XIAO Pei-fen, LUO Jing
DOI: 10.3969/j.issn.1671-7171.2019.09.002
【
Objective
】 To investigate the effect of local continuous irrigation and chemotherapy on lumbar tuberculosis in stage I posterior debridement, bone graft fusion and internal fixation. 【
Methods
】From January 2010 to January 2014, 55 consecutive patients with lumbar tuberculosis were treated in our hospital. Among them, 30 patients underwent stage I posterior debridement, bone graft fusion and internal fixation (group A), and 25 patients underwent stage I posterior debridement, bone graft fusion and internal fixation combined with local continuous irrigation and chemotherapy (group B). The clinical efficacy and imaging results of the two groups were compared. 【
Results
】All patients were followed up for 23 (12~36) months. The intervertebral bone graft fusion time in the group B was 6.5 (5~10) months, which was significantly shorter than that in the group A [8.5 (6~12) months] (P<0.05). Erythrocyte sedimentation rate (ESR) in the group B was significantly lower than that in the group A at 6 weeks after operation (P<0.05), but ESR returned to normal at the last follow-up. The neurological symptoms of the two groups recovered to varying degrees. The wounds healed in the first stage. No chronic sinus formation, internal fixation rupture, loosening and other phenomena occurred in either group. 【
Conclusion
】The treatment of lumbar tuberculosis by posterior debridement, bone graft fusion and internal fixation combined with local continuous irrigation and chemotherapy can significantly accelerate the fusion of bone graft and the regression of inflammation, which is worthy of clinical application.

DOI: 10.3969/j.issn.1671-7171.2019.09.007
【
Objective
】To investigate the effects of betel nut chewing, smoking and drinking on the age of onset of oral squamous cell carcinoma (OSCC). 【
Methods
】A total of 488 patients with oral squamous cell carcinoma from August 2014 to September 2015 in a third-level hospital of Hunan Province were collected. The relationship between bad living habits and age of onset of oral squamous cell carcinoma was drawn by Kaplan-Meier method.COX risk model was used to assess the risk ratio and 95% CI confidence interval between bad living habits and age at onset.【
Results
】Betelnut chewing in earlier than 21 years of age has an effect on the age of onset of oral squamous cell carcinoma [aHR = 1.8, 95% CI (1.5~2.1),P<0.001]. The greater the amount of betel nut chewing, the longer the exposure time, the greater the risk of early onset (P<0.001). Smoking had an effect on the age of onset (P=0.039). The age of onset of oral squamous cell carcinoma was 3 years earlier in smokers than in non-smokers [aHR=1.6, 95% CI (1.2~2.2),P=0.001]. Compared with non-smokers and non-drinkers, those who chewed betel nut and smoked earlier than 21 years of age had 9 years earlier onset age [aHR=2.8, 95% CI (2.0~3.9)].【
Conclusion
】Betelnut chewing and early smoking are risk factors for the early onset of oral squamous cell carcinoma. and can advance the onset age of OSCC.

HUANG Jian, WU Ji-wei, LUO Rui-bin,et al
DOI: 10.3969/j.issn.1671-7171.2019.09.023
【
Objective
】To investigate the clinical significance of dynamic monitoring of serum TGF-β1 in children with hypoxic ischemic encephalopathy (HIE). 【
Methods
】A total of 40 children with HIE admitted to the neonatology department of our hospital from January 2017 to December 30, 2018 were enrolled. According to the HIE clinical classification criteria, they were divided into mild group (19 cases), moderate group (19 cases) and severe group (10 cases). Twenty healthy newborns of hospital delivery during the same period were included in the control group. Levels of peripheral blood transforming growth factor-β1 (TGF-β1) were measured and neonatal neurobehavioral assay (NBNA) were examined in all groups. The diagnostic value of TGF-β1 for moderate to severe HIE was analyzed using ROC curves. 【
Results
】There were no significant differences in gender, gestational age, mode of delivery, maternal primipara, abortion history, intrauterine infection and premature rupture of membranes in the four groups (P>0.05). There were significant differences in TGF-β1 levels between 24 h and 72 h after birth (P<0.05). However, one week after birth, there was no significant difference in TGF-β1 levels in all groups (P>0.05). The decrease of TGF-β1 level and NBNA score of the HIE children reflected the severity of HIE; the difference was statistically significant (P<0.05). At 24 h, 72 h, and 1 week after birth, TGF-β1 levels were positively correlated with NBNA scores (r=0.523, 0.513, 0.428). At 24 h after birth, the optimal critical point for TGF-β1 diagnosis of moderate-severe HIE was 15.6 μg/L, the sensitivity was 71.5%, the specificity was 79.5%, and the area under the AUC curve was 0.78. 【
Conclusion
】In the order of mild-moderate-severe HIE, TGF-β1 levels decrease and are positively correlated with NBNA scores.

