705 Effects of Immersive Training Program on Emotional Regulation, Care Communication Ability and Job Burnout of ICU Medical Staff

【Objective】 To explore the effects of immersive training program on emotional regulation, care communication ability and job burnout of medical staff in Intensive Care Unit (ICU). 【Methods】 A total of 76 on-the-job ICU medical staff in our hospital from January 2022 to January 2024 were enrolled. The scores of emotional regulation belief [Emotional Regulation Belief Scale (ERBS)], caring ability [Caring Ability Inventory (CAI)], interpersonal communication competence [Interpersonal Communication Competence Questionnaire (ICCQ)] and job burnout [Maslach Burnout Inventory-General Survey (MBI-GS)] were compared before and after training. 【Results】 After training, the scores of regulatory value, emotional restraint, emotional coercion and total ERBS score of medical staff were higher than those before training (P<0.05). The scores of cognitive awareness, courage, patience and total CAI score after training were significantly higher than those before training (P<0.05). The scores of three dimensions including communication skills, communication cognition and communication tendency, as well as the total ICCQ score after training were higher than those before training (P<0.05). After training, the scores of emotional exhaustion, depersonalization, personal accomplishment and total MBI-GS score were lower than those before training (P<0.05). 【Conclusion】 The immersive training program adapted to ICU working scenarios can strengthen the emotional regulation belief of medical staff, improve their caring ability and interpersonal communication ability, and effectively alleviate job burnout.

708 Association between Neutrophil to Lymphocyte Ratio and All-Cause Mortality in Elderly Patients with Cardiovascular Disease

【Objective】 To investigate the relationship between neutrophil to lymphocyte ratio (NLR) and all-cause mortality in elderly patients with cardiovascular disease (CVD). 【Methods】 A total of 3206 elderly patients with CVD were included from the National Health and Nutrition Examination Survey of America from 1999 to 2018. The participants were divided into four groups according to the quartiles of NLR: Q1 (NLR<1.74), Q2 (1.74≤NLR< 2.37), Q3 (2.37≤NLR < 3.23), and Q4 (NLR≥3.23). The association between NLR and all-cause mortality was analyzed, and the predictive value of NLR for all-cause mortality was evaluated. 【Results】 The Kaplan-Meier survival analysis showed that the higher NLR level group had a lower survival rate. The Cox proportional hazards regression model showed that after adjusting for confounding factors, there is a significant positive association between elevated NLR levels and the risk of all-cause mortality. Compared with the lowest NLR level group Q1, the highest NLR level group Q4 had a 45% increased risk of all-cause mortality (P<0.05). Restricted cubic spline analysis showed a non-linear positive correlation between NLR and all-cause mortality risk (P<0.05). The time-dependent receiver operating characteristic (ROC) curve analysis showed that the area under the curve for predicting all-cause mortality in elderly CVD patients using NLR at 1, 3, 5, and 10 years were 0.649, 0.625, 0.617, and 0.597, respectively. 【Conclusion】 Elevated NLR levels are independently associated with increasing all-cause mortality in elderly patients with cardiovascular disease, which helps identify high-risk individuals in this population.

713 Application Effect of Safflower Extract Combined with Vacuum Sealing Drainage in the Treatment of Patients with Chronic Infected Wounds

【Objective】 To explore the application effect of safflower extract combined with vacuum sealing drainage in the treatment of patients with chronic infected wounds. 【Methods】 A total of 80 patients with chronic infected wounds admitted to our hospital from October 2023 to March 2024 were selected and divided into observation group and control group by random number table method, with 40 cases in each group. The observation group was treated with safflower extract combined with vacuum sealing drainage for wound management, while the control group received conventional debridement, dressing change combined with vacuum sealing drainage. The wound healing rate, pain degree [Visual Analogue Scale (VAS) score] and quality of life [Wound Quality of Life Scale (Wound-QoL) score] were compared between the two groups. 【Results】 At 1, 2 and 4 weeks after treatment, the wound healing rates of the observation group were (17.46±6.28)%, (36.51±8.34)% and (60.32±10.47)% respectively, which were significantly higher than (8.20±4.97)%, (21.31±7.12)% and (37.70±9.65)% of the control group (P<0.05). The VAS scores of the observation group at 1, 2 and 4 weeks after treatment were (4.8±1.1), (3.2±0.9) and (1.8±0.7), significantly lower than (5.6±1.2), (4.9±1.1) and (3.9±0.8) in the control group (P<0.05). The Wound-QoL scores of the observation group were significantly better than those of the control group at each time point after treatment (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】 Safflower extract combined with vacuum sealing drainage has a remarkable clinical effect in treating chronic infected wounds. It can promote wound healing, relieve pain and improve quality of life, which provides an effective method for the management of chronic infected wounds.

716 Effects of Endurance Active Exercise Training on Motor Endurance, Limb Function and Quality of Life in Patients with Cerebral Hemorrhage

【Objective】 To explore the effects of endurance active exercise training on motor endurance, limb function and quality of life in patients with intracerebral hemorrhage (ICH). 【Methods】 A total of 87 ICH patients admitted to our hospital from October 2022 to October 2024 were selected and divided into observation group (n=45) and control group (n=42) by random number table method. The control group received routine nursing, while the observation group received endurance active exercise training on the basis of routine nursing. Before and after intervention, the two groups were compared in limb function [Fugl-Meyer Assessment (FMA) score], motor endurance [10-meter Maximum Walking Speed (MWS), Timed Up and Go Test (TUGT)], and quality of life [Stroke Impact Scale (SIS) score]. 【Results】 After intervention, the upper and lower limb function scores of both groups were higher than those before intervention, and the observation group was higher than the control group (P<0.05). The MWS of both groups increased and TUGT time shortened after intervention; the observation group had higher MWS and shorter TUGT than the control group (P<0.05). The scores of strength, hand function, mobility, daily activities, memory and thinking, communication, emotion and participation in both groups were all higher than those before intervention, and the observation group was superior to the control group (P<0.05). 【Conclusion】 Endurance active exercise training can significantly improve limb function, enhance motor endurance and quality of life in ICH patients, which is worthy of clinical reference and promotion.

