ZENG Xukai,HE Pan,WANG Zhijun
DOI: 10.3969/j.issn.1671-7171.2023.12.001
【Objective】 To investigate the effects of miR-181a and miR-138 on the proliferation of chondrosarcoma cells. 【Methods】A total of 20 samples of osteosarcoma and 20 samples of chondrosarcoma tissues were collected. qRT PCR was used to detect the expression levels of miR-181a and miR-138. miRNA mimetics were used to promote the expression of miR-181a and miR-138 in chondrosarcoma cells SW1353. Cell counting was used to detect the effects of miR-181a or miR-138 overexpression alone or in combination on the growth ability of SW1353 cells. The binding sites of miR-181a, miR-138 and RAS family associated domain protein 8 (RASSF8) were predicted using the TargetScan database, and gene experiments were validated using dual luciferase reporter technology.【Results】Compared with chondrosarcoma tissue, the expression of miR-181a and miR-138 was significantly upregulated in chondrosarcoma tissue (P<0.05); Compared with the negative control group, the cell growth ability of the miR-181a and miR-138 overexpression groups was significantly increased (P<0.05), and the combined overexpression group of miR-181a and miR-138 showed the most significant upregulation of cell growth ability; According to predictions, both miR-181a and miR-138 have binding sites with RASSF8; Compared with the cotransfected wild-type RASSF8+scaffold group, the luciferase activity in the cotransfected wild-type RASSF8+miR-181a group and wild-type RASSF8+miR-138 group was significantly reduced (P<0.05); Compared with the cotransfected mutant RASSF8+scaffold group, there was no significant change in luciferase activity in the cotransfected mutant RASSF8+miR-181a group and the mutated RASSF8+miR-138 group (P>0.05). 【Conclusions】miR-181a and miR-138 may promote the growth of chondrosarcoma cells by inhibiting RASSF8.

GU Jianing,LIU Dan,DI Ruomin,et al
DOI: 10.3969/j.issn.1671-7171.2023.12.008
【Objective】To explore the prognostic value of combined detection of serum cystatin C (cys C) and total bilirubin (TBIL) in the prognosis of high-risk non-ST-segment elevation myocardial infarction (NSTEMI). 【Methods】 The clinical data of 173 patients with NSTEMI who underwent percutaneous coronary intervention (PCI) treatment in our hospital from November 2018 to November 2020 were retrospectively analyzed. Serum cys C and TBIL levels were detected before PCI. Patients were followed up for 28 days to record the incidence of the patients' major adverse cardiovascular events (MACE). According to whether the patients had MACE at 28 days after operation, they were divided into the occurrence group (occurrence of MACE) and non-occurrence group (non-occurrence of MACE). General patient information was collected. Logistic regression analysis was used to analyze the risk factors of MACE in NSTEMI patients within 28 days after PCI. The receiver operating characteristic curve (ROC) was drawn, and the area under curve (AUC) was used to evaluate the predictive value. 【Results】Within 28 days after the operation, 29 of the 173 patients had MACE. The incidence of MACE was 16.76%. The age of > 65 years old, Killip grade (Ⅲ-Ⅳ), the proportion of cases with culprit blood vessels ≥2, and the pre-PCI blood levels of creatinine, N-terminal B-type brain natriuretic peptide (NT-proBNP), uric acid (UA), serum cys C and TBIL in the occurrence group were higher than those in the non-occurring group (P<0.05). Logistic regression analysis showed that age>65 years old, number of culprit blood vessels ≥2, serum cys C and TBIL levels before PCI were risk factors for MACE in NSTEMI patients within 28 days after PCI (P<0.05).ROC results showed that the serum cys C and TBIL levels before PCI, and the combination of both cys C and TBIL detection for predicting the AUC values of MACE in NSTEMI patients within 28 days after PCI were 0.785, 0.744,and 0.915 (all P<0.05). 【Conclusion】The detection of both serum cys C and TBIL levels before PCI has a certain value in predicting the occurrence of MACE in NSTEMI patients within 28 days after PCI, while the combined detection of cys C and TBIL has even higher predictive value.

