Effect of Sirt1 agonists SRT1720 on Mice with Acute Lung Injury

【Objective】
To observe the therapeutic effect of Sirt1 agonist SRT1720 on lipopolysaccharide (LPS)-induced acute lung injury (ALI) in mice.
【Methods】
Adult male C57bl/6 mice were randomly divided into control group, SRT1720 group, ALI group and ALI+SRT1720 group (8 mice in each group). Mice were injected intraperitoneally with LPS (10 mg/kg) to induce ALI model. Mice in the SRT1720 group and in the ALI+SRT1720 group were intraperitoneally injected with SRT1720 (100 mg/kg) 1 hour earlier before lPS injection. Then All mice were sacrificed 12 hours after LPS injection. H&E staining was used to observe pathological changes of lung tissue. The wet/dry ratio of lung tissue was measured. Inflammatory cells in bronchoalveolar lavage fluid (BLAF) were counted, and myeloperoxidation (MPO) activity in lung tissue was detected. The gene and protein expression of inflammatory factors (TNF-α and IL-1β) in lung tissue of mice were detected by real-time PCR and ELISA, respectively.
【Results】
Compared to mice in the control group, mice in the SRT1720 treatment alone group had no effect on the morphology and inflammation of lung tissue. Intraperitoneal injection of LPS can induce pathological damage in lung tissue of mice, increase lung wet/dry ratio, increase the number of total cells and neutrophils in BALF, increase MPO activity, and increase the expressions of TNF-α and IL-1β in lung tissue. The mice in the ALI+SRT1720 group showed that SRT1720 treatment significantly reduced the changes described above.
【Conclusion】
Sirt1 agonist SRT1720 can attenuate LPS-induced ALI in mice, and its mechanism is related to inhibition of inflammatory response.

Effect of Estrogen on the ENT Expression of Vimentin and E-cadherin in Lung Adenocarcinoma SPC-A-1 cells

【Objective】
To investigate the effect of estrogen on the expression of epithelial mesenchymal transition markers of E-cadherin and vimentin in SPC-A-1 cell line.
【Methods】
SPC-A-1 cell line was stimulated with different concentrations of estrogen; the expression of EMT markers was detected by Q-RT-PCR and Western blot; the effect of different concentrations of estrogen on cell migration was detected by Transwell test. The difference between the groups was calculated.

【Conclusion】
Appropriate concentration of estrogen can promote the expression of vimentin, inhibit the expression of E-cadherin, and improve the ability of cell migration. When the concentration of estrogen is 1×10

mol/L, the effect of promoting vimentin expression, inhibiting E-cadherin expression and promoting cell migration is most significant. Estrogen can promote the EMT process of SPC-A-1 cell line.

Correlation between Plasma Arteriosclerosis Index and Uric Acid Level in Patients with Hypertension

【Objective】
To investigate the relationship between plasma arteriosclerosis index (AIP) and uric acid level in patients with different grades of hypertension.
【Methods】
A total of 443 patients with hypertension who were treated for the first time in the Department of cardiovascular medicine of our hospital from January to December 2018 wae selected as the research objects. According to the classification standard of hypertension, the patients were divided into three groups:hypertension group 1 (n=102), hypertension group 2 (n=177), hypertension group 3 (n=164). The blood biochemical indexes were detected by automatic blood cell and biochemical immune analyzer, and the general clinical data of patients were recorded at the same time.
【Results】
There were significant differences in age, body mass index (BMI), uric acid (UA), creatinine (CR), cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), non-high-density lipoprotein cholesterol (non-HDL-C), low-density lipoprotein cholesterol (LDL-C), apolipoprotein B (apoB), and plasma atherosclerotic index (AIP) among the three groups. There were significant differences in the levels of hypertension, UA, TG, non-HDL-C, HDL-C, ApoA1 and AIP between the hyperuricemia group and the non hyperuricemia group (P<0.05). Spearman correlation analysis showed that UA was positively correlated with AIP, TG, non-HDL-C and apoB (P<0.05), and negatively correlated with HDL-C and ApoA1 (P<0.05). =0.35, P<0.01). AIP had a good predictive value for patients with hyperuricemia (P<0.01). The area under ROC curve AUC=0.855, 95% CI(0.815, 0.895), sensitivity 87.5%, specificity 73.5%.
【Conclusion】
AIP is positively correlated with the UA level of hypertension patients. Under different hypertension grades, the blood uric acid level and AIP level of patients have significant differences. The higher the grading, the higher the level of AIP and the higher the level of blood uric acid.

Risk Factors for Osteoporosis in Patients with Type 2 Diabetes Mellitus

【Objective】
To investigate the risk factors for osteoporosis (OP) in patients with type 2 diabetes mellitus (T2DM).
【Methods】
A total of 417 patients with T2DM were selected, and bone mineral density (BMD) of femoral neck and lumbar spine 1-4 were measured by dual-energy X ray absorptiometry. According to the T value of BMD, 207 patients were divided into T2DM combined with OP group (T value≤-2.5,207 cases) and 210 patients were divided into simple T2DM group (T value>-2.5,210 cases). Meanwhile, 200 healthy subjects were enrolled as normal control group. Then, the age, course of diabetes mellitus, body mass index (BMI), HbA1c, fasting insulin (FINS), lipid and BMD were compared and analyzed among the three groups. The risk factors for OP in patients with T2DM were evaluated.
【Results】
Compared to simple T2DM group, the age, course of diabetes mellitus, HbA1c and total cholesterol (TC) were significantly higher (P<0.05), whereas high density lipoprotein cholesterol (HDL-C), BMD were significantly lower in the T2DM combined with OP group (P<0.05). Correlation analysis showed that HbA1c and TC were negatively related with BMD of femoral neck and lumbar spine 1-4, while HDL-C was positively related with BMD of lumbar spine 1-4 (P<0.05). In multiple stepwise Logistic regression showed that course of diabetes mellitus, HbA1c, HDL-C and TC were independent factors for T2DM combined with OP (P<0.05). The receiver operating characteristic curve showed that the course of diabetes mellitus >14.51 years, HbA1c<9.32%, HDL<1.12mmol/L and TC>5.67mmol/L in patients with T2DM increased the risk of OP. The area under curve were more than 0.6.
【Conclusion】
The extended course of diabetes mellitus, hyperglycemia and abnormal lipid metabolism are independent risk factor for OP in patients with T2DM.

