Effect of Lumbar Internal Fixation on the Application of Automatic Exposure Control Technology

【Objective】
To investigate the application value of automatic exposure control (AEC) technology in digital radiography (DR) of lumbar spine after lumbar internal fixation.
【Methods】
A total of 262 cases of patients with lumbar spine in standing position were collected from May to June in 2019 by using AEC technology in Siemens DR system. The presence of metal internal fixation implants was checked, and the tube electric flow (MAS) and dose area product (DAP) in the image information were recorded, and the differences were compared.
【Results】
In standing position, 56 cases were implanted with metal internal fixation, 206 cases were without internal fixation. The mAs of patients with and without internal fixation were 72±41,59±28. The difference was statistically significant (t=2.62, P<0.05), DAP was 228±149,181±97,the difference was statistically significant (t=2.83,P<0.05). The mAs of patients with and without internal fixation were 76±38,70±33. The difference was not statistically significant (t=1.15, P>0.05), DAP was 264±140,225±122, the difference was statistically significant (t=2.09,P<0.05).
【Conclusion】
During DR follow-up after lumbar internal fixation, AEC technology may lead to unnecessary high mAs and increase the risk of radiation dose. It is recommended to give priority to the use of clinically verified and approved manual photographic exposure parameters.

Quercetin Alleviates LPS-induced Acute Lung injury in Mice by Regulating Endoplasmic Reticulum Stress Signals

【Objective】
To investigate the effect of quercetin (QUE) on acute lung injury (ALI) induced by lipopolysaccharide (LPS) in mice and study its mechanism.
【Methods】
Sixty male C57bl/6 mice were randomly divided into 4 groups (15 in each group): normal group (CON group), LPS group and QUE treatment group. The ALI mice were constructed by intraperitoneal injection of LPS (10 mg/kg). The CON group was given the same amount of normal saline, while the QUE treatment group were given QUE 25 mg/kg 2 hours before the administration of LPS. The mice in both CON group and The LPS group were given the same amount of DMSO. HE staining was used to detect lung histopathological changes. Western blotting was used to detect the expression of PERK and key proteins in the apoptosis pathway. Lung tissue oxidative stress levels were detected with malondialdehyde(MDA). Counted Inflammatory cells in bronchoalveolar lavage fluid (BLAF) and myeloperoxidase (MPO) activity in lung tissue were used to detect with inflammatory response.
【Results】
Quercetin alone had no effect on the morphology and inflammation of lung tissue in mice. Intraperitoneal injection of LPS induced pathological damage of mouse lung tissue which increased the number of total cells and neutrophils in BALF, increased MPO activity, and increased the levels of TNF-α and IL-1β in lung tissue. However, the QUE prevention group and the QUE treatment group significantly alleviated the above changes. Western blotting results showed that QUE pretreatment inhibited the expression of proteins related to the endoplasmic reticulum stress PERK (double-stranded RNA-dependent protein kinase ER kinase) signaling pathway and reduced the expression of caspase-3, a key protein in the apoptosis pathway.
【Conclusion】
Quercetin can reduce ALI in mice induced by LPS by inhibiting endoplasmic reticulum stress (ERS) and reducing oxidative stress and inflammation. The mechanism is related to the inhibition of PERK signaling pathway of ERS.

Application of Intact Mesentery Separation in Laparoscopic Appendectomy

【Objective】
To investigate the technique of intact mesenteric separation in laparoscopic appendectomy.
【Methods】
The data of 450 patients undergoing laparoscopic appendectomy from July 2016 to July 2018 were collected in our hospital, to summarize and analyze the operation time, intraoperative bleeding, postoperative complications, and hospital stays.
【Results】
All 450 patients successfully were completed by laparoscopic appendectomy under the intact mesangial separation technique. The operation time was 25-90min, with an average of 35min. The intraoperative bleeding was 0-10mL. The hospitalization time was 3-6 days. There were 6 cases of postoperative wound infection, and no stump fistula, abdominal abscess, postoperative bleeding and other complications occurred,the patients were followed up for 1-3 years, and no adhesive ileus occurred.
【Conclusion】
The intact mesentery separation technique is safe and effective, which can be applied in clinical practice.

Expression and Clinical Significance of IL-6, sFlt-1 and VEGF in Serum of Pregnant Women with Gestational Hypertension

【Objective】
To observe the levels of serum interleukin-6 (IL-6), soluble vascular endothelial growth factor receptor 1 (sFlt-1), vascular endothelial growth factor (VEGF) in patients with pregnancy-induced hypertension and the relationship between academic indicators.
【Methods】
A total of 80 cases of pregnant women with gestational hypertension in our hospital from January to December 2020 were selected as the observation group, and 80 cases of normal pregnant women in the same hospital during the same period were selected as the control group. The levels of serum IL-6, sFlt-1, VEGF and hemorheology were detected and compared between the two groups, and the correlation of each index and the influencing factors of gestational hypertension were analyzed.
【Results】
The levels of IL-6, sFlt-1, whole blood low shear reduced viscosity (LRV), plasma viscosity (PV) and fibrinogen (Fib) in the observation group were significantly higher than those in the control group, and the levels of VEGF in the observation group were significantly lower than those in the control group (all P<0.05); However, there was no significant difference in HRV and MRV between the two groups (P>0.05). IL-6 was positively correlated with LRV and PV (P<0.05), sFlt-1 was positively correlated with LRV, PV and Fib (P<0.05), VEGF was negatively correlated with LRV, PV and Fib (P<0.05). Logistic analysis showed that IL-6, sFlt-1, LRV, PV and VEGF were the risk factors of pregnancy induced hypertension (P<0.05).
【Conclusion】
The serum levels of IL-6 and sFlt-1 are increased, VEGF is decreased and hemorheological indexes are abnormal; There is correlation between IL-6, sFlt-1, VEGF and hemorheology indexes. Clinical intervention can be carried out according to the above indexes to ensure the safety of mother and infant.

