Effect of Schwann Cells on Repairing Rabbit Radius Defect with Tissue Engineered Bone Combined with Osteoblasts

【Objective】To observe the effect of Schwann cells on repairing radius defects with bone tissue engineering scaffolds composited with osteoblasts in rabbit.【Methods】Bone marrow mesenchymal stem cells (BMSCs) were isolated and cultured, then induced to differentiate into osteoblasts (IOBs). Schwann cells (SCs) were extracted from neonatal rabbit, which were identified by S-100 immunofluorescence. New Zealand rabbits were used to establish the model of radial defect. The models were randomly divided into 5 groups. group A was blank control group, while bone defects were implanted with nano-chitosan/hydroxyapatite porous scaffolds in group B. and bone defects were implanted with osteoblasts and scaffolds in group C. Furthermore, bone defects were implanted with Schwann cells and scaffolds in group D. and bone defects were implanted with Schwann cells, osteoblasts and scaffolds in group E. After 8 weeks, immunohistochemistry, HE staining, gross morphology and X-ray were performed to evaluate the formation of new bone in the radial defect of rabbits in each group.【Results】The results of X-ray examination after 8 weeks postoperatively showed that the bone defect was partially healed in B, C and D groups. Groups C and D showed better effects than group B. The bone defect in group E was healed completely, while the bone defect in group A weren't healed at all. Compared to group B or C or D after 8 weeks, HE staining showed that the number of osteoblasts and trabecular meshwork were the highest in group E. Also, bone repair was the fastest in group E. Immunohistochemistry showed that the expression of BDNF and NGF in group E were significantly higher than other groups.【Conclusion】Schwann cells (SCs) can significantly promote the repair of rabbit radius bone defects by engineering scaffold composited with osteoblasts, which was closely related to the expression of BDNF and NGF.

Comparative Analysis of Anesthetic Effect and Postoperative Cognitive Dysfunction of Different Anesthetic Drugs in Middle-aged and Elderly Patients

【Objective】To compare the anesthetic effect and postoperative cognitive dysfunction of different narcotic drugs in middle-aged and elderly patients.【Methods】A total of 94 elderly patients undergoing general anesthesia in our hospital were divided into two group according to the order of admission, with 47 cases in each group. In the observation group (Group A), patients were received etomidate and remifentanil, and patients in the control group (Group B) received propofol and remifentanil. The cognitive function of the two groups was recorded before and after operation and 3 days after operation. The operation intubation were analyzed.【Results】The mini mental status checklist (MMSE) score of Group A on day 3 after surgery (23.31±0.98) was significantly higher than that of Group B (21.05±1.02, P<0.05). The disappearing time of eyelash reflex and consciousness as well as fixation time of eyeball in Group A were significantly longer than group B (P<0.05). Before induction of anesthesia, SBP (systolic blood pressure), DBP (diastolic blood pressure) and HR (heart rate) of patients in the two groups showed no significant difference (P>0.05). 3min after intubation, the SBP, DBP and HR in Group A were significantly lower than those before the induction of anesthesia (P<0.05), and the decreasing value of Group A was significantly higher than that of Group B (P<0.05).【Conclusion】The use of propofol in elderly patients during anesthesia is effective. However, compared to the usage of etomidate, it has greater impact on cognitive function and patient's stress response. So the clinical anesthesia should be selected according to the specific circumstances of the patient.

Effect of Dexmedetomidine on Hepatic Ischemia Reperfusion Injury in Cholestatic Rats

