1 Effect of Dietary Nutrition and Health Education on Pregnant Women

【Objective】To explore the effect of dietary nutrition and health education on the weight and pregnancy outcome of pregnant women. 【Methods】Ninety cases of primipara with 13 weeks of single pregnancy in the clinic of obstetrics and gynecology of our hospital from June 2013 to June 2015n were selected, and were divided into the observation group and control group on the basis of odd /even serial number, 450 cases in each group. The observation group was given the nutrition and health education during pregnancy and the individual nutrition guidance, while the control group was only given the guidance of physiological health knowledge during pregnancy. Body mass, pregnancy induced hypertension, pregnancy with anemia, macrosomia, cesarean section rate, dystocia, uterine atony, incidence of neonatal asphyxia were compared between the two groups of pregnant women during pregnancy.【Results】The growth rate of the observation group in the normal range was significantly higher than that of the control group (P<0.01), and the rate of body weight gain and weight gain was too much lower than that of the control group (P<0.01); the pregnancy induced hypertension, pregnancy anemia, macrosomia, dystocia, uterine atony incidence rate of the observation group were lower than those of the control group (P<0.05), cesarean section rate of the observation group was significantly lower than that of the control group (P<0.01).【Conclusion】 Dietary nutrition and health education during pregnancy has a good effect on controlling the growth of body weight during pregnancy, which can significantly reduce the incidence of complications during pregnancy and improve pregnancy outcomes.

4 Expression of BRAC-1 and RAD51 in Esophageal Squamous Cell Carcinoma and its Clinical Significance

【Objective】To determine the expression of BRCA1 and RAD51 in the esophageal squamous cell carcinoma(ESCC)and its significance. 【Methods】The expression of BRCA1 and RAD51 in 120 cases of esophageal squamous cell carcinoma and adjacent normal tissues and 60 cases of intraepithelial neoplasia were detected by SP immunohistochemical method.The relationship between the positive expression of BRCA1 and RAD51 and clinicopathological features was analyzed and compared, and the prognostic factors of esophageal squamous cell carcinoma were analyzed by COX regression analysis.【Results】The positive expression of BRCA1 in esophageal squamous cell carcinoma was mainly localized in cytoplasm, and the positive expression of BRCA1 was significantly higher than that of intraepithelial neoplasia and adjacent normal tissues (P<0.05). The positive expression in esophageal squamous cell carcinoma was located in the nucleus, and the positive expression of RAD51 was significantly higher than that of epithelial and normal esophageal mucosa tissues(P<0.05). Correlation analysis showed that the expression of BRCA1 was positively correlated with the expression of RAD51 in esophageal squamous cell carcinoma (P<0.05). Lymph node metastasis and RAD51 are the main factors affecting the prognosis of patients with esophageal squamous cell carcinoma.【Conclusion】The positive expression of BRCA1 and RAD51 in esophageal squamous cell carcinoma is associated with poor prognosis, which is expected to be a new biomarker for evaluating the biological behavior of esophageal squamous cell carcinoma.

8 Effects of High Flux Hemodialysis on TLC, Immunoglobulin Level and QOL in Treatment Chronic Renal Failure Uremia

【Objective】To investigate the effects of high flux hemodialysis on TLC, immunoglobulin level and QOL in the treatment of patients with chronic renal failure uremia . 【Methods】A total of 100 patients with chronic renal failure uremia from December 2012 to December 2015 in our hospital were collected and randomly divided into experiment group and control group with 50 cases in each group. Patients in the control group were treated by hemodialysis filtration treatment, three times a week; patients in the experiment group were treated by high flux hemodialysis therapy, three times a week, 28 d as a course of treatment. The TLC, immunoglobulin level and QOL of the two groups were compared before and after treatment . 【Results】 At the end of the treatment, the effective rate of the experiment group (96.0%,48/50) was higher than that of the control group (86.0%,43/50), the difference was statistical significance (P<0.05); Compared with before treatment, the serum creatinine level of the two groups were decreased , immunoglobulin IgA, IgM and IgG levels were increased, lung function TLC level increased , quality of life index (QOL) score increased (P<0.05), the difference was statistical significance . Compared with control group, the serum creatinine level of the experiment group was lower , immunoglobulin IgA, IgM and IgG levels were higher,TLC lung function level was higher, quality of life index (QOL) scored was higher (P<0.05), the difference was statistical significance ; Compared with the control group, the clinical complications rate of the experiment group was lower, the difference was statistical significance(P<0.05). 【Conclusion】Compared with hemodialysis, high flux hemodialysis treatment can better improve the patient's lung function, enhance immune function, improve the patient's QOL, which is worthy of clinical application.

