SKOV 3 ,and provide the theoretical ba-sis for its application in ovarian cancer .

Comparative Study of the Status of Metabolic Syndrome Among the Military Medical Examina-tion Population in 5 Years

To analyze the status and related risk factors of metabolic syndrome (MS) among elderly medical examination people in PLA troop within 5 years in order to provide the basis for formulating the prevention and control strategy .[Methods] A total of 17541 elderly subjects in medical examination center of our hospital of a PLA troop in 2008 were chosen .The same population were reexamined in 2013 ,and 10124 cases were enrolled in the study .The changes of medical examination indexes among the people within 5 years were compared .The incidence of cardiovascular and cerebrovascular disease and the fatality rate were ana-lyzed .[Results]Compared with 2008 ,BMI ,waist circumference (WC) ,blood lipid ,blood glucose ,blood pressure ,the prevalence of MS and the prevalence of obesity in 2013 were increased ,and there were significant differences( P <0 .05) .The prevalence of cardiovascular and cerebrovascular disease was 19 .8% ,and the fa-tality rate was 5 .0% .[Conclusion]Compared with 2008 ,the prevalence of MS among military physical exam-ination population is markedly increased .Prevention should be taken according to the influencing factors of MS .

Comparison of the Efficacy and Quality of Life in Schizophrenia Patients with Depression Trea-ted by Aripiprazole vs . Risperidone

To explore the efficacy of risperidone vs .aripiprazole for the treatment of schizophrenia patients with depression and the influence on quality of life in these patients .[Methods] Totally 204 schizophrenia patients with depression were randomly divided into risperidone group ( n =102) and aripiprazole group( n =102) . All patients were given risperidone and aripiprazole for 24 weeks ,respectively .Before and 4 ,12 and 24 weeks after treatment ,all patients were evaluated by using positive and negative syndrome scale (PANSS) ,Hamilton depression scale(HAMD) ,clinical global impression scale(CGI-SI) and quality of life scale(QOL-100) .[Results]There was no significant difference in scores of PANSS ,HAMD ,CGI-SI and QOL-100 between two groups( P>0 .05) .After 4 weeks of treatment ,the scores of two groups were significantly improved with the prolongation of treatment time . HAMD scores at the end of 12 and 24 weeks after treatment and CGI-SI scores at the end of 4 weeks after treatment in aripiprazole group were better than risperidone group ( P<0 .05) .QOL-100 scores in aripiprazole group at the end of 24 weeks after treatment were better than risperidone group ( P<0 .05) .[Conclusion]Aripiprazole for the treat-ment of depression after schizophrenia has better long-term efficacy ,and can more improve the patients'quality of life .

Application Value of High-flux Hemodialysis in Patients with End-stage Renal Disease
To explore the application value of high-flux hemodialysis(HFHD) in patients with end-stage renal disease(ESRD) .[Methods]Clinical data of 136 ESRD patients undergoing hemodialysis for more than 1 year were collected .According to ultrafiltration coefficient of dialyzer ,all patients were divided into low-flux[ultrafiltration coefficient was 11 .3 mL/(h · mmHg)] 8LR group(group A) ,high-flux[ultrafil-tration coefficient was 50 mL/(h · mmHg)] PS15 group(group B) and high-flux[ultrafiltration coefficient was 71 mL/(h · mmHg)] 17R group(group C) .Blood urea nitrogen(BUN) ,creatinine ,serum phosphorus and potassium clearance ,serum intact parathyroid hormone(iPTH),β2-microglobulin(β2-MG),serum albumin (ALB) ,leptin ,homocysteine ,vitamin B12 and other indicators were compared among 3 groups .The incidence of complications such as joint disease ,carpal tunnel syndrome and cardiovascular diseases in 3 groups after di-alysis were analyzed and compared .[Results] There was no significant difference in blood BUN ,serum creati-nine ,serum phosphorus ,serum potassium and other small molecular substances among 3 groups( P>0 .05) . The clearance rates of iPTH ,β2-MG ,ALB ,leptin ,homocysteine ,vitamin B12 and other middle or giant mo-lecular substances in group B and group C were better than group A ( P>0 .05) .The clearance rate of middle or giant molecular toxin in group C was the highest( P >0 .05) .The incidence of complications such as joint disease ,carpal tunnel syndrome and cardiovascular disease in group B and C was lower than that in group A ( P<0 .05) ,but the incidence of hypotension in group B and C was higher than that in group A ( P <0 .05) . There was no significant difference in infection rate and mortality among 3 groups( P >0 .05) .[Conclusion]The clearance effect of HFHD on small molecular substances is similar to low-flux hemodialysis(LFHD) ,but its effect on middle or giant molecular substances is obviously better than LFHD ,and can reduce the incidence of joint disease ,carpal tunnel syndrome and cardiovascular disease .But the incidence of adverse reactions such as hypotension should be paid attention .

Relationship Between Ferritin and Microinflammation Status in the Early Stage of Diabetic Ne-phropathy

To explore the relationship between ferritin and microinflammation status in the early stage of diabetic nephropathy(DN) .[Methods]Totally 120 patients with type 2 diabetes mellitus(T2DM) in our hospital were divided into normal proteinuria group ,early DN group and clinical DN group with 40 patients in each group .Meanwhile ,40 healthy subjected were selected as the control group .Serum ferrtin and inflammatory fac-tors(hs-CRP ,TNF-αand IL-6) of 4 groups were measured .[Results] Compared with the control group ,serum ferritin and inflammatory factors were increased obviously ( P<0 .01) .With the progression of DN ,serum ferritin and inflammatory factors were increased correspondingly ,and there was significant difference between DN groups ( P <0 .05) .The elevation of serum ferritin in DN patients was positively related with inflammatory factors ( r >0 .1 ,P<0 .01) .[Conclusion]The overload of iron or over high ferritin may participate in body microinflammation status of patients with early DN and induce the elevation of body inflammatory factors ,and thus participate in the genesis and development of DN .

Expressions of Stem Cell Makers CD133 and CK19 in Nasopharyngeal Carcinoma and Their Clini-cal Significance
To explore the expressions of CD133 and CK19 in nasopharyngeal carcinoma and their clini-copathological significance .[Methods]Immunohistochemical method was used to detect the expressions of CD 133 and CK19 in 112 cases of nasopharyngeal carcinoma and 20 cases of normal tissues .Clinical significance was analyzed .[Re-sults]The expression of CD133 in nasopharyngeal carcinoma was higher than that in normal tissue ,and there was sig-nificant difference between primary carcinoma and metastatic carcinoma ( P < 0 .01) ,but there was no significant difference in the expression of CD133 among groups with different age ,sex ,pathological type and cervical lymph node metastasis .There was no significant difference in the expression of CK 19 between nasopharyngeal carcinoma tissues and normal tissues( P<0 .01) ,but there was significant difference between primary carcinoma and metastatic carci-noma or cervical lymph node metastasis and no cervical lymph node ( P<0 .01) .[Conclusion]CD133 may play an im-portant role in the induction of nasopharyngeal carcinoma metastasis ,and can be regarded as one of the specific mark-ers of nasopharyngeal carcinoma stem cells .CK19 expression is associated with tumor metastasis ,but not suitable to be used as the specific maker of nasopharyngeal cancer stem cells .