For suspected placenta previa patients, the results of cesarean section were used as pathological diagnostic criteria and the diagnostic accuracy of two kinds of detection was evaluated. 【
Results
】All the patients were confirmed by surgery and pathology. Among the 64 cases of placenta previa, 35 (54.69%) cases were of central type, 9 (14.06%) cases were of partial type, 11 (17.19%) cases were of marginal type, and 9 (14.06%) cases were of low type. The accuracy of MRI in the diagnosis of placenta previa in the posterior wall of the uterus in the third trimester was 98.44%, and only 1 case was misdiagnosed, that is, partial misdiagnosis was central type. The diagnostic accuracy of color doppler ultrasound was 95.31%, and 3 cases were misdiagnosed, among which 2 cases were borderline misdiagnosed as central type and 1 case was partially misdiagnosed as central type. The accuracy of MRI in the diagnosis of placenta previa in the posterior wall of the uterus in the third trimester was slightly higher than that of color doppler ultrasound, but the difference was statistically significant (P>0.05). 【
Conclusion
】In the clinical diagnosis of placenta previa via the posterior wall of the uterus in the third trimester of pregnancy, color doppler ultrasound is the first choice for detection, and MRI can be performed for those suspected of placenta previa when necessary. The two diagnostic techniques have high accuracy in the diagnosis of placenta previa via the posterior wall of the uterus in the third trimester of pregnancy.

Therapeutic Effect of Sinus Endoscopic Surgery Combined with Chinese Medicine Nasal Lavage on Sinusitis
CHEN Hong-chang, LIAO Lei, ZHOU Guang-yao,et al
DOI: 10.3969/j.issn.1671-7171.2019.09.029
【
Objective
】To explore the clinical effect of sinus endoscopy plus traditional Chinese medicine (clearing heat, dampness and phlegm) nasal lavage on sinusitis patients. 【
Methods
】A total of 82 patients with sinusitis admitted to our hospital from January 2016 to January 2017 were randomly divided into the observation group (n=41, endoscopic sinus surgery combined with Chinese medicine nasal lavage) and the control group (n=41,endoscopic sinus surgery combined with antibiotics saline nasal lavage). The clinical effects of the two groups were compared. 【
Results
】The nasal airway resistance at 8 and 12 weeks after treatment in the observation group was lower than that in the control group, and the olfactory function score of the observation was also less than that of the control group(P<0.05).The time of mucosal epithelialization after treatment in the observation group was shorter than that in the control group, so the total effective rate 97.56% of the observation group was higher than that of the control group 85.37%, and the recurrence rate 4.88% of the observation group was lower than that of the control group 21.95% (χ
2
=3.904、5.144,P<0.05).The volume of nasal cavity (NCV) and the minimum cross section area of the nasal cavity (NMCA) after treatment in the observation group were more than those of the control group. The total score of quality of life (SNOT-20) score was lower than that of the control group(P<0.05).【
Conclusion
】The nasal lavage of traditional Chinese medicine after endoscopic sinus surgery can effectively reduce the nasal airway resistance and improve the patient's sense of smell for patients with sinusitis. The overall effect is good.

Comparison of Clinical Effects of Different Anesthetics on Spinal-epidural Anesthesia in cesarean section
LI Ting, CAO Wen-liang, LI Kai,et al
DOI: 10.3969/j.issn.1671-7171.2019.09.030
【
Objective
】To explore the anesthetic effect of ropivacaine and bupivacaine on spinal-epidural anesthesia in cesarean section. 【
Methods
】A total of 220 cases of cesarean section pregnant women admitted to our hospital were randomly divided into two groups, with 110 cases in each group. The observation group was anesthetized with ropivacaine. the control group was anesthetized with bupivacaine. The anesthesia effect and adverse reactions were observed in the two groups. 【
Results
】The average arterial pressure and heart rate of the two groups after anesthesia were lower than those before anesthesia (P<0.05). The mean arterial pressure and heart rate at 5 minutes after anesthesia in the observation group were higher than those in the control group (P<0.05). The incidence of adverse reactions and intraoperative hypotension in the observation group were lower than those in the control group (P<0.05). The sensorimotor block in the observation group was better than that in the control group after anesthesia (P<0.05). 【
Conclusion
】Both ropivacaine and bupivacaine are effective in spinal-epidural anesthesia for cesarean section. However, ropivacaine has less effect on pregnant women, and the incidence of adverse reactions are mild. So it is worthy of popularization.

WANG Xiao-xia, LIU Yue-ying, XU Wen-gang
DOI: 10.3969/j.issn.1671-7171.2019.09.031
【
Objective
】To investigate the clinical efficacy of L-carnitine combined with astragalus injection in the treatment of acute viral myocarditis (AVM) and explore its effect on serum myocardial enzyme markers. 【
Methods
】The clinical data of 104 patients with acute viral myocarditis received admitted our hospital were retrospectively studied. According to the different methods of administration, they were divided into the observation group (L-carnitine combined with Astragalus injection) and the control group (L-carnitine treatment only). There were 52 cases in each group. The therapeutic effects of the two groups were compared. Levels of serum myocardial enzyme markers and incidence of adverse reactions before and after treatment were compared as well. 【
Results
】After 2 weeks of treatment, the total effective rate of the observation group was 80.7% (42/52), which was higher than that of the control group 59.6% (31/52), and the difference was statistically significant (P<0.05). The levels of serum CK, CK-MB and CTnT in the two groups after treatment were lower than those before treatment; the CK, CK-MB and CTnT levels of the observation group were lower than those of the control group. The difference was statistically significant (P<0.05). There were no significant differences in the incidence of adverse reactions such as headache, allergic reaction, liver and kidney function damage, and dyspnea in the two groups (P>0.05).【
Conclusion
】L-carnitine combined with astragalus injection is effective in the treatment of acute viral myocarditis. It can effectively improve cardiac function, decrease levels of serum myocardial enzyme markers but not increase drug adverse reactions, which is worthy of clinical application.