Screening and Identifying the Hub Genes in Esophageal Squamous Cell Carcinoma Based on GEO and TCGA Data

【Objective】
To screen and identify the key pivotal genes of ESCC by bioinformatics methods based on transcriptome data of ESCC from geo and TCGA databases.
【Methods】
GSE17351 dataset and RNA-seq data of ESCC were downloaded from GEO and TCGA databases. Then Limma package of R software was applied to screen differentially expressed genes; ClusterProfiler package was used to perform GO analysis and KEGG Pathway enrichment; STRING tool was used to analyze the interaction network among proteins encoded by differential genes and the interactome was visualized by Cytoscape software; Cytoscape's Cytohubba plug-in was subjected to analyze key hub genes among interactome; The prognostic values of hub genes were analyzed by K-M plotter.
【Results】
A total of 167 differentially expressed genes were screened by GEO and TCGA; The differentially expressed genes were mainly related to GO processes such as mitosis, ATPase activity and DNA helicase activity, and were significantly enriched in DNA cell cycle, DNA replication, mismatch repair, oocyte meiosis, ECM receptor interaction, p53 and Wnt signaling pathways; A total of 10 key genes were screened, including BUB1,MAD2L1,NUF2,KIF18A,CENPK,CENPN,CENPQ and SPC25,which were negatively correlated with the prognosis of patients with ESCC.
【Conclusion】
Ten key pivotal genes of ESCC were screened, and eight of them were related to prognosis, which provided potential targets for molecular mechanism research and prognosis judgment of ESCC.

Risk Factors of Multiple Cerebral Infarction Complicated with Pneumonia in the Elderly

【Objective】
To investigate the risk factors with multiple cerebral infarction complicated with pneumonia in the elderly patients.
【Methods】
Forty-five elderly patients with multiple cerebral infarction complicated with pneumonia were enrolled in our hospital as the observation group, and 45 elderly patients with multiple cerebral infarction without pneumonia were selected as the control group. The basic clinical data, inflammation index, coagulation function and platelet parameters of the two groups were observed, and the related factors of multiple cerebral infarction complicated with pneumonia were analyzed.
【Results】
The age, number of infarctions, proportion of history of lung disease, and NIHSS scores in the observation group were higher than those in the control group (P<0.05). The levels of HS-CRP, IL-18,procalcitonin and SSA in the observation group were significantly higher than those in the control group (P<0.05). There was no significant difference in serum lipid levels between the two groups (P>0.05). The PDW (platelet distribution width) of the observation group was higher than that of the control group, while PCT and PLT were lower than those of the control group (P<0.05). There was no significant difference in coagulation function between the two groups (P>0.05). Multivariate analysis showed that platelet parameters, inflammatory markers, advanced age, and NIHSS scores were associated with pneumonia in the elderly patients with multiple cerebral infarction (P<0.05).
【Conclusion】
The elderly patients with multiple cerebral infarction complicated with pneumonia who are in aged, severe neurological deficits, high level of inflammatory response and platelet abnormalities need to be paid special attention clinically.

Role of TLRs-NF-κΒ Signaling Pathway in Children with Severe Pneumonia

【Objective】
To investigate the role of Toll-like receptor-nuclear transcription factor-κΒ (TLRS-NF-κΒ) signaling pathway in children with severe pneumonia.
【Methods】
A total of 120 children with severe pneumonia from April 2019 to December 2019 were selected as observation group; A total of 89 healthy children were selected as the control group. The levels of interleukin-6 (IL-6), interleukin-8 (IL-8), tumor necrosis factor-α (TNF-α) and C-reactive protein (CRP) in children were determined by enzym-linked immunosorbent assay (ELISA). The percentage of TLRS cells with restricted presence type (TLR2, TLR4) positive cells was determined by flow cytometry. The Pearson correlation analysis software was used to analyze the correlation between the positive rates of TLR2 and TLR4 and the levels of HMGB1, TNF-α and CRP.
【Results】
The levels of serum IL-6, IL-8, TNF-α and CRP in children with severe pneumonia in the observation group were higher than those in control group (P<0.05). The results of flow cytometry showed that the percentage of TLR2 positive cells in the observation group was (59.30±5.71) %, and the percentage of TLR4 positive cells in the observation group was (59.70±9.48) %, which were both higher than that in the control group (0.20±0.06) %, and the percentage of TLR4 positive cells was (0.40±09) % (P<0.05). Pearson correlation analysis showed that TLR4 and TLR2 in TLRS-NF-κΒ signaling pathway were positively correlated with the levels of inflammatory factors IL-6, IL-8, TNF-α and CRP in children with severe pneumonia (P<0.05).
【Conclusion】
TLRS-NF-κΒ signaling pathway is highly expressed in children with severe pneumonia, which is strongly correlated with inflammatory factors. Strengthening TLRS-NF-κΒ signaling pathway determination can guide clinical diagnosis and treatment.

Correlation between Expression of S-phase Kinase Associated Protein,p27kip1 Protein and Clinicopathological Features of Gastric Cancer

【Objective】
To investigate the relationship between the expression of S phase kinase-related protein (skp2) and p27kip1 protein in primary gastric cancer and the development of tumorigenesis.
【Methods】
A total of 60 cases of gastric cancer tissue samples obtained in our hospital were selected as gastric cancer group, and 60 cases of gastric cancer adjacent tissue samples were selected as control group. The positive expression rates of Skp2 and p27kip1 were detected by immunohistochemistry.The positive rates of Skp2 protein and p27kip1 Protein in gastric cancer tissues with different lesion size, histological grade, degree of invasion, lymph node metastasis and TNM stage were analyzed.
【Results】
The positive expression rate of Skp2 protein in the gastric cancer group was 68.33% (41/60), which was higher than 23.33% (14/60) in the control group; the positive expression rate of p27kip1 Protein in the gastric cancer group was 28.33% (17/60), which was lower than 73.33% (44/60) in the control group, and there was significant difference between the two groups (P<0.05); the positive expression rate of Skp2 protein in gastric cancer tissues with different TNM stages, lymph node metastasis, histological differentiation and tumor invasion was significantly different (P<0.05); the positive expression rate of Skp2 protein in gastric cancer tissues with different TNM stages and lymph node metastasis was significantly different (P<0.05).
【Conclusion】
The expression of Skp2 protein is up-regulated and p27kip1 Protein is down regulated in gastric cancer, which may be related to the occurrence and development of gastric cancer.