720 Effects of Intravenous Thrombolysis with rhTNK-tPA on Cardiac Function and Short-Term Neurological Function in Patients with Acute Cerebral Infarction

【Objective】 To explore the effects of intravenous thrombolysis with rhTNK-tPA on cardiac function and short-term neurological function in patients with acute cerebral infarction. 【Methods】 The clinical data of 106 patients with acute cerebral infarction admitted to our hospital from January 2022 to October 2023 were retrospectively analyzed. According to different treatment regimens, the patients were divided into Group A (treated with rhTNK-tPA intravenous thrombolysis) and Group B (treated with alteplase intravenous thrombolysis), with 53 cases in each group. The two groups were compared in cardiac function [Stroke Volume (SV), Left Ventricular Ejection Fraction (LVEF)], neurological function [National Institutes of Health Stroke Scale (NIHSS) score], activities of daily living [Barthel Index (BI) score] and the incidence of complications. 【Results】 At 21 days after treatment, LVEF and SV in both groups were higher than those before treatment, and Group A was higher than Group B (P<0.05). At 21 days after treatment, NIHSS scores of both groups were lower than before treatment, while BI scores were higher than before treatment; Group A had lower NIHSS scores and higher BI scores than Group B (P<0.05). The incidence of intracerebral hemorrhage after thrombolysis and the recurrence rate of cerebral infarction at 6 months after treatment in Group A were 13.21% (7/53) and 5.66% (3/53), respectively, versus 16.98% (9/53) and 11.32% (6/53) in Group B; there were no significant differences between the two groups (χ2=0.294, 1.093, all P>0.05). 【Conclusion】 Intravenous thrombolysis with rhTNK-tPA for acute ischemic stroke can more effectively improve patients' cardiac function and neurological function, and enhance activities of daily living. Its safety is equivalent to alteplase, which possesses important clinical application value.

723 Efficacy of Sequential Nutritional Support Combined with Refined Interventions in the Treatment of Patients with Stroke

【Objective】 To investigate the efficacy of sequential nutritional support combined with refined intervention measures in the treatment of patients with stroke. 【Methods】 A total of 78 patients with strokes were randomly divided into a control group and an observation group, with 39 patients in each group. The control group received routine nursing interventions, while the observation group received sequential nutritional support combined with refined interventions. Nutritional indicators, neurological function, swallowing function, quality of life, and the incidence of complications were compared between the two groups before and after intervention. 【Results】 After intervention, levels of hemoglobin, albumin, prealbumin, and total protein in the observation group were significantly higher than those in the control group (P<0.05). Swallowing function grades in the observation group were better than those in the control group (P<0.05). The National Institutes of Health Stroke Scale (NIHSS) scores in the observation group were lower than those in the control group (P<0.05), while World Health Organization Quality of Life-BREF (WHOQOL-BREF) scores in the observation group were higher than those in the control group (P<0.05). The total incidence of complications in the observation group was significantly lower than that in the control group (P<0.05). 【Conclusion】 Sequential nutritional support combined with refined intervention measures can significantly improve nutritional status, swallowing function, and neurological function in patients with stroke, reduce complications, and enhance quality of life.

727 Application of Gene Testing-Guided Individualized Medication in Patients with Primary Hypertension

【Objective】 To investigate the application of gene testing-guided individualized medication in patients with primary hypertension. 【Methods】 A total of 164 patients with primary hypertension admitted to our hospital from June 2023 to June 2025 were enrolled and randomly divided into a control group and an observation group, with 82 patients in each group. Patients in the control group received conventional antihypertensive therapy, while those in the observation group received gene testing-guided individualized medication. Clinical efficacy, blood pressure control [systolic blood pressure (SBP), diastolic blood pressure (DBP), and blood pressure control rate], treatment adherence [Hypertension Treatment Adherence Scale], and the incidence of adverse reactions were compared between the two groups. 【Results】 The total effective rate in the observation group was higher than that in the control group, with a statistically significant difference (P<0.05). After treatment, systolic blood pressure (SBP) and diastolic blood pressure (DBP) in the observation group were lower than those in the control group, and the blood pressure compliance rate was higher than that in the control group, with statistically significant differences (P<0.05). After treatment, the scores of medication compliance behavior, avoidance of inappropriate medication behavior, management of daily living activities, smoking and alcohol habit management behavior, as well as the total compliance score in both groups were higher than those before treatment. The observation group had higher scores of medication compliance behavior, avoidance of inappropriate medication behavior and total compliance score than the control group, with a statistically significant difference (P<0.05). The total incidence of adverse reactions in the observation group was lower than that in the control group, with a statistically significant difference (P<0.05). 【Conclusion】 Gene testing-guided individualized medication can significantly improve blood pressure control, enhance treatment adherence, and reduce drug-related adverse reactions in patients with primary hypertension, demonstrating high clinical application value.

731 Application Effect of Early Warning Risk Management Model in Prevention and Control of Vascular Crisis after Digit Replantation

【Objective】 To explore the application effect of early warning risk management model in the prevention and control of vascular crisis after digit replantation. 【Methods】 A total of 100 patients undergoing digit replantation for hand trauma in our hospital from January 2023 to January 2025 were selected and divided into control group (routine management mode) and observation group (early warning risk management model) by random number table method, with 50 cases in each group. The levels of stress hormones (cortisol, epinephrine, norepinephrine), stress response [Stress Response Questionnaire (SRQ) score], negative emotions [Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS) score] were compared between the two groups before and after intervention, and the incidence of postoperative vascular crisis was statistically analyzed. 【Results】 After intervention, the levels of cortisol, epinephrine and norepinephrine in both groups were lower than those before intervention, and the observation group was lower than the control group (P<0.05). After intervention, the SRQ scores of both groups decreased, and the observation group was significantly lower than the control group (P<0.05). The SAS and SDS scores of the observation group were lower than those of the control group after intervention (P<0.05). The incidence of vascular crisis in the observation group was significantly lower than that in the control group (χ2=4.891, P<0.05). 【Conclusion】 The early warning risk management model can effectively improve patients' physiological and psychological stress status, and exert a remarkable effect on preventing postoperative vascular crisis.

