Association of KCNJ11 Promoter Methylation with Insulin in Patients with Newly Diagnosed Type 2 Diabetes Mellitus

【Objective】To investigate the relationship between KCNJ11 promoter methylation and insulin levels in patients with type 2 diabetes mellitus (T2DM).【Methods】From April 2014 to April 2015, 15 patients with T2DM initially treated in our hospital were selected. Those HbA1c>9% were enrolled in the blood-sugar severely elevated group (group A, n=5),and those HbA1c ≤9% were enrolled in the blood-sugar mildly elevated group (group B,n=10); 15 cases of normal physical examination at the same period were enrolled in the control group (group C); The DNA methylation level of 46 CpG sites in the 5 'end 300 bp of the KCNJ11 gene promoter was determined by the sulfite method.【Results】The BMI in the group A was higher than that in the group B and the group C, and the difference was significant (P<0.05); There was a significant difference in fasting blood glucose between the three groups (P<0.05);HbA1c was significantly different between the three groups in the one-one contrast (P<0.05); FIns comparison between the three groups had obvious difference (P<0.05); CpG+62, A, CpG+66, CpG+351, CpG+62 + CpG+66 methylation rate of Group A was lower than those of Group B and Group C, and Group B was lower than Group C, there were statistical differences (P<0.05); The methylation rates of 3 methylation sites in Group A and Group B were negatively correlated with FINS.【Conclusion】The methylation of KCNJ11 promoter region was negatively correlated with insulin levels in type 2 diabetes mellitus patients.

Short-term Efficacy of Microscope Assisted Periodontal Flap Surgery in the Treatment of Moderate to Severe Periodontitis

【Objective】To investigate the short-term efficacy of microscope assisted periodontal flap surgery in the treatment of moderate to severe periodontitis. 【Methods】From July 2013 to December 2014, the clinical data of 50 cases with moderate to severe periodontitis treated by microscope assisted periodontal flap surgery (study group) and 46 cases treated by periodontal flap surgery under common view (control group) were retrospectively analyzed. The oral health status [oral health index-simplified (OHI-S)]:Di-s and Ci-s, periodontal indexes probing depth [(PD), clinical attachment level (CAL), sulcus bleeding index (SBI)] were compared when patients were enrolled in the study (T0), 1 month (T1), 3 months (T2) and 6 months (T3) after surgery. The differences in pain scores [numerical rating scale (NRS)] and inflammatory factors [serum tumor necrosis factor α (TNF-α), interleukin -6 (IL-6), C-reactive protein (CRP)] before surgery, 24h and 72h after surgery were recorded. 【Results】At T1 to T3, the DI-S and CI-S indexes of the two groups were significantly lower than those of T0(P<0.05), but there was no significant difference between the groups at the same time point (P>0.05); PD, CAL and SBI with the two groups of patients were significantly lower than those of T0, and at the same time point, the observation group was significantly lower than the control group, the difference was statistically significant (P<0.05).At 24 h after operation, the scores of NRS in two groups were significantly higher than those before operation, but the score of observation group was lower than that of control group, the difference was statistically significant (P<0.05); At 72 h after operation, the scores of NRS in two groups were lower than those before operation and at 24 h after operation, and the score of observation group was lower than that of control group (P<0.05); At 24 h and 72 h after operation, the levels of TNF-α, IL-6 and CRP in the two groups were significantly lower than those before the operation, and the observation group was lower than that of the control group, the difference was statistically significant (P<0.05). 【Conclusion】Assisted periodontal flap surgery under microscope can effectively reduce the degree of pain in patients with moderate and severe periodontitis, improve the depth of periodontal pocket, reduce the loss of clinical attachment and relieve the bleeding of gingival sulcus, and is worthy of clinical application.

Comparative Study of Orthodontic Lingual Fixator and Lamination Retainer in Oral Orthodontic Mandibular Fixation

【Objective】To compare the application value of orthodontic lingual fixator and lamination retainer in oral orthodontic mandibular fixation. 【Methods】From January 2014 to December 2015 84 cases of oral orthodontic patients in our hospital were randomly divided into two groups, 42 cases in each group. Group A was wearing orthodontic lingual fixed retainer, while Group B group wearing retainer. After 1 year of follow-up, the Little anterior teeth irregularity index, the width of the canine tooth arch and the width of the molar arch were compared between the two groups when the appliance was removed and the retainer was worn 1 years later and periodontal tissue [plaque index (PI), gingival index (GI), calculus index (CI)], gingival crevicular fluid composition [(PS), sulfide alkaline phosphatase (ALP), aspartate aminotransferase (AST)] wearing after 1 week and 6 months after, and wearing comfort degree of the two groups were counted.【Results】(1) Maintainence effect: There was no significant difference between the Little index, the width of the dental arch and the width of the dental arch (P> 0.05) between the two groups at the time of removal and 1 year after the appliance of the retainer; 1 year after wearing the retainer, Little index, canine arch width, molar teeth, arch width of the two groups were changed, but the difference was not statistically significant comparing with fixed appliance removal (P>0.05);(2)Periodontal tissue: There was no significant difference in PI, GI and CI between the two groups after one week of wearing the holder (P>0.05); after 6 months, PI, GI and CI were increased in both groups, but the change of the group B was smaller than that of the group A,the difference was statistically significant (P<0.05); (3) Gingival crevicular fluid composition: After 1 week wearing the holder, the difference of PS and ALP and AST between the two groups was not statistically significant (P>0.05); after 6 months, the levels of PS and ALP and AST in the two groups were increased, but the change of the group B was smaller than that of the group A,the difference was statistically significant (P<0.05); (4) Comfort: Comfort of the A group was 92.86% (39/42), which was higher than that of the group B 73.81% (31/42), the difference was statistically significant (P<0.05). 【Conclusion】Both orthodontic tongue side fixation and lamination retainer can achieve good retention effect in the oral orthodontic mandibular fixation, the former has a high comfort but has a relatively large impact on the periodontal tissue of the patient, the latter has little effect on oral health, but the comfort and wear compliance are relatively low.

