1249 Comparison of the Efficacy Between Retrograde and Antegrade Palm Venous Anastomosis and Re-plantation after Fingertip Amputation
Objective To compare clinical efficacy between retrograde and antegrade palm venous anastomosis in the replantation after fingertip amputation so as to provide the basis for choosing best method to recover finger-tip function in patients with fingertip amputation .[Methods] Totally 80 patients with fingertip amputation in our hospital from Oct .2010 to Oct .2013 were analyzed retrospectively .According to the replantation method for fin-gertip amputation ,all patients were divided into observation group and control group .The observation group un-derwent retrograde palm venous anastomosis and replantation ,while the control group underwent antegrade palm venous anastomosis and replantation .All patients were followed up for 1 year .The efficacy after operation was compared between two groups .[Results] The survival rates of fingertips in observation group and control group 1 year after operation were 91 .3% and 72 .3% ,respectively .The number of fingers with good shape in two groups was 89 and 77 fingers ,respectively .The number of fingers with good nail growth in two groups was 89 and 77 fin-gers ,respectively .In the observation group ,the two point discrimination was (5 .1 ± 0 .4)mm ,and the number of fingers with good touch ,normal pain sense and good motor function was 88 ,83 and 77 fingers ,respectively .In the control group ,the two point discrimination was (6 .2 ± 0 .6)mm ,and the number of fingers with good touch , normal pain sense and good motor function was 74 ,66 and 62 fingers ,respectively .There were significant differ-ences between two groups ( P <0 .05) .[Conclusion]Retrograde palm venous anastomosis and reimplantation has advantages such as high survival rate ,good shape and nail similar the normal finger ,good recovery of postopera-tive sense and motor function ,etc .Therefore ,it is an effective surgical procedure .

1252 Effect of Ulinastatin Combined with Xarelto on Coagulation Function of Patients after Hip Replacement

Objective To explore the effect of ulinastatin combined with Xarelto on coagulation function of patients after hip replacement .[Methods] Totally 102 patients undergoing elective hip replacement in our hospital from Feb .2012 to March 2014 were chosen .According to the stratified randomization method ,all patients were divided into treatment group and control group with 51 in each .The control group received oral Xarelto combined with other conventional treatment ,while the treat-ment group received ulinastatin on the basis of the treatment regimen of the control group .The coagulation parameters and clin-ical manifestations were compared between two groups .[Results]There was no significant difference in prothrombin time(PT) , activated partial thromboplastin time(APTT) ,thrombin time(TT) ,fibrinogen(FIB) ,D-dimer(DD) ,C-reactive protein(CRP) and thromboxane B2(TXB2) between two groups before treatment( P>0 .05) .The PT ,APTT and TT in treatment group 2d after treatment were obviously higher than those of control group ,but DD and FIB in treatment group were obviously lower than those in control group ,and there was significant difference( P<0 .05) .The TXB2[(0 .26 ± 0 .10)μg/L] ,CRP[(33 .81 ± 12 .48)mg/L] ,total drainage volume[(349 .86 ± 119 .75)mL] and hospital stay [(7 .26 ± 2 .85)d] in treatment group 5d after operation were lower than those in control group [(0 .31 ± 0 .14)μg/L ,(39 .42 ± 14 .79)mg/L ,(417 .03 ± 134 .72)mL and (9 .35 ± 3 .86)d]( P <0 .05) .The patients with concurrent infection ,large bleeding of incision ,elevated serum creatinine and blood urea in treatment group were obviously less than that of control group .The patients with nausea(6 ,11 .76% ) in treat-ment group was obviously more than that of treatment group (0 ,0 .00% ) ,and there was significant difference( P <0 .05) .[Conclusion] Ulinastatin combined with Xarelto can significantly improve the hypercoagulable state of patients after hip re-placement and reduce the incidence of deep vein thrombosis ,and plays an important role in promoting wound healing and immu-nity recovery .

1255 Assessment of Correlation and Concordance of GEM Premier 3000 Blood Gas Analyzer and BC-5800 Blood Cell Analyzer for Detecting Hemoglobin and Hematocrit Assessed by Bland-Altman Method
Objective To assess the correlation and concordance of the results of bedside blood gas analyzer (GEM Premier 3000) and blood cell analyzer (BC-5800) for measuring hemoglobin and hematocrit with the results of central la-boratory assessed by Bland-Altman method .[Methods]Hemoglobin and hematocrit of 100 arterial blood samples were measured by Mairui BC-5800 blood analyzer and GEM Premier 3000 analyzer .The results were taken for correlation anal-ysis and Bland-Altman analysis .[Results] The correlation coefficient(r) of hemoglobin measured by two methods was 0 . 807( P<0 .01) ,and the regression equation was Y =24 .163+0 .742X .The correlation coefficient (r) of hematocrit measured by two analyzers was 0 .709( P<0 .01) ,and the regression equation was Y=8 .881+0 .635X .There was good correlation of the data between two methods .Bland-Altman analysis showed that 95% confidence intervals of either he-moglobin or hematocrit were not within clinically acceptable range .The concordance of measurements by two analyzers was unsatisfactory .[Conclusion] There is good correlation of hemoglobin and hematocrit between two methods ,but the concordance is unsatisfactory .Bedside immediate measurement can not completely substitute for conventional measure-ment .

1259 Study on Biological Characteristics and Coverage on Clinical Strains of Wide Tropism Pseudomonas Aeruginosa Phages

Objective To explore biological characteristics of wide tropism pseudomonas aeruginosa (PA ) in order to provide the ideas for PA infection control .[Methods]PA phages were separated from sewer of five hospi-tals .After PA phages were purified by double-agar negative colony formation method ,wide tropism phages were screened .The characteristics were observed and one-step growth line was drawn .Nucleic acid was extracted for enzyme digestion analysis .[Results] Totally 8 strains of PA phages were isolated .Three strains of wide tropism phages were screened ,in which PAp-3 showed wider phage spectrum .PAp-3 was transparent ,round and clear boundary plaque .PAp-3 genome was 23kb .When MOI was 0 .001 ,the titer of progeny PA phage produced by the infected host was highest(6 .4 × 1010 PFU/mL) .The incubation period of the infected host bacteria was 30min ,and the outbreak time was 55min ,and the burst size was 100 .The engulfed coverage rate of cross combination of 3 wide tropism phages on 20 strains of the isolated PA phages was 95 .00% (19/20) .[Conclusion] Wide tropism phage has wider spectrum ,shorter incubation period large burst size ,higher engulfed coverage rate of cross combi-nation ,and can provide the basis for the phage used as antibacterial biological agents .

1262 Analysis of Exposure and Clinical Features of 31 Patients with Furious Rabies

Objective To improve the recognition about rabies through analyzing the clinical features of rabies .[Methods] The general data ,clinical features and relevant examination results of 31 patients with rabi-es were analyzed .[Results]Clinical manifestations of patients with rabies in the group was furious .The most typical symptoms could be presented .Hydrophobia was observed in 30 patients(96 .7% ) and aerophobia was observed in 29 patients(93 .5% ) .Among 23 patients with exposure history ,19 patients did not carefully clean their wound after exposure ,and 20 patients did not receive rabies vaccine .All patients did not receive rabies immunoglobulin vaccine .The 31 patients died at 2~8 days after the onset in spite of active therapy .[Conclu-sion]Previous exposure history and clinical symptoms can be used for the prompt diagnosis of rabies .Heath education about the effective measures of the prevention of rabies after exposure should be strengthened .

