Relationships of Prognosis with Levels of Serum Tumor Necrosis Factor-α and Procalcitonin in Patients with Lupus Nephritis Complicated with Acute Renal Injury
Abstract:【Objective】To explore the relationships of prognosis with levels of serum tumor necrosis factor-α (TNF-α) and procalcitonin (PCT) in patients with lupus nephritis (LN) complicated with acute renal injury (AKI). 【Methods】According to whether AKI occurred,136 patients with LN were divided into the AKI group (n=84) and the non-AKI group (n=52). Meanwhile,85 healthy volunteers in our hospital were selected as the control group. PCT,TNF-α,homocysteine (Hcy) and serum creatinine (SCr) levels were measured in all subjects. The 24-hour urinary protein level of LN patients was detected by urine analyzer and the sequential organ failure score (SOFA) of LN patients was evaluated. Patients in the AKI group were followed up for 3 months:they were divided into the survival group and the death group according to the survival situation. Bivariate Logistic regression was used to analyze the relationship between related indicators and prognosis. 【Results】Compared to the control group,the serum levels of PCT,TNF-α,SCr and Hcy in both AKI group and non-AKI group were higher (P<0.05). compared with the non-AKI group,24-hour urinary protein,SOFA score and serum PCT,TNF-α,SCr and Hcy levels in the AKI group were higher (P<0.05). Compared with the survival group,the levels of serum PCT and TNF-α in the death group increased significantly (P<0.05). Using bivariate logistic regression analysis,serum PCT and TNF-α were risk factors for prognosis of lupus nephritis complicated with AKI (P<0.05). 【Conclusion】The levels of serum PCT and TNF-α in patients with lupus nephritis who complicated with AKI are much higher,which may provide a reference for predicting the prognosis of patients.
