1665 Effect of Cofilin-1 Vverexpression on the Invasion, Proliferation and Apoptosis of Human Glioma Cells

【Objective】 To analyze the expression level of Cofilin-1 in glioma cell lines and explore its effects on proliferation, apoptosis and invasion of glioma cells. 【Methods】 Cofilin-1 overexpression plasmid was constructed and transfected into human glioma cell line U-87MG cells. The transfection efficiency of Cofilin-1 was detected by Real-time PCR and Western blot. The proliferation of U-87MG was assessed by MTT assay; Transwell assay was used to detect cell invasion; Contents of VEGF and MMP-9 in cell culture supernatant were measured by ELISA; Western blot was used to detect the protein levels of bax, Bcl-2, AKT, and p-AKT. 【Results】 Cofilin-1 overexpression plasmids were constructed by gene cloning technique and transfected into human glioma cell line U-87MG cell line. Real-time PCR and Western blot were used to test the effect of Cofilin-1 transfection. The levels of vascular endothelial growth factor (VEGFA) and matrix metalloproteinase-9 (MMP-9) in the supernatant of cultured cells were determined by ELISA assay. The expressions of bax, Bcl-2, AKT and p-AKT proteins were detected by Western blot. MTT assay was used to detect cell proliferation and transwell chamber assay was used to detect cell invasion ability.ELISA assay was used to detect vascular endothelial growth factor (VEGFA) and matrix metalloproteinase-9 (MMP-9) in supernatant of cell culture. The expression of apoptosis-related proteins (bax and Bcl-2, AKT and p-AKT) was detected by Western blot.【Conclusion】 Overexpression of Cofilin-1 can promote the proliferation and invasion of glioma cells. The mechanism may be that the activation of AKT pathway may play a role in Cofilin-1, which may become a potential therapeutic target for glioma.

1669 Effect of Hydromorphone on Patient-Controlled Analgesia after Knee Arthroplasty in Elderly Patients

【Objective】 To study the effect of hydromorphone on patient-controlled epidural analgesia (PCEA) after knee arthroplasty in elderly patients.【Methods】 Ninety-two elderly patients undergoing knee arthroplasty in Guizhou Orthopaedic Hospital (August 2015 to December 2016) were divided into two groups by random digital table. All the patients were operated under combined spinal-epidural anesthesia. The patients in the control group were treated with sufentanil for patient-controlled analgesia, and the patients in the observation group were treated with hydromorphone hydrochloride for patient-controlled analgesia. 【Results】 The number of pain pump compression at 24 hours after operation in the observation group was less than that in the control group, but there was no significant difference between the observation group and the control group in the number of pain pump compressions and the Ramsay score at 48 hours after operation (P>0.05). VAS scores in the observation group were significantly lower than those in the control group (P<0.05). There was no significant difference in the level of TNF- α, NE and Cor between the two groups before operation (P>0.05). The level of TNF- α, NE and Cor in the observation group was lower than those in the control group at 24 hours and 72 hours after operation. There was statistical significance (P<0.05). There was no significant difference in the adverse reaction rate between the two groups (P>0.05). 【Conclusion】 The analgesic effect of hydromorphone in elderly patients after knee arthroplasty is significant. It can relieve inflammatory injury and stress reaction and is safe.

1672 Study on the Life Quality and Related Influencing Factors in Post-surgical Patients with Hypertensive Intracerebral Hemorrhage in Basal Ganglia

