Relationship between Serum 25-(OH)D, VDBP, Inflammatory Factors and Bronchopulmonary Dysplasia in Premature Infants

【Objective】
To study the effects of serum 25-(OH)D, vitamin D binding protein and cytokine levels on the development of bronchopulmonary dysplasia in preterm infants.
【Methods】
A prospective analysis of 161 preterm infants delivered in our hospital from January 2016 to may 2019 was conducted. The patients were divided into observation group (BPD children) and control group (non BPD children) according to whether BPD occurred on the 28th day after birth. According to oxygen inhalation, BPD children were divided into mild BPD group and moderate severe BPD group. The levels of serum 25-(OH)D, TNF-α, IL-10 and VDBP in different groups were compared. The risk factors of BPD in preterm infants were analyzed by multivariate logistic regression.
【Results】
The scores of Apgar/1min and Apgar/5min in in the observation group were significantly lower than those in the control group. The proportion of patent ductus arteriosus, delayed neonatal sepsis and neonatal pneumonia in the observation group was higher than that in the control group. The duration of continuous positive airway pressure, total oxygen inhalation time and hospitalization time in the observation group were longer than those in the control group (P<0.05). At birth, the levels of serum 25-(OH)D, VDBP and IL-10 in moderate and severe BPD group were lower than those in the mild BPD group (P<0.05). After 4 weeks, there was no significant difference in the levels of 25-(OH)D, VDBP and IL-10 between the two groups (P>0.05). Delayed neonatal sepsis, neonatal pneumonia and low levels of 25-(OH)D, VDBP and IL-10 may be associated with BPD in preterm infants.
【Conclusion】
Delayed neonatal sepsis and neonatal pneumonia are the risk factors of BPD in preterm infants. The serum levels of 25-(OH)D, VDBP and IL-10 in children with BPD are decreased, which may be useful indicators for early prediction of BPD and assessment of its severity.

Study on the Outcome of Monochorionic Diamniotic Twins with Selective Intrauterine Growth Restriction

【Objective】
To investigate the prognosis of selective intrauterine growth restriction (sIUGR) in MCDA twin pregnancy.
【Methods】
A total of 85 cases of MCDA pregnancy with sIUGR diagnosed and treated in our hospital from January 2018 to December 2019 were selected as the observation group, and 79 cases of normal pregnancy in the same period were selected as the control group. The prognosis of the normal group and the sIUGR group was compared.
【Results】
The gestational weeks of delivery in the sIUGR group were earlier than those in the normal group, the difference in birth weight was greater than that in the normal group, and the incidence of intrauterine death (IUFD) was higher than that in the normal group (P<0.01); the gestational weeks of delivery in the type Ⅱ sIUGR group were earlier than those in type Ⅰ and type Ⅲ, the difference of birth weight was greater than that of type Ⅰ and type Ⅲ, and the incidence of IUFD was higher than that of type Ⅰ and Ⅲ (P<0.01).The incidence of abnormal umbilical cord in the sIUGR group was higher than that in the normal group.
【Conclusion】
The prognosis of MCDA twins complicated with sIUGR is poor. The incidence of preterm birth, poor birth weight and intrauterine mortality rate of small fetus are higher in type Ⅱ sIUGR twins. Ultrasonography can accurately classify sIUGR and has important value for clinical prognosis evaluation.

Prenatal Ultrasound Characteristics of Fetal Hypospadias and Its Clinical Value

【Objective】
To retrospectively analyze the features of prenatal ultrasonography of fetal hypospadias and its clinical significance.
【Methods】
From January 2019 to December 2019, the ultrasound data of 41 cases of fetal hypospadias and 1 case of missed diagnosis were analyzed retrospectively. The follow-up results of live births after delivery or induced labor were compared with those of prenatal ultrasound examination and the ultrasound image characteristics of fetal hypospadias were analyzed.
【Results】
Prenatal ultrasound showed 41 cases of fetal hypospadias, 34 of which were confirmed after live birth or induced labor. Among the 34 cases of hypospadias accurately diagnosed by prenatal ultrasound, 19 cases were simple hypospadias and 15 cases were combined with other abnormal hypospadias. One missed case showed only a round and blunt penis head on prenatal ultrasound without clear diagnosis before delivery. However, it was clinically confirmed as coronary groove hypospadias after live birth and delivery; 7 misdiagnosed cases included 2 normal male external genitalia, 2 short penises, 1 case of congenital prepuce, 1 case of webbed penis, and congenital adrenal cortex 1 case of hyperplasia. The prenatal ultrasound diagnosis rate was 80.95% (34/42), the misdiagnosis rate was 16.67% (7/42), and the missed diagnosis rate was 2.38% (1/42). Among the 34 confirmed cases of hypospadias, there were 17 cases of penile type and 17 cases of proximal type. Ultrasound image features of penis type were: fetal penis was short; the degree of curvature was lighter than that of proximal hypospadias; the echo of the penis was slightly enhanced (or no obvious change); the end of the penis was slightly flat; the scrotum was not separated, and there was no "tulip sign". While the ultrasound image characteristics of proximal type showed: the penis was short and small in varying degrees, or the penis was more severely curved to the ventral side; the end of the penis was round and blunt, pointing to the anus; the skin on the dorsal side of the head of the penis was thickened and looks like a turban; the scrotum on both sides was completely or partially separated, and the penis was in the middle showing "tulip sign"; the penis scrotum was completely or partially transposed.
【Conclusion】
Prenatal ultrasound examination has important diagnostic value for fetal hypospadias.

