1 Effect of Glu Receptor Antagonist on Memory Impairment in Rats after Electroconvulsive Shock

【Objective】To investigate the effect of glutamate (Glu) receptor antagonist on memory impairment in rats after electroconvulsive shock (ECT).【Methods】A total of 24 healthy male SD rats were randomly divided into the blank group, ECT group and MK-801+ECT group, with 8 rats in each group. Rats in the blank group were injected with normal saline 2 mL, and rats in the ECT group were treated with ECT. Rats in the MK-801+ECT group were injected with 2 mL MK-801 into their tail veins before ECT. Morris water maze learning and memory tests were used to explore memory impairment in rats, and the content of Glu and GABA in rat brain was determined by HPLC.【Results】The escape latency of ECT and MK-801+ECT groups was significantly longer than that of the blank group (t=7.708, 3.408, P<0.05), and the escape latency of the MK-801+ECT group was significantly shorter than that of ECT group; there was statistical difference (t=5.137, P<0.05). The percentage of swimming rats in the ECT and MK-801+ECT groups decreased significantly compared to the control group (t=12.039, 4.886, P<0.05). However, the percentage of swimming rats in the MK-801+ECT group was significantly higher than that in the ECT group, and the difference was statistically significant (t=7.543, P<0.05). The number of rats passing through the original platform in the ECT and MK-801+ECT groups decreased significantly compared to that in the blank group (t=9.459, 2.156, P<0.05). In addition, the number of rats passing through the original platform in the MK-801+ECT group was significantly higher than that in the ECT group, and there was statistical difference (t=9.508, P<0.05). Glu and GABA content levels in the ECT and MK-801+ECT groups rat brain were significantly higher than those in the blank group, and there were significant differences (ECT group: t=6.701, 5.639, MK-801+ECT group: t=2.630, 2.233, P<0.05). The Glu and GABA contents in the brain of rats in the MK-801+ECT group were significantly lower than in the ECT group, and there was statistical difference (t=4.789, 3.881, P<0.05).【Conclusion】Glu receptor antagonist MK-801 can alleviate the memory impairment in rats after electroconvulsive therapy as well as reduce the content of Glu and GABA in the brain, which has important research significance.

4 A Simple and Efficient In Vitro Isolation and Culture System for Bone Marrow-Derived Precursor Cells (DC) in Rats

【Objective】To establish a simple and efficient in vitro isolation and culture system for bone marrow-derived precursor cells (DC) in rats.【Methods】 Granulocyte macrophage colony stimulating factor (GM-CSF) and interleukin -4 (IL-4) were used to induce and separate rat bone marrow hematopoietic progenitor cells to get immature dendritic cells after 7 d (imDC), immunomagnetic flow cytometry was used before and after purification of the surface of imDC to test positive expression rate of OX62. Lipopolysaccharide (LPS) lasted for 2 D to stimulate imDC maturation to get mature dendritic cells (mature dendritic cell, mDC), flow cytometry was used to detect imDC and mDC cell surface CD86, CD80, RT1B positive expression rate ; ELISA was used to detect the secretion of IL-12 in vitro in two groups of cells, mixed lymphocyte reaction (MLR) was used to determine lymphocyte proliferation ability of cells in two groups.【Results】Purified imDC surface OX62 positive expression rate was 91.9%, significantly higher than that before purification 82.8% (P<0.05); imDC, CD80, CD86 on the surface of the positive expression rates of RT1B were 11.48%, 6.25%, 77.2%, stimulated by LPS after the maturity of the mDC surface CD86, CD80 and RT1B positive expression rates were 91.38%, 93.54%, 98.6% imDC; cell IL-12 secretion level (36±9) pg/mL, was significantly lower than that of mDC (228 ±27) pg/mL (P<0.01) MLR results in mDC group; ability to stimulate T lymphocyte proliferation was significantly higher than that of the imDC group.【Conclusion】In this experiment, the cultured cells were identified by three aspects (cell morphology observation, surface molecular markers and functional status), and it was proved that the combination of GM-CSF+IL-4 could induce high purity and large numbers of DC in vitro; through the purification of immunomagnetic beads, a relatively large number of DC with high purity can be obtained to meet the experimental requirements.

8 Evaluation of AMH in Ovarian Function Before and After Laparoscopic Gynecological Surgery

【Objective】To evaluate the value of anti-mullerian hormone (AMH) in the evaluation of ovarian function before and after laparoscopic gynecologic surgery.【Methods】From January 2016 to December 2016, 370 cases of gynecological diseases diagnosed and treated in gynecology and obstetrics department in our hospital were selected.There were 80 cases in ovary group, 90 cases in hysteromyoma group and 200 cases in fallopian tube group. The levels of AMH were compared at the time before operation, 6 weeks and 6 months after operation.【Results】The serum AMH levels in patients with ovarian chocolate cysts and those with non-chocolate cysts at 6 weeks and 6 months after operation were significantly lower than those before operation (P<0.05). The level of serum AMH 6 of months after operation was significantly lower than that of 6 weeks after the operation (P<0.05).However, the level of AMH in patients with ovarian chocolate cysts was significantly lower than that in patients with non-chocolate cysts (P<0.05). There was no significant difference in serum AMH between subtotal hysterectomy patients and hysteromyomectomy patients (P>0.05). 6 weeks and 6 months after operation, the serum AMH level in both groups was significantly lower than that before operation (P<0.05), and the level of serum AMH at 6 months after operation in both groups was significantly lower than that at 6 weeks after operation (P<0.05). The serum AMH level of subtotal hysterectomy patients was lower than that of hysteromyomectomy patients in the same period. There was no significant difference in serum AMH level between the unilateral salpingotomy group and the unilateral salpingotomy group + the contralateral salpingotomy group (P>0.05). The serum AMH level in unilateral salpingotomy group at months after operation was significantly lower than that of preoperation (P<0.05). There was no significant difference in serum AMH levels between the unilateral salpingotomy group and the unilateral salpingotomy group + the contralateral salpingotomy group (P>0.05).【Conclusion】Six months after operation, laparoscopic excision of ovarian cysts had adverse effects on ovarian reserve function, and the ovarian reserve of chocolate ovarian cysts decreased more obviously than that of non-chocolate ovarian cysts. Laparoscopic subtotal hysterectomy has a negative effect on ovarian reserve, while uterine myomectomy has little effect. The unilateral salpingotomy group and the unilateral salpingotomy group + the contralateral salpingotomy group under laparoscopy have adverse effects on ovarian reserve function.Appropriate surgical methods should be adopted according to the patient's condition and requirement.