【Objective】 To investigate the quality of life and related influencing factors in post-surgical patients of hypertensive intracerebral hemorrhage in the basal ganglia. 【Methods】 The clinical data of 152 patients with hypertensive cerebral hemorrhage in basal ganglia who underwent surgical treatment in our hospital from January 2014 to November 2016 were retrospectively analyzed. The SF-36 health survey scale (SF-36) was used to assess the patient's quality of life, and to analyze age, gender, marital status, place of residence, occupation, smoking history, drinking history, cerebral hemorrhage, cerebral midline shift, the occurrence of cerebral hernia, operation time after bleeding, state of consciousness , the occurrence of cerebral hemorrhage breaking into the ventricles and other factors affecting on the quality of life. 【Results】 The scores of SF-36 were physical function 15.6±31.5 points, physiological function 37.0±24.3 points, body pain 74.9±17.1 points , overall health 11.6±12.0 points, emotional function 13.5±31.4 points, mental health 51.7±13.9points, activity 46.2±12.5 points, social function 61.8±17.2 points, and total score 312.3±159.9 points. Univariate analysis showed that age, education level, the incidence of living with children, cerebral hemorrhage, cerebral midline shift, cerebral hernia, operation time after hemorrhage, state of consciousness, and the occurrence of cerebral hemorrhage breaking into the ventricles intraventricular due to surgical treatment of hypertensive cerebral hemorrhage in the basal ganglia were the influencing factors on the quality of life in post-surgical patients of hypertensive intracerebral hemorrhage in the basal ganglia. The results of multivariate analysis showed that age, the incidence of living with children, cerebral hemorrhage, cerebral midline shift, and the occurrence of cerebral hemorrhage breaking into the ventricle were independent factors affecting the quality of life in post-surgical patients of hypertensive intracerebral hemorrhage in the basal ganglia.【Conclusion】 Independent factors influencing the quality of life in post-surgical patients of hypertensive intracerebral hemorrhage in the basal ganglia are age, the incidence of living with children, cerebral hemorrhage, cerebral midline shift, and the occurrence of cerebral hemorrhage breaking into the ventricle (intraventricular hemorrhage).

1675 Effect of miR-423-5p Expression on the Proliferation of Ovarian Cancer Cells

【Objective】 To evaluate the expression of miR-423-5p in ovarian cancer tissues and investigate the effect of miR-423-5p on the proliferation of ovarian cancer cells. 【Methods】 The expression of miR-423-5p in 25 pairs of ovarian cancer and adjacent tissue was determined by real-time PCR. The expression of miR-423-5p in four ovarian cancer cell lines (SK-OV-3, A2780, COC1 and OVCAR3) was measured as well. The miR-423-5p-low-expressed cells were transfected with miR-423-5p mimic or NC mimic and the miR-423-5p-high-expressed cells were transfected with miR-423-5p inhibitor or NC inhibitor. Cell proliferation activities were examined by MTT assay. The expression level of ING-4 (growth inhibitory gene 4) was measured by real-time PCR. Dual-Luciferase reporter assay was also used to examine whether or not ING-4 was a direct target of miR-423-5p. 【Results】 The level of miR-423-5p was significantly increased in ovarian cancer tissues compared to cancer-adjacent tissue (P<0.05). A2780 cell showed the lowest level and OVCAR3 cell showed the highest level of miR-423-5p among the four ovarian cancer cell lines. A2780 cells transfected with miR-423-5p mimic significantly increased cell proliferation activity while also remarkably inhibited the expression of ING-4 compared to NC mimic transfected A2780 cells. In contrast, OVCAR3 cells after transfection with miR-423-5p inhibitor significantly inhibited cell proliferation and increased ING-4 level when compared to NC inhibitor transfected cells. Dual-Luciferase reporter assay showed that when compared to wt-ING-4 3'UTR+NC mimic group, the relative luciferase activity was significantly increased in wt-ING-4 3'UTR+miR-423-5p mimic group (P<0.05). However, there were unchanged in mut-ING-4 3'UTR+NC mimic group and mut-ING-4 3'UTR+miR-423-5p mimic group. 【Conclusion】 miR-423-5p is closely related to the occurrence and development of ovarian cancer. It may may be a novel target in the prevention and treatment of ovarian cancer.

1679 Effect of lncRNA BC002811 on Invasion and Metastasis of Gastric Cancer Cells

【Objective】 To detect the expression of lncRNA BC002811 in gastric cancer tissues and cell lines, and to study its function regulating the migration and invasion in gastric cancer cells. 【Methods】 Real-time PCR was used to detect the expression of BC002811 in gastric cancer tissues and cell lines. Gastric cancer cell stable strains with overexpression and down-regulation of lncRNA BC002811were established. Subsequently, the migration and invasion capability of different strains were analyzed with Scratch (wound-healing) test and transwell assay, respectively. Finally, real-time quantitative PCR and Western blot were used to investigate the molecular mechanism of BC002811 to promote tumor progression.【Results】 The relative expression of lncRNA BC002811 was significantly up-regulated in gastric cancer tissues and cell lines compared to corresponding adjacent normal tissues and normal gastric cell line, respectively(P<0.05). When lncRNA BC002811 was over-expressed, the migration and invasion ability of gastric cancer cells were enhanced and the expression of matrix metalloproteinase-2 (MMP2), MMP9 and Vimentin (one of the mesenchymal cell markers) increased. While the expression of E-cadherin (one of the epithelial cell markers) decreased(P<0.05). When the expression of lncRNA BC002811 was inhibited, the opposite results occurred. 【Conclusion】 lncRNA BC002811 promoted the migration and invasion in gastric cancer cells and contributed to the tumor deterioration.