NING Wenjun,HU Weibing
DOI: 10.3969/j.issn.1671-7171.2023.12.009
【Objective】To investigate the relationship of cognitive function with levels of platelet to lymphocyte ratio (PLR) and neutrophil to lymphocyte ratio (NLR)in patients with acute ischemic stroke (AIS). 【Methods】Among 80 AIS patients, 54 with cognitive impairment were classified as group A, and 26 with normal cognitive function were classified as group B. The general data, as well as the levels of PLR, NLR, interleukin-1β (IL-1β), and interleukin-18(IL-18) in both groups were compared. The value of these indicators in predicting cognitive impairment in AIS patients was analyzed using the receiver operating characteristic (ROC) curve analysis. Multifactorial Logistic regression analysis was used to clarify the risk factors for cognitive dysfunction. 【Results】There were no statistically significant differences between the two groups in terms of gender, age, comorbidity with hypertension, diabetes, hyperlipidemia and coronary heart disease, history of alcohol consumption, and severity of stroke (P>0.05). The levels of PLR, NLR, IL-1β, and IL-18 in the group A were significantly higher than those in the group B (P<0.05). ROC analysis confirmed that the serum levels of PLR, NLR, IL-1β, and IL-18 could be used to predict cognitive impairment in AIS patients, with areas under the curve of 0.707, 0.769, 0.827, and 0.929 respectively (all P<0.05). Multifactorial Logistic regression analysis confirmed that PLR>3.355, NLR>6.598, IL-1β>28.565 pg/mL, and IL-18>22.554 pg/mL are risk factors for cognitive impairment in AIS patients. 【Conclusion】Changes in the levels of PLR, NLR, IL-1β, and IL-18 are closely related to the occurrence and development of cognitive dysfunction after AIS and they are risk factors for cognitive impairment in patients.

DU Aimin,TUO Zhen,SUN Sha,et al
DOI: 10.3969/j.issn.1671-7171.2023.12.010
【Objective】To explore the relationship of Montreal Cognitive Assessment (MoCA) score with serum levels of protein kinase C (PKC), insulin-like growth factor-1 (IGF-1), and brain-derived neurotrophic factor (BNDF) in epilepsy patients. 【Methods】Based on the MoCA scores, 80 epilepsy patients treated in our hospitals were divided into the cognitive impairment group (group A with MoCA score <26) and the non-cognitive impairment group (Group B with MoCA score ≥26). The basic information, electroencephalogram results, and serum indicators [PKC, IGF-1, BNDF, Interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-α)] of the two groups were compared. The relationship of cognitive impairment with serum PKC, IGF-1, and BNDF levels was analyzed using Receiver Operating Characteristic (ROC) curve analysis. Multivariate Logistic regression analysis was used to identify the risk factors for cognitive impairment in epilepsy patients. 【Results】Among the 80 patients, 52 (65.00%) cases had cognitive impairment and were categorized into the group A, while 28 (35.00%) cases did not have cognitive impairment and were categorized into the group B. The proportion of patients with generalized seizures and epileptiform waves on electroencephalogram in Group A was higher than that in Group B. The levels of PKC, IGF-1, and BNDF in Group A were lower than in Group B (P<0.05). There were no significant differences in age, gender, duration of illness, family history of epilepsy, history of febrile seizures, IL-6, and TNF-α levels between the two groups (P>0.05). ROC analysis confirmed that the levels of PKC, IGF-1, and BNDF could be used to predict cognitive impairment, with area under the curve (AUC) values of 0.927, 0.732, and 0.862, respectively (all P<0.05). Multivariate Logistic regression analysis showed that generalized seizures, epileptiform waves on electroencephalogram, PKC<3.015 pg/mL, IGF-1<126.57 μg/L, and BNDF<7.635 μg/L were risk factors for cognitive impairment (P<0.05). 【Conclusion】MoCA scores in epilepsy patients are positively correlated with serum levels of PKC, IGF-1, and BNDF. Lower scores of MoCA indicate more severe cognitive impairment and lower serum levels of PKC, IGF-1 and BNDF. Generalized seizures, epileptiform waves on electroencephalogram, PKC<3.015 pg/mL, IGF-1<126.57 μg/L, and BNDF<7.635 μg/L are all risk factors for cognitive impairment in epilepsy patients, which should be given clinical attention.