Effect of Aerobic Exercise Combined with Electrical Stimulation on Cardiopulmonary Fitness in Patients with Stroke in a short Duration

【Objective】
To investigate the effect of aerobic exercise combined with electrical stimulation on cardiopulmonary fitness in patients with stroke in a short duration.
【Methods】
Sixty patients with stroke were divided into aerobic exercise combined with electrical stimulation group (A), aerobic exercise group (B) and electrical stimulation group (C) according to the random number table method, with 20 cases in each group. Among three groups, all patients received conventional exercise training including gait, balance, strength, and range of motion activities. Additionally, patients in group A, group B and group C received aerobic exercise combined with electrical stimulation, aerobic exercise and electrical stimulation, respectively, the duration of the therapy was 4 weeks. The variables of cardiopulmonary exercise test before and after treatment including maximal oxygen consumption (VO
2max
), respiratory exchange ratio (RER), maximal heart rate, systolic blood pressure, diastolic blood pressure and exercise duration were compared among three groups.
【Results】
There were no obvious differences of VO
2max
, RER, maximal heart rate, systolic blood pressure, diastolic blood pressure and exercise duration among patient of the three groups before treatment. VO

and exercise duration after treatment in patients of group A were higher than those before treatment (P<0.05), and VO
2max
and exercise duration after treatment were higher in group A than in group B and C (P<0.05). There were no significant differences of RER, maximal heart rate, systolic blood pressure and diastolic blood pressure after treatment in patients of group A compared to those before treatment (P>0.05). Compared with that before treatment, each variable after treatment in patients of group B and C had no obvious change (P>0.05), and the variable after treatment in patients of group B had no significant difference compared to corresponding variable after treatment in patients of group C (P>0.05).
【Conclusion】
Aerobic exercise combined with electrical stimulation can improve VO
2max
in patients with stroke in a short duration, which is an effective rehabilitation method for improving cardiopulmonary fitness in patients with stroke in a short duration.

Effect of Ultrasound-guided Nerve Block on Postoperative VAS Score and Serum Pain Mediator in Patients with Total Knee Arthroplastry

【Objective】
To investigate the effects of ultrasound-guided never block on postoperative visual analogue pain score (VAS), serum serotonin (5-HT), β-endorphin (β-EP), plasma substance P (SP) and interleukin 6 (IL-6) in patients undergoing total knee arthroplasty (TKA).
【Methods】
Ninety patients with lower extremity nerve block TKA in our hospital were randomly divided into two groups with 45 cases each. The experimental group was treated with ultrasound-guided nerve block analgesia and the control group was treated with intra-injection of analgesic drugs. The VAS scores of the patients at rest and activity within 48 hours after operation were compared between the two groups. The serum 5-HT, β-EP, SP, IL-6 were compared between the preoperative and postoperative 24h. 6 levels.
【Results】
No matter of resting and active states, the VAS scores of patients in the experimental group at 6h, 12h, 24h, 48h after operation were lower than those in the control group at the corresponding time points. The differences were statistically significant (P<0.05). Before operation, the serum of levels of EP, SP and IL-6 the two groups were no significant differences (P>0.05). The serum SP and IL-6 levels in the experimental group at 24h after surgery were lower, while the levels of 5-HT and β-EP were significantly higher than those in the control group (P<0.05).
【Conclusion】
Ultrasound-guided nerve block has better analgesic effects on TKA patients through regulating the release of pain-related factors in patients.

Application of hs-cTnI , NT-proBNP and D-D in Patients with Acute Cerebral Hemorrhage

【Objective】
To investigate value of diagnosis and application of high-sensitivity troponin I (hs-cTnI), N-terminal pro-brain natriuretic peptide (NT-proBNP) and D-dimer (D-D) in patients with acute cerebral hemorrhage.
【Methods】
A total of 63 patients with acute cerebral hemorrhage were selected as the observation group, meanwhile, 63 healthy volunteers who were admitted to our hospital for physical examination at the same period were selected as the control group. The levels of hs-cTnI, NT-proBNP and D-D in both control and observation groups were detected before treatment. According to the volume of hemorrhage, patients in the observation group were further divided into small, moderate and severe hemorrhage sub-groups. After 7 days of treatment, levels of hs-cTnI, NT-proBNP and D-D in patients were detected again. ROC curves of hs-cTnI, NT-proBNP and D-D were drawn to analyze the efficacy in diagnosis of acute cerebral hemorrhage.
【Results】
The levels of hs-cTnI, NT-proBNP and D-D in observation group were higher than those in control group (P<0.01). Compared to before treatment, the levels of hs-cTnI, NT-proBNP and D-D in observation group decreased after treatment. The levels of hs-cTnI, NT-proBNP and D-D in the small hemorrhage sub-group were lower than those in the moderate hemorrhage sub-group and severe hemorrhage sub-group (P<0.05); And the levels of hs-cTnI, NT-proBNP and D-D in the moderate hemorrhage sub-group were lower than that in the severe hemorrhage sub-group (P<0.05). The sensitivity and specificity in the combined determination of hs-cTnI, NT-proBNP and D-D was higher than any single hs-cTnI, NT-proBNP, or D-D application in the diagnosis of acute cerebral hemorrhage (P<0.05).
【Conclusion】
The serum levels of hs-cTnI, NT-proBNP and D-D in patients with acute cerebral hemorrhage rapidly increase. With the condition improves after treatment, serum hs-cTnI, NT-proBNP and D-D levels decrease. Application of hs-cTnI, NT-proBNP, and D-D combination can provide better assessment of acute cerebral hemorrhage prognosis and guidance of clinical treatment.

Correlation Analysis of NAG and β2-MG with Renal Function and Inflammation Indexes in Patients with Chronic Nephritis

【Objective】
To explore the levels and clinical significance of NAG and beta 2-MG in patients with chronic nephritis.
【Methods】
The patients with chronic nephritis admitted in our hospital were selected as the observation group, and healthy adults who underwent physical examination were selected as the control group. The differences of urinary NAG, beta 2-MG, blood inflammatory factors (hs-CRP and TNF-a) and renal function indexes (SCr, BUN and ALB) between the two groups were. compared The correlation of urinary NAG and beta 2-MG with blood inflammatory factors and renal function indexes in patients with chronic nephritis was analyzed.
【Results】
The levels of NAG and beta 2-MG in chronic nephritis group which were higher than those in control group (P<0.01). Meanwhile the levels of hs-CRP, TNF-a,SCr and BUN in chronic nephritis group were higher than those in control group (P<0.01). However, the RBC, HGB and ALB levels in patients with chronic nephritis were lower than those in the control group (P<0.01). Both NAG and β2-MG levels in patients with chronic nephritis were positively correlated with levels of hs-CRP, TNF-α, SCr and BUN, while they were negatively correlated with levels of RBC, HGB and ALB.
【Conclusion】
The levels of NAG and beta 2-MG in patients with chronic nephritis are higher,which are correlated with inflammatory factors and renal function.