1683 Comparison of Two Antiplatelet Regimens in the Treatment of Clopidogrel Resistant Acute Coronary Syndrome

【Objective】 To investigate the clinical efficacy of different antiplatelet drug therapy in clopidogrel resistant patients with acute coronary syndrome (ACS).【Methods】 From July 2015 to June 2017, 220 ACS patients with clopidogrel resistance were selected. All patients received percutaneous coronary intervention (PCI). They were randomly divided into observation group and control group with 110 cases in each group. The control group was treated with clopidogrel + aspirin + cilostazol and the observation group with tigrilol + aspirin. The clinical efficacy and safety of the two treatments were compared.【Results】 There was no significant difference in platelet inhibition rate induced by adenosine diphosphate (ADP), arachidonic acid (AA) between the two groups (all P>0.05). The platelet inhibition rate of the ADP pathway in the observation group was significantly lower than that in the control group at 1 and 6 months postoperatively (P<0.05). The incidence of cardiovascular death, myocardial infarction and recurrent angina pectoris in the observation group was lower than that in the control group, and the incidence of thrombus in stent was higher than that in the control group, but there was no significant difference between the two groups (P>0.05). There was no significant difference between the two groups in the occurrence of severe bleeding, slight hemorrhage, rash, gastrointestinal reaction, thrombocytopenia and dyspnea (P>0.05).【Conclusion】 The therapy of ACS patients with clopidogrel resistance by aspirin, cilostazol and clopidogrel, or tigrilol as a replacement of clopidogrel has good clinical efficacy and good safety.

1686 Curative Efficacy of Minimally Invasive Intracranial Pressure Monitoring Probe in Implantation of Heavy Head Trauma

【Objective】 To study the curative efficacy of minimally invasive intracranial pressure monitoring probe in implantation of severe craniocerebral trauma and its significance. 【Methods】 The clinical data of 40 patients with severe craniocerebral trauma treated in our hospital were retrospectively analyzed. According to the different monitoring methods of ICP, It was divided into observation group (with minimally invasive implant probe) and control group (routine general anesthesia borehole implantation monitoring probe) with 20 cases in each group.According to the monitoring results, the patients were given corresponding treatment and followed up for a period of one year after the treatment. Glasgow prognosis score (GOS), NIHSS, complications and death were compared between the two groups. 【Results】 In the observation group, the operation time and intraoperative bleeding volume were (13.47 ±3.58) min, (65.21 ±6.11) mL,significantly lower than those in the control group (36.68 ±2.62) min, (97.43±12.63) mL, the difference was statistically significant (P<0.05). Before treatment, there was no significant difference in GOS,NIHSS score between the two groups (P>0.05). After treatment, the GOS score of the two groups was higher than that before treatment ,and the observation group was higher than the control group, the difference was statistically significant (P<0.05). The NIHSS score of the two groups was lower than that before treatment(P<0.05). The NIHSS score in the observation group was significantly lower than that in the control group,the difference was statistically significant (P<0.05).The incidence of complications in the observation group was 25.00% (5 / 20), which was significantly lower than that in the control group (60.00%,12 / 20). The difference was statistically significant(χ2=5.013,P=0.025<0.05). The total mortality of patients in the observation group was 20.00% (4 / 20) which was significantly lower than that in the control group (50.00% 10 / 20), and the difference was statistically significant(χ2=6.241,P=0.013<0.05).【Conclusion】 In the treatment of severe craniocerebral trauma patients, the microinvasive intracranial pressure probe implantation can also accurately monitor the intracranial pressure in patients, and has the advantages of reducing operation time, reducing trauma, simple operation and fewer complications, etc. It can also improve the prognosis of patients and reduce the mortality rate, which is worthy of clinical application.

