Effects of Long-term Use of Adefovir Dipivoxil on Renal Functions in Patients of Chronic Hepatitis B

【Objective】To investigate the effects of long-term use of adefovir dipivoxil (ADV) on renal functions in patients of chronic hepatitis B (CHB).【Methods】A total of 317 patients with CHB who were treated with ADV (10 mg/d) alone or combined with oral administration of for more than 24 months were selected and divided into kidney injury group (118 cases)and non renal injury group (199 cases)according to the renal function damage.Serological, virological and renal function tests in the past two years were analyzed. By χ2 test, Binary Logistic Regression Analysis, ROC curve analysis and other statistical methods, the effects of elements including the gender, age, time of use ADV, ADV combined with other antiviral drugs, HBsAg, HBeAg, HBV, DNA, ALT, GLU and other factors on early renal injury were accordingly investigated.【Results】Chi-square test showed that: there was significant difference in age and ADV medication time, but there was no significant difference between the other factors; Binary Logistics Regression Analysis showed that: age and ADV medication time had significant effect on the dependent variable of early renal damage; ROC curve analysis showed that: age and ADV duration had a significant corresponding sensitivity on early renal injury, the relevant Youden's index was 0.616, 0.422.【Conclusion】Age and treatment time of ADV are risk factors of renal damage incidents; the age of 47.5 years old can be used as a critical reference year of kidney damage events; the time of ADV medication is 3.175 years, which can be used as the critical reference life events of early renal injury occurred.

Expression of IL-1 β, GSTM1, hMLH1 in Gastric Carcinoma and its Relationship with Clinical Pathology

【Objective】To study the expression of IL-1β, GSTM1 and hMLH1 in gastric carcinoma and its relationship with clinical pathology.【Methods】Two hundred patients with gastric carcinoma (gastric carcinoma group) and 200 patients with benign gastric lesions (control group) were enrolled in the study. The expression of IL-1β, GSTM1 and hMLH1 was detected, and its relationship with clinical pathology was analyzed.【Results】The rates of IL-1β(T/T), GSTM1 deletion and low expression of HhMLH1 in the gastric carcinoma group were significantly higher than those in the control group (P<0.05). Age>60 years old, tumor diameter <5cm and low tumor differentiation were strongly correlated with high expression rate of IL-1 βT/T genotype. Male and age >60 years old were much correlated with high rate of GSTM1 deletion. Tumor location and TNM stage Ⅲ~Ⅳwere highly correlated with low expression of hMLH1. Logistic regression analysis showed that IL-1β(T/T) (OR=3.124), GSTM1 deletion (OR=4.576) and low expression of hMLH1 (OR=7.534) were risks for the 5-years survival rate of patients with gastric carcinoma, and there was synergistic effect among the three genes (P<0.05, OR>1).【Conclusion】Expression of IL-1 βT/T genotype, GSTM1 deletion and low expression of hMLH1 are closely related to the development of gastric carcinoma. There is synergistic effect among the three genes. Carrying genes with interactions will increase the risk of gastric carcinoma and death.

Preliminary Study on HCC EMT Transformation and Drug Resistance Induced by Hypoxia

【Objective】To observe the effect of hypoxia on invasion and metastasis of hepatocellular carcinoma (HCC) cells and to investigate the mechanism of drug resistance in HCC cells induced by hypoxia.【Methods】HCC hypoxia model was established by using HepG2 and SMMC-7721 cells. The morphological changes were observed by inverted microscope; invasion and metastasis of tumor cells in vitro were detected by Transwell migration and invasion assays; the cell activity was tested by MTT assay. Epithelial mesenchymal transition (EMT) related protein including E-cadherin, Vinmentin, N-cadherin and the expression of tumor drug resistance gene ABCG2 were detected by Western blotting. Apoptosis and cell cycle changes were detected by flow cytometry.【Results】HCC cells transformed from epithelial like phenotype to mesenchymal cell like phenotype after 48 h of hypoxia culture; the expression of E-cadherin decreased significantly and the expression of Vimentin and N-cadherin increased significantly under hypoxia (P<0.05); at the same time, the migration and invasion ability of HCC cells increased significantly under hypoxia (P<0.05); hypoxia can lead to a significant increase in the number of HepG2 and SMMC-7721 arrested in quiescent phase (G