Relationship between Incidence of Intradialytic Hypotension and Cognitive Function in Hemodialysis Patients

【Objective】
To analyze the relationship between the incidence of intradialytic hypotension (IDH) and cognitive function in hemodialysis patients.
【Methods】
A total of 182 patients with end-stage renal disease who underwent maintenance hemodialysis (MHD) in our hospital were selected for the study. The incidence of IDH within 3 months of MHD were recorded and the factors affecting IDH were analyzed. The cognitive function and quality of life in MHD patients with IDH and non-IDH were evaluated by assessment of MMSE and SF-36 for 6 months and 12 months. The survival rate of IDH and non-IDH patients were compared.
【Results】
(1) Of the 182 patients, IDH incidence was 33.0% (60/182). The age and ultrafiltration rate of patients in the IDH group were higher than those in the non-IDH group, while the plasma albumin and residual urine were lower than those in the non-IDH group (P<0.05). Logistic analysis found that age, ultrafiltration rate, and plasma albumin were independent influencing factors of IDH (P<0.05). (2)There were no significant differences in baseline cognitive function and quality of life score between the two groups before MHD (P>0.05). After 6 and 12 months of treatment, the MMSE and SF-36 scores in the IDH group were lower than those in the non-IDH group (P<0.05). The incidence of cognitive dysfunction in the IDH group at 6 and 12 months were 13.3% and 23.3%, respectively, which were higher than those in the non-IDH group 3.3% and 8.2%, respectively, (P<0.05). (3) Kaplan-Meier method and Log-rank test analysis showed that the survival time of IDH group was shorter than that of non-IDH group (P<0.05).
【Conclusion】
IDH has a high incidence in MHD patients. Age, low protein and ultra-ultrafiltration are risk factors of IDH. The occurrence of IDH is closely related to cognitive impairment and poor prognosis.

Effect of Ultrasound-guided Paravertebral Nerve Block Anesthesia on Stress Response and Hemodynamics in Patients with Lung Cancer

【Objective】
To observe the effect of ultrasound-guided paravertebral nerve block anesthesia on lung cancer patients and its influence on stress index and hemodynamics.
【Methods】
One hundred Patients with lung cancer who underwent surgery from May 2016 to May 2018 in our hospital were selected as the study subjects. According to the different anesthetic methods, the patients in the observation group were given ultrasound-guided paravertebral nerve block anesthesia and patients in the control group received general anesthesia. The effects of anesthesia were observed and the differences of hemodynamics, stress index and immune function were compared between the two groups.
【Results】
The Visual simulation (VAS)scores of the patients at both 4h and 12 h after surgery in the observation group were lower than those in the control group ( P<0.05). There was no significant difference in Ramsay scores between the two groups (P>0.05). There was no difference in hemodynamic parameters, immune function and stress index between the two groups before anesthesia. The levels of SBP,MAP,HR and cortisol , TNF-α in the observation group at 4 hours after anesthesia were lower than those in the control group ( P<0.05). At 4 hours after operation, the levels of CD4

in the observation group were higher than those in the control group (P<0.05).
【Conclusion】
Ultrasound-guided paravertebral nerve block anesthesia has a good anesthetic effect for lung cancer patients, and has little effect on hemodynamics and immune function. It has a good application value.

Analysis of Clinical Characteristics and Treatment of 50 Cases of Juvenile Dermatomyositis

【Objective】
To study the clinical characteristics, therapeutic effect and prognosis of juvenile dermatomyositis.
【Methods】
Fifty cases of juvenile dermatomyositis hospitalized in Hunan children's Hospital from July 2011 to January 2020 were analyzed retrospectively. The onset, clinical characteristics, laboratory examination, auxiliary examination, treatment, follow-up and prognosis were analyzed.
【Results】
In 50 juvenile dermatomyositis, the ratio of male to female patients was 1.08∶1, with an average age of 8.5±4.5 years. Of which, 11 children had a history of upper respiratory tract infection before the onset of the disease. . There were 2 cases who had typical skin lesions without myositis symptoms. They both had weakened muscle strength but normal muscle tension. The most common visceral damage was interstitial pneumonia. All the children had typical skin lesions, but no muscle atrophy and calcinosis. No malignant tumor was found. All 50 cases were treated with glucocorticoid; and 9 out of 50 cases received glucocorticoid combined with methotrexate. The therapeutic effect and prognosis were good.
【Conclusion】
Juvenile dermatomyositis is a rare disease in children, which is characterized by skin damage and muscle weakness. Muscle enzyme and electromyogram are very useful for the diagnosis of juvenile dermatomyositis. Glucocorticoid therapy is effective, if necessary, combined with immunosuppressant.

Influencing Factors of Motor Function in Maintenance Hemodialysis Patients with End-stage Uremia

【Objective】
To investigate the influencing factors of motor function in patients with end-stage uremic maintenance hemodialysis (MHD).
【Methods】
From June 2018 to June 2020, 137 MHD patients with end-stage uremia in our hospital were selected. All patients had MHD ≥ 6 months. Muscle strength and cardiopulmonary endurance were measured in patients with end-stage uremia on non dialysis days to evaluate motor function. According to the motor function, the patients were divided into motor function decline (6 min walking distance < 350 m, male grip strength < 65 kg or female grip strength < 30 kg) and normal motor function (6 min walking distance ≥ 350 m, male grip strength ≥ 65 kg or female grip strength ≥ 30 kg were regarded as normal motor function). Univariate analysis of end-stage uremic MHD patients with motor function influencing factors; multivariate logistic regression analysis of end-stage uremic MHD patients with independent risk factors of motor function.
【Results】
Among 137 MHD patients with end-stage uremia, 65 cases had motor dysfunction, the incidence was 47.44%. The pedaling force and grip strength of both lower limbs in the motor function decline group were lower than those in the normal motor function group, and the 6-minute walking distance was shorter than that in the normal motor function group (P<0.05). The results of univariate analysis showed that there were no significant differences in gender, body mass index (BMI), primary disease, SCr,BUN, HDL-C, LDL-C and TC between the two groups (P>0.05); age, dialysis time and blood pressure were not significantly different between the two groups (P>0.05) There were significant differences in erythrocyte protein and diabetes mellitus (P<0.05). Multivariate logistic regression analysis showed that age, dialysis time, hemoglobin and diabetes mellitus were independent risk factors of exercise function.
【Conclusion】
The motor function of MHD patients with end-stage uremia is affected by many factors, including age, dialysis time, hemoglobin and diabetes mellitus as its independent risk factors. In order to improve the motor function of patients, it is necessary to take reasonable and effective exercise methods.