【Objective】To investigate the effect of dexmedetomidine (Dex) on hepatic ischemia reperfusion injury (IRI) in cholestatic rats.【Method】A total of 24 cholestatic rats modeled by bile duct ligation were randomly divided into 3 groups (n=8): sham operated group (A group), hepatic IRI group (B group) and Dex group (C group). The hepatic portal triad were clamped by Pringle maneuver and maintained for 30 minutes. After 4 hours of reperfusion, blood samples were obtained for testing levels of total bilirubin (TBIL), direct billirubin (DBIL), alanine transaminase (ALT), aspartate transaminase (AST), and tumor necrosis factor-α (TNF-α). Liver tissues homogenates were obtained to test values of superoxide dismutase (SOD) and malondialdehyde (MDA), and were observed after hematoxylin-eosin (HE) staining for histopathological assessment.【Results】After 4 hours of reperfusion, TBIL and DBIL levels in three groups were not significantly different (P>0.05). Serum ALT, AST and TNF-α levels, tissue MDA contents in the B group were significantly higher than those in the A group (P<0.05). Tissue SOD activities in the B group were significantly lower than the A group (P<0.05). Serum AST and TNF-α levels, tissue MDA contents in the C group were significantly lower than the B group (P<0.05). Histopathological changes (liver tissue destruction, hepatic cord disorders and inflammatory cell infiltration) in the B group were more obvious than in the A and C groups.【Conclusion】Dexmedetomidine may attenuate hepatic IRI injury in cholestatic rats, possibly by alleviating systemic inflammation and oxidative stress.

Effects of Liraglutide on CA19-9, CA724, CEA and SF in Type 2 Diabetic Patients with Bone Abnormalities

【Objective】To investigate the effects of liraglutide on levels of CA19-9 (carbohydrate antigen 19-9), CA724 (carbohydrate antigen 724), CEA (carcinoembryonic antigen) and serum ferritin (SF) in patients with type 2 diabetes mellitus patients with abnormal bone mass.【Methods】A total of 182 patients with type 2 diabetes combined with bone abnormalities and who received treatment in the Department of Endocrinology in our hospital from December 2015 to March 2017 were randomly divided into the observation group and the control group, with 91 cases in each group. Patients of two groups of were given dietary adjustments, exercise intervention and insulin glargine intensive treatment. According to blood glucose levels, patients in the control group received metformin treatment and patients in the observation group received liraglutide treatment. Both groups were treated for at least 3 months. The fasting blood glucose (FPG), 2 hour postprandial blood glucose (2hPG), glycosylated hemoglobin (HbA1c) and fasting insulin (FINS) levels before and after treatment were compared between the two groups of patients. Meanwhile, the changes in tumor markers and SF levels before and after treatment were compared between the two groups as well.【Results】Before treatment, there was no significant difference in FPG, 2hPG, HbA1c, and FINS levels between the two groups (P>0.05). After treatment, the FPG, 2hPG, HbA1c, and FINS levels in the two groups of patients were significantly lower. The levels of 2hPG and FINS in the observation group were significantly lower than those of the control group; the difference were with statistically significant (P<0.05). Levels of CA19-9, CA724, CEA and SF showed no significant differences before treatment (P>0.05). However, after treatment, they were significantly decreased in both groups. CA19-9, CA724, CEA and SF levels in the observation group after treatment were significantly lower than those of the control group. The differences were statistically significant (P<0.05).【Conclusion】Liraglutide can effectively reduce blood glucose levels and improve insulin sensitivity in patients with type 2 diabetes mellitus complicated with abnormal bone mass. It can significantly reduce the levels of CA19-9, CA724, CEA and SF as well. Thus, it may reduce the risk of diabetic complications and occurrence of malignant tumor.

Difference of Corneal Morphology and Astigmatism in Children with Congenital Ptosis of Different Ages and Different Degrees

【Objective】To investigate the corneal morphology and astigmatism in different age groups of moderate and severe congenital ptosis and analyze the relationship between ptosis and age.【Methods】Eighty-three cases (114 eyes) of moderate and severe congenital blepharoptosis diagnosed by ophthalmology in Hunan Children's Hospital were collected in detail and examined by ophthalmology. According to the age of the children, they were divided into two groups: group A ( 3 ≤ age<5) and group B ( 5 ≤ age<12). The corneal morphology and astigmatism of patients with blepharoptosis at different ages were analyzed, and the differences of corneal morphology and astigmatism among patients with blepharoptosis at different ages were compared.【Results】In 114 eyes, there was no significant difference in corneal astigmatism 〔Cyl (D)〕, surface rule index (SRI), surface asymmetry index (SAI) and total astigmatism of retina [C (D)]in patients with different degrees of ptosis in the same age group (P>0.05). The results of preoperative corneal topography showed that the ratio of inverse rule and oblique astigmatism increased with age, and the difference was statistically significant (P<0.05). In the moderate blepharoptosis group, there were 37.0% children with conformable astigmatism, 25.9% with reverse astigmatism, 37.0% with oblique astigmatism. In the severe blepharoptosis group, 65.0% of children with conformable astigmatism, 11.7% of converse astigmatism and 22.3% of oblique astigmatism, there was significant difference between the two groups (P<0.05).【Conclusion】Children with ptosis increase with age.The proportion of oblique and inverse astigmatism increased with age, but there was no significant correlation with the degree of blepharoptosis. It is suggested that eyelid surgery should be performed at the age of 3 and 5 years, and refractive correction should be performed after operation.