11 Effects of Metformin Combined with Liraglutide on Islet β Cell Function, Liver Function and Treg Cells in Peripheral Blood of Patients with T2DM and NAFLD

【Objective】To investigate the effects of metformin combined with liraglutide on islet β cell function, liver function and Treg cells in peripheral blood of patients with type 2 diabetes mellitus (T2DM) and non-alcoholic fatty liver (NAFLD).【Methods】From January 2015 to December 2016, 93 patients with T2DM complicated with NAFLD were selected and were randomly divided into the observation group (n=47) and the control group (n=46). Two groups of patients were treated with metformin, the observation group received subcutaneous injection of liraglutide, the control group were given subcutaneous injection of insulin glargine. The course of treatment was 3 months. The levels of HbAlcN, FPGN, 2hPGN, FINS, HOMA-IR, HOMA-β, ALT, AST, GGT, TC, TG, HDL-C, LDL-C, and the ratio of Treg cells in peripheral blood to CD4+ T cells were compared between the two groups before and after treatment, the decrease in the observation group was more significant than that in the control group (P<0.05). The Treg/CD4+ in peripheral blood of the two groups before treatment was 5.62 ±1.80 % and 5.74 ±1.53% respectively, there was no significant difference between the two groups (P>0.05).【Results】Compared with that before treatment, the level of HbAlc, FPG, 2hPG and HOMA-IR decreased after treatment in both groups (P<0.05) and the value of HOMA-βincreased significantly (P<0.05).The level of HbA1c in the observation was lower than that of the control group after treatment, the difference was significant (t=2.230, P=0.028), and the rest indexes has no statistical significance (P>0.05).There was no significant difference in liver function and lipid metabolism between the two groups before treatment (P>0.05). The levels of ALT, AST and GGT in the observation group were lower than those before treatment, and the difference was statistically significant (P<0.05). The levels of ALT, AST and GGT in the control group were not significantly different from those before treatment (P>0.05). The levels of TC, TG, HDL-C and LDL-C in the two groups were lower than those before treatment, the difference was statistically significant (P<0.05), and the Treg/CD4+ in the observation group was significantly lower than that in the control group (P<0.05), the difference was statistically significant (P<0.05).【Conclusion】Metformin combined with Lilalutide in the treatment of T2DM patients with NAFLD can effectively restore the function of islet β cells, reduce the degree of insulin resistance, and achieve the purpose of blood glucose control, which is worthy of clinical application.

15 Effect of Ginkgo Biloba Extract Combined with Comprehensive Audiovisual Training on Plasma Nitric Oxide, Oxidative Stress and Exercise Capacity in Patients with Parkinson's Disease

【Objective】To observe the effect of Ginkgo biloba extract combined with comprehensive visual and auditory training on the levels of plasma nitric oxide (NO), reduction of oxidative stress and capacity of exercise in patients with Parkinson's disease (PD).【Methods】A total of 60 patients with primary PD admitted to the Department of Neurology in the hospital during May 2014 to May 2016 were enrolled in the study. The patients were evenly divided into the observation group and the control group using a random number table, with 30 cases in each group. Patients in the control group weregiven Ginkgo biloba extract injectionswhile patients in the observation group were given visual and auditory combined training on the basis of Ginkgo biloba extract injections. After 3 months of treatment, the levels of serum vasoactive substances [ (NO, endothelin (ET)], indices of oxidative stress [superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), malondialdehyde (MDA)], scores on the Unified Parkinson's Disease Rating Scale (UPDRS) Part Ⅲ (UPDRS Ⅲ), scores on the Berg balance, and rates of adverse reactions during treatment were compared between the two groups.【Results】After treatment, NO, ET, SOD, MDA and GSH-Px changed significantly. NO, SOD and GSH-Px were significantly higher in the observation group than the control group (P<0.05) while ET and MDA were significantly lower than the control group (P<0.05). After treatment, the UPDRS Ⅲ score of the observation group was significantly lower and the Berg balance score was significantly higher than those of the control group (P<0.05). After treatment, the walk test time was significantly shorter in the observation group than the control group, and 6 minutes walking distance, stride length and walking speed were significantly higher in the observation group than the control group (P<0.05). There was no significant change of frequency in the two groups before and after treatment (P>0.05). There was no significant difference in the incidence of adverse reactions between the two groups during treatment (P>0.05).【Conclusion】Ginkgo biloba extract combined with audio-visual comprehensive training can significantly relieve oxidative stress as well as improve the balancing function and walking ability of patients. It is conductive to improving the motor function of patients and the low incidence of adverse reactions is low.