Study on the Feasibility of Low Concentration Contrast Medium in Coronary CT Angiography for Standard Body Weight People
To explore the value of low concentration iodine contrast medium combined with low voltage in prospective ECG-trigger gated coronary artery CT angiography (CTA) .[Methods] Totally 59 patients [body mass index(BMI)<24 kg/m2 ] suspected as coronary heart disease were collected continuously and divided into 3 groups randomly .Group A( n=25) and group B( n=14) received 270 mgI/mL and 320 mgI/mL contrast medium and 80 kV and 100 kV tube voltage respectively ,while group C( n = 20) received 370mgI/mL contrast medium and 120 kV tube voltage . Prospective ECG-trigger sequence scan was performed in all patients of 3 groups .The effective radiation dose(ED) was recorded .Mean CT value of aorta root was measured .CTA image quality of all patients was scored .[Results] The ED in group A ,B and C were (2 .00 ± 0 .56) ,(2 .51 ± 0 .52) and (4 .17 ± 1 .55)mSv respectively ,and there were significant differences among 3 groups( P <0 .05) .The enhanced degree of aortic root in group A ,B and C were (486 .8 ± 113 .98) ,(407 .43 ± 83 .63) and (439 .73 ± 72 .12)HU re-spectively ,and there was significant difference only between group A and group B ( t =2 .28 ,P <0 .05) .Group A ,B and C showed 225 ,126 and 126 coronary segments ,respectively .The excellent rate of group A ,B and C were 94 .67% ,89 .68% and 94 .67% respectively ,and there was significant difference between group B and C (χ2= 4 .00 ,P<0 .05) ,but there was no significant difference between group A and group B or between group A and group C(χ2 = 4 .00 and 0 .17 ,all P >0 .05) .[Conclusion] Low concentration iodine contrast medium combined with low tube voltage in coronary artery CTA scan for standard and low body weight people can obtain satisfactory diagnosis image quality and reduce injection dose and ED of iodine obviously .

Value of Copeptin and APACHE Ⅱ Score in Evaluating the Prognosis of Patients with Acute Pa-raquat Poisoning
To explore clinical value of Copeptin ,acute physiology and chronic health evaluation II (APACHE II) in the evaluation of the prognosis of acute paraquat poisoning (APP) .[Methods]Totally 126 pa-tients with APP were selected .When clinical death during hospital stay was taken as the endpoint ,all patients were divided into survival group( n =58) and death group( n=68) .Venous blood 3ml of all patients was drawn at 2h ,24h and 7d after admission .The level of Copeptin was detected .APACHE Ⅱ score was also continuously recorded .[Results] The poisoning dose in death group was significantly greater than that in survival group ,while there was no significant difference in rescue time ,first time of hemoperfusion and perfusion frequency ( P>0 .05) . The level of Copeptin in death group at 2h after admission was significantly higher than that in survival group ( P<0 .05) ,while there was no significant difference in APACHE Ⅱ score between two groups ( P >0 .05) .The level of Copeptin in survival group at 24h after admission was decreased to the normal ,but that in death group was still significantly higher than that in survival group ( P < 0 .05) ,and there was no significant difference in A-PACHE Ⅱ score between two groups( P>0 .05) .The level of Copeptin in death group at 7d after admission was continuously decreased ,but APACHE Ⅱ score was increased obviously ,and there was significant difference be-tween two groups( P <0 .05) .Oral dose of pesticides in patients with APP was positively correlated with Copep-tin and APACHE Ⅱ score .[Conclusion]Continuous measurement of Copeptin and APACHEⅡ score has impor-tant clinical significant in the evaluation of the prognosis of patients with APP and reasonable distribution of medi-cal resources .

Analysis on Allergens of 2000 Cases of Urticaria in Dongguan Region

To analyze and explore the distribution of allergens in 2000 patients with urticaria in Dongguan region .[Methods] A total of 2000 patients with urticaria were selected .Standard measurement method Allergopharm pricking needle stock solution was used for allergen test .The result of the allergens was analyzed .[Results]Among all the allergens of 2000 patients with urticaria ,dust mites ,cockroaches and house dust mites were the most commonly seen , and shrimp ,bee and mussels were the second .[Conclusion] Dust mites ,cockroaches and house dust mites are the fre-quent allergens of patients with urticaria in Dongguan region .

Clinical Efficacy of Twice Intensity Modulated Radiation Therapy Plans for Locally Advanced Nasopharyngeal Carcinoma

To explore the necessity and feasibility of twice intensity modulated radiation thera-py(IMRT ) plans for locally advanced nasopharyngeal carcinoma through the dosiology comparison of IMRT therapy plan by stages for twice .[Methods]A total of 50 patients with locally advanced nasopharyngeal carci-noma were chosen as the research objects and randomly divided into experimental group and control group with 25 patients in each group .The patients in two groups were treated by chemotherapy for 2~4 periods ,and then IMRT radiotherapy .The experimental group was given twice IMRT plan design ,while the control group was given once IMRT plan design .The dose changes of target area and the surrounding sensitive organs in two groups were observed and compared .[Results] Mean dose(Dmean) and V30 of bilateral parotid in control group were obviously higher than those in experimental group ,and there was significant difference between two groups( P <0 .05) .Dmax and V50 of brain stem and spinal cord in control group were obviously higher than those in experimental group ,and there was significant difference between two groups ( P<0 .05) .[Con-clusion]The twice IMRT plans for the treatment of locally advanced nasopharyngeal carcinoma have a certain advantage for ensuring radiation dose of target area and normal tissue dose and protecting parotid function and important organs such as brain stem and spinal cord effectively .

To evaluate the efficacy of proximal femoral intramedullary nail(PFNA) and the in-verted femoral condyle LISS titanium plate for the treatment of intertrochanteric fracture .[Methods] A total of 65 inpatients with femoral intertrochanteric fracture in our hospital from Aug .2010 to June 2012 were col-lected .All patients were treated by surgery .Among 57 patients with follow up ,27 patients were fixed with PFNA and 30 patients were fixed with the inverted femoral condyle LISS titanium plate .The average operation time ,blood loss ,length of incision ,fracture healing time ,internal fixation failure cases ,thigh pain score (VAS) at 6 months after operation and postoperative hip score (Harris score) were compared between two groups .[Results] Of 65 patients ,57 patients were followed up for 6~24 months .Intraoperative blood loss , length of incision and internal fixation failure cases in PFNA group were less than those in the inverted femoral condyle LISS titanium plate group .There was no significant difference in operation time ,fracture healing time ,VAS score at 6 months after the operation and Harris score between two groups .[Conclusion] The two methods in the treatment of intertrochanteric fracture can obtain satisfactory efficacy .PFNA has simple opera-tion and little trauma ,so it is better than the inverted femoral condyle LISS titanium plate .