Effects of Laparoscopic Ovarian Cyst Torsion Reduction Surgery on Ovarian Reserve Function

【Objective】To investigate the effect of laparoscopic torsion reduction of ovarian cyst pedicle on ovarian reserve function.【Methods】The clinical data of 80 patients with ovarian cyst pedicle torsion from January 2015 to July 2017 were analyzed retrospectively. Among them, 40 cases were treated with laparoscopic reduction of ovarian cyst pedicle torsion (observation group), 40 cases with laparoscopic appendectomy (control group). ), follicle stimulating hormone (FSH), anti-Mullerian duct hormone (AMH), basic antral follicle count, peak systolic velocity of ovarian artery (PSV), end diastolic velocity (EDV), pulsatile index (Pi), resistance index (RI)]and the incidence of complications were compared between the two groups.【Results】The operative time and bleeding volume in the observation group were more than those in the control group, and the difference was statistically significant (P<0.05), but there was no significant difference in the hospitalization time between the two groups (P>0.05). in the observation group was significantly higher than that in the control group (P<0.05). The decrease rate of AFC, PSV, EDV in observation group was lower than that in control group (P<0.05), and the difference between the two groups was statistically significant (P<0.05). In both groups, the FSH, AMH, RI, PI were higher than those of preoperation. The increase of FSH in the observation group was higher than that in the control group (P<0.05), and the increase rate of AMH, RI, PI in the observation group was lower than that in the control group, the difference was statistically significant (P<0.05). There were no complications such as biliary tract injury, subcutaneous emphysema, hypercapnia in the two groups.【Conclusion】Laparoscopic transposition of ovarian cyst pedicle can preserve the attachment, which has little effect on the ovarian reserve function after operation, and has high safety, so it is worth popularizing in clinic.

Role of CRH Family in the Inflammatory Pathway of Human Placental Trophoblast Cells

【Objective】To investigate the role of corticotropin-releasing hormone (CRH) family in the inflammatory signaling pathways of nuclear factor κB (NF-κB) and mitogen-activated protein kinase (MAPK) in human placental trophoblast.【Methods】Human placental trophoblastic cell line Jeg-3 cells were treated with CRH, UCN, UCN2and UCN3. CRH-receptor antagonists were used to co-incubate Jeg-3 cells with CRH family. Western blot was used to evaluate the expression of NF-κB by CRH family and the effects on phosphorylation of IκB and MAPK.【Results】The phosphorylation of p38 MAPK in Jeg-3 cells was significantly increased after CRH treatment. After Jeg-3 cells were treated with UCN2, the expression of NF-κB protein increased significantly, and the phosphorylation of IκB and MAPK increased significantly. After treatment with CRH-receptor antagonists, the NF-κB and MAPK pathway were inhibited. UCN and UCN3 had no effect on NF-κB and MAPK pathway.【Conclusion】UCN2 is involved in the activation of inflammatory neuropeptides of NF-κB and MAPK signaling pathway during inflammation of human placental trophoblastic cells. CRH is an inflammatory neuropeptide involved in the activation of the MAPK signaling pathway. CRH and UCN2 may be involved in the motor system for pregnancy and childbirth.