); under hypoxia, the resistance of HCC cells to cisplatin was significantly increased (P<0.05); the expression of ABCG2 protein was increased under hypoxic conditions (P<0.05).【Conclusion】Hypoxia induces HCC EMT transformation and causes invasion and metastasis. At the same time, hypoxia may also induce resistance to cisplatin.

Effect of Different Puncture Protocols on Diagnosis, Complications and Pain of Rectal Cancer Guided by Ultrasound

【Objective】To compare transrectal ultrasound (transrectal ultrasound, TRUS) - guided transperineal prostate biopsy (TRUS-TP) and transrectal prostate biopsy (TRUS-TR) for prostate cancer diagnosis rate, complications and pain.【Methods】A total of 186 patients with suspicious prostate cancer were randomly divided into TRUS-TP group and TRUS-TR group, and the diagnosis rate, puncture, pathogens distributiont of the two methods were compared.【Results】The positive rate of TRUS-TP group was 37.63% (35/93), compared with TRUS-TR group 35.48% (33/93), the difference was not statistically significant (P>0.05); the postoperative mild pain in the group TRUS-TP was higher than that in the group TRUS-TR, and the incidence of mild rectal bleeding was lower than that in the group TRUS-TR, the difference was statistically significant (P<0.05), but there is no significant difference in other indexes (P>0.05); the incidence of bacteriuria and bacterial blood in the group TRUS-TP was significantly lower than that in the group TRUS-TR (P<0.05). There was no significant difference in the incidence of fever, perineal pain and lower urinary tract symptoms between the two groups (P>0.05); there was no significant difference between the two groups of pathogenic bacteria (P>0.05).【Conclusion】There was no difference between the two methods of TRUS-TP and TRUS-TR in the diagnosis of prostate cancer. Although TRUS-TP has higher pain level after puncture, it has the advantages of less complications and lower infection rate, which is worthy of clinical application.

Effect of siRNA Silencing VDAC1 on Proliferation and Cell Cycle of MCF-7 Cells and Its mechanism

Effect of Oxycodone Combined with Dexmedetomidine in General Anesthesia on Anesthesia Related Indexes in Patients with Increased Serum Direct Bilirubin and Alkaline Phosphatase

【Objective】To investigate the effect of combined use of oxycodone and dexmedetomidine (Dex) in general anesthesia on anesthesia related indexes in patients with increased serum direct bilirubin (DBIL) and alkaline phosphatase (ALP).【Methods】Sixty cases of patients with increased DBIL and ALP caused by obstructive jaundice or liver cell jaundice were selelcted, and the patients were divided into control group (M group) and oxycodone combined with Dex group (Q group). The M group received combined inhalation and intravenous anesthesia while the Q group were treated with intravenous infusion of oxycodone 4mg at 10min before induction of anesthesia and infusion of Dex 0.5 μg/(kg·h) till 25 min before skin closure. The anesthesia related situation between the two groups was compared.【Results】There was no significant difference in extubation time between the two groups (P>0.05), and the total amount of propofol and remifentanil in the group Q was less than that in the group M

Establishment of Reference Intervals of Plasma D-dimer in Women During Third Trimester of Pregnancy in Hunan

