Effects of Morphine Treatment on the Protein Levels and Currents of L-type Voltage-gated Calcium Channels in the Rostral Ventromedial Medulla of Rats

【Objective】To investigate the changes of voltage-gated calcium channels (VGCCs) in the rostral ventromedial medulla of SD rats under morphine-induced hyperalgesia. 【Methods】The expression of a1C and a1H subunit proteins in the rostral ventromedial medulla (RVM) was detected by western blot. The VGCCs current of RVM neurons was recorded by the whole-cell patch-clamp technique. 【Results】Our results indicated that the expression of α1C subunit in the RVM significantly increased under morphine-induced hyperalgesia, which was followed by the significant increase in total VGCCs currents and L-type VGCCs currents. 【Conclusion】This data implicates that α1C-subunit-containing L-type VGCCs in RVM may play an essential role as a cellular mechanism leading to hyperalgesia.

Prevalence of Tc17 Cells in the Peripheral Blood of Patients with Osteosarcoma and its Clinical Significance

【Objective】To investigate the prevalence of Tc17 cells in the peripheral blood of patients with osteosarcoma. 【Methods】Flow cytometry was used to evaluate the prevalence of Tc17 cells in the peripheral blood of 41 patients with osteosarcoma and 17 healthy people. In addition, the concentration of IL-17 was measured. The relationship between peripheral blood Tc17 cell level with the concentration of IL-17 and tumor stage was determined statistically.【Results】The prevalence of Tc17 cells in the peripheral blood of patients with osteosarcoma (3.18±1.99%) increased greatly with increased levels of IL-17, compared with the normal control subjects (0.72±0.46%). The differences were statistically significant (P<0.01). The percentage of Tc17 cells was not associated with the concentration of IL-17 or the stages of osteosarcoma. 【Conclusion】The prevalence of Tc17 cells is increased in the peripheral blood of patients with osteosarcoma.These cells may negatively regulate antitumor immune response of patients.

Umbilical Cord Mesenchymal Stem Cells Combined with Cyclsporine in the Treatment of Aplastic Anemia:A 5-Case Report and Literature Review

【Objective】To observe the efficacy and safety of therapy with umbilical cord mesenchymal stem cells (MSC) combined with cyclosporine A (CSA) in aplastic anemia (AA). 【Methods】Five patients with aplastic anemia from January 2011 to December 2015 were collected and reviewed, which included four severe (SAA) cases and one non-severe (NSAA) case. All patients were treated with mesenchymal stem cells (intravenous infusion) and cyclosporine (intraosseous injection). The third to fifth generation of MSC cells were administered intravenously (1×10
6
/kg) to patients once a week for four consecutive weeks. Blood routine and bone narrow smears before and after treatment were checked. Adverse reactions after treatment were recorded. Reports on aplastic anemia published recently were searched for and summarized. 【Results】During an average follow-up period of 28 months ( 6 to 64 months) after MSC plus CSA therapy, two out of five cases were recovered, one was relieved, one was improved, and one was invalid, which was the NSAA case. 【Conclusion】To some extent, the therapeutic method of umbilical cord mesenchymal stem cells combined with cyclosporine for aplastic anemia was effective. There were no significant adverse reactions.

Research on the Prognosis-related Factors of Elderly Patients with Diffuse Large B-cell Lymphoma

【Objective】To discuss the prognosis-related factors of elderly patients with diffuse large B-cell lymphoma (DLBCL). 【Methods】We retrospectively analyzed the clinical data from 70 cases of elderly patients with initial diffuse large B-cell lymphoma from January 2009 to December 2014. The parameters included gender, age, Ann-Arbor staging, B symptoms, ECOG score of performance status (ECOG-PS), serum level of lactate dehydrogenase (LDH), international prognostic index (IPI) and bone marrow invasion. We also compared the survival of patients who received CHOP chemotherapy and R-CHOP chemotherapy.The correlation between these indexes and prognosis was analyzed. 【Results】The median survival of all 70 cases of elderly patients with DLBCL was 41.67 months and the OS rate of 2 years was 69.2%. Single factor analysis showed that age, Ann-Arbor staging, bone marrow involvement, ECOG score, IPI score, value of LDH before treatment and therapeutic regimen were all related with prognosis (P<0.05 or P<0.01). COX regression multiple-factor analysis indicated that ECOG score、IPI score、bone marrow invasion and chemotherapy were the independent factors for predicting the prognosis of the elderly patients with DLBCL (P<0.01). 【Conclusion】ECOG score、IPI score、bone marrow invasion and chemotherapy were independent predictive and prognostic factors in elderly patients with DLBCL. R-CHOP chemotherapy has more advantages than CHOP chemotherapy in improving prognosis of elderly patients with DLBCL.

Effect of Comprehensive Assessment of Elderly on Treatment of Elderly Hypertension in Patients with Metabolic Syndrom

【Objective】To evaluate the effect of comprehensive geriatric assessment on the treatment of geriatrichypertension in patients with metabolic syndrom. 【Methods】After treatment, the average systolic and diastolic blood pressure of the control group and the intervention group decreased,the average systolic blood pressure and diastolic blood pressure difference were statistically significant in the intervention group before and after treatment(P<0.05);the average systolic blood pressure and diastolic blood pressure in the intervention group were lower than those in the control group after treatment(
P
<0.05).After treatment, 24hSBP, 24hDBP, dDBP, dSBP, nSBP, nDBP of the control group and the observation group were decreased, but the decrease in the control group was not significantly, the difference was not statistically significant (P>0.05); 24hSBP, 24hDBP, dSBP, dDBP, nSBP, nDBP in the observation group after treatment were significantly lower than those before treatment, the 24hSBP and dSBP were significantly lower than those in the control group, the difference was statistically significant (mean P<0.05).After treatment, 24hSBPCV, 24hDBPCV, dSBPCV, dDBPCV, nSBPCV, nDBPCV in the observation group were significantly lower than those before treatment, and 24hSBPCV, dSBPCV, dDBPCV were significantly lower than those in the control group, the difference was statistically significant (mean P<0.05).【Conclusion】Comprehensive geriatric assessment is helpful to control the level of blood pressure and blood pressure variability and adopt proper intervention in elderly patients with metabolic syndrome.