1265 Changes of Cytokines and B-type Natriuretic Peptide in Serum of Children with Hand,Foot and Mouth Disease at Different Stage

Objective To explore the changes of cytokines tumor necrosis factor (TNF) ,interleukin-6(IL-6) and B-type natriuret-ic peptide BNP in children with different stages of hand ,foot and mouth disease in order to provide the reference for clinical evaluation of the patients .[Methods]Totally 292 patients with different stages of hand ,foot and mouth disease in our hospital from May 2011 to Oct .2013 were chosen .There were 156 children in stage 1 ,and 106 children in stage 2 ,and 20 children in stage 3 and 10 children in stage 4 .Blood was drawn within 24h after admission .TNF ,IL-6 and BNP were determined .The changes of cytokines and BNP in children with different stages of hand ,foot and mouth disease were observed and compared .[Results]Compared with healthy control group ,the levels of TNF ,IL-6 and BNP in different stage were increased obviously .The levels of TNF ,IL-6 and BNP of patients in stage 2 and stage 3 were obviously higher than those in stage 1 ,and those in stage 4 were obviously higher than those in stage 2 and stage 3 ,and there was significant difference ( P<0 .05) .There was no significant difference in the levels of TNF ,IL-6 and BNP be-tween stage 2 and stage 3( P>0 .05) .[Conclusion]The levels of TNF ,IL-6 and BNP increase with the severity of pediatric patients . The levels of TNF ,IL-6 and BNP can be used as the reference indexes for evaluating the condition of patients with hand ,foot and mouth disease .

1267 Value of Adenosine Stress Myocardial Perfusion Imaging in the Diagnosis of Coronary Heart Disease
Objective To evaluate the accuracy and safety of adenosine stress myocardial perfusion imaging in the diagnosis of coronary heart disease .[Methods]Totally 75 patients suspected as coronary heart disease in our hospital from Feb .2011 to Feb .2013 received adenosine 99m Tc-methoxy isobutyl isonitrile(MIBI) myocardial per-fusion imaging .According to the weight of 0 .14 mg/(kg · min) ,adenosine was administrated by infusion pump and venous double pathway .Myocardial perfusion imaging spectrum was used for quantitative analysis .Coronary angiography was performed in all patients after 1~3 days .[Results]Of 75 patients ,32 patients had normal coro-nary angiography ,and 43 patients had more than one stenosis vessel≥50% in which 14 cases were single vascular lesion ,and 16 cases were double vascular lesion ,and 13 cases were three vascular lesion .There were 85 diseased vessels .Involvement was found in 38 left anterior descending arteries(LAD) ,28 left circumflex arteries(LCX) ,21 right coronary arteries(RCA) and 2 left main(LM) .The sensitivity ,specificity ,accuracy ,positive predictive val-ue and negative predictive value of adenosine stress myocardial perfusion imaging in the diagnosis of coronary heart disease were 88 .3% (38/43) ,87 .5% (28/32) ,88 .0% (66/75) ,90 .5% (38/42) and 84 .8% (28/33) ,respective-ly .The sensitivity ,specificity ,accuracy ,positive predictive value and negative predictive value of adenosine stress myocardial perfusion imaging in the diagnosis of coronary artery lesion were 75 .9% (63/83) ,93 .7% (133/142) , 87 .1% (196/225) ,94 .0% (63/67) and 84 .2% (133/142) ,respectively .The sensitivity of adenosine stress myo-cardial perfusion imaging in the diagnosis of single ,double and 3 vascular lesions were 50% (7/14) ,62 .5% (10/16) and 92 .3% (12/13) ,respectively .The sensitivity of adenosine stress myocardial perfusion imaging in the diag-nosis of LAD ,LCX and RCA were 86 .1% (31/36) ,55 .6% (15/27) and 85% (17/20) ,respectively .The total in-cidence rate of adverse reaction of adenosine was 80% (60/75) .[Conclusion]Adenosine stress myocardial perfusion imaging in the diagnosis of coronary heart disease has high sensitivity ,specificity and mild adverse reaction ,so it is a safe and effective method in the assistant diagnosis of coronary heart disease .

1270 Protective Effect of Morphine Postprocessing on Myocardial Ischemia Reperfusion Injury in Rats
Objective To explore the protective effect of morphine postprocessing on myocardial ischemia/reperfu-sion injury(I/R) in rats .[Methods]A total of 40 male healthy SD adult rats were randomly divided into 4 groups with 10 rats in each group .Sham group(S group) only underwent thoracotomy ,and left anterior descending coronary artery was separated ,and blood flow was not blocked for 150min .The I/R group(IR group) underwent the blockage of left anterior descending coronary artery for 30min ,and then the reperfusion for 120min .Morphine postprocessing 1 group(M1 group) received morphine 0 .1mg/kg within 1min immediately after ischemia ,and then the reperfusion for 120min .Morphine postprocessing 2 group(M2 group) received morphine 0 .3mg/kg within 1min immediately after ischemia ,and then the reperfusion for 120min .At the end of the reperfusion ,the infarct size(IS) was measured .The expression of cytochrome C oxidase(CcO) in myocardium was detected by Western blotting .[Results] Compared with IR group ,the IS in M1 group and M2 group were decreased( P <0 .05) .The IS in M2 group was decreased more obviously than that in M1 group ,and there was significant difference( P<0 .05) .The expression of CcO in M1 group and M2 group were increased ( P<0 .05) .[Conclusion]Morphine postprocessing has protective effect on myocardial I/R injury which may be related to the promotion of CcO expression in myocardium .

1273 A Meta-analysis of the Influential Factors of Nonalcoholic Fatty Liver Disease in Chinese People

To explore the influential factors of nonalcoholic fatty liver disease (NAFLD ) among Chinese people in order to provide the evidence basis for the prevention and control of NAFLD .[Methods] Key-words such as'NAFLD','risk factors','influential factors'and'case-control'were searched from in CNKI ,WanFang , VIP ,CBM ,PubMed and EMbase databases between database establishment and Dec .2012 .The studies about NAFLD among Chinese people were collected .Meta-analysis method was used to obtain a comprehensive and quantitative assessment of the risk factors of NAFLD .[Results]The 21 literatures were enrolled in the study .The total number of cases and controls were 7546 and 27065 ,respectively .The OR values for obesity ,male ,hyper-triglyceridemia ,elevated low-density lipoprotein cholesterol(LDL) ,hypertension ,waist ,hyperglycemia ,elevated diastolic blood pressure(DBP) ,age and elevated alanine transaminase(ALT) were more than 1 ,while the OR val-ues for high-density lipoprotein cholesterol(HDL) and physical exercise were less than 1 .[Conclusion] The major risk factors for NAFLD among Chinese people are obesity ,male ,hypertriglyceridemia ,elevated LDL ,hyperten-sion ,abnormal waist ,hyperglycemia ,elevated DBP ,age and elevated ALT .The protective factors of NAFLD are HDL and physical exercise.

