Abstract To observe clinical efficacy of epalrestat combined with alprostadil for the treatment of diabetic perineuropathy(DPN) .[Methods] Totally 88 patients with type 2 diabetes mellitus(T2DM) peripheral neuropathy were randomly divided into control group ( n =44) and observation group( n = 44) .Patients in two groups received strict diet control ,appropriate exercise ,oral blood glucose lowering drug or insulin injection to control blood glucose ,and also intramuscular injection with mecobalamin 0 .5mg once daily .On the basis of the treatment of the control group ,the observation group additionally received oral epalrestat 50mg 3 times per day and alprostadil 20 μg+ 0 .9% sodium chloride by intravenous injection once daily .The treatment course was 2 weeks .[Results] The total effective rate of the improvement of subjective symptoms in observation group (97 .7% ) was significantly higher than that in control group(72 .7% )( P<0 .05) .Motor nerve conduction velocity (MNCV) in two groups was obviously improved ,and the improvement in observation group was better than control group , and there was significant difference ( P < 0 .01) .[Conclusion] The combination of epalrestat ,alprostadil and mecobalamine injection is markedly effective in the treatment of DPN .