1690 Effect of Ischemic post-conditioning on Cardio-renal Function in Patients with Myocardial infarction (STEMI) after PCI

【Objective】 To investigate and discuss the effect of ischemic post-conditioning on the cardio-renal function in patients with STEMI (ST-segment elevation myocardial infarction) after PCI (percutaneous coronary intervention). 【Methods】 A total of 106 patients with myocardial infarction from Jan 2014 to Jan 2016 in our hospital were randomly divided into the observation group (53 cases) and the control group (53 cases). Patients in the control group received PCI procedure directly.The observation group was treated with ischemic post-conditioning before performing PCI. The cardio-renal function levels and rate of major adverse cardiovascular events were compared in the two groups. 【Results】 The rate of electrocardiogram ST-segment decline immediately after the procedure was significantly higher in the observation group than in the control group (P<0.05). WMIS, MIS and the incidence of reperfusion arrhythmia during the procedure were significantly lower in the observation group than in the control group (P<0.05).The rate of contrast-induced renal impairment (CI-AKI) serum creatinine value 48 hours after the procedure, and the rate of one-year major adverse cardiovascular events (MACE) were significantly lower in the observation group than in the control group (P<0.05). 【Conclusion】 Ischemic post-conditioning can effectively alleviate myocardial ischemia-reperfusion injury after PCI in patients with STEMI, reduce the incidence of postoperative contrast nephropathy and adverse cardiovascular events, and have a good protective effect on the heart and kidney function of patients.

1693 Application of Traditional Chinese Medicine Foot Bath in Peripheral Neuropathy Caused by Velcade Multiple Myeloma Patients

【Objective】 To observe the clinical effect of the traditional Chinese medicine foot bath on the Peripheral Neuropathy (PN) caused by velcade (Trade name: Wanke) in multiple myeloma (MM) patients. 【Methods】 Sixty cases of MM patients with PN caused by velcade were divided into control group and intervention group with 30 cases in each group by the random number table method. The control group was given with routine intervention and intramuscular injection of adenosine cobalt ammonium,(1.5mg/d,qd). The intervention group was treated with traditional Chinese medicine foot bath based on the common nursing therapy of the control group. The significant efficiency, the total efficiency ,the motor nerve conduction velocity (MNCV) of the common peroneal nerve and tibial nerve, the sensory nerve conduction velocity (SNCV) of superficial peroneal nerve were compared between the two groups before and after treatment. 【Results】 After treatment, the total effective rate in the intervention group was significantly higher than that in the control group (P<0.05). The MNCV of common peroneal nerve and tibial nerve and the SNCV of superficial peroneal nerve in both groups were significantly higher than those before treatment (P<0.05), and the intervention group was significantly higher than the control group, and the difference was statistically significant (P<0.05).【Conclusion】 Chinese medicine foot bath has good curative effect on PN of MM patients caused by velcade (Wanke), and can improve nerve conduction velocity.

1696 Significance of HIF-1α, VEGF and Bcl-2 Expression in Cervical Carcinoma

【Objective】 To explore the expression and significance of hypoxia inducible factor-1α (HIF-1α), vascular endothelial growth factor (VEGF) and B-cell lymphoma-2 (Bcl-2) in cervical carcinoma. 【Methods】 The expression of HIF-1α, VEGF and Bcl-2 in 82 cases of cervical carcinoma and normal cervical tissues was detected by immunohistochemical method. The relationship between the three markers (HIF-1α, VEGF and Bcl-2) and clinical pathological parameters was analyzed. 【Results】 The positive rate of HIF-1α in cervical carcinoma 80.48% (66/82) was higher than that in normal cervical tissues 2.43% (2/82) (P<0.05). The positive rate of VEGF in cervical carcinoma 82.93% (68/82) was higher than that in normal cervical tissues 3.65% (3/82) (P<0.05). The positive rate of Bcl-2 in cervical carcinoma 73.17% (60/82) was higher than that in normal cervical tissues 30.48% (25/82) (P<0.05). The positive expression rates of HIF-1α, and VEGF in cervical carcinoma with differential degree, clinical stage and lymph node metastasis were statistically significant (P<0.05), but not correlated with age, gender and lesion size. The positive expression rate of Bcl-2 wasn't related to those clinical parameters (differential degree, clinical stage, lymph node metastasis, age, gender and lesion size) (P>0.05).According to Pearson correlation analysis, HIF-1α expression was positively correlated with the expression of VEGF and Bcl-2 in cervical cancer tissues (r=0.685, P=0.010; r=0.532, P=0.025, respectively); VEGF was positively correlated with Bcl-2 expression (r=0.584, P=0.033).【Conclusion】 The high expression of HIF-1α, VEGF and Bcl-2 in cervical carcinoma can be used as reference indicators to determine the occurrence and lymph node metastasis of cervical carcinoma.