Therapeutic Effect of Irinotecan Combined with Bevacizumab on Platinum-Resistant Recurrent Ovarian Cancer

【Objective】
To investigate the efficacy of Irinotecan combined with bevacizumab in the treatment of platinum-resistant recurrent ovarian cancer.
【Methods】
A total of 118 patients with platinum-resistant recurrent ovarian cancer treated in our hospital from Sep. 2016 to Sep. 2018 were randomly divided into the control group and the observation group with 59 cases in each group. The control group was treated with Irinotecan, while the observation group was treated with Irinotecan combined with bevacizumab. The short-term efficacy, serum tumor markers such as human epididymal secretory protein 4 (HE4), carbohydrate antigen 125 (CA125) and carcinoembryonic antigen (CEA), long-term efficacy and adverse reactions of the two groups were compared.
【Results】
After treatment, the objective remission rate (ORR) in the observation group was significantly higher than that in the control group (57.63% vs 37.29%, P<0.05). The levels of HE4,CA125, and CEA in both groups were lower than those before treatment, and those in the observation group were significantly lower than those in the control group (P<0.05). The total survival time (OS) and progression-free survival (PFS) of patients in the observation group were significantly longer than those in the control group (P<0.05). The incidence of hemoglobin decrease, renal function damage, alanine aminotransferase increase, central granulocyte decrease, thrombocytopenia, and peripheral neuritis in the two groups were compared (P>0.05). The incidence of grade 0 leukopenia in the observation group was significantly higher than that in the control group (P<0.05). The incidence of grade Ⅲ-Ⅳ diarrhea and nausea and vomiting in the observation group was significantly lower than that in the control group (P<0.05).
【Conclusion】
Irinotecan combined with bevacizumab is effective in the treatment of platinum-resistant recurrent ovarian cancer. It can also improve PFS and OS in patients with platinum-resistant ovarian cancer, and the adverse reactions are tolerable. It is worthy of clinical application.

Impact of Motivational Interviews Based on Trans-theoretical Model on Disease Cognition and Self-efficacy of Asthma Patients

【Objective】
To explore the impact of motivational interviews based on trans-theoretical model on disease cognition and self-efficacy of asthma patients.
【Methods】
A total of 194 patients with asthma in our hospital from August 2017 to March 2018 were selected and randomly divided into observation group and control group, with 97 cases in each group. The control group was given routine nursing and health education intervention, and the observation group was given motivational interview health education based on cross theoretical model. The cognitive level of disease, self-efficacy and quality of life were compared between the two groups.
【Results】
Five patients in each group dropped out of the study, and 92 patients completed the study. After the intervention, the cognitive level score and high-level cognitive rate of the observation group were higher than those before the intervention of the same group, the cognitive level of the control group was higher than that of the same group before the intervention, and the cognitive level score and high-level cognitive rate of the observation group were higher than those of the control group, the difference was statistically significant (P<0.05). The scores of self-management, general self-efficacy, achievement of results and coping problems in the two groups were higher than those before intervention in the same group, and those in the observation group were higher than those in the control group (P<0.05). The scores of activity limitation of the two groups were significantly higher than those of the same group before intervention. The activity limitation, asthma symptoms, psychological status and care for their own health of the observation group were significantly higher than those of the control group, and the difference was statistically significant (P<0.05).
【Conclusion】
Motivational interviewing based on trans-theoretical model can improve the cognitive level of asthma patients, promote the development of self-efficacy and improve the quality of life of patients.

Efficacy of Endoscopic Balloon Dilatation Combined with Glucocorticoid in the Treatment of Esophageal Cancer Stenosis after Radiotherapy

【Objective】
To investigate the effect of balloon dilatation combined with hormone in the treatment of esophageal cancer after radiotherapy.
【Methods】
A total of 68 patients with esophageal cancer stenosis after radiotherapy were randomly divided into two groups, 34 cases in each group. The control group was treated with endoscopic balloon dilatation, while the observation group was treated with glucocorticoid. The curative effect, complications and dysphagia before and after treatment were compared between the two groups. Albumin (ALB), transferrin (TF), hemoglobin (Hb), lymphocyte count (TCL), body weight, nutritional status and immune function were compared between the two groups.
【Results】
After intervention, the stenosis in the observation group was improved, and the total effective rate was 100%, which had no significant difference compared with 94.12% of the control group (P>0.05). The cure rate of the observation group was 79.41%, which was significantly higher than 55.88% of the control group (P<0.05). There was no significant difference in the incidence of complications between the two groups (P>0.05). At the time of reexamination, the proportion of grade 0, grade 1-2 in the two groups was significantly higher than that before treatment (P<0.05). The proportion of grade 0 in the observation group was significantly higher than that in the control group (P<0.05). Observation of the Curative Effect of Chinese Medicine Combined with Fimotong in the Treatment of Patients with Premature Ovarian Failure

【Objective】
To study the therapeutic effect of Chinese medicine (Ovarian Zaoshuai Recipe) combined with Fimotong on follicular growth hormone (FSH), serum inhibin B (INHB) and estrogen (E

【Methods】
A total of 102 patients with premature ovarian failure treated in our hospital from August 2015 to August 2018 were divided into two groups according to different treatment methods. The control group was treated with Fimotong, while the observation group was treated with ovariectomy recipe on the basis of Finmotong. The clinical efficacy of the two groups after treatment was analyzed.
【Results】
The total effective rate was 92.16% (47/51) in the observation group and 76.47% (39/51) in the control group. The total effective rate in the observation group was higher than that in the control group (P<0.05). There were no significant differences in FSH, INHB and E
2
level between the two groups before treatment (P>0.05). However, 3 and 6 months after treatment, serum INHB and E
2
levels of the observation group were significantly higher than those of the control group, and the FSH level in the observation was significantly lower than that in the control group. The differences were statistically significant (P<0.05). There was no significant difference in Kupperman score between the two groups before treatment (P>0.05). While the Kupperman score in the observation group was lower than that in the control group at 3 months and 6 months after treatment. The recurrence rate was 9.80% in the observation group and 33.33% (17/51) in the control group. The recurrence rate in the observation group was lower than that in the control group (P<0.05).
【Conclusion】
The treatment of premature ovarian failure with Chinese medicine combined with Western medicine can improve the expression of FSH, INHB, and E

, reduce the course of disease, improve the curative effect, and result in a low recurrence rate.