12 Association of Plasma Fetuin-A with Acute Myocardial Infarction

【Objective】To investigate the association of plasma Fetuin-A to acute myocardial infarction (AMI) amongst the Chinese Han nationality in Changwu District, Jiangsu. 【Methods】 The ELISA method was used to detect plasma Fetuin-A in two groups with 86 cases of AMI patients (observation group) and 45 subjects without coronary artery disease (control group). 【Results】The plasma Fetuin-A in AMI patients (426.14±133.27g/L) was significantly higher than that in the control group (278.02±137.10g/L). The difference was statistical significance (P<0.01). After Logistic regression analysis with CAD (coronary artery disease) correction of risk factors (age, gender, cigarette, hypertension and diabetes). Linear regression analysis showed that only age in the traditional risk factors for coronary heart disease had a relation to plasma Fetuin-A (P<0.05). 【Conclusion】Plasma Fetuin-A is related to AMI risks.

14 Observation of Kidney Injuries after Ureterscopy Lithotripsy

【Objective】To discuss the injury and recovery of the glomeruli and renal tubule after ureterscopy lithotripsy(URSL). 【Methods】We performed URSL (holmium laser lithotripsy) for 215 patients with ureter stones. The study subjects were divided into the upper ureteral calculi (Group A) and distal ureteral calculi (Group B) according to the position of the stones, as well as the <30 min (Group C) and ≥30 min (Group D) according to operation time. By assessing the patients' indicators of urinary microprotein (mALB), N-acetyl-β-glucosaminidase (NAG), retinol binding protein (RBP) before operation and 24 hours, one week, one month, two months after operation, we evaluated the injuries of the glomeruli and renal tubule caused by URSL surgery, and investigated the effects of stone position and operation period on the injury and the injury recovery progress. 【Results】The patients' mALB, NAG, and RBP after operation were significantly above those before the operation (P<0.01) but then gradually declined after one to two months. The higher the position of the stone and the longer the operation time, the more dramatic the increase of NAG was after operation (P<0.05). 【Conclusion】Ureterscopy lithotripsy causes injury to both the glomeruli and renal tubule. The injury of renal tubule was more serious when there was a higher stone position and longer operation time. The injury was self-restoring after one to two months after operation.

17 Clinical Significance of Concentration Detection of Gastric Juice α1-antitrypsin in Diagnosis of Gastric Cancer

【Objective】To explore the clinical significance of detection of gastric juice α1-antitrypsin in diagnosis of gastric cancer.【Methods】114 patients with gastric cancer (gastric cancer group), 43 patients with benign gastric diseases (benign gastric diseases group) and 25 healthy people (control group) who underwent upper gastrointestinal endoscopic examination in our hospital from February 2011 to April 2013 were selected as research subjects. The levels of concentration of gastric juice α1-antitrypsin of these three groups were examined respectively by ELISA. The experimental and clinical data were analyzed statistically.【Results】The concentration of gastric juice α1-antitrypsin (1368.7 ± 498.4) μg/dL in gastric cancer group was significantly higher than that in the benign gastric disease group (345 ± 96.5) μg/dL and the healthy group (31.2±17.3) μg/dL, and the difference was statistically significant (P<0.05); Gastric cancer sensitivity, specificity, accuracy of gastric juice α1-antitrypsin exams were 90.4%, 89.7% and 90.1%; the α1 antitrypsin concentration of Ⅲ~Ⅳ gastric cancer patients was significantly higher than that of I ~ II patients, the difference was statistically significant (P<0.05).【Conclusion】The concentration detection of α1-antitrypsin in gastric juice can improve the accuracy in the diagnosis of gastric cancer and has great clinical value of further studying.

20 Study of Acute Kidney Injury and Related Risk Factors on Children with Acyanotic Congenital Heart Disease after CPB Cardiac Surgery

【Objective】To study the incidence of acute kidney injury (AKI) and related risk factors on children with acyanotic congenital heart disease after cardiopulmonary bypass (CPB) cardiac surgery.【Methods】One hundred and twenty-eight patients with CPB surgery in our hospital from October 2013 to October 2015 were selected as the subjects.They were divided into AKI group and non AKI group on the basis with AKI complicated or not after treatment, with 25 cases in AKI group and 103 in non AKI group . The clinical data of the two groups were compared, and the related factors of AKI were determined by single factor analysis. Multivariate Logistic regression analysis was used to determine the independent factors.【Results】Single factor analysis showed that age, body mass, accompanied with pneumonia, heart failure, serum creatinine, uric acid, operative time, aortic clamping time, postoperative blood transfusion volume, mechanical ventilation time, postoperative hypotension, whole blood activated clotting time and hospitalization time were all of the related factors on children with acyanotic congenital heart disease after cardiopulmonary bypass (CPB) cardiac surgery complicated with AKI. Multivariate Logistic regression analysis showed that age, body mass, accompanying pneumonia, concomitant heart failure, operation time, aortic occlusion time were independent factors.【Conclusion】 The complicated AKI after cardiac surgery on children with acyanotic congenital heart disease is a reversible injury at early stage, Prompt treatment is effective on the prognosis of diseased children.