【Objective】To establish reference intervals for D-dimer in women of late pregnancy in Hunan area.【Methods】Six hundred and seventy-eight healthy singleton pregnancy women, 418 healthy twins or multiple pregnancy women(≥28 weeks)and 208 healthy non-pregnant women received Physical Examination in the hospital from January 2014 to March 2016 were selected. Automatic biochemical analyzer was used to detect the concentration of plasma D-dimer(D-dimer unit, DDU) by immune turbidimetric method.【Results】The plasma levels of D-dimer in third trimester of pregnancy were higher than those of non-pregnant women(P<0.05), the plasma D-dimer level of twins or multiple pregnancy women was significantly higher than that of the singleton pregnancy women in the same period of time (P<0.05). Late pregnant women plasma D- dimer ( D- dimer units, DDU) reference interval: single pregnancy group: 28~35 weeks <3.64 μg/mL; 36~39 weeks<6.12 μg/mL; 40~41weeks<6.84 μg/mL; twins /multiple pregnancy group: 28~31 weeks<6.26 μg/mL; 32~35 weeks<7.13 μg/mL; 36~39 weeks<8.00 μg/mL.【Conclusion】This study preliminarily established the reference interval of plasma D-dimer by immune turbidimetric method in late pregnancy women in Hunan Province, which is suitable for reference in the whole province.

Curative Effect Evaluation of Short-wave Ultraviolet Ray Therapy Apparatus for Oral Mucositis of Acute Leukemia Patients

【Objective】To explore the curative effect evaluation of short-wave ultraviolet ray therapy for oral mucositis(OM) with acute leukemia patients.【Methods】Acute leukemia patients with oral mucositis were divided into the observation group and the control group according to the order of admission. Patients in the control group were treated by conventional methods, while patients in the observation group received irradiation treatment by ZYY - 9 enhanced short-wave ultraviolet ray therapy apparatus in addition to conventional treatment. The therapeutic effect for OM in the two groups was observed within 14 days.【Results】The recovery rate of the observation group was 89.3% which was obviously higher than that of the control group (64.3%) within 14 days, and the difference was statistically significant (P<0.001). In the observation group, the average time required for the disappearance of pain, the 50% reduction in ulcer area of OM and oral mucositis healing time were significantly shorter than those in the control group with statistically significant difference (P<0.001).【Conclusion】Short-wave ultraviolet irradiation treatment can obviously improve the recovery rate of OM and shorten its healing time. The instrument is convenient and safe to use with broad clinical popularization and application.

Adverse effects of PM2.5 on HaCaT Cells and Relevant Mechanisms

【Objective】To explore the effects of PM2.5 on HaCaT cells and whether or not PDTC will alleviate the adverse effects caused by PM2.5.【Methods】HaCaT cells were co-cultured with PM2.5, PDTC, PM2.5+PDTC for 24 hours, respectively. HaCaT cells without any treatment were set as control. The content of malondialdehyde (MDA), reactive oxygen species (ROS) level, the activities of superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px)_ and the expression of NF-kB in the cells from different groups were measured and compared.【Results】Cell viability decreased with the increasing of PM2.5 concentrations, the cell viabilities of 200, 400 and 800 μg/ml groups were significantly lower than the normal group (P<0.05). Compared with PM2.5 group, the MDA contents, the level of ROS and the expression of NF-kB of the cells treated with PM2.5+PDTC, or PDTC, or vehicle treated cells were significantly lower while the activities of SOD and GSH-Px were significantly higher (P<0.05). Furthermore, the levels of MDA, ROS and NF-κB p65 protein in PM2.5 + PDTC group were significantly higher than those in control group and PDTC group【Conclusion】PM2.5 was able to decrease the cell viability and cause oxidative damage to HaCaT cells while PDTC can alleviate the damages to certain extent.

Finite Element Analysis of the Sagittal Axial Dynamic Pedicle Screw and Rod System for Unstable Lumbar

