TIAN Qi-wei, HU Xi-heng, LI Yang-le
DOI: 10.3969/j.issn.1671-7171.2020.09.004
【Objective】
To evaluate the sexual function and social psychological status after partial penectomy due to penile cancer, and explore the relationship of sexual dysfunction with anxiety and depression in penile cancer patients after partial penectomy.
【Methods】
Between December 2015 and January 2020, a total of 96 patients treated with partial penectomy due to penile cancer in our hospital were prospectively analyzed in the study. Sexual function (International Erectile Function Index Questionnaire, IIEF-15), age, body mass index (BMI) , penile length(PL)in the flaccid state after partial penectomy, surgical method (partial penectomy or partial penectomy combined with bilateral inguinal lymph node dissection) and psychological factors (Self-rating Anxiety Scale/SAS and Self-Rating Depression Scale/ SDS scores) were assessed. The IIEF-15, SAS and SDS scores of preoperative and postoperative were compared. Univariate and multivariate linear regression analyses were performed.
【Results】
Ninety-six patients were included in our study. The median age was 58 years old, and the median penis length (PL) was 4.3 cm. The preoperative IIEF-15, SAS and SDS scores were significantly different from the postoperative scores. Postoperative erectile function, orgasm ability, sexual desire, satisfaction with sexual life and overall satisfaction were significantly lower than those before surgery, and SAS and SDS scores were significantly higher than those before surgery. There was no statistically significant difference in the IIEF-15 scores of patients treated with partial penectomy and partial penectomy + bilateralinguinal lymph node dissection. Erectile function, orgasmic ability, sexual desire, and intercourse satisfaction were negatively associated with SAS/SDS score. Results also showed that erectile function, sexual desire, and overall satisfaction were negatively associated with age, while intercourse satisfaction was positively associated with PL.
【Conclusion】
The sexual function of patients with penile cancer decreases after partial penectomy, and the degree of anxiety and depression increase in patients after surgery. The sexual function of penile cancer patients after partial penectomy was negatively correlated with anxiety and depression. In addition, the sexual function was negatively affected by age but positively correlated with postoperative penis length.

DOI: 10.3969/j.issn.1671-7171.2020.09.008
【Objective】
To investigate the clinical characteristics and survival status of patients with hypopharyngeal carcinoma.
【Methods】
A retrospective analysis of the clinical data of 148 patients with hypopharyngeal carcinoma admitted to our hospital was analyzed. According to the patient's 5-year survival rate, patients were divided into the survival group of 55 cases and the death group of 93 cases. The age, gender, smoking status, T grade, N grade, lesion location, clinical grade, functional status (PS) score, and treatment methods were compared between the two groups. The factors affecting the survival status of patients with hypopharyngeal carcinoma were determined by univariate analysis. Independent factors influencing the survival of patients with hypopharyngeal carcinoma were determined by multivariate analysis.
【Results】
Univariate analysis showed that the factors influencing the survival of patients with hypopharyngeal carcinoma included smoking, T grade, N grade, clinical grade, PS score, and treatment. Multivariate analysis also showed that smoking, T grade, N grade,PS score, and treatment were independent factors influencing the survival of patients with hypopharyngeal cancer. Among the 148 patients, the total number of complications in 5 years was 26, with an incidence of 17.6%.
【Conclusion】
The factors influencing the survival status of patients with hypopharyngeal carcinoma include smoking, TNM staging and surgical treatment. In order to improve the survival rate of patients, the combined therapy should be the main treatment.

DOI: 10.3969/j.issn.1671-7171.2020.09.010
【Objective】
To investigate the analgesic effect of dexmedetomidine (Dex) combined with dextrozine in laparoscopic cholecystectomy(LC).
【Methods】
A total of 90 patients undergoing LC in our hospital from June 30, 2017 to June 30, 2019 were randomly divided into the Dex group, the combination group (Dex combined with dextrozine) and the control group, with 30 cases in each group. The operation time, propofol dosage, recovery time and postoperative adverse reactions were recorded in the three groups. The heart rate (HR) and mean arterial pressure (MAP) at different times during operations (before surgery, after pneumoperitoneum, the moment of gallbladder excision, and the moment of laryngeal mask removal) were measured and recorded. The postoperative pain (VAS score) and sedation (Ramsay score) were compared among the 3 groups. Blood glucose, cortisol, and blood norepinephrine (NE) levels were compared as well before and after surgery.
【Results】
The dosage of propofol and sufentanil in the combination group was lower than that in the Dex group and the control group (P<0.05), but there were no significant differences in the operation time and recovery time among the three groups (P>0.05). There was no statistically significant difference in the incidence of adverse reactions between the three groups (P>0.05). The VAS scores at 1, 6, 12, and 24 h after operation were lower in the combination group than in the control group and the Dex group (P<0.05). The Ramsay scores at 1, 6 h after operation of the combination group were higher than those of the control group and the Dex group (P<0.05). There were no significant differences in HR and MAP among the three groups before operation, after pneumoperitoneum, and at the moment of the gallbladder excision (P>0.05). When the laryngeal mask was pulled, the HR and MAP in the combined group were lower than those in the control group and the Dex group (P<0.05). At the end of the operation, the blood glucose, cortisol and NE in the combined group were lower than those in the control group and the Dex group (P<0.05).
【Conclusion】
Dexmedetomidine combined with dextrozine has significant analgesic and sedative effects during laparoscopic cholecystectomy, which can also reduce the stress response. It is pretty safe for LC.