1277 Impact of Tanshinone on Cerebral Perfusion and Hemodynamics of Cerebral Vasospasm after Sub-arachnoid Hemorrhage

Objective To explore the effect of tanshinone on cerebral perfusion and hemodynamics of vaso-spasm(CVS) after subarachnoid hemorrhage(SAH) .[Methods]Totally 200 SAH patients in our hospital were randomly divided into control group( n=100) and observation group( n=100) .The control group received nimo-dipine for the treatment of CVS ,while the observation group received tanshinone for the treatment of CVS on the basis of the treatment of control group .Cerebral perfusion and hemodynamic changes were observed and compared between two groups .[Results]Cerebral blood flow (CBF) and cerebral blood volume(CBV) of the cortex of frontal lobe and temporal lobe in observation group after treatment were markedly higher than those in control group , while the mean transit time(MTT) in observation group was less than that in control group ,and there was signifi-cantdifferencebetweentwogroups(P < 0.05).Erythrocytesedimentationrate(ESR),fibrinogen(Fb),blood viscosity under low shear rate(nbl) and blood viscosity under high shear rate(nbh) in observation group after treat-ment were lower than those in control group ,and there was significant difference( P<0 .05) .[Conclusion] Tan-shinone can effectively enhance the dilation of cerebral vessels ,improve blood viscosity and platelet aggregation , increase cerebral blood flow ,ameliorate cerebral perfusion and reduce the incidence of CVS .

1280 Clinical Efficacy of Nedaplatin Combined with Docetaxel for the Treatment of Advanced Non-small Cell Lung Cance r

Objective To observe and compare the short-term efficacy and adverse reaction of nedaplatin and docetaxel combination chemotherapy regimen vs .cisplatin and docetaxel combination chemotherapy regimen for the treatment of patients with advanced non-small cell lung cancer(NSCLC) .[Methods] According to chemotherapeutics ,70 patients with NSCLC in our hospital were divided into nedaplatin combined with docetaxel group ( n=36) and cisplatin combined with docetaxel group ( n=34) .Nedaplatin group was given nedaplatin 25 mg/(m2 · d) at d1 ~d3 and docetaxel 75mg/m2 at d1 .Cisplatin group was given cisplatin 25 mg/(m2 · d) at d1 ~d3 .The course was 21 days .After 2 cycles of chemotherapy ,the efficacy and adverse reaction were evaluated by WHO standard .[Results] In nedaplatin group ,1 patient was complete remission(CR) ,and 14 pa-tients were partial remission(PR) ,and 18 patients were stable(SD) ,and 3 patients were progression(PD) ,and the effective rate(RR) was 41 .7% (15/36) .In cisplatin group ,there were CR in 1 patient ,PR in 13 patients ,SD in 18 patients and PD in 2 patients ,and the RR was 41 .2% (14 /34) .There was no significant difference in effective rate between two groups ( P >0 . 05) .The rate of gastrointestinal tract reaction in nedaplatin group and cisplatin group was 5 .6% (2/36) and 29 .4% (10/34) re-spectively ,and there was significant difference between two groups( P<0 .01) .The rate of renal toxicity in nedaplatin group and cisplatin group was 0% (0/36) and 5 .9% (2/34) respectively ( P <0 .01) .The rate of leucopenia in nedaplatin group and cisplatin group was 72 .2% (26/36) and 70 .6% (24/34) ,but there was no significant difference between two groups ( P >0 .05) .[Conclusion] The effective rate of nedaplatin combined with docetaxel regimen for the treatment of NSCLC is not lower than that of cisplatin combined with docetaxel regimen .The gastrointestinal toxicity of nedaplatin combined with docetaxel reg-imen is less than that of cisplatin combined with docetaxel regimen .The main adverse reaction is bone marrow suppression . There is no difference in leucopenia between two regimens .The decreasing of platelet in nedaplatin combined with docetaxel regimen is more obvious than that in cisplatin combined with docetaxel regimen .

1283 Expression and Significance of MMP2 mRNA in Human Gliomas
Objective To explore the expression and significance of matrix metalloproteinase-2(MMP2) mRNA in human gliomas .[Methods]The expression of MMP2 mRNA in 38 human glioma tissues and 7 nor-mal human brain tissues was detected by RT-PCR .[Results] The expression of MMP2 mRNA was not detec-ted in 7 normal human brain tissues .Among 38 cases of gliomas ,the expression of MMP2 mRNA in 10 pa-tients of 18 patients with low-grade malignant gliomas was detected ,and the positive rate was 55 .6% (10/18) . The expression of MMP2 mRNA in 17 patients of 20 patients with high-grade malignant gliomas was detected , and the positive rate was 85% (17/20) .The positive rate of MMP2 mRNA in high-grade malignant glioma (grade Ⅲ ~ Ⅳ) was markedly higher than that in low-grade malignant gliomas (grade I-Ⅱ ) ,and there was significant difference between two groups( P<0 .05) .[Conclusion The expression of MMP2 mRNA in malig-nant gliomas is high and positively related with the malignancy degree of gliomas .

1286 Observation on the Efficacy of Sandwich Technique in Treatment of Deep Dental Wedge-shaped De-f ec ts

Objective To explore clinical efficacy of sandwich technique in the treatment of deep dental wedge-shaped defects .[Methods]Totally 86 patients(302 teeth) with deep dental wedge-shaped defects were chosen .Ac-cording to patients'will ,all patients were divided into two groups .The observation group( n=43 ,152 teeth) was treated with sandwich technique ,while the control group( n=43 ,150 teeth) were treated with glass ionomer ce-ment .The successful rate was compared between two groups .[Results]The successful rate of observation group after 6 months was 97 .36% ,which was obviously higher than that in control group (88 .67% )( P <0 .05) .The successful rate of observation group after 1 year was 90 .79% ,which was obviously higher than that in control group(79 .33% )( P <0 .05) .[Conclusion] Sandwich technique is easy to operate ,and has certain efficacy in the reparation of deep wedge-shaped defect ,and can prevent the secondary caries and restore the shape and beauty of the diseased teeth .Therefore ,it is worthy of clinical promotion .

1288 Effect of Psychological Support on Postoperative Illness Uncertainty in Patients Undergoing Radical Prostatectomy

Objective To explore the effect of psychological support on illness uncertainty of patients un-dergoing radical prostatectomy .[Methods] According to patients'will ,72 patients undergoing an radical pros-tatectomy were divided into psychological support group (intervention group ,n = 40) and non-psychological support group(control group ,n=32) .General material questionnaire and Chinese version of Mishel uncertain-ty illness scale(MUIS-A) were used for the survey .The survey time lasted at least 3 months after the opera-tion .[Results] The average score in intervention group was 45 .65 ,which was markedly lower than that in control group(80 .91) .Two-way ANOVA showed that psychological support had a significant effect on Mishel illness uncertainty score .[Conclusion] Psychological support is an effective nursing intervention for reducing postoperative uncertainty of patients .Therefore ,it is worthy of clinical promotion .

1291 Study of the Impact of Subclinical Hypothyroidism on Chronic Complications of Type 2 Diabetes Mellitus
Objective To explore the correlation between subclinical hypothyroidism and chronic complications of type 2 di-abetes mellitus(T2DM ) .[Methods] Totally T2DM patients with subclinical hypothyroidism (hypothyroidism group) were cho-sen .Other 50 T2DM patients without subclinical hypothyroidism (control group) were chosen randomly .Biochemical indexes and complications were compared between two groups .[Results] Age ,sex ratio ,duration of diabetes mellitus and the inci-dence of coronary heart disease in hypothyroidism group were obviously higher than those in control group .The levels of total cholesterol(TC) ,triglyeride(TG) ,low density lipoprotein cholesterol(LDL-C) 24h-urinary albumin excretion rate(UAER) , high sensitive C-reactive protein(hs-CRP) ,thyroid stimulating hormone(TSH) ,thyroid peroxidase antibody(TPO-Ab) and thyroglobulin antibody(TG-Ab) in hypothyroidism group were obviously higher than those in control group ,but ankle/brachial index(ABI) and the ratio of E peak(cardiac rapid filling period) to A peak(cardiac slow filling period) in hypothyroidism group were lower than those in control group( P<0 .05) .There was no significant difference in the incidence of diabetic retinopathy (DR) between two group .The incidence rates of diabetic nephropathy (DN) ,diabetic polyneuropathy(DPN) and diabetic pe-riphery artery disease(PAD) in hypothyroidism group were significantly higher than those in control group [Conclusion] Pa-tients with T2Dm is easy to have subclinical hypothyroidism .Subclinical hypothyroidism may affect the occurrence of chronic complications of diabetes mellitus .