instability model to establish the single-segment dynamic fixation model and the single-segment strong fixation model. Under the constraints of L5 vertebral endplate and under axial 500N load and 10 N·m torque, five working conditions of normal physiological axial load, flexion, extension, lateral bending and rotation of the human lumbar were simulated respectively. adjacent segment activity, the intervertebral disc stress, the stress of the adjacent facet joints and the stress of the instrument were analyzed and compared.【Results】The axial dynamic pedicle screw rod system fixation of unstable lumbar vertebrae can make the motion range of the fixed segment of intervertebral joint have a proper range of motion, which was larger than that of the strong fixation group, less than the complete spinal group. The effect of the model on the lumbar motion of the adjacent segments was also less than that of the rigid fixation group. At the same time, the stress of the fixed segment intervertebral disc was greater than that of the rigid fixation group, but the stress of the instrument was less than that of the strong pedicle screw rod system, while the stress of the adjacent segment of lumbar intervertebral disc and facet joint was smaller than that of the rigid fixation group.【Conclusion】The application of dynamic pedicle screw rod system in fixation of unstable lumbar spine not only retains the lumbar fixation segments appropriate activity, and but also provides immediate stability. At the same time, the system bears the load with the spinal column, which can effectively avoid the complications of internal fixation failure, caused by stress shielding and stress concentration of the instrument. It can also reduce the stress of intervertebral disc and intervertebral joint in the adjacent segment. Compared with the rigid fixation, its influence on adjacent segment lumbar activity is rather small, which theoretically, is beneficial to reduce and prevent adjacent segment degeneration.

Effect of Human Urinary Kallikrein in Treatment of Acute Cerebral Infarction and its Effects on Brain Metabolites

【Objective】To evaluate the effect of human kallidinogenase(HUK) on the changes of brain metabolites in patients with acute ischemic cerebral infarction (AIS) by magnetic resonance spectroscopy.【Methods】Fifty-seven patients diagnosed as AIS by MRI in 72 h were randomly divided into two groups. The control group was given routine basic therapy and symptomatic supportive care. The observation group was treated with HUK on the basis of the control group. The levels of N-acetyl aspartate (NAA) and creatine (Cr) in the brain were measured by magnetic resonance spectroscopy (MRS) before and after treatment. The NIHSS was performed by the National Institutes of Health, Improved Rankin scale (mRs) score, Barthel index (BI) to evaluate clinical efficacy.【Results】The values of NAA, NAA /Cr in peripheral infarcted area, NIHSS score and mRS were not significantly different between the two groups (P> 0.05) before the treatment. One week after treatment, the NAA and NAA/Cr in peripheral infarcted area of the two groups were higher than those before treatment (P<0.05).In the observation group, the increase of NAA and NAA/Cr in peripheral infarcted area was higher than that in the control group (P<0.05).In regard to the NIHSS scores of the two groups after treatment, the mRS score decreased and the BI index increased compared with those before treatment(P<0.05), and the improvement degree of the observation group was better than that of the control group (P<0.05).【Conclusion】HUK treatment can improve the metabolism and clinical function of the brain cells around the infarcted area in AIS patients, and protect the nerve cells.

Evaluation of Synovial Evidence in the Diagnosis of Early Rheumatoid Arthritis by Ultrasound