Clinical Value of Thoracoscopic Reduction and Percutaneous Intramedullary Nailing in the Treatment of Middle Clavicle Fracture

【Objective】
To investigate the clinical value of thoracoscopic reduction and percutaneous intramedullary nailing fixation in the treatment of middle clavicular fracture.
【Methods】
A total of 51 patients with middle clavicle fracture were retrospectively analyzed. According to different surgical methods, they were divided into the control group and the observation group. Patients in the control group (n=33) were treated with traditional manual reduction and percutaneous intramedullary nailing fixation, while 18 patients in the observation group were treated with thoracoscopic-assisted reduction and percutaneous intramedullary nailing fixation. The operation time, intraoperative blood loss, VAS score and fracture healing time of the two groups were compared. The clinical efficacy of the two groups was evaluated by constant Murley score 6 months after operation, and the complications of the two groups during the follow-up period were recorded.
【Results】
In the control group, 4 cases were difficult to reduce during the operation, thus they were temporarily changed to open reduction. Patients in both groups were followed up for 6-18 months, with an average follow-up time of (11.67±3.12) months. The operation time of the observation group was longer than that of the control group (P<0.05). There was no significant difference in the amount of blood loss, VAS score on the day after surgery, and fracture healing time between the two groups (all P>0.05). However, 6 months after surgery, the functional activity, pain, shoulder joint mobility and the scores of muscle strength in the two groups of patients were higher than those before operation (all P<0.05), but there was no statistically significant difference in the scores of the two groups of patients (P>0.05). Follow-up results showed that all patients received anatomical reduction and were healed by first intention. In the control group, 1 case (3.45%) had difficulty in removing intramedullary nail and 1 case (3.45%) had fracture nonunion,whose fracture was successfully healed after 9 months postoperatively with a locking plate revision. The rest of the patients in the two groups had no complications such as intramedullary nail breakage or fracture nonunion during the follow-up period, and all achieved bony union.
【Conclusion】
Although thoracoscopic-assisted reduction and intramedullary nailing internal fixation for the treatment of mid clavicle fracture has a long operation time, no fluoroscopy is needed during the operation, so radiation exposure is avoided; and the clinical effect is equivalent to that of traditional manual reduction and percutaneous intramedullary nail internal fixation.

Relationship of MMP-9, MIG and Fibroin A Expression with Prognosis of Colon Cancer

【Objective】
To investigate the relationship of matrix metalloproteinase-9 (MMP-9), migration-inducible protein-7 (MIG-7) and fibroin A (FLNa) expression with pathological factors and disease-free survival (DFS) in patients with colon cancer.
【Methods】
A total of 160 patients with colon cancer admitted to the Fourth People's Hospital of Shaanxi Province for surgery were selected. The postoperative colon cancer tissues was collected to detect MMP-9, MIG-7and FLNa expression by immunohistochemistry in cancer tissues and adjacent tissues. The correlation of MMP-9, FLNa and MIG-7 expression with colon cancer pathological factors and DFS was analyzed.
【Results】
Compared to colon cancer adjacent tissues, the expression levels of MMP-9 and MIG-7 were up-regulated (P<0.05), while the expression level of FLNa was down-regulated (P<0.05). Compared to patients with stage Ⅰ-Ⅱcolon cancer, non-lymph node metastasis and no vascular invasion, the expression levels of MMP-9 and MIG-7 in the tissues of colon cancer patients with stage Ⅲ, lymph node metastasis, and vascular invasion were up-regulated (P<0.05), and the expression of FLNa was down-regulated (P<0.05). The median follow-up time was 18.9 months. There were 25 relapses, 20 metastases, and 9 deaths. COX multivariate analysis showed that MMP-9, MIG-7, FLNa, TNM staging and lymph node metastasis were independent influencing factors of DFS in colon cancer patients (P<0.05).
【Conclusion】
The expression levels of MMP-9 and MIG-7 are up-regulated in colon cancer, and the expression of FLNa is down-regulated. These gene expression levels are related to pathological factors and DFS in colon cancer patients.

Effect of High-purity Urinary Gonadotropin Combined with Letrozole in the Treatment of Infertility Patients with Polycystic Ovary Syndrome

【Objective】
To investigate the effect of high-purity urinary tropulin (Hp-HMG) combined with letrozole in the treatment of infertility patients with polycystic ovary syndrome.
【Methods】
Eighty infertility patients with polycystic ovary syndrome who were treated in our hospital from January 2016 to October 2020 were enrolled. The patients were divided into Hp-HMG plus letrozole group and Hp-HMG group according to the touch ball method. Hp-HMG group received Hp-HMG 75-150 U/d treatment (for 7 consecutive days), Hp-HMG plus letrozole group received letrozole 2.5 mg/d (for 3 consecutive days) plus Hp-HMG 75-150 U /d (for 4 consecutive days). The blood lipid levels, uterus and follicular conditions, ovulation, pregnancy, and adverse reactions were observed.
【Results】
There was no significant difference in serum lipid levels between the two groups before treatment (P>0.05). After treatment, the levels of TC and TG in Hp-HMG plus letrozole group were significantly lower than those in the Hp-HMG group (P<0.05). There was no significant difference in LDL and HDL between the two groups (P>0.05). There were no significant differences in the thickness of endometrium and uterine volume between the two groups before and after treatment (P>0.05). After treatment, the number of mature follicles, the average diameter of mature follicles, the maximum follicle diameter, the number of 10-14 cm follicles, and the number of >14 cm follicles in Hp-HMG plus letrozole group were higher than those in Hp-HMG group (P<0.05). The number of ovulation and the clinical pregnancy rate of Hp-HMG plus letrozole group were higher than those of Hp-HMG group, the difference was statistically significant (P<0.05). There were no ovarian hyperstimulation syndrome, unruptured follicular luteinization syndrome and abnormal blood routine in both groups. One case of redness was found in the Hp-HMG group.
【Conclusion】
Hp-HMG combined with letrozole is safe in the treatment of polycystic ovary syndrome infertility, and can improve ovulation rate and pregnancy rate and improve blood lipid levels.

Relationship of RIZ1 and CD133 Expression Levels with Development of Serous Ovarian Cancer

【Objective】
To investigate the expression and clinical significance of retinoblastoma protein-binding zinc finger structure gene (RIZ1) and 5-time transmembrane protein (CD133) in serous ovarian cancer tissues.
【Methods】
We selected 70 cases of ovarian cancer patients (ovarian cancer group) and 70 cases of normal ovarian tissue obtained from surgery of uterine fibroids in our hospital from January 2017 to June 2018 (the control group).Expression levels of RIZ1 protein and CD133 protein in the two groups were detected by immunohistochemical staining and analyzed hierarchically according to histological grade, FIGO stage and lymph node metastasis.
【Results】
The positive expression rates of RIZ1 protein and CD133 protein in ovarian cancer tissues were 32.86%(23/70)and 51.43%(36/70), respectively. The positive expression rates of RIZ1 protein and CD133 protein in the control group were 85.71%(60/70) and 11.43%(8/70), respectively. The difference between the two groups was statistically significant (P<0.05). The difference in positive expression rate of RIZ1 protein in ovarian cancer tissues of patients with different histological differentiation was statistically significant (P<0.05). However, the difference in positive expression rate of RIZ1 protein in ovarian cancer patients with different FIGO stages and lymph node metastasis was not statistically significant (P>0.05). The differences in positive expression rate of CD133 in ovarian cancer tissues of patients with different histological differentiation, different FIGO stages, and whether lymph node metastasis occurred were statistically significant(P<0.05).
【Conclusion】
The expression of RIZ1 protein is down-regulated and the expression of CD133 protein is up-regulated in serous ovarian cancer tissues. These are closely related to the occurrence and development of tumors.