Value of UE Combined with Microcalcification Enhanced Technique in the Differential Diagnosis of Benign and Malignant Breast Nodules

【Objective】To analyze the clinical value of ultrasound elastography (UE) combined with microcalcification enhanced technique in differential diagnosis of benign and malignant breast nodules. 【Methods】The clinical data of 80 cases of patients with breast masses (95 nodules) who were examined with ultrasound in the hospital during March 2013 ~ February 2016 were retrospectively analyzed. All patients completed the two-dimensional ultrasound, UE and ultrasound firefly microcalcification enhancement scanning. Pathological findings were used as controls, and the clinical value of UE combined with microcalcification enhanced technique in differential diagnosis of benign and malignant breast nodules was analyzed. 【Results】The sensitivity, specificity and accuracy of two dimensional ultrasound in differential diagnosis of benign and malignant breast nodules were 85.19% (46/54), 65.85% (27/41) and 76.84% (73/95), respectively, while those of ultrasound firefly microcalcification enhanced technique were 85.71% (54/68), 77.78% (21/27) and 78.95% (75/95), respectively, those of UE Itoh 5 method were 88.14% (52/59), 77.78% (28/36) and 84.21% (80/95), respectively, and those of UE+firefly microcalcification enhanced technique were 95.08% (58/61), 94.12% (32/34) and 94.74% (90/95), respectively. The diagnostic specificity of UE+firefly microcalcification enhanced technique was higher than that of two dimensional ultrasound, firefly technique or UE Itoh 5 method (χ2=5.141, 4.026, 4.026, P<0.05), and its accuracy was higher than that of two dimensional ultrasound or firefly technique (χ2=4.014, 3.966,P<0.05). 【Conclusion】The combination of UE and firefly micro calcification enhancement technique has higher clinical value in identifying benign and malignant breast nodules, and has higher sensitivity, specificity and accuracy. It can be used as an effective means for screening breast diseases.

Application of Dexmedetomidine Combined with Sufentanil Conscious Sedation in Daytime Surgery

【Objective】To investigate the effect and feasibility of dexmedetomidine combined with sufentanil in sedation during the daytime surgery in the department of general surgery.【Methods】Six-four cases of patients undergoing daytime surgery were randomly divided into two groups by random, 32 cases in each group. Dexmedetomidine group (group A) received dexmedetomidine 0.3 g/kg before operation, and sufentanil 0.1g/kg 10min after intravenous injection. Dexmedetomidine was pumped between 0.2 and 0.4 g/kg min in operation to maintain anesthesia; Propofol group (group B) received sufentanil 0.1 g/kg + propofol 1.5mg/kg, intraoperative pumpe propofol 3~61.5 mg/(kg·h) for anesthesia maintenance. The operation of the two groups was compared, and the changes of mean arterial pressure point (MAP) , heart rate (HR), oxygen saturation (SpO2), OAA/S score at time points such as pre-operation (T0), the beginning of the operation (T1), 10min surgery (T2), 60min surgery (T3) and a chamber (T4) were recorded.【Results】The recovery time and length of stay in recovery room of the group A were shorter than those of the group B, and the difference was significant (P<0.05). T1~T3, MAP, SpO2 and OAA/S score of the group A were higher than those of the group B (P<0.05). There was no significant difference in HR between the two groups at different time points (P>0.05). There was no significant difference in operation memory between the two groups (P>0.05); The incidence rates of hypotension, lethargy and respiratory depression in the dexmedetomidine group were lower than those in the propofol group (P<0.05). 【Conclusion】Compared with propofol, the combination of dexmedetomidine and sufentanil for sedation during daytime surgery in the department of general surgery has more advantages in shortening postoperative wake time, maintaining hemodynamic stability and reducing adverse reactions. Although the effect of adaptive forgetting effect is relatively low, it will not affect the normal operation.

Study on the Relationship between Basilar Artery Tortuosity and Posterior Circulation Infarction

【Objective】To investigate the correlation between basilar artery tortuosity and posterior circulation infarction. 【Methods】A total of 87 patients with posterior circulation cerebral infarction who were admitted to the hospital between June 2014 and June 2016 were included in the cerebral infarction group. Another 60 healthy persons who underwent magnetic resonance angiography (MRA) in the same period were selected as the control group. The basic data of the two groups were analyzed, and the relationship between basilar artery tortuosity and posterior circulation cerebral infarction was analyzed by multivariate Logistic regression analysis. 【Results】The probabilities of diabetes, hypertension, smoking, basilar artery tortuosity ≥ grade 2 and vertebral artery dominance in the cerebral infarction group (40.23%, 65.52%, 33.33%, 56.32% and 63.22%, respectively) were higher than those in the control group (13.33%, 28.33%, 16.67%, 18.33%,41.67%, respectively) with significantly statistical difference (P < 0.05) The tortuosity coefficient (TC), theoretical basilar artery length (BAL) and bending length (BL) in the cerebral infarction group were higher than those in the control group (P<0.05). Logistic regression analysis showed that diabetes, hypertension, basilar artery tortuosity ≥ 2, TC and BL were the independent risk factors for posterior circulation cerebral infarction. 【Conclusion】The basilar artery tortuosity is related to the incidence of posterior circulation infarction. Diabetes, hypertension and smoking are the independent risk factors for posterior circulation cerebral infarction.

Study on the Correlation between the Severity of Chronic Hepatitis B and Serum Cholinesterase

【Objective】To investigate the serum cholinesterase (CHE) in patients with different degrees of chronic hepatitis B (CHB). 【Methods】The serum CHE, hyaluronic acid (HA), total bile acid (TBA) and coagulation indexes in 60 patients with CHB and 50 healthy persons (control group) were detected. 【Results】Levels of serum CHE and fibrinogen (FIB) in patients with mild, moderate and severe CHB were significantly lower than those in the control group, while levels of HA, TBA, thrombin time (TT), prothrombin time (PT) and activated partial thromboplastin time (APTT) were significantly higher or longer than those in the control group, and the difference was significanta (P<0.05). Serum CHE and FIB levels decreased with exacerbations of chronic hepatitis B, and HA, TBA increased, and TT, PT, and APTT were prolonged.Serum CHE and FIB levels decreased with exacerbations of chronic hepatitis B, and HA, TBA increased, and TT, PT, and APTT were prolonged.Pearson correlation analysis showed that serum CHE was negatively correlated with HA, TBA, TT, PT, APTT (P<0.05), and positively correlated with FIB (P<0.05).【Conclusion】Serum CHE can effectively reflect the severity of CHB, and its level is related to HA, TBA and the four indexes of coagulation.