23 Analysis of Relationship between iNOS Polymorphisms and the Susceptibility to Gastric Cancer

【Objective】 To investigate the relationship between inducible nitric oxide synthase (iNOS) gene polymorphism and susceptibility to gastric cancer in patients with non cardia gastric cancer.【Methods】 Eighty patients with gastric cardia cancer who were diagnosed by gastroscopy or treated in our hospital from April 2014 to 2016 were enrolled in this study as GC group. 88 cases of gastric ulcer (gastric ulcer) diagnosed by gastroscope and pathology in our hospital in the same period were selected as GU group; Another 140 healthy persons were selected as control group (group C). DNA extraction, PCR amplification, time of flight mass spectrometry were used to analyze the genetic polymorphism of iNOS-277 loci of the three groups.【Results】There was no difference between the GU group and the group C (P>0.05). There was significant difference between the GC group and the group C (P<0.01). There was no difference between the GC group and GU group (P>0.05). Expression of iNOS-277 T/T genotype is an independent risk factor for gastric ulcer and gastric cancer.【Conclusion】 iNOS gene polymorphisms is associated with susceptibility to gastric ulcer and gastric cancer. -277 locus T/T genotype is an independent risk factor for gastric ulcer and gastric cancer.

26 Influencing Factors of Defecation Patten in Elderly Patients with Hip Fractures 2 Weeks after Surgery

【Objective】To investigate the changes and influencing factors of defecation pattern and its influencing factors in elderly patients with hip fracture 2 weeks after operation.【Methods】Ninety-eight patients with hip fractures treated by surgery were studied. The changes and the influencing factors of defecation pattern were analyzed 2 weeks after surgery. Patients with constipation 2 weeks after operation were set as constipation group, those without constipation as control group, analysis of the influencing factors of constipation was conducted.【Results】From admission to the preoperation, 15.31% (15/98) in elderly patients (over 60 years old) reflected the presence of defecation 1~2, 47.96% (47/98) reflected the existence of varying degrees of defecation difficulties, 16.33% (16/98) had constipation; 2 weeks postoperation , defecation 1 ~ 2 increased to 36.73% (36/98). Defecation difficulty increased to 68.37% (67/98), and constipation incidence increased to 30.61% (30/98); 22.45% (22/98) defecation pattern did not return to normal 2 weeks after operation. Items like activity time ≤ 60 min, combined depression and anxiety, the use of opioids, incorrect defecation habits, incorrect diet , fruit and vegetable ≤ 2 species and the liquid intake ≤1.5 L of the constipation group were higher than those of the control group (P<0.05); Logistic regression analysis showed that the use of opioids was an independent risk factor for constipation in hip surgery patients. Activity time, eating fruits and vegetables, liquid intake were protective factors.【Conclusion】 The incidence of change of defecation pattern in patients with hip fracture 2 weeks after operation is relatively higher, which is closely related to the use of opioids, time of exercises and intake volume of fruits and vegetables.

29 Effects of Dexmedetomidine on Oxidation Resistance, NO Level and Cardiopulmonary Function in Patients Undergoing Lung Surgery

【Objective】To explore the effects of dexmedetomidine on oxidation resistance, nitric oxide (NO) level and cardiopulmonary function in patients undergoing lung surgery. 【Methods】A total of 76 patients who received pulmonary surgery under general anesthesia were divided into a control group (group C) and a dexmedetomidine group (group D), with 38 cases each. Tumor necrosis factor (TNF-α), interleukin-6 (IL-6), NO level, malonaldehyde (MAD), and superoxide dismutase (SOD) were detected before opening the chest (T0), 60min after OLV (T1) and at the end of operation (T2). Stroke index (SI), cardiac index (CI), airway plateau pressure, air way resistance and lung compliance at T0 to T2 were determined as well. Adverse drug reactions in two groups during the perioperative period were recorded. 【Results】Compared to those at T0, TNF-α, IL-10, TNF-α, IL-6, IL-10, NO, MDA in the two groups at T1 and T2 were significantly increased, while SOD level was significantly decreased (P<0.05). Compared to group C at T1 and T2, TNF-α, IL-10, NO, MDA in the group D were significantly lower; however, SOD was significantly higher (P<0.05). In group D, there were no significant changes in SI, CI, airway plateau pressure, air way resistance and lung compliance at T1 and T2 compared to T0 (P>0.05). The above indexes in group C at T1 were not changed (P>0.05), but SI, CI at T2 were significantly decreased. Airway plateau pressure, airway resistance and lung compliance were significantly increased (P<0.05). Compared to group C at T2, SI and CI in group D were significantly higher and airway plateau pressure, air way resistance and lung compliance were significantly lower (P<0.05). The adverse reaction rate during the perioperative period of group D was 7.89%, was significantly lower than that of group C, which was 26.32% (P<0.05). 【Conclusion】 The application of dexmedetomidine in lung surgery during the perioperative period can reduce inflammatory levels and alleviate oxidative stress injury. It is beneficial to the recovery of cardiopulmonary function.