Significance of HbA1c Screening in the Diagnosis of Postpartum Recurrence of Gestational Dia-betes Mellitus and Its Impact on Newborns

To explore the efficacy of hemoglobin A 1c(HbA1c) screening in the diagnosis and detec-tion of 6-month-postpartum recurrence of gestational diabetes mellitus (GDM ) and the influence of interventional ther-apy on body weight and blood glucose of newborns .[Methods] Totally 231 GDM patients and 240 healthy pregnant women were enrolled in the study .Body weight ,fasting blood glucose(FBG) ,oral glucose tolerance test(OGTT) and HbA1 level of newborns at 6 weeks after delivery were measured and compared .The recurrence rate of diabetes mellitus was calculated .[Results] The recurrent case number of GDM patients detected by FBG ,OGTT and HbA1c at 6 weeks after delivery were 7(3% ) ,8(3 .5% ) and 14(6 .1% ) ,respectively .The sensitivity and specificity of HbA1c≥43mmol/mol for the detection of diabetes mellitus were 100% and 92 .6% ,respectively .The sensitivity and specificity of HbA1c≥48mmol/mol for the detection of diabetes mellitus were 73 .6% and 98 .5% ,respectively . There was no significant difference in blood glucose and body weight between newborns of GDM patients and normal newborns( P>0 .05) .[Conclusion] HbA1c≥48mmol/mol is more suitable for the early diagnosis of GDM postpar-tum diabetes mellitus .

Observation on the Efficacy of Multiplex-endoscopy Combination Therapy for Complex Renal Cal-culi
To explore the efficacy of multiplex-endoscopy combination therapy for complex renal calculi .[Methods]A total of patients with complex renal calculi received multiplex-endoscopy combination thera-py .Among them ,35 patients underwent rigid combined with flexible ureteroscope lithotripsy ,while 26 patients underwent single-channel percutaneous nephroscopy combined with flexible ureteroscope lithotripsy .[Results] In the 35 patients undergoing rigid combined flexible ureteroscope lithotripsy ,the success rate was 97 .14% (34/35) , and the stone free rate was 83 .86% (29/35) ,and the surgery time was 50~100min ,and the length of stay after operation was 2~7 days .In the 26 patients undergoing single-channel percutaneous nephroscope combined with flexible ureteroscope lithotripsy ,all the stones in renal pelvis and many stones in kidney calices were eliminated , and the stone free rate in the first stage was 80 .77% (21/26) ,and the total stone free rate was 92 .30% (24/26) , and the surgery time was 50~140min ,and the indwelling time of nephrostomy tube was 5~35 days and the length of stay after operation was 6~10 days .[Conclusion] Multiplex-endoscopy combination therapy for complex renal calculi can improve the stone free rate ,reduce operation wound ,shorten operation time and decrease operation risk and complications .Therefore ,it is a better treatment method for complex renal calculi .

Study of Clinical Efficacy of Gastrointestinal Motility Drug Combined with Azintamide for the Treatment of Bile Reflux Gastritis

To explore the clinical efficacy of azintamide combined with gastrointestinal motili-ty drug mosapride for the treatment of bile reflux gastritis .[Methods]Totally 120 patients with bile reflux gastritis in our hospital from Dec .2011 to Dec .2013 were selected and randomly divided into control group ( n=60) treated with aluminum magnesium carbonate and mosapride and the observation group ( n=60) treated with azintamide and mosapride .Clinical efficacy was compared between two groups .[Results] After treat-ment ,the total effective rate in observation group was 86 .67% ,which was significantly higher than that in control group ,and there was significant difference between two groups ( P < 0 .05) .Compared with control group ,the symptom scores in observation group were obviously improved ,and there was significant difference between two groups( P < 0 .05) .The frequency of bile reflux and most general reflux time in observation group were obviously lower than those in control group ,and there was significant difference ( P < 0 .05) .[Conclusion] Azintamide combined with gastrointestinal motility drug mosapride can not only promote regular bile secretion ,but also increase gastrointestinal motility and food digest ability ,improve clinical uncomfortable symptoms and is helpful for the improvement of pathogenetic condition and quality of life through the synergis-tic effect .Therefore ,it is worthy of clinical application .

Comparative Study Between the Scores and Gastroscopic Findings of Gastroesophageal Reflux Disease in Adolescents
To explore the questionnaire scores and gastroscopic findings of gastroesophageal reflux disease(GERD) in adolescents .[Methods]Totally 51 patients with GRED in our hospital from Jan . 2010 to Jan .2014 were chosen .Other 50 concurrent patients without GRED in the same department were cho-sen as the control group .The diagnostic critical value was calculated .The diagnostic value between question-naire scores of GERD(GERDQ) and gastroscopy of GERD was compared .[Results]The distribution result of GERDQ scores of patients showed that GERDQ in GERD group was mainly 8~11 scores which the proportion of patients was 62 .7% ,while that in control group was mainly 7~9 scores which the proportion of patients was 50 .0% .There were 42 patients with positive results of gastroscopy (82 .4% ) and 9 patients with negative results(17 .6% ) in GERD group ,while there were 14 patients with positive results (28 .0% ) and 36 patients with negative results (72 .0% ) in control group .According to ROC result pf GERDQ score ,the diagnostic sensitivity and specificity were the best when the critical value was 7 .5 .The sensitivity ,specificity ,positive predictive value and negative predictive value were 84 .3% ,56 .0% ,66 .6% and 77 .4% ,respectively .[Con-clusion]Both GERDQ score and gastroscopy for the diagnosis of GERD have good effect .If the critical value of GERDQ for the diagnosis of GERD was 7 .5 and the relative gastroscopic findings are mainly LA classifica-tion A and Band in adolescents ,the pathogenetic condition of GERD in adolescents is generally milder .

Observation of the Efficacy of 25G Minimally Invasive Vitrectomy for the Treatment of Malig-nant Glaucoma

To explore clinical efficacy of 25G minimally invasive vitrectomy via flat portion of ciliary body for the treatment of patients with malignant glaucoma .[Methods] A total of 21 patients with ma-lignant glaucoma undergoing 25G minimally invasive vitrectomy that central and peripheral basal vitreous body were excised in ophthalmology department of our hospital from April 2012 to Sept .2013 were chosen .All pa-tients were followed up for 6 months after operation .The cornea ,the depth of anterior chamber ,intraocular pressure and complications before and after operation were observed .[Results]Corneal edema of patients after operation was obviously regressive .The depth of anterior chamber increased from preoperative (1 .16 ± 0 .32) mm to postoperative (2 .37 ± 0 .59)mm ,and there was significant difference between before and after operation ( P <0 .05) .Intraocular pressure decreased from preoperative (33 .23 ± 6 .81)mmHg to postoperative (16 .92 ± 4 .51)mmHg ,and there was significant difference between before and after operation ( P<0 .05) .The fun-dus oculi could be observed clearly .There was no serious complication .[Conclusion]The 25G minimally inva-sive vitrectomy via flat portion of ciliary body can deepen anterior chamber ,decrease intraocular pressure ,alle-viate the symptoms and reduce the complication ,so it is one of safe and effective methods for the treatment of malignant glaucoma .

Laminectomy for the Treatment of Intraspinal Tumor
To explore clinical experience of laminectomy for the removal of intraspinal tumor .[Methods] Sixteen patients with intraspinal tumor from June 2010 to June 2013 underwent laminoplasty after posterior laminectomy for the removal of intraspinal tumor .All patients were followed up for average 12 months .The change of neurological function of patients before and after operation was compared retrospective-ly .[Results] Of 16 patients ,13 patients underwent total resection and 3 patients underwent subtotal resec-tion .Nerve root pain after operation was recovered within 1 week .Limb and body motor sensory disorder were gradually improved within 3 months .Of 3 patients with sphincter dysfunction ,2 patients were obviously improved within half a year ,and 1 patient had no obvious improvement of sphincter dysfunction .All patients were followed up in clinic services .The average follow up time was 12 months .MRI showed no tumor recur-rence .Among 2 patients with unilateral low limb weakness due to intraoperative spinal root drag ,1 patient re-covered well after half a year of follow up ,and 1 patient had no obvious improvement .No other complications occurred .[Conclusion]Posterior laminectomy for the treatment of intraspinal tumor is safe ,reliable and satis-f ied .