【Objective】To investigate the value of ultrasonography in the early diagnosis of RA in 2010 ACR/EULAR classification criteria.【Methods】Fifty-four cases of early patients with suspected RA were strictly selected, and all patients were tested by ultrasound of 38 jionts [GSUS≥1 and PDUS(+)]and clinical physical examination; the number and location of affected joints were counted, and the results of the three methods were applied to the 2010 ACR/EULAR classification standard score, and the score results were statistically evaluated;At the same time, the patients were scored by ACR 1987 classification criteria.The sensitivity, specificity and the different classification criteria were calculated according to the use of DMARDs and(or) the RA judged rheumatoid specialist as a "gold standard" for the diagnosis of RA on the base of "2010 ACR/EULAR Classification Standard Joint Initiative". The difference between different classification criteria (Methods) was compared.【Results】Of the 54 cases, 37 were diagnosed as RA with the above criteria, 39 patients met 2010 ACR/EULAR GSUS(+)standard, 21 cases met the standard of 2010 ACR/EULAR PDUS (+), 27 cases met the 2010 ACR/EULAR clinical physical examination standard, and18 cases met the ACR 1987 standard.Of the different ultrasonic evaluation standard GSUS ≥1, the comparison of PDUS (+) and clinical physical examination on the detection rate of affected joints, the detection rate of GSUS ≥1 of all joints, the detection rate of PDUS (+) metatarsophalangeal jointthe and the GSUS≥1 total detection rate were all higher than those of the clinical examination, the difference was statistically significant (P<0.05).The standard sensitivity of 2010 ACR/EULAR ultrasound was higher than 2010 ACR/EULAR clinical physical examination standard and ACR 1987 standard:the sensitivity of 2010 GSUS≥1 was the highest, and ACR 1987 was the lowest; the specificity of PDUS (+) was the highest, and the lowest was 2010 GSUS≥1; the diagnostic coincidence rate was 2010 GSUS≥1, and ACR 1987 was the lowest;the highest Youden's index was PDUS (+), the highest Kappa value was 2010 GSUS≥1; 2010 PDUS (+) was the highest in the diagnostic reliability (AUC).【Conclusion】Different ultrasonic evaluation criteria can detect subclinical synovitis in multiple joints where as that the usual clinical examination is unable to find out, among which the GSUS≥1 and PDUS (+) metatarsophalangeal joint hold the higher detection rate. Evidence of affected joint involvement provided by the different ultrasound evaluation standards is applied to the 2010 ACR/EULAR classification standard, whose efficiency is higher than that of ACR 1987 standard and ACR/EULAR 2010clinical examination standard.The affected joint evidence provided by 2010 PDUS (+) has higher diagnostic efficacy.

Value of Neoadjuvant Interventional Chemotherapy in the Treatment of Locally Advanced Cervical Cancer and Its Effect on Cytokine Expression

【Objective】To investigate the value of neoadjuvant interventional chemotherapy in the treatment of locally advanced cervical cancer and its effect on the expression of MMP-2, Fas and FasL cytokines.【Methods】A total of 60 cervical cancer patients in stage of IB ~ IIB, with tumor diameters greater than 4 cm were chosen for the study. They were randomly divided into groups A and B, each with 30 subjects. Patients in group A was treated with artery interventional neoadjuvant chemotherapy plus radical surgery, while patients in group B were treated with conventional intravenous chemotherapy plus radical surgery. The efficacy and occurrence of side effects were observed. The expression of MMP-2, Fas and FasL cytokines in two groups both before and after treatment was detected by immunohistochemistry.【Results】The effective rate of group A (83.33%) was significantly higher than that of group B (73.33%) (χ

=2.116, P=0.045). The effective rate of Ib was significantly higher than that of IIa and IIb; IIa was significantly higher than that of IIb; the effective rate of chemotherapy in patients with squamous cell carcinoma was significantly higher than that of adenocarcinoma; the efficiency of low differentiation of cervical cancer was significantly higher than that of middle differentiation and high differentiation (P<0.05). In group A, there were 14 cases of chemotherapy side effects, including vomiting /nausea (6 cases), moderate anemia (2 cases), hypogastrium pain (4 cases) and alopecia (2 cases). Effect of Compound Kushen Injection Combined with Docetaxel on NSCLC Patient after the Failure of First-line Chemotherapy Treatment

【Objective】To investigate the effect of compound Kushen injection combined with Docetaxel on serum tumor markers in patients with non-small cell lung cancer (NSCLC) after the failure of first-line chemotherapy treatment.【Methods】A total of 80 patients with NSCLC after the failure of first-line chemotherapy treatment were randomly divided into observation group (treated with compound Kushen injection combined with docetaxel) and control group (treated with docetaxel). The short-term efficacy, adverse reactions, serum tumor markers levels before and after treatment and changes of cellular immune function were compared between the two groups, OS and PFS of the two groups were statistically analyzed.【Results】The short-term effective rate and disease control rate of the observation group were 25% (10/25) and 62.50% (25/40), respectively, which were significantly higher than 10% (4/40) and 37.50% (15, 40) of the control group, and the difference was significant (P<0.05);There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05);Compared with before treatment, the levels of serum carcinoembryonic antigen (CEA), carbohydrate antigen -125 (CA-125) and cytokeratin 19 fragment (CYFRA21 - 1) were significantly decreased in the two groups after treatment (P<0.05), and the above indexes in the observation group were significantly lower than those in the control group (P<0.05);The median OS and PFS in the observation group were 13.50 months and 9 months respectively, which were significantly longer than those in the control group at 10.20 months and 7.40 months, and the difference was significant (P<0.05).【Conclusion】Compound Kushen injection combined with docetaxel can significantly reduce the serum tumor markers in patients with NSCLC, improve the cellular immune function, prolong OS, PFS, and do not increase toxicity