Comparative Analysis of the Effect of Different Retrograde Island Flap in the Repair of Thumb Trauma

【Objective】To explore the clinical effect of thenar retrograde island flap and radial dorsal thumb retrograde flap to repair thumb wound patients and to provide reference for clinical promotion. 【Methods】A total of 88 thumb injury patients from February 2013 to June 2016 in our hospital were selected as the research object. According to surgical repair of injured thumb position, patients were divided into 2 groups: the thenar group and the thumb radial dorsal group. Operation time, intraoperative blood loss, postoperative visual pain score (VAS), postoperative recovery of joint activity and sensory, measurement of blood flow to the thumb and patients' satisfaction evaluation were recorded and evaluated in the two groups.【Results】The operative time, intraoperative blood loss and postoperative VAS score of the thenar group were significantly lower than those of the thumb radial dorsal group (
P
<0.05). Patients in the thenar group showed more satisfaction with dorsal part, and patients in the thumb radial dorsal group were more satisfied with ventral thumb and fingertip. The postoperative joint activity and sensory measurement scores of the thumb radial dorsal group were significantly higher than those of the thenar group. The recovery time after surgery was shorter and the blood flow to thumb in the thenar group was greater than those of the thumb radial dorsal group. <0.05) . 【Conclusion】The radial dorsal thumb retrograde flap repair is suitable for repairing the sensory function of the ventral part of thumb, and thenar retrograde island flap is suitable for repairing the dorsal part of thumb, which has less sensory function.

Application of Opioid Analgesic Drug Docetaxel in Clinical Anesthesia

【Objective】 To observe the clinical effect of docetaxel, an opioid analgesic, on clinical anesthesia. 【Methods】Ninety patients were randomly divided into the control group (n=45) and the observation group (n=45). The control group was treated with pentazocine anesthesia, while the observation group was treated with the opioid analgesic drug docetaxel. The anesthesia effect and safety of the two groups were compared. 【Results】There was no significant difference in the onset time of anesthesia between the two groups (P>0.05). The time of awakening and orientation recovery was shorter in the observation group than that in the control group (P<0.05). The dosage of propofol in the observation group was lower than that in the control group (P<0.05). There was no significant difference in the VAS score between the two groups before surgery (P>0.05). In addition, there was no significant difference 12 hours after surgery. The VAS scores at 1 h, 2 h and 4 h post-operation in the observation group were significantly lower than those in the control group (P<0.05) . The incidence of postoperative complications in the observation group was 8.89%, which was lower than that in the control group, which was 17.78% . The difference was statistically significant (P<0.05). 【Conclusion】The perioperative opioid analgesic effect of docetaxel is ideal and safe. It is worthy of popularization and application.

Observation of Inactivation Effect of Liushen Pill Combined with Paraformaldehyde Devitalizing Agent for Chronic Milk Teeth of Pulpitis

【Objective】To observe inactivation effect of liushen pill combined with paraformaldehyde devitalizing agent for chronic milk teeth of pulpitis. 【Methods】A total of 108 children with chronic milk teeth of pulpitis treated in dental clinic of our hospital from June 2013 to June 2015 were selected as the research objects, they were divided by random number table into the observation group (n=58) and the control group (n=50). Liushen pill and paraformaldehyde devitalizing agent were used to carry on pulp devitalization in the observation group, paraformaldehyde devitalizing agent was used in the control group. Pain situation after the medicine, excellent rate of inactivation and complication rate in the two groups were compared. 【Results】94.8% (55/58) patients had no pain or mild pain after the medicine in the observation group, which was significantly higher than the control group [72.0% (36/50)](P<0.05). Excellent rate of inactivation in the observation group was 93.0% (54/58), which was significantly higher than the control group [70% (35/50)](
P
<0.05). No complications occurred in the observation group, complication rate in the control group was 8%(4/50), there was a significant difference in the two groups (
P
<0.05). 【Conclusion】 Application of liushen pill and paraformaldehyde devitalizing agent in deciduous molar inactivation can strengthen inactivation effect and pain inhibitory effect effectively with high security, has a high reference and application value.

Effect of Double-loop Alcohol on Inflammatory Factors, Gastrointestinal Function and Liver Function in Rats with Liver Cirrhosis

【Objective】To study the effect of double loop alcohol on inflammatory factors, gastrointestinal function and liver function in rats with liver cirrhosis. 【Methods】Sixty male SD rats were selected and randomly divided into control group, model group and bicyclol low dose group, middle dose group, high dose group. The liver cirrhosis model was established by subcutaneous injection of carbon tetrachloride. Bicyclol low dose, middle dose and high dose groups were given 0.1g/kg and 0.2g/kg, 0.3 g/kg of bicyclol orally. At 8 weeks after intervention, serum was collected and serum levels of inflammatory factors (IL-1, IL-8, TNF-α), gastrointestinal mucosal barrier function indexes (PCT, LPS, D-lactate) and liver function indexes (ALT, AST) were determined. 【Results】 At 8 weeks after the intervention, serum IL-1, IL-8, TNF-α, ALT, AST, PCT, LPS, D-lactate contents of rats in the model group were significantly higher than those in the control group (t=13.729~23.179, P<0.05). Serum IL-1, IL-8, TNF-α, ALT, AST, PCT, LPS, D-lactate contents of rats in bicyclol low dose, middle dose and high dose groups were significantly lower than those of model group (t=6.324~20.938,P<0.05). As bicyclol dose increased, serum IL-1, IL-8, TNF-α, ALT, AST, PCT, LPS, D-lactate contents decreased (t=2.224~7.456,P<0.05). 【Conclusion】Double-loop alcohol has protective effect on the gastrointestinal function and liver function of liver cirrhosis model rats, and can inhibit the synthesis of inflammatory factors, which is an ideal drug for the treatment of liver cirrhosis.