Urine α-1 Microglobulin Level in Prediction of Preeclampsia in Pregnant Rats

【Objective】To evaluate the predictive value of urine α-1 microglobulin (α

-MG) level for preeclampsia (PE) during gestational rats.【Methods】A total of 26 female SD rats under SPF condition were confirmed to be pregnant. These SD female rats were randomly divided into the PE model group and the normal pregnancy group, with 13 in each group. From the 11th day of pregnancy, rats in PE model group received Angiotensin Ⅱ-type 1 receptor antagonist (AT1-AA) while mice in normal pregnancy group received equal volume of saline by tail vein injection for 7 days. The tail artery systolic pressure was measured by animal's noninvasive blood pressure tester from the 10th day of pregnancy. The urine of pregnant rats of the two groups was collected on the 8th, 12th, 16th, and 20th day of pregnancy. The concentration of urine α

-MG in the two groups was detected by rate immunoassay. Caesarean section was performed before pregnancy end. The average number of newborn rats, the rate of deformity and stillbirth rate were compared between the two groups. The weight of newborn rats and wet weight of placenta were measured.【Results】At 12 to 21 days of gestation, the systolic blood pressure of the tail artery in the PE model group was significantly higher than that of the normal pregnancy group (P<0.05). Correlation between Sleep Disturbance and Motor, Non-motor Symptoms in Patients with Parkinson's Disease

【Objective】To study the correlation between sleep disturbance and motor, non-motor symptoms in patients with Parkinson's disease (PD).【Methods】A total of 265 PD patients with PD from January 2016 to December 2017 were enrolled the study and divided into sleep disturbance group and non-sleep disturbance group according the score of Pittsburgh sleep quality index (PSQI). The baseline data, results of Unified Parkinson's Disease Rating Scale Ⅲ (UPDRSⅢ), H-Y stage, Montreal Cognitive Assessment (MoCA), the Scale For Outcomes in PD For Autonomic Symptoms (SCOPA-AUT), Fatigue Scale-14 (FS-14) and non-motor symptoms scale (NMSS) were compared between the two groups. Pearson correlation coefficient was used to analyze the correlation between PSQI score and other results, and multivariate logistic regression was performed to explore the influencing factors of sleep disturbance.【Results】⑴In all 265 patients, sleep disturbance occurred in 107 cases, whose incidence was 40.4%. There were no significant differences in the baseline data including age, sex, course of disease and onset of disease between the two groups (
P
<0.05). ⑵The UPDRS-Ⅲ score in sleep disturbance group was significantly higher than that in the non-sleep disturbance group (
P
<0.05). There was significant difference in H-Y stage between the two groups, and the proportion of stage Ⅴ~Ⅵ in the sleep disturbance group was significantly higher than that in the non-sleep disturbance group (P<0.05). The result of correlation analysis showed that PSQI score of all patients had a significant positive correlation with the UPDRS-Ⅲ score and H-Y stage (r=0.216 and 0.153,

<0.05). ⑶The incidences of non-motor symptoms including orthostatic dizziness, restless legs, losing interest in things around, sad/depression, anxiety/tension, difficult concentrating, loss memory, constipation, unexplained pain and gustatory or olfactory dysfunction, SCOPA-AUT and FS-14 scores in the sleep disturbance group were significantly higher than those in the non-sleep disturbance group, and the MoCA score was significantly lower than that in the non-sleep disturbance group (P<0.05). The result of correlation analysis showed that PSQI scores were positively correlated with SCOPA-AUT and FS-14 scores (r=0.306 and 0.328, P<0.05), and negatively correlated with MoCA score (r=-0.266, P<0.05). ⑷Multivariate logistic regression showed that sad/depression, increased UPDRS-Ⅲ and SCOPA-AUT score were the independent risk factors of sleep disturbance (P<0.05), while increased MoCA score was an independent protective factor (P<0.05).【Conclusion】PD patients have high incidence of sleep disturbance, and it is not only related to motor symptom, but also closely correlated with NMS including depression, autonomic dysfunction and cognitive impairment.

Influencing Factors of In-Stent Restenosis after PCI in Elderly Patients with Coronary Heart Disease