Predictive Effect of Serum Vascular Endothelial Cadherin on the Severity and Survival of Septic Shock

【Objective】
To explore the predictive effect of serum vascular endothelial cadherin (VE-Cad) on the assessment of the severity of septic shock and the survival status.
【Methods】
A total of 78 patients with septic shock who were treated in our hospital were selected as subjects. Serum VE-Cad levels were measured in all patients.According to the value of VE-Cad, patients were divided into three groups: VE-cad level <1ng/ml group(A group, n=29),VE-cad level was 1~ 5 ng/ml group(B group, n=27),VE-cad level >5 ng/ml group(C group, n=22).Vascular endothelial growth factor (VEGF), tumor necrosis factor (TNF-α) and interleukin-6 (IL-6) levels, APACHE Ⅱ(acute physiology and chronic health) score and SOFA (sequential organ failure estimates) score, and survival rate were compared among patients within three groups.
【Results】
The levels of VEGF, TNF-α and IL-6 in the three groups of patients from low to high order was group A < group B < group C, and the differences between the groups were significant (P<0.05). The order from low to high scores of APACHEⅡ and SOFA in the three groups scores was group A < group B < group C; and the differences between each group were significant (P<0.05). The survival rate of patients in group A is 72.41% (21/29), which is higher than 55.56% (15/27) in group B and 40.91% (9/22) in group C; and the differences between the three groups were significant (P<0.05).
【Conclusion】
Serum VE-Cad is closely related to the severity of the disease in patients with septic shock. The higher the serum VE-Cad, the more severe the inflammatory response and illness, and the lower the survival rate.

Application of Perineal Non-invasive Delivery Technology Combined with Free Position Delivery in Non-invasive Delivery of Primipara

【Objective】
To investigate the effect of perineal non-invasive delivery technology combined with free position delivery in non-invasive delivery of primipara and its effect on maternal perineal incision rate.
【Methods】
A total of 124 women with non-invasive maternal delivery from March to 20, 2015 were selected. The random number table was divided into the control group (n=62) and the observation group (n=62). The control group was given conventional methods for delivery. The observation group was treated with perineal non-invasive delivery technique combined with free position delivery Labor, postpartum hemorrhage, neonatal Apgar score, perineal injury rate and maternal pain during labor were compared between the two groups.
【Results】
There was no significant difference in the 5 min Apgar score between the observation group and the control group (P>0.05). The total labor and postpartum hemorrhage in the observation group were shorter (less) than in the control group (P<0.05). The 1 min Apgar score was higher than that of the control group (P<0.05). The incidence of perineal incision, perineal tear and perineal edema after delivery was lower than that of the control group (P<0.05). After childbirth, the pain rate of grade 0 and grade Ⅰ in the observed group was higher than those of the control group (P<0.05). While the pain rate of grade Ⅱ and Ⅲ in the observation group was lower than that of the control group (P<0.05).
【Conclusion】
Invasive non-invasive delivery technology combined with free position delivery for primipara non-invasive delivery can shorten the labor process, reduce postpartum hemorrhage, improve neonatal Apgar score, and reduce maternal perineal incision rate. It is worthy of popularization and application.

Effects of Dexmedetomidine Combined with Spinal Epidural Anesthesia on Inflammatory Stress Response and Respiratory Function in Patients Undergoing TURP

Expression and Significance of TSH, TPOAb and TGAb in Serum of Patients with Graves' Disease

【Objective】
To investigate the expression level and significance of thyrotropin (TSH), thyroid peroxidase antibody (TPOAb) and thyroglobulin antibody (TGAb) in serum of patients with diffuse goiter with hyperthyroidism Graves Disease.
【Methods】
A total of 130 Graves patients (observation group) treated in our hospital from January 2017 to June 2018 were selected, and 100 healthy volunteers were selected as control group, serum TSH, TPOAb and TGAb levels were measured.
【Results】
The level of TSH in the observation group was significantly lower than that in the control group, while the level of TPOAb and TGAb in the observation group was significantly higher than that in the control group, and the differences were statistically significant (P<0.05); the level of TSH, TPOAb and TGAb in the observation group with and without exophthalmos had no statistically significant difference (P>0.05). In the observation group, there were 33 patients with hypothyroidism 6 months after treatment; the levels of TPOAb and TGAb in patients with early-onset hypothyroidism before treatment were significantly higher than those in patients without hypothyroidism (P<0.05).
【Conclusion】
The serum levels of TSH, TPOAb and TGAb were significantly abnormal in patients with Graves' disease. The levels of TPOAb and TGAb before treatment may be related to the occurrence of hypothyroidism after treatment, but the levels of TSH, TPOAb and TGAb have no significant relationship with the presence of exophthalmos.