Effect of Safflower Polysaccharides on Tumor Size and Cell Growth Inhibition in Rats Liver Cancer

【Objective】To explore the inhibitory effect of safflower polysaccharides (SPS) on the proliferation of rat liver cancer cells. 【Methods】Fifty male SD rats were randomly divided into the normal control group, the positive control group and the SPS high (135mg/kg), middle (45mg/kg) and low-dose (15mg/kg) groups, with 10 rats in each group. Rats in the normal control group were treated with gavage of normal saline, while the remaining groups were treated with gavage of 10mg/kg diethylnitrosamine (DEN) to liver establish gastric cancer models. The SPS subgroups rats were treated with gavage of different doses of SPS. After 16 weeks of intervention, all rats were sacrificed. Rat weight, tumor nodules and tumor volume were recorded. The effect of different doses of SPS (0mg/mL, 0.32mg/mL, 0.64mg/mL, 1.28mg/mL) on the growth inhibition rate (IR) of rat liver cancer CBRH-7919 cells was detected by MTT colorimetric assay. 【Results】 The rat weight in normal control group was significantly higher than those of other groups (
P
<0.05). Body weight of SPS subgroups was significantly higher than that of the positive control group, while the number of tumor nodules and tumor volume were significantly lower (
P
<0.05). The body weight of high dose group was significantly higher than that of middle and low dose groups (

<0.05), while the number of tumor nodules and tumor volume were significantly lower (

< 0.05). MTT assay showed that the inhibition rates of rat liver cancer CBRH-7919 cells at 48h and 72h after the SPS treatment were significantly higher than that of the same dose group at 24h treatment (

Clinical Detection Value of Biochemical and Immune Parameters in Patients with HIV and Hepatitis B Virus Superinfection

There were no significant differences in the Hb, PLT and RBC levels between the HIV group and C group (P>0.05). 【Conclusion】A HIV / HBV superinfection may cause the body's immune function to decrease further and their condition to exacerbate, but it may actually reduce liver inflammatory injury in patients with HBV. It has no significant effect on HIV-induced anemia.

Clinical research on Injectable Compound Venom Recipe Using in Postoperative Chemotherapy of Gastric Carcinoma Patients

【Objective】To investigate clinical effect of the injectable compound venom recipe on the immune function of gastric carcinoma patients underwent postoperative chemotherapy. 【Methods】The patients (84 cases) with gastric cancer were randomly divided into control group and observation group according to the age, gender, tumor location and pathological stage of patients. The control group were treated by 12 course of chemotherapy program FOLFOX4; the patients in observation group were additionally implemented the treatment with the injectable compound venom recipe during FOLFOX4 chemotherapy therapy. It's compared that the change of immune indicators before and after chemotherapy and the improvement of patients' life quality, the incidence of adverse reaction during the process of chemotherapy . 【Results】 In the immune indicator detection experiments before and after chemotherapy, we observed that the patients in observation group possessed significantly higher levels of peripheral blood immunoglobulin (IgG, IgA, IgM) and complement components (C3 and C4) than those of the control group (P<0.05), and the number and proportion of CD4
+
T cells in peripheral blood of patients in the observation group were significantly higher than that of the control group (P<0.05). KPS score of quality of life after chemotherapy proved that the scores of observation group patients was significantly higher than that of the control patients, while the effectiveness of chemotherapy in observation group was also significantly higher than that in the control group (P<0.05). Comparing with control group, the incidence of adverse reaction as diarrhea, infection, thrombocytopenia, leukopenia, lower hemoglobin, the decrease of glutamic-oxalacetic transaminase and glutamic-pyruvic transaminase were significantly lower than those in the control group (P<0.05); Whereas, the incidence of adverse reaction as anorexia, hair loss, nausea, vomiting, arrhythmia, creatinine elevation, urea nitrogen increase, total bilirubin increase and pain, had no significant difference between the control and the observation group (P>0.05). 【Conclusion】The useage of injectable compound venom recipe can significantly improve immune function and life quality during postoperative chemotherapy for patients with gastric cancer, and reduce the side reaction of chemotherapy, which is worthy of clinical application.

Clinical Analysis of Pedicle Screw by Minimally Invasive Techniques to Treat Unstable Thoracolumbar Fractures

【Objective】To investigate the clinical efficacy of the minimally invasive pedicle screw fixation for the treatment of unstable thoracolumbar fractures. 【Methods】Thirty-seven patients of unstable thoracolumbar fracture without neurological symptoms from January 2011 to October 2012 were seen in the Departments of Orthopedics of the First and Second Hospitals of Xi'an Jiaotong University. There were 24 males and 13 females, aged 19 to 69 years (mean 31.8 ± 8.5 years). All cases were treated with minimally invasive percutaneous pedicle screw fixation techniques. The operation time, blood loss, vertebral height before and after surgery, Cobb's angle changes, and postoperative complications were collected and statistical analysis was conducted. 【Results】The operation time was 1.1 (0.75 ~ 1.5) h, the bleeding volume was 20~100 (40.5±10) mL, and the follow-up time was 10~36 (19.8±5.6) months. There were significant differences in VAS scores four weeks and one year after operation compared to that on one day before operation (P<0.05), showing a significant reduction in pain. The percentage of vertebral body height and Cobb's angle were significantly higher than those before treatment, and the difference was statistically significant (P<0.05). The percentage of vertebral height and Cobb's angle before treatment and after treatment were significantly different (P<0.05). However, there were no statistical difference between these values one month and twelve months after surgery (P>0.05). Intraoperative complications accounted for three cases (8.1%), caused by improper pedicle screw placement resulting in piercing the outer wall, while there were no rupture of the dura mater and vascular injury patients. However, follow-up showed fracture healed without much deformity and height loss. The twelve-month follow-up had two patients with a total of three loose pedicle screws. There were two cases (5.4%) of ruptured pedicle screw; one case was seen six months after operation and another case was seen twelve months after operation. Only one case (2.7%) showed pedicle screw withdrawal twenty-four months after operation.【Conclusion】Minimally invasive percutaneous pedicle screw technology can minimize the damage on stability of the disease, not only to achieve the purpose of stabilizing the spine, but also to reduce intraoperative trauma. Under the premise of strict control of surgical indications, it can be widely used as a basic technology.