Clinical Efficacy and Safety of Etanercept Combined with Methotrexate in Treatment of Moderate to Severe Rheumatoid Arthritis

【Objective】To study the clinical efficacy and safety of etanercept combined with methotrexate in the treatment of patients with moderate to severe rheumatoid arthritis (RA) . 【Methods】Eighty-four cases of severe RA patients were randomly divided into experimental group (etanercept + methotrexate) and control group (hormone + methotrexate), 42 cases in each group.The clinical efficacy and side effects of the two groups were compared after 24 weeks of continuous treatment.【Results】After 24 weeks of treatment, the synovial membrane thicknesss of erythrocyte sedimentation rate (ESR), C reactive protein (CRP), rheumatoid factor (RF), disease activity score (DAS28) and wrist, the second and third metacarpophalangeal joints (MCP2,3), the second and third proximal interphalangeal joints (PIP2,3) and the knee of the two groups were lower than those before treatment (P<0.05),and the experimental group was more significant than the control group (P<0.05);The efficacy of ACR20, ACR50 and ACR70 in the experimental group were 83.33% (35/42), 61.90% (26/42), and 42.86% (18/42),and that of the control group were 76.19% (32/42),47.62% (20/42), 21.43% (9/42); the efficiency of ACR70 in the experimental group was higher than that of the control group (P<0.05);The incidence of adverse reactions in the experimental group was 26.19%, which was higher than that in the control group (19.05%) (P<0.05).【Conclusion】Etanercept combined with methotrexate has better clinical outcome in treatment of severe active RA , but the incidence of adverse is higher.

Value of Indocyanine Green Excretion Test in Predicting Liver Injury in Patients of Multidrug-resistant Tuberculosis during Antituberculosis Treatment

【Objective】To assess the value of indocyanine green excretion test in predicting liver injury in patients of multidrug-resistant tuberculosis(MDR-TB) during antituberculosis treatment.【Methods】Clinical data of 137 patients with drug-resistant pulmonary tuberculosis treated in our hospital from January 2015 to December 2016 were analyzed retrospectively. 1d before the start of antituberculosis treatment and the first month and the third month after the start of treatment, indocyanine green (ICG) excretion test was carried out respectively to assess the patient's liver function; The ICG plasma clearance rate (PDRICG) and the 15 min retention rate (R15ICG) of ICG were detected and compared with the conventional liver function index.【Results】One month of after anti TB treatment, the patient's PDRICG decreased significantly and R15ICG increased significantly (P=0.007, 0.001); 3 months after treatment, PDRICG and R15ICG recovered significantly and almost returned to pre treatment levels (P=0.002,0.001); The levels of PDRICG and R15ICG were significantly worse before treatment (P=0.01,0.016) in patients with liver injury; Before the start of treatment, and in the first month and three months after the start of treatment, the PDRICG and R15ICG were significantly positively correlated with the corresponding time points of TBil (r=0.816 and 0.1767,P<0.001),and significantly negatively correlated with PA (r=-0.312 and -0.423,P< 0.001) and ALB (r=-0.231 and -0.177,P<0.001); there was no significant correlation between ALT, AST, PT, and INR (P>0.05).【Conclusion】PDR and R15 are valuable clinical trials to evaluate the liver injury in drug-resistant tuberculosis patients during anti-tuberculosis treatment and are worthy of popularization in clinical practice.

Efficacy of Voriconazole in the Treatment of Chronic Renal Insufficiency with Pulmonary Fungal Infection

【Objective】To investigate the effect of voriconazole in the treatment of chronic renal insufficiency(CRF) with pulmonary fungal infection(PFI).【Methods】A total of 82 CRF patients with PFI were divided into two groups , with 41 cases in each group. The control group and experimental group were each treated with fluconazole and voriconazole, respectively. The clinical efficacy, vital signs, renal function [blood urea nitrogen (BUN), creatinine clearance rate (Ccr), 24h urine protein quantitation], drug safety and the recurrence rate after one year after treatment were observed in both groups.【Results】After treatment, the total effective rate was 87.80% (36/41) in the experimental group, which was significantly higher than that in the control group 68.29% (28/41). The incidence rate of adverse reactions and one-year recurrence rate in the experimental group were lower than those in the control group (P<0.05).Before treatment, there were no significant differences in vital signs and renal function between the two groups (P>0.05). After treatment, body temperature, pulse, respiration, systolic blood pressure, diastolic blood pressure, BUN, and 24h urinary protein quantitation were significantly decreased in both groups, while Ccr was significantly increased. However, the change in these indicators in the experimental group was more significant than that in the control group (P<0.05).【Conclusion】Voriconazole is more effective in the treatment of CRF with PFI and it shows a safer and lower recurrence rate as well. It is worthy of promotion in clinical application.

Correlation between Dyslipidemia and Stroke in Patients undergoing Maintenance Hemodialysis

【Objective】To study the correlation between dyslipidemia and stroke in patients undergoing maintenance hemodialysis (MHD).【Methods】Atotal of 120 patients undergoing MHD (observation group) and 60 cases of healthy persons (control group) were enrolled in the study. According to the occurrence of stroke, the patients were divided into stroke group and non-stroke group. The blood lipids were compared among groups.【Results】Levels of triglyceride (TG), high-density lipoprotein cholesterol (HDL-C) and low-density lipoprotein cholesterol (LDL-C) showed significant differences between the observation group and the control group (P<0.05). Levels of TG and LDL-C in stroke group were (2.76±0.94) mmol/L and (3.41±0.72) mmol/L, respectively while those in the non-stroke group were (1.68±0.79) mmol/L and (2.75±0.73) mmol/L, respectively (P<0.05). Multivariate Logistic regression analysis showed that the independent risk factors for stroke in patients undergoing MHD included age, dialysis time, systolic blood pressure before dialysis, TG and LDL-C levels. 【Conclusion】Dyslipidemia easily occur in patients undergoing MHD. Advanced age, long dialysis time, systolic blood pressure before dialysis, TG and LDL-C levels are the risk factors for the occurrence of stroke.