【Objective】

【Methods】
All 150 maternal women who had natural childbirth in our hospital received intrathecal injection of sufentanil combined with low-dose ropivacaine. According to the concentration of sufentanil, they were divided into group A, group B and group C, indicating sufentanil administration at concentrations of 3μg, 5μg, and 7 μg, respectively. The anesthetic effects of the three groups were recorded and the differences of maternal and neonatal outcomes among the three groups of were compared.
【Results】
The VAS scores and Ramsay scores in the order of group A-group B-group C at 1h and 4h after anesthesia showed trend from high to low and the differences among the three groups were statistically significant (P<0.05). The levels of SBP and HR at 4 h after anesthesia in the three groups showed the highest in group A and the lowest in group B, with group C in the middle. The difference was statistically significant (P<0.05). Among three groups, there was no significant difference in the incidence of neonatal aspiration pneumonia, intracranial hemorrhage and Apgar score after birth (P>0.05). The incidence of nausea, headache and urinary retention in group A and group B was lower than that in group C (P<0.05).
【Conclusion】
Intrathecal injection of 5 μg sufentanil combined with low-dose ropivacaine has good effect in labor analgesia, little effect on maternal hemodynamics, and low complication rate , which shows good application value.

【Methods】

WI sequence standard. The apparent diffusion coefficient (ADC) and anisotropy (FA) values of each segment of spinal cord were obtained based on the results of DTI. The responsible segment of cervical spinal cord function damage was determined, and then cervical decompression surgery was performed. JOA score, NDI score and SF-36 quality of life score were recorded before operation, 6 months and 12 months after operation were recorded respectively. Post-operation X-ray film and CT were reviewed to understand the situation of internal fixation. MRI scan and DTI were reviewed at 6 and 12 months after operation, and relevant image data were collected. The changes of ADC value and FA value of spinal cord before and after operation were compared, and the correlation between ADC value and JOA score, NDI score and SF-36 quality of life score was analyzed.
【Results】
There were statistically significant differences in JOA score, NDI score, SF-36 score, ADC value and FA value before operation, 6 months after operation and 12 months after operation (P<0.05). JOA score and SF-36 score were negatively correlated with ADC value, and positively correlated with FA value (P<0.05). The NDI score was positively correlated with ADC value, and negatively correlated with FA value (P<0.05).
【Conclusion】
For patients with multilevel cervical spondylotic myelopathy, the decrease of FA value, the increase of ADC value, the gradual decrease of JOA score, the gradual increase of NDI score, and the decrease of SF-36 score of quality of life indicate that the more serious the cervical spinal cord lesion, the more serious the neurological dysfunction, and the worse the quality of life.

Effect of Parathyroidectomy Combined with Autotransplantation on Klotho and FGF23 in Maintenance Hemodialysis Patients

【Objective】
To evaluate the efficacy of parathyroidectomy combined with anterior wall autotransplantation (tPTX+AT) in the treatment of maintenance hemodialysis (MHD) patients with severe secondary hyperparathyroidism (SPTH), and investigate the effect on soluble Klotho protein and fibroblast growth factor 23 (FGF23) levels.
【Methods】
Ninety patients complicated with MHD and SPTH, who underwent tPTX+AT treatment, were enrolled in the study. The incidence of complications, biochemical parameters, Klotho and FGF23 levels before and 1, 3, 6 and 12 months after treatment were recorded.
【Results】
All patients underwent successful operation. There were 3 cases of postoperative hemorrhage, 2 cases with postoperative infection, 1 case with laryngeal nerve injury, 1 case with arrhythmia, 4 cases with hypocalcemia, and 1 case with high potassium. All of them got improved after active treatment. The levels of serum calcium, blood phosphorus, calcium and phosphorus product, iPTH and ALP at 1, 3, 6 and 12 months after surgery were lower than those before treatment (P<0.05). At 3, 6, and 12 months after surgery, Hb, Hct, and SF were higher than those before surgery (P<0.05). While the dose of rHuEPO and CRP were lower than those before surgery (P<0.05). There was no significant difference in Kt/V (urea clearance index) before and after surgery (P>0.05). FGF-23 in patients at 1, 3, 6 and 12 months after surgery was lower than that before surgery, and Klotho was higher than that before surgery (P<0.05).
【Conclusion】
tPTX+AT has less complication in MHD with SPTH patients and high safety. It can improve calcium metabolism, correct nutrition and inflammation status, and can reduce FGF-23 and increase Klotho level.

Influence of TGF-β Protein and AGEs Expression on Development of Cataract in Diabetic Patients

【Objective】
To investigate the expression levels and significance of transforming growth factor β(TGF-β) and advanced glycation end products (AGEs) in anterior capsule tissue of lens and serum of diabetic patients with cataract.
【Methods】
Sixty-seven patients with diabetes mellitus complicated with cataract (the observation group) and 50 patients with simple cataract (the control group) were selected from January 2018 to January 2019 in our hospital. The expression of TGF-β protein in anterior capsular tissue of lens was detected by Western blot and the level of AGEs and HbA1c in serum was detected by ELISA.
【Results】
The relative expression of TGF-β protein in the lens anterior capsular tissue and serum AGEs of the observation group were (0.982±0.172) and (18.82±1.82)g/L, respectively, which were significantly higher than those of the control group (0.563±0.180) and (10.95±2.00) g/L, respectively. The differences were statistically significant (P<0.05). The serum AGEs level of patients with HbA1c ≥6.5% in the observation group was (19.96±1.88)g/L, which was significantly higher than that of patients with HbA1c <6.5% (17.78±1.89) g/L, the difference was statistically significant (P<0.05).
【Conclusion】
In diabetic patients with cataract, the levels of TGF-β in lens anterior capsule tissue and AGEs in serum increased. AGEs are related to glycosylated hemoglobin, TGF-β and AGEs may play a role in the process of diabetic cataract.