1294 Analysis of the Changes of Arterial Stiffness in Elderly Patients with Diabetes Mellitus Merged Metabolic Syndrome
To explore the changes and influential factors of arterial stiffness in elderly patients with diabetesmellitus (DM) mergedmet abolicsyndrome(MS).[Methods]Totally 246 elderly patients with DM in our sanatorium were chosen .According to the merging or not ,all patients were divided into merging MS group( n=120) and no merging MS group( n=126) .Other 62 healthy old people were chosen as the control group .Pulse wave conduction velocity (PWV) ,blood pressure ,blood lipid ,fasting blood glucose(FBG) ,high-sensitivity C-re-active protein(hs-CRP) ,fasting insulin(FINS) ,insulin resistance index (HOMA IR) ,waist circumference and body mass index(BMI) of all patients were measured .[Results] Neck-femoral artery PWV in DM patients was significantly higher than that in control group(P<0.05).Multiple-factor regression analysis showed that PWV was positively correlated with FBG ,hs-CRP ,FINS ,blood pressure ,blood lipid ,patients'age and HOMA-IR .[Conclusion]Senile DM can significantly increase arterial stiffness .Arterial stiffness has a rising trend due to the merging MS.

1296 Study on the Efficacy of Abstergent Autologous Blood Transfusion Combined with Fresh Frozen Plasma in Preventing Coagulation Disorders after Operation
Objective To explore the efficacy of abstergent autologous blood transfusion combined with fresh frozen plasma in preventing coagulation disorders after operation .[Methods] Totally 90 patients with hemorrhea in our hospital from Jan .2012 to Sept .2013 were chosen .According to blood loss volume ,all pa-tients were divided into small volume group(<400 mL) ,middle volume group(400~800mL) and large volume group(>800ml) .On the basis of the quantitative relation between blood transfusion volume and coagulation function of patients ,the concrete scheme about the combined fresh frozen plasma transfusion was made .Fi-brinogen(FIB) ,platelet(PLT) and prothrombin time(PT ) in 3 groups at different time point were analyzed and compared .[Results] Compared with before blood transfusion ,FIB and PLT in 3 groups after blood trans-fusion were decreased significantly and PT was prolonged obviously ( P <0 .05) .Compared with after blood transfusion ,FIB and PLT in 3 groups 18h after blood transfusion were increased obviously and PT was short-ened significantly( P < 0 .05) .[Conclusion]The concrete scheme about the combined fresh frozen plasma transfusion can improve the safety and practicality of abstergent autologous blood transfusion .

1299 Effect and Mechanism of Pioglitazone for the Treatment of Nonalcoholic Fatty Liver Disease in Rats

Objective To explore the effect and mechanism of pioglitazone for the treatment of nonalcoholic fatty liver disease(NAFLD) in rats .[Methods] Thirty six male SD rats were randomly divided into normal control group (group NG) ,pioglitazone treatment group(group PIOG) and high-fat diet group(group FG) .All rats were fed for 12 weeks .Group NG was fed with standard diet ,and the rest two groups were fed with high-fat diet .Group PIOG was giv-en pioglitazone by lavage at 8~12 weeks ,while other 2 groups were given distilled water by lavage in the same period . The levels of fasting serum alanine aminotransferase(ALT) ,aspartate aminotransferase(AST) ,triglycerides(TG) ,total cholesterol(TC) ,fasting glucose(FPG) ,fasting blood insulin(FINS) ,fasting insulin resistance index (FIRI) ,tumor nec-rosis factor-α(TNF-α) and nitric oxide(NO) were compared among 3 groups .HE staining method was used to analyze the pathologic change of hepatic tissue section .The morphological change of Kupffer cells (KCs) was observed .[Results]The levels of FIRI ,TG and TC in group FG were higher than those in group NG ,and there was significant difference ( P<0 .05) .Hepatic tissues showed big bubble fatty degeneration ,inflammatory cell infiltration and dot-like necrosis .KCs in liver tissue had morphological change .The levels of TNF and NO produced by KCs were positively correlated with pathological changes of liver tissue( P<0 .05) .The levels of FIRI ,TG and TC in group PIOG were lower than those in group FG ,and there was significant difference ( P<0 .05) .The degree of liver steatosis was improved ,but the inflam-matory cell infiltration and hepatocyte ballooning degeneration were still visible .The shape and function of liver KCs were still abnormal .[Conclusion]Pioglitazone can partly delay the development of NAFLD induced by high-fat diet .The mech-anism may be associated with the improvement of insulin resistance and the decreasing of blood lipids ,but not related with the regulation of KCs .

1303 Efficacy of Fluvastatin Combined with Valsartan for the Treatment of Patients with Diabetic Ne-phropathy and Its Impact on ET-1 and CGRP

Objective To observe the clinical efficacy of fluvastatin combined with valsartan for the treatment of pa-tients with diabetic nephropathy (DN) and its impact on endothelial-1(ET-1) and calcitonin gene related peptide(CGRP) .[Methods]Totally 70 patients with DN were randomly divided into observation group and control group with 35 cases in each .On the basis of routine treatment ,the observation group received fluvastatin combined with valsartan ,while the control group received valsartan .The 24h quantitative urinary protein ,blood levels of ET-1 and CGRP ,clinical total effi-cacy and the incidence of adverse reaction were compared between two groups after 3 months .[Results] Compared with before treatment ,the 24h quantitative urinary protein and blood ET-1 in two groups after treatment were decreased ,but blood CGRP was increased ,and there was significant difference ( P<0 .05 or P<0 .01) .The improvement of above in-dexes in observation group was better than the control group ( P<0 .05) .The clinical total effective rate of observation group was better than that of the control group (91 .43% vs .74 .29% )( P<0 .05) .There was no significant difference in the incidence of adverse reactions between two groups ( P >0 .05) .[Conclusion]Fluvastatin combined with valsartan for the treatment of DN is safe and effective .The renal protective effect except lowing blood fat and blood pressure may be related to the regulation of ET-1 and CGRP in blood .

1306 Clinical Analysis of Comprehensive Therapy for 70 Patients with Hypopharyngeal Carcinoma
Objective To explore the efficacy and prognostic factors of comprehensive therapy for hypopharyngeal carcinoma .[Methods]Clinical data of 70 patients with hypopharyngeal carcinoma in our hospital from June 2008 to Oct . 2010 were analyzed retrospectively .According to 2002 international union against cancer (UICC) staging standard ,3 pa-tients were stageⅠ,and 10 patients were stageⅡ ,and 22 patients were stage Ⅲ and 35 patients were stage IV .According to different treatment methods ,70 patients were divided into radical intensity modulated radiation therapy plus chemother-apy group(group A ,n=30) and surgery plus postoperative routine radiation group (group B ,n=40) .Radiotherapy was given 2~4 weeks after the complete healing of postoperative wound .[Results] The 1-,3- and 5-year overall survival rates of all 70 patients were 82 .9% (58/70) ,61 .5% (43/70) and 38 .5% (27/70) ,respectively .The local control rates of group A and group B were 66 .7% (20/30) and 70 .0% (28/40) respectively ,but there was no significant difference be-tween two groups( P>0 .05) .The 1-,3-and 5-year overall survival rates of group A and group B were 83 .3% (25/30) ,63 .0% (19/30) ,38 .0% (11/30) ,82 .5% (33/40) ,58 .5% (23/40) and 39 .0% (15/40) respectively ,but there was no significant difference between two groups ( P>0 .05) .[Conclusion]Radical intensity modulated radiation therapy combined with chemotherapy can not only obtain the same efficacy as surgery ,but also keep the laryngeal function of pa-tients and improve the quality of life .