Expression and Significance of miR-125b in Ovarian Cancer Cells

【Objective】
To investigate the effect and mechanism of miR-125b on energy metabolism and proliferation in ovarian cancer cells.
【Methods】
Real-time PCR was used to detect the expression level of miR-125b in human ovarian epithelial cells and ovarian cancer cells. The proliferation of ovarian cancer cells was recorded by CCK-8. Glucose uptake and lactic acid production in ovarian cancer cells were measured by

H-2DG) method and lactic acid quantitative kit. Western blot was performed for the expression of hexokinase 2 (HK2) in ovarian cancer cells after miR-125b-mimics overexpression.
【Results】
The real time PCR results showed that the expression level of miR-125b in ovarian cancer cells was significantly lower than that in normal ovarian epithelial cells (P<0.05). The CCK-8 results demonstrated that the proliferation capacity of ovarian cancer cells was significantly reduced after transfected with miR-125b-mimics for 48h and 72h compared with the control group (P<0.05). The quantitative results of

H-2DG and lactic acid showed that glucose intake and lactic acid production were significantly reduced after overexpression of miR-125b in ovarian cancer cells (P<0.05); and aerobic glycolysis was significantly inhibited. Western blot results showed that the expression level of HK2 protein decreased after overexpression of miR-125b in ovarian cancer cells (P<0.05).
【Conclusion】

Prognostic Value of Expression of LncRNA MINCR in Patients with Decompensated Cirrhosis

【Objective】
To investigate the prognostic value of expression of LncRNA MINCR in patients with decompensated cirrhosis.
【Methods】
From January 2017 to January 2018, 120 patients with decompensated cirrhosis were selected and divided into survival group and death group according to the follow-up survival status. The relative expression of LncRNA MINCR was detected by real-time fluorescence quantitative polymerase chain reaction (qRT-PCR). Pearson method was used to analyze the correlation between LncRNA MINCR and the clinical characteristics of patients with cirrhosis. ROC was used to analyze the diagnostic performance of each index for the prognosis of decompensated cirrhosis. Cox regression analysis was used to analyze the independent risk factors influencing the prognosis of patients.
【Results】
Pearson analysis showed that the expression level of LncRNA MINCR was positively correlated with TBIL, INR, CTP and MELD (P<0.05), but not with ALB, Cr and Na (P>0.05). ROC curve analysis showed that the best critical point of LncRNA MINCR for predicting death in patients with cirrhosis was 2.32, the sensitivity was 96.71%, the specificity was 90.50%, and the area under AUC curve was 0.973. Multivariate Cox regression analysis showed that LncRNA MINCR was an independent risk factor for death in decompensated cirrhosis (HR:1.13;95%CI:1.07-1.19).
【Conclusion】
LncRNA MINCR has a certain value in judging the condition and prognosis of patients with decompensated cirrhosis. The method is simple and easy to use, which is worth popularizing.

Comparison of Curative Effect of Different Viscosities of Cement Cement Vertebroplasty on Osteoporotic Vertebral Compression Fractures in Elderly Patients

【Objective】
To investigate the effect of cementum vertebroplasty with different viscosities on osteoporotic vertebral compression fractures in elderly patients .
【Methods】
A total of 97 elderly patients with osteoporotic vertebral compression fractures treated in our hospital were selected and divided into observation group (n=54) and control group (n=43) according to the viscosity of the bone cement used during the operation. The control group received low-viscosity bone cement percutaneous vertebroplasty (PVP), and the observation group received high-viscosity bone cement PVP. The clinical and radiological evaluation indexes, bone metabolism indexes, bone cement dosage, leakage rate, and other complications were compared between the two groups of patients.
【Results】
Comparison between two groups of clinical efficacy assessment indicators VAS score, ODI index, SF-36 score and imaging efficacy assessment indicators Cobb angle of kyphosis and anterior edge height of the vertebral body, The preoperative difference was not significant (P>0.05), The difference was significant at 1 week, 3 months and half a year after operation (P<0.05); Within the group, the differences were significant at 1 week, 3 months, and 6 months after operation (P<0.05). The comparison between the two groups of BALP, BGP, and CTX groups was not significant before surgery (P>0.05), and the difference at 3 months after surgery was significant (P<0.05). Within the group, the difference between 3 months and before surgery was significant (P<0.05). There was no difference in the amount of bone cement injected between the two groups of single vertebral body and double vertebral body (P> 0.05). The leakage rate of bone cement in the observation group was 9.26% (5/54) lower than that in the control group by 25.58% (11/43) (P<0.05). The incidence of other complications in the observation group was 11.11% (6/54) and the control group was 23.26% (10/43). The difference was not significant (P>0.05).
【Conclusion】
High-viscosity bone cement PVP for osteoporotic vertebral compression fractures in elderly patients has a lower leakage rate than low-viscosity bone cement, which can significantly relieve pain, improve vertebral function, improve quality of life, and improve bone metabolism. Study on Effects of Sevoflurane in Radical Resection of Lung Cancer and Its Influence on Vascular Endothelial Growth Factor, COX-2, MMP-2 and MMP-9

【Objective】
To investigate the effect of sevoflurane in radical resection of lung cancer and its influence on the levels of vascular endothelial growth factor, COX-2, MMP-2 and MMP-9 in patients with lung cancer.
【Methods】
A total of 80 patients undergoing radical resection of lung cancer in our hospital were divided into the control group and the observation group. The control group was given isoflurane inhalation anesthesia, while the observation group was given sevoflurane inhalation anesthesia. The anesthesia effect of the two groups was observed, and the differences of blood levels of vascular endothelial growth factor, COX-2, MMP-2 and MMP-9 between the two groups during perioperative period were compared.
【Results】
There was no difference in preoperative immune function between the two groups. Two hours after operation, the levels of CD3

in the observation group were lower than those in the control group, while the level of CD8

was higher than that in the control group (P<0.05). The levels of vascular endothelial growth factor, COX-2, MMP-2 and MMP-9 in the observation group at 2 hours after operation were lower than those in the control group. The VAS scores of patients in the observation group at 4 hours and 12 hours after operation were 1.48±0.35) and 1.34±0.27), respectively, which were lower than those in the control group (2.11±0.42 and 2.15±0.55,respectively) (P<0.05). There was no significant difference in Ramsay scores between the two groups (P>0.05). The satisfaction rate of patients in the observation group was 97.50%, which was significantly higher than that of patients in the control group (82.50%).The difference between the two groups was significant (P<0.05).
【Conclusion】
Sevoflurane is effective in radical resection of lung cancer, and has little effect on the levels of vascular endothelial growth factor, COX-2, MMP-2 and MMP-9. It has good clinical application.