Influence of Different Dialysis Membrane on the Prognosis of Maintenance Hemodialysis Patients

【Objective】To observe the effect of different dialysis membrane on the prognosis of patients with maintenance hemodialysis (MHD). 【Methods】One hundred and five patients with MHD were divided into three groups according to different dialysis membrane. There were 35 patients with maintenance hemodialysis in each group. Group A was dialyzed with B-16H hollow fiber hemodialyzer, group B was dialyzed with F15 hollow fiber hemodialyzer and group C was treated with CA-HP-150 hemodialyzer. Depression levels were evaluated by self-rating depression scale (SDS) and self-rating anxiety scale (SAS). Meanwhile blood C reaction protein (CRP), interleukin-6 (IL-6), advanced glycosylation end products (AGEs), the levels of three groups of parathyroid hormone (PTH), phosphorus (P), calcium (Ca), serum albumin (ALb), and hemoglobin level (Hb) were measured. 【Results】There were no significant differences in terms of SAS and SDS scores among three groups before treatment (P>0.05). After different dialysis, the SAS and SDS scores in group C were lower than those in group A and B (P<0.05). The levels of CRP, IL-6,AGEs,PTH, and P in group C were significantly lower than those in group A and B (P<0.05). The Ca,Alb, and Hb levels of group C were significantly higher than those of group A and B (P<0.05). 【Conclusion】CA-Hp-150 hemodialyzer is an ideal therapy for maintenance hemodialysis patients. It can improve the patients' mineral metabolism disorders and alleviate the depressive symptoms. It is worthy of popularization and application.

Influencing Factors of Poorly Expected Effect of Calcium Supplementary Treatment in Patients with Maintenance Hemodialysis-induced Osteoporosis

【Objective】To investigate the influencing factors of poor effect of calcium supplementary treatment in patients with maintenance hemodialysis (MHD)-induced osteoporosis. 【Methods】A total of 136 patients with MHD-induced osteoporosis were analyzed retrospectively. According to the effect of calcium supplementary treatment, they were divided into the excellent group (n=84) and the poor group (n=52). General information was compared between the two groups. Analysis was performed to analyze the related factors influencing poor effect of calcium supplementary treatment in patients with MHD-induced osteoporosis. 【Results】 There were no significant differences in BMI levels, hyperlipidemia and hypertension status, and bone mineral density T value before calcium supplement between the two groups (P>0.05). The proportions of age, gender (female), diabetes rate, MHD time, and unhealthy living habits (smoking, drinking, staying up overnight) during the treatment in the excellent group were significantly lower than those in the poor group, while daily walking distance and time was significantly more than that of the poor group (P<0.05). The multivariate logistic regression analysis showed that age, gender, diabetes, MHD time, daily walking habits, smoking, drinking, and staying up late were the related factors influencing the effect of calcium supplementary treatment in patients with MHD-induced osteoporosis (P<0.05). 【Conclusion】Age, being female, diabetes, MHD time, smoking, drinking and staying up late during the treatment are the independent risk factors influencing the effect of calcium supplementary treatment in patients with MHD-induced osteoporosis, while daily exercise is the protective factor.

Effects of Oxycodone Injection as Preemptive Analgesia on Postoperative Analgesia in Breast Cancer Patients after Modified Radical Mastectomy

【Objective】To explore the postoperative analgesia effect of oxycodone injection as preemptive analgesia in patients who underwent patient-controlled intravenous analgesia (PCIA) after modified radical mastectomy.【Methods】Ninety patients with breast cancer (ASA Ⅰ~Ⅱ) who were scheduled for modified radical mastectomy and required PCIA were randomly divided into 3 groups, A、B and C, with 30 cases in each group. Patients in A group received the preemptive analgesia with oxycodone (0.1mg/kg) 10 minutes before anesthesia induction, while patients in B and C groups received intravenous injection of saline (10mL) . After surgery, all patient received PCIA with different concentrations of sufentanil,[A and B group received sufentanil 1.0 μg/(kg·100 mL) while C group received sufentanil 2.0 μg/(kg·100 mL)]. PCIA settings were: background infusion dose at 2 mL/h, patient-controlled dose of 3mL, and lockout interval for 15 minutes. Other drugs were the same in each group. Each patient's awakening and extubation time, the VAS at 2h, 12h, 24h, and 48h postoperation and adverse events (nausea, vomiting and dizziness) were observed and recorded. 【Results】There were no statistical significance in awakening and extubation times among the three groups(P>0.05).Compared with the patients in B group, the patients' VAS in A and C groups were significantly lower(P<0.05). However, there was no statistically significant difference between the A and C groups(P>0.05).Compared with the patients in the C group, the numbers of adverse events in A and B groups were significantly lower(P<0.05), but there was no statistically significant difference between A and B groups(P>0.05).【Conclusion】Preemptive analgesia with oxycodone injection can be effectively and safely used in patients undergoing modified radical mastectomy. It has no effect on patient's awakening and extubation time and can lower the total dosage of sufentanil given, which reduces the incidence of adverse reactions.