19 Effects of Amiodarone on Late Relapse and Atrial Structure after Radiofrequency Ablation for Paroxymal Atrial Fibrillation

【Objective】To investigate the effect of amiodarone on the recurrence and atrial structure in patients with paroxysmal atrial fibrillation after radiofrequency ablation.【Methods】The clinical data of 100 patients with paroxysmal atrial fibrillation admitted in our hospital from December 2012 to December 2014 were retrospectively analyzed. All patients were treated with radiofrequency ablation and were divided into two groups: control group (routine treatment + losartan, n=50) and observation group (treated with amiodarone on the basis of the control group). The recurrence after operation was compared between the two groups 1 week, 1 month, 3 months, 12 months, 18 months after treatment. The levels of serum hs-CRP were compared between the two groups before and after operation. The left atrial internal diameter (LAD) and the left atrial volume (LAV) were also compared.【Results】There was no significant difference in recurrence rate between the two groups at 1 week postoperatively (P>0.05). The recurrence rate at 1 month, 3 months, 12 months and 18 months after operation in the observation group was significantly lower than that in the control group, the difference was statistically significant (P<0.05).The serum hs-CRP levels in the observation group and the control group were significantly higher than those before operation at 3 months and 6 months after operation. The LAV in the control group was significantly lower than that before operation 18 months after operation, the difference was statistically significant (P<0.05). 6 months, 12 months and 18 months after operation, LAD and LAV in the observation group was significantly lower than that in the control group, the difference was statistically significant (P<0.05).【Conclusion】Amiodarone can reduce the inflammatory response in patients after radiofrequency ablation, and significantly reduce the short-term and long-term recurrence rate of paroxysmal atrial fibrillation after radiofrequency ablation. At the same time, it can reduce left atrial diameter and improve cardiac function.

22 Short Term Efficacy of Ticagrelor Combined with Thrombolytic Therapy for Patients with ST-segment Elevation Myocardial Infarction(STEMI) Complicated with Type 2 Diabetes and Its Effects on Level of Inflammatory Factors

【Objective】To explore the short term efficacy of ticagrelor combined with thrombolytic therapy for patients with ST-segment elevation myocardial infarction (STEMI) complicated with type 2 diabetes and its effects on level of inflammatory factors.【Methods】A total of 70 acute STEMI patients with type 2 diabetes, who underwent thrombolytic therapy in our hospital from February 2015 to February 2017, were randomly divided into the ticagrelor group (n=35) and the clopidogrel group (n=35) according to the random number table method. Both groups were given aspirin anti-platelet aggregation and other conventional treatment. In addition, the ticagrelor group was given ticagrelor and urokinase, while the clopidogrel group was given clopidogrel and urokinase The total clinical effective rate and the percentage of ST segment regression >50% after 1h and 2h thrombolysis were compared between the two groups. The levels of High sensitive C reactive protein (hs-CRP) and interleukin-6 (IL-6) before and 1d, 3d, 7d after treatment were detected. The incidence of recurrent myocardial infarction within 1 week, as well as the left ventricular end diastolic diameter (LVEDD)and left ventricular ejection fraction (LVEF)after 1 week were recorded.【Results】There were no significant differences in serum hs-CRP and IL-6 levels between the two groups before treatment (P> 0.05). The levels of hs-CRP and IL-6 in both groups were significantly lower at 1, 3 and 7 days after treatment compared to before treatment. Hs-CRP and IL-6 level changes in the ticagrelor group were significantly lower than those in the clopidogrel group; the difference was statistically significant (P <0.05). The total effective rates of the ticagrelor group were 80.0% (28/35) and 94.3% (33/35) after thrombolysis for 1and 2 h, respectively, which were significantly higher than those of the clopidogrel group 71.5% (25/35)and 88.6% (31/35), respectively; the differences were statistically significant (P<0.05).The percentage of ST segment regression >50% after 1h and 2h thrombolysis in the ticagrelor group were 74.3% and 97.1%, respectively, which were significant higher than those in the clopidogrel group 62.9%and 91.4%, respectively;those of the ticagrelor group were higher than the clopidogrel group (P<0.05). After 1 week of treatment, the LVEDD in the ticagrelor group was significantly lower than that in clopidogrel group (45.2 ± 3.4) mm vs (50.3 ± 3.9) mm, and the LVEF in the ticagrelor group was significantly higher than that in the clopidogrel group (62.8±4.2)% vs (55.2 ± 3.8)%; the difference was statistically significant (P <0.05). The incidence of reccurrent infarction within 1 week in the ticagrelor group was 0.0%, while the incidence in the clopidogrel group was 5.7% (2/35); the difference was statistically significant (P<0.05).【Conclusion】The short term efficacy of ticagrelor combined with thrombolytic therapy for patients with STEMI complicated with type 2 diabetes is good. The treatment significantly improves the success rate of thrombolysis and cardiac function, reduces inflammatory factors after myocardial infarction and the rate of recurrent myocardial infarctions.

26 Effect of Expression Levels of Serum Regenerating Gene Protein 4 and Carbohydrate Antigen19-9 in Patients with Early Gastric Cancer

【Objective】To explore the expression levels of serum regenerated gene protein 4 (REG4) and carbohydrate antigen 19-9 (CA19-9) in patients with early gastric cancer.【Methods】A total of 90 cases of patients diagnosed with early gastric cancer from June 2015 to June 2016 in our hospital were selected as the observation group and 90 healthy persons in the same time frame were selected as the control group. Blood samples from the two groups were collected. The serum REG4 expression levels were detected by enzyme linked immunosorbent assay (ELISA) and the serum CA19-9 expression levels were detected by chemiluminescence immunoassay. The histopathological parameters of tumor staging, tissue type, differentiation degree and distribution were determined by gastroscopy and histopathological examination. The serum REG4 and CA19-9 levels were then compared between the two groups. The relationships between the levels of serum REG4and CA19-9 with different gender, age and other pathological parameters in the observation group were analyzed.【Results】The levels of serum REG4 and CA19-9 in the observation group were significantly higher than those in the control group (P<0.05). The levels of serum REG4 and CA19-9 in stage Ⅰb patients were significantly higher than in stage Ⅰa patients (P<0.05). The levels of RGE4 and CA19-9 in the patients of highly differentiated were significantly higher than those in the moderately differentiated, while the patients of moderately differentiated were significantly higher than those in the low/undifferentiated (P<0.05). The levels of serum RGE4 and CA19-9 were significantly higher in the patients with tumor cells that infiltrated the submucosa than those inwith tumor cells that infiltrated the mucosa (P<0.05). The serum levels of RGE4 and CA19-9 in the patients of tumor diameter of >3 cm were significantly higher than in patients with tumor diameter of 2~3 cm (P<0.05); the patients with tumor diameter of 2~3 cm had significantly higher than serum levels of those with tumor diameter of 1~2 cm; the patients with tumor diameter of 1~2 cm had significantly higher serum levels than those with tumor diameter of <1 cm (P<0.05). There were no significant differences in REG4 and CA19-9 expression with differing gender, age, tissue type and tumor distribution (P>0.05).【Conclusion】The serum REG4 and CA19-9 levels are significantly elevated in early gastric cancer patients and are closely associated with tumor staging, differentiation degree, infiltration depth, and tumor size.