1309 Evaluation of Dysphagia after Radiotherapy for Nasopharyngeal Cancer
Objective To evaluate the swallowing status of nasopharyngeal cancer after radiotherapy .[Methods]The swallowing status of 106 patients with nasopharyngeal cancer 1 year before and after treatment was evaluated regularly .The evaluation methods included patient-reported dysphagia(PRD) ,observer-repor-ted dysphagia(ORD) and objective examination videofluoroscopy (VF) score .According to maximal ORD scores from 3 months to 12 months after radiotherapy ,all patients were divided into 5 groups .PRD and VF scores were compared among 5 groups .[Results] PRD scores except overall curse dimension of dysphagia de-grees in group 1 were obviously lower than those in other groups ,but there was no significant difference in dysphagia degree among group 2 ,3 ,4 and 5 .The positive rate of dysphagia according to PRD score was high-er than the positive rate of dysphagia according to ORD and VF scores .[Conclusion]Dysphagia as a objective symptom ,especially mild dysphagia may be underestimated by clinicians .Dysphagia of nasopharyngeal cancer after radiotherapy is likely to become one of the important tasks for our otolaryngology-head-neck surgeons .

1313 Influence of Different Hemostatic Methods on Ovarian Function in Ovarian Endometriotic Cyst Undergoing Laparoscopic Cystectomy
Objective To explore the influence of different hemostatic methods on ovarian function in o-varian endometriotic cyst undergoing laparoscopic cystectomy .[Methods] Totally 66 patients with ovarian en-dometriotic cyst underwent laparoscopic cystectomy .All patients were divided into microscopic suture group ( n =36) and electrocoagulation group( n=30) .Estradiol(E2 ) ,follicle stimulating hormone(FSH) ,luteini-zing hormone(LH) ,the number of sinusoidal follicles ,normal ovarian function and the declined ovarian re-serve function in two groups were observed before and 6 months after operation .[Results]There was no signif-icant difference in the normal rate of ovarian function and the declining rate of ovarian reserve function before operation bwteem two groups( P>0 .05) .After operation ,the normal rate of ovarian function in suture group was markedly higher than that in electrocoagulation group [83 .33% (30/35) vs 53 .33% (16/30)]( P<0 .05) , but the declining rate of ovarian reserve function was markedly lower than that in electrocoagulation group [16 . 67% (6/36) vs .46 .67% (14/30)]( P<0 .05) .The E2 and number of sinusoidal follicles in suture group were (198 .4 ± 24 .90)pmol/L and (7 .13 ± 2 .25) ,which were markedly higher than those in electrocoagulation group[(153 .40 ± 16 .8)pmo/L and (5 .8 ± 0 .9)]( P<0 .05) .FSH and FSH/LH in suture group were (10 .56 ± 4 .56)U/L and (2 .35 ± 1 .26) ,which were markedly lower than those in electrocoagulation group [(13 .72 ± 2 .81)U/L and (282 ± 0 .88)U/L]( P <0 .05) .[Conclusion] During laparoscopic ovarian conservation opera-tion ,correct microscopic suture hemostasis has smaller ovarian injury and can better reserve ovarian function than routine electrocoagulation .

1316 Clinical Analysis of "Exudative Change" of Ocular Fundus in Infants
Objective To retrospectively analyze "exudative change"of ocular fundus examined by RetCam II .[Methods] The overall fundus photography with RetCam II after mydriasis under superficial anesthesia were per-formed in 12186 infants for screening out fundus "exudative change".Cases were counted for obtaining the per-centage .[Results] Fundus "exudative change"of 489 patients(4 .01% ) were found .The punctiform effusion was the most common .In eye diseases ,there were 202 retinopathy of prematurity (ROP)(41 .31% ) ,12 familial exuda-tive vitreoretinopathy (2 .45% ) ,8 Coats disease(1 .64% ) and 2 Norrie disease(0 .40% ) .In original diseases ,there were 176 cytomegalovirus infection(36 .00% ) ,10 neonatal asphyxia(2 .04% ) ,8 anemia(1 .64% ) ,8 septicemia (1 .64% ) and 63 other diseases(12 .88% ) .[Conclusion]"Exudative change" not only occurs in eye diseases ,but also occurs in children with systemic diseases .Because infants do not cooperate the examination ,RetCam II is the best choice in order to avoid the missing of peripheral retinopathy .

1318 Clinical Observation of Dynamic Sugar Monitoring for 24h-Blood Glucose Drift Change in Pa-tients with Type 2 Diabetes Mellitus

Objective To explore clinical significance of 24h-blood glucose drift change and fluctuation trend in patients with type 2 diabetes mellitus (T2DM ) in order to provide clinical basis for the reasonable treatment scheme .[Methods]Reland dynamic glucose monitoring system(CGMS) was used for continuous 72h dynamic sugar monitoring in 325 T2DM patients .According to blood glucose fluctuation coefficient ,all pa-tients were divided into ≤2 .0 group and >2 .0 group .Glycosylated hemoglobin(HBA1C) ,24h-average blood glucose(24hMBG) ,mean blood glucose before and after 3 meals ,24h-maximum amplitude of glycemic excur-sion(LAGE) ,24h-mean amplitude of glycemic excursion(MAGE) and frequency (NGE) ,blood glucose fluc-tuation coefficient and the percentage of time of different blood glucose levels in 24h(PT ) of all patients were understood .The characteristics of 24h-blood glucose drift change were compared between two groups .[Re-sults]Among 275 patients with blood glucose fluctuation coefficient >2 .0 ,126 patients(45 .8% ) had hypogly-cemia .Among 50 patients with blood glucose fluctuation coefficient ≤2 .0 ,18 patients(36% ) had hypoglyce-mia .HBA1C did not reflect the amplitude and frequency of blood glucose fluctuation .The incidence of hyper-glycemia and the time and amplitude of blood glucose drift ≥11 .1mmol/L in the group with blood glucose fluc-tuation coefficient>2 .0 were obviously higher than those in the group with blood glucose fluctuation coefficient≤2 .0( P <0 .05) .[Conclusion] Compared with the group with blood glucose fluctuation coefficient ≤2 .0 , blood glucose drift in the group with blood glucose fluctuation coefficient >2 .0 is higher ,and drift amplitude is increased .Both groups have hypoglycemia .CGMS contributes to the detailed assessment of the trend and characteristics of blood glucose drift change in patients with T 2DM .