The diagnostic sensitivity, specificity and accuracy were 97.83%, 85.71% and 95.00%, respectively, and the positive and negative predictive values were 95.74% and 90.31% respectively. Clinical Characteristics Analysis of Acute Ischemic Stroke with Early Stage Negative Magnetic Resonance Diffusion-Weighted Imaging

【Objective】To study the clinical characteristics and analyze the possible reasons for acute ischemic stroke with early stage negative magnetic resonance diffusion-weighted imaging (DWI) detection.【Methods】Three cases of acute ischemic stroke with early negative magnetic resonance diffusion-weighted imaging were collected from January 2014 to February in our hospital and the clinical characteristics were retrospectively analyzed.【Results】Among the three cases, the magnetic resonance diffusion-weighted imaging of the first case was negative 7 hours after onset of disease, while the diffusion weighted imaging show positive of dorsolateral medulla oblongata recent infarction 64 hours after onset. The diffusion-weighted imaging of the second case was negative 6 hours after onset of disease, while the diffusion-weighted imaging showed positive of recent infarction on the right side of basal ganglia 52 hours after onset, which was 28 hours after the symptoms aggravated. The diffusion-weighted imaging of the third case was negative 5 hours after onset of disease, while the diffusion weighted imaging showed positive of right dorsal medulla oblongata recent infarction 64 hours after onset.【Conclusion】Magnetic resonance diffusion-weighted imaging is an effective tool in diagnosing acute ischemic stroke in the early stage, which shows positive image within a few minutes after onset. However, we noticed that there are the large number of acute ischemic stroke patients with DWI negative in early stage and that some DWI positive image can show up to 120 hours after onset. Therefore, when DWI is negative while the acute ischemic stroke symptoms are present, we should do the proper treatment and re-examine DWI in order to make early diagnosis and reduce misdiagnosis or missed diagnosis.

Influence of Calcitriol Soft Capsules on Urine Protein of Patients with Diabetic Nephropathy

【Objective】To study the influence of calcitriol soft capsules on urine protein of patients with diabetic nephropathy (DN). 【Methods】From January 2014 to April 2016, 86 patients with DN were chosen as study objects. All patients were divided into 2 groups: the control group and the study group. The 43 patients in the control group were treated with anti-hypertension/ hyperglycemia/ hyperlipidemia, while the 43 patients in the study group were treated with calcitriol soft capsules for 3 months in addition to anti-hypertension/ hyperglycemia/ hyperlipidemia. The effect rate, SCr, uAER, eGFR and UAlb/Cr were compared between two groups. 【Results】After treatment, the uAER, SCr and UAlb/Cr of patients in the two groups decreased significantly (P<0.05), but the levels in the study group showed more dramatic decrease (P<0.05). There was no significant change in eGFR before and after treatment in the two groups (P>0.05). Compared to before treatment, the systolic blood pressure (SBP), fasting blood glucose (FPG), glycosylated hemoglobin (HbA1c) and low density lipoprotein cholesterol (LDL-C) in two groups were significantly decreased (P<0.05), while 1,25(OH)

Evaluation of Bone Mineral Density (BMD) and Bone Strength of Femoral Bone in Patients with Type 1 Diabetes Mellitus under Quantitative CT

Clinical Significance of Serum CRP, Trypsin -2 and PAF in Early Diagnosis of Acute Pancreatitis after ERCP

【Objective】To discuss clinical significance of serum CRP, trypsin -2 and PAF in early diagnosis of acute pancreatitis after ERCP. 【Methods】 A total of 108 cases underwent ERCP in the department of digestive medicine from Jan 2014 to Dec 2016 was selected as subjects. After ERCP, the patients were divided into pancreatitis group, high amylase group and control group according to the occurrence of acute pancreatitis. Amylase, CRP, trypsin -2 and PAF were detected before ERCP , 6h and 24h after ERCP . 【Results】 At 6h after ERCP, serum amylase and CRP levels of the pancreatitis group and the high amylase group increased significantly; at 24h after ERCP, serum amylase and CRP levels of the pancreatitis group continued to rise, and the high amylase group decreased, which were higher than those before ERCP,the differences were statistically significant(P<0.05). At 6h and 24h after ERCP, serum amylase and CRP levels of the pancreatitis group were the highest, the next was the high amylase group, and the control group was the lowest, which showed significant difference in the three groups(P<0.05). At 6h after ERCP, trypsin -2 and PAF levels of the pancreatitis group and the high amylase group increased, but decreased 24h after ERCP, which were higher than those before ERCP(P<0.05). At 6h and 24h after ERCP, serum trypsin-2 and PAF levels of the pancreatitis group were the highest, the next was the high amylase group, and the control group was the lowest, which showed significant difference statistically(P<0.01). 【Conclusion】 CRP, trypsin-2 and PAF levels increased in early stage after ERCP in patients with acute pancreatitis.CRP,trypsin-2 and PAF can be detected through clinical examination to determine whether patients are with acute pancreatitis,so as to prevent and deal with it in time.

Assessment of Optical Quality in Patients Underwent Cataract Surgery through Different Orientations of Clear Corneal Incision by Dual Channel Optical Quality Analysis System

【Objective】 To compare the changes of optical quality in patients underwent cataract surgery with different orientations of clear corneal incision by dual channel optical quality analysis system(OQAS).【Methods】Sixty-four patients with age-related cataract in our hospital from January to May 2016 were divided into two groups according to the location of the transparent corneal incision;the Group A was treated with clear corneal incision located at the temporal side of the eyes, while the Group B was treated with a clear corneal incision at the top of the cornea, which was located around 12 clock.The objective scattering index ( OSI) values and OQAS values ( OV100%、OV20% and OV9% ) of the two groups were compared. 【Results】 (1)The uncorrected visual acuity and corrected visual acuity of all patients were increased gradually from 1d to 1 week after operation, and the visual acuity and corrected visual acuity of the Group A were higher than those of the Group B at the same time point after operation , the difference was significant (P<0.05); (2) The OSI values of the two groups of patients were higher than those before operation at 1d and 1 week after operation, and there was statistical significance in each time point(P<0.05). The OSI value of the Group A was significantly lower than that of the Group B at 1 D and 1 week after operation, with statistical significance (P<0.05);The OV9% values of the two groups of patients at each time point were higher than those before the operation at 1d, 1 week,1 month and 3 months, all of which had statistical significant(P<0.05) .The OV9% values of the Group A at each time point were significantly lower than those in the Group B, the difference was statistically significant (P<0.05). 【Conclusion】 The application of clear corneal incision located at the temporal side of the eye in cataract surgery may acquires less intraocular scattering than that of clear corneal incision at the top of the cornea with a faster vision recovery and better optical quality.Yet,the long-term effect needs to be further studied.