32 Application of Rockall Risk Score in Evaluation of Acute Upper Gastrointestinal Bleeding in Cirrhosis and Nursing Countermeasures

【Objective】To evaluate the risk of patients with acute upper gastrointestinal bleeding (AUGIB) by cirrhosis with the Rockall risk score and to apply the clinical effects of corresponding nursing measures. 【Methods】In the year 2014, 57 patients with acute upper gastrointestinal bleeding (observation group) by cirrhosis were assessed by Rockall risk score. According to the evaluation results, patients were graded and applied for special care. A total of 51 AUGIB patients were recruited from January 2013 to December 2013 for routine nursing care as the control group. The clinical efficacy, hemostasis time, hospital stay, re-bleeding rate, complication incidence and mortality rate of the above two groups were compared. 【Results】The overall clinical effective rate was 91.23% in the observation group and 76.47% in the control group. The clinical efficacy of the observation group was better than that of the control group (P<0.05). The re-bleeding rate and the clinical mortality in the observation group were 8.77% and 1.75%, respectively. The re-bleeding rate and the clinical mortality rate in the observation group were 27.45% and 9.80%,respectively.The complication incidence in the observation group was 5.26%, and the complication rate in the control group was 25.49%;the complication rate in the observation group was significantly lower than that in the control group (P <0.01). In the observation group, the time of hemostasis was 2.09 ± 0.59 days and the hospital stay was 12.06 ± 2.45 days,while in the control group, the hemostasis time was 3.26 ± 0.92 days and the hospital stay was 16.76 ± 3.02 days. The hemostasis time and hospitalization time of the observation group were significantly less than those of the control group (P<0.05). The total satisfaction rate was 92.98% in the observation group and 76.47% in the control group. The satisfaction degree of the nursing group in the observation group was significantly more than that in the control group (P<0.01). 【Conclusion】The assessment of Rockall risk score in patients with acute upper gastrointestinal hemorrhage of liver cirrhosis can guide clinical corresponding special nursing care and thus effectively reduce the incidence of complications, shorten the rehabilitation time and improve Clinical efficacy and patient satisfaction.

36 Expression of MMP-9, bcl-2, and the HSP90α Protein in Non-Small Cell Lung Cancer Tissues and its Cinical Significance

【Objective】To investigate the expression of matrix metalloproteinase-9 (MMP-9), B-cell lymphoma-2 gene (bcl-2), and heat shock protein 90 alpha (HSP90α) in non-small cell lung cancer (NSCLC) tissues and its clinical significance.【Methods】The pathological tissues of 62 patients with NSCLC were collected and taken as the observation group. There were 30 cases in which the normal lung tissues were adjacent to the cancer tumors with a distance to the tumor of a margin > 3 cm. These 30 cases were determined to be the control group. The expression levels of MMP-9, bcl-2, and HSP90α in pathological tissues and cancer adjacent tissues were determined, and the correlation between the expression of each protein and clinical pathological factors was analyzed.【Results】The expression level of and positive rate of MMP-9 mRNA, bcl-2 mRNA, and HSP90α mRNA in the observation group were all higher than those in the control group (P<0.05). There were significant differences in the positive rates of MMP-9 and HSP90 protein between patients with NSCLC of different TNM stages, with or without lymph node metastasis (P<0.05). There were significant differences in the positive rates of bcl-2 protein between patients with NSCLC with different tissue types (P<0.05). 【Conclusion】The positive rates of MMP-9, bcl-2, and HSP90α in non small cell lung cancer tissues are higher than those in cancer adjacent normal tissues. Expression of MMP-9 and HSP90 is related to TNM stage and lymph node metastasis, and expression of bcl-2 protein is related to NSCLC tissue types.

39 Relationship among BNP, Iron Overload and Early Cardiac Dysfunction in Children with Severe β-thalassemia

【Objective】To investigate the relationship among BNP, iron overload and early cardiac dysfunction in severe β-thalassemia children. 【Methods】A total of 42 patients with severe β thalassemia (observation group) from January 2014 to January 2016 in our hospital were enrolled in this study. In the same period time, 40 healthy children were collected as control group. All cases were checked by echocardiography for cardiac parameters and their blood specimen were measured for hemoglobin, BNP, ferritin and other indicators. According to whether there was presence of diastolic dysfunction or not, the observation group were further divided into two subgroups, E / E '<9 and E / E'≥9 groups, based on the ratio of E / E ' (early diastolic mitral flow velocity / septal early diastolic velocity) . In between the two subgroups, patients' cardiac parameters and blood biochemical indices were compared as well. 【Results】BNP and ferritin levels in the observation group were significantly higher than those in the control group while hemoglobin was lower; the differences were statistically significant (P<0.01). LVED (left ventricular end diastolic diameter), LVES (left ventricular end systolic diameter), LA (left atrial diameter), RV (right ventricular diameter), LAVI (left atrial volume index) and LVMI (left ventricular mass index) in the observation group were significantly higher than those in the control group, the differences were statistically significant (P<0.05). E (early diastolic mitral flow velocity), A (late diastolic mitral flow velocity) and E / E ' (septal early diastolic velocity) in the observation group were higher than those in the control group, while IVRT (isovolumic relaxation time), S' (septal systolic velocity) and E' (septal early diastolic velocity) were lower than those in the control group; the differences were statistically significant (P<0.05). In the E / E' ≥9 subgroup, E, E / E ', LVES, LVED, LA, LAVI and LVMI were higher than those in the E / E' <9 subgroup (P<0.05), however, its E 'is lower than in the E/E' <9 subgroup (P<0.05). BNP and ferritin levels were positively correlated in the observation group (r=0.939,P=0.000); levels of BNP and hemoglobin showed no significant correlation (r=0.102,P=0.436). In the observation group, BNP was negatively correlated with S' and A' (septal late diastole) (r=-0.300,-0.421,P=0.043,0.031). BNP showed no correlation with E/E'(r=0.082,P=0.519). In the E / E '<9 subgroup, BNP and ferritin levels were positively correlated (r=0.835,P=0.000) and BNP and E / A ratio was positively correlated (r=0.438,P=0.029). In E / E' ≥9 group, BNP levels and ferritin was positively related (r=0.949,P=0.000) and BNP was positively correlated with LAVI (r=0.396,P=0.026). Further BNP showed negative correlation with S' and A' (r=-0.401,-0.409,P=0.035,0.036). 【Conclusion】BNP levels and diastolic dysfunction showed no significant correlation in severe β-thalassemia children. However, BNP had significant correlation with iron overload, which to some extent reflects cardiac dysfunction.