【Objective】To analyze the influencing factors of restenosis after coronary stent implantation by percutaneous coronary intervention (PCI) in elderly patients with coronary heart disease in Hezhou area.【Methods】A total of 937 elderly patients with coronary heart disease in Hezhou area receiving treatment at the People's Hospital of Hezhou City from June 2012 to January 2017 were the subjects of this study. Clinical data of all patients were collected to analyze the related factors of restenosis after coronary stent implantation by PCI in senior coronary heart disease.【Results】A total of 47 cases of stent restenosis occurred in the 937 patients with coronary stents, accounting for 5.02% of the total study group. Smoking history, hypertension, diabetes, LDL-C≥2.6mmol/L, stent diameter<3.5mm, the number of stent ≥ 2 pieces, stent length≥30mm, tandem stent, withdrawl of aspirin, family history of coronary heart disease, no high-dose statins were independent risk factors for restenosis after coronary stenting by PCI in elderly patients with coronary heart disease (P<0.05).【Conclusion】The factors that affect the restenosis after coronary stenting in senior patients in Hezhou are complex and need to be combined with the basic disease and coronary angiography results to reasonably assess the risk factors. In cases where there is a high risk of coronary stent restenosis, it is necessary to increase the secondary prevention of coronary heart disease with drug treatment regimens, strive to reduce the occurrence of coronary stent restenosis and improve the prognosis of senior patients with coronary heart disease.

Clinical Significance of Serum miR-130a, HMGA1 and SCC-Ag Expression in Patients with Cervical Cancer

【Objective】To investigate the clinical significance of serum miR-130a, HMGA1 and SCC-Ag expression in patients with cervical cancer.【Methods】A total of 29 patients with cervical cancer (group A) from March 2016 to June 2017, 32 patients with cervical intraepithelial neoplasia (group B) and 28 healthy volunteers (group C) from March 2016 to June 2017 at our hospital were selected as the subjects of this study. The expression levels of miR-130a, HMGA1 and SCC-Ag in the three groups were compared and analyzed to explore the relationship of miR-130a, HMGA1 and SCC-Ag with clinicopathological features of patients with cervical cancer. ROC curve were used to compare the diagnostic efficacy of miR-130a, HMGA1, SCC-Ag and combined detection in the diagnosis of cervical cancer.【Results】The expression levels of miR-130a, HMGA1 and SCC-Ag in cervical cancer, cervical intraepithelial neoplasia group and healthy control group all showed decreas, and the difference between groups was statistically significant (F miR-130a =78.79, F HMGA1=155.40, FSCC-Ag=79.98, P<0.05).The expression of miR-130a, HMGA1 and SCC-Ag in serum was not related to the age of patients with cervical cancer (P>0.05); however, their expression levels were related to the pathological types of cervical cancer. In addition, miR-130a, HMGA1 and SCC-Ag were highly expressed in squamous cell carcinoma and had low expression in adenocarcinoma (P<0.05).The levels of miR-130a HMGA1 and SCC-Ag were also associated with FIGO stage, which showed high expression in stage III ~ IV and low expression in stage Ⅰ~Ⅱ tumors (P<0.05).The area under the ROC curve of combined detection for diagnosis of cervical cancer was AUC=0.914, 95% CI:0.846 ~ 0.982, and the sensitivity and specificity were 85.19% and 82.35%, respectively, which were higher than those of each index alone. By comparing the area under the ROC curve, it was found that the diagnostic efficiency of combined detection was higher than that of HMGA1 and SCC-Ag alone, and the difference was statistically significant (Z HMGA1=1.99, ZSCC-Ag=2.04, P<0.05). Though AUC combination>AUCmiR-130a, however, the difference was not statistically significant (Z=0.99, P>0.05).【Conclusion】The serum levels of miR-130a, HMGA1 and SCC-Ag in patients with cervical cancer are high, and their expression levels are related to the pathological type and FIGO stage. The combined detection of the three markers has a higher diagnostic value for cervical cancer.

Application of MR-DWI Combined with TRUS in the Diagnosis and Gleason Classification of Prostate Cancer

【Objective】To investigate the diagnostic value of magnetic resonance diffusion imaging (MR-DWI) combined with intestinal ultrasonography (TRUS) in prostate cancer and its application in Gleason classification.【Methods】From June 2015 to July 2017, 98 patients with benign and malignant prostatic diseases were studied. MR-DWI and TRUS were performed before operation. The apparent diffusion coefficient was recorded and compared with pathological diagnosis. The sensitivity, specificity and accuracy of different detection methods were compared, and the correlation between ADC and Gleason score was analyzed.【Results】Among 98 cases, 43 cases were prostate cancer, 40 cases were benign prostatic hyperplasia, 15 cases were prostatitis. The ADC value of prostate cancer patients was significantly lower than that of prostatic hyperplasia patients and prostatitis patients. The difference was statistically significant (P<0.05). The order of ADC in different TNM stages of prostate cancer patients was Ⅰ>Ⅱ>Ⅲ>Ⅳ, but there was no significant difference between the two groups (