1321 Inhibitory Effect of Endothelial Progenitor Mediated by CGRP on Proliferation of Vascular Smooth Cell Induced by Angiotensin Ⅱ
Objective To explore the role of calcitonin gene-related peptide(CRGP) in endothelial progen-itor cells(EPCs) inhibiting the proliferation of vascular smooth muscle cells (VSMCs) induced by angiotensin II (AngII) .[Methods] The expression of CGRP in EPCs was determined by using ELISA .EPCs condition me-dium was blocked by CGRP antibody .The effect of EPCs condition medium on DNA synthesis ability ,total protein level ,cell survival and cell cycle induced by AngII was observed .[Results]EPCs could express and se-cret high level of CGRP .After CGRP blockage ,the inhibitory effect of EPCs condition medium on AngII-in-duced DNA synthesis ability ,total protein level ,cell survival and cell cycle progression of VSMCs was lower than that in control group( P<0 .05) .[Conclusion]EPCs inhibits the proliferation of VSMCs induced by An-gII through the paracrine secretion of CGRP .

1324 Value of the Detection of Plasma Homocysteine in Early Warning of Type 2 Diabetes Mellitus

Objective To explore the correlation of plasma homocysteine (Hcy) level with the occurrence and devel-opment of type 2 diabetes mellitus(T2DM) .[Methods] Plasma Hcy ,fasting glucose(FPG) ,glycosylated hemoglobin (HbA1c) and urine microalbumin(UMALB) in 42 T2DM patients(T2DM group) and 64 patients with impaired fasting glucose(IFG)(IFG group) were measured and compared with those in 86 healthy subjects(control group) .The correlation of Hcy with FPG ,HbA1c and UMALB was analyzed .[Results] Plasma levels of Hcy ,HbA1c ,FPG and UMALB in T2DM group were higher than those in control group significantly ( P <0 .05) .The average values of FPG ,Hcy and UMALB in IFG group were higher than those in control group significantly ( P<0 .05) .HbA1C was increased slightly , but there was no significant difference ( P>0 .05) .Pearson correlation analysis showed that Hcy was positively correlated with FPG( r =0 .465 ,P<0 .05) ,HbA1c( r =0 .246 ,P<0 .05) and UMALB( r =0 .775 ,P<0 .05) .[Conclusion]Plasma Hcy level is closely related with the occurrence and development of T 2DM .The detection of Hcy in patients with IFG is of important significance for preventing T 2DM .

1326 Microsurgical Treatment of 90 Cases of Petroclival Meningioma
Objective To summarize clinical experiences and microsurgical technique for the excision of petroclival meningiomas .[Methods]Clinical data of 90 patients with petroclival meningioma confirmed by sur-gery in our hospital from Jan .1991 to Dec .2012 was analyzed retrospectively .Of these patients ,35 patients with meningioma extending to supratentorium from mid-superior clivus underwent the surgery through the combined supratentorial-infratentorial approach ,and 50 patients with meningioma extending to infratentorium from mid-inferior clivus underwent the surgery through infratentorial sigmoid sinus approach .Five patients with meningioma extending to petrous apex and cavernous sinus underwent the surgery through subtemporal transpetrosal-transtentorial approach .[Results] Of 93 patients(93 operations) ,3 patients underwent re-oper-ation due to postoperative hemorrhage .Endoscopic total resection of the tumor was achieved in 72 patients (80% ) ,and subtotal resection was achieved in 11 patients(12% ) ,and partial resection was achieved in 8 pa-tients(8% ) .The 70 patients got clinical cure and recovered their normal life ,and 18 patients needed the care in life ,and 2 patients died .[Conclusion]Correct operation approach ,skillful microsurgical technique ,inten-sive postoperative observation and active treatment of complications are helpful to improve total resection rate of petroclival meningioma and decrease mortality and disability rate .

1329 Analysis of the Correlation of Cholecystectomy with Metabolic Syndrome and the Risk Factors

Objective To analyze the correlation between cholecystectomy and metabolic syndrome (MS) in order to provide the basis for the better realization and the decreasing of the damage of MS .[Methods] The health examination data of 4537 subjects attending health examination in our hospital at 2012 were analyzed retrospectively .The correlation between cholecystectomy and the prevalence of MS and the constituents was analyzed .[Results]The prevalence of gallstones ,cholecystectomy and MS in all subjects were 5 .9% ,3 .4%and 32 .7% ,respectively .The prevalence of MS in patients undergoing cholecystectomy (63 .5% ) was signifi-cantly higher than that in patients with gallstones (45 .5% ) and patients without gallbladder disease(30 .7% ) . Further multiple logistic regression analysis showed that cholecystectomy was related with the risk factors of MS(OR :2 .181 ,95% CI:1 .295-3 .674) .[Conclusion] The prevalence of MS in patients with cholecystecto-my is increased significantly ,and cholecystectomy is associated with the risk factors of MS .

1332 Role of Bispectral Index for Guiding Propofol Dosage in Drug Abuse Patients Undergoing Painless Induced A-bortion

Objective To monitor bispectral index (BIS) of drug abuse patients undergoing painless induced abortion in or-der to find proper propofol dosage for drug abuse patients .[Methods]A total of 20 drug abuse patients undergoing painless in-duced abortion in our hospital from 2009 to 2013 were randomly selected as experiment group ,while 20 non-drug abuse patients undergoing painless induced abortion were selected as control group .Human chorionic gonadotropin(HCG) was positive before the operation .Intrauterine pregnancy was confirmed by B-ultrasound .All patients were fast for 4 to 6 hours before the proce-dure .After positioned in lithotomy position ,BIS monitoring apparatus was connected to correct position and propofol TCI pump .The initial target-controlled concentration in plasma was 4μg/mL and regulated by BIS value ,hemodynamics and body movement of patients with 0 .3~0 .5μg/mL per time .When BIS maintained 45 to 60 ,eyeash reflex disappeared and no body movement after forceps clip for uterus was found ,the operation began .The mean arterial pressure(MBP) ,heart rate(HR) , SpO2 and BIS before anesthesia(T0 ) ,when eyeash reflex disappearance/no body movement after forceps clip(T1 ) ,at the be-ginning of curettage after uterine dilation(T2 ) ,after operation(T3 ) and at anesthesia recovery (T4) were recorded .Propofol TCI pump was checked after the operation .Total propofol dosage and maintenance plasma target-controlled concentration were recorded .The number of body movement ,respiratory depression ,operation time and recovery time were also recorded .[Re-sults]Under the anesthesia depth with the same value of BIS ,there was significant significance between two groups ( P <0 .05) .The propofol dosage in experiment group was (179 ± 51)mg ,which was obviously higher than that in control group [(124 ± 27)mg] .The recovery time in experiment group was obviously shorter than that in control group ,and there was sig-nificant difference( P <0 .05) .There was no significant difference in the number of body movement and respiratory depression between two groups( P >0 .05) .[Conclusion] Propofol dosage for drug abuse patients undergoing painless induced abortion with maintenance plasma target-controlled concentration of (6 .8 ± 1 .4)μg/mL is markedly higher than that for non-drug abuse patients ,but the recovery time is obviously shorter than that for non-drug abuse patients .