DOI: 10.3969/j.issn.1671-7171.2023.12.015
【Objective】To investigate the correlation between serum nuclear factor-kappa B (NF-κB) inflammatory signaling pathway and renal function indicators such as blood urea nitrogen (BUN), serum creatinine (Scr), urinary albumin excretion rate (UAER), β2-microglobulin (β2-MG), and estimated glomerular filtration rate (eGFR) in patients with type 2 diabetic nephropathy (T2DN). 【Methods】Eighty-seven newly diagnosed T2DN patients (Group A), eighty patients with primary type 2 diabetes mellitus without nephropathy (Group B), and seventy healthy volunteers (Group C) were enrolled in the study. Clinical data, renal function indicators, and serum levels of NF-κB, tumor necrosis factor-alpha (TNF-α), interleukin-1β (IL-1β),interleukin-6(IL-6), and high-sensitivity C-reactive protein (hs-CRP) were compared among the three groups. Pearson correlation analysis was used to analyze the relationship of renal function indicators with serum levels of NF-κB, TNF-α, IL-1β, IL-6 and hs-CRP, in T2DN patients.【Results】Fasting blood glucose and 2-hour postprandial blood glucose in both group A and group B were higher than those in group C (P<0.05). Other clinical data showed no significant difference among the three groups (P>0.05). Levels of BUN, Scr, UAER, and β2-MG in group A were higher than those in groups B and C (P<0.05), while level of eGFR in group A was lower than that in groups of B and C (P<0.05). There were no significant differences in these above indicators between group B and group C (P>0.05). Serum levels of NF-κB, TNF-α, IL-1β, IL-6, and hs-CRP in group A were higher than those in Groups B and C (P<0.05); The above indicators of group B also were higher than those of group C (P<0.05). Pearson correlation analysis showed that serum levels of NF-κB, TNF-α, IL-1β, IL-6, and hs-CRP in T2DN patients were positively correlated with levels of BUN, Scr, UAER, and β2-MG (P<0.05), while they were negatively correlated with eGFR level (P<0.05). 【Conclusion】Activation of the NF-κB inflammatory signaling pathway mediates the pathogenesis of T2DN. NF-κB, TNF-α, IL-1β, IL-6 and hs-CRP are significantly correlated with renal function indicators.

HUANG Junfeng,YANG Le,YANG Sen
DOI: 10.3969/j.issn.1671-7171.2023.12.021
【Objective】To explore the predictive value of the modified Daping Orthopaedic Surgery Risk Scoring System (DORSSSP) score combined with neutrophil to lymphocyte ratio (NLR) level for postoperative complications in elderly patients with hip fractures. 【Methods】A total of 160 elderly patients with hip fractures admitted to our hospital from January 2019 to December 2022 were enrolled in this study. After admission, all patients underwent surgical treatment, and a 3-month follow-up was conducted to record the occurrence of postoperative complications. According to the occurrence of postoperative complications, patients were divided into the complication group and the non-complication group. Relevant clinical data of patients such as gender, age, body mass index, the modified DORSSSP score and the NLR level were collected. Single factor and multivariate logistic regression were used to analyze the factors influencing the occurrence of postoperative complications in patients. The predictive value of the modified DORSSSP score combined with NLR level detection for postoperative complications in patients was evaluated using the subject performance characteristic (ROC) curve. 【Results】The postoperative complication rate of elderly patients with hip fracture was 31.88% (51/160). The results of univariate analysis showed that there were no statistical differences in gender, body mass index (BMI), fracture location, surgical method, ASA grade, comorbid hypertension, and operative time between the two groups (P>0.05). Age, proportion of comorbid diabetes mellitus, C-reactive protein (CRP) level, and white blood cell count (WBC) level were significantly higher in the complication group than in the non-complication group (P<0.05). The modified DORSSSP scores and NLR level of the patients in the complication group were significantly higher than those in the uncomplicated group (P<0.05). Multivariate logistic regression analysis showed that high modified DORSSSP score (OR=1.755), high NLR level (OR=2.147), advanced age (OR=2.658), and high CRP (OR=2.324) level were independent risk factors for the development of postoperative complications (P<0.05). ROC curve analysis showed that the area under the curve (AUC) of the modified DORSSSP score, NLR level and the combined prediction of both DORSSSP score and NLR for postoperative complications in elderly hip fracture patients were 0.686, 0.714 and 0.898, respectively. 【Conclusion】The modified DORSSSP score and NLR level are risk factors for postoperative complications in elderly patients with hip fractures. The combination of both modified DORSSSP scoring and NLR level provides better predictive value for the evaluation of the occurrence of postoperative complications in patients.