The sensitivity of MR-DWI, TURS and MR-DWI+TRUS to diagnose prostate cancer were 86.0.4%, 74.48%, 95.34%, specificity 83.63%, 78.18%, 89.09%, respectively, and the accuracy rates were 84.69%, 74.49%, 91.83%, respectively. The accuracy of MR-DWI TRUS in the diagnosis of prostate cancer was the highest, which was significantly higher than that of the other two methods. The correlation analysis showed that the ADC value of prostate cancer patients was negatively correlated with Gleason score.【Conclusion】MR-DWI combined with TRUS has high accuracy in diagnosis and prognosis of prostate cancer.

【Objective】To study the effect of biphasic insulin aspart 50 (BIAsp50) and biphasic human insulin 50 (BHI50) in type 2 diabetes mellitus (T2DM) and on 2h postprandial plasma glucose (2hPG), serum levels of soluble intercellular adhesion molecule-1 (sICAM-1), soluble vascular cell adhesion molecule (sVCAM-1), monocyte chemoattractant protein-1 (MCP-1) and tumor necrosis factor-α (TNF-α).【Methods】66 patients with T2DM without previous insulin treatment in our hospital from January 2016 to January 2017 were enrolled the study and randomly divided into BIAsp50 group and BHI50 group, 33 cases in each group. Patients in the two groups were given BIAsp50 and BHIsp50 through subcutaneous injection twice daily. Fasting plasma glucose (FPG), 2hPG and its increment, incremental area under curve (IAUC) within postprandial 2h, serum levels of inflammatory markers including sICAM-1, sVCAM-1, MCP-1 and TNF-α were compared between the two groups before and after treatment.【Results】⑴There were no significant differences in baseline data, plamsa glucose level and inflammation markers between the two groups before treatment (P>0.05). ⑵ At 6 and 12 weeks after treatment, plasma glucose level were significantly decreased in the two groups (P<0.05).There was no significant difference in FPG between the two groups after treatment (P>0.05). 2hPG and its increment, IAUC within postprandial 2h in the BIAsp50 group at 6 and 12 weeks after treatment were significantly lower than those in BHI50 group, and HbA1c at 12 weeks after treatment was significantly lower than that in the BHI50 group (P<0.05). ⑶The total incidence of hypoglycemia in the BIAsp50 group was 12.1% and significantly lower than in the BHI50 group 33.3% (P<0.05), but there were no significant differences in incidences of each type of hypoglycemia between the two groups (P>0.05). ⑷At 6 and 12 weeks after treatment, serum levels of inflammatory markers were significantly decreased in the two groups (P<0.05). Serum levels of inflammatory markers in the BIAsp50 group at 6 and 12 weeks after treatment were significantly lower than those in the BHI50 group, and sICAM-1, MCP-1 at 12 weeks after treatment were significantly lower than those in the BHI50 group (P<0.05).【Conclusion】Compared with BHI50, BIAsp50 has better control effect on postprandial plasma glucose in patients with T2DM, and can decrease the incidence of hypoglycemia, and serum levels of sICAM-1, sVCAM-1, MCP-1 and TNF-α were lower, thus the improvement of injury of vascular endothelial cells and inflammation response is more prominent.

Analysis of Factors Related to the Number of Lymph Nodes Detected in Laparoscopic Radical Resection of Rectal Cancer