30 Comparison of Acesodyne and Hemostatic Effects of Three Filling Materials after Chronic Rhinosinusitis Function Endoscopic Sinus Surgery

【Objective】To compare acesodyne and hemostatic effects of three filling materials after function endoscopic sinus surgery in patients with chronic rhinosinusitis (CRS).【Methods】A total of 93 patients with chronic rhinosinusitis treated in our hospital from July 2013 to December 2015 were selected as the research objects. They were divided by random number table into group A, B and C (with 31 cases in each group). Function endoscopic sinus surgery was performed in all patients, vaseline gauze was used as filling materials to stop bleeding after the operation in group A, AVF expansion hemostatic sponges were used in group B, and self-made flixonase collagen sponges were used in group C. Pain VAS score, postoperative bleeding rate, epithelial process and adverse reaction rate at postoperative 4h, 12h, 24h, 48h in the three groups were compared.【Results】Group C was the lower than both groups A and B in VAS pain scores at postoperative 4h, 12h, 48h among the three groups, and the differences were statistically significant (P<0.05). The scores of groups A, B and C were (5.4±0.7)score, (4.7±0.5)score, (3.3±0.4)score respectively. The score in group B was higher than group A (P<0.05). Postoperative bleeding rates of group A, B and C were 71.0%, 64.5%, 54.8% respectively; there were no significant differences (P>0.05). Among the three groups, group C was the highest in mucous epithelization of operative cavity rate 3 months and 6 months after the operation; they were 71.0%, 96.8% respectively, which were higher than group A and B (all P<0.05), while there was no significant difference in group A and B (P>0.05). No filling related adverse reactions occurred in the three groups.【Conclusion】Hemostatic effect of vaseline gauze, AVF expansion hemostatic sponges and self-made flixonase collagen sponges to stop bleeding after chronic rhinosinusitis function endoscopic sinus surgery is similar, but the acesodyne effect of self-made flixonase collagen sponges is the most desirable. It is conducive to mucous epithelization of operative cavityand has the greatest clinical application value .

33 Effects of Dot Matrix Laser Combined with Compound Betamethasone Injection on Serum TGF-β1, BMP-7, EGF Level Changes and Prognosis in Patients with Hypertrophic Scar

【Objective】To investigate the effects of dot matrix laser combined with compound betamethasone injection on serum transforming growth factor-β1 (TGF-β1), bone morphogenetic protein-7 (BMP-7), epidermal growth factor (EGF) levels and prognosis in patients with hypertrophic scar (HS).【Methods】From January 2013 to October 2016, 98 patients with HS in our hospital were selected and divided into control group and observation group according to the treatment plan, 49 cases in each group. The control group was given simple dot matrix laser treatment, the observation group received compound betamethasone injection treatment on the basis of the control group. The serum TGF-β1, BMP-7, EGF levels, negative emotions [ (SAS) and depression (SDS)] scores before and after treatment and post-treatment scar (VSS) scores after treatment were compared between the two groups, and the incidence of adverse reactions was statistically analyzed.【Results】 (1)Serum-related factors: Before treatment, the serum TGF-β1, BMP-7 and EGF levels were not statistically significant between the two groups (P>0.05).But after treatment, the serum TGF-β1 and EGF levels in the observation group were significantly lower than those in the control group and BMP-7 was significantly higher than that in the control group (P<0.05); (2) Scar score: The VSS each dimension scores in the observation group were lower than those in the control group, the difference was statistically significant (P<0.05); (3) Adverse reactions: There was no significant difference in the incidence of adverse reactions between the observation group 12.24% (6/49) and the control group 10.20% (5/49) (P>0.05). 37 Comparison of Ultrasound-guided Continuous Femoral Nerve Block and Adductor Canal Block in Analgesia after Total Knee Arthroplasty

【Objective】To compare the effects of ultrasound-guided continuous femoral nerve block (CFNB) and adductor canal block (CACB) in analgesia after total knee arthroplasty (TKA).【Methods】A total of 86 patients who underwent unilateral TKA were randomly and equally divided into two groups. Patients undergoing ultrasound-guided CFNB on operation side were included as the CFNB group while patients undergoing ultrasound-guided CACB were included as the CACB group.The postoperative analgesic effects were compared between the two groups.【Results】There was no significant difference in heart rate (HR), mean arterial pressure (MAP) and oxygen saturation (SpO2) between the two groups at all time points (P>0.05). The resting-state and motion-state VAS scores showed a firstly increasing and then decreasing trend (P>0.05). There was no significant difference in the above-mentioned indices at all time points (P>0.05). The quadriceps femoris strength of two groups showed a gradually increasing trend (P<0.05). At time points 2, 6, 12 and 24hrs after operation, the scores in CACB group were significantly higher than those in CFNB group (P<0.05). The knee joint activity of two groups showed a gradually increasing trend (P<0.05). On the 1st, 3rd and 7th days after operation, the knee joint activities of CACB group were significantly better than those of CFNB group (P<0.05). There was no significant difference between the two groups in the number of medication for patient-controlled analgesia and the use of pethidine (P>0.05). The incidence of adverse reactions of opioid agents in the two group did not show any significant difference (P>0.05).【Conclusion】After TKA, the analgesic effects of CACB and CFNB are similar. However, the effect of CACB on quadriceps femoris strength and knee joint activity is less, which is slightly more conducive to postoperative rehabilitation.