Effect of Dexmedetomidine on Post-Anesthesia Delirium in Children Undergoing Tonsillectomy with Sevoflurane Combined Anesthesia

【Objective】To study the effect of dexmedetomidine on post-anesthesia delirium in children undergoing tonsillectomy with sevoflurane anesthesia. 【Methods】A total of 80 cases of tonsillectomy in children under general anesthesia were randomly divided into an observation group and a control group, with 40 cases in each group. All patients received intravenous injections of fentanyl 0.2 g/kg, propofol 3mg / kg, and cis atracurium 0.2mg/kg for the intubation of the endotracheal tube. Then, all patients inhaled 2%~3.5% sevoflurane to maintain anesthesia during operation. After inserting endotracheal tube, patients in the observation group received dexmedetomidine injection 0.5μg/kg while patients in the control group received the same dosage of saline injections. Post-anesthesia agitation in the two groups during the anesthesia recovery period was compared. 【Results】PAED (Pediatric Anesthesia Emergence Delirium Scale) score of the observation group (8.0±1.5) was significantly lower than that of the control group (11.9±1.7), and the difference was statistically significant (P<0.05). The incidence of agitation and adverse reactions in the observation group was significantly lower than that in the control group (P<0.05). The PACU (Post-Anesthesia Care Unit) retention time of the observation group was significantly lower than that of the control group (P<0.05). The operation time and extubation time of the control group and observation group was not statistically significant (P>0.05). 【Conclusion】Dexmedetomidine can reduce infant restlessness and help children smoothly wake up during the anesthesia recovery period. It also reduces postoperative adverse reactions. So, it is worth being popularized in clinical application.

【Objective】To explore the curative effects of pelvic floor muscle biofeedback rehabilitation exercise schemes in patients with stress urinary incontinence of different types of pressure. 【Methods】 One hundred and forty-six patients with SUI in our hospital from January 2013 to January 2016 were selected and divided into four groups according to different types of SUI (perimenopausal group) and postpartum SUI (postpartum group), 73 cases in each group. Patients were treated with pelvic floor muscle biofeedback rehabilitation exercise for 8 weeks. The two groups were compared in the items of pelvic floor muscle strength of the experimental results before and after treatment, the 1h urine pad overall curative effect, the overall efficacy, ICI -Q -SF (The International Consultation on Incontinence Questionnaire -Short Form) score, PFIQ -7 (Pelvic Floor Impact Questionnaire-Short form7) score and the compliance status.【Results】 After treatment, pelvic floor muscle strength, 1h urine pad test results, overall efficacy, ICI-Q-SF score, PFIQ-7 score of the two groups of patients were significantly improved compared with those before treatment and those of the postpartum group were even better; the compliance in the rational use of drugs, regular review, reasonable diet and other items of the two groups of patients were good, but the compliance of postpartum group was more superior(P<0.05).【Conclusion】 Pelvic floor muscle biofeedback rehabilitation exercise is helpful to the recovery of SUI pelvic floor muscle strength with better compliance. Compared with perimenopausal SUI, postpartum SUI effect is better and worthy of clinical application.

【Objective】 To observe the effects of different anesthesia methods on physiological and biochemical changes in patients undergoing bronchoalveolar lavage (BAL) under mechanical ventilation . 【Methods】A total of 80 cases who underwent BAL under mechanical ventilation in ICU were selected as study subjects and were randomly divided into two groups, 40 cases in each group. The control group were treated with lidocaine surface anesthesia, while the observation group were treated with lidocaine surface anesthesia combined with fentanyl anesthesia. The changes of heart rate, blood pressure, body temperature, blood gas parameters and biochemical indicators were compared between the two groups after operation. 【Results】 There were no significant differences between the two groups at the beginning of anesthesia, after 10min, 20min and 30min of anesthesia and at the end of anesthesia (P>0.05). Risk Factors Analysis of Multiple Segmental Osteoporotic Vertebral Compression Fracture

【Objective】To study the risk factors of multiple segmental osteoporotic vertebral compression fractures (OVCF). 【Methods】Retrospectively analysis the clinical data of 211 cases of the patients with OVCF from August 2010 -August 2016 was conducted. The subjects were divided into single segment group (134 cases) and multiple segment group (77 cases), according to the fresh fracture section. Analysis of Factors Related to the Incidence of Atrial Fibrillation in Healthy Population

【Objective】To investigate the factors related to the incidence of atrial fibrillation in healthy population.【Methods】Clinical data of 11,375 patients who were examined in our hospital from June 2013 to June 2016 were retrospectively analyzed.The patients were divided into atrial fibrillation group (n=52) and non atrial fibrillation group (n=11,323) according to the occurrence of atrial fibrillation. The clinical data of the two groups were compared and analyzed.【Results】A total of 52 patients with atrial fibrillation were enrolled in a total of 11375 healthy subjects,Among them, 31 cases (59.61%) were paroxysmal atrial fibrillation (AF), and persistent atrial fibrillation (AF) was found in 16 patients (30.76%), and permanent atrial fibrillation (AF) was found in 5 patients (9.63%);The age and heart rate of atrial fibrillation group were significantly higher than those of non atrial fibrillation group. The incidence of diabetes mellitus and taking antihypertensive drugs were significantly higher than those of the non atrial fibrillation group; The body mass index (BMI), diastolic pressure (DBP), systolic pressure (SBP) of the atrial fibrillation (AF) group were statistically significant compared with.Those of the non-atrial fibrillation (AF) group (mean P<0.05). High heart rate, high BMI, high DBP, high SBP and stroke history were risk factors for atrial fibrillation in healthy population(mean P<0.05).【Conclusion】High heart rate, high BMI, high DBP, high SBP, and stroke history are risk factors for atrial fibrillation in healthy population. Attention should be payed to strengthen the monitoring of such patients for early detection and early treatment.