【Objective】To investigate the factors influencing the number of lymph nodes detected after laparoscopic radical resection of rectal cancer.【Methods】Clinical data of 87 patients with rectal cancer undergoing laparoscopic radical resection of rectal cancer from February 2016 to March 2017 were selected. According to whether the patients received neoadjuvant therapy (including radiotherapy and chemotherapy) before operation, they were divided into neo-adjuvant therapy group and non-neoadjuvant treatment group. All kinds of clinicopathological factors which may affect the number of lymph nodes detected were analyzed.【Results】The maximum diameter of the tumor, the length of the specimen, the depth of tumor invasion and the number of lymph nodes detected by the operation were significantly correlated, the difference was statistically significant (P<0.05). However, sex, age, tumor location, pathological type, tumor differentiation, TNM staging, preoperative CEA and surgical procedure had no correlation with the number of lymph nodes detected (P>0.05). The average number of total lymph nodes in the non neoadjuvant therapy group, the average number of intestinal lymph nodes and the number of mesenteric lymph nodes were significantly higher than those in the neoadjuvant therapy group, the difference was statistically significant (P<0.001). The maximum diameter of the tumor, the length of the specimen, the depth of tumor invasion, and whether the neoadjuvant therapy was accepted or not were independent factors affecting the number of lymph nodes more than 12.【Conclusion】The maximum diameter of tumor, the length of specimen, and the depth of tumor invasion were related to the number of lymph nodes detected in rectal cancer patients. Patients with minimal tumor diameter, small specimen length, low depth of tumor infiltration, and preoperative neoadjuvant therapy are more likely to detect 12 lymph nodes in postoperative specimens.

Effect of Wardmate Pairing Rehabilitation Model on the Rehabilitation Efficacy of Hemiplegic Patients with Cerebral Infarction

【Objective】To investigate the effect of wardmate pairing rehabilitation model on the rehabilitation efficacy of hemiplegic patients with cerebral infarction.【Methods】A total of 100 cases of hemiplegic patients with cerebral infarction admitted to our hospital from July to December of 2016 were selected as the research objects, and divided into observation group and control group according to the random number table method, 50 cases in each group. Patients in the control group were treated with routine nursing, and based on this, patients in the observation group performed the wardmate pairing rehabilitation model. After 3 months of intervention, the motor function, balance function, self-care ability, living quality and other indexes between the two groups were compared.【Results】The scores of Fugl-Meyer assessment (FMA) and Berg balance scale (BBS) in observation group were significantly higher than those in the control group (t=8.682, 5.749, P<0.05, P<0.01); the scores of functional comprehensive assessment scale (FCA) and modified Barthel index (BI) were significantly higher than those in the control group (t=9.811, 7.765, P<0.05); the scores of stroke impact scale (SIS) such as strength, hand function, mobile function and emotions were significantly higher than those in the control group (t=8.239, 13.337, 13.110, 10.773, P<0.05, P<0.01).【Conclusion】Wardmate pairing rehabilitation model is helpful to enhance the motor and balance functions of hemiplegic patients with cerebral infarction, improve their self-care ability and enhance their living quality.

Clinical Effects Comparison of Catheter Directed Thrombolysis and Peripheral Venous Thrombolysis in Patients with Acute Lower Deep Vein Thrombosis

【Objective】To compare the clinical efficacy of catheter directed thrombolysis (CDT) and peripheral venous thrombolysis (AVT) in the treatment of acute lower deep vein thrombosis (LDVT).【Methods】A total of 70 cases of patients with acute LDVT in Shuyang People's Hospital were collected for the study and divided into the CDT group (n=36, treated with inferior vena cava filter and CDT) and the AVT group (n=34, treated with AVT). The clinical efficacy, venous angiography, coagulation and fibrillation indexes and complications were compared between the two groups.【Results】The significant curative effects were shown in the two groups after operation. The circumference difference of legs and uninjured limb after thrombolysis was significantly smaller than that before thrombolysis (P<0.05), and the decrease rate and swelling rate of the CDT group were better than those of the AVT group (P<0.05). and the concentrations of D-dimer and FDP at 24h of thrombolysis were significantly higher than those before operation (P<0.05), and the concentrations at the end of thrombolysis were significantly lower than those at 24h of thrombolysis or those before thrombolysis (P<0.05). And the PIB level at 24h of thrombolysis was significantly lower than that before operation (P<0.05) and significantly higher than that at the end of thrombolysis (P<0.05), and the changes degrees of the above indexes in the CDT group were significantly greater than those in the AVT group. The total incidence rate of postoperative complications was 30.55% (11/36)in the CDT group and 20.58% (7/34)in the AVT group, therefore there was no significant difference in the postoperative complications between the two groups (χ

=0.91, P>0.05).【Conclusion】CDT and AVT both have good effects in the treatment of acute LDVT. There are no significant differences in the incidence rate of complications in the use of CDT and AVT, but the swelling rate and thrombolysis rate of CDT are better than those of AVT, therefore CDT is more suitable for LDVT treatment.