Analysis of Related Factors Affecting the Clinical Prognosis of Children with Tourette's Syndrome

【Objective】
To explore the related factors affecting the clinical prognosis of children with multiple tourette syndrome.
【Methods】
From December 2017 to August 2018, 75 children diagnosed with multiple tourette syndrome in the Department of Children Health of our hospital were selected. The patients were followed up for 1 year, a unified questionnaire survey scale was adopted, and univariate and multivariate Logistic regression analysis was used to analyze the related factors affecting the prognosis of the patients.
【Results】
There were statistically significant differences between the improved and uncured children in terms of family history of mental illness, number of repeated illness, severity score, and number of repeated respiratory tract infections (
P
<0.05). Logistic regression analysis showed that the number of recurrent cases (OR=1.072,95%CI:1.030~1.116), severity score (OR=1.100,95%CI: 1.025~1.181), recurrent respiratory tract infection (OR=1.279,95%CI:1.153~1.419), and family history of mental illness (OR=0.182,95%CI: 0.034~0.964) were independent risk factors affecting the prognosis of children with TIC (P<0.05).
【Conclusion】
There are many factors restricting the prognosis of Tourette's disease in children, such as the number of relapses, the severity of the disease, recurrent respiratory tract infection, etc.; fully mastering the prognostic factors of Tourette's disease and taking relevant preventive measures can effectively control the recurrence of the disease.

Perfusion Parameter Characteristics and Diagnostic Value of Contrast-enhanced Ultrasound in Patients with Papillary Thyroid Carcinoma

【Objective】
To investigate the value of contrast-enhanced ultrasound (CEUS) in the diagnosis of papillary thyroid carcinoma (PTC).
【Methods】
Ninety patients with PTC confirmed by pathology (the PTC group) and 90 patients with benign thyroid nodules (the control group) were enrolled in our hospital. CEUS was used to compare the peak intensity of contrast-enhanced curve-intensity curve between the two groups(PEAK), peak time (TTP), local blood volume (RBV), local blood flow (RBF), maximum perfusion intensity (SI

), mean perfusion intensity (SI

), and contrast-enhanced ultrasound mode results.
【Results】
The values of PEAK, RBF, SI
max
and SI
mean
in the PTC group were lower than those in the control group (P<0.05). TTP and RBV values in the PTC group were not significantly different from those in the control group (P>0.05). There was no significant difference in the uniform enhancement, enhancement intensity, and early regression between the PTC group and the control group (P>0.05). If the gold standard for diagnosis was determined by pathological results, then the sensitivity of CEUS in differential diagnosis of PTC and benign thyroid nodules was 94.44% and the specificity was 86.67%.
【Conclusion】

A Comparative Study on the Clinical Effects of Circumcision with Two Different Kinds of Disposable Circumcision Suture Devices

【Objective】
To compare the clinical effects of improved disposable circumcision suture device and disposable circumcision suture device in circumcision.
【Methods】
A total of 108 patients with long prepuce or phimosis from March to August 2018 were randomly divided into two groups. Patients in the observation group (n=56) received improved-disposable circumcision suture device, while patients in the control group (n=52) received disposable circumcision suture device. Operation time, intraoperative bleeding, number of cases of transfer surgery, postoperative pain, postoperative edema, postoperative nailing time, postoperative second operation, and postoperative satisfaction were compared between the two groups with different kinds of disposable circumcision suture devices.
【Results】
There were no significant differences (P>0.05) in operation time (P=0.057), intraoperative transition to open (P=0.558), second operation (P=0.481), postoperative pain (P=0.27), or postoperative satisfaction (P=0.856). There were statistically significant differences (P<0.05) in intraoperative bleeding (P<0.001), postoperative edema (P<0.001), and postoperative nailing time. Intraoperative bleeding of the improved disposable circumcision suture device with silicone ring was better than that of the disposable circumcision suture device; the possibility of changing operation was less than the control group. At the same time, the postoperative edema was more obvious than the control group. The postoperative nailing time in the observation group was lower than that in the control group (P<0.05). There was no statistically significant difference of postoperative satisfaction between the two groups (P>0.05).
【Conclusion】
Both kinds of disposable circumcision suture devices have their own advantages and disadvantages. Clinicians should choose suitable suture device according to their own experience and patient's condition.

Clinical Efficacy of Microwave Ablation Combined with Chemotherapy in Treatment of Non-small Cell Lung Cancer Patients with EGFR-TKIs Resistance

【Objective】
To investigate the efficacy of CT-guided percutaneous microwave ablation combined with chemotherapy in the treatment of EGFR-TKIs-resistant non-small cell lung cancer (NSCLC).
【Methods】
A total of 89 patients with NSCLC who were admitted to our hospital were recruited in the study. According to different treatment methods after NSCLC became EGFR-TKI drug resistance, patients were divided into two groups: the microwave ablation group (n=48) and the chemotherapy group (n=41). The efficacy and safety of the two groups of patients were compared. Factors associated with progression-free survival (PFS) and overall survival (OS) of NSCLC patients were analyzed.
【Results】
The objective response rate of treatment in the microwave ablation group was 93.75% (45/48), which was higher than that in the chemotherapy group [78.05% (32/41)]. The difference was statistically significant (P<0.05). Both groups of patients had different degrees of adverse reactions, and the symptoms were improved after symptomatic treatment. The average PFS of the microwave ablation group was 16.94 months, which was higher than that of the chemotherapy group 14.42 months (P<0.05). The average OS of patients in the microwave ablation group was 20.19 months, which was higher than the average OS of the chemotherapy group 19.66 months, (P<0.05).The results of Cox univariate and multivariate analysis showed that PFS and OS were poor in NSCLC patients with stage Ⅲ-Ⅳ stage, lymph node metastasis and tumor diameter >3 cm.
【Conclusion】
For patients with EGFR-TKIs-resistant NSCLC, microwave ablation combined with chemotherapy can effectively control the tumor progress and improve the prognosis of patients.