Diagnostic Value of Serum PCAT-1 Combined with CEA and CA199 in Patients with Colorectal Cancer

【Objective】
To explore the diagnostic value of individualized and combined detection of serum prostate cancer-associated lncRNA transcript 1(PCTA-1), carcinoembryonic antigen(CEA)and saccharide antigen199 (CA199) levels in patients with colorectal cancer (CRC).
【Methods】
A total of 182 newly diagnosed CRC patients (CRC group), 80 patients with colorectal polyps (benign group) and 80 healthy people (control group) were selected as the research objects.Serum PCTA-1, CEA and CA199 levels were measured by fasting venous blood collection, and the changes of serum PCTA-1, CEA and CA199 levels were compared between groups and between clinicopathological features.ROC curve was drawn to analyze the diagnostic value of serum PCTA-1, CEA, CA199 levels and combined diagnosis for CRC.
【Results】
The serum levels of PCAT-1, CEA and CA199 in the CRC group were significantly higher than those in the benign group and the control group, and those in the benign group were significantly higher than those in the control group, with statistical significance (P<0.05). With the decrease of differentiation of CRC, the increase of clinical TNM pathological stages and lymph node metastasis, the serum levels of PCAT-1, CEA and CA199 showed an upward trend. There was significant difference between the groups with serum CEA and CA199 levels (P<0.05).The diagnostic AUC of serum PCAT-1, CEA, CA199, PCAT-1+CEA, PCAT-1+CA199 and their combination were 0.791, 0.631, 0.683, 0.822, 0.848 and 0.836, respectively. The sensitivity was 75.25%, 60.21%, 59.25%, 68.70%, 73.35% and 70.05%, and the specificity was 65.32%, 77.22%, 80.25%, 79.36%, 83.75% and 75.73%, respectively.The diagnostic sensitivity of PCAT-1 to CRC was higher than that of CEA and CA199, but the diagnostic specificity was lower than that of CEA and CA199 (P<0.05); the diagnostic value of PCAT-1+CEA, PCAT-1+CA199, PCAT-1+CEA+CA199 was higher than that of PCAT-1, CEA and CA199 alone, and the diagnostic specificity of PCAT-1 combined with CA199 could significantly improve the diagnostic specificity of CRC (P<0.05).
【Conclusion】
The elevation of serum PCAT-1, CEA and CA199 levels contributes to the early diagnosis of CRC, and joint detection has more diagnostic advantages, especially PACT-1 combined with CA199 detection, which can reduce the missed diagnosis rate of CRC and has clinical significance.

Risk Factors of Early Mortality in Patients with Severe Left Ventricular Dysfunction after Cardiac Surgery

【Objective】
To investigate the predictors of early postoperative mortality in patients with severe left ventricular dysfunction undergoing cardiac surgery.
【Methods】
A total of 106 eligible cardiac surgeries performed by the same surgeon from 2012 to 2019 were retrospectively analyzed. All patients were divided into death group (n=14) and survival group (n=92) according to the survival status of perioperative patients. Univariate and multivariate analysis were performed.
【Results】
Univariate analysis showed that age, body fat mass index (BMI), white blood cell count (WBC), C-reactive protein (CRP), N-terminal pro brain natriuretic peptide (NT proBNP) and operation mode were significantly associated with early postoperative death (P<0.1). Combined with previous literature, creatinine (SCr) and troponin T (cTnT) were also included in the multivariate logistic regression analysis. Finally, it was found that SCr, WBC, cTnT and operation mode were independent risk factors for early postoperative death in patients with low ejection fraction (all OR > 1, P<0.05).
【Conclusion】
SCr, WBC, cTnT and surgical methods were independent risk factors for early postoperative death in patients with low ejection fraction. These patients should be fully evaluated before operation, and timely anti-inflammatory, myocardial nutrition and other treatments should be taken to avoid operation in acute myocardial infarction stage.

Efficacy and Prognosis of DSA Interventional Fusion Therapy in Patients with Early Stage Femoral Head Necrosis

【Objective】
To investigate the efficacy and prognosis of digital subtraction angiography (DSA) interventional fusion therapy in patients with early stage femoral head necrosis.
【Methods】
A total of 118 patients with early stage femoral head necrosis who received treatment in our hospital from January 2016 to June 2017 were selected as the research subject. According to the different treatment methods, patients were divided into the observation group (n=68) and the control group (n=60). Patients in the observation group received DSA interventional fusion therapy, while patients in the control group received core decompression and bone grafting. The clinical efficacy and prognosis of patients in the two groups were analyzed and compared.
【Results】
Before treatment, there was no statistically significant differences of both Harris score for hip function and the pain index Ficat score between the two groups (P>0.05). After 6 months and 12 months of treatment, the Harris score in the observation group was higher than that in the control group, while the Ficat pain index score in the observation group was lower than that in the control group. The differences between the two groups were statistically significant (P<0.05). The incidence of adverse reactions in the observation group was lower than that in the control group, and the difference with statistical significance (P<0.05).
【Conclusion】
DSA interventional fusion therapy for the treatment of early stage femoral head necrosis can significantly improve the function of hip function and relieve pain symptoms, which shows a good prognosis and worthy clinical application.

SOX18 Expression with Patients' Long-term Prognosis in Prostate Cancer

A total of 140 patients with prostate cancer treated in our hospital from January 2012 to March 2017 were selected as the prostate cancer group, and 112 patients with benign prostatic hyperplasia were selected as the control group. The expression of SOX18 was detected by immunohistochemical staining, and the prognosis of patients was followed up.

Effect of Continuous Lumbar Plexus Block on Cognitive Function in Patients after Knee Joint Replacement

【Objective】
To explore the effect of continuous lumbar plexus block on cognitive function in patients after knee replacement surgery.
【Methods】
We selected 82 patients with knee replacement surgery who were treated in our hospital were as subjects. All patients were randomly divided into two groups, with 41 cases in each group. Patients in the control group received intravenous analgesia, while patients in the observation group received continuous lumbar plexus block for the surgery.The postoperative recovery time, the VAS score, the incidence of adverse reactions, the changes of the simple mental state scale and cognitive dysfunction were compared between the two groups.
【Results】
The recovery time of the observation group, the VAS score at 12h and 24h after operation in the observation group were lower than those in the control group(P<0.05).The incidence of adverse reactions such as nausea and vomiting, dizziness, urinary retention in the observation group was 12.20%,which was lower than 31.71% in the control group (P<0.05).The MMSE score of the observation group was 24.73±3.16, and only 1 case (2.44%) of cognitive dysfunction occurred. However, the MMSE score of the control group was 20.38±2.91, and the cognitive dysfunction occurred in 5 cases (12.20%). The differences between the two groups were statistically significant. (P<0.05).
【Conclusion】
Continuous lumbar plexus block is more effective in patients undergoing knee replacement surgery, which can accelerate the postoperative recovery time of patients and has a positive effect on reducing pain, postoperative complications and cognitive dysfunction. It is worth promoting clinically.