【Objective】
To investigate the effect of lumbar plexus sciatic nerve block combined with low-dose propofol anesthesia on hemodynamics, postoperative cognitive function and complications in elderly patients with intertrochanteric fracture.
【Methods】
Sixty patients with intertrochanteric fracture in our hospital from September 2017 to September 2019 were selected and randomly divided into observation group and control group (30 cases in each group). The control group was operated under general anesthesia, and the observation group was treated with lumbar plexus sciatic nerve block combined with low-dose propofol anesthesia. The hemodynamic indexes [heart rate (HR) and mean arterial pressure (MAP)], postoperative cognitive function [Montreal Cognitive Assessment Scale (MoCA), Mini Mental State Examination (MMSE) score], anesthetic effect and complications of the two groups were compared.
【Results】
The MAP and HR index levels of the two groups at T1 were significantly lower than those at T0, but the map level of the control group at T
1
was significantly lower than that of the observation group, and the difference was statistically significant (P>0.05); The excellent and good rate of anesthesia effect in the observation group was significantly higher than that in the control group (86.67% vs 60.00%), and the differences were statistically significant (P<0.05). The MoCA and MMSE scores of the two groups at 1 day and 3 days after operation were lower than those before operation, but the MoCA and MMSE scores of the observation group were significantly higher than those of the control group. The incidence of complications in the observation group was significantly lower than that in the control group (16.67% vs 40.33%), and the differences were statistically significant (P<0.05).
【Conclusion】
Lumbar plexus sciatic nerve block combined with low-dose propofol anesthesia for elderly patients with intertrochanteric fracture has the advantages of stable hemodynamics and significant anesthetic effect, which can effectively reduce the occurrence of postoperative cognitive dysfunction and postoperative complications.

Effect of Transverse Abdominal Fascia Block on Postoperative Pain, Liver and Kidney Function in Patients with Hepatocellular Carcinoma

【Objective】
To investigate the effect of transverse abdominal fascia block on postoperative pain and liver and kidney function in patients with liver cancer.
【Methods】
A total of 90 patients with liver cancer who received surgical treatment in our hospital from July 2017 to July 2020 were selected and randomly divided into observation group (n=45) and control group (n=45). The control group was given rest combined with general anesthesia, and the observation group was given transverse abdominal fascia block.Visual analogue scale (VAS), C-reactive protein (CRP) and tumor necrosis factor -α(TNF-α),Interleukin-6 (IL-6), aspartate aminotransferase (AST), alanine aminotransferase (ALT), albumin (ALB), blood urea nitrogen (BUN) and serum creatinine (SCR) were measured and compared between the two groups at different time points after treatment ; The incidence of liver insufficiency, ascites, and wound healing were recorded.
【Results】
The VAS scores at 4 h, 8 h, 12 h and 24 h after operation in the observation group were lower than those in the control group (P<0.05); The levels of CRP and TNF-αand IL-6 in the two groups immediately after operation were higher than those before operation; The levels of CRP, TNF -αand IL-6 at 4 h and 24 h after operation in the observation group were significantly lower than those in the control group. The expression levels of AST and ALT in the two groups immediately after operation, 4 h and 24 h after operation were higher than those before operation, and the observation group was lower than the control group, the differences were statistically significant (P<0.05); There was no significant difference in ALB expression between the two groups at each time point (P>0.05). BUN and SCr were increased and then decreased in the two groups immediately after operation, 4 h after operation and 24 h after operation, and the expression level in the observation group was lower than that in the control group (P<0.05). During the treatment, the total incidence of liver insufficiency, ascites, wound healing and other complications in the observation group was lower than that in the control group (6.67% vs 26.67%), the difference was significant (P<0.05).
【Conclusion】
Transverse abdominal fascia block has significant analgesic effect in patients with liver cancer surgery, which can reduce the level of inflammatory cytokines, protect the liver and kidney function of patients, and is conducive to the rapid recovery of patients after surgery.

Correlation of Serum hs CRP and sST2 with the Condition and Prognosis of Patients with Acute Coronary Syndrome

【Objective】
Objective to investigate the correlation between serum high sensitivity C-reactive protein (hs CRP), soluble growth stimulating gene 2 protein (sST2) and the condition and prognosis of patients with acute coronary syndrome (ACS).
【Methods】
Sixty patients with ACS were selected as ACS group, and 60 healthy people were selected as control group. The serum hs CRP and sST2 levels of the two groups were detected, and the receiver operating characteristic curve (ROC) was drawn to analyze their predictive value for ACS. Pearson linear correlation was used to analyze the correlation between serum hs CRP, sST2 levels and grace. According to the prognosis of ACS patients, they were divided into good prognosis group and poor prognosis group. The serum hs CRP and sST2 levels of the two groups were compared, and the predictive value of the two groups for the prognosis of patients was analyzed.
【Results】
The serum hs CRP and sST2 in ACS group were higher than those in control group (P<0.05). The area under the curve of serum hs CRP and sST2 in predicting ACS was 0.648 and 0.746 respectively. When hs CRP>14.650 mg / L and sST2 > 15.275 ng / ml, the risk of ACS was higher. The grace score of ACS patients was (125.32±34.76). Pearson linear correlation analysis showed that serum hs CRP and sST2 were positively correlated with grace score (all P<0.05). Of the 60 patients with ACS, 15 had poor prognosis and 45 had good prognosis. The serum hs CRP and sST2 in poor prognosis group were significantly higher than those in good prognosis group (P<0.05). The area under the curve of serum hs CRP and sST2 in predicting poor prognosis of ACS was 0.676 and 0.694 respectively. When hs CRP>15.420 mg/L and sST2 > 23.500 ng/mL, the risk of poor prognosis of ACS was higher.
【Conclusion】
Serum hs CRP and sST2 have a certain predictive value for ACS. They are positively correlated with grace score of ACS patients, and can be used as important indicators to predict the prognosis of ACS patients.