Study on the Expression of MMP-9 and CD34 in Bladder Urothelial Carcinoma and Their Corre-lation
To explore the expression of matrix metalloproteinase-9(MMP-9) and human leuko-cyte differentiation antigen-34(CD34) in bladder urothelial carcinoma and its significance .[Methods] The ex-pression of MMP-9 and CD34-labelled microvessel density (MVD) in 60 cases of bladder urothelial carcinoma and 15 cases of normal bladder were measured by S-P immunohistochemical method .[Results]The expression rate of MMP-9 and the average CD34-labelled MVD in bladder urothelial carcinoma were 58 .33% (35/60) and 20 .91 ± 8 .83 respectively ,which were obviously higher than those in normal bladder tissues [6 .67% (1/15) and (9 .49 ± 2 .730 ,respectively)] ,and there was significant difference between two groups( P <0 .01) .The expression rate of MMP-9 and the average CD34-labelled MVD in bladder urothelial carcinoma were correlated with the degree of histological differentiation ,invasion depth(clinical stage) and tumor size( P <0 .05) ,but not correlated with single or multiple tumor ,age and sex( P > 0 .05) .The CD34-labelled MVD in patients with positive expression of MMP-9 was increased with the intensity of MMP-9 expression ,and there was posi-tive correlation between them ( P <0 .01) .[Conclusion] MMP-9 expression and CD34-labelled MVD in blad-der urothelial carcinoma are increased obviously ,and can be used to be the important indicators of malignancy and prognosis of bladder urothelial carcinoma .

Effect of Triacontanol on Myocardial Ischemia-reperfusion Injury in Isolated Rat Heart

To explore the effect of triacontanol(TRIA) on myocardial ischemia-reperfusion in-jury(IR) in isolated rat cardiac IR model .[Methods]Langendorff perfusion device was used to establish rat i-solated heart IR model .All rats were randomly divided into normal group ,different dose TRIA+IR group and TRIA(2 .5 g/ml) group .The normal group was given whole heart continuous perfusion for 90min .The IR group was given the balance for 30min and then reperfusion for 30min after 30min-ischemia .The TRIA-IR group was given 25 ,15 ,5 and 2 .52 .5 g/ml TRIA at 5min before ischemia and then the same treatment as IR group .The TRIA group was given the medication for 5min after 30min-balance and then continuous perfusion for 30min .Heart rate(HR) ,coronary flow (CF) ,left ventricular pressure (LVP) and maximum velocity of LVP variation(dp/dtmax ) were observed .The level of creatine kinase(CK) in coronary effluent was measured .[Results]Compared with normal group ,IR could induce the obvious heart dysfunction as shown by a decrease in HR ,CF ,LVP and dp/dtmax and an increase in CK level .Heart perfusion with different doses of TRIA (25 , 15 ,5 and 2 .5 g/mL) for 5min aggravated IR-induced cardiac dysfunction and even heart arrest as shown by a further decrease in HR ,CF ,LVP and dp/dtmax and an increase in CK level .TRIA(2 .5 g/mL) alone also in-hibited rat cardiac function which was not recovered for continuous 30min .[Conclusion]TRIA can inhibit cardi-ac function and aggravate myocardial IR injury in Langendorff rat .

To observe the changes of retinal nerve fiber layer (RNFL ) thickness around optic disc and macular retinal thickness in patients with type 2 diabetes mellitus (T2DM ) by optical coherence tomography (OCT) .[Methods]OCT was used to measure the changes of RNFL thickness around optic disc and macular retinal thickness in normal control group ,non-diabetic retinopathy (NDR) group and non-proliferative diabetic retinopathy (NPDR) group .[Results]RNFL thickness beside optic papilla in NDR group was thinner than that in control group , but that in NPDR group was thicker than that in control group .Macular retinal thickness in NDR group was thinner than that in control group ,but that in NPDR group was thicker than that in control group .[Conclusion] OCT can quantitatively observe the changes of RNFL thickness around optic disc and macular retinal thickness ,and is a reliable method for the diagnosis and treatment of diabetic retinopathy .

Study of the Effect of Homocysteine on Macroangiopathy in Patients with Type 2 Diabetes Mel-litus
To explore the effect of serum total homocysteine (tHcy ) on macroangiopathy in patients with type 2 diabetes mellitus(T2DM ) .[Methods] Totally 1441 T2DM patients from 29 tertiary grade A hospitals in Shanghai ,Beijing ,Guangdong ,Nanjing ,Liaoning ,Tianjin and Zhejiang were collected .The Fasting serum tHcy was measured .All patients received physical examination .The levels of blood glucose , blood lipids and vitamin B6 were also measured .The related data of diabetic complications were obtained by questionnaire survey .[Results]The incidence rate of diabetic macroangiopathy was 16 .17% (233/1441) .Ser-um tHcy level of the group with complications was (14 .28 ± 8 .76)μmol/L ,which was significantly higher than that of the group without complications[(11 .55 ± 5 .50)μmol/L]( P <0 .001) .After adjustment for age and sex ,the odds ratio(OR) value of diabetic macroangiopathy in the group with serum tHcy >15μmol/L was 2 .345(95% CI 1 .665 to 3 .302) ,and that of diabetic coronary artery diseases was 1 .932(95% CI 1 .104~3 .380) ,and that of diabetic cerebrovascular diseases was 2 .056(95% CI 1 .331~3 .176) ,and that of diabetic peripheral vascular diseases was 3 .004(95% CI 1 .656~5 .450) .[Conclusion] Hyperhomocysteinaemia can in-crease the risk of macroangiopathy in patients with T 2DM .

Comparison of Efficacy and Complications of Two Surgical Methods for the Treatment of Inva-sive Pituitary Adenomas
To explore the efficacy of microscopic vs .neuroendoscopic single nostril transsphe-noidal approach surgical methods for the treatment of invasive pituitary adenomas .[Methods]Totally 60 pa-tients with invasive pituitary adenoma in our hospital from Jan .2013 to Sept .2013 were enrolled in the study . All patients were randomly divided into two groups .The observation group( n =30) underwent neuroendo-scopic single nostril transsphenoidal approach surgery ,while the control group( n=30) underwent microscop-ic single nostril transsphenoidal approach surgery .The efficacy was compared between two groups .[Results]The operation time ,intraoperative blood loss ,postoperative hospital stay and other indicators in observation group were obviously better than the control group ,and there was significant difference between two groups ( P <0 .05) ,but there was no significant difference in vision improvement and total tumor resection rate be-tween two groups( P >0 .05) .After operation ,nasal septum defect and sinus infection rate in control group were obviously higher than those in observation group .The incidence rate of total complications in control group was 73 .3 % (22/30) which was obviously higher than that in observation group(20% ,6/30) ,and there was significant difference( P <0 .05) .[Conclusion] Neuroendoscopic single nostril transsphenoidal approach surgery for the treatment of invasive pituitary adenoma has very significant efficacy with short operation time , less intraoperative blood loss ,and can shorten postoperative hospitalization time and reduce the incidence of postoperative complications ,and is helpful for the patients'recovery .