1699 Expression of CACUL1 in Breast Cancer and Its Correlation with Psychosocial Factors

【Objective】 To analyze the expression of CACUL1 in breast cancer and its correlation with psychosocial factors. 【Methods】 A total of 169 patients with breast cancer who underwent radical mastectomy in our hospital from June 2014 to March 2018 were enrolled for the study. The expression level of CACUL1 was compared between cancer tissues and adjacent tissues through quantitative real-time polymerase chain reaction (qPCR), Western blot and immunohistochemical staining (IHC). According to the result of IHC, patients were divided into the high expression group (n=103) and the low expression group (n=66). Baseline data, Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale-17 (HAMD-17), Social Support Rating Scale (SSRS), Eysenck Personality Questionnaire (EPQ) and FACT-B were compared between two groups. 【Results】 ⑴The CACUL1 expression of mRNA (2.6±0.8 vs. 1.0±0.1,P<0.001), protein (1.9±0.5 vs. 1.0±0.2,P<0.001) and IHC score (55.6±20.9 vs. 18.6±7.2,P<0.001) in breast cancer tissues were significantly higher than those in adjacent tissues. ⑵Age, monthly income < 5000 yuan and the proportion of stage III-IV in the high expression group were significantly higher than those in low expression group (P<0.05). ⑶The scores of HAMA, HAMD-17, neuroticism and psychoticism of EPQ, social/family status, emotional status and total scores of FACT-B in the high expression group were significantly higher than those in the low expression group, and the scores of subjective support, utilization of social support and total score of SSRS were significantly lower than those in the low expression group (P<0.05). 【Conclusion】 The expression level of CACUL1 in breast cancer tissues is significantly higher than that in adjacent tissues, and it is related to psychosocial factors. Patients with high expression of CACUL1 in breast cancer tissues have more negative emotions, lower social support level, lower quality of life and worse tumor staging.

1703 Changes of Fear Conditioning Memory and Morphology of Dendritic Spines in Hippocampal Neurons in the Chronic Phase of Mice with Temporal Lope Epilepsy

【Objective】 To investigate the changes of fear conditioning memory and the morphological changes of dendritic spines in hippocampal neurons in the chronic phase of mice with temporal lobe epilepsy.【Methods】 Temporal lobe epilepsy models were induced by intraperitoneal injection of Kainic acid (KA) in mice. The mice were divided into two groups: KA group (n=8) which received KA and the control group (CN,n=4) which were intraperitoneal injection of saline instead. After two months, emotional learning and memory were evaluated by fear-conditioning test, and changes in dendritic spines of hippocampal neuron in both groups of mice were observed by Golgi Staining. 【Results】 In the contextual test, the percentage of connective fear memory freezing time in the KA group was (17.50±0.88)%, which was significantly lower than that in the CN group (60.24±3.01)%, and the difference was statistically significant (P<0.05). In the tone cued test, the percentage of freezing time in the sound-connected fear memory of the KA group was (49.01±2.45)%, which was significantly lower than that of the CN group (79.57±3.98)%, and the difference was statistically significant (P<0.05). Compared to the control group, Golgi staining showed that the density of dendritic spines in hippocampal neurons in KA group was significantly reduced. The results of image analysis showed that the dendritic spine length of hippocampal neurons and the density of dendritic spines of hippocampal neurons in KA group were (2.1±0.11) μm, (10±0.5)/10 μm, respectively, which were significantly lower than that of CN group. (2.5±0.13) μm, (12±0.6)/10 μm, respectively, and the differences were statistically significant (P<0.05).【Conclusion】 The mice with KA-induced temporal lope epilepsy in chronic phase have the impaired fear conditioning memory, which may be associated with the morphological changes of of dendritic spines in hippocampal neurons.