Clinical Analysis of Conservative Treatment of Torsion Reset and Benign Tumor Resection in Ipsilateral Oviduct and Ovary Retained

【Objective】To explore the conservative treatment of the oviduct and ovary retained in the ipsilateral benign ovarian tumor in young patients.【Methods】One hundred and eighty-three patients diagnosed with ovarian neoplasms in our hospital from January 2011 to December 2014 were selected and divided into pedicle torsion group and torsion group, respectively according to whether combined pedicle torsion or not. Treatment of benign ovarian tumor torsion reset reduction and simple tumor stripping were performed.Frozen sections were then taken for histopathological examination; intraoperative and hospitalization time were compared between the two groups of patients.【Results】In 183 cases, 91 cases (49.7%) were diagnosed as non neoplastic ovarian cysts (including endometriosis), 69 cases of benign tumor (37.7%), 20 cases (11%) of primary malignant ovarian tumor, 2 cases of borderline tumors (1.1%) and 1 case of metastatic colorectal cancer (0.55%). In 160 cases of benign tumors, 131 cases (81.8%) may undergo conservative surgery to preserve the affected fallopian tubes and ovaries. The median follow-up time of 20 patients with primary malignant ovarian tumors was 36 (1~62) months. During this period, 2/20 (10%) recurred, all of which were germ cell types (immature teratoma and yolk sac tumor). 131 patients who had received conservative surgery to retain the ipsilateral fallopian tubes and ovaries were cured. The operative time and length of hospital stay were shorter in torsion group than in torsion group (P<0.05).【Conclusion】Conservative ovarian conservative surgery is effective and safe in the treatment of benign ovarian tumors in young patients, and the operation is better if no tumor torsion is found.

Expression of MIF and Cyr61 in Gingival Carcinoma and Potential Clinical Application

【Objective】To explore the expression of macrophage migration inhibitory factor (MIF) and cysteine-rich 61 (Cyr 61) in gingival carcinoma and their clinical application.【Methods】A total of 58 patients with gingival carcinoma treated in our hospital from April 2012 to July 2016 were recruited for the study. Tumor tissues from all 58 cases were collected along with 11 cases of randomly picked adjacent normal gingival tissues among those 58 patients. The expression of MIF and Cyr61 was determined by immunohistochemical staining method, and the clinical data and pathological features of patients were summarized.【Results】The positive expression rate of MIF in gingival carcinoma tissues was found to be 70.7% (41/58), which was significantly higher than that in adjacent normal tissues 36.4%(4/11). The positive expression rate of Cyr61 in gingival carcinoma tissues was 89.7% (52/58), which was significantly higher than that in the adjacent normal tissues 54.5% (6/11). The positive expression of MIF and Cyr61 had no obvious correlation with the age and gender of patients, while they had a significant positive correlation with the differentiation degree of gingival carcinoma and lymph node metastasis. Both MIF and Cyr61 were positive in 37 cases (63.8%). There was a positive correlation between MIF and Cyr 61 (r=0.355, P<0.05).【Conclusion】The expression levels of MIF and Cyr61 are high in gingival carcinoma tissues and they are correlated with tumor differentiation and lymphatic metastasis. MIF and Cyr 61 are positively correlated with each other, which can be used as a reference index for clinical diagnosis and treatment of gingival carcinoma.