Expression of Mta-1 in Thyroid Tumor and Its Correlation with Tumor Angiogenesis

To explore the expression of metastasis associated gene (Mta-1) in thyroid tumor and its correlation with tumor angiogenesis .[Methods] Paraffin-embedded tissue samples from 160 cases of thyroid tumor were selected .Among them ,120 cases were thyroid cancer and 40 cases were thyroid adenoma .Other 50 cases of normal thyroid tissue were selected .The expression of Mta-1 and microvessel count (MVC) were detected by using immunohistochemical SP method .The correlation between Mta-1 and tumor angiogenesis was analyzed .[Results]The positive expression rate of Mta-1 and MVD value in thyroid cancer were obviously higher than those in normal thyroid and thyroid adenoma ,and there was significant difference( P<0 .05) .The positive expression of Mta-1 and MVD in 120 cases of thyroid cancer were closely related to lymph node metastasis ,peplos integrity and TNM stage ( P <0 .05) ,but not related to other clinicopathological indicators ( P > 0 .05) .There was positive correlation between Mta-1 expression and MVD( r =0 .625 ,P < 0 .01) .[Conclusion]Abnormal expression of Mta-1 is closely related with the occurrence ,infiltration ,lymph node metastasis and other malignant potentials of thyroid cancer ,and may in-volve in tumor neo-angiogenesis of thyroid cancer .

Value of Neutrophil Gelatinase-associated Lipocalin in the Diagnosis of Preeclampsia
To explore the value of neutrophil gelatinase-associated lipocalin(NGAL) in the di-agnosis of preeclampsia(PE) .[Methods] The parturients who received prenatal examination in the outpatient department of our hospital from Jan .2010 to Nov .2012 were randomly enrolled in the study .Among them , 50 cases were diagnosed as PE(observation group) .Other 30 healthy single-birth parturients were selected as control group in the same period .Clinical data of two groups were collected .ELISA was used to detect and compare NGAL value in blood and urine .[Results] There was no significant difference in maternal age and progestation body mass index (BMI) between observation group and normal control group ( P >0 .05) .There were significant differences in fetal birth weight ,systolic pressure ,diastolic pressure and NAGL in blood and urine between two groups( P <0 .01) .The sensitivity and specificity of blood NGAL in the diagnosis of PE were 78% (39/50) and 90% (27/30) ,respectively .Kappa test showed that k value was 0 .65 with good consis-tency .[Conclusion] The expression of NGAL in patients with PE is increased and may be used as a marker for predicting PE .

Study of the Limit Loading Test of Growing Rod Internal Fixation System of Sliding Pedicle Sc rew
To compare between the growing rod internal fixation system of sliding pedicle screw and the universal spine system(USS) by the limit loading test in order to provide the basis for further improvement and clinical application of the system in the future .[Methods] A total of 12 spine specimens of fresh porcine(3 thoracic vertebra functional units ) were randomly divided into the sliding group (attachment screws in the 3rd vertebra and sliding pedicle screws in 1st vertebra) and fixation group (attachment screws in 1st and 3rd vertebra) .The limit loading of antexion and rotation state of USS and the growing rod internal fix-ation system of sliding pedicle screw was tested .All data were analyzed by SPSS 18 .0 software .[Results]At antexion state ,screw loosening occurred in the sliding group at about 400N ,while screw loosening occurred in the fixation group at about 1800N ,and there was significant difference between two groups ( P <0 .05) .At rotation state ,screw loosening occurred in the sliding group at about 5 degrees ,while screw loosening oc-curred in fixation group at about 10 degrees ,and there was significant difference between two groups ( P <0 .05) .[Conclusion] The limit loading of the growing rod internal fixation system of sliding pedicle screw is smaller than that of USS .It indicates that the growing rod internal fixation system of sliding pedicle screw has some deficiencies which need to be further optimized in order to increase the strength of the fixation .

To assess the impact of perioperative synthetic insulation measures on the result of operation for puerperas with hemorrhagic shock .[Methods] Totally 200 patients were randomly divided into two groups .The control group was treated with conventional surgical method ,while the observation group was given perioperative synthetic insulation measures .Blood pressure ,heart rate ,respiration ,nasopharyngeal temperature ,blood coagulation ,blood gas analysis results ,hematocrit ,blood lactic acid value ,the incidence of postoperative shivering ,the rate of successful rescue and hospital stay were compared between two groups .[Results] The vital signs of patients in observation group were all better than those of patients in control group ( P <0 .05) .The incidence of postoperative shivering in observation group was 16 .00% ,which was lower than that in control group(37 .00% ) ,and the rate of successful rescue was 93% ,which was higher than that in control group(76 .00% ) .The length of stay in observation group were (10 .20 ± 5 .10)d ,which was shorter than that in control group .There were significant differences( P<0 .05) .[Conclusion]Perioperative synthetic insulation measures can reduce the incidence of maternal complications of puerpera with hemorrhagic shock , improve the success rate of operation rescue ,help the recovery of patients and shorten the hospital stay .

To explore clinical significance of umbilical blood gas analysis ,C-creative protein (CRP) and brain injury in the diagnosis and prognosis evaluation of neonatal asphyxia .[Methods] A total of 60 neonates with asphyxia in our department from Sept .2011 to Dec .2012 were chosen and divided into mild asphyx-ia group( n=36) and severe asphyxia group( n=24) .In the same period ,30 healthy full-term neonates in our de-partment were selected as control group .Umbilical arterial blood of all neonates was drawn and treated by antico-agulation by heparin for blood gas analysis .Other femoral venous blood was drawn to detect CRP level .The inci-dence of hypoxic ischemic encephalopathy and mortality was used to assess the prognosis of patients in two groups .[Results] PH and PaO2 in umbilical arterial blood of mild and severe asphyxia groups were decreased significantly , while PaCO2 and BE in umbilical arterial blood and plasma CRP in femoral vein of mild and severe asphyxia groups were significantly higher than those of control group( P <0 .05) .By comparison between mild group and severe group ,the severer the asphyxia ,the more significant the changes( P <0 .05) .The incidence of hypoxic ischemic encephalopathy and mortality were related with the degree of asphyxia .Compared with mild asphyxia group ,the incidence and mortality in severe asphyxia group were increased significantly ( P<0 .05) .[Conclusion] The degree of neonatal asphyxia is closely related with acid-base balance disturbance and hypoxic ischemic encephalopathy . The synthetic evaluation by combining Apgar score ,umbilical blood gas analysis and plasma CRP can be used in the diagnosis and prognosis of neonatal asphyxia .

Comparison of the Efficacy of Carbamazepine vs .Compound Betamethasone for the Treatment of Primary Trigeminal Neuralgia

To explore clinical efficacy of carbamazepine vs .compound betamethasone for the treatment of primary trigeminal neuralgia(PTN) .[Methods]A total of 80 patients with PTN in our hospital ca-ses of patients with PTN from Jan .2013 to Jan .2014 were selected and divided into carbamazepine group (group A) and compound betamethasone group(group B) by random draw method .The treatment course was 4 weeks .The efficacy and safety were compared between two groups .[Results] The total effective rates in group B at 1 ,2 ,3 and 4 weeks after treatment were obviously higher than those in group A ,and there was significant difference between two groups( P <0 .05) .The VAS and QS scores in group B at 1 ,2 ,3 and 4 weeks after treatment were lower than those in group A ,and there was significant difference between two groups( P <0 .05) .The incidence rates of complications such as nausea ,drowsiness and skin rash in group B were obviously lower than those in group A ,and there was significant difference in the total incidence rate of adverse reaction between two groups ( P<0 .05) .[Conclusion]Compared with carbamazepine ,compound be-tamethasone can more significantly alleviate the pain degree of PTN patients and improve sleep quality ,and has more remarkable efficacy ,good tolerance ,low incidence of adverse reaction and high safety ,and may be used as the preferred drug for clinical treatment of PTN .