1335 Clinical Significance of the Detection of Serum Cystatin C,CA125 and Osteoprotegerin in Pa-tients with Coronary Heart Disease

Objective To explore the changes of serum levels of cystain C (CysC) ,CA125 and osteoprote-gerin(OPG) in patients with coronary heart disease(CHD) and their clinical significance .[Methods]A total of 158 CHD patients in our hospital were chosen as experimental group .Other 95 healthy subjects were chosen as healthy control group .Serum levels of CysC ,CA125 and OPG were compared among different groups and dis-eased vessels by using immunoturbidimetry ,chemiluminescence immunoassay and ELISA .[Results] Com-pared with healthy control group ,serum levels of CysC ,CA125 and OPG in CHD group were obviously in-creased( P <0 .05 or P<0 .01) .Serum levels of CysC ,CA125 and OPG in single ,double and multiple vessel disease groups were gradually increased ,while left ventricular ejection fraction (LVEF) was gradually de-creased ,and there was significant difference( P<0 .05 or P<0 .01) .Serum levels of CysC ,CA125 and OPG level in NYHA Ⅰ ,Ⅱ ,Ⅲ and Ⅳ groups were gradually increased ,but LVEF was gradually decreased ,and there was significant difference( P <0 .05 or P <0 .01) .Serum levels of CysC ,CA125 and OPG were posi-tively correlated with the number of coronary disease vessels and cardiac function classification ( P <0 .01) , and negatively correlated with LVEF ( P <0 .01) .[Conclusion] Serum levels of CysC ,CA125 and OPG can not only evaluate the degree of cardiac function impairment ,but also is helpful for early diagnosis of CHD ,as-sessment of the severity of coronary artery disease and prognosis .

1338 Clinical Research of the Protective Effect of Nifedipine on Brain Injury in Patients with Cere-bral Hemorrhage

Objective To explore the protective effect of nifedipine on brain injury in patients with cere-bral hemorrhage .[Methods] A total of 58 patients with cerebral hemorrhage were randomly divided into rou-tine treatment group and nifedipine treatment group .Clinical neurologic impairment score ,Barthel index (BI) score ,hematoma volume and edema area volume of patients before and after treatment were observed .[Re-sults] Total effective rate of nifedipine group was 75% ,which was markedly higher than that of routine treat-ment group(60% ) .Compared with routine treatment group ,the decline value of clinical neurological impair-ment score and BI score were elevated and the dependency level of daily living activity was markedly reduced in nifedipine group after treatment .Hematoma volume in two groups after treatment was obviously decreased , but there was no significant difference in the decline value of hematoma volume between two groups after treat-ment .Edema area of two groups after treatment was also decreased ,but the decline value of edema area vol-ume in nifedipine group was obviously elevated .[Conclusion] Nifedipine can obviously alleviate brain edema , protect nerve cells ,decrease neurologic impairment level and improve quality of life of patients with cerebral hemorrhage .

1341 Comparison of Clinical Efficacy of 3D Patch vs .Plain Film Patch for Repairing Inguinal Hernia
Objective To comparatively analyze the clinical efficacy of 3D patch vs .plain film patch via ab-dominal preperitoneal hernia repair for the treatment of inguinal hernia .[Methods] Totally 113 patients undergo-ing abdominal preperitoneal hernia repair in our hospital from Aug .2011 to Aug .2013 were collected .The obser-vation group( n=56) underwent 3D patch repair without fixation ,while the control group( n = 57) underwent plain film repair with metal nail fixation .The operation time ,pain score ,recurrent case and the incidence of post-operative complications were compared between two groups .[Results] There was no significant difference in oper-ation time and hospital day between the observation group and control group ( P>0 .05) .The pain scores in obser-vation group at d1 and d2 after operation were 1 .5 ± 0 .3 and 0 .4 ± 0 .5 respectively ,and those in control group were 2 .6 ± 0 .7 and 1 .1 ± 0 .4 respectively ,and there was significant difference between two groups ( P <0 .05) . The recurrence rates of observation group and control group were 1 .79% (1/56) and 7 .02% (4/57) respectively , and there was significant difference between two groups( P<0 .05) .The incidence rates of wound infection in ob-servation group and control group were 5 .36% (3/56) and 7 .02% (4/57) ,and there was no significant difference between two groups( P >0 .05) .The incidence rates of scrotal hematoma in observation group and control group were 1 .79% (1/56) and 10 .53% (6/57) respectively ,and there was significant difference( P <0 .05) .[Conclu-sion]Clinical efficacy of 3D patch via abdominal preperitoneal hernia repair for the treatment of inguinal hernia is better than plain film patch .The efficacy is certain .

1343 Expression and Significance of AEG-1 in Large Intestinal Cancer

Objective To explore the expression and significance of AEG-1 in large intestinal cancer .[Methods] A total of 40 patients with large intestinal cancer (observation group) and 40 patients with benign intestinal mucosa(control group) of our hospital in 2008 were selected .Immunohistochemical method was used to detect the expression of AEG-1 .The correlation between AEG-1 and clinicopathological indicators was ana-lyzed .[Results]The expression level of AEG-1 in large intestinal cancer tissue of observation group was signif-icantly higher than that in control group( P <0 .05) .The expression of AEG-1 in large intestinal cancer had no correlation with gender ,age ,pathological differentiation degree and carcinoembryonic antigen (CEA ) in colorectal carcinoma ,but was related to TNM stage and lymph node metastasis .[Conclusion] The overexpres-sion of AEG-1 may be related to the occurrence ,development and metastasis of large intestinal cancer .

1346 Comparison of the Efficacy Between Postauricular Approach Sandwich Method and Underlay Method for Tympanic Membrane Repair

Objective To explore and compare the efficacy between postauricular sandwich method and underlay method for tympanic membrane repair .[Methods] Totally 80 patients(92 ears) requiring tympanic membrane repair were collected and randomly divided into sandwich method group ( n =42 ,46 ears) and un-derlay method group( n =38 ,46 ears) .Postoperative auditory rehabilitation ,pre-and post-operation speech frequency ,average auditory threshold ,tympanic membrane 3 months after operation and the healing rate 6 months after operation were observed .[Results] The postoperative auditory rehabilitation in sandwich method group was better than underlay method group( P<0 .05) .Postoperative speech frequency and average audito-ry threshold in sandwich method group were obviously higher than those in underlay method group ( P <0 .01) .There was significant difference in the incidence of reperforation and otopiesis 3 months after the fol-low-up between two groups( P<0 .05) .There was significant difference in the healing rate 6 months after the operation between two groups ( P < 0 .05) .[Conclusion]The short-term efficacy in postauricular approach sandwich method is the same as that in underlay method ,but it is more beneficial for auditory rehabilitation , so it is an ideal method for the treatment of tympanic membrane perforation .

1349 Observation on the Efficacy of the Modified Intranasal Endoscopic Dacryocystorhinostomy for the Treatment of Acute Attack of Chronic Dacryocystitis
Objective To explore the efficacy of the modified intranasal endoscopic dacryocystorhinostomy for the treatment of acute attack of chronic dacryocystitis .[Methods] A total of 26 patients with acute attack of chronic dacryo-cystitis undergoing the modified intranasal endoscopic dacryocystorhinostomy were collected .The curative rate ,effective rate and complications were observed .[Results]All 26 patients successfully underwent the operation with less intraopera-tive blood loss .Dacryocyst inflammation of patients was controlled about 4d after operation .All patients were followed up for 6 months .The curative rate was 92 .3% (24/26) .One patient with epiphora had the washing liquid reflux between upper and lower lacrimal canaliculi and was evaluated as improvement .The effective rate was 96 .2% (25/26) .One pa-tient with epiphora and secretions had the stoma scar atresia under endoscope and was evaluated as inefficiency .The ineffi-ciency rate was 3 .8% (1/26) .No obvious complication occurred .[Conclusion] Acute phase dacryocystorhinostomy for the treatment of chronic dacryocystitis patients with acute attack is safe and feasible .