【Objective】To observe the clinical efficacy of double micro catheter technique combined with endovascular embolization in the treatment of elderly patients with intracranial aneurysms.【Methods】The clinical data of 35 elderly patients with intracranial hemorrhagic aneurysms treated in our hospital from January 2013 to December 2016 were retrospectively analyzed. The patients were divided into two groups according to the different surgical methods: the observation group (treated by double microcatheter technique combined with interventional therapy) and the control group (treated by microsurgery, n=17). The Fugi-Meyer activity function score, clinical effect and complications were compared between the two groups before and after treatment.【Results】There was no significant difference in Fugi-Meyer score between the two groups before treatment (P>0.05). The Fugi-Meyer score after treatment in the observation group was significantly higher than that before treatment, and the Fugi-Meyer score in the observation group after treatment was significantly higher than that in the control group. The difference was statistically significant (P<0.05). The total effective rate of the observation group was 94.44% (17/18) and that of the control group was 82.35% (14/17), the difference between the two groups was statistically significant (χ2=4.752, P=0.021<0.05).【Conclusion】Double microcatheter technique combined with interventional embolization in the treatment of intracranial hemorrhagic aneurysms in the elderly has the advantages of good clinical effect, high safety and less complications, so it is worth popularizing in clinic.

43 Analysis of Sequential Interventional Therapy for Liver Metastasis from Colorectal Cancer

【Objective】To investigate the clinical effect of sequential interventional therapy for colorectal liver metastases of colorectal cancer and to explore an effective treatment for improving the prognosis.【Methods】A total of 126 colorectal cancer patients with liver metastasis who were treated in our hospital from October 2013 to October 2016 were selected as study subjects. According to the different treatment methods, they were divided into a routine chemotherapy group and sequential interventional therapy group, with 63 cases in each group. Patients in chemotherapy group received FOLFOX chemotherapy while patients in the sequential intervention group were treated with radiofrequency catheter ablation (TACE) and radiofrequency catheter ablation (RFA) at the third month and the sixth month after they developed liver metastases, followed by iodized oil emulsifier embolization via catheterization at the end of chemotherapy. The therapeutic effect, side effects and survival rate of the two groups were then compared.【Results】The proportion of complete remission (CR) and partial remission (PR) in the sequential intervention group was significantly higher than that in the chemotherapy group, and the disease progression (PD) rate was significantly lower than that in the chemotherapy group. The treatment efficacy rate of the sequential intervention group was significantly higher than that of the chemotherapy group (P<0.05); The rate of positive serum alpha-fetoprotein (AFP) returning to normal level in the sequential intervention group was higher than that in the chemotherapy group (P<0.05).The no change rate and increasing rate of AFP in sequential intervention group were significantly lower than those in the chemotherapy group (P<0.05); there was no difference in the serum descending rate (P>0.05). The 6-month, 12-month survival rate and median survival time in the sequential intervention group were significantly higher than those in the chemotherapy group (P<0.05); There was no significant difference in the incidence of side effects between the two groups (25.4% vs 20.63%, χ2=0.224, P=0.636;P>0.05).【Conclusion】Sequential interventional therapy in unresectable colorectal cancer patients with live metastasis is a reliable treatment method. It can extend the survival time of patients and reduce the adverse effects of chemotherapy. It is worth promoting sequential interventional therapy in clinical application.

46 Clinical Comparison of Laparoscopic and Traditional Trans-ventral Operation in the Treatment of Early Endometrial Cancer

【Objective】To clinically compare of laparoscopic surgery and traditional trans-ventral surgery in the treatment of early endometrial cancer.【Methods】A total of 96 patients with endometrial cancer undergoing hysterectomy in our hospital from April 2014 to April 2016 were selected and divided into two groups randomly: the traditional trans-ventral group and the laparoscopic group, with 48 cases in each group. All study subjects were given general anesthesia and routine sterilization. The patients of the traditional trans-ventral group received traditional trans-ventral operation, while the patients of laparoscopic group received laparoscopic surgery . The operation duration, total operative bleeding volume, postoperative exhaustion time, length of hospital stay, complication and recurrence rate after surgery of the two groups were compared and analyzed.【Results】The operation duration of laparoscopic group was (80.67±12.11)min, while that of the traditional transventral operation was (119.69±18.64)min. The total loss of blood during operation in the laparoscopic group was less than that of traditional trans-ventral group. Both the postoperative exhaustion time and length of hospital stay of the laparoscopic group were much shorter than those of the traditional trans-ventral group; the difference was statistically significant (P<0.05).The incidence of intraoperative complications (bladder injury, bowel injury, etc.) in the laparoscopic group was 2.0% (1/48), which was significantly lower than that in the traditional trans-ventral surgery group (16.7%, 8/48). Postoperative complications (venous thrombosis, ureteral fistula, incision infection, distant metastasis, etc.) and recurrence rate of the two groups showed no significant difference (P>0.05).【Conclusion】In the treatment of early endometrial cancer, laparoscopic surgery is superior to the traditional trans-ventral surgery in operation duration, total operative bleeding volume, and intraoperative complication rate. .The complication after operation and recurrence rate of two operation methods are essentially the same..