Effects of Different Intensity of Physical Training Combined with Fluoxetine on Anxiety of College Students in Beijing Area

【Objective】To evaluate the effects of different intensity of physical training combined with fluoxetine on anxiety of college students in Beijing area , and provide reference for the improvement of anxiety of college students. 【Methods】Three hundred and twenty cases in Beijing area from February 2013 to February 2017 students diagnosed as anxiety were selected and randomly divided into high intensity group, moderate intensity group, low intensity group and the control group, 80 cases in each group. The subjects were given with fluoxetine treatment; the high intensity group, the moderate intensity group and the low intensity group carried out the corresponding intensity of physical training, 3 times a week for 14 weeks. Before and after the intervention ,the self rating Anxiety Scale (SAS), manifest anxiety scale (MAS) and symptom Checklist-90 (SCL-90) changes in anxiety factor scores were compared within the groups. 【Results】4 weeks after intervention, the scores of SAS, MAS and SCL-90 anxiety factors in the high-intensity group, the middle intensity group and the low intensity group were lower than those before intervention, and the middle intensity group was the most obvious, followed by the high-intensity group and the low intensity group;14 weeks after the intervention, SAS, MAS and SCL-90 anxiety factor score of the control group was recovered to the level before the intervention, SAS, MAS and SCL-90 anxiety factor score of the other three groups were significantly decreased after 4-week interventionin; the middle intensity group decreased most obviously, followed by the high intensity group and the low intensity group, the difference was statistically significant (P<0.05). 【Conclusion】On the basis of drug intervention, physical training combined with physical training can significantly improve anxiety among college students. Compared with high-intensity and low intensity physical training, moderate intensity exercise prescription has better effect on improving anxiety level.

Effect of Goserelin Combined with Bicalutamide Intermittent Treatment on Prostate Specific Antigen and Immune Function of Prostate Cancer Patients

【Objective】To study the effect of goserelin combined with bicalutamide intermittent treatment on prostate specific antigen and immune function of prostate cancer(PC) patients. 【Methods】A total of 85 patients with advanced PC were divided into the observation group (43 cases) and the control group (42 cases). Patients in the observation group received goserelin and bicalutamide intermittent treatment, while patients in the control group received goserelin and bicalutamide treatment continuously. Prostate specific antigen (PSA), vascular endothelial growth factor (VEGF), immune function, and adverse reactions before treatment and 3, 6, 9 and 12 months after treatment were compared between the two groups. 【Results】After 3 months, 6 months, 9 months, and 12 months treatment, serum PSA, F-PSA and VEGF in two groups were significantly lower than those before treatment (P<0.05). The content of CD3+, CD4+T, and ratio of CD4/CD8 in peripheral blood were significantly higher than those before treatment(P<0.05), while the content of CD8+ was signficantly lower than that before treatment(P<0.05).However, there were no statistical differences of PSA, F-PSA, VEGF, CD3+, CD4+T, CD4/CD8 and CD8+ in the two groups(P>0.05); And there were no statistical difference of adverse reactions between the two groups (6.98% vs 11.90%)(P>0.05). 【Conclusion】The efficacy of goserelin combined with bicalutamide by means of intermittent treatment is comparable to that of continuous treatment, which can effectively reduce the concentrations of PSA and regulate immune function.

【Objective】To investigate the effects of individualized nutritional intervention based on nutritional risk assessment on nutritional status and life quality of elderly chemotherapy patients with advanced colorectal cancer. 【Methods】A total of 114 cases of elderly chemotherapy patients with advanced colorectal cancer admitted to hospital from April 2014 to April 2016 were selected as the research objects and divided into the observation group and the control group using the random number table method, with 57 cases in each group. Patients in the control group received routine nursing intervention, while patients in the observation group were received individual nutritional intervention based on nutritional risk assessment. The nutritional status and life quality of patients in the two groups were compared. 【Results】The levels of serum pre-albumin (PA), albumin (ALB), hemoglobin (Hb) and other indicators in the observation group were significantly higher than those in the control group (P<0.05); the scores of life quality including cognitive function, physical function, social function and emotional role were significantly higher than those in Control Group(P<0.05). 【Conclusion】Individualized nutritional intervention based on nutritional risk assessment can significantly improve the nutritional status as well as the life quality of elderly chemotherapy patients with advanced colorectal cancer.

Protective Effects of Dexmedetomidine on the Brain during Anesthesia of Heart Valve Replacement Surgery

【Objective】To evaluate Dexmedetomidine for its cerebral protective effects during anesthesia of heart valve replacement surgery. 【Methods】A total of 100 patients undergoing heart valve replacement under extracorporeal circulation were randomly divided into the observation group and the control group (n=50). Patients in the observation group received intravenous infusion of dexmedetomidine (1.0 μg/kg) 10 min before the induction of conventional anesthesia, followed by intravenous infusion of 0.5 μg/(kg·h) to the end of surgery. The control group received the same procedure with the same amount of saline. Carotid venous blood oxygen saturation (jugular venous oxygen saturation, SjvO2), internal jugular venous blood oxygen content (arterial venous oxygen content difference, Da-jvO2) and cerebral oxygen uptake rate (cerebral extraction of oxygen, CERO2) were recorded at the following time points: immediately before the start of extracorporeal circulation (T1), opening the ascending aorta (T2), 10 min after extracorporeal circulation ending(T3), 6 h after surgery (T4) and 24 h after surgery(T5). Meanwhile serum S-100β protein and neuron specific enolization enzyme (neuronspecific enolase, NSE) level were measured as well. 【Results】Compared to T1 time point, levels of SjvO2 and S-100β, NSE at other time points was significantly increased in both the observation group and the control group, while CERO2 was significantly decreased (P<0.05). At T2 and T3, SjvO2 was significantly higher in the observation group than in the control group. Da-jvO2, CERO2, S-100β protein and NSE were significantly lower than in the control group (all P<0.05).【Conclusion】Dexmedetomidine can improve cerebral oxygen metabolism, reduce cerebral ischemia and hypoxia and, to some extent, provide neuroprotective effects during anethesia of heart valve replacement surgery.