Analysis of the Risk Factors for Unexplained High Fever Following Splenectomy in Patients with Portal Hypertension

【Objective】To explore the risk factors for unexplained high fever following splenectomy in patients with portal hypertension (PHT).【Methods】The clinical data of 60 patients treated with splenectomy pericardia vascular disconnection in our hospital from January 2012 to December 2015 were retrospectively analyzed. According to the inclusion criteria of postoperative fever, they were divided into the observation group (n=26), with the duration of unexplained fever over 7d, and the highest temperature no less than 38℃ and the control group (n=34). Univariate and multivariate analyses were performed to screen the high-risk factors for postoperative unexplained high fever.【Results】The univariate analysis showed that the differences between the two groups in preoperative liver function grade, total bilirubin (TBIL), intraoperative blood loss and intraoperative ascites were statistically significant (P<0.05). Multivariate Logistic analysis showed that the liver function grade, TBIL, intraoperative blood loss and intraoperative ascites were the high-risk factors for unexplained high fever after splenectomy in PHT patients (P<0.05).【Conclusion】Preoperative poor liver function grade, high TBIL level, large amount of intraoperative blood loss and increased intraoperative ascites volume are the major risk factors for high fever after splenectomy in patients with portal hypertension.

Expression of CD133 and MVD in Triple-Negative Breast Cancer

【Objective】To detect the expression of tumor stem cell marker CD133 and micro vessel density (MVD) in triple negative breast carcinoma (TNBC), and to analyze their clinical value.【Methods】A total of 41 patients with triple-negative breast cancer were enrolled in our hospital from January 2010 to January 2012 for this study. CD133 expression and MVD count were measured by immunohistochemical method ElivisionTM plus, and the follow-up data of all patients were arranged and analyzed.【Results】There were significant differences in age, menstrual status, lymph node metastasis and histological grade between TNBC patients and non - TNBC patients
(P<0.05).However, there were no significant differences in tumor size, pathological type and clinical stage (P>0.05). Out of the total 41 TNBC cases, 18 had CD133 positive expression, accounting for 43.90% of the total samples. In these cases, the tumor size, clinical stage and lymph node metastasis were closely related to the positive rate of CD133 (P<0.05). The mean MVD count of TNBC patients was (20.95 ± 7.8) which was significantly higher than that of non-TNBC group (15.02 ± 9.3); the difference was significant (t=3.916, P<0.05).MVD counts in lymph node metastasis and clinical staging were statistically significant (P<0.05). The MVD value of CD133 positive in TNBC patients was (23.34 ± 8.4), which was significantly higher than that of CD133 negative (18.11 ± 5.5), and the difference was significant (t=2.403, P<0.05). The mean time of disease-free survival (DFS) and overall survival (OS) time of CD133 positive expression in TNBC were (25.5 ± 3.6) months and (32.3±2.5) months, respectively. These were significantly lower than those of CD133 negative expression in TNBC DFS (58.2 ± 4.2) months and OS (54.6 ± 3.3) months, respectively; The difference was significant (P<0.05). In this study, the median value of MVD in 41 patients with TNBC was 21. Our results showed that the 3-year and 5-year survival rates of MVD≥21 were significantly lower than those of MVD<21; the difference was significant (P<0.05 ).【Conclusion】CD133 expression and MVD count are closely related to lymphatic metastasis and clinical stage in TNBC patients. Therefore, the clinical progress and prognosis of TNBC patients can be determined by measuring CD133 expression and MVD count and is worthy of clinical application.