Treatment of Paraquat Poisoning and Analysis of the Influential Factors of Prognosis

To explore the treatment method of paraquat poisoning and analyze the influential factors of prognosis .[Methods]Clinical data of 34 patients with paraquat poisoning admitted to our hospital from Aug .2011 to May 2013 were analyzed retrospectively .All patients were divided into survival group and death group .The effect of the poisoning concentration ,first vomiting time ,the visiting time ,first perfusion time and complications on the prognosis was compared between two groups .The changes of paraquat concen-tration and superoxide dismutase(SOD) before and after blood perfusion were measured .[Results] Of 34 pa-tients ,10 patients survived(survival group) and 24 patients died (death group) ,and the mortality rate was 70 .59% .Plasma levels of paraquat after blood perfusion were obviously decreased ,but SOD activity was obvi-ously increased ,and there was significant difference between before and after blood perfusion ( P<0 .05) .The poisoning dose in the death group was obviously more than that in the survival group ,but the first vomiting time in the death group was obviously earlier than that in the survival group ,and there was significant differ-ence between two groups( P<0 .05) .The visiting time and first blood perfusion time in the death group were obviously later than those in the survival group ,and there was significant difference between two groups ( P<0 .05) .The incidence of complications in the death groups is higher than that in the survival group ,and there was significant difference between two groups( P<0 .05) .[Conclusion] Emergent first-aid measure should be performed as quickly as possible for patients with paraquat poisoning .Many factors can affect the prognosis . The poisoning doses ,SOD activity ,the concentration of paraquat ,first vomiting time ,the visiting time ,first blood perfusion time and complications are the very critical for the prognosis .Therefore ,early comprehensive treatment to deal with patients with paraquat poisoning should be taken in clinical practice .

One Case of Giant Temporal Facial Recurrent Dermatofibrosarcoma Protuberans and Literature Review
To explore clinical features ,treatment and prognosis of dermatofibrosarcoma protu-berans(DFSP) .[Methods]Clinical data of 1 patient with rare giant temporal facial recurrent DFSP were ana-lyzed retrospectively and discussed combining with the literature .[Results]The patient underwent the second operation with wide excision ,local tissue flap repair and free skin graft .The patient received radiotherapy of temporal and facial lesions .No recurrence was found during 1 year of follow up .[Conclusion]DDSP is easy to recur after local excision .Wide excision is the key to reduce the recurrence rate .Clinicians should raise the awareness of the disease ,take early correct diagnosis ,regular and complete initial treatment and the combined therapy .

Change of Homocysteine in Patients with Type 2 Diabetic Nephropathy and Its Significance

To analyze the change of serum homocysteine(Hcy) in patients with type 2 diabetic nephropathy and its significance .[Methods] Patients with type 2 diabetes mellitus in our hospital from Feb . 2012 to Dec .2013 were selected .According to urinary albumin excretion rate(UAER) determination results , all patients were divided into group A (simple diabetic group) ,group B(diabetes mellitus with early nephropa-thy) ,group C(diabetes mellitus with clinical nephropathy ) and group D(normal healthy control group) .FBG , SCr ,BUN and Hcy were measured and analyzed by statistics .[Results] Compared with control group ,Hcy levels in group A ,B and C were markedly increased ,and there was significant difference( P <0 .05) .[Con-clusion] Hyperhomocysteinaemia is an independent risk factor of type 2 diabetic nephropathy .

To establish digital bladder by using polar coordinate system in order to label the lo-cation of bladder lesion accurately .[Methods]With the properly filled bladder as round shape ,internal ure-thral orifice geometrical center as pole and left and right ureteral opening line perpendicular bisector as polar axis ,the polar coordinate system was set up .[Results]The polar coordinate for labeling the location of bladder lesion was a simple method ,and could obtain intuitive ,convenient and high accurate location parameter data , and also met clinical demand for communication and operation location .[Conclusion] Polar coordinate system for establishing digital bladder is feasible in theory and convenient in practice ,and has wide application pros-pect ,so it is worthy of promotion .

Study of the Relationship Between Estrogen and Osteoporosis Among Postmenopausal Women with Type 2 Diabetes Mellitus
To explore the changes of estrogen level and bone density among postmenopausal women with type 2 diabetes mellitus(T2DM ) and osteoporosis (OP) and their correlation .[Methods] Postm-enopausal women were divided into T2DM group(group T ,n=62) ,T2DM with osteoporosis group(group P , n =58) and normal control group(group N ,n=60) .Bone metabolism ,glucose metabolism ,sex hormone and other indicators among three groups were compared .[Results]Bone alkaline phosphatase(sBAP) ,osteocalcin (sOC) ,type I collagen cross-linked C end peptide(sCTx) and urine type I collagen cross-linked N end peptide (uNTx) of group P were obviously lower than those of group T .Compared with group T and group N ,serum FSH and LH were elevated ,but E2 was decreased in group P( P <0 .05) .Correlation analysis showed that bone mineral density (BMD) had significantly negative correlation with the duration of diabetes ,age ,HbA1C , FPG ,FSH ,LH and uNTx/Cr ,but no correlation with sBAP ,sOC and sCTx .[Conclusion]Osteoporosis of postmenopausal women with T2DM is related to glucose metabolism and estrogen level .

Clinical Analysis of 90 Diabetic Patients with Bloodstream Infection

To explore the distribution of pathogens and clinical characteristics of diabetes mel-litus(DM ) patients with bloodstream infection .[Methods] Clinical data of 90 DM patients with bloodstream infection in our hospital were analyzed retrospectively .[Results] The most common pathogens of DM patients with bacteremia were Escherichia coli followed by Staphylococcus and Klebsiella pneumonia ,which accounted for 44 .4% ,27 .7% and 15 .6% respectively ,and the detection rate of fungi was 1 .1% .The pathogens of DM patients with sepsis were Klebsiella pneumonia ,Staphylococcus ,Escherichia coli and others in turns ,which accounted for 45 .8% ,20 .8% ,20 .8% and 12 .6% ,respectively .Compared with DM patients without sepsis , patients with sepsis were older ,long duration of fever ,high level of HbA1c ,high C-reactive protein (CRP) and low serum lipids ,and there was significant difference between two groups( P <0 .01 ,P <0 .05) .[Con-clusion] The specificity of DM combined with bloodstream infection should be emphasized so as to guiding clin-ical diagnosis and treatment .