Effect of Complement C4 Level on Treatment of Idiopathic Membranous Nephropathy

【Objective】
To investigate the curative effect and predictive value of baseline complement C4 level in the treatment of idiopathic membranous nephropathy (IMN).
【Methods】
A total of 132 patients with IMN who were admitted to our hospital were treated with cyclophosphamide + glucocorticoids. Their clinical and biochemical data were collected at admission. The patients were followed up. According to the treatment effect, patients were divided into the remission group and the non-remission group. The predictive value of baseline complement C4 level on remission rate was analyzed, and the influencing factors of disease remission were analyzed as well.
【Results】
The median follow-up time was 25 months and the average follow-up time was (24.1±3.9) months. A total of 102 patients (77.3%) achieved disease remission and 30 patients did not. ROC curve analysis showed that the baseline complement C4 level predicting area under the curve of IMN disease remission was 0.76 (P=0.005), the sensitivity was 65.2%, and the specificity was 79.8%. Disease remission was associated with C3, C4, urine protein, albumin and pathological stage (P<0.05). Logistic analysis showed that C4, albumin and pathological staging were independent influencing factors of disease remission in IMN patients (P<0.05). On admission, patients with higher C4 level, lower albumin and stageⅢ-Ⅳ IMN had lower disease remission rate after treatment.
【Conclusion】
Baseline complement C4 level has predictive value for IMN treatment effect. IMN patients with elevated C4 on admission have a lower rate of disease remission after treatment and should be given clinical attention.

Influence of Empagliflozin Combined with Metformin on IL-17A, IL-22, IFN-γ Levels in Elderly Patients with Type 2 Diabetes

【Objective】
To investigate the efficacy of empagliflozin combined with metformin in the treatment of elderly patients with type 2 diabetes (T2DM) and their effects on levels of blood glucose, interleukin-17A (IL-17A), IL-22, and interferon-γ (IFN-γ).
【Methods】
A total of 108 elderly patients with type 2 diabetes in our hospital from January 2020 to July 2020 were selected and divided into the observation group (n=54) and the control group (n=54) based on a simple random number table method. On the basis of routine intervention, patients in the control group received metformin alone for 3 months' treatment, while patients in the observation group received both metformin and empagliflozin for 3 months. The clinical efficacy, blood glucose indicators [fasting blood glucose (FPG), 2 h postprandial blood glucose (2hPG), glycosylated hemoglobin (HbA1c)], islet function indicators [fasting insulin (FINS), 2 h postprandial insulin ( PINS), insulin resistance index (HOMA-IR), pancreatic islet β cell function index (HOMA-β)],IL-17A, IL-22, IFN-γ levels, blood lipids [low density lipoprotein cholesterol (LDL-C), High-density lipoprotein cholesterol (HDL-C), triacylglycerol (TG), total cholesterol (TC)], blood pressure [diastolic blood pressure (DBP), systolic blood pressure (SBP)], BMI before and after treatment, and the incidence of adverse reactions were calculated and compared between the two groups.
【Results】
The total effective rate of the observation group (94.44%) was higher than that of the control group (81.48%) (P<0.05). The levels of FPG, 2hPG, HbA1c HOMA-IR, IL-17A, IL-22, IFN-γ, LDL-C, TG, TC, DBP, and SBP in the observation group were lower than those in the control group after 1 month and 3 months of treatment (P<0.05); While after 1 month and 3 months of treatment, the levels of HOMA-β, FINS, HDL-C and PINS in the observation y group were higher than those in the control group, the differences were all statistically significant (P<0.05). There was no significant difference in the incidence of adverse reactions between the observation group (9.26%) and the control group (5.56%) (P>0.05).
【Conclusion】
Metformin combined with empagliflozin in the treatment of elderly type 2 diabetes can effectively reduce blood glucose, improve islet function, regulate blood lipids and blood pressure, reduce BMI, improve therapeutic effects, and down-regulate serum IL-17A, IL-22. It is worthy of clinical application.

Expression of Serum FGF-21 and Apelin in Pregnant Women with Hypothyroidism and Correlation with Blood Lipids and Low Birth Weight Neonates

【Objective】
To investigate the expression of serum fibroblast growth factor 21 (FGF-21) and Apelin in pregnant patients with hypothyroidism and correlation with blood lipids and low-birthweight neonates.
【Methods】
From January 2018 to June 2019, 85 patients with hypothyroidism during pregnancy were selected as the observation group, and 90 normal pregnant women were selected as the control group. Thyroid hormone levels, blood lipid levels, serum FGF-21, Apelin expression levels were compared between the two groups, and the correlation of serum FGF-21 and Apelin expression levels with blood lipid and low birth weight was analyzed.
【Results】
The TSH level of the observation group was significantly higher than that of the control group (P<0.05), while the levels of FT3 (free triiodothyronine) and FT4 (free thyroxine) were significantly lower than those in the control group (P<0.05). The levels of FGF-12, Apelin, TG, TC and HDL in the observation group were significantly higher than those in the control group (P<0.05), and the level of LDL in the observation group was significantly lower than that in the control group (P<0.05). The incidence of low birth weight newborns in the observation group was 2.35% (2/85), which was lower than 10.00%(9/90) in the control group. 【Conclusion】
Serum Apelin and FGF-21 expression levels increased in patients with hypothyroidism during pregnancy, and the expression levels of Apelin and FGF-21 were positively correlated with lipid metabolism indicators such as TG and TC. It may participate in the occurrence and development of the disease by affecting the patient's metabolic level, and ultimately affect the perinatal outcome.

Correlation Analysis Between Serum and Colonic Mucosal IL-10,TNF-α,Tyrosin and Sealin-1 in Different Types of Irritable Bowel Syndrome

【Objective】
To study the correlation between serum and colonic mucosal IL-10, TNF-α, tyrosin (PYY) and sealin-1 in different types of irritable bowel syndrome (IBS).
【Methods】
The data of 90 patients with IBS, according to the symptomatic types, were divided into the diarrhea group (n=50) and the constipation group (n=40). In the same period, 40 healthy people were selected as the control group. The levels of serum and colonic mucosa IL-10, TNF-α, tyrosin and sealin-1 were compared among the three groups. The correlation of IL -10,TNF-α, PYY and sealin-1 in colonic mucosal and serum was analyzed.
【Results】
Levels of IL-10 and sealin-1 in the control group were higher, while levels of TNF-α and PYY in the control group were lower than those in the diarrhea group (P<0.05). IL-10 and sealin-1 levels in the diarrhea group were lower than those in the constipation group; and TNF-α and PYY levels in the diarrhea group were higher than those in the constipation group (P<0.05). There was no significant difference in TNF-α level between the control group and the constipation group (P>0.05). The positive expression rates of IL-10 in the colonic mucosa of the control group, diarrhea group and constipation group were 37.50% (15/40), 24.00% (12/50), and 55.00% (22/40), respectively; The difference was statistically significant (P<0.05). The positive expression rate of TNF-α in the colonic mucosa of the control group was 22.50% (9/40), which was significantly lower than that in the diarrhea group by 68.00% (34/50) (P<0.05). the positive expression rate of PYY in the colonic mucosa of the control group was 27.50% (11/40), which was significantly lower than that in the diarrhea group (76.00%, 38/50) (P<0.05). The positive expression rate of TNF-α and PYY in the colonic mucosa of the control group was not statistically significant compared to the constipation group (P>0.05). 【Conclusion】
In patients with IBS, there is an imbalance between anti-inflammatory factors and pro-inflammatory factors. It can prevent or control the development of IBS by regulating the expression of PYY and sealin-1, thereby reducing the inflammatory response and promoting clinical efficacy.