Comparison of Effect and Safety between Modified Prone with Leg Separation and Traditional Prone Position after Lithotomy in Treatment of Complex Kidney Stones

【Objective】
To compare the effect and safety of modified prone posture with leg separation and traditional prone posture after lithotomy in the treatment of complex kidney stones.
【Methods】
A total of 120 patients with complex kidney stones treated in our hospital were randomly divided into the observation group (n=60) and the control group (n=60). Patients in the observation group were in the modified prone with leg separation to receive percutaneous nephrolithotomy(PCNL); while patients in the group were in the conventional prone after lithotomy for PCNL. The surgical efficacy and complications were compared between the two groups.
【Results】
The operation time and channel establishment time of the observation group were shorter than those of the control group; and the intraoperative blood loss in the observation group was lower than that in the control group (P<0.05). In the observation group, the hospitalization time and the first exhaust time were shorter, the reoperation rate was lower, and the rate of stone removal was higher than in the control group (P<0.05). The VAS score of the observation group was lower than that of the control group after surgery (P<0.05). There was no significant difference in the incidence of complications between the two groups (P>0.05). The quality of life score in the observation group at 4 weeks after operation was higher than that in the control group; and the difference was statistically significant (P<0.05).
【Conclusion】
For patients with complicated renal calculi, the use of modified prone posture with leg separation during percutaneous nephrolithotripsy can shorten the operation time, increase the rate of stone removal, promote postoperative recovery, and improve the quality of life without increasing complications.

Effect of Laparoscopic Assisted AlPPS in Primary Liver Cancer and its Influence on Serum miR-145, miR-143 and Immune indexes

【Objective】
To investigate the effect of laparoscopic assisted ALPPS(associating liver partition and portal vein ligation for staged hepatectomy)one-stage surgery in the treatment of primary liver cancer (PLC) and its influence on the levels of microrna-145 (miR-145), miR-143 and immune indexes.
【Methods】
A total of 63 PLC patients from April 2017 to may 2020 in our hospital were selected and divided into observation group (laparoscopic assisted AlPPS,n=32) and control group (open AlPPS,n=31). The operation and complications of the two groups were compared. The serum humoral immune function indexes (immunoglobulin M (IgM), immunoglobulin A (IGA), immunoglobulin G (IgG)) and liver function indexes (alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBiL) were compared between the two groups before and 3 days after operation. The levels of miR-145 and miR-143 were compared between the two groups before operation, 3 days and 1 week after operation.
【Results】
The postoperative exhaust time, hospital stay and intraoperative blood loss of the observation group were shorter than those of the control group, and the difference was statistically significant (P<0.05). The incidence of postoperative complications in the observation group was 12.50% (4/32), which was lower than 29.03 (9/31) in the control group, but the difference was not statistically significant (χ

The levels of serum IgM, IgA and IgG in the two groups were lower than those before operation, but the observation group was higher than the control group, the difference was statistically significant (P<0.05); the serum ALT, AST, TBiL levels were higher than those before operation, but the observation group was lower than the control group, the difference was statistically significant (P<0.05). The serum levels of miR-145 and miR-143 in the observation group were lower than those in the control group 3 days and 1 week after operation, and the difference was statistically significant (P<0.05).
【Conclusion】
Laparoscopic assisted AlPPS one-stage surgery for PLC patients has the advantages of small trauma, less bleeding and rapid postoperative recovery. It can protect liver function, reduce immune function damage, and reduce the expression of miR-145 and miR-143 in serum.

Application of An Internet Platform in Parental Health Education for Children with Influenza

【Objective】
To explore the effect of health education based on an internet platform on parental disease cognition and health behavior of children with influenza.
【Methods】
A total of 116 parents of children with influenza were enrolled in the study from July 2016 to June 2017. They were randomly divided into the treatment group and the control group with 58 each by prospective study. The control group was given routine health education and nursing interventions. The treatment group was given routine health education and nursing interventions in combination with web-based health education. The parental cognitive status, health behaviors, clinical symptoms and secondary infections of the two groups were compared.
【Results】
The scores of flu-related knowledge, transmission routes, and preventive measures of the treatment group were significantly higher than those of the control group (P<0.05). Health behaviors such as not spitting, washing hands frequently, coughing and sneezing, covering the mouth and nose, suffering from flu home isolation, and having the willingness to receive influenza vaccine during the high season of flu were significantly higher than those of the control group (P<0.05). The time of fever, the disappearance time of dizziness symptoms, and the disappearance time of nasal symptoms were shorter than that in the control group. The secondary infection rate of 25.86% in the treatment group was lower than of 53.45% in the control group (P<0.05).
【Conclusion】
Health education based on an internet platform can help raise the parental cognitive level of children with influenza, promote the development of healthy behaviors, and prevent the incidence of secondary infections in children.

Relationship between Pulmonary Infection and PCT, sTREM-1 and hs-CRP Levels in Patients with NSCLC

【Objective】
To study the relationship between pulmonary infection and levels of procalcitonin (PCT), myeloid trigger receptor (sTREM-1) and hypersensitive C-reactive protein (hs-CRP) in patients with non-small cell lung cancer (NSCLC).
【Methods】
A total of 37 cases of NSCLC patients who received surgical treatment in our hospital and developed postoperative pulmonary infection were selected as the study subjects, and were set as the pulmonary infection group. In addition, 37 cases of NSCLC patients who did not show postoperative pulmonary infection were set as the non-pulmonary infection group.Serum PCT, sTREM-1 and hs-CRP were detected in both groups after blood samples were collected 24h after surgery. ROC curve was used to analyze the value of serum PCT, sTREM-1 and hs-CRP in predicting postoperative pulmonary infection in NSCLC patients. Logistic regression was used to analyze whether PCT, sTREM-1 and hs-CRP were risk factors for postoperative pulmonary infection in NSCLC patients.
【Results】
The levels of PCT, sTREM-1 and hs-CRP in the lung infection group were significantly higher than those in the non pulmonary infection group (P<0.05). The lower curve areas of PCT, sTREM-1 and hs-CRP were 0.829, 0.738 and 0.955 respectively by ROC analysis. Logistic regression analysis showed that PCT>4.715, sTREM-1 > 4.410, hs-CRP>8.245 were risk factors of postoperative pulmonary infection in NSCLC patients.
【Conclusion】
The high expression of PCT, sTREM-1 and hs-CRP in NSCLC patients with postoperative pulmonary infection is a risk factor of postoperative pulmonary infection.