To investigate the correlation between the changes of serum angiotensin Ⅱ (Ang Ⅱ), aldosterone (ALD), high sensitivity C-reactive protein (hs CRP) and red blood cell distribution width (RDW) in patients with essential hypertension complicated with atrial fibrillation.
【Methods】
Nighty cases of patients with simple primary hypertension (control group) and 140 cases of patients with essential hypertension combined with atrial fibrillation were selected as the observation group. The serum levels of Ang Ⅱ, ALD, hs CRP and RDW were detected, and the left atrial diameter (LAD), left ventricular end systolic diameter (LVESD) and left ventricular end diastolic diameter (LVEDd) were compared between the two groups.
【Results】
The levels of AngⅡ, ALD, hs-CRP, RDW and LAD in the observation group were significantly higher than those in the control group (P<0.05); there was no significant difference in LVESD and LVEDd between the two groups (P>0.05). The levels of AngⅡ, ALD, hs-CRP and RDW in patients with persistent atrial fibrillation were significantly higher than those in patients with paroxysmal atrial fibrillation (P<0.05). AngⅡand ALD were positively correlated with LAD (P<0.05), while hs-CRP and RDW levels were not significantly correlated with LAD (P>0.05).
【Conclusion】
The serum levels of AngⅡ, ALD, hs-CRP and RDW in patients with hypertension complicated with atrial fibrillation were significantly higher than those in patients with simple hypertension.

were the risk factors of early diabetic foot (β value was 0.448, 0.206 and 0.044,respectively.) and they all showed statistical significance (P<0.001). The ROC curve showed that area under curve (AUC) of Hcy, SAA and Lp-PLA was 0.918, 0.892 and 0.967, respectively. The combination AUC of three parameters was 0.99.
【Conclusion】
Form this study we know that the levels of Hcy,SAA and Lp-PLA

【Objective】
To investigate the changes and significance of serum cystatin C, high-sensitivity C-reactive protein (hs-CRP) and endothelin-1 (ET-1) in patients with coronary artery slow flow phenomenon (CSFP).
【Methods】
From January 2016 to December 2019, 88 CSFP patients with coronary heart disease confirmed by coronary angiography (observation group) were selected, and 90 patients with normal coronary blood flow and no obvious coronary stenosis were selected as control group. The coronary flow frame number, coronary microcirculation resistance index, serum Cys-C, hs CRP and ET-1 levels were compared between the two groups, and the correlation between serum Cys-C, hs CRP, ET-1 and coronary microcirculation resistance index was analyzed.
【Results】
The left anterior descending artery, right coronary artery, left circumflex artery and overall mean coronary blood flow frame number in the observation group were higher than those in the control group, the difference was statistically significant (P<0.05).The left anterior descending artery, right coronary artery, left circumflex artery and the total microcirculation resistance index of the observation group were higher than those of the control group, the difference was statistically significant (P<0.05).The levels of Cys-C, hs CRP and ET-1 in the observation group were higher than those in the control group (P<0.05). The levels of serum Cys-C, hs CRP and ET-1 in the observation group were significantly positively correlated with coronary microcirculation resistance index (P<0.05).
【Conclusion】
The levels of serum Cys-C, hs CRP and ET-1 in CSFP patients are significantly increased, and they are significantly correlated with the increase of coronary microcirculation resistance index.

Predictive Value of Thrombopoietin and Hypoxia Inducible Factor-1α in High Risk Infants with Cerebral Palsy

【Objective】
To investigate the predictive diagnostic value of thrombopoietin (TPO) and hypoxia inducible factor-1α(HIF-1α) in high-risk infants with cerebral palsy (CP).
【Methods】
A total of 120 cases of CP high-risk infants was selected as the research object, and 35 cases of healthy newborns in the same period were selected as the control group. The high-risk infants were followed up until CP was diagnosed or excluded. According to the follow-up results, CP high-risk infants were divided into CP Group (n=36) and non CP Group (n=73); CP Group was divided into mild subgroup (n=14), moderate subgroup (n=10) and severe subgroup (n=12). The levels of serum TPO and HIF-1αin each group were compared, and the ROC curve was drawn to analyze the diagnostic value of serum TPO and HIF-1αfor CP. Spearman linear correlation was used to analyze the correlation between serum TPO and HIF-1αlevels in CP high-risk infants.
【Results】
The serum TPO level in CP Group was higher than that in control group but lower than that in non CP Group; there were significant differences among control group, non CP Group, CP Group and each subgroup (P<0.05). The serum HIF-1 α level in CP Group was higher than that in control group and non CP Group, and that in non CP Group was higher than that in control group. There were significant differences among control group, non CP Group, CP Group and subgroups with different disease severity (P<0.05). ROC curve analysis showed that the area under the curve (AUC) of serum TPO in the diagnosis of CP was 0.785, the sensitivity was 77.80%, the specificity was 79.45%, and the best cut-off value was 596.75 pg / ml; the AUC of serum HIF-1αin diagnosing CP was 0.779, the sensitivity was 80.06%, the specificity was 80.56%, and the best cut-off value was 5.72 ng / ml. Spearman linear correlation analysis showed that there was a negative correlation between serum TPO and HIF-1α(r=-0.334,P<0.001).
【Conclusion】
The levels of serum TPO and HIF-1αin high-risk infants with CP are higher than those in healthy newborns. The changes of serum TPO and HIF-1αlevels play an important role in the pathogenesis of CP.

To explore the value of serum 25-(OH)D in evaluating the degree of liver fibrosis in patients with type 2 diabetes mellitus complicated with non-alcoholic fatty liver disease (NAFLD).
【Methods】
In this study, 60 patients with type 2 diabetes mellitus complicated with NAFLD were selected as the observation group, and 60 patients with type 2 diabetes mellitus admitted in the same period were selected as the control group. The serum levels of 25-(OH)D, FPG, 2hPG and HbA1c were compared between the two groups. Meanwhile, Spearman correlation coefficient was used to analyze the correlation between 25-(OH)D level and stages of liver fibrosis.
【Results】
There was no significant difference in the levels of FPG, 2hPG and HbA1c between the two groups (P>0.05). The level of 25-(OH)D in the observation group was significantly lower than that that in the control group (P<0.05). 【Conclusion】
The level of 25-(OH)D in patients with type 2 diabetes complicated with NAFLD is significantly reduced. The level of 25-(OH)D is negatively correlated with the stages of liver fibrosis, which can provide reference to evaluate the stages of liver fibrosis in patients with type 2 diabetes mellitus and NAFLD.