DOI: 10.3969/j.issn.1671-7171.2023.12.026
【Objective】 To explore the correlation between electrocardiogram ischemic grading, myocardial injury markers, and no reflow in patients with acute ST segment elevation myocardial infarction. 【Methods】A retrospective analysis was conducted on the clinical data of 80 patients with acute ST segment elevation myocardial infarction admitted to our hospital from January 2019 to January 2022. According to the electrocardiogram ischemic grading, they were divided into grade Ⅱ ischemic group (n=32) and grade Ⅲ ischemic group (n=48). We compared the levels of two groups of myocardial injury biomarkers [high-sensitivity troponin (hs-cTnI), N-terminal B-type brain natriuretic peptide(NT-proBNP), and creatine kinase isoenzyme (CK-MB)], and used receiver operating characteristic (ROC) curves to analyze the value of myocardial injury biomarker indicators in predicting grade Ⅲ ischemia in acute ST segment elevation myocardial infarction. Logistic multiple factor stepwise regression was used to analyze the influencing factors of electrocardiogram ischemia grade Ⅲ in patients with acute ST segment elevation myocardial infarction. 【Results】The proportion of patients in the grade Ⅱ ischemic group without reflow was lower than that in the grade Ⅲ ischemic group, and the levels of hs-cTnI and NT-proBNP were lower than those in the grade Ⅲ ischemic group. The levels of CK-MB were higher than those in the grade Ⅲ ischemic group (P<0.05). ROC curve analysis showed that hs-cTnI, NT-proBNP, and CK-MB levels can all be used for predicting grade Ⅲ ischemia in acute ST segment elevation myocardial infarction, with areas under the curves of 0.861, 0.706, and 0.792 (P<0.05), respectively. Multivariate logistic regression analysis showed that no reflow, hs-cTnI ≥ 1.545 ng/mL, NT-proBNP ≥ 1075.005 ng/mL, and CK-MB<38.253 ng/mL were risk factors for ischemic grade Ⅲ on electrocardiogram in acute ST segment elevation myocardial infarction (P<0.05). 【Conclusion】There are differences in the levels of myocardial injury markers and reflow in patients with acute ST segment elevation myocardial infarction with different electrocardiographic ischemic grades, and myocardial injury markers and no reflow can be used to predict the ischemic grade of acute ST segment elevation myocardial infarction electrocardiograms.

YANG Lisha
DOI: 10.3969/j.issn.1671-7171.2023.12.028
【Objective】To explore the influencing factors of early brain injury (EBI) in patients with high-grade aneurysmal subarachnoid hemorrhage (aSAH). 【 Methods 】 A retrospective analysis was conducted on the clinical data of 182 high-grade aSAH patients admitted to our hospital from March 2017 to March 2022. They were divided into EBI group (n=90) and non EBI group (n=92) based on the occurrence of EBI. We collected and compared baseline data from two groups of patients, and used multivariate logistic regression analysis to identify independent risk factors for EBI in high-grade aSAH patients.【Results】 The EBI group had a higher proportion of age, hyperglycemia, hypomagnesemia, hyponatremia, and Fisher's grade Ⅲ-Ⅳ than the non EBI group (P<0.05), and higher levels of serum periosteal protein, IL-6, and S100B protein than the non EBI group (P<0.05). Multivariate logistic regression analysis showed that age ≥ 60 years old, hyperglycemia, hypomagnesemia, hyponatremia, Fisher's grading (Ⅲ-Ⅳ), periosteal protein≥270.00 ng/mL, IL-6≥47.10 pg/mL, S100B protein ≥ 2.05 μg/L was an independent risk factor for EBI in high-grade aSAH patients (P<0.05).【Conclusion】 There are many risk factors for EBI in high-grade aSAH patients, which are not only related to age, hyperglycemia, hypomagnesemia, hyponatremia, and Fisher's grading, but also to serum periosteal protein, IL-6, and S100B protein levels. In clinical practice, these risk factors should be identified as early as possible and targeted preventive measures should be taken.