734 Predictive Value of Serum miR-122 and IBIL/CYP450 for Neonatal Hyperbilirubinemia Complicated with Bilirubin Encephalopathy

【Objective】 To explore the predictive efficacy of miR-122 and IBIL/CYP450 for bilirubin encephalopathy (ABE) in neonates with neonatal hyperbilirubinemia (NHB). 【Methods】 A total of 63 neonates with NHB admitted to our hospital from April 2021 to August 2024 were enrolled as observation group, and divided into complication subgroup (n=22) and simple hyperbilirubinemia subgroup (n=41) according to whether complicated with ABE. Meanwhile, 45 healthy neonates born in our hospital during the same period were selected as control group. Serum levels of miR-122, CYP450 and indirect bilirubin (IBIL) were compared among the three groups. Multivariate logistic regression analysis was used to screen the risk factors of ABE in NHB neonates. Receiver operating characteristic (ROC) curve was plotted to analyze the predictive value of miR-122 and IBIL/CYP450 for ABE in NHB neonates. 【Results】 The levels of miR-122, TSB, IBIL and IBIL/CYP450 in the simple subgroup were higher than those in the control group but lower than those in the complication subgroup (P<0.05). The CYP450 levels in the simple subgroup and complication subgroup were both higher than those in the control group (P<0.05), while there was no significant difference in CYP450 level between the simple subgroup and complication subgroup (P>0.05). Multivariate logistic regression analysis showed that elevated miR-122, TSB, IBIL and IBIL/CYP450 were independent risk factors for ABE in NHB neonates (P<0.05). ROC curve analysis indicated that the combined prediction efficacy of miR-122 and IBIL/CYP450 was superior to that of single index prediction (P<0.05). 【Conclusion】 The combination of miR-122 and IBIL/CYP450 has good application value in predicting ABE in neonates with NHB.

738 Application Effect of Individualized Comprehensive Airway Management Scheme Based on Nurse-Patient Collaborative Management in Patients with Bronchial Asthma

【Objective】 To explore the application effect of individualized comprehensive airway management scheme with nurse-patient collaborative management in patients with bronchial asthma. 【Methods】 A total of 92 patients with bronchial asthma admitted to our hospital from June 2022 to January 2025 were selected and divided into the control group (routine airway nursing) and the observation group (individualized comprehensive airway management scheme of nurse-patient collaborative management on the basis of routine nursing) by random number table method. Pulmonary function indicators [Forced Vital Capacity (FVC), Forced Expiratory Volume in One Second (FEV1), Peak Expiratory Flow (PEF)], quality of life [Asthma Quality of Life Questionnaire (AQLQ)], medication adherence [Medication Adherence Report Scale for Asthma (MARS-A)] and asthma control level [revised Asthma Control Test (r-ACT)] were compared between the two groups before intervention and 6 months after intervention. 【Results】 After intervention, the scores of FVC, FEV1, PEF, each dimension of AQLQ and MARS-A in the observation group were higher than those in the control group, while the r-ACT score was lower than that in the control group, with statistically significant differences (P<0.05). 【Conclusion】 The individualized comprehensive airway management scheme based on nurse-patient collaborative management can effectively improve pulmonary function, quality of life, medication adherence and disease control level of patients with bronchial asthma. It is an effective individualized nursing strategy and worthy of clinical promotion and application.

742 Intervention Effect of Whole-course Pharmaceutical Care Based on IMB Model on Elderly Patients with Type 2 Diabetes Mellitus in Community

【Objective】 To explore the intervention effect of whole-course pharmaceutical care based on Information-Motivation-Behavioral (IMB) model on elderly patients with type 2 diabetes mellitus in the community. 【Methods】 A total of 104 elderly patients with type 2 diabetes mellitus managed by Huamu Community Health Service Center in Pudong New Area of Shanghai from January 2023 to December 2024 were selected and divided into control group and observation group by random number table method, with 52 cases in each group. The control group received routine community pharmaceutical management intervention, while the observation group received whole-course pharmaceutical care intervention based on the IMB model. The blood glucose levels [Fasting Plasma Glucose (FPG), 2-hour Postprandial Plasma Glucose (2hPPG), Glycated Hemoglobin (HbA1c)], medication adherence [Morisky Medication Adherence Scale-8 (MMAS-8) score], incidence of adverse reactions, self-management ability [Summary of Diabetes Self-Care Activities (SDSCA) score] and quality of life [The 36-Item Short Form Health Survey (SF-36) score] were compared between the two groups. 【Results】 After intervention, the levels of FPG, 2hPPG and HbA1c in the observation group were lower than those in the control group (P<0.05); the MMAS-8 score, each dimension score of SDSCA and SF-36 in the observation group were higher than those in the control group; the total incidence of adverse reactions was lower than that in the control group, and all differences were statistically significant (P<0.05). 【Conclusion】 Whole-course pharmaceutical care based on the IMB model can effectively control blood glucose levels, improve medication adherence and self-management ability, optimize quality of life, and reduce the incidence of adverse reactions in elderly community patients with type 2 diabetes mellitus.