Efficacy of Fluoxetine Hydrochloride Combined with Xiaoyao Pill for the Treatment of Postpartum Depression in Puerperas after Induction of Labor in Second Trimester
To explore the efficacy of fluoxetine hydrochloride combined with Xiaoyao pill for the treatment of postpartum depression in puerperas after induction of labor in second trimester .[Methods]A total of 52 puerperas with postpartum depression after induction of labor in second trimester were randomly divided into control group and observation group with 26 cases in each group .The control group was only treated with fluoxe-tine hydrochloride ,while the observation group was treated with fluoxetine hydrochloride and Xiaoyao pill .The treatment in two groups lasted four weeks .The efficacy and adverse reaction of two treatment methods were as-sessed by Hamilton depression scale(HDMA) and adverse reaction scale(TESS) .[Results]The total effective rate of the observation group was 96 .15% ,which was higher than that(92 .31% ) of the control group( P>0 .05) ,but the full recovery rate of the observation group was significantly higher than that of the control group ( P<0 .05) . With the prolongation of treatment time ,the deducted rate of HDMA in two groups was increased gradually ( P<0 .05) .The TESS score in observation group at 1st and 2nd week after treatment was obviously less than that in control group[(6 .87 ± 2 .38 ,6 .06 ± 2 .71) vs .(9 .12 ± 3 .03 ,8 .01 ± 2 .97)]( P <0 .05) .[Conclusion] The part-ner treatment of Xiaoyao pill can increase the efficacy of fluoxetine hydrochloride for the treatment of postpartum depression and decrease the adverse reaction in a certain degree .

Analysis of the Efficacy of Epalrestat Combined with Alprostadil for the Treatment of Type 2 Dia-betic Perineuropathy

To observe clinical efficacy of epalrestat combined with alprostadil for the treatment of diabetic perineuropathy(DPN) .[Methods] Totally 88 patients with type 2 diabetes mellitus(T2DM) peripheral neuropathy were randomly divided into control group ( n =44) and observation group( n = 44) .Patients in two groups received strict diet control ,appropriate exercise ,oral blood glucose lowering drug or insulin injection to control blood glucose ,and also intramuscular injection with mecobalamin 0 .5mg once daily .On the basis of the treatment of the control group ,the observation group additionally received oral epalrestat 50mg 3 times per day and alprostadil 20 μg+ 0 .9% sodium chloride by intravenous injection once daily .The treatment course was 2 weeks .[Results] The total effective rate of the improvement of subjective symptoms in observation group (97 .7% ) was significantly higher than that in control group(72 .7% )( P<0 .05) .Motor nerve conduction velocity (MNCV) in two groups was obviously improved ,and the improvement in observation group was better than control group , and there was significant difference ( P < 0 .01) .[Conclusion] The combination of epalrestat ,alprostadil and mecobalamine injection is markedly effective in the treatment of DPN .

Observation on the Effect of Early Massage Combined with Hyperbaric Oxygen or Hydrotherapy on Recent Nerve Motor Function of Severe Neonatal HIE

To explore the effect of early massage combined hyperbaric oxygen therapy ,hydro-therapy on recent nerve motor function of severe neonatal hypoxic-ischemic encephalopathy(HIE) .[Methods]Totally 497 patients with HIE were collected .According to the treatment methods ,all patients were divided into group A ,B and C .Group A received simple early massage ,and group B received massage combined with hyperbaric oxygen therapy ,and group C received massage combined with hydrotherapy .Motor function ,nerve 20-item score criteria(NBNA) score ,body weight ,height ,daily milk intake after treatment were compared a-mong 3 groups .[Results]After treatment ,the proportion of taking grade Ⅲ action in group A was obviously lower than that in group B and group C( P <0 .05) ,but there was no significant difference between group B and group C( P>0 .05) .NBNA scores in group B and group C at 2 and 4 weeks after treatment were signifi-cantly higher than those in group A ( P <0 .05) ,but there was no significant difference in NBNA score be-tween group B and group C( P>0 .05) .The weight ,height and daily milk intake in group B and group C at 4 weeks after treatment were significantly better than group A ( P <0 .05) ,but there was no significant differ-ence between group B and group C( P>0 .05) .[Conclusion]Early massage combined with hyperbaric oxygen or hydrotherapy can promote neural motor function rehabilitation in pediatric patients with HIE and is condu-cive to neonatal growth and development .Therefore ,it is worthy of clinical promotion .

Significance of Micro-inflammatory State Associated Factors in the Diagnosis of Early Diabetic Nephropathy

To explore the significance of micro-inflammatory state factors high-sensitivity C-reactive protein(hs-CRP) ,interleukin-6(IL-6) and tumor necrosis factor-alpha(TNF-α) in the diagnosis of ear-ly diabetic nephropathy(DN) .[Methods]A total of 77 patients diagnosed as type 2 diabetes mellitus(T2DM ) according to the WHO criteria in 1999 were chosen .According to urinary albumin excretion rate (UAER) ,all patients were divided into normal UAER group (NDM group ,UAER< 30mg/24h) ,early DN group (group EDN ,UAER = 30-300mg/24h ,continuous micro-albuminuria state) and clinical DN group (group CDN , UAER>300mg/24h ,continuous great albuminuria state ) .Other 33 healthy subjects were chosen as group CON .Above micro-inflammatory factors in all subjects were measured .[Results] The levels of hs-CRP in group EDN and group CDN were significantly higher than those in group CON and group NDM ( P <0 .01) . The levels of IL-6 in group EDN and group CDN group were significantly higher than those in group CON ( P<0 .01) .The levels of TNF-αin 3 experimental groups were significantly higher than those in group CON ( P<0 .01) ,and there were significant differences among 3 experimental groups( P <0 .01) .[Conclusion] The early micro-inflammatory state factors hs-CRP ,TNF-αand IL-6 in DN patients are elevated ,which indicates that inflammation occurs in early nephropathy of diabetic patients .

Study on the Efficacy of Laparoscopic Combined with Gonadotropin Releasing Hormone Agonist for the Treat-ment of Moderate or Severe Endometriosis
To explore clinical efficacy of laparoscopy combined with gonadotropin-releasing hormone agonists (GnRH-a) for the treatment of moderate or severe endometriosis .[Methods]Clinical data of 50 patients with endometriosis in Hunan provincial people's hospital from Mar .2010 to March 2013 were analyzed retrospectively .According to the treatment methods ,all patients were divided into group A and group B .Preoperative combined with postoperative treatment group (group A ,n=15) was given subcutaneous injection with GnRH-a for 2~3 times before laparoscopy and re-injection for 3 times after laparoscopy .Postoperative treatment group(group B ,n =35) was given subcutaneous injection with GnRH-a for 3~6 times after laparoscopy .All patients were followed up for 12 months after operation .Operation time ,intraoperative blood loss ,post-operative recovery ,pain symptom visual analog scale (VAS ) ,recurrence and adverse reaction were compared between two groups .[Results] The operation time in group A and group B were (65 ± 12)min and (92 ± 16)min ,respectively .The amount of bleeding in group A and group B were (105 ± 12)mL and (190 ± 29)ml ,respectively .The time of out-of-bed activity and anus exhaust time in group A after operation were earlier than those in group B .The VAS scores in group A and group B at 12-month follow up after operation were decreased from preoperative 3 .7(1 .7~6 .5) and 3 .2(1 .4~6 .4) points to 1 .6(1 .0~3 .6) and 1 .6(1 .1~3 .5) points ,respectively .The recurrence rates in group A and group during follow up were 7% (1/15) and 6%(2/35) ,respectively .There was no significant difference between two groups .[Conclusion] Laparoscopy combined with pre-operative and postoperative GnRH-a therapy for the treatment of moderate or severe endometriosis can shorten the operation time ,decrease intraoperative blood loss and promote postoperative recovery .However ,laparoscopy combined with preopera-tive and postoperative GnRH-a therapy has the same efficacy in improving postoperative pain and reducing the recurrence rate as the postoperative GnRH-a therapy .