The randomized digital table method was divided into 50 cases in the study group and the control group. The study group collected surgical bleeding and returned. Allogeneic blood was added when blood transfusion was insufficient; allogeneic blood transfusion was only used in the control group; prothrombin time (PT), activated partial thrombin time (APTT), and thrombin time were compared between the two groups before surgery, 1 day after surgery, and 3 days after surgery. (TT), plasma D-dimer (DD), fibrinogen (FIB), pH, hemoglobin (Hb), hematocrit (Hct), platelet (PLT).
【Results】
After repeated measures of variance analysis, the PT, APTT, TT, FIB, and DD values of the study group and the control group were compared at three time points before, 1 day, and 3d after surgery. Statistically significant (P<0.05); the PT, APTT, and TT 、 FIB values in the study group were lower than those in the control group (P<0.05), and the DD values were higher than the control group (P<0.05). P<0.05); The pH, Hct, and PLT measurements of the study group and the control group were statistically significant at the three time points before, 1 day, and 3d after surgery(P<0.05); The measured values of pH, Hct and PLT in the study group were higher than those in the control group at 1 day after operation and 3 days after operation (P<0.05). The complication rate of the study group was 4.00% lower than that of the control group. 16.00%, the difference was statistically significant (P<0.05).
【Conclusion】
Autologous blood transfusion in patients with craniocerebral trauma surgery is beneficial to reduce the impact on coagulation function and has high safety.

Analgesic Effect of Selective Cyclooxygenase-2 Inhibitor on Type I Incision in Laparoscopic Cholecystectomy

【Objective】
To study the value of selective cyclooxygenase-2 inhibitors in perioperative patients undergoing laparoscopic cholecystectomy with type I incision.
【Methods】
A total of 84 patients with gallstone underwent laparoscopic cholecystectomy type I incision were selected. According to the different methods of perioperative pain relief, they were divided into observation group and control group, 42 cases respectively. The patients in the observation group were treated with selective cyclooxygenase-2 inhibitors (celecoxib and perrickib sodium) during the perioperative period. The control group took routine analgesia, compared the postoperative pain, the expression of COX-2, PGE

and the incidence of adverse reactions.
【Results】
At 12 h, 24 h and 48 h after operation, the VAS score of the two groups was significantly higher than that before operation, and the observation group was lower than that of the control group, the differences were statistically significant (P<0.05). Correlation and Clinical Significance of Serum Levels of Ischemia Modified Albumin and Monocyte Chemoattractant Protein 1 with Gensini Score in Patients with Acute Coronary Syndrome

【Objective】
To study the correlation between the changes of serum ischemia modified albumin (IMA), monocyte chemoattractant protein-1 (MCP-1) and Gensini score in patients with acute coronary syndrome (ACS).
【Methods】
98 patients with ACS in our hospital from May 2018 to may 2019 were selected, and their serum IMA and MCP-1 levels were detected to evaluate Gensini score. The general information, serum IMA, MCP-1 levels and Gensini score of patients with poor prognosis and good prognosis were compared. The correlation between serum IMA, MCP-1 levels and Gensini score, the influencing factors of poor prognosis and the predictive value of combined detection for poor prognosis of patients with ACS were analyzed.
【Results】
TC, TG, LDL-C, HDL-C, IMA, MCP-1 and Gensini scores of patients with poor prognosis were higher than those of patients with good prognosis (P<0.05); Spearman correlation analysis showed that serum IMA, MCP-1 levels of ACS patients were positively correlated with Gensini score (P<0.05); logistic multiple regression analysis showed that elevated serum IMA and MCP-1 were risk factors for poor prognosis of ACS patients (P<0.05); ROC curve showed that serum IMA and MCP-1 levels of ACS patients were positively correlated with Gensini score (P<0.05) The AUC, sensitivity and specificity of combined detection were 0.865, 82.70% and 86.74% respectively, which were higher than those of single detection of IMA and MCP-1 (P<0.05).
【Conclusion】
the serum levels of IMA and MCP-1 in ACS patients are significantly increased, which is closely related to Gensini score. Combined detection can improve the diagnostic efficiency, and has important clinical application value.

Comparison of PVP and PKP in the Treatment of OVCF

【Objective】
To compare the clinical efficacy of percutaneous kyphoplasty (PKP) and percutaneous vertebroplasty (PVP) in the treatment of osteoporotic vertebral compression fractures (OVCF).
【Methods】
A total of 76 patients with OVCF were divided into the PKP group (41 cases) and the PVP group (35 cases) according to different operation methods. Clinical efficacy was compared between the two groups.
【Results】
Compared to before operation, the height of anterior vertebral body in both groups were higher after operation, and the kyphosis Cobb angle and visual analogue scale (VAS) were lower(P<0.05). Compared to before treatment, the Oswestry disability index (ODI) scores of the two groups decreased significantly 1 month after treatment, and the Japanese Orthopaedic Association (JOA) low back pain scores increased significantly (P<0.05). Compared to the PVP group, ODI scores of the PKP group decreased significantly, and the JOA scores increased significantly (P<0.05). Compared to the PVP group, the PKP group had higher anterior vertebral height, lower kyphosis Cobb angle, lower bone cement leakage rate, and higher bone cement injection volume and average treatment cost (P<0.05). There was no significant difference in the incidence of adjacent vertebral fracture and postoperative VAS score between the two groups (P>0.05).
【Conclusion】
Both PKP and PVP can safely and effectively improve the clinical efficacy of OVCF patients and relieve the pain of patients. Compared to PVP, PKP has higher treatment cost, but it can better restore the vertebral shape, improve the vertebral height, and reduce the occurrence of bone cement leakage.