746 Efficacy of Hepatic Arterial Infusion Chemotherapy Combined with Transarterial Chemoembolization in the Treatment of Advanced Hepatocellular Carcinoma and Liver Metastases

【Objective】 To investigate the clinical efficacy of hepatic arterial infusion chemotherapy (HAIC) combined with transarterial chemoembolization (TACE) in patients with advanced hepatocellular carcinoma and liver metastases. 【Methods】 A total of 110 patients with advanced hepatocellular carcinoma or liver metastases treated in our hospital between April 2023 and May 2025 were included and divided into a control group and an observation group (n=55 in each group) according to different treatment regimens. The control group was treated with conventional TACE, whereas the observation group received TACE combined with HAIC. Clinical efficacy, tumor-related markers, liver function parameters, and the incidence of adverse reactions before and after treatment were compared between the two groups. 【Results】 The objective response rate and disease control rate in the observation group were higher than those in the control group (P<0.05). After treatment, serum alpha-fetoprotein levels and maximum tumor diameter were lower in the observation group compared with the control group (P<0.05). Liver function parameters in the observation group were lower, whereas serum albumin levels were higher than those in the control group (P<0.05). Health status scores of patients in the observation group were higher than those in the control group (P<0.05). No statistically significant differences were observed in the incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】 HAIC combined with TACE improves short-term efficacy and enhances liver function and functional status in patients with advanced hepatocellular carcinoma and liver metastases without significantly increasing adverse reactions.

750 Application Value of High-Frequency Ultrasonography Combined with TSH Detection in the Diagnosis of Thyroid Carcinoma and Analysis of its Clinicopathological Characteristics

【Objective】 To explore the application value of high-frequency ultrasonography combined with thyroid-stimulating hormone (TSH) detection in the diagnosis of thyroid carcinoma and the analysis of its clinicopathological characteristics. 【Methods】 The clinical data of 90 patients with thyroid carcinoma (observation group) admitted to The First People's Hospital of Zhengzhou from January 2022 to December 2024 were retrospectively analyzed, and the clinical data of 90 patients with thyroiditis admitted during the same period were selected as the control group. All patients underwent high-frequency ultrasonography and serum TSH detection. Receiver operating characteristic (ROC) curve was used to analyze the diagnostic value of combined high-frequency ultrasonography and TSH detection for thyroid carcinoma. Spearman correlation analysis was adopted to explore the correlations of high-frequency ultrasonic features and TSH levels with clinicopathological characteristics of thyroid carcinoma patients. 【Results】 There was no significant difference in the proportion of internal hypoechogenicity between the two groups (P>0.05). The proportions of microcalcification, irregular boundary, blood flow grade 2-3 and TSH level in the observation group were higher than those in the control group, while the proportions of peripheral anechoic area and blood flow grade 0-1 were lower than those in the control group (P<0.05). The sensitivity, specificity and area under the curve (AUC) of combined high-frequency ultrasonography and TSH detection in diagnosing thyroid carcinoma were 96.7%, 97.8% and 0.963 respectively, showing higher diagnostic efficacy than single high-frequency ultrasonography or single TSH detection. Spearman correlation analysis showed that tumor diameter, lymph node metastasis, clinical stage, tumor differentiation degree and extrathyroidal invasion were positively correlated with internal hypoechogenicity, irregular boundary, blood flow grade and TSH level (P<0.05); tumor diameter and lymph node metastasis were positively correlated with microcalcification and peripheral anechoic area (P<0.05); clinical stage, tumor differentiation degree were negatively correlated with microcalcification and peripheral anechoic area (P<0.05);extracellular infiltration is negatively correlated with microcalcification foci. 【Conclusion】 High-frequency ultrasonography combined with TSH detection has high diagnostic value for thyroid carcinoma. Ultrasonic features and TSH levels are significantly correlated with clinicopathological characteristics such as tumor diameter, lymph node metastasis and clinical stage, which can provide reference for the diagnosis and condition evaluation of thyroid carcinoma.

754 Effects of Integrated Intervention Mode of Self-Care plus Assisted Care Based on Multidisciplinary Cooperation Combined with Remote Support on Self-Care Ability and Quality of Life of Colorectal Cancer Stoma Patients

【Objective】 To explore the effect of the integrated intervention mode of self-care plus assisted care based on multidisciplinary cooperation combined with remote support on self-care ability and quality of life in patients with colorectal cancer stoma. 【Methods】 A total of 60 patients with colorectal cancer who underwent enterostomy in the Department of Gastrointestinal Surgery of our hospital from March 2024 to June 2025 were selected and divided into the control group and the observation group by random number table method, with 30 cases in each group. The control group received routine nursing, while the observation group adopted the integrated intervention mode of self-care plus assisted care based on multidisciplinary cooperation combined with remote support on the basis of routine nursing. The self-care ability [Exercise of Self-Care Agency Scale (ESCA)], quality of life [Chinese Version of Colorectal Ostomy Health Scale (C-COH)], anxiety emotion [Self-Rating Anxiety Scale (SAS)] and nutritional status [Patient-Generated Subjective Global Assessment (PG-SGA)] were compared between the two groups at discharge, 1 month and 3 months after discharge. 【Results】 At 1 month and 3 months after discharge, the ESCA and C-COH scores of both groups were higher than those at discharge, while the SAS and PG-SGA scores were lower than those at discharge. At discharge, 1 month and 3 months after discharge, the SAS and PG-SGA scores of the observation group were lower than those of the control group, and the ESCA and C-COH scores were significantly higher than those of the control group (P<0.05). There was no significant difference in the incidence of complications between the two groups (P>0.05). 【Conclusion】 The integrated intervention mode of self-care plus assisted care based on multidisciplinary cooperation combined with remote support can significantly improve patients' self-care ability and nutritional status, enhance quality of life and relieve anxiety, which is worthy of clinical promotion.