Effect of Laparoscopic Assisted Vaginal Surgery on Serum Levels of IGF-I, CA125 and Quality of Life in Elderly Patients with Early Endometrial Cancer

【Objective】To study the effect of laparoscopic assisted vaginal surgery on serum levels of IGF-I, CA125 and quality of life in elderly patients with early endometrial cancer. 【Methods】One hundred and four elderly patients with early endometrial cancer were randomly divided into the study group and the control group with 52 patients in each group. The control group was treated with traditional open surgery, and the study group were treated with laparoscopic assisted vaginal surgery. The time of operation, the amount of blood loss, the number of lymph node dissection, the time of anal exhaust and the time of hospitalization and the incidence of postoperative complications, and the plasma levels of CA125, IGF- 1 and quality of life SF-36 were measured before and after the operation were observed. The recurrence, metastasis rate and 1 year survival rate were counted and compared between the two groups.【Results】The amount of blood loss, the time of anal exhaust, the time of hospitalization and the incidence of postoperative complications of the study group were significantly lower than those of the control group(P<0.05). The operation time and the number of lymph node dissection in the 2 groups were not statistically significant (P>0.05). The serum levels of CA125 and IGF-1 before and after operation were not statistically significant (P>0.05). There was no significant difference in the 1 year survival rate and recurrence rate between the two groups (P>0.05);The SF-36 quality of life scale, total physiological score and psychological scores of the observation group were significantly higher than those of the control group (P<0.05). 【Conclusion】The laparoscopic assisted vaginal surgery for early stage endometrial carcinoma in the elderly can achieve the same extent as open surgery in range of operation, effect of resection and postoperative survival rate, and can reduce surgical trauma and the incidence of postoperative complications, improve the quality of life, which is worthy of clinical application.

【Objective】To evaluate the diagnostic effect of echocardiography on left heart valve calcification. 【Methods】A total of 86 patients with left heart valve calcification were enrolled in our hospital from January 2016 to December 2016, and underwent transthoracic echocardiography. 【Results】In 86 patients with left heart valve calcification, aortic valve calcification was the largest, accounting for 81.40% (70/86), and aortic valve insufficiency was 41.86% (36/86);In 86 patients with left ventricular valve calcification, only 31.40% (27/86) of their left atrium increased ,simple left ventricular enlargement accounted for 13.95% (12/86); left atrium and left ventricle increased, accounting for 12.79% (11/86);Left ventricular hypertrophy was seen, in which 63 cases had ventricular thickening of 1~5 mm, accounting for 73.26% (63/86).Left ventricular diastolic function decreased in 72 cases, accounting for 83.27% (72/86).In the left ventricle of 86 patients with left ventricular valve calcification, the aortic reflex enhanced and the elasticity became worse.The aortic internal diameter was 30~35 mm, accounting for 62.79% (54/86),those of 35~40 mm accounted for 31.40% (27/86),and those >40 mm accounted for 5.81% (5/86).【Conclusion】Echocardiography can better reflect the location of calcification and the changes of the structure and function of the heart and aorta, and it is easy to operate and worthy of clinical application.

Meta-analysis of Efficacy and Safety of Insulin and Oral Anti-diabetic Drugs in the Initial Treatment of Newly Diagnosed Type 2 Diabetes

【Objective】To systematically review the efficacy and safety of insulin and oral anti-diabetic drugs in the treatment of newly diagnosed type 2 diabetes (T2DM) patients. 【Methods】We searched the database to collect published randomized controlled trials (RCTs) of insulin therapy and oral anti-diabetic drugs(OADs) in the treatment of newly diagnosed T2DM patients. Clinical characteristics and references' quality were evaluated by Cochrane bias risk assessment tools. The Meta-analysis was performed by using the RevMan 5.3 software. 【Results】Ten articles including a total of 807 cases were analyzed. The meta-analysis shows that:① in the patients of newly diagnosed diabetes, the reductions of FPG, 2hPG and HbA1c were significantly more than in insulin group than in OADs group;② the improvement of β-cell function (HOMA- β) and remission rate were significantly better in insulin group than in OADs group;③ there were no significant difference in hypoglycemic events or weight changes during treatment between the two groups. 【Conclusion】For newly diagnosed type 2 diabetes, initial use of insulin therapy can more effectively control blood glucose, and relieve HOMAβ compared to use of OADs. There were no significant difference in hypoglycemic events and body weight changes of the two treatments.

Anesthetic Effects of Sciatic Nerve Block and Femoral Nerve Block Guided by Ultrasound and Nerve Stimulator in High Ligation and Stripping of Great Saphenous Vein

【Objective】To compare and analyze the anesthetic effects of sciatic nerve block and femoral nerve block guided by Ultrasound and nerve stimulator in high ligation and stripping of great saphenous vein. 【Methods】Seventy-two cases of patients with varicosis of great saphenous veins who were going to be treated with surgery under nerve block anesthesia were randomly divided into the observation group (ultrasound guided) and the control group (nerve stimulator guided). The block effects were compared between the two groups. 【Results】The success rate of block in the observation group was 100.00% while that in the control group was 88.89% (P<0.05). The onset time of sensory block was significantly earlier, duration was significantly longer and time of firstly using auxiliary analgesics was significantly later in the observation group than the control group (P<0.05). The excellent rate of anesthesia in the observation group was 97.22% while that in the control group was 83.33% (P<0.05). The excellent and good rate in the two groups was 100.00%. The total score of anesthetics around neural stem in the observation group was (136.8±6.34) while that in the control group was (115.36±10.58),the difference was significant (P<0.05). There was no significant difference between the two groups in the incidence of postoperative complications (P>0.05). 【Conclusion】Compared with the nerve stimulator, the success rate of ultrasound guided sciatic nerve and femoral nerve block is significantly higher, the onset time is earlier, the duration is longer, and the quality of block is higher.