Effect of Buyang Huanwu Decoction Combined with Butylphthalide on Neurological Function and Prognosis of Patients with Ischemic Stroke

【Objective】
To investigate the effects of Buyang Huanwu Decoction combined with butylphthalide on neurological function and prognosis in patients with ischemic stroke.
【Methods】
A total of 110 patients with ischemic stroke in our hospital from March 2017 to February 2019 were selected and randomly divided into observation group and control group, 55 cases in each group. The control group was treated with butylphthalide, and the observation group was treated with Buyang Huanwu Decoction on the basis of the control group. The clinical efficacy, neurological function, prognosis and adverse reactions of the two groups were observed.
【Results】
The total effective rate of the observation group was 92.73% (51/55), which was significantly higher than 74.55% (41/55) of the control group (χ
2
=6.6425, P<0.05). After treatment, NIHSS and MRS scores of the two groups were significantly decreased, and the observation group was significantly lower than the control group (P<0.05); the Scandinavian Stroke Scale (SSS) scores of the two groups were significantly increased, and the observation group was higher than the control group (P<0.05). The incidence of adverse reactions in the observation group was 3.64% (2/55), while that in the control group was 14.54% (8/55), the difference was statistically significant (P<0.05).
【Conclusion】
Buyang Huanwu Decoction Combined with butylphthalide in the treatment of ischemic stroke can significantly improve the neurological function of patients with good prognosis, less adverse reactions, safe and effective, which is worthy of clinical application.

Analysis of 5-year Survival Rate and Prognosis of Breast Cancer with Ipsilateral Supraclavicular Lymph Node Metastasis

【Objective】
To investigate the 5-year survival rate and prognostic factors of breast cancer patients with ipsilateral supraclavicular lymph node metastasis.
【Methods】
A total of 110 locally advanced triple negative invasive breast cancer (TNBC) with ipsilateral supraclavicular lymph node metastasis but no distant metastasis admitted to our hospital were enrolled in the study. The patients all received 4-6 cycles of neoadjuvant chemotherapy, followed by surgery and radiotherapy. Patients' 5-year survival rates were recorded and factors affecting disease-free survival (DFS) and overall survival (OS) were analyzed.
【Results】
The follow-up time was 54-72 months. The median follow-up time was 65.2 months. A total of 49 deaths occurred. The 5-year survival rate was 55.45% (61/110) with median survival time 31.9 months. The 5-year survival rates of patients with negative supraclavicular lymph node metastasis and pathological complete remission (pCR) after surgery was 73.68% (28/38) and 81.82% (18/22), which were higher than those of supraclavicular lymph node-positive metastasis 52.78% (38/72) and non-pCR patients 54.55% (48/88), (P<0.05). Kaplan-Meier method and Log-rank test showed that the size of supraclavicular lymph nodes before chemotherapy, Ki-67 level in pre-chemotherapy breast tissue, supraclavicular lymph node status after chemotherapy, axillary lymph node status, subclavian lymph node status, pCR and ipsilateral supraclavicular lymph node status after chemotherapy were correlated with DFS (P<0.05); and age, Ki-67 staining in pre-chemotherapy breast cancer, supraclavicular lymph node status, axillary lymph node status, and pCR were associated with OS as well (P<0.05). Cox multivariate analysis showed that the metastatic status of supraclavicular lymph node, axillary lymph node and subclavian lymph node, and pCR were independent influencing factors of DFS (P<0.05) after chemotherapy. Age, supraclavicular lymph node status after chemotherapy and pCR were independent factors of OS (P<0.05).
【Conclusion】
The 5-year survival rate of triple-negative breast cancer with ipsilateral supraclavicular lymph node metastasis is improved after comprehensive treatment. The status of supraclavicular lymph node and pCR after chemotherapy are important factors influencing prognosis.

Effect of THR through Anterior Minimally Invasive Approach on the Levels of HB, CRP, IL-6 and the Range of Hip Joint Motion Flexion and Extension in Patients with Femoral Neck Fracture

【Objective】
To investigate the effect of total hip arthroplasty (THR) through anterior minimally invasive approach on the levels of hemoglobin (HB), C-reactive protein (CRP), interleukin-6 (IL-6) and the range of motion of hip joint flexion and extension in patients with femoral neck fracture.
【Methods】
The clinical data of TRH patients (observation group,n=42) and traditional posterior approach thr patients (control group,n=42) were retrospectively analyzed from January 2017 to December 2019 in our hospital. The perioperative indexes (operation time, incision length, intraoperative blood loss, postoperative drainage, length of hospital stay), preoperative and postoperative blood indexes (Hb, Hct), inflammatory indexes (CRP, IL-6), hip flexion and extension range of motion and hip function score (Harris score) were compared between the two groups.
【Results】
There were statistically significant differences in the perioperative indexes (except operation time) between the two groups (P<0.05); after 24 h, the levels of Hb and Hct in the two groups were significantly lower than those before operation (P<0.05), but the observation group was significantly higher than the control group (P<0.05); 24 h, 7 d after operation, the levels of Hb and Hct in the two groups were significantly lower than those before operation (P<0.05). The levels of CRP and IL-6 in the two groups were significantly higher than those before operation (P<0.05), but the observation group was significantly lower than the control group (P<0.05); after 3, 6, 12 months, the hip joint flexion and extension activity and Harris score of the two groups were significantly increased (P<0.05), the above indexes of the observation group were significantly higher than those of the control group at 3 months after operation (P<0.05), and the hip of the observation group at 6 and 12 months after operation was significantly higher than that of the control group (P<0.05) The range of motion of joint flexion and extension was significantly higher than that of the control group (P<0.05), but there was no significant difference in Harris score between the two groups (P>0.05).
【Conclusion】
Anterior minimally invasive THR is more conducive to postoperative rehabilitation, shorten hospital stay, reduce inflammatory reaction, increase the range of motion of hip joint flexion and extension, shorten the recovery time of hip joint function, which is worthy of clinical application.