49 Clinical Observation of Reverse Island Flap of Dorsal Branch of Digital Artery in Reconstruction of Fingertip Defect

【Objective】To investigate the clinical effect of reconstruction sensory of reverse dorsal digital island flap of fingertip defects.【Methods】A total of 70 patients with 78 fingertip defects in our hospital from January 2015 to December 2016 were collected in the study. All patients underwent reconstruction sensory of reverse dorsal digital island flap and anastomosis of nerve branch and intrinsic nerve stump.【Results】In this study, skin blisters occurred in 5 cases of 7 fingers 3d after operation and healed after symptomatic treatment. The remaining flaps successfully survived and wounds healed very well. The rate of superficial sensory recovery at 6 months after operation was 100.00%, which was significantly higher than 76.92% and 0.00% at 3 months and 1 month after operation (P<0.05). All patients recovered from deep pain at one month after operation. At 6 months after operation, the two-point discrimination was (5.48±1.14) mm, which was significantly less than that of the postoperative 3 months (P<0.05). The sensory recovery of the flap at 6 months postoperatively was significantly better than that at 1 and 3 months postoperatively and the ratio of S4 reached 61.54% (48/78).【Conclusion】Reconstruction sensory of reverse dorsal digital island flap of fingertip defects is an effective method to restore the skin flap.

52 Efficacy Comparison between Non-cemented and Cemented Prosthesis on Bilateral Total Hip Arthroplasty in Elderly Patients with Femoral Neck Fracture

【Objective】To compare the clinical efficacy of simultaneous non-cemented and cemented prostheses on bilateral total hip arthroplasty in elderly patients with femoral neck fractures.【Methods】The clinical data of 147 patients with femoral neck fracture admitted to our hospital from March 2014 to June 2015 were analyzed retrospectively. According to the type of the material for the total hip arthroplasty, patients were divided into the non-cemented prosthesis group (NCP group) and cemented prosthesis group (CP group). The perioperative indexes, Harris score and excellent rate, postoperative pain and perioperative complications were compared between the two groups. The clinical efficacy of the two groups were evaluated.【Results】The operation time and intraoperative blood loss of patients in the NCP group were (89.6±5.8) min and (83.5±14.6) mL, respectively, which were significantly higher than those in the CP group, which were (119.7±8.2) and (112.8±21.3) mL (P<0.05), respectively. There was no significant difference in hospitalization time (d) between the two groups [ (9.6±1.7) vs (9.8±1.6) d] (P>0.05). The Harris score and excellent and good rate in the NCP group were at 3 and 6 months after operation were (72.3±5.9), (65.8±5.1) points and 72.2%, 56.9%, respectively, which were significantly higher than those in the CP group (61.5±4.4), (53.6±3.2) points and 52.0%, 37.3% (P<0.05), respectively, but the index was lower than that of CP group one month after operation. The incidence of postoperative pain (total pain rate was 33.3%) in the NCP group was lighter than that in the CP group (50.6%) (P<0.05). The incidence of perioperative complications (bed sores, urinary tract infection, incisional infection and adverse cardiovascular events) were also lower in the NCP group than the CP group (7.0% vs 20.0%, P<0.05).【Conclusion】Bilateral simultaneous non-cemented total hip arthroplasty in the treatment of femoral neck fracture in elderly patients has better clinical effect, which can effectively reduce the patient pain level and reduce complications during perioperative adverse cardiovascular events. The main shortcoming lies in that immediate fixation (short-term) ineffective. Therefore, clinicians should choose the appropriate implant for implantation according to the different types of prosthesis in order to improve the effect of hip joint fixation.

56 Survey on Sexual Function, Depression and Quality of Life in Patients with Cervical Cancer

【Objective】To survey on the status of sexual function, depression and quality of life in patients with cervical cancer and investigate the relationship among those situations .【Methods】A total of 137 women, who were diagnosed as cervical cancer in our hospital from June 2016 to June 2017, completed the questionnaire survey. Sexual function was evaluated by the Female Sexual Function Index (FSFI), depression was assessed by the Hospital Anxiety and Depression Scale (HADS), and the quality of life by the Functional Assessment of Cancer Therapy-General version 4 (FACT-G). Descriptive statistics, ANOVA, Scheffé's tests, and Pearson correlations were computed with software SPSS Win 21.0.【Results】The average scores of sexual function depression and the quality of life in the participants of this study were 4.83±1.16, 11.08±5.06 and 57.33±8.47, respectively. Among them, 31 patients (22.63%) had no depression, 46 patients (33.57%) had mild depression, and 60 patients (43.79%) had moderate to severe depression. The scores of sexual function and quality of life in patients of ages 20~39 were significantly higher than those of other age groups. The score of sexual function in patients below college education were significantly higher than those of patients with college education. Patients with employment had higher sexual function scores than those of non-employment patients. The score of sexual function of patients whose main caregivers were parents were higher than those whose main caregivers were their children, spouses and other caregivers. The scores of sexual function of patients with 5-fluorouracil plus cisplatin were higher than those of other chemotherapeutic drugs; the differences were statistically significant (P<0.05). Pearson correlation analysis showed that there was a significant negative correlation between sexual function and depression (r=-0.408, P<0.001), and a significant positively correlated with quality of life (r=0.303, P<0.001) in patients with cervical cancer. In addition, the survey showed sexual function was positively correlated with physical well-being, social well-being, and functional well-being (P=0.001), but not with psychological well-being (P=0.223).【Conclusion】This study reveals cervical cancer patients with lower sexual function tended to have lower quality of life and higher severity of depression, which should be actively intervened to improve the quality of life of those patients.

【Objective】To investigate the effect of inhalation anesthesia on E-selectin, matrix metalloproteinase (MMP) and immune function recovery in patients with lung cancer .【Methods】Eighty-six cases of radical resection of lung cancer were selected in our hospital and were randomly divided into sevoflurane group (n=43) and isoflurane group (n=43). The levels of E-selectin, MMP-2, MMP-9, interleukin-6 (IL-6), tumor necrosis factor -ɑ and soluble interleukin-2 receptor (sIL-2R) were monitored and compared at the time of anesthesia induction (T0), 1h after operation start (T1), 2h (T2) and 1h after operation (T3).【Results】The serum levels of E-selectin, MMP-2, MMP-9 and TNF-ɑ in the group A were lower than those in group B at time of T1、T2、T3 (P<0.05). The level of serum IL-6 in the group A was lower than that in group B at time of T1, T2 and T3 (P<0.05).There was no significant difference in the level of sIL-2R between the two groups at each time point (P>0.05).【Conclusion】Sevoflurane inhalation anesthesia can effectively inhibit the increase of E-selectin, MMP-2 and MMP-9, which is beneficial to the recovery of the immune function of the patients.