DOI: 10.3969/j.issn.1671-7171.2023.12.029
【Objective】To investigate the effect of supervised pelvic floor rehabilitation training combined with neuromuscular electrical stimulation on urinary dynamics in postpartum stress urinary incontinence (SUI) patients. 【Methods】A total of 260 postpartum SUI patients from two hospitals were randomly divided into two groups, with 130 patients in each group. The patients in the control group received supervised pelvic floor rehabilitation training, while patients in the observation group received supervised pelvic floor rehabilitation training combined with neuromuscular electrical stimulation. The treatment outcomes, incidence of complications, changes in functional urethral length (LES), maximum urethral closure pressure (PMUC), maximum urethral measurement pressure (PMU), progesterone (P), luteinizing hormone (LH), estradiol (E2), as well as the scores of the Incontinence Quality of Life Questionnaire (I-QOL) and Female Sexual Function Index (FSFI) were compared between the two groups. 【Results】The total effective rate in the observation group was 90.77%(118/130), which was significantly higher than 78.46%(102/130) in the control group. After treatment, the levels of LES, PMUC, PMU, P, LH, E2, I-QOL scores, and FSFI scores in both groups were significantly higher than those before treatment(P<0.05). Furthermore, the observation group after treatment showed higher levels of the above parameters than those in the control group after treatment (P<0.05). The incidence of complications in the observation group was 10.77%(14/130), which was significantly lower than the 20.00%(26/130) in the control group (P<0.05). 【Conclusion】Supervised pelvic floor rehabilitation training combined with neuromuscular electrical stimulation can more effectively improve urinary dynamic indicators in postpartum SUI patients to result in better treatment outcomes.

DOI: 10.3969/j.issn.1671-7171.2023.12.030
【Objective】To explore the myocardial injury and changes in T lymphocytes in children with rotavirus enteritis and their correlation. 【Methods】 A retrospective analysis was conducted on 80 children with rotavirus enteritis admitted to our hospital from January 2019 to January 2022 (observation group). According to the severity of dehydration, the observation group was divided into non dehydration group (n=26), mild dehydration group (n=32), and moderate dehydration group (n=22). Another 80 children with enteritis who tested negative for rotavirus and were admitted to our hospital during the same period were selected as the control group. Myocardial injury biomarkers between two groups and cases with different degrees of dehydration [Creatine kinase (CK), creatine kinase isoenzyme (CK-MB), lactate dehydrogenase (LDH), α-hydroxybutyrate dehydrogenase (α-HBDH)] levels and T lymphocyte subpopulations (CD3+, CD4+, CD8+, and calculate the CD4+/CD8+ ratio) were compared.【Results】The levels of CK, CK-MB, LDH, and α-HBDH in the observation group were significantly higher than those in the control group, with statistical significance (P<0.05). The levels of CK, CK-MB, LDH, and α-HBDH in the mild and moderate groups of children were higher than those in the non dehydration group, while those in the moderate group were higher than those in the mild group, with statistical significance (P<0.05). The CD8+ in the observation group was higher than that in the control group, and the CD3+, CD4+, and CD4+/CD8+ values were lower than those in the control group, with statistical significance (P<0.05). The CD8+ levels in the mild and moderate groups were higher than those in the non dehydrated group, and the levels of CD3+, CD4+, and CD4+/CD8+ were lower than those in the non dehydrated group. Moreover, the above indicators in the mild group were significantly different from those in the moderate group (P<0.05). 【Conclusion】Myocardial injury markers and T lymphocyte subsets can accurately evaluate the dehydration status of children with early-stage viral enteritis, which is of great significance for guiding clinical treatment in the later stage.

DOI: 10.3969/j.issn.1671-7171.2023.12.032
【Objective】To explore the related influencing factors of postoperative incisional infection after laparoscopic cholecystectomy (LC).【Methods】A retrospective analysis of 80 patients treated with LC in our hospital from April 2020 to April 2022. Patients were divided into the incision infection group (n=20) and the incision non-infection group (n=60) based on whether postoperative incision infection occurred. General data of patients in the incision infection group were compared to those in the incision non-infection group. Logistic multiple regression analysis was then performed on single factors with statistical significance, and the receiver operating characteristic (ROC) curve was used to analyze the clinical value of relevant influencing factors in predicting postoperative incisional infection after LC.【Results】The proportion of patients with incision infection in terms of age >55 years old, surgery time>60 minutes, emergency surgery, and gallbladder rupture in the incision infection group was significantly higher than that in the incision non-infection group; and the difference was statistically significant (P<0.05). The results of logistic multiple regression analysis showed that surgical time, emergency surgery, and gallbladder rupture were the influencing factors of incision infection after LC surgery (P<0.05). ROC curve analysis showed that the area under the curve (AUC) of surgical time, emergency surgery, and gallbladder rupture for predicting incision infection after LC surgery were 0.758, 0.733, and 0.808, respectively. While their Youden Index was 0.467, 0.441, and 0.617, respectively. 【Conclusion】The risk factors for incision infection during laparoscopic cholecystectomy are the operation time, emergency surgery and gallbladder rupture. Clinically, targeted treatment plans need to be formulated for such patients.