758 Effect of Dexmedetomidine Administered before Anesthesia Induction on Postoperative Cognitive Function in Elderly Patients Undergoing Gynecological Laparoscopic Surgery

【Objective】 To investigate the effect of dexmedetomidine (Dex) administered before anesthesia induction on postoperative cognitive function in elderly patients undergoing gynecological laparoscopic surgery. 【Methods】 A total of 88 elderly patients scheduled for gynecological laparoscopic surgery were randomly divided into an observation group and a control group, with 44 patients in each group. Before anesthesia induction, the observation group received 0.5 μg/kg Dex, while the control group received an equal volume of normal saline. Perioperative indicators were compared between the two groups. Diastolic blood pressure (DBP), systolic blood pressure (SBP), heart rate (HR), norepinephrine (NE), and renin activity were measured before administration (T0), 30 min after administration (T1), 10 min before extubation (T2), and 10 min after extubation (T3). Mini-Mental State Examination (MMSE) scores, Visual Analogue Scale (VAS) scores, and the incidence of adverse reactions were recorded before and after surgery. 【Results】 There were no statistically significant differences between the two groups in operation time, anesthesia time, intraoperative blood loss, or fluid infusion volume (P>0.05). The awakening agitation score (RS) in the observation group was lower than that in the control group (P<0.05). Significant differences were observed in SBP, DBP, and HR at T1, T2, and T3 between the two groups (P<0.05). NE and renin activity levels at T2 and T3 also showed significant differences between the two groups (P<0.05). VAS scores at postoperative day 3 were lower than those at postoperative day 1 in both groups (P<0.05), and the VAS scores in the control group were higher than those in the observation group (P<0.05). The postoperative MMSE score in the control group was lower than that before surgery of the control group and it was lower than that in the observation group after surgery (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】 Administration of Dex before anesthesia induction in elderly patients undergoing gynecological laparoscopic surgery can reduce the risk of postoperative cognitive dysfunction and postoperative adverse reactions, with good safety.

762 Effects of Comprehensive Nursing Based on Health Belief Model on Self-management Ability and Health Belief of Patients with Gestational Hypertension

【Objective】 To explore the effect of comprehensive nursing based on the health belief model on self-management ability and health belief in patients with gestational hypertension. 【Methods】 A total of 180 patients with gestational hypertension admitted to our hospital from May to December 2025 were enrolled and divided into the control group (receiving routine nursing intervention) and the observation group (receiving comprehensive nursing intervention based on health belief model) according to the admission time, with 90 cases in each group. The blood pressure [systolic blood pressure (SBP), diastolic blood pressure (DBP)], emotional status [Generalized Anxiety Disorder-7 (GAD-7) score, Patient Health Questionnaire-9 (PHQ-9) score], self-management ability [Hypertension Patients Self-Management Behavior Rating Scale (HPSMBRS) score] and health belief [Health Belief Model Scale (HBMS) score] were compared between the two groups. 【Results】 After intervention, the levels of SBP, DBP, GAD-7 score and PHQ-9 score in the observation group were lower than those in the control group (P<0.05), while the scores of all dimensions of HPSMBRS and HBMS in the observation group were higher than those in the control group (P<0.05). 【Conclusion】 Comprehensive nursing based on the health belief model can improve blood pressure status, relieve anxiety and depression, enhance self-management ability and strengthen health belief in patients with gestational hypertension.

766 Effects of Acupoint Injection Combined with Infrared Irradiation on SF-MPQ Score and Serum Substance P, IL-1β Levels in Patients with Intractable Postherpetic Neuralgia

【Objective】 To explore the effects of acupoint injection combined with infrared irradiation on the Short-Form McGill Pain Questionnaire (SF-MPQ) score and serum levels of substance P and interleukin-1β (IL-1β) in patients with intractable postherpetic neuralgia (PHN). 【Methods】 A total of 90 patients with intractable PHN admitted to our hospital from October 2022 to December 2024 were selected and divided into the observation group and the control group by random number table method, with 45 cases in each group. The observation group was treated with acupoint injection combined with infrared irradiation, while the control group received conventional infrared irradiation alone. Both groups were treated continuously for 4 weeks. Before and after treatment, the two groups were compared in pain degree [Pain Rating Index (PRI), Visual Analogue Scale (VAS) and Present Pain Intensity (PPI) in SF-MPQ], negative emotions [Patient Health Questionnaire-9 (PHQ-9), Generalized Anxiety Disorder-7 (GAD-7)], quality of life [Generic Quality of Life Inventory-74 (GQOL-74)], as well as serum levels of substance P and IL-1β. 【Results】 After treatment, the scores of PRI, VAS, PPI, PHQ-9 and GAD-7 in the observation group were significantly lower than those in the control group (P<0.05); serum substance P and IL-1β levels were lower than those in the control group (P<0.05); while the GQOL-74 score was higher than that in the control group (P<0.05). 【Conclusion】 Acupoint injection combined with infrared irradiation has a remarkable curative effect on intractable PHN. It can effectively relieve pain and negative emotions, and improve patients' quality of life, which is worthy of clinical promotion and application.

769 Value of Combined Detection of Hepatitis C Virus Antibody, Albumin and Alanine Aminotransferase in Diagnosis of Hepatitis C

【Objective】 This study aims to explore the application value of combined detection of hepatitis C virus antibody (HCV-Ab), albumin (ALB) and alanine aminotransferase (ALT) in the diagnosis of hepatitis C. 【Methods】 A total of 96 patients who underwent viral hepatitis screening in the hospital from January 2023 to January 2025 were selected as the observation group. Meanwhile, 96 healthy volunteers who received physical examination in the same hospital during the same period were selected as the control group. Venous blood samples were collected from both groups for the laboratory tests. The levels of HCV-Ab, ALB and ALT were compared between the two groups, and the diagnostic efficacy of HCV-Ab and hepatitis C virus ribonucleic acid (HCV-RNA) was analyzed.Plot the receiver operating characteristic (ROC) curve to analyze the diagnostic efficacy of each indicator alone and in combination for hepatitis C. 【Results】 With the HCV-RNA test result as the reference, among the 96 patients in the observation group, 56 were positive for HCV-RNA and 40 were negative; 52 were positive for HCV-Ab and 44 were negative. The HCV-Ab test result showed good consistency with the HCV-RNA test result (Kappa value=0.704). The levels of HCV-Ab and ALT in the observation group were significantly higher than those in the control group, while the ALB level was significantly lower than that in the control group, and all differences were statistically significant (all P<0.05). ROC curve analysis showed that the combination of HCV-Ab, ALB, and ALT for the diagnosis of hepatitis C had an AUC of 0.979 (95%CI: 0.957-1.000), with a sensitivity of 91.7% and specificity of 97.9%, which were significantly superior to those of each single indicator (P<0.05). 【Conclusion】 The combined detection of HCV-Ab, ALB, and ALT can more comprehensively reflect hepatitis C infection and liver function impairment by complementing the strengths of each indicator, thereby providing a more reliable laboratory reference scheme for the clinical diagnosis of hepatitis C.