【Objective】 To evaluate the value of SYNTAX score for evaluation in disease risks and short-term prognosis in acute coronary syndrome (ACS) patients.【Methods】 From January 2014 to January 2018, 320 patients with ACS were recruited. According to the GRACE score, the patients with ACS were stratified at risk.Finally, 70 cases of low, medium and high risk patients were included, a total of 210 cases were studied. All patients were treated with PCI after admission. SYNTAX scores were calculated according to the results of intraoperative coronary angiography. The MACE was recorded during hospitalization. The differences of SYNTAX scores in patients with different risk stratification were compared, and the correlation between GRACE scores and SYNTAX scores was analyzed . At the same time, (ROC) was used to evaluate the value of SYNTAX score as a risk stratification evaluation index, and the relationship between SYNTAX score and MACE during hospital treatment was analyzed.【Results】 The SYNTAX score of the high risk group was higher than that of the low and moderate risk groups, (11.09 ±2.69), (21.58 ±5.34) and (24.89 ±6.61). Pearson correlation analysis showed that there was a positive correlation between Grace Score and SYNTAX score (r=0.621). In addition, the area under the curve (AUC)(95%CI)of ROC for identifying high-risk ACS patients with SYNTAX score was 0.82(0.76~0.87). At the same time, the level of SYNTAX score in the MACE group was significantly higher than that in the non- MACE group (28.26 ±4.72) and (15.35 ±6.11). The difference was statistically significant.【Conclusions】 SYNTAX score as a marker for assessing risk stratification and predicting short-term prognosis in patients with ACS is of great clinical significance.

1709 Relationship between IgM Deposition and Pathological Changes of Urinary Protein and Kidney in Adult IgAN Patients

【Objective】 To investigate the relationship between the immunoglobulin M (IgM) deposition and the level of urinary protein and renal pathological changes in adult patients with IgA nephropathy (IgAN). 【Methods】 A total of 86 patients with IgAN who were admitted to our hospital from January 2012 to January 2017 were selected for the detection of IgM deposition by direct immunofluorescence. Patients were divided into the IgM positive group (n=40) and the IgM negative group(n=46). Total urinary protein, urinary protein excreation rate(UAER), urine β2 microglobulin (β2-MG), urinary α1 microglobulin (α1-MG), and Katafuchi were analyzed. 【Results】 The total urinary protein, UAER, and urine β2-MG in the IgM-positive group were higher than those in the IgM-negative group (P<0.05). There was no significant difference in urinary α1-MG levels between the two groups (P>0.05). Glomerular score, tubulointerstitial score, and total score in the IgM positive group were higher than those in the IgM negative group (P<0.05). There was no significant difference in renal vascular scores between the IgM positive group and the IgM negative group (P>0.05) Also, there was no significant difference in the Lee's grade between the IgM positive group and the IgM negative group (Z=-1.471, P=0.141>0.05).【Conclusion】 IgM deposition in patients with IgAN can promote further damage to renal function and aggravate the pathological damage of the kidney.

1712 Application of Disposcope and Shikani Optical Stylet in Tracheal Intubation of General Anesthesia for Elderly Hypertensive Patients

【Objective】 To comparatively analyze the application of disposcope and shikani optical stylet in tracheal intubation of general anesthesia for elderly hypertensive patients. 【Methods】 A total of 84 elderly hypertensive patients who planned to be treated with tracheal intubation general anesthesia were randomly divided into group A (disposcope) and group B (shikani optical stylet), with 42 cases in each group. Levels of HR, DBP, SBP, plasma cortisol (COR) and endothelin (ET) were compared between the two groups before anesthesia (T0), after induction of anesthesia (T1), immediately after intubation (T2) and 2min after intubation (T3). The success rate of tracheal intubation, intubation time and incidence of complications were compared between the two groups. 【Results】 There were no significant differences in the levels of HR, DBP, SBP, plasma COR and ET between the two groups at different time points (P>0.05). There were no significant differences in the success rates and failure rates of one-time tracheal intubation between group A and group B [90.48% (38/42) and 2.38% (1/42) vs 92.86% (39/42) and 0.00% (0/42)](P>0.05). Also, there was no significant difference between the two groups in the average intubation time (P>0.05). No complications such as snoring or sore throat happened in both groups. 【Conclusion】 The effect of disposcope tracheal intubation is similar to that of shikani optical stylet in elderly hypertensive patients under general anesthesia. They both can effectively maintain the stability of hemodynamics with mild stress response, and provide high success rate of one-time intubation and safety.