42 Clinical Observation of Miniplate Combined with Ring Jaw Ligation in Children with Mandibular Fracture

【Objective】 To summarize the effects of the miniplate combined with ring jaw ligation fixation in children of mandibular fracture. 【Methods】 Clinical data of sixty-nine cases of children with mandibular fracture in our hospital from February 2011 to December 2014 were selected and analyzed retrospectively. They were treated with the miniplate combined with ring jaw ligation fixation by the same dentist. The perioperative indexes and postoperative curative effect were observed and analyzed. 【Results】 The operative time of 69 cases was 36.8 ~ 47.3 min (average 41.0±6.8) min. The phenomenon of repeated bleeding did not ocure. The occlusal relationship was improved significantly in 59 cases. A small opening in the anterior region (occlusion) occurred in 3 cases. They were treated with the craniomaxillary elastic bandage. Two cases with condylar fracture were treated with indwelling ring operation combined with intermaxillary ligation. No gingival ulceration and bleeding occurred during the postoperative follow-up stage. The average healing time was (3.1 ± 0.3) months. After operation, 47 cases was normal over the mouth opening degree , and 20 cases were limited with first degree and 2 cases with second degree . 3 months after the operation, the mouth opening degrees in all of the cases were returned to normal. The mouth opening degrees in 3 months after operation were significantly improved compared with the time immediately after surgery. However, no statistical difference between the time immediately after surgery and 3 months postoperation was observed in GI, PLI and SBI(P>0.05). 【Conclusion】 The miniplate combined with ring jaw ligation is reliable effect in the treatment children's mandibular fracture. The therapy has no adverse effect in the recovery of mouth opening degree and the periodontal oral health.

46 Clinical Efficacy and Safety of Gestrinone and Levonorgestrel-Releasing Intrauterine System (LNG-IUS) in Treatment of Adenomyosis

【Objective】To discuss the clinical efficacy and safety of gestrinone and levonorgestrel-releasing intrauterine system (LNG-IUS) in the treatment of adenomyosis. 【Methods】One hundred and sixty-eight cases of adenomyosis in our hospital from February 2014 to March 2016 were selected and divided into the observation group and the control group by random number table method. The observation group was treated with LNG-IUS, while the control group with gestrinone. The degree of dysmenorrhea, uterine volume, endometrial thickness, menstrual volume, hemoglobin levels and adverse reactions were compared between the two groups before and after treatment.【Results】 Compared with the conditions before treatment, dysmenorrhea and menorrhagia of the two groups of patients after treatment were significantly relieved. Hemoglobin levels were significantly increased, and the dysmenorrhea, menorrhagia and anemia symptoms of the observation group were improved (P<0.05); Compared with the control group before treatment, the endometrial thickness and uterine volume of the observation group after treatment were significantly reduced (P<0.05); there was no significant difference in endometrial thickness and uterine volume in the control group before and after treatment (P>0.05); Compared with the control group, the incidence of adverse reactions was obviously lower in the observation group (P<0.05).【Conclusion】The effect of LNG-IUS in treatment of adenomyosis is significantly better than that of gestrinone, and it is safe and worthy of clinical application.

49 Clinical Analysis of Different Surgical Incisions in the Treatment of Breast Fibroma

【Objective】To explore the therapeutic effect of different surgical incisions for breast fibroma. 【Methods】 A total of 320 patients with breast fibroma were enrolled in our hospital from January 2015 to January 2016. The patients were divided into control group and study group according to the random number method. The former was treated with radial incision, The operation time, bleeding volume and postoperative complication rate were compared between the two groups. 【Results】 The total effective rate was 80.0% in the control group and 100.0% in the study group, the difference was statistically significant(P<0.05).The operation time was (43.16 ± 3.08) min in the control group and the intraoperative blood loss was(125.83±7.42)mL.Operative time was(20.84±2.67) min and the blood loss was (6.27±1.46) ml in study group, the difference was statistically significant(P<0.05).The complication rate was 11.9% in the control group and 3.75% in the study group, the difference was statistically significant (P<0.05). 【Conclusion】 The operation of ring-areola incision in the treatment of breast fibroma is better. It can reduce the operation time, blood loss, improve the clinical curative effect and reduce the complication. It is suitable for clinical application.