773 Effects of Different Statins Combined with Dual Antiplatelet Therapy on Lipid Metabolism Indexes and Cardiac Function in Patients with Acute ST-Segment Elevation Myocardial Infarction after PCI

【Objective】 To compare the effects of different statins combined with dual antiplatelet therapy on lipid metabolism indexes and cardiac function in patients with acute ST-segment elevation myocardial infarction after percutaneous coronary intervention (PCI). 【Methods】 A total of 80 patients with acute ST-segment elevation myocardial infarction who underwent PCI in our hospital from April 2020 to August 2024 were enrolled and divided into the control group (n=37) and the observation group (n=43) by random number table method. The control group received atorvastatin combined with dual antiplatelet therapy, while the observation group received rosuvastatin combined with dual antiplatelet therapy. The two groups were compared in cardiac function indicators [Left Ventricular Ejection Fraction (LVEF), Left Ventricular End-Systolic Diameter (LVESD), Left Ventricular End-Diastolic Diameter (LVEDD)] and laboratory indexes [cardiac troponin I (cTnI), creatine kinase isoenzyme (CK-MB), N-terminal pro-brain natriuretic peptide (NT-proBNP), high-density lipoprotein cholesterol (HDL-C), apolipoprotein A1 (ApoA1), apolipoprotein E (ApoE), triglyceride (TG), apolipoprotein B (ApoB), low-density lipoprotein cholesterol (LDL-C), and platelet aggregation rate (PAR)]. The incidence of major adverse cardiovascular events (MACE) and adverse reactions within half a year after PCI were recorded in both groups. 【Results】 After treatment, LVEF increased, LVESD and LVEDD decreased in both groups; the observation group had higher LVEF and lower LVESD, LVEDD than the control group (P<0.05). After treatment, the levels of cTnI, CK-MB, NT-proBNP and PAR in both groups were lower than those before treatment; cTnI, NT-proBNP and PAR in the observation group were lower than those in the control group (P<0.05). After treatment, HDL-C and ApoA1 increased, while TG, LDL-C, ApoE and ApoB decreased in both groups; the observation group had higher HDL-C, ApoA1 and lower TG, LDL-C, ApoE, ApoB than the control group (P<0.05). There were no significant differences in the incidence of MACE and total adverse reaction rate within half a year after PCI between the two groups (P>0.05). 【Conclusion】 Rosuvastatin combined with dual antiplatelet therapy is superior to atorvastatin combined with dual antiplatelet therapy in repairing damaged myocardium, improving cardiac function and regulating lipid metabolism disorder in patients with acute ST-segment elevation myocardial infarction after PCI.

777 Application Value of Low-Dose Enhanced Pulmonary Artery CTA in the Diagnosis of Pulmonary Embolism in Puerperal Women

【Objective】 To explore the clinical application value of low-dose enhanced pulmonary artery computed tomography angiography (CTA) in the diagnosis of pulmonary embolism among puerperal women. 【Methods】 A total of 82 postpartum women with suspected pulmonary embolism who underwent pulmonary artery CTA examination in our hospital from January 2022 to January 2025 were enrolled. According to the dosage of iodinated contrast agent, they were divided into conventional-dose group and low-dose group, with 41 cases in each group. The two groups were compared in terms of vascular enhancement CT value, excellent and good rate of image quality, radiation dose parameters including volume CT dose index (CTDIvol), dose-length product (DLP) and effective dose (ED), as well as the incidence of adverse reactions related to examination. 【Results】 The CT values of the left and right pulmonary veins in the low-dose group were lower than those in the conventional-dose group, with statistically significant differences . There was no significant difference in the excellent and good rate of image quality between the two groups . The radiation dose indicators of CTDIvol, DLP and ED in the low-dose group were significantly lower than those in the conventional-dose group.The overall incidence of adverse reactions in the low-dose group was markedly lower than that in the conventional-dose group . 【Conclusion】 Low-dose enhanced pulmonary artery CTA can effectively reduce radiation exposure dose and lower the risk of adverse reactions induced by iodinated contrast agents on the premise of ensuring the image quality required for clinical diagnosis. It is more suitable for the special physiological state of puerperal women, and possesses high clinical popularization and application value.