1715 Clinical Effect of First-stage Single-channel Percutaneous Nephroscope Combined with Soft Transurethral Ureteroscope for the Treatment of Antler-type Renal Calculi

【Objective】 To explore the clinical curative effect of first-stage single-channel percutaneous nephroscope(PCNL) combined with soft transurethral ureteroscope for the treatment of antler-type renal calculi. 【Methods】 A total of 186 patients with antler-type renal calculi were randomly divided into two groups, with 93 cases each. Patients in the control group received traditional PCNL while patients in the observation group received first-stage single-channel PCNL combined with soft transurethral ureteroscope treatment. The operation status, the rate of calculi clearance, and the rate of postoperative renal vascular superselective embolization after surgery were compared between the two groups.【Results】 The rate of calculi clearance was 98% (91/93) in the observation group, which was higher than in control group, which was 85% (79/93), and the difference was statistically significant(P<0.05). The rate of postoperative renal -vascular superselective embolization was 2.15% (2/93) in the observation group, which was significant lower than that in control group, which was 6.45% (6/93)(P<0.05). The operation time, the amount of the bleeding during operation and the time of hospitalization after operation were significant less in the observation group were than in the control group. The difference between the two groups was statistically significant(P<0.05). 【Conclusion】 First-stage single-channel PCNL combined with soft transurethral ureteroscopy for the treatment of antler-type renal calculi has the advantage of high calculi clearance rate, less bleeding, and a shorter hospitalization after operation.It was worthy of clinical application.

1717 Value of Color Doppler Ultrasonography and MRI in the Diagnosis of Placental Implantation

【Objective】 To evaluate the diagnostic value of color Doppler ultrasound (CDUs) and magnetic resonance imaging (MRI) in placenta accreta.【Methods】 From July 2012 to June 2017, the data of color Doppler ultrasound and MRI of 100 cases clinically diagnosed as placenta accreta in Heze second people's Hospital were analyzed retrospectively. The results were compared with the clinical and pathological findings to analyze the diagnostic value of two methods in placenta accreta.【Results】 The clinical and pathological findings showed 64 cases of placenta accreta, 28 cases of posterior wall implantation and 36 cases of anterior wall implantation ; 36 cases were non-placental accreta. 86 cases were diagnosed by color Doppler ultrasound (see figure 1a), 4 cases were misdiagnosed . The kappa values of the two kinds of examinations compared with the clinical and pathological results were 0.707 and 0.733 ,which were greater than 0.7, respectively. The two kinds of examinations were highly consistent with the pathological diagnosis. The sensitivity and specificity of color Doppler ultrasound were 84.38% (54/64) 88.89% (32/36) respectively, and there was no significant difference compared with MRI 93.75% (60/64) 77.78% (28/36) (P>0.05). In the placenta accreta of anterior wall, color Doppler ultrasound showed abundant blood flow signal after placenta, the proportion was 83.33%, significantly more than 61.11% of MRI , the difference was statistically significant (P<0.05).In the posterior wall placenta accreta, MRI showed that the posterior placental space disappeared, and undefined ratio of placenta to uterus was 92.86% and 85.71%, which was significantly higher than that of 67.86% and 60.71 % of color Doppler ultrasound,the difference was statistically significant (P<0.05).【Conclusion】 The diagnostic efficacy of color Doppler ultrasound and MRI in placenta accreta is the same. Color Doppler ultrasound has advantages in the diagnosis of anterior wall placenta accrete ,and MRI is superior to color ultrasound in the diagnosis of posterior wall placenta implantation. Therefore, MRI can be used as an important supplement of B ultrasound screening and color ultrasound reexamination.

1720 Predictive Value of Serum PCT and CRP Levels for Anastomotic Leakage of Rectal Cancer after Laparoscopic Total Mesenteric Resection

【Objective】 To investigate the value of combined detection of serum PCT (procalcitonin) and CRP (C-reactive protein) in predicting anastomotic leakage of rectal cancer after laparoscopic total mesenteric resection. 【Methods】 The clinical data of 328 patients with rectal cancer who underwent laparoscopic total mesenteric resection in our hospital from April 2013 to March 2017 were analyzed retrospectively. Patients were divided into anastomotic leakage group (n=28) and non-anastomotic leakage (n=300) according to the occurrence of anastomotic leakage. The levels of serum procalcitonin (PCT) and C-reactive protein (CRP) at 1 day, 3 days, and 5 days after surgery were compared between the two groups. The ROC curves of anastomotic leakage predicted by PCT and CRP at different time points were drawn, and the sensitivity and specificity of PCT combined with CRP for predicting anastomotic leakage were evaluated. 【Results】 The serum level of PCT in the anastomotic leakage group was higher than that of the non-anastomotic leakage group at 1 d, 3 d, and 5 d after operation (P<0.05). There were no significant differences in serum CRP levels between the two groups at 1 and 3 days after surgery (P>0.05). However, the serum level of CRP was higher in the anastomotic leakage group than in the non-anastomotic leakage group at 5 days after operation (P<0.05). Serum levels of PCT at 1 day, 3 days, 5 days after operation and level of CRP at 5 days after operation had significantly predictive value for anastomotic leakage . The critical values were 130.50 pg/ml and 111.50 pg/ml, 96.25 pg/ml, and 37.00 mg/L, respectively.. The sensitivity of PCT combined with CRP for predicting anastomotic leakage was 95.83% and specificity was 75%.【Conclusion】 Postoperative detection of serum PCT and CRP has great predictive value for the occurrence of anastomotic leakage after laparoscopic rectal cancer total mesenteric resection. The combined detection of PCT and CRP can obtain higher sensitivity and specificity.