Correlation between Prognosis and Depth of Invasion and Postoperative Chemotherapy in Primary Bladder Adenocarcinoma

【Objective】
To investigate the effect of tumor invasion depth and postoperative adjuvant chemotherapy on the prognosis of patients with primary bladder adenocarcinoma.
【Methods】
A total of 78 patients with primary bladder adenocarcinoma treated in our hospital were divided into survival group (49 cases) and death group (29 cases) according to the 3-year follow-up results. The age, gender, operation mode, pathological stage, pathological differentiation degree, tumor number, maximum diameter of lesion, depth of invasion, lymph node metastasis and postoperative adjuvant chemotherapy were compared between the two groups. Cox proportional hazard regression model was used to analyze the related factors affecting the prognosis of patients.
【Results】
There was no significant difference in age, gender, operation mode, tumor number and maximum diameter between the two groups (P>0.05); there were significant differences in pathological stage, tumor differentiation degree, depth of invasion, postoperative adjuvant chemotherapy between survival group and death group (P<0.05); Cox proportional hazard model analysis showed that: the increased pathological stage of primary bladder adenocarcinoma, the decreased tumor differentiation and myometrial invasion were independent risk factors for poor prognosis (P<0.05). Postoperative adjuvant chemotherapy had no significant effect on the prognosis of patients (P>0.05).
【Conclusion】
There are many factors that affect the prognosis of patients with primary bladder adenocarcinoma. Myometrial invasion is one of the independent risk factors. However, the effect of postoperative adjuvant chemotherapy on prognosis is not significant, which may need to be further extended for further study.

Expression and Significance of Serum Silent Information Regulatory Factor-2-related Enzyme-1 (SIRT1) with Cardiac Function NYHA Classification in Patients with Chronic Heart Failure

【Objective】
To investigate the relationship between serum silent information regulator 2-related enzyme 1 (SIRT1) level and cardiac function NYHA classification in patients with chronic heart failure (CHF).
【Methods】
A total of 103 patients with CHF in our hospital from January 2017 to June 2018 were selected as the observation group, and 51 healthy volunteers in the same period were selected as the control group. Serum SIRT1 levels were detected. Serum SIRT1 levels of patients with different NYHA grades in the two groups were compared. The correlation between serum SIRT1 levels and NYHA grading was analyzed. The serum SIRT1 levels of patients with different prognosis were compared after 2 years of follow-up.
【Results】
The serum SIRT1 level in the observation group was lower than that in the control group and the difference was statistically significant (P<0.05). With increase of NYHA grade, the serum SIRT1 level decreased and the difference was statistically significant (P<0.05). The Pearson correlation coefficient model analysis showed that the serum SIRT1 level was negatively correlated with NYHA grade (P<0.05) The 2-year mortality rate was 23.47% (23/98) and serum SIRT1 level of the patients who died was lower than that of the patients who survived (P<0.05).
【Conclusion】
Serum SIRT1 level in CHF patients is significantly decreased and the degree of reduction is closely related to cardiac function and prognosis, which can provide reference for clinical evaluation of cardiac function and prognosis and guide the improvement of treatment plan.

Value of MDT in Improving the Quality of Life of Children with Brain Tumors

【Objective】
To explore the new diagnosis and treatment based on MDT (multi-disciplinary team) for children with brain tumor.
【Methods】
A total of 122 children with brain tumor admitted to our hospital from January 2017 to December 2018 were analyzed and divided into observation group and control group according to random number table method, with 61 cases in each group. The control group received conventional diagnosis and treatment, and the observation group received multidisciplinary collaborative process treatment. The improvement of clinical symptoms and quality of life were compared between the two groups.
【Results】
The proportion of postoperative headache, dizziness, nausea and vomiting, fundus edema, tumor compression symptoms in the observation group were lower than those in the control group, the difference was statistically significant (P<0.05). Before treatment, there was no significant difference in the scores of quality of life between the two groups (P>0.05); after treatment, the scores of quality of life in the observation group were higher than those in the control group, and the difference was statistically significant (P<0.05).
【Conclusion】
New diagnosis and treatment based on MDT can improve the clinical prognosis and promote the fast recovery.

Correlation Analysis of Serum Apolipoprotein A1, B levels with the Number of Coronary Artery Lesions and Gensini Score in Patients with Coronary Heart Disease

【Objective】
To analyze the correlation between serum apolipoprotein A1 (ApoA 1) and B (ApoB) and the number of coronary artery lesions and Gensini score in coronary heart disease (CHD).
【Methods】
A total of 300 cases of CHD patients treated in Xi'an Third Hospital and People's Hospital of Xinjiang Autonomous Region from March 2016 to October 2018 were selected and divided into acute myocardial infarction (AMI) group (n=86), unstable angina pectoris (UAP) group (n=102), stable angina group (SAP) group (n=112). Coronary artery lesions were examined by angiography and Gensini score was performed. The serum ApoA 1 and ApoB were detected. The correlation between serum levels of apolipoprotein A1 and B and the number of coronary artery lesions and Gensini score was analyzed.
【Results】
The serum AopA1 level of the SAP group was the highest, the UAP group was the second, the AMI group was the lowest; the serum ApoB level of the AMI group was the highest, the UAP group was the second, the SAP group was the lowest, the differences were statistically significant (P<0.05). The proportion of multi vessel lesions and Gensini score of the AMI group were the highest,the UAP group was the second, the SAP group was the lowest, the difference was statistically significant (P<0.05). Pearson analysis showed that AopA1 was negatively correlated with the number of coronary artery lesions and Gensini score (r=-0.690, -0.653, P<0.001), while ApoB was positively correlated with it (r=0.713, 0.685, P<0.001).
【Conclusion】
Serum ApoA1 and apoB levels are correlated with the number of coronary artery lesions and Gensini score in CHD patients.