780 Effects of an Evidence-Based Optimized Operating Room Intervention Process on Patients with Acute Myocardial Infarction Undergoing Interventional Therapy

【Objective】 To investigate the effects of an operating room intervention process optimized by evidence-based theory on patients with acute myocardial infarction (AMI) undergoing interventional therapy. 【Methods】 A total of 84 patients with AMI were randomly divided into an observation group and a control group, with 42 patients in each group. The control group received routine interventions, while the observation group received an evidence-based optimized intervention process in addition to routine care. Electrocardiographic parameters [heart rate (HR), QT interval (QTc), and QT dispersion (QTd)], myocardial injury markers [cardiac troponin I (cTnI) and N-terminal pro-brain natriuretic peptide (NT-proBNP)], quality of life [Myocardial Infarction Dimensional Assessment Scale (MIDAS) scores], as well as the incidence of nursing defects and adverse events were compared between the two groups before and after the intervention. 【Results】 After the intervention, HR and QTd in the observation group were significantly lower than those in the control group, while QTc in the observation group was significantly higher than that in the control group (P<0.05). Levels of cTnI and NT-proBNP in the observation group were significantly lower than those in the control group (P<0.05). MIDAS scores in the observation group were significantly lower than those in the control group (P<0.05). In addition, the overall incidence of nursing defects and adverse events in the observation group was significantly lower than that in the control group (P<0.05). 【Conclusion】 Application of an operating room intervention process optimized by evidence-based theory can significantly improve electrocardiographic parameters and reduce myocardial injury in patients with AMI undergoing interventional therapy, enhance quality of life, and decrease the incidence of nursing defects and adverse events.

784 Efficacy of Guipi Decoction Combined with a Cerebral Circulation Therapeutic Device in the Treatment of Mild Cognitive Impairment in the Elderly

【Objective】 To investigate the efficacy of Guipi Decoction combined with a cerebral circulation therapeutic device in the treatment of mild cognitive impairment (MCI) in the elderly. 【Methods】 A total of 40 elderly patients with MCI admitted to our hospitals from November 2024 to July 2025 were randomly divided into a control group (treated with Guipi Decoction) and an observation group (treated with Guipi Decoction combined with a cerebral circulation therapeutic device), with 20 cases in each group. Clinical efficacy between the two groups was compared. Traditional Chinese medicine (TCM) symptom scores, Mini-Mental State Examination (MMSE) scores, Montreal Cognitive Assessment (MoCA) scores, and Activities of Daily Living (ADL) scores were evaluated before and after treatment. 【Results】 The overall effective rate in the observation group (100.00%) which was higher than that in the control group (80.00%) (P<0.05). After treatment, TCM symptom scores in both groups were lower than those before treatment, while MMSE, MoCA, and ADL scores were higher than those before treatment. Furthermore, the scores for all TCM symptoms in the observation group showed even lower than those in the control group (P<0.05), and the MMSE, MoCA, and ADL scores were higher than those the control group (P<0.05). 【Conclusion】 Guipi Decoction combined with a cerebral circulation therapeutic device can significantly improve cognitive function and TCM clinical symptoms in elderly patients with MCI, and enhance their activities of daily living.

787 Analysis of Prognostic Influencing Factors in Patients with Out-of-Hospital Cardiac Arrest

【Objective】 To explore the risk factors of poor prognosis at discharge in patients with out-of-hospital cardiac arrest (OHCA). 【Methods】 A retrospective analysis was conducted on the clinical data of 264 OHCA patients treated by Shanghai Songjiang District Medical Emergency Center from January 2022 to January 2024. According to the survival status at discharge, the patients were divided into survival group (n=182) and death group (n=82). Univariate analysis and multivariate Logistic regression analysis were used to screen the independent risk factors affecting the prognosis of patients at discharge. 【Results】 Univariate analysis showed that there were statistically significant differences between the survival group and the death group in bystander CPR, no-reflow time, CPR duration, epinephrine dosage, lactic acid level, APACHE Ⅱ score and WIC score (P<0.05). Multivariate Logistic regression analysis showed that no bystander CPR, no-reflow time > 5 min, CPR duration > 15 min, epinephrine dosage > 5 mg, high APACHE Ⅱ score, elevated lactic acid level and high WIC score were all independent risk factors for in-hospital death of OHCA patients (P<0.05). 【Conclusion】 The discharge prognosis of OHCA patients is closely related to bystander CPR implementation, no-reflow time, CPR duration, epinephrine dosage, APACHE Ⅱ score, lactic acid level and WIC score. Medical staff can carry out early intervention and optimize cardiopulmonary resuscitation measures according to the specific conditions of patients.

791 Effects of Preserving Partial Denonvilliers' Fascia during Laparoscopic Total Mesorectal Excision on Postoperative Complications and Urinary Function in Male Rectal Cancer Patients

【Objective】 To explore the effects of preserving partial Denonvilliers' fascia during laparoscopic total mesorectal excision (TME) on postoperative complications and urinary function in male patients with rectal cancer. 【Methods】 A total of 100 male patients with middle and low rectal cancer admitted to our hospital from December 2021 to December 2023 were selected and divided into the control group and the observation group by random number table method, with 50 cases in each group. The control group underwent TME without preserving Denonvilliers' fascia, while the observation group underwent TME with partial Denonvilliers' fascia preserved. All patients were followed up for 1 year after operation. The operative indicators, postoperative urinary function, urodynamic parameters, postoperative erectile function, complications and prognosis were compared between the two groups. 【Results】 There were no significant differences in operation time, intraoperative blood loss and postoperative first exhaust time between the two groups (P>0.05). The incidence of postoperative urinary dysfunction in the observation group was 10.00% (5/50), which was lower than 28.00% (14/50) in the control group (χ2=5.263, P<0.05). One month after surgery, the maximum urinary flow rate, maximum urethral pressure and maximum detrusor systolic pressure in the control group were lower than those before operation; the above indicators in the observation group were higher than those in the control group (all P<0.05). There were no significant differences in the above urodynamic indicators between preoperation and 1 month after operation in the observation group (P>0.05). The postoperative incidence of erectile dysfunction (ED) in the observation group was 8.00% (4/50), lower than 28.00% (14/50) in the control group (χ2=6.775, P<0.05). The total incidence of postoperative complications was 10.00% (5/50) in the observation group and 12.00% (6/50) in the control group, with no significant difference (χ2=0.102, P>0.05). There was no significant difference in overall survival rate between the two groups (P=0.714). 【Conclusion】 For male rectal cancer patients undergoing TME, the short-term efficacy and safety are comparable between non-preservation and partial preservation of Denonvilliers' fascia. However, partial preservation of Denonvilliers' fascia can significantly improve patients' urinary and erectile function.