62 A Prospective Study of Partial Fistula Incision Combined with Anal Fistula Embolization in the Treatment of Sphincter Type Ⅰ Anal Fistula

【Objective】To investigate the clinical effect of partial fistula incision combined with anal fistula embolization in the treatment of sphincter type Ⅰ anal fistula.【Methods】From December 2013 to October 2016, 58 patients with sphincter type Ⅰ anal fistula in Beijing Erlonglu Hospital were randomly divided into the control group and the study group, with 29 cases in each group. Patients in the control group were treated with sphincter fistula ligation and patients in the study group were treated with partial fistula incision combined with anal fistula. Postoperative related indicators (scar area, wound healing time, pain duration, length of hospital stay) were measured, as well as the incidence of complications one month after surgery, anus function indexes (anal resting pressure, rectal resting pressure, anal maximum systolic pressure) pre-operatively and on the first day post-operation, and anal incontinence score (Wexner) and pain score (VAS) on the 1st and 7th day postoperation. The measurements were statistically compared between the two groups.【Results】The scar area, the time of wound healing, pain duration and the length of hospital stay in the study group were less than those in the control group, the differences were statistically significant (P <0.05); There were no significant differences in the anal resting pressure, rectal resting pressure, anal maximum systolic pressure between the two groups before operation (P>0.05), while these measurements in the study group after operation were significantly higher than those in the control group (P<0.05). The Wexner score of the two groups before operation showed no significant difference (P>0.05); however, the Wexner score on the first day after operation and VAS scores on the 1st and 7th day after operation in the study group were significantly lower than those in the control group (P<0.05). The incidence of complications in the study group 3.45% (1/29), which was lower than that in the control group (27.58%, 8/29); the difference was statistically significant (P<0.05).【Conclusion】Treatment of sphincter type Ⅰ anal fistula using partial fistula incision combined with anal fistula embolization shows better clinical effect with less trauma and pain after surgery. It does not affect the anus function greatly and has much less complications.

【Objective】To discuss effects of valsartan combined with atorvastatin on hs-CRP, LDL-C and SCr levels and kidney function of patients with early diabetic nephropathy.【Methods】From June 2013 to October 2016, 92 patients with DN were randomly divided into observation group and control group with 46 cases in each group. The control group was treated with valsartan alone, the observation group was treated with atorvastatin on the basis of the control group, and the LDL-C levels of hs-CRPN SCr were compared between the two groups before and after treatment. Renal function index and incidence of adverse reactions. The levels of hs-CRP, LDL-C, renal function indexesβ2-MG, GFR, UAER and incidence of adverse reaction were compared between the two groups before and after treatment.【Results】There was no significant difference in the levels of hs-CRP, SCR and LDL-C between the two groups before treatment (P>0.05). After treatment, the levels of hs-CRP, SCR and LDL-C decreased significantly in the two groups, and the difference was statistically significant (P<0.05). There was no significant difference in UAER, β2-MG and GFR between the two groups before treatment (P>0.05). After treatment, UAER, β2-MG and GFR decreased in both groups (P<0.05). UAER, β2-MG decreased significantly in the observation group (P<0.05). However, there was no significant difference in GFR between the two groups after treatment (P>0.05). The incidence of adverse reactions in the observation group was 19.57% (9/46), which was significantly lower than that in the control group 36.96 % (17/46), the difference was statistically significant (P<0.05).【Conclusion】Valsartan combined with Atovastatin in the treatment of early DN patients can effectively reduce the levels of hs-CRP, SCr and LDL-C, improve renal function and reduce the incidence of adverse reactions, which is worthy of clinical application.

68 Observation on Pathological Down-staging and the Tolerance of Patients with Colorectal Cancer Treated by Modified Short-course Radiotherapy

【Objective】To explore the effect of modified short course radiotherapy on pathological stage and patient tolerance in the treatment of colorectal cancer.【Methods】Seventy-four patients with colorectal cancer in our hospital were randomized into the observation group and the control group. Both groups were treated with neoadjuvant radiotherapy and chemotherapy, but the observation group was treated with modified short-course radiotherapy and the control group was treated with routine radiotherapy. Changes of clinical staging were compared between the two groups in 3 months postoperativly. Length of hospital stay and costs were recorded, and the radiotherapy related adverse reactions were evaluated to assess the patient's tolerance.【Results】There was a significant decrease in the staging of the two groups of patients after radiotherapy, and the difference was significant compared with that before the intervention (P<0.05). However, there were no significant differences between the two groups (P>0.05); Length of hospital stay, costs of radiotherapy and chemotherapy, and other expenses in the observation group were significantly lower than those in control group (P<0.05); There were no significant differences in the incidence rate and grade of bone marrow suppression, radiation dermatitis and radiation enteritis between the two groups (P>0.05).【Conclusion】The short-term down-staging rate and curative effects of modified short-course radiotherapy are similar to those of routine radiotherapy, and does not increase the incidence rates of adverse reactions induced by radiotherapy. It can shorten the length of hospital stay, and reduce financial burden of patients. It has high clinical application value.