GAO Yi-kai, YUAN Yue, YIN Chang-jiang
DOI: 10.3969/j.issn.1671-7171.2020.09.015
【Objective】
To investigate the clinical effect of craniotomy hematoma removal combined with extraventricular drainage in the treatment of hypertensive intracerebral hemorrhage into ventricle .
【Methods】
A retrospective analysis was performed on 100 patients with hypertensive cerebral hemorrhage who underwent surgery in our hospital from January 2016 to May 2018. Among them, 50 patients in the observation group were treated with craniotomy hematoma evacuation and extraventricular drainage, while 50 patients in the control group were treated with simple craniotomy hematoma clearance only. Glasgow coma score (GCS) and intracranial pressure (ICP) were compared between the two groups at different time points before and after surgery. The serum N-terminal pro-brain natriuretic peptide (NT-proBNP) and high mobility group box 1 protein (HMGB-1) levels of the two groups were analyzed before and 2 weeks after surgery. Prognosis and surgical complications were evaluated by the modified Rankin scale questionnaire (MRS) at 6 months postoperatively.
【Results】
There were no significant differences in GCS scores and ICP measurements between the two groups before and 1 day after surgery (P>0.05). The GCS scores of the observation group at 3 days and 7 days after surgery were higher than those of the control group, while the ICP values of the observation group were lower than those of the control group (P<0.05). There was no significant difference between the two groups in the determination of NT-proBNP and HMGB1 before operation (P>0.05). At 2 weeks after surgery, the levels of NT-proBNP and HMGB1 in the observation group were lower than those in the control group (P<0.05). At 3 months after operation, the prognosis rate of the observation group was 58.00% (29/50), which was higher than that of group B (38.00%, 19/50). The difference was statistically significant (χ
2
=4.006, P=0.045<0.05). The complication rate in the observation group was 14.00% (7/50) , which was lower than 22.00% (11/50) in the control group, however, the difference was not statistically significant (χ
2
=4.006,P=0.298>0.05).
【Conclusion】
In patients with hypertensive cerebral hemorrhage breaking into the ventricle, the effect of treatment with craniotomy hematoma evacuation combined with extraventricular drainage is superior to the treatment with extraventricular drainage alone. It is beneficial to the recovery of blood-brain barrier and the prognosis of patients.

CAI Rui-yu, WAHG Hai-ping, BAI Bei-bei
DOI: 10.3969/j.issn.1671-7171.2020.09.017
【Objective】
To investigate the effect of old lacunar infarction (LI) on the prognosis of patients with primary cerebral infarction.
【Methods】
The clinical data of 145 patients with primary cerebral infarction with old LI history were retrospectively analyzed. According to the number of old Li, the patients were divided into group A (lesions≤4) and group B (lesions > 4). The grade of cerebral microbleeds, white matter lesions and intracranial artery calcification were compared between the two groups. The factors influencing the prognosis were analyzed by regression analysis.
【Results】
Compared with the group A, the proportion of grade 3 cerebral microbleeds in the group B was significantly increased (P<0.05), the proportion of grade 2 and 3 white matter lesions was significantly increased (P<0.05), and the proportion of intracranial artery calcification score above 3 points was significantly increased (P<0.05). Spearman analysis showed that the number of old LI lesions was positively correlated with the grade of cerebral microbleeds, the degree of white matter lesions and the score of intracranial artery calcification (
Rs
=0.735,0.774,0.695,P<0.05). Multivariate COX regression analysis showed that the number of old LI lesions, grade of cerebral microbleeds, grade of white matter lesions, intracranial artery calcification score, no use of antiplatelet drugs after discharge and no use of statins were independent risk factors for the prognosis of patients with primary cerebral infarction (P<0.05).
【Conclusion】
The number of old LI lesions is closely related to cerebral microvascular lesions, white matter lesions and intracranial artery calcification score, and are independent risk factors for the prognosis of patients with initial cerebral infarction