Clinical Efficacy of Two Kinds of Nephron-Sparing Surgeries in the Treatment of Stage T1 Renal Carcinoma

【Objective】To investigate the clinical efficacy of two kinds of nephron-sparing surgeries in the treatment of stage T1 renal carcinoma. 【Methods】The clinical data of 74 patients with stage T1a renal carcinoma treated by nephron-sparing surgery (NSS) in our hospital from January 2014 to December 2015 were retrospectively analyzed. According to the surgical method, the patients were divided into the laparoscopic group (40 cases) and laparotomy group (34 cases). The state of surgery and postoperative recovery, the incidence of complications and the positive rate of resection margin were compared between the two groups. The serum creatinine (SCr) level was determined, and the total glomerular filtration rates (GRF) of double kidneys were calculated. All patients were followed up for 7~28 months. The state of recurrence and metastasis in the two groups was recorded. 【Results】There was no significant difference between the two groups in postoperative drainage time and the positive rate of resection margin (P>0.05). The surgical time and renal ischemia time of the laparoscopic group were longer than those of the laparotomy group, while the intraoperative blood loss and length of hospital stay were less and shorter than those of the laparotomy control group (P<0.05). After surgery, GFR levels decreased in the two groups (P<0.05), but there was no significant difference between the two groups (P>0.05). The incidence of postoperative complications was lower in the laparoscopic group than in the laparotomy group (10.0% vs 29.41%) (χ2=4.515,P<0.05). There was no recurrence or metastasis in the two groups. 【Conclusion】The tumor control effect of laparoscopic NSS and open NSS is similar. However, laparoscopic NSS can shorten the postoperative recovery time and reduce the incidence of postoperative complications. Though the time of renal ischemia is prolonged, renal damage is not aggravated.

Clinical Characteristics and Therapeutic Effect of 16 Patients with Bilateral Sudden Deafness

【Objective】To investigate the clinical characteristics and therapeutic effect of bilateral sudden deafness. 【Methods】The clinical data of 16 patients with bilateral sudden deafness admitted to our hospital in 2014 were analyzed retrospectively. 【Results】Among the 16 patients, there were 7 patients with tinnitus in which 4 cases were cured, 1 case was markedly effective, 1 case was effective and 1 case was ineffective.; The total effective rate was 85.7%. The remaining 9 patients who were not associated with tinnitus included 3 cases were cured, 1 case was markedly effective, 2 cases were effective and 3 cases were ineffective, the total effective rate was 55.6%. Of the 16 patients, 5 patients were under 60 years old. Among these, 4 cases were cured, 1 case was ineffective and the total effective rate was 80%. The remaining 11 cases were over 60 years old which included 3 cases that were cured, 2 cases that were markedly effective, 3 cases that were effective and 3 cases that were ineffective; the total effective rate was 72.7%. The total effective rate of patients with tinnitus and age ≤ 60 years seemed higher than of those who were not associated with tinnitus, but the difference was not statistically significant (P>0.05).【Conclusion】Bilateral sudden deafness is a complicated disease afflicting many people. Its clinical features are complicated. It is necessary to adopt different treatment methods for different types of hearing curves.

Effect of Ursodeoxycholic Acid Combined with Ademetionine on the Liver Function and Pregnancy Outcome in Patients with Intrahepatic Cholestasis of Pregnancy

【Objective】To investigate the effect of ursodeoxycholic acid combined with methionine on liver function and pregnancy outcome in patients with intrahepatic cholestasis of pregnancy (ICP).【Methods】A total of 84 patients with ICP from February 2014 to October 2016 in our hospital were divided into two groups according to the treatment plan, each with a total of 42 cases. On the basis of conventional therapy, the control group was given ursodeoxycholic acid treatment, the observation group was given ursodeoxycholic acid + ademetionine treatment. Before and after treatment, serum total bile acid (TBA), liver function index [aspartate aminotransferase (AST), alanine aminotransferase (ALT) levels, pruritus (Ribalta) score, negative emotional anxiety (HAMA), depression (HAMD)] score of the two groups were compared. The pregnancy outcome and the neonatal Apgar score of the two groups were counted. 【Results】①serum TBA and liver function: before treatment, there was no significant difference in serum TBA, AST and ALT levels between the two groups (P>0.05). After treatment, the serum levels of TBA, AST and ALT in the observation group were lower than those in the control group, the difference was statistically significant (P<0.05);②pruritus: before treatment, there was no significant difference in Ribalta score between the two groups (P>0.05). After treatment, the Ribalta scores of the two groups were reduced, and the observation group was lower than the control group, the difference was statistically significant (P<0.05);③negative emotion: before treatment, there was no significant difference between the HAMA and HAMD scores of the two groups (P>0.05). After treatment, the scores of HAMA and HAMD in the observation group were lower than those in the control group, the difference was statistically significant (P<0.05);④pregnancy outcome and neonatal Apgar score: The rate of cesarean section, premature delivery and postpartum hemorrhage in the observation group was [28.57% (12/42), 9.52% (4/42), 4.76% (8/42), (2/42)], which were lower than those of the control group [50.00% (21/42), 26.19% (11/42), 19.05%(8/42)], Apgar score was higher than that of the control group, the difference was statistically significant (P<0.05), There was no significant difference in the incidence of postpartum hemorrhage and neonatal asphyxia between the two groups (P>0.05). 【Conclusion】Ursodeoxycholic acid combined with methionine treatment given for ICP patients can effectively reduce serum TBA levels, relieve itching symptoms, improve liver function and eliminate negative emotions, and thus help improve pregnancy outcomes.