Effect of Nano-Carbon Tracer on Radical Resection of Thyroid Cancer

【Objective】To study the effect of nano-carbon tracer on radical operation of thyroid cancer. 【Methods】 A total of 120 patients underwent radical operation of thyroid cancer in our hospital were selected as study subjects and were divided into the nano-carbon group and the routine group, 60 cases in each group. The routine group were treated with traditional radical operation of thyroid cancer and bilateral central lymph node dissection. The nano-carbon group were injected with Carbon Nanoparticles Suspension Injection before operation, after the full development, the same operation was performed. The operation situation and effect of the two groups were compared.【Results】The number of dissected lymph nodes, levels of postoperative serum parathyroid hormone and blood calcium in the nano-carbon group [(8.19±1.77), (23.43±7.52) ng/L, (2.25±0.33) mmol/L] were significantly higher than those in the routine group [(5.38±1.92), (16.56±6.38) ng/L, (1.92±0.24) mmol/L], the difference was significant (

The rate of remove parathyroid gland mistakenly and the incidence of postoperative complications in the nano-carbon group (8.3%, 16.7%) were significantly lower than those in the routine group (21.7%, 36.7%) (P<0.05). 【Conclusion】Nano-carbon tracer has the advantages of rapid onset, high safety and long residence time, which can improve the effect of lymph node dissection, and has good identification and protective effect on parathyroid glands.

P colloid under ultrasound guided. The clinical efficacy of the two groups were compared, and the patients were followed up for at least 1 year after operation. The recurrence of the two groups were recorded.【Results】In the observation group: 158 cases (84.9%) were cured, effective in 23 cases (12.4%), ineffective in 5 cases (2.7%), the effective rate was 97.3% (181/186); in the control group: i139 cases were cured (74.7%), effective in 38 cases (20.4%), invalid in 9 cases (4.9%), the effective rate was 95.1%(177/186).The effective rate of the observation group was significantly higher than that of the control group, and the difference was statistically significant (P=0.014). All patients were followed up for one year. There were no recurrence cases in the two groups.【Conclusion】The therapy of injection of

Observation of the "Y" Reconstructive Method to Treat the Axillary Scar Contracture After Breast Cancer Surgery

【Objective】To observe the effects of the "Y" reconstructive method, "Z" plasty method, and touching wall exercise method on the treatment of the axillary scar contracture after breast cancer surgery. Movement of the shoulder joint after these three kinds of treatment was observed to evaluate the effectiveness of the treatment. 【Methods】Seventy-eight patients in our hospital from March of 2008 to November of 2012 were retrospectively analyzed in this study. The 78 patients were randomly divided into three groups: group A used the "Y" reconstructive method, group B used the "Z" plasty, and group C used the touching-the-wall method. Patients in both group A and B were given the external fixation support after the, drugs to prevent and control scar hyperplasia, and started exercise in the early stages. The patients in Group C only did the touching-the-wall exercise. Three months later, we began to observe and analyze the outcomes. The curative evaluation standards we incorporated can be divided into four grades: cured with an obvious effect, better effect, some effect, and no effects. 【Results】Among the 28 patients of Group A, 18 of them were cured with obvious effect, 8 had better effects and 2 of them had some effect-the cure rate was 64.29%. Among the 27 patients of Group B, 12 were cured, 8 had better effects effect and 7 of them had some effect-the overall cure rate was 44.44%. Among the 23 patients of Group C, 5 were cured, 9 had better effects, and 9 had some effect. The cure rate was 21.74%. The statistical analysis showed that the cure rate of group A was significantly higher than group B and group C(P<0.05). Though the cure rate was also higher in group B than in group C, the difference was not statistically significant (P>0.05).【Conclusion】The "Y" reconstructive method is superior to both the "Z" plasty and touching-the-wall exercise in treating axillary scar contractures after breast cancer surgery. The "Y" reconstructive method can optimally improve shoulder joint movement which was formerly inhibited by axillary scar contractures.

Value of Health Education as the Club Management Mode in the Treatment of Diabetic Patients

【Objective】To explore the effects of the club management model on diabetic health education and patients' self-management ability. 【Methods】A total of 97 patients with diabetes from April 2013 to September 2015 at our hospital who participated in diabetic health education club activities were collected. Patients in the diabetic club completed 6 months of club activities (learning, training, studying and practicing activities related to diabetes). Knowledge level exams, glycemic control, self-management skills and life quality scores before and after the club management in all patients were analyzed. 【Results】After health education in the club, the passing rate on the diabetes knowledge questionnaire was significantly increased (P<0.05). Levels of fasting blood glucose (FPG), postprandial 2h blood glucose (2hPG) and glycated hemoglobin (HbAlc) were significantly lower (P<0.05). Body mass index (BMI) and blood lipid levels were improved significantly (P<0.05). Patients' self-management skills and quality of life scores had improved significantly as well (P<0.05). 【Conclusion】The results show that club management in diabetic health education helps to improve diabetic patients' self-management skills and quality of life, which help to control the disease and reduce complications.

Effect of Autonomic nervous Function on Retinal Vascular Diameter and Carotid Intima-media Thickness (CIMT) in Patients with Hypertension

【Objective】To investigate the effect of autonomic nervous function on retinal vascular diameter and carotid intima-media thickness (CIMT) in patients with hypertension.【Methods】One hundred and twenty-eight patients with hypertension admitted to our hospital from September 2015 to September 2016 were selected as the subjects; 24h dynamic electrocardiogram was monitored by heart rate variability (HRV); those with the normal R-R interval standard deviation (SDNN) >100 ms were divided to normal group (n=54), and those with SDNN <100 ms were divided to abnormal group(n=74); the blood pressure, retinal vessel diameter and CIMT were compared between the two groups.【Results】 Low frequency (LF), high frequency (HF), SDNN heart rate variability (HRV) index of the abnormal group were significantly lower than the normal group,the blood pressure variability (BPV) was significantly higher than that of the normal group, the difference was statistically significant (P<0.05); the central retinal artery diameter (CRAE), the central retinal vein diameter (CRVE) and the fractal dimension (Df) of the abnormal group were significantly lower than those of the normal group, the difference was statistically significant (P<0.05); the abnormal group CIMT level was significantly higher than that of the normal group, the difference was statistically significant (P<0.05). By logistic regression analysis, SDNN was an independent risk factor for BPV, CRAE, Df and CIMT
(P<0.05).【Conclusion】 Autonomic nervous function has significant effects on HRV, retinal vessel diameter and CIMT vascular lesion in patients with hypertension.