51 Comparative Analysis of the Effect of Rouviere Groove Guided Positioning and the Conventional Positioning in Laparoscopic Cholecystectomy

【Objective】To analyze the effect of Rouviere groove guided positioning and conventional positioning in laparoscopic cholecystectomy.【Methods】One hundred and eight cases of laparoscopic cholecystectomy in our hospital from April 2015 to April 2016 were randomly divided into control group and observation group, each group of 54 cases.The control group underwent traditional positioning, the observation group received Rouviere groove guided positioning. Liver function like the total bilirubin (TBIL) ,the aspartate aminotransferase (AST), the alanine aminotransferase (ALT), the operation time and complication rate of the two groups were compared after treatment. 【Results】There was no significant difference in TBIL, AST and ALT between two groups before operation (P>0.05); After cholecystectomy, the serum TBIL, AST, ALT levels in the two groups were higher than those before operative , the observation group was lower than that in the control group, the difference was statistically significant (P<0.05). The operation time of the control group (67.54 ± 7.42) min was significantly higher than that of the observation group (47.38±5.69)min, the difference was statistically significant (P<0.05). There were 4 cases of bile leakage and bile duct injury in the control group, the complication rate was 14.82% (8/54); The observation group had 2 cases of bile leakage, the incidence of complications was 3.70% (2/54), the difference between the two groups was statistically significant (P<0.05).【Conclusion】The clinical effect of Rouviere groove guided positioning in laparoscopic cholecystectomy is superior to the conventional positioning operation.

54 Correlation Analysis between Modified Alvarado Score System and Pathology of Acute Appendicitis During Pregnancy

【Objective】To analyze the correlation between the modified Alvarado score system and pathological results of acute appendicitis during pregnancy and thus, to explore its application in the diagnosis of acute appendicitis during pregnancy. 【Methods】Clinical data of 69 cases of appendectomy during pregnancy were collected from January 2004 to January 2013 in our hospital. Correlation between the results of Alvarado scores and pathological diagnosis was analyzed. 【Results】The patients in the simplex group of appendicitis scored an average of 4.2±1.6 points and the patients in the progressive group scored an average of 7.0±1.9 points; there was significant difference between the two groups (P<0.01). The result also showed a positive correlation between scoring system and pathologic results (P<0.01). 【Conclusion】Modified Alvarado score system may facilitate the early diagnosis of acute appendicitis and the determination of the surgical treatment during pregnancy. It has important value in reducing the non-necessity emergency operation rate and appendix perforation risk.

56 Meta-Analyisis of the Efficacy of Octreotide in Patients with Refractory Ascites due to Cirrhosis

【Objective】 To evaluate the efficacy and safety of octreotide in patients with refractory ascites due to cirrhosis. 【Methods】 The relevant randomized controlled trials were searched throughout PubMed, Embase, Cochrane Library, CBMdisc, CNKI, Chinese VIP database, and WanFang database. All articles were identified by a search from January 2000 to July 2015. Trials comparing the efficacy of octreotide plus routine medical therapy versus routine medical therapy in patients with refractory ascites due to cirrhosis were included. Two investigators independently assessed the methodological quality of included trials and performed the data extraction. Meta-analysis was carried out with Review Manager Software 5.2. 【Results】 Ten eligible trials including 757 patients were included and assessed. According to the results of the meta-analysis, octreotide plus routine medical therapy was remarkably more effective than routine medical therapy in terms of total effective rate [OR=2.86, 95%CI(1.90, 4.31),P<0.05], 24-hour urine volume [MD=259.80, 95%CI(249.45, 270.15),P<0.05], abdominal perimeter [MD=-11.72, 95%CI(-13.30, -10.14),P<0.05], and body weight [MD=-4.80, 95%CI(-7.02, -2.58),P<0.05]. 【Conclusions】 Octreotide can significantly increase the 24-hour urine volume, reduce the abdominal perimeters and body weight, and raise the total effective rate in patients with refractory ascites due to cirrhosis.

61 Comparison of the Application of Ultrasound-guided and Nerve Stimulator-positioning Axillary Brachial Plexus Block in Anesthesia for Patients Undergoing Lower Elbow Surgery

【Objective】To compare the application value between nerve stimulator positioning combined with ultrasound-guided and simple ultrasound-guided axillary brachial plexus block in anesthesia for patients undergoing lower elbow surgery. 【Methods】Sixty patients undergoing lower elbow surgery were selected as the study subjects and were divided into the observation group and the control group, with 30 cases in each. The control group was treated with ultrasound-guided axillary brachial plexus block while the observation group was treated with nerve stimulator positioning combined with ultrasound-guided axillary brachial plexus block. The operation time, onset time of block of relevant nerves, duration of nerve block, success rate of block within 0.5h and success rate of one-time puncture were observed and compared between the two groups. The incidence of related complications was recorded. 【Results】The total success rate of brachial plexus block within 0.5h in the observation group was significantly higher than that in the control group (100.0% vs 86.7%), while the incidence of related complications was significantly lower than that in the control group (6.7% vs 26.7%) (P< 0.05). The average operation time and block onset time of radial nerve, ulnar nerve, median nerve and musculocutaneous nerve of patients in the observation group were significantly shorter than those of control group, while the duration of nerve block was significantly longer than that of the control group (P<0.05). There was no significant difference in the success rate of one-time puncture between the two groups (P>0.05). 【Conclusion】The application of nerve stimulator combined with ultrasound-guided axillary brachial plexus block in patients undergoing lower elbow surgery can improve the accuracy of positioning and the success rate of axillary brachial plexus block. It has positive effects on reducing the risk of related complications and extending the duration of the nerve block.