【Objective】To explore the application and effect of PDCA (Plan-Do-Check-Action) cycle management on the infection control of the mother-infant ward.【Methods】PDCA cycle management was implemented in our hospital's maternity ward (mother-infant wards) since January 2015. We selected 280 cases of newborns before PDCA implementation (June 2013 to December 2014) and 280 cases after implementation (January 2015 to June 2016) for this study. The passing rate of health situation monitoring test and incidences of various types of infections before and after the implantation of PDCA cycle management were compared.【Results】After the implementation of PDCA cycle management, the passing rate of health situation monitoring such as the air of month-infant ward, nipples, feeding-bottles, disinfectant, object surfaces and medical personnel's hands were significantly higher than before the implementation (χ2=5.306~14.023, P<0.05, P<0.01). In addition, the incidences of neonatal pneumonia, thrush, diarrhea, abdominal distension and septicemia were significantly lower than before (χ2=4.353~29.232, P<0.05, P<0.01).【Conclusion】PDCA cycle management can significantly improve the health situation of mother-infant ward and reduce the incidence of neonatal infection.

73 Prognostic Value of Free Fatty Acid Level in the Cerebrospinal Fluid of Patients with Acute Ischemic Stroke

【Objective】To evaluate the prognostic value of free fatty acid (FFA) levels in cerebrospinal fluid (CSF) of patients d with acute ischemic stroke (AIS).【Methods】A total of 321 cases of acute ischemic stroke patients from January 2012 to December 2015 in our hospital were selected as study subjects. The National Institutes of Health Stroke Scale (NIHSS) score on admission was applied to assess specific severity degree of stroke. The levels of FFA from CSF and serum were measured using an enzyme cycling method. The prognostic outcomes of those stroke patients were evaluated by the modified Rankin scale scores 90days after admission. The prognostic value of FFA was analyzed by Logistic regression analysis.【Results】There was a positive relationship between CSF FFA and NIHSS score.Additionally, there was a positive relationship between the severity of stroke and serum levels of FFA. The FFA levels of CSF and serum in 231 patients with favorable outcome were significantly lower than those in 90 patients with unfavorable outcomes (P<0.05). The FFA levels of CSF and serum in 272 cases of survival patients were significantly lower than those in 49 dead patients (P<0.05). Multivariate analysis results showed that there was an increased risk of unfavorable outcome associated with FFA level in CSF ≥0.301 mmol/L (OR=5.28, P<0.0001).【Conclusion】The FFA level in CSF at admission was suggested to be an independent short-term prognostic marker in patient with AIS.

76 Expression of Androgen Receptor and p53 In the Prognosis of Three-Negative Breast Cancer

【Objective】To investigate the relationship between the expression of androgen receptor and p53 in the prognosis of three negative breast cancer.【Methods】From January 2010 to December 2011, the clinical data of 87 patients with TNBC (three negative group) and 231 patients without TNBC (non-three negative group) were collected. The expression of AR and p53 was detected by immunohistochemistry and its relationship with clinicopathological features and prognosis was analyzed .【Results】The positive rates of p53 and AR in the three negative group were 49.43 % (43/87) and 37.93 % (33/87), respectively, which were significantly higher than those in the non-three negative group p 33.77 % (78/231) and 27.3 % (63/231), the differences were statistically significant (χ2=9.451, P=0.025;χ2=8.287, P=0.031). The positive expression of AR was related to lymph node metastasis (P<0.05), the positive expression of p53 was related to histological grade and molecular type (P<0.05), there was no correlation between other markers and the positive expression of p53 and AR (P>0.05). The 5-year local recurrence rate and distant metastasis rate in the three negative group was significantly higher than those in the non-three negative group (P<0.05). The 5-year survival rate was significantly lower than that in non-three negative group (P<0.05).【Conclusion】The expression of p53 and AR in TNBC was up-regulated, which was related to the differentiation of tumor cells and the status of lymph node metastasis. p53 and AR can be used as predictors of prognosis in patients with breast cancer .

79 Therapeutic Effect of Gu Hong Injection on Angina Pectoris after PCI in Patients with Coronary Heart Disease and its Influence on Inflammatory Reaction and Vascular Endothelial Function

【Objective】To investigate the therapeutic effect of Gu Hong Injection on angina pectoris after percutaneous coronary intervention (PCI) in patients with coronary heart disease and its influence on inflammatory reaction and vascular endothelial function.【Methods】From January 2014 to December 2016, 114 patients with angina pectoris treated by PCI operation in our hospital were selected.According to the random number method; they were divided into observation group and control group with 57 cases in each group. Patients in the control group were treated with aspirin enteric-coated tablets, clopidogrel sulfate tablets, isosorbide mononitrate tablets and metoprolol tartrate tablets. The observation group was treated with Gu Hong Injection on the basis of the use of the control group. All the patients were treated for 2 weeks. The frequency and duration of angina pectoris before and after treatment were compared between the two groups. The therapeutic effect of angina pectoris was evaluated. Levels of serum Hs-CRP, tumor necrosis factor -α (TNF-α), interleukin-6 (IL-6) and endothelin-1 (ET-1) and nitric oxide (no) were compared between the two groups before and after treatment. At the same time, Doppler color ultrasound was used to evaluate vascular endothelium-dependent diastolic (FMD) function.【Results】After treatment, the attack frequency and duration of angina pectoris in the two groups were lower than those before treatment, and the attack frequency and duration of angina pectoris in the observation group were significantly lower than those in the control group. The difference was statistically significant (P<0.05). The total effective rate of treatment in the observation group was 91.2% (52/ 57), which was significantly higher than that in the control group 82.5% (47/ 57), and the difference was statistically significant (χ2=5.368, P<0.05). After treatment, the levels of serum Hs-CRP, TNF-and IL-6 were significantly decreased in both groups. The level of serum Hs-CRP, TNF-and IL-6 in the observation group was significantly lower than that in the control group (P<0.05). After treatment, the serum ET-1 level in the two groups was significantly decreased, the levels of NO and FMD increased significantly; the serum ET-1 level in the observation group was significantly lower than that in the control group. The NO and FMD were significantly higher than those of the control group, the difference was statistically significant (P<0.05). 【Conclusion】On the basis of routine treatment after PCI operation for coronary heart disease, Guhong injection significantly reduce the frequency of recurrent angina pectoris after PCI and the level of serum inflammatory factors, and improve vascular endothelial function.