Diagnostic Value of IgG, IgE and CA19-9 in Autoimmune Pancreatitis

【Objective】
To detect the levels of IgG, IgE and CA19-9 in patients with AIP, and to evaluate the diagnostic value of single and combined detection for AIP.
【Methods】
From January 2016 to June 2020, 112 patients with pancreatitis in our hospital were selected, including 54 patients with acute pancreatitis (AP group), 23 patients with chronic pancreatitis (CP Group), 35 patients with type 1 AIP (AIP group), and 30 healthy volunteers (control group) in our hospital during the same period, The sensitivity and specificity of IgG4, IgE and CA19-9 in the diagnosis of AIP were detected by drawing the receiver operating curve.
【Results】
There was no significant difference in serum IgG1, IgG2 and IgG3 levels among the four groups (P>0.05); The serum IgG4 levels of the four groups were significantly higher than those of the control group (P<0.05). There were significant differences in serum IgG, CA19-9 and IgE levels among the four groups (P<0.05). The serum IgG levels in the AP group, the CP Group and the AIP group were higher than those in the control group, and the Serum CA19-9 and IgE levels in the AIP group were higher than those in the AP group, the CP Group and the control group. ROC curve was drawn. The results showed that the sensitivity and specificity of combined detection of IgG4, IgE and CA19-9 were 87.3% and 94.1% respectively, which were higher than those of single detection of each index.
【Conclusion】
The levels of IgG4, IgE and CA19-9 in AIP patients were higher than those in AP and CP patients. The combined detection of the three can improve the sensitivity and specificity in the diagnosis of AIP.

Effect Analysis of Dynamic Blood Glucose Monitoring Combined with CSII in the Treatment of Elderly Brittle Diabetes Mellitus

【Objective】
To analyze the effect of dynamic blood glucose monitoring combined with continuous subcutaneous insulin injection (CSII) in the treatment of elderly brittle diabetes mellitus.
【Methods】
A total of 94 elderly patients with brittle diabetes mellitus in our hospital were enrolled between September 2018 and September 2020. They were randomly divided into the control group and the observation group with 47 cases in each group by the random number table method. All patients were treated with CSII to control blood glucose. On this basis, patients in the control group received eight-stage fingertip blood glucose monitoring while patients in the observation group received dynamic blood glucose monitoring. The clinical efficacy, blood glucose compliance (time of blood glucose compliance, insulin dosage), blood glucose steady-state indicators before treatment and after 7 d of treatment [standard deviation of blood glucose (SDBG), largest amplitude of glycemic excursions (LAGE), mean of daily differences (MODD), fasting plasma glucose coefficient of variation (FPG-CV), postprandial glucose excursion (PPGE)] and occurrence of hypoglycemia were compared between the two groups.
【Results】
After 7 days of treatment, the total effective rate was 97.87% (46/47) and 78.72%(37/47), respectively, in the observation group and in the control group. The total effective rate of treatment in observation group was higher than that in control group (P<0.05) and the difference was statistically significant (χ
2
=8.340,P=0.004). The time of blood glucose compliance and insulin dosage in observation group were 3.53±1.14 d and 32.83±3.56 U/d, respectively, which were lower than those in control group (6.39±1.35 d and 41.02±3.76, respectively). The differences were statistically significant (t=11.097, 10.844, P<0.05). The SDBG, LAGE, MODD, FPG-CV and PPGE after 7 d of treatment in the two groups were decreased compared to those before treatment, and the above indexes of observation group were lower than those of control group (P<0.05). The incidence rates of hypoglycemia events and unperceived hypoglycemia events in observation group were 2.02±0.65 times/person and 0.54±0.19 times/person, which were lower than those in control group (4.31±1.09 times/person and 1.70±0.53 times/person, perspectively) (P<0.05).
【Conclusion】
Dynamic blood glucose monitoring combined with CSII has a definite efficacy on elderly brittle diabetes mellitus. It can improve blood glucose fluctuations and reduce hypoglycemia events.

Clinical Analysis of Laparoscopic Radical Orchiectomy in the Treatment of Testicular Tumors

【Objective】
To assess the feasibility and outcomes of laparoscopic radical orchiectomy in the management of testicular tumor.
【Methods】
From October 2019 to September 2020, 7 patients with testicular tumor in our hospital were retrospectively analyzed. All patients successfully underwent laparoscopic radical orchiectomy under general anesthesia. The peritoneum was incised above the inner ring, the vas deferens and spermatic cord were ligated respectively, the spermatic cord was pulled out and the testis was removed, and the peritoneum incision was sutured. The operation time, hospital stay and complications were recorded. The levels of tumor markers including alpha fetoprotein (AFP), human chorionic gonadotropin (HCG) and lactate dehydrogenase (LDH) were compared before and after operation.
【Results】
The average operation time of 7 young patients was (103.9±25.8) min, and the average length of stay was (6.6±1.9) d, intraoperative blood loss was (12.1±5.7)mL. According to Clavien Dindo evaluation, there were 2 cases of grade I postoperative complications, including 1 case of subcutaneous emphysema during operation, which subsided spontaneously on the second day after operation, 1 case of postoperative wound pain, which needed analgesic treatment, and no grade Ⅱ-Ⅴ complications were found. There were no postoperative complications such as wound infection, urinary tract infection, abdominal pain, inguinal hernia and intestinal obstruction. The average follow-up time was (6.1±4.7) months, no local tumor recurrence and distant metastasis. There was significant difference in HCG level before and after operation (P<0.05), but no significant difference in AFP and LDH level (P>0.05).
【Conclusion】
Laparoscopic radical orchiectomy is technically feasible, safe, and effective.

Application of Combined Thoracic and Laparoscope in Elderly Esophageal Cancer and Observation of Short-Term Postoperative Recovery

【Objective】
To study the application and short-term postoperative recovery of thoracic and laparoscopic combined treatment for elderly patients with esophageal cancer.
【Methods】
Seventy patients aged 60 to 80 who were treated with esophageal cancer in our hospital from July 2016 to July 2018 were selected and randomly divided into observation group and control group with 35 patients in each group. The observation group was treated with chest and laparoscope combined treatment, while the control group was treated with traditional surgery, The operation time, intraoperative bleeding volume, number of lymph node dissection, extubation time and hospitalization time were compared between the two groups. The 1-year survival was compared between the two groups, and the T cell population in venous blood was measured before and after operation to analyze the immune function of the two groups. Comparisons of postoperative complications.
【Results】
The amount of bleeding during operation in the observation group was significantly less than that in the control group (P<0.05), the time of extubation after operation was significantly lower than that in the control group (P<0.05), and the length of hospitalization after operation was significantly shorter than that in the control group (P<0.05).