Effects of Metformin Combined with Pioglitazone on Body Fat Composition, α and β Cell Function in Patients with Schizophrenia and Type 2 Diabetes Mellitus

【Objective】To analyze the effect of early hypoglycemic agents (metformin and pioglitazone) on the changes in body fat composition, α and β cell function in patients with schizophrenia (SP) and type 2 diabetes mellitus (T2DM).【Methods】A total of 180 patients with SP and T2DM treated in our hospital from January 2015 to June 2016 were selected in the study. Patients were divided into the control group and the observation group by random number table method, with 90 cases in each group. Both groups were given routine basic treatment. The control group was treated with metformin while the observation group was treated with metformin and pioglitazone. Changes in glucose metabolism, body fat composition, α and β cell function before and after treatment were determined and the therapeutic effects were compared between the two groups. The incidence of adverse reactions was statistically analyzed.【Results】After treatment, the fasting blood glucose (FPG), postprandial 2h blood glucose (2hPG) and glycosylated hemoglobin (HbA1c) in the two groups were decreased (P<0.05), but the decrease of 2hPG was more obvious in the observation group than in the control group (P<0.05). After the treatment, the insulin resistance index (HOMA-IR) was decreased and insulin secretion index HOMA-β was increased in both groups (P<0.05). HOMA-β was higher in the observation group than the control group, while HOMA-IR and glucagon levels were lower in the observation group than the control group (P<0.05). After treatment, the total cholesterol (TC) and low-density lipoprotein cholesterol (LDL-C) were decreased, while triglyceride (TG) and high-density lipoprotein (HDL-C) were increased (P<0.05). The levels of TC and LDL-C in observation group after treatment were lower than those in the control group (P<0.05). After treatment, the body weight and body fat percentage (BF) in the observation group were lower than those in the control group (P<0.05), and there was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).【Conclusion】The body fat percentage is high, function of islet α and β cells is decreased in patients with SP and T2DM. The early treatment with metformin and pioglitazone can improve the body fat composition and optimize the function of islet α and β cells.

Effect of Active Silver Ion Antibacterial Liquid on Oral Ulcer Induced by Chemotherapy and Plasma Cytokines in Children with Acute Lymphoblastic Leukemia

【Objective】To investigate the effect of active silver ion antibacterial liquid on the prevention of oral ulcer and the cytokine in plasma of children with acute lymphoblastic leukemia.【Methods】A total of 118 cases of acute lymphoblastic leukemia diagnosed in our hospital from January 2015 to February 2017 were selected; according to the random number method, all the patients were randomly divided into the observation/study group (59 cases) and the control group (59 cases); all of the children completed the examination after admission and were treated with standard regimen for chemotherapy, routine oral nursing intervention during chemotherapy; the control group was given 0.9% saline or 1% hydrogen peroxide;on the basis of in the control group, the study group was treated with cleaning up by the active silver ion antibacterial mouthwash oral 5~10 ml, every time, gargle 5 min daily in the morning and evening and after meals each time, to those with oral ulcer recovery. The incidence of oral ulcers and the recovery time of oral ulcers were recorded and recorded in the two groups; the severity of oral ulcer was evaluated according to the standard of acute and subacute toxicity of anticancer drugs; at the same time before treatment and one week after treatment respectively, plasma interleukin -2 (IL-2) and interferon-g (IFN-g), tumor necrosis factor alpha (TNF- alpha) and other cytokines were determined by enzyme-linked immunosorbent assay (ELISA).【Results】The incidence rate of oral ulcer and oral ulcer recovery time of the study group with were significantly lower than the control group, the difference was statistically significant (P<0.05); oral ulcer severity of the study group was significantly lower than that of the control group, the two groups have statistical significance in children with oral ulcer severity difference (Z=25.162, P<0.05); after treatment, TNF-ɑ of the two groups of children was significantly decreased, IL-2 and IFN-g were significantly increased; after treatment, the level of TNF-ɑ of the study group was significantly lower than that of the control group, while the IL-2 and IFN-g levels were significantly higher than those the control group, the differences were statistically significant (P<0.05).【Conclusion】Active silver ion antibacterial liquid can effectively prevent children from acute lymphoblastic leukemia oral ulcers induced by chemotherapy, and promote the prognosis, improve plasma IL-2, IFN-g and TNF-ɑ levels, and enhance the immune function of children. It is worthy of clinical application.