64 Application Value of MRI in Differential Diagnosis of Brucellar Spondylitis and Spinal Tuberculosis

【Objective】 To explore the application value of MRI in the differential diagnosis of brucellar spondylitis and spinal tuberculosis. 【Methods】The MRI data of 40 cases with brucellar spondylitis and 50 cases with spinal tuberculosis from January 2012 to October 2016 were retrospectively analyzed. 【Results】There were significant differences between the brucellar spondylitis group and the spinal tuberculosis group in the sites of destroyed vertebral bodies [front side, back side and both sides (25.00%, 60.00%, 15.00%) vs (68.00%, 16.00%, 16.00%)], abscess in paravertebral soft tissues (7.50% vs 74.00%), intervertebral disc destruction [limited, diffuse (45.00%, 17.50%) vs (18.00%, 40.00%)], intervertebral space stenosis (27.50% vs 50.00%), intervertebral disc signal abnormalities (80.00% vs 30.00%) and intervertebral space disappearance (0.00% vs 20.00%) (P<0.05). The pathogenic sites, number of vertebral involvement and the incidence of kyphosis showed no significant differences (P>0.05). 【Conclusion】 The MRI findings such as pathogenic sites and number of vertebral involvement in brucellar spondylitis and spinal tuberculosis are similar, but the differences in the sites of vertebral destruction and intervertebral space stenosis are significant, which can be used as important MRI signs for the differential diagnosis of the two diseases.

67 Plasma IL-12 and MCP-1 in Patients with Coronary Artery after Percutaneous Coronary Intervention

【Objective】To explore the changes of plasma IL-12 and MCP-1 for patients with coronary artery after percutaneous coronary intervention, and the correlation with short-term prognosis. 【Methods】Clinical data of 268 patients with CHD from January 2013 to December 2015 in our hospital were set as the observation group, and 50 healthy volunteers underwent coronary angiography with normal coronary arteries were selected in the same period of time in our hospital as the control group. The plasma levels of IL-12 and MCP-1 were compared between the two groups. The incidence of adverse cardiovascular events in CHD patients was observed, and the correlation between IL-12 and MCP-1 levels and adverse cardiovascular events were analyzed.【Results】Before surgery, plasma IL-12, MCP-1 levels in observation group were significantly higher than those of the control group, the difference was significant (mean P <0.05); After surgical treatment of PCI, a significant decrease in the above levels were found in the observation group, the difference was significant (mean P<0.05), while there was no significant difference before and after angiography in the control group, the difference was not statistically significant(mean P>0.05); Plasma IL-12 and MCP-1 levels were positively correlated with major adverse cardiovascular events (mean P<0.05). 【Conclusion】Plasma IL-12 and MCP-1 levels were significantly lower in patients with CHD after PCI; this could be correlative with the short term adverse prognosis of patients with CHD.

70 Clinical Comparison of External Fixation and Endoscopic Open Reduction with Internal Fixation in the Treatment of Zygomatic Arch Fractures

【Objective】To compare the clinical effects between external fixation and endoscopic open reduction with internal fixation in the treatment of zygomatic arch fractures. 【Methods】The clinical data of 80 patients with zygomatic arch fractures who underwent external fixation (control group,n=40) and endoscopic open reduction with internal fixation (study group,n=40) in our hospital between June 2011 and April 2013 were analyzed retrospectively. All subjects were followed up for 3 years. The one-stage healing rate of incision, incidence of postoperative complications and prognosis of the patients (incidence of residual scar, excellent and good rate of occlusion, complete reduction rate of fractures) were compared. The differences in mouth-opening degree before and after operation were recorded. 【Results】After operation, the incision of both groups showed one-stage healing and there were no cases of facial paralysis, head and face skin numbness, limited chewing function, facial depression or other complications (P>0.05). There was 1 case of mild hair loss and 1 case of slight swelling of the face in study group after operation, and the symptoms spontaneously remitted. In the control group, there was 1 case with mild restriction of mouth-opening, and after 1 ~ 2 months of mouth-opening function training, it returned to normal. At 3 years after operation, the mouth-opening degree of both groups was significantly higher than that of before operation (P<0.05), but there was no significant difference between the two groups (P>0.05). At the last follow-up, there was presence of facial symmetry and the CT review showed good healing of zygomatic arch fractures, without residual scarring. The effective rate of occlusion and complete reduction rate of fractures was 100% (P>0.05). The occluding relation was basically the same as before injury. 【Conclusion】Both external fixation and endoscopic open reduction with internal fixation are of high application value in the clinical treatment of patients with zygomatic arch fractures. Patients can choose the optimal operation according to the actual situation to create conditions for improving the recovery effect of prognosis.