Influencing Factors of Early Neurological Deterioration in Patients with Cerebral Infarction

【Objective】To explore the influencing factors of early neurological deterioration in patients with cerebral infarction.【Methods】A total of 108 patients with cerebral infarction treated in our hospital were selected from January 2013 to January 2016. According to whether or not the patient was suffering from early neurological deterioration, patients were divided into the deterioration group (n=38) and the non-deterioration group (n=70). The differences of baseline data, past history, clinical signs and laboratory results were observed and compared between the two groups, and by multiple factor analysis, the significant factors that affected the early neurological deterioration were observed.【Results】The proportion of hypertension, diabetes, TIA or stroke, hyperlipidemia, and atrial fibrillation history in the deterioration group was higher than that in the non-deterioration group(P<0.05). Compared to the non-deterioration group, patients in the deterioration group had higher body temperature, higher scores of nerve function defect, lower systolic and diastolic blood pressure(P<0.05), higher level of D-dimer and leukocyte, longer prothrombin time and lower level of C reactive protein(P<0.05). Multivariate logistic regression analysis showed that parameters high score of nerve function defect (OR=1.993), high body temperature (OR=3.012), lower systolic blood pressure (OR=2.942), low diastolic blood pressure (OR=1.838), hypertension (OR=3.522), diabetes mellitus (OR=1.050), transient ischemic attack (TIA) or stroke (OR=1.442), and atrial fibrillation (OR=3.522)] were significant independent risk factors that occurred in the early neurological deterioration(P<0.05).【Conclusion】High score of neural function defect, fever, lower systolic and diastolic pressure, hypertension, diabetes, stroke, TIA and atrial fibrillation history are high-risk factors for early neurological deterioration occurred in patients with cerebral infarction. Therefore, clinical attention should be paid to those influencing factors for prevention.

Application of Combined Detection of Serum Human Epididymis Protein E4, CA125, and CA199 in the Diagnosis of Gynecologic Malignant Tumors

【Objective】To explore the application value of the combined detection of serum human epididymis protein (HE4), carbohydrate antigen 125 (CA125) and CA199 in the diagnosis of gynecologic malignant tumors. 【Methods】Forty-two patients with gynecologic malignant tumors (group A) and 42 patients with gynecologic benign tumors (group B) were selected as study subjects. All cases underwent detection of HE4, CA125 and CA199. The positive rates of the three markers and ROC were compared. 【Results】The expression levels of HE4, CA125 and CA199 individually and in combination in group A were significantly higher than those in group B (P<0.05)。The positive rate, AUC under ROC curve, accuracy, specificity and sensitivity for the detection of the combination of markers in group A were higher than those of CA125 and CA199 alone (P<0.05), but those parameters did not show significant difference compared to those of HE4 alone (P>0.05 ). However, the sensitivity of the combined detection was significantly higher than that of HE4 alone (P<0.05).【Conclusion】The value of HE4 in diagnosing gynecologic malignant tumors is higher than CA125 and CA199 alone. The combined diagnosis of the three markers can further improve the differential diagnosis rate.

【Objective】To analyze the effect of rapid rehabilitation in laparoscopic cholecystectomy and its effect on postoperative recovery. 【Methods】One hundred and eighty cases of laparoscopic cholecystectomy who were treated in our hospital from October, 2012 to May, 2016 were randomly divided into observation group and control group, 90 cases in each group. In the observation group, rapid rehabilitation measures were adopted, and the control group received routine perioperative measures. The operation time, blood loss, postoperative time, postoperative defecation time, postoperative complications and hospital stay were observed. 【Results】The operation time of the observation group was shorter than the control group, bleeding was less than that of the control group, but the difference was not statistically significant (P>0.05); the time of getting out of bed after surgery of the observation group, intestinal exhaust time, defecation time, postoperative hospitalization time were significantly shorter than those of the control group, the difference was statistically significant (P<0.05). postoperative vomiting, abdominal distension, the incidence of hypoglycemia of the observation group was 11.11% (10/90), the incidence of postoperative complications of the control group was 22.22% (20/90), postoperative complications in the observation group was significantly lower than the control group, the difference was statistically significant (χ

Efficacy of Large-dose Atorvastatin on Emergency PCI of Stress Gyperglycemia Patients with Acute Myocardial Infarction

【Objective】 To explore effects of large-dose atorvastatin on short term efficacy of emergency PCI and cardiovascular events on patients with stress hyperglycemia (SHG) acute myocardial infarction (AMI).【Methods】Sixty-eight cases of AMI with SHG were randomly divided into observation group and control group, each with 34 cases. All patients received conventional antiplatelet drugs and emergency PCI treatment. In addition, the observation group was given high dose oral atorvastatin (80mg/d, 40mg/d), while the control group was treated with conventional dose (20mg/d).The myocardial perfusion and cardiac function were compared between the two groups, and the incidence of adverse cardiovascular events in 28d was recorded in the two groups.【Results】There was no significant difference in postoperative CK-MB peak, ST segment resolution good (drop≥50%) rate, myocardial perfusion good rate of the two groups in postoperative myocardial perfusion (P>0.05); postoperative 7d LVEF in the observation group was significantly higher than that in the control group (P<0.05); the incidence of heart failure of the observation group 8.82% was significantly lower than that of the control group (29.41%) in 28d follow-up (P<0.05), there was no significant difference in incidence of severe arrhythmia, stenocardia, recurrent myocardial infarction and death in the two groups (P>0.05). 【Conclusion】 Compared with routine dose, large dose atorvastatin has obvious advantages in increasing LVEF 7d after emergency PCI and reducing incidence of 28d heart failure in AMI patients with SHG.