1351 Study on the Relationship of Plasma Soluble Myeloid Cell Triggering Receptor-1 and Gene Ex-pression in Patients with Acute Pancreatitis
Objective To explore the relationship of plasma soluble myeloid cell triggering receptor-1 (sTREM-1) and gene expression with the severity of patients with acute pancreatitis .[Methods]A total of 32 patients with mild acute pancreatitis (MAP) ,25 patients with severe acute pancreatitis (SAP) and 10 healthy volunteers in our hospital from Aug .2011 to Aug .2013 were enrolled in the study .Enzyme-linked immu-nosorbent assay(ELISA) was used to detect plasma level of sTREM-1 at 1 ,3 and 7 days after admission .The expression of TREM-1 gene was examined by using reverse transcriptase polymerase chain reaction (RT-PCR) . APACHE Ⅱ and SOFA scores were used for assessing the condition of patients .[Results] Plasma levels of sTREM-1 in MAP group at 1 ,3 and 7 days after admission were (386 .24 ± 190 .79)ng/L ,(237 .22 ± 118 .64) ng /L and (143 .34 ± 92 .23)ng/L respectively ,and those in SAP group were (849 .56 ± 412 .31)ng/L , (674 .61 ± 347 .47)ng /L and (419 .56 ± 241 .57)ng/L respectively ,and there were significant differences be-tween two groups( P <0 .01) .Plasma levels of sTREM-1 were positively correlated with APACHE Ⅱ and SOFA score( r =0 .645 ,P <0 .05;r =0 .713 ,P <0 .05) .[Conclusion] Plasma levels of sTREM-1 is posi-tively correlated with the severity of patients with AP ,but the expression of TREM-1mRNA gene in SAP group is not increased .

1354 Analysis of Risk Factors of the Late symptomatic cerebral vasospasm after the Clipping of Rup-tured Intracranial Aneurysm
Objective To explore the risk factors of the Late symptomatic cerebral vasospasm (LSCVS) after the surgery of ruptured intracranial aneurysm .[Methods]Clinical data of 51 patients undergoing craniot-omy clipping of ruptured anterior circulation intracranial aneurysms in our hospital from Sept .2012 to Oct . 2013 were analyzed retrospectively .The risk factors of LSCVS were analyzed by using non-conditional logistic regression method .[Results] The patients'age ,Hunt-Hess grade ,intraoperative aneurysm rupture ,brain swelling ,chronic anemia and operation time were the possible risk factors .Intraoperative responsibility vaso-spasm was an independent risk factor .[Conclusion] Improving surgical skills and actively reducing the risk factors of cerebral vasospasm during the operation are the important measures to decrease the occurrence of LSCVS .

1357 Total Hip Arthroplasty for the Treatment of Osteoarthritis Secondary to Developmental Dyspla-sia of the Hip
Objective To observe the clinical efficacy of total hip arthroplasty(THA) for the treatment of osteoarthritis secondary to developmental dysplasia of the hip (DDH) .[Methods]A total of 12 patients(12 hips) with osteoarthritis secondary to DDH from Aug .2008 to Dec .2012 included 2 patients with Crowe typeⅠ ,2 patients with Crowe type Ⅱ ,1 patient with Crowe type Ⅲ and 7 patients with Crowe type Ⅳ .Preopera-tive Harris score was 46 .8 ± 15 .2 ,and average limb shortening was 4 .5cm(3~6 .5cm) .All patients under-went total hip arthroplasty of non-femoral shortening osteotomy .Among them ,5 patients(5 hips) underwent femoral head autograft and acetabular reconstruction .[Results]All patients were followed up for the average 28 (12~67) months .One patient with proximal femoral split during the operation had bony healing by steel wire bundling during the operation at 12 months of follow up .No prosthesis loosening was found in all patients at the final follow up .Five patients underwent acetabular autograft and had no absorption of the grafted bone block by reexamination 12 months after operation ,and the grafted bone block was fused with the grafted bone bed .One patient(lengthening for more than 4cm) with numbness in lateral leg after operation recovered after 1 month .No complications such as joint dislocation ,infection ,ectopic ossification and deep venous embolism oc-curred in all patients .Harris score was (85 .48 ± 9 .67) at the final follow up ,and there was significant differ-ence between the final follow up and before operation( t=4 .52 ,P<0 .05) .[Conclusion]For patients with os-teoarthritis secondary to DDH ,precise measurement and evaluation before the operation ,complete soft tissue lysis during operation and correct management of acetabulum and femur can obtain good short-term clinical ef-f icacy .

1361 Effect of All-trans Retinoic Acid on Endothelial Cell Proliferation and Nitric Oxide Production in Human Umbilical Vein
Objective To explore the effect of all-trans retinoic acid(ATRA) on proliferation and nitric oxide (NO) production of endothelial cells .[Methods]Human umbilical vein endothelial cells (ECV-304) in vitro were randomly divided into 5 groups including blank control group(added by ATRA solvent dimethyl sulfoxide ,DMSO) and other 4 groups(added by ATRA 10-9 ,10-8 ,10-7 and 10-6 mol/L ,respectively) .The cells were collected at 24h ,48h and 72h .Flow cytometry and MTT method were used to analyze the effect of different concentration of ATRA on ECV-304 proliferation .The effect of ATRA on NO production of endothelial cells was observed .NO level in cell culture fluid was measured .[Results]Low concentration(10-9 mol/L~10-7 mol/L) of ATRA could promote the proliferation of ECV-304 in a concentration-dependent manner ,and there was significant difference be-tween ATRA groups and control group( P <0 .01) ,and the effect was increased with the prolongation of action time .High concentration(10-6mol/L) of ATRA showed slight inhibition effect on ECV-304 ,but there was no significant difference( P >0 .05) .Low concentration(10-9 mol/L~10-7mol/L) of ATRA could promote the NO production of endothelial cells ,and the effect was increased with the rising of ATRA concentrations ,but there was no significant difference between ATRA 10-6 mol/L group and control group( P > 0 .05) .[Conclusion]ATRA with proper concentration can promote the proliferation and nitric oxide production of endothelial cells .

1364 Value of Holter Monitoring in the Diagnosis of Arrhythmias in Young Infants
Objective To explore the value of Holter monitoring in the diagnosis of arrhythmia in young infants .[Methods] Totally 86 young infants in our hospital from June 2010 to May 2013 were selected as the subjects .Routine electrocardiogram(ECG) and Holter monitoring(DCG) were performed in all patients .[Re-sults] The positive rates of ECG and DCG were 23 .2% and 77 .9% ,respectively .The efficacy of DCG was obviously better than ECG ,and there was significant difference( P <0 .05) .[Conclusion] DCG is of impor-tant clinical value in the diagnosis of arrhythmia in young infants .

1366 Clinical Observation on Sodium Hyaluronate for Preventing 42 Cases of Tendon Adhesion

Objective To observe the clinical efficacy of sodium hyaluronate for preventing tendon adhesion .[Methods]Totally 42 cases of finger tendon rupture(65 stripes) underwent one-stage reparation .During the opera-tion ,the modified Kessler method was used for the suture of tendon .Sodium hyaluronate 2ml was injected into tendon sheath canal .The skin was sutured .The finger activity was observed after operation .[Results] After the follow up for average 5 months ,the efficacy was assessed by using the total active finger activity (TAM) standard . The efficacy was good in 44 stripes ,and the good and excellence rate was 77 .2% .Two fingers with poor efficacy underwent tenolysis after half a year and had good function recovery .[Conclusion]Sodium hyaluronate can prevent or alleviate tendon adhesion ,so it is an ideal biological materials to prevent tendon adhesion .