Expression Levels and Significance of MMR Proteins in Patients with Endometrial Cancer

【Objective】
To analyze the expression levels and significance of mismatch repair (MMR) proteins in patients with endometrial cancer (EC).
【Methods】
A total of 196 samples of EC tissues retained in the pathology department of our hospital from January 2013 to January 2016 were collected as subjects. The expression levels of four mismatch repair (MMR) proteins: MutL protein homolog 1 (MLH1), Postmeiotic protein isolate 2 (PMS2), MutS protein homolog 2 (MSH2) and MutS protein homolog 6 (MSH6) were detected by immunohistochemistry, and the relationship between their expressions and clinicopathological characteristics of the patients was analyzed. Meanwhile the survival of the patients was observed as well.
【Results】
Among the 196 EC samples, the proportion of simultaneous expressions of four MMR proteins of MLH1, PMS2, MSH2 and MSH6 was 72.45% (142/196) and the expression of MMR protein deletion accounted for 27.55% (54/196). The deletion of MMR proteins were related to age, cell differentiation degree, myometrial infiltration degree, tumor site and lymph node metastasis of EC patients (P<0.05),while the lack of MMR protein expressions were not significantly correlated with combined basic diseases, tumor pathological types and FIGO stage (P>0.05). The median survival time and 3-year survival rate in simultaneous expressions of MMR proteins were 37.89 months and 90.14% (128/142), respectively, and the median survival time and 3-year survival rate in deletion expressions of MMR proteins were 37.56 months and 88.89% (48/54), respectively. There was no statistically significant difference in the median survival time between patients with simultaneous expressions of MMR proteins and patients with deletion expressions of MMR proteins (P>0.05).
【Conclusion】
The deletion expressions of MMR proteins are associated with the occurrence and development of EC. The deletion rates of MMR protein expressions are significantly increased among patients with age ≤50 years old, low differentiation,≥50% myometrial infiltration, involvement of lower uterine segment and lymph node metastasis. Deletion expressions of MMR proteins may be associated with poor prognosis of patients.

Clinical Application of Laparoscopic Ultrasound-assisted Microwave Ablation in the Treatment of Hepatic Hemangioma

【Objective】
To investigate the efficacy and technical aspects of laparoscopic ultrasound-assisted microwave ablation in the treatment of hepatic hemangioma.
【Methods】
The clinical data of 9 patients with hepatic hemangioma larger than 5 cm in diameter undergoing laparoscopic ultrasound-assisted microwave ablation from March 2018 to October 2019 were retrospectively analyzed.
【Results】
Laparoscopic ultrasound-assisted microwave ablations were performed successfully in all the 9 patients without any open conversion. The average operation time was (189±58) min, average ablation time was (21±4.8) min, average blood loss was (138±245) mL, and the average length of postoperative hospital stay was (7.8±1.4) d. The main complications included hemoglobinuria in 4 cases (44.44%), fever in 4 cases (44.44%), and intrahepatic biloma in one case (11.11%), but no serious complications such as pneumothorax, postoperative hemorrhage, skin burned and adjacent organ injuries occurred. Postoperative follow-up was conducted for 6 to 12 months. There were 8 cases (88.89%) with complete elimination of lesions, and 1 case (11.11%) with lesion reduction more than 75%.
【Conclusion】
Laparoscopic ultrasound-assisted microwave ablation is safe and effective in treatment of hepatic hemangioma, and it is recommended to be used in clinical practice.

Diagnostic Value of Combined Detection of Human Leukocyte Antigen (HLA-E) and High-risk Human Papillomavirus in High-grade Squamous Intraepithelial Lesions and Cervical Squamous Cell Carcinoma

【Objective】
To investigate the diagnostic value of combined detection of human leukocyte antigen E (HLA-E) and high-risk human papillomavirus (HR-HPV) in high-grade squamous intraepithelial lesion (HSIL) and cervical squamous cell carcinoma.
【Methods】
Forty patients with low-grade squamous intraepithelial lesions (LSIL, LSIL group), 40 patients with HSIL (HSIL group), and 40 patients with cervical squamous cell carcinoma (cervical squamous cell carcinoma group) were selected from January 1, 2017 to April 30, 2019 in our hospital. Forty patients with chronic cervicitis who were treated at the same time were selected as the control group. All patients received HR-HPV and liquid-based cytology (TCT). The expression of HLA-E was detected by immunohistochemistry. The diagnostic value of HLA-E combined with HR-HPV and TCT combined with HR-HPV in HSIL and cervical squamous cell carcinoma were observed.
【Results】
The positive rate of HLA-E expression in cervical squamous cell carcinoma was higher than that in the HSIL group, the LSIL group and the control group (P<0.05). The positive rate of HLA-E expression in the HSIL group was higher than that in the LSIL group and the control group (P<0.05). There was no significant difference in the positive rate of HLA-E expression between the control group and the control group (P>0.05). The positive rate of HR-HPV in the cervical squamous cell carcinoma and HSIL group was higher than that in the LSIL group and the control group (P<0.05). The positive rate of the HR-HPV in the LSIL group was higher than that in the control group (P<0.05). There was no significant difference in the positive rate of HR-HPV between the cervical squamous cell carcinoma group and the HSIL group (P>0.05). The accuracy of TCT in the diagnosis of chronic cervicitis, LSIL, HSIL, and cervical squamous cell carcinoma was 35.0%, 42.5%, 40.0%, and 52.5%, respectively. The ROC curve showed that the AUC of HLA-E combined with HR-HPV in the diagnosis of HSIL and cervical squamous cell carcinoma was 0.737, and the specificity was 68.1% and 70.8%.There was no significant difference in AUC between TCT combined with HR-HPV and HLA-E combined with HR-HPV (P>0.05).
【Conclusion】
HLA-E is highly expressed in HSIL and cervical squamous cell carcinoma. HLA-E combined with HR-HPV is of high value in the diagnosis of HSIL and cervical squamous cell carcinoma, which is equivalent to TCT combined with HR-HPV.

Comparison of Clinical Effects of Craniotomy Clipping and Interventional Embolization in the Treatment of Cerebral Aneurysmal Subarachnoid Hemorrhage

【Objective】
To investigate the clinical effect of craniotomy clipping and interventional embolization in the treatment of cerebral aneurysmal subarachnoid hemorrhage (aSAH).
【Methods】
The clinical data of 96 patients with aSAH admitted to our hospital from February 2014 to may 2018 were retrospectively analyzed. According to the different treatment methods, they were divided into clipping group (n=41) and interventional group (n=55). The interventional group was treated with interventional embolization, and the clipping group was treated with craniotomy and clipping. The prognosis, incidence of complications, immune function and cytokine levels were compared between the two groups.
【Results】

Before operation, there was no significant difference in serum IL-6 and TNF-α levels between the two groups (P>0.05); 7 days after operation, the serum levels of IL-6 and TNF -α in the interventional group were lower than those in the clipping group (P<0.05). 3 months after operation, the prognosis of the interventional group was better than that of the clipping group,but there was no significant difference(P>0.05). 3 months after operation, the complication rate of the interventional group was 20.0% (11 / 55), which was lower than that of the clipping group 56.1% (23/41), the difference was statistically significant (P<0.05).
【Conclusion】
The clinical effect of interventional embolization in the treatment of aSAH patients is better, and it has less impact on the immune function of patients, which helps to reduce the incidence of complications, which is worthy of clinical application.