795 Efficacy of Buyang Huanwu Decoction Combined with Plum Blossom Needle Percussion in the Treatment of Delayed Ischemic Neurological Deficits

【Objective】 To investigate the efficacy of Buyang Huanwu Decoction combined with plum blossom needle percussion in the treatment of delayed ischemic neurological deficits (DINDS). 【Methods】 A total of 112 elderly patients with DINDS admitted to our hospital from January 2022 to February 2024 were randomly divided into a control group and an observation group, with 56 cases in each group. Patients in the control group received plum blossom needle percussion therapy, while those in the observation group were treated with Buyang Huanwu Decoction in addition to plum blossom needle percussion therapy of the control group. The treatment course lasted for 4 weeks. Clinical efficacy, neurological function [assessed by the National Institutes of Health Stroke Scale (NIHSS)], hemorheological parameters, cerebral hemodynamic indices, neurotrophic factor levels, traditional Chinese medicine (TCM) syndrome scores, and adverse reactions were compared between the two groups before and after treatment. 【Results】 After treatment, the total effective rate in the observation group was higher than that in the control group (P<0.05). The NIHSS scores, hemorheological parameters, cerebral hemodynamic indices, neurotrophic factor levels, and TCM syndrome scores in the observation group were all significantly better than those in the control group (P<0.05). No serious adverse reactions were observed in either group during the treatment period. 【Conclusion】 Buyang Huanwu Decoction combined with plum blossom needle percussion can improve cerebral hemodynamics and hemorheology in patients with DINDS, promote the recovery of neurological function, and demonstrate good efficacy and safety.

【Objective】 To investigate the efficacy of Xuesaitong Capsules combined with nicergoline in patients with anterior circulation acute ischemic stroke (AIS). 【Methods】 A total of 100 patients with anterior circulation AIS treated in our hospitals from August 2021 to August 2023 were randomly divided into a Xuesaitong group and a combination group, with 50 patients in each group. The Xuesaitong group was treated with Xuesaitong Capsules alone, while the combination group was treated with combination of Xuesaitong Capsules and nicergoline. Clinical efficacy, cognitive function [Mini-Mental State Examination (MMSE) score], neurological function [National Institutes of Health Stroke Scale (NIHSS) score], activities of daily living [Barthel Index (BI) score], and safety were compared between the two groups before and after treatment. 【Results】 The total effective rate in the combination group was higher than that in the Xuesaitong group (P<0.05). After treatment, MMSE and BI scores in the combination group were higher than those in the Xuesaitong group, while NIHSS scores of the combination group were lower than that of the control group (P<0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】 Xuesaitong Capsules combined with nicergoline show significant efficacy in the treatment of anterior circulation AIS, improving cognitive function, neurological function, and activities of daily living, with good safety.

802 Predictive Value of Combined Detection of Follicle-Stimulating Hormone, Estradiol and Progesterone in Female Infertility

【Objective】 To investigate the predictive value of combined detection of follicle-stimulating hormone (FSH), estradiol (E2) and progesterone (P) in female infertility. 【Methods】 A total of 76 infertile patients admitted to our hospital from March 2022 to March 2025 were selected as the observation group, and 76 healthy women who underwent physical examination in our hospital during the same period were selected as the control group. Univariate and multivariate Logistic regression analyses were used to screen influencing factors of female infertility, and receiver operating characteristic (ROC) curve was used to analyze the predictive efficacy of FSH, E2, P alone and in combination for female infertility. 【Results】 The levels of FSH, prolactin, luteinizing hormone and testosterone in the observation group were significantly higher than those in the control group, while the levels of E2 and P were significantly lower than those in the control group (P<0.05). There were no significant differences in age and body mass index (BMI) between the two groups (P>0.05). Multivariate Logistic regression analysis showed that elevated FSH, decreased E2 and decreased P were independent risk factors for female infertility (P<0.05). ROC curve analysis showed that the area under the curve (AUC) of combined detection of FSH, E2 and P in predicting female infertility was significantly larger than that of single detection of each index (P<0.05). Conclusion Abnormal levels of FSH, E2 and P are closely related to female infertility. 【Conclusion】 Combined detection of the three indicators can improve the predictive efficacy for female infertility and provide a reference for clinical prediction of female infertility.

805 Application Effect of Bedside Bicycle Exercise Combined with Outpatient Continuous Nursing Intervention in Parturients Undergoing Cesarean Section

【Objective】 To explore the application effect of bedside bicycle exercise combined with outpatient continuous nursing intervention in parturients after cesarean section. 【Methods】 A total of 120 parturients who underwent cesarean section in our hospital from January 2024 to October 2024 were selected and divided into control group (routine nursing intervention) and observation group (bedside bicycle exercise combined with outpatient continuous nursing intervention) by lottery method, with 60 cases in each group. The postoperative recovery, feeding patterns, psychological status [Edinburgh Postnatal Depression Scale (EPDS) score, General Well-Being Schedule (GWB) score], quality of life [Nottingham Health Profile (NHP) score] and complication incidence were compared between the two groups. 【Results】 The recovery time of bowel sounds, first exhaust time, lochia duration and hospital stay in the observation group were significantly shorter than those in the control group (P<0.05). After intervention, the breastfeeding rate of the observation group was higher than that of the control group, while the mixed feeding rate was lower (P<0.05). The GWB score of the observation group was higher and the EPDS score was lower than those of the control group (P<0.05). Each dimension score of NHP in the observation group was higher than that in the control group (P<0.05). The total incidence of complications in the observation group was lower than that in the control group (P<0.05). 【Conclusion】 Bedside bicycle exercise combined with outpatient continuous nursing intervention can promote postoperative recovery of cesarean section parturients, increase the breastfeeding rate, improve maternal psychological state and quality of life, and reduce the incidence of complications.