Preventive Effect of Somatosensory Interactive Games on the Progression of Senile Dementia Patients

【Objective】
To analyze the preventive effect of somatosensory interactive games on the progression of senile dementia patients.
【Methods】
Seventy-six patients with Alzheimer's disease who were treated in our hospital from September 2016 to August 2017 were included in the study. They were divided into the observation group (35 cases) and the control group (41 cases) according to the random number method. The control group received routine care and the observation group was given a somatosensory interactive game intervention in addition to routine care. The cognitive function, depression, activities of daily living, and subjective well-being were compared between the two groups.
【Results】
Before intervention, there were no significant differences in structural imitation, language ability, recall ability, attentional computational power, or memory and orientation between the two groups (P>0.05). After intervention, the observation group's structural imitation, language ability, recall ability, attentional calculation power, memory, and orientation force scores were significantly higher than those of the control group (P<0.05). Before the intervention, there was no significant difference in depression scores and activities of daily living between the two groups (P>0.05). After the intervention, of depression scores and activities of daily living in the observation group were significantly lower than those in the control group (P<0.05). Before the intervention, there was no significant difference between the two groups in loneliness and dissatisfaction, age attitude and agitation score (P>0.05). After the intervention, the observation group's loneliness and dissatisfaction, age attitude and agitation scores were significantly higher than the control group (P< 0.05).
【Conclusion】
Somatosensory interactive games can effectively improve cognitive function, depressive symptoms, and activities of daily living in patients with Alzheimer's disease, as well as enhance their subjective well-being.

Effect of Body Mass Index on Long-term Prognosis of Elderly Patients with Coronary Heart Disease after Percutaneous Poronary Intervention

【Objective】
To analyze the effect of body mass index (BMI) on the long-term prognosis of elderly patients with coronary heart disease after percutaneous coronary intervention (PCI).
【Methods】
From June 2014 to December 2018, 278 patients with stable coronary heart disease aged over 60 years were selected and divided into normal weight group (BMI of 18.5-24.0 kg/m
2
,n=81), overweight group (BMI of 24.1-28.0 kg/m
2
,n=134) and obesity group (BMI>28.0 kg/m
2
,n=63) according to BMI. The clinical data of the patients in the admitted were collected to analyze the influence of BMI on the long-term prognosis of patients.
【Results】
There were no significant differences in age, diabetes mellitus, hypertension, hyperlipidemia, white blood cell (WBC), platelet (PLT), hemoglobin (HB), high density lipoprotein cholesterol (HDL-C) and total cholesterol (TC) levels among the three groups (P<0.05). The 2-year mortality and incidence of adverse events in normal weight group were 8.64% and 32.10%, which were significantly higher than those in overweight group (2.24%, 17.16%) and obesity group (1.59% and 11.11%) (P<0.05). Logistic regression analysis showed that age≥70 years, waist circumference ≥ 90 cm, diabetes mellitus, hypertension, hyperlipidemia and normal BMI were independent risk factors for adverse events after PCI in patients with stable coronary heart disease (P<0.05).
【Conclusion】
Although the elderly patients with coronary heart disease with BMI ≥ 24.0 kg/m
2
have more risk factors than those with normal BMI, their long-term prognosis after PCI is better than those with normal BMI, and the risk of death is lower than that of patients with normal BMI.

Value of Red Blood Cell Distribution Width and Serum Calcium Ratio in Evaluating the Condition and Prognosis of Patients with Acute Pneumonia

【Objective】
To investigate the value of red blood cell distribution width (RDW-CV) and serum calcium ratio in patients with acute pneumonia.
【Methods】
According to the prognosis, 100 patients with acute pneumonia were divided into good prognosis group (n=50) and poor prognosis group (n=50). The differences of age, gender, course of disease, symptoms, pulse rate>100 beats/min, gastrointestinal symptoms, diabetes mellitus, white blood cell count (WBC), platelet count (PLT), hemoglobin (HB), C-reactive protein (CRP), D-Dimer (D-D), RDW-CV and serum calcium ratio were compared between the two groups. Logistic regression analysis was used to identify the risk factors of poor prognosis in patients with acute pneumonia.
【Results】
The pulse rate > 100 beats/min, gastrointestinal symptoms and diabetes mellitus in poor prognosis group were significantly more than those in good prognosis group (P<0.05); PLT, CRP and D-D levels in poor prognosis group were significantly higher than those in good prognosis group (P<0.05);the ratio of RDW-CV to serum calcium in poor prognosis group was significantly higher than that in good prognosis group (P<0.05); Logistic regression analysis confirmed that pulse rate>100 beats/min, gastrointestinal symptoms, diabetes mellitus, PLT ≤ 115.23×10

/L, CRP≥ 82.39 mg/L, D-D≥814.68 μg/L, RDW-CV and serum calcium ratio≥6.90 were risk factors of poor prognosis in patients with acute pneumonia.
【Conclusion】
For patients with acute pneumonia whose RDW/serum calcium ratio is more than or equal to 6.90, we should pay close attention to it and take corresponding treatment measures in time to improve the prognosis of patients.