Difference in the Incidence of Cardiovascular Events in Patients with Type 2 Diabetes Mellitus in Yi and Han Nationalities in Liangshan Area

【Objective】To study the incidence of cardiovascular events in patients with type 2 diabetes mellitus (T2DM) in Yi and Han nationalities in Liangshan area. 【Methods】A total of 290 patients with T2DM (Yi and Han nationalities) in Xichang city and rural areas near Xichang of Liangshan, Sichuan from May 2014 to May 2016 were selected as study subjects by the random stratified cluster sampling method. According to the presence of cardiovascular events, the patients were divided into the cardiovascular event group (n=96) and the non-cardiovascular event group (n=194). The clinical data and physiological and biochemical indexes of the patients were collected and detected. The incidence rate of cardiovascular events and risk factors were compared between the two nationalities. 【Results】The indexes of obesity, hypertension, dyslipidemia, smoking history, drinking history, body mass index (BMI), systolic blood pressure (SBP), diastolic blood pressure (DBP), total cholesterol (TC), triglyceride (TG), and low-density lipoprotein (LDL-C) in cardiovascular event group were higher than those in non-cardiovascular event group. There were significant differences between the two groups (P<0.05).However, high-density lipoprotein cholesterol (HDL-C) was lower in the cardiovascular group than that in the non-cardiovascular event group, and the difference was statistically significant (P<0.05). The incidence of cardiovascular events in patients with T2DM of Han nationality was higher than that in Yi nationality (37.0% vs 24.4%) (P<0.05). 【Conclusion】There is a higher risk of cardiovascular events in patients with T2DM of Han nationality than of Yi nationality in Liangshan. The risk of cardiovascular events in patients with T2DM of Yi and Han nationalities in the Liangshan area can be reduced by intervening the risk factors such as obesity, hypertension, dyslipidemia, smoking history and drinking history.

Effect of Frozen Thawed Embryo Transfer Cycle and Transplantation Time on Pregnancy Outcome in Patients with Repeated Implant Failure

【Objective】To analyze the effect of frozen-thawed embryo transfer (FET) cycle time on pregnancy outcome in patients with repeated RIF failure (repeated implantation failure).【Methods】A total of 80 patients with RIF undergoing FET in the hospital were randomly divided into control group and observation group, 40 cases in each group. The combination of down-regulation and hormone replacement was used for FET; In the control group, the embryos were transplanted at 3 d after progesterone administration; The observation group was delayed 4~5 D and underwent embryo transfer, and the changes of sex hormone levels and pregnancy outcomes were compared between the two groups.【Results】①There was no significant difference between the two groups in the number of embryos transferred, the number of high-quality embryos transferred, and the thickness of endometrium on the day of transplantation (P>0.05). ②Before transplantation, there was no statistically significant difference in sex hormone levels between the two groups (P>0.05). On the day of transplantation, the levels of luteinizing hormone (LH),follicle stimulating hormone (FSH) and progesterone (P) in observation group were lower than those in the control group, while estradiol (E2) level was higher than that in the control group (P<0.05). ③The clinical pregnancy rate of the observation group was higher than that of the control group (P<0.05), and the embryo implantation rate was slightly higher than that in the control group. The rates of early abortion, ectopic pregnancy and multiple pregnancy were slightly lower than those in control group (P>0.05). 【Conclusion】The delayed application of down regulation combined with hormone replacement for FET can improve the endometrial receptivity and the clinical pregnancy rate of patients with RIF.

Relationship between Adiponectin Dynamic Changes and Left Ventricular Systolic Function in Patients with Acute Myocardial Infarction before and after Percutaneous Coronary Intervention

【Objective】To explore the relationship between the adiponectin (APN) dynamic changes and left ventricular systolic function in patients with acute myocardial infarction (AMI) before and after percutaneous coronary intervention (PCI). 【Methods】A total of 100 cases of AMI patients treated with elective PCI in our hospital from January 2015 to January 2016 were selected as the observation group, and 90 healthy examiners with the matching gender and age were selected as the control group. The levels of APN were detected in the both group before PCI as well as 1 month and 3 months after operation. The two-dimensional speckle tracking imaging was used to detect the left ventricular systolic function [left ventricular end diastolic volume (LVESV), left ventricular end systolic diameter (LVESD) and left ventricular ejection fraction (LVEF)] of the observation group. The correlation between APN and LVESV, LVESD, LVEF was analyzed. 【Results】The APN level before operation, 1 month and 3 months after operation were lower in the observation group than those in the control group (P<0.05). The APN level of the observation group was higher at 1 month and 3 months after operation than it was before operation (P<0.05), and the APN level at 3 months after operation was higher than that at 1 month before operation (P<0.05). The levels of LVESV and LVESD at 1 month or 3 months after operation in the observation group were lower than those before operation, while the LVEF level was higher than that before operation (P<0.05).The levels of LVESV and LVESD at 3 months after operation were lower than those at 1 month after operation, while the LVEF level was higher than that at 1 month after operation (P<0.05). Pearson correlation analysis showed that serum APN was negatively correlated with the LVESV and LVESD (r=-0.716, -0.618,P<0.05), and positively correlated with LVEF (r=0.689,P<0.05). 【Conclusion】APN level of AMI patients after PCI is closely related with left ventricular systolic function.

Factors Relate to Postoperative Renal Stone Residue in Patients with Precutaneous Nephrolithotomy

【Objective】To explore the influencing factors of postoperative kidney stone residue in patients after percutaneous nephrolithotomy (PCNL) surgery.【Methods】A total of 279 patients with renal stones from February 2014 to April 2016 in our hospital were selected as the subject. All underwent percutaneous nephrolithotomy and postoperative kidney stone residue in each case was recorded. Stone residue and its related clinical data were analyzed by univariate and multivariate logistic regression analysis. 【Results】The kidney stones in the 279 patients showed a surgical removal rate of 83.15% and a residual rate of 16.85%. Stone residual rate in patients with stone diameter>1.0cm, mixed stones, stones load>700mm2, staghorn stones, multiple stones, renal insufficiency, and operative time> 60 min was significantly higher than that in patients without the above symptoms; the difference was statistically significant (P<0.05 ). Based on multivariate logistic regression analysis, independent risk factors for renal stone residue after PCNL were stone diameter >1.0cm, mixed stones, stones load>700mm2, staghorn stones, multiple stones, and renal dysfunction (P<0.05). 【Conclusion】There is a risk of residual kidney stones in patients after PCNL surgery. The influencing factors are stone diameter, stone load, presence of mixed stones, staghorn calculi, multiple stones, and renal dysfunction. Therefore, these independent risk factors should be paid more intention to kidney stone patients.

