Abstract  【Objective】To investigate the changes of red blood cell distribution width (RDW) in patients with type H hypertension complicated with carotid atherosclerosis. 【Methods】A total of 168 patients with type H hypertension admitted to our hospital from May 2019 to December 2020 were prospectively selected and randomly divided into an observation group (with carotid atherosclerosis) and a control group (without carotid atherosclerosis) according to whether they had carotid atherosclerosis. There were 84 patients in each group. The general data and RDW levels of the two groups of patients were compared, and the carotid atherosclerosis in patients with type H hypertension with different RDW levels was compared. Pearson correlation analysis was used to analyze the correlation between RDW and clinical indicators.【Results】The proportion of patients in the observation group with hypertension grade 3, 24-hour dynamic systolic blood pressure (24hSBP), RDW, and homocysteine (Hcy) were significantly higher than those in the control group, and the differences were statistically significant (P<0.05). The 24hSBP, hemoglobin (Hb), and Hcy levels in the high RDW subgroup were higher than those in the low RDW subgroup, and the differences were statistically significant (P<0.05). The number and area of carotid plaque and carotid intima-media thickness (IMT) in the high RDW subgroup were significantly higher than those in the low RDW subgroup (P<0.05). Pearson correlation analysis showed that RDW was positively correlated with 24hSBP(r=0.419), Hcy (r=0.327), atherosclerotic plaque area (r=0.506) and IMT (r=0.238) (all P<0.05). 【Conclusion】RDW is the influencing factor of carotid atherosclerosis in patients with type H hypertension. Clinicians can evaluate the carotid atherosclerosis in patients with type H hypertension according to the changes of RDW.