1723 Clinical Effect of Lumbar Cistern Drainage Combined with 3H therapy in the Subarachnoid Hemorrhage after Aneurysm Clipping Operation and its Effect on Vasoactive Factors

【Objective】 To study the clinical effect of lumbar cistern drainage combined with hypertension, high blood volume and hemodilution (3H) on subarachnoid hemorrhage after aneurysm clipping operation and its effect on vasoactive factors. 【Methods】 From May 2016 to May 2017, 116 patients with subarachnoid hemorrhage were randomly divided into two groups: the control group (n = 58) and the observation group (n=58). The control group was treated with lumbar cistern drainage after aneurysm clipping. The observation group combined with 3 H therapy on the basis of the control group. The therapeutic effects, postoperative complications, serum C-reactive protein (CRP) and S100 levels, plasma diastolic factor (NO), contraction factor (ET-1), thromboxane B2 (TXB2) and human 6-keto-prostacyclin (6-K-PGF1α) levels were compared between the two groups.【Results】 The effective rate of treatment in the observation group was 93.1%, which was significantly higher than that in the control group 70.4%(P<0.05); the headache score and cerebral vasospasm in the observation group were significantly lower than those in the control group (P<0.05), and the craniocerebral hydrocephalus and mortality were significantly lower than those in the control group (P<0.05). Before intervention, there was no significant difference in serum CRP,S100 level and plasma NO,ET-1,TXB2 and 6-K-PGF1α levels between the two groups (P>0.05), and after intervention, the indexes in the observation group were significantly lower than those in the control group (P<0.05).【Conclusion】 The treatment of subarachnoid hemorrhage by lumbar cistern drainage combined with 3H after aneurysm clipping is safe and reliable. It can reduce complications and ensure survival rate.

1727 Correlation of DNA Methyltransferase 1 Gene Polymorphism with Colon Cancer Susceptibility Related Factors

【Objective】 To investigate the interaction of DNA methyltransferase 1 (DNMT1) gene polymorphism with colon cancer susceptibility related factors. 【Methods】 A total of 110 patients with colon cancer treated in Department of Oncology of our hospital from February 2013 to December 2017 were selected as the observation group, and 50 healthy volunteers in the same time period were selected as the control group. A self-made questionnaire on epidemiology of colon cancer was used to investigate the basic information, history of previous digestive tract diseases and dietary habits. The polymorphism of s16999593 was detected by fluorescence quantitative PCR. The differences of genetic polymorphisms and related factors in two groups, as well as correlation with colon cancer susceptibility were analyzed. 【Results】 In the observation group and the control group, there were significant differences in the aspects of intestinal polyps, colitis, cholecystitis, drinking, intake of fresh fruits and vegetables, and intake of pickled, spicy and greasy foods (P<0.05). Individuals carrying at least one C allele (CT+CC) were 2.250 times more likely to develop colon cancer than individuals with wild homozygous TT genotypes (95% CI 1.105 to 4.581, P=0.024). Polymorphism of s16999593 was positively correlated with intestinal polyps, colitis, cholecystitis, drinking, less intake of fresh fruits and vegetables, and more intake of pickled, spicy and greasy foods (P<0.05). 【Conclusion】 Individuals carrying at least one C allele of the s16999593 locus have an increased risk of colon cancer. S16999593 polymorphism is positively correlated with intestinal polyps, colitis, cholecystitis, drinking, and frequent intake of pickled, spicy, greasy foods, which increases the risk of colon cancer. It is negatively correlated with more intake of fresh fruits and vegetables, which reduces the risk of colon cancer.