Clinical Treatment and Efficacy Evaluation of Early Deep Wound Infection after Lumbar Discectomy

To explore clinical treatment methods of early deep wound infection after lumbar discectomy ,and e-valuate the efficacy .[Methods] Totally 17 patients diagnosed as early deep infection after lumbar discectomy in orthopedic de-partment of our hospital from May 2006 to May 2014 .Among all patients ,13 patients underwent anterior and/or posterior re-moval of infection and catheter lavage ,while 4 patients received non-surgical treatment including antibiotics(>4~6 weeks) ac-cording to bacterial culture and drug sensitivity test and bed rest ,and then wore external fixation brace and took out-of-bed ac-tivity at 1~2 weeks after reaching the curative standard of infection .[Results] VAS score of low back pain in 13 surgical pa-tients at 24~28h after operation was improved from preoperative (8 .7 ± 2 .1) points to (3 .2 ± 1 .3) points ,and the duration of antibiotic usage was (35 ± 2)d .Two patients and other patients undergoing relaxation suture could get well healing of wound 4 weeks and 12~16d after stitch removing .The symptoms of 4 patients with non-surgical therapy were obviously improved af-ter about 4 weeks of treatment ,and the average time of antibiotic usage was (28 ± 1)d ,and VAD score of low back pain was improved from preoperative (7 .9 ± 2 .2) points to (2 .3 ± 1 .4) points .All patients were discharged from the hospital after white blood cell(WBC) ,C-reactive protein(CRP) and/or erythrocyte sedimentation rate (ESR) in all patients returned to the normal reexamined by laboratory ,and MRI signal was the normal or obviously improved .The average hospitalization time was (74 ± 2 .5)d .After follow up for 6~60 months ,all patients had no recurrence of infection .Routine X-ray examination and/or CT showed bony fusion and the recovery of spinal stability .MRI showed the normal or obviously improved signals .All pa-tients got clinical healing .[Conclusion] The effective antibiotic therapy (> 4~6 weeks) and bed rest is the main treatment method for patients diagnosed as early deep wound infection after lumbar discectomy .If the drug therapy is invalid with the persistent rising of temperature and white blood cell and without the decreasing or with the rising of CRP and EST persistently in short term ,or the radiographic image shows the destroyed signal in soft tissue or end plate ,the surgical procedure should be undertaken in time which can alleviate the pain quickly ,shorten the treatment course and get clinical healing .

Application of Different Inflated Double-barreled Laryngeal Mask in Gynecological Laparoscop-ic Anesthesia
To explore the efficacy and safety of different inflated double-barreled laryngeal mask in gynecological laparoscopic anesthesia .[Methods] Totally 121 patients undergoing gynecological lapa-roscopy in our hospital from June 2010 to June 2013 were enrolled in the study .According to different intra-cuff inflation capacity in cyst ,all patients were randomly divided into group A ( n =30) ,group B( n =30) , group C( n = 30) and group D( n = 31) .After the induction of general anesthesia ,NO .4 laryngeal masks without gas were intubated in laryngeal of patients .The mask cuffs of patients in 4 groups were filled with dif-ferent capacity of gases(group A with 12ml ,group B with 18ml ,group C with 24ml and group D with 30ml) . The hemodynamic changes and the relationship between intra-cuff pressure(ICP) and complications after the intubation of laryngeal mask in 4 groups were recorded .[Results]There was no significant difference in hemo-dynamic changes among 4 groups after filled with different capacity of gases ( P > 0 .05) .The incidence of perioperative complications in group A were lower than that in group B ,C and D ,and there was significant difference( P <0 .05) .By comparison of ICP among 4 groups ,ICP of group A was the lowest ,and there was significant difference between 4 groups( P < 0 .05) .[Conclusion]The use of NO .4 inflated laryngeal mask with 12ml gases in gynecological laparoscopic anesthesia can reduce the incidence of perioperative complications markedly .Therefore ,it is worthy of clinical application .

To observe the protective effect of remote ischemia preconditioning on myocardium and kidney of patients with rheumatic heart disease during valve replacement .[Methods]A total of 80 patients with rheumatic heart disease undergoing valve replacement were chosen and randomly divided into control group(group A ,n=40) and remote ischemia preconditioning group(group B ,n =40) .In group B ,the right thighs(2~3cm near knee joint) of patients were given 600mmHg pressure by tourniquet for 5min after aortic clamping under cardiopulmonary bypass ,and then the balloon was unclamped for 5min .The cycle was repeat-ed for 3 times .Venous blood samples were drawn to measure troponin I (cTnI) ,B-type natriuretic peptide (BNP),urea nitrogen(BUN),serum creatinine(Scr) and acute renal injury factor(Kim-1) before surgery (T1 ) ,5min before aortic de-clamping(T2 ) ,30min(T3 ) ,4h(T4 ) and 24h(T5 )after aortic de-clamping .[Re-sults]There was no significant difference in age ,weight ,cardiopulmonary bypass time ,aortic clamping time and preoperative cTnI ,BNP ,BUN ,Scr and Kim-1 between two groups( P>0 .05) with comparability .Com-pared with T1 ,the cTnI in two groups at T3 ,T4 and T5 were increased significantly ,while those in group B were significantly lower than those in group B ( P < 0 .05) .Compared before operation ,the BNP in two groups at T5 was increased significantly ,while that in group B was significantly lower than that in group A ( P<0 .05) .Compared with before operation ,BUN and Scr in two groups at T5 were increased significantly ,and those in group B were significantly lower than those in group A ( P<0 .05) .Compared with before operation , Kim-1 in two groups at T4 and T5 was increased significantly ,and that in group B was significantly lower than that in group B( P <0 .05) .[Conclusion]Remote ischemic preconditioning can protect the myocardial and re-nal function in patients with rheumatic heart disease undergoing valve replacement .

Effect of Nursing Quality on the Image Quality of PET-CT Scanning and Management Strategy
To explore the effect of nursing quality on the image quality of PET-CT scanning .[Methods] The nursing factors such as the preparation before examination ,drug injection method and doctor's advice after injection in 57 patients receiving PET-CT scanning were analyzed .The success rate of PET-CT scanning was calculated .The nursing factor affecting the image quality was analyzed .[Results] The success rate of 57 patients was 100% .There were 44 cases(77 .2% ) with 1st image ,9 cases(15 .8% ) with 2nd image , 4 cases(7% ) with 3rd image and 0 case with 4th image .The influential factors of poor PET-CT image included over high physiogenic aggregation in enteric cavity caused by insufficient hydration in intestinal tract of 9 cases in which 18F-FDG uptake in digestive tract of 4 cases was obviously decreased after receiving the delayed scan-ning ,and 3 cases had abnormal increased glucose metabolism due to tracer leakage ,and 1 case had urine con-tamination ,and 1 case had poor concordance between PET and CT image .[Conclusion] Nursing quality can directly affect the quality of PET-CT scanning .Controlling blood sugar level ,strengthening mental nursing for patients ,taking sufficient intestinal hydration ,avoiding drug injection leakage ,advising patients to refrain from contamination of clothes and skin by urine and properly using the sedative for dysphoria patients are very important to improve the quality of PET-CT image .