Observation on Effects of Quality Control Circle Activity on Negative Emotion and Pain Degree of Patients with Trauma in Emergency Department

【Objective】To analyze the effects of quality control circle activity on improving the negative emotion and pain degree of the patients with car accident trauma in the emergency department. 【Methods】A total of 5492 cases of car accident trauma in the Department of Emergency in our hospital during November 2015~2016 October were collected as the research object. During this period, our hospital developed the quality control circle (QCC) activity.Comparison of negative emotion, degree of pain and intangible achievement index among car accident trauma patients before and after the QCC activity was analyzed. 【Results】Patients' anxiety (SAS) and depression (SDS) scores after QCC activity were significantly lower than those before QCC activity (P<0.01). The average level of pain and the most severe degree of pain after QCC were significantly lower than before (P<0.01). After QCC activity, cohesion of the nursing staff team, sense of responsibility, problem-solving skills, quality management methods, harmonious degree score were significantly higher than before (P<0.05). 【Conclusion】The quality control circle activity is helpful to alleviate the negative emotion and degree of pain of the patients with trauma in the emergency department, and to enhance team cohesion and improve the quality of nursing.

Study on the Relationship between Hyperlipidemia and Progression and Prognosis of Acute Recurrent Pancreatitis

【Objective】To study the relationship between hyperlipidemia and the progression and prognosis of acute recurrent pancreatitis. 【Methods】A total of 110 patients with acute recurrent pancreatitis (ARP) were divided into the mild ARP group and the severe ARP group, according to the progression of disease. At the same time, 50 healthy persons were selected as the control group. The baseline data, blood lipid indexes, CRP levels, APACHEII score and mortality rate were compared between the two groups. Pearson correlation analysis was performed to analyze the relationship between blood lipid indexes and APACHEII score. 【Results】The levels of TG, HDL-C, LDL-C and CRP showed significant differences between groups (P<0.05). The APACHEII scores in the severe ARP were significantly higher than those in the mild ARP group before and after treatment (P<0.05). The mortality rates of the severe ARP group and the mild ARP group were 9.52% and 4.41%, respectively (P>0.05). Pearson correlation analysis showed that APACHEII score was related to TG, LDL-C and HLD-C (P<0.05). 【Conclusion】Hyperlipidemia is related to the severity of ARP. So, active treatment of ARP and simultaneous lipid-lowering intervention can effectively improve the prognosis of patients.

Significance of Ultrasound Cardiac Structure in Evaluation of Non-valvula Atrial Fibrillation Stroke Risks

【Objective】To explore the significance of ultrasound cardiac structure in evaluation of non-valvular atrial fibrillation (AF) stroke risks. 【Methods】A total of 91 AF patients, including 38 cases with stroke and 53 cases without stroke were collected retrospectively. The data were compared to 102 recently recruited AF patients, who served as the control group. All the subjects above accepted ultrasonic cardiogram to detect right atrium diameter (RAD), interventricular septal thickness (IVS), left ventricular diameter (LVD), posterior wall of left ventricle (LVPW) and left atrium diameter (LAD). Further analysis of the correlation between clinic symptom scores (CHADS2 and CHA2DS2-VASc scores) and cardiac structural parameters in the control group were evaluated.【Results】The retrospective study indicated that IVS (OR=2.698,P=0.018), LVPW (OR=2.703,P=0.013) and LAD (OR=3.708,P=0.022) were the risk factors of stroke in AF patients. Our study also showed that RAD (rs=0.562,P=0.020), IVS (rs=0.497,P=0.013), LVPW (rs=0.398,P=0.046) and LAD (rs=0.701,P=0.007) were positively correlated with CHADS2 scores in AF patients. Similarly, positive correlations were found between CHA2DS2 - VASc scores and IVS (rs=0.668,P=0.019), LVPW (rs=0.481,P=0.032), LAD (rs=0.649,P=0.011). 【Conclusion】Indicators of ultrasound cardiac structure have a certain significance in predicting the risk of stroke in non-valvular atrial fibrillation patients.

Therapeutic Effect of Two-port Thoracoscopic Lobectomy with Systematic Lymph Node Dissection on Non-small Cell Lung Cancer

【Objective】To study the therapeutic effect of two-port thoracoscopic lobectomy with systematic lymph node dissection on non-small cell lung cancer(NSCLC).【Methods】A total of 220 patients with NSCLC who were diagnosed and prepared to undergo lobectomy with systematic lymph node dissection from March 2012 to March 2014 in our department were enrolled in this study. Patients were randomly divided into the observation group and the control group. Patients in the observation group underwent two-port thoracoscopic lobectomy with systematic lymph node dissection while patients in the control group underwent routine three-port thoracoscopic lobectomy with systematic lymph node dissection. The operation duration, numbers and stations of lymph nodes dissected, numbers experienced intraoperative transt (open thoracotomy / two to three-port thoracoscopic lobectomy switch), ambulation time, chest tube duration, numbers of complications, hospital stay after surgery were observed in both groups. Visual analog scale(VAS)was used to evaluate the severity of postoperative pain of patients on the 1st, 3rd and 7th day after operation, and the analgesic dosages used after operation were also recorded. The follow-up information were collected, and the incidence rates of local recurrence and distant metastasis were compared. The 3-year overall survival rate and progress free survival rate were also analyzed. 【Results】No significances were found between the two groups in operation duration, numbers and stations of lymph nodes dissected, incidence rates of intraoperative transit , and chest tube duration(P>0.05). The ambulation time and hospital stay after surgery of the observation group were significantly shorter than those of the control group(P<0.05). The complication rate of the observation group was 26.21%, which was lower than that of the control group(26.47%), but the difference had no statistical significance(P>0.05). The VAS score of the observation group on the 1st day was lower than that of the control group with no significance(P>0.05), while the scores on the 3rd and 7th day were significantly lower than those of the control groups(P<0.05). The analgesic dosages used after operation of the observation group were significantly less than those of the control group(P<0.05). No significance existed between the two groups in the incidence rates of local recurrence and distant metastasis(P>0.05). The difference in 3-year overall survival rate, progress free survival rate and median survival time between the two groups were also not significance(P>0.05).【Conclusion】Compared to the three-port thoracoscopic lobectomy with systematic lymph node dissection, the two-port thoracoscopic lobectomy with systematic lymph node dissection can efficiently dissected the cancer tissue, and the therapeutic effect is desirable.It can alleviated postoperative pain and promote the rehabilitation of patients.