HAN Tao, CHEN Qin
DOI: 10.3969/j.issn.1671-7171.2020.09.020
【Objective】
The aim of this study is to explore the expression and significance of Cdc42 (cell division cyclin42) in the hippocampus tissue of rats with neurotoxicity of sevoflurane.
【Methods】
Forty-two Sprague Dawley (SD) rats of 7 days old were randomly divided into three groups (n=14): group A, group B and group C. Rats in group A were exposed to 2% sevoflurane and 80% oxygen inhalation once for 2 hours. Rats in group B were exposed to 2% sevoflurane and 80% oxygen inhalation 2 hours/day for three consecutive days. Rats in group C received only 80% oxygen inhalation for 2 hours. Rats were sacrificed at 3 hours after experimental models completed and the hippocampus tissues were collected to measure the expression of Cdc42 and cleaved caspase3 (cysteine proteolytic enzyme 3, CI-Csp3) by western blot. Spatial learning and memory ability of adult rats were detected by Morris water maze (MWM) test at 45-49 days after birth.
【Results】
Compared to the group C, the expression of Cdc42 and Cl-Csp3 in the hippocampus of the group A had no significant difference (P>0.05), whereas the expression of Cdc42 and Cl-Csp3 in the hippocampus of group B increased significantly (P<0.05). In place navigation test, there was no significant difference of the escape latency between group C and group A (P>0.05). The escape latency of rats in the group B were significantly longer than that of C group (P<0.05). In probe trial test, the percent time in target quadrant in group A was not different from that of group C (P>0.05), While the percent time in target quadrant in group B decreased (P<0.05).
【Conclusion】
Multiple inhalation of sevoflurane can induce neurotoxicity and memory dysfunction, which may be related to the increase in the expression of Cdc42.

Study on the Influencing Factors of Anemia in 387 Premature Infants
ZHANG Li, SHEN Jian-jia, WANG Xiao-li
DOI: 10.3969/j.issn.1671-7171.2020.09.028
【Objective】
To investigate the major risk factors in the development of anemia among premature infants (gestational age <37 weeks) in hospital.
【Methods】
The medical data of 571 premature infants admitted to our department of neonatology from Janurary 2016 to December 2017 were retrospectively reviewed. There were 387 premature infants included in the study after excluding 36 cases of congenital anemia and 148 cases of short hospitalization with only one blood collection. According to complication with anemia or not,all 387 infants were divided into two groups: anemia group and non-anemia group. At the same time, data on birth weight, gestational age at birth, comorbidities, basal hemoglobin (HB) level, blood volume collected during hospitalization, and maternal hemoglobin value at the end of pregnancy were collected to analyze the related high-risk factors associated with anemia of prematurity during hospitalization.
【Results】
There were statistically significant differences between the two groups of infants with gestational age, birth weight, length of hospitalization, gestational age, and the proportion of newborns' weight <2000 g (P<0.05). The blood sampling volume and the incidence of anemia in preterm infants less than 32 weeks gestational age were significantly higher than those of 33 weeks gestational age preterm infants and 36 weeks gestational age premature infants, the difference was statistically significant (P<0.05). The incidence of NRDS, Apnea, CPAP, FI in the anemia group were higher than those in the non-anemia group, and the difference was statistically significant(P<0.05).Multiple factor Logistic regression analysis revealed that anemia of prematurity was associated with blood loss from laboratory testing and the basal hemoglobin level. The lower basal HB level and the greater blood sampling volume were risk factors for anemia in preterm infants (P<0.05).
【Conclusion】
Anemia of prematurity is associated with the gestation, birth weight, the basal hemoglobin level and the blood loss due to laboratory testing. We should pay attention to the basal hemoglobin level, and decrease the blood loss from laboratory blood collection.

【Objective】
To investigate the effect of scenario-simulated health education on disease cognition and self-management ability of patients with chronic obstructive pulmonary disease (COPD) in China.
【Methods】
A total of 94 COPD patients from January to December 2019 was selected and divided into observation group (July to December 2019) and control group (January to June 2019) according to the length of stay, 47 cases in each group. The control group was given routine health education, and the observation group was given scenario simulation health education. After 12 weeks of follow-up, the cognitive level of disease, self-management ability and lung function were compared between the two groups.
【Results】
After 12 weeks of follow-up, the disease cognitive level scores of the two groups were higher than those before the intervention, and the observation group was significantly higher than the control group, the differences were statistically significant (P<0.05). In addition to emotional management, the scores of all dimensions of self-management ability in the two groups were higher than those before the intervention, and the scores of symptom management, daily life management, information management and self-efficacy in the observation group were higher than those in the control group (P<0.05). The ratio of FEV1 to FVC in the observation group was significantly higher than that in the control group (P<0.05).
【Conclusion】
Scenario simulation health education can improve the cognitive level of COPD patients and enhance their self-management ability, which is of great significance to improve lung function.