To investigate the effect of omeprazole combined with clarithromycin and metronidazole in the treatment of Helicobacter pylori associated gastritis in children on the level of inflammatory factors and immune function.
【Methods】
Eighty children with Helicobacter pylori associated gastritis in our hospital were selected and divided into control group and observation group according to different treatment methods, 40 cases in each group. The control group was treated with omeprazole + amoxicillin + metronidazole, and the observation group was treated with omeprazole + clarithromycin + metronidazole. The clinical effects of the two groups were analyzed.

Clinical Efficacy of Chinese Medicine Combined with Paclitaxel and Carboplatin in the Treatment of Ovarian Cancer

【Objective】
To investigate the clinical effect of traditional Chinese medicine (TCM) Yiqi-Huoxue (invigorating qi and activating blood circulation) combined with paclitaxel and carbohydrates in the treatment of ovarian cancer and the changes of related factors.
【Methods】
A total of 82 patients with ovarian cancer treated in our hospital from May 2016 to February 2019 were randomly divided into two groups with 41 cases in each. The control group was treated with paclitaxel combined with carboplatin, and the observation group was treated with traditional Chinese medicine Yiqi-Huoxue for invigorating qi and activating blood circulation on the basis of the control group. The clinical effect of the two groups after treatment was analyzed.
【Results】
The overall effective rate in the observation group (73.17%) was higher than that in the control group (51.22%), and the difference was statistically significant (P<0.05). There was no significant difference in tumor markers and cellular immune indexes between the two groups before treatment (P>0.05). After treatment, the levels of IL-2 (IL-2) and interferon-gamma (INF- γ) in the observation group were higher than those in the control group (P<0.05), and the levels of carbohydrate antigen (CA199), carbohydrate antigen (CA125), human epididymal protein 4 (HE4) and IL-10 (IL-10) were lower than those in the control group (P<0.05). There was no significant difference in TCM syndrome score and Karst (KPS) score between the two groups before treatment (P>0.05). After treatment, the score of KPS in the observation group (79.69±6.14) was higher than that in the control group (68.59±5.20), and the score of TCM syndromes (13.02±2.89) in the observation group was lower than that in the control group (17.98±3.28; P<0.05).
【Conclusion】
Traditional Chinese medicine for invigorating qi and activating blood circulation combined with paclitaxel can improve the cellular immune level, decrease the expression level of tumor markers and improve the quality of life in patients with ovarian cancer.

Prognostic Value of Serum HbA1c, sST2 and NT-proBNP Levels in Patients with Acute Coronary Syndrome

【Objective】
To investigate the prognostic value of glycosylated hemoglobin (HbAl C), soluble growth stimulating gene 2 protein (sST2), N-terminal pro brain natriuretic peptide (NT- proBNP) in patients with acute coronary syndrome (ACS).
【Methods】
The clinical data of 70 patients with ACS admitted to our hospital from January 2017 to March 2019 were selected. All patients were taken blood on the day of admission to detect the levels of serum HbA1c, sST2 and NT-proBNP. According to the severity of the disease, the patients were divided into single vessel disease group, double vessel disease group and three vessel disease group. The levels of serum HbA1c, sST2 and NT-proBNP in the three groups were compared, and the relationship between the three serum indexes and the severity of ACS was observed. According to the prognosis, the patients were divided into good prognosis group and poor prognosis group. The levels of serum HbA1c, sST2 and NT-proBNP were compared between the two groups, and the receiver operating characteristic curve (ROC) was drawn to analyze the predictive value of each index for poor prognosis.
【Results】
The levels of HbA1c, sST2 and NT-proBNP in the three vessel disease group were higher than those in the single vessel disease group and the double vessel disease group, and the indexes in the double vessel disease group were higher than those in the single vessel disease group (P<0.05). The levels of HbA1c, sST2 and NT-proBNP in poor prognosis group were significantly higher than those in good prognosis group (P<0.05). The area under the curve of serum HbA1c, sST2 and NT-proBNP in predicting poor prognosis of ACS patients were 0.748, 0.666 and 0.664, respectively. Taking HbA1c>5.675 mmol/L, sST2 >24.195 ng/mL, NT-proBNP>666.725 ng/L as cut-off values, patients had a high risk of poor prognosis.
【Conclusion】
With the severity of ACS, serum HbA1c, sST2 and NT-proBNP levels are higher, and the three have a certain predictive value for the poor prognosis of patients.

Correlation between NT-proBNP Level and Cardiac Remodeling in Elderly Hypertensive Patients with Preserved Ejection Fraction

【Objective】
To investigate the correlation between N-terminal pro brain natriuretic peptide (NT-proBNP) and cardiac remodeling in elderly patients with hypertension and ejection fraction preserved heart failure (HFPEF).
【Methods】
A total of 306 elderly patients with hypertension were selected as the research objects. The patients with HFpEF were included in the HFpEF group (n=96), the patients with heart failure HFrEF) with reduced ejection fraction (HFrEF) were included in the hfref group (n=52), the patients with heart failure with intermediate ejection fraction (HFmrEF) were included in the HFmrEF group (n=50), and the patients with simple hypertension were included in the control group (n=108). The general information, left ventricular ejection fraction (LVEF), left ventricular end diastolic diameter (LVEDD) and maximum early diastolic velocity (E) of mitral valve orifice were compared among the four groups The ratio of peak to peak a (E/A), cardiac index (CI), left ventricular mass index (LVMI), NT-proBNP. Spearman rank correlation or Pearson correlation coefficient were used to analyze the correlation between cardiac function, NT-proBNP and cardiac remodeling.
【Results】
There were significant differences in serum NT-proBNP level, E/A, LVMI, CI between the four groups (P<0.05); LVEF, LVEDD, E/A, LVMI among the HFpEF group, the HFmrEF group and then HFrEF group were statistically significant (P<0.05). Spearman correlation analysis showed that NT-proBNP was negatively correlated with LVEF and E/A (P<0.01); NT-proBNP was positively correlated with LVEDd and LVMI (P<0.01); cardiac function classification was positively correlated with LVMI, LVEDD, NT-proBNP (P<0.05), and cardiac function classification was negatively correlated with LVEF and E/A (P<0.05). Pearson correlation analysis showed that there was no significant correlation between NT-proBNP and LVEDD, E/A, LVMI in the HFmrEF group (all P>0.05), but only significant correlation with LVEF (r=0.315, P=0.026); there was no significant correlation between NT-proBNP and LVEF, LVEDD, E/A, LVMI in the HFrEF group (P>0.05).
【Conclusion】
NT-proBNP is negatively correlated with E /A and LVEF in elderly patients with hypertension complicated with HFpEF; it is positively correlated with LVEDD and LVMI, which has higher screening value for HFpEF, but the application value of echocardiography in HFmrEF and HFrEF patients still needs further study.