73 Enhancement Effects of Plasma Exchange Combined with Adrenal Cortical Hormone on Rapidly Progressive Glomerulonephritis

【Objective】To explore the clinical efficacy of intensive plasma exchange and glucocorticoid therapy on rapidly progressive glomerulonephritis. 【Methods】 Fifty-nine patients with rapidly progressive glomerulonephritis (RPGN) were analyzed retrospectively. All patients were treated with combined with adrenocortical hormone therapy. The patient's outcomes were analyzed.【Results】Patients accepted intensive plasma exchange treatment with an average of 13.2±2.6 times, had an average hospital length of 72.8±11 days , average hematuria disappearance procedure of 27.4±11.6 days, mean edema subsided time of 15.8±5.1 days, and blood pressure recovery of 14.7±4.2 days. After 4 weeks and 8 weeks of treatment, there were evidence the showed improvement of ALB, eGFR, and 24h urine volume with statistical significances (P<0.05). On the other hand, urine protein, serum creatinine (SCR), anti GBM (anti-glomerular basement membrane) antibody value were reduced significantly after treatment, the difference were also statistically significant (P<0.05). Of those 59 patients, 15 (25.42%) were cured, 34 improved (57.63%), and 10 were invalidated (16.95%). The 10 invalid patients after discharge were treated with long-term hemodialysis in our outpatient hemodialysis center. 【Conclusion】Intensive plasma exchange combined with adrenal cortical hormone treatment is valuable for rapidly progressive glomerulonephritis.

76 Clinical Features, MRI Findings and Curative Effect of 40 Cases of Deep Infiltrating Endometriosis

【Objective】 To investigate the clinical features, MRI image representation and curative effect of deep infiltrating endometriosis (DIE). 【Methods】 Clinical data of 85 patients with endometriosis diagnosed in our hospital from February 2014 to February 2016 were retrospectively analyzed, including the clinical, MRI imaging and treatment. The clinical features of DIE were compared with those of non DIE group, and the MRI imaging findings and therapeutic effects of DIE were observed.【Results】40 cases in 85 patients were confirmed with DIE pathologically, the incidence of severe dysmenorrhea and dyspareunia, adnexal cyst activity, uterosacral ligament nodules of the DIE group were 92.5%, 65%,72.5%, 67.5% ,which were significantly higher than those in the non DIE group (n=45) 33.3%, 11.1%, 42.2%,17.8%. The difference was statistically significant (P<0.05). 5 cases of vaginal rectum nodules were found in the DIE group, posterior fornix blue nodules in 15 cases, but none of them were found in the non DIE group. MRI showed that accumulated uterine uterosacral ligament were found in 27 cases (uterosacral ligament thickening, asymmetry or ligament nodules); T1WI, T2WI sequences are mainly high signal. The total effective rate of treatment was 90.6%. The clinical symptom score and CA125 level 6 months after the operation were significantly lower than those before operation (P<0.05) . 【Conclusion】 The typical clinical symptoms of patients with DIE are severe dysmenorrhea and dyspareunia. Rectovaginal nodules and posterior fornix blue nodules are sensitive manifestations for diagnosis of DIE; MRI can accurately display the range of DIE, which plays an important role in preoperative diagnosis of DIE. According to the clinical symptoms and MRI results, the effect of corresponding treatment methods will be good.

79 Diagnostic Value of Circulating Tumor Cells Combined with Tumor Markers in the Detection of Advanced Non-small Cell Lung Cancer

【Objective】To explore the diagnostic value of circulating tumor cells (CTC) combined with serum tumor markers in patients with advanced non-small cell lung cancer (NSCLC). 【Methods】A total of 84 patients with pathological diagnosis of ⅢB ~Ⅳ NSCLC were selected as the observation subject (NSCLC group ). In addition, 60 patients with benign lung lesions served as the control group. MACS negative enrichment joint immunocytochemistry was used to detect peripheral blood CTC. Meanwhile, ECL method usage compared the levels of CTC, serum CEA, CA125 and CA199 in peripheral blood between the two groups. 【Results】Generally, the detection levels were significantly higher in the observation group than in the control (P<0.05) in terms of CTC, CEA, CA199 and CA125 positive rates. Judging by CTC detection, the differential diagnosis of NSCLC patients had a sensitivity of 64.29% and a specificity of 96.67%. CEA differential diagnosis of NSCLC was 47.62% in sensitivity and 86.67% in specificity. Differential diagnosis of NSCLC by CA199 resulted in sensitivity of 36.90% and specificity of 76.67%. In the combination of CTC + CEA + CA125 + CA199, the differential diagnosis of NSCLC patients reached sensitivity of 88.10% and specificity of 96.67 %.【Conclusion】CTC combined with serum tumor markers for differential diagnosis of patients with advanced NSCLC can be significantly improved.