Clinical Observation on 60 Cases of Uterine Prolapse Treated by External Suspension of Rectus Abdominis

【Objective】To investigate the curative effect of treatment of uterine prolapse by external suspension of rectus abdominis.【Methods】From January 2013 to March 2017, 60 cases of Ⅱ ~ Ⅲ degree prolapse of uterus were selected and treated by external suspension of rectus abdominis. The index, curative effect and recurrence rate were recorded.【Results】Of 60 patients, the operative time was 57.86 ±23.42 days, the intraoperative bleeding was 80.79 ±28.82 mL, the postoperative exhaust time was 23.22 ±4.10 min / h, the visual analogue pain score was 4.86 ±1.42, and the hospital stay was 6.18 ±1.39 days. In addition, there was no pelvic and abdominal organ injury and bladder injury. 57 cases were cured in 60 cases, the cure rate was 95.00 and only 2 cases recurred and the recurrence rate was 3.33%.【Conclusion】The treatment of uterine prolapse by external suspension of rectus abdominis muscle is effective, high safety and low recurrence rate.

Effect of Empathetic Nursing on Emotional Health and Treatment Compliance of AIDS Patients Undergoing Antiviral Therapy

【Objective】To investigate the effects of empathetic nursing in AIDS patients undergoing antiviral therapy.【Methods】A total of 84 AIDS patients who received antiviral therapy in our hospital from June 2014 to May 2016 were selected as study subjects. They were divided into the observation group and control group using a random number table, with 42 cases in each group. Patients in the control group were given routine nursing interventions including health education, psychological intervention and medication supervision. Patients in the observation group received empathetic nursing intervention on the basis of the routine nursing intervention outlined above. During a follow-up after 12 months, the degree of anxiety and depression, treatment compliance, treatment effect and other indicators of the two groups were compared.【Results】The scores on the self-rated anxiety scale (SAS) and self-rated depression scale (SDS) in the observation group were significantly lower than those in the control group (P<0.05). Correlation between Self-esteem, Hostility, Attribution Bias and Loneliness in Schizophrenics

【Objective】To explore the relationship between self-esteem, hostility attribution bias and loneliness in schizophrenic patients.【Methods】The clinical data of 147 schizophrenic patients from February 2013 to August 2017 in our hospital were selected. According to the Rosenberg Self-esteem scale (Rosenberg Self-esteem scale) score, they were divided into two groups: high self-esteem group (group H) and lower self-esteem group (group L), the Chinese version of ambiguity, purpose and hostility questionnaire (AIHQ-C) and the Chinese version of Lonely scale (UCLAA) were used to evaluate the hostility attribution bias and loneliness. At the same time, the positive and negative symptom scale (PANSS) was used to compare the differences of hostility attribution bias, self-esteem level and loneliness between the two groups, and to analyze the correlation between the self-esteem level and hostility attribution bias and loneliness.【Results】Loneliness scores of the group H were significantly lower than those of the group L, the difference was statistically significant (t=2.6511, P<0.05); AIHQ-C, HB, BB and AB of the group H were significantly lower than those of the group L, the difference was statistically significant (t=2.9049, t=3.7436, t=3.1781, P<0.05); The Pearson correlation analysis showed that patients with schizophrenia SES and UCLA score, AIHQ-C HB, BB, AB, PANSS total score, positive symptoms, negative symptoms were negatively correlated (r=-0.337,-0.176,-0.167,-0.258,-0.198,-0.291,-0.208, and P<0.05); the spirit of in patients with UCLA score and AIHQ-C HB, BB, AB division, PANSS total score, positive symptoms, negative symptoms were positively correlated (r=0.193, 0.336, 0.397, 0.346, 0.364, 0.224, P<0.05).【Conclusion】The patients with lower self-esteem have obvious hostile attributional bias and higher loneliness level. Relationships of Insulin Resistance with Metabolic Syndrome and with Cardiovascular Events in Patients with Hypertension

【Objective】To investigate the relationships of insulin resistance with metabolic syndrome and with cardiovascular events in patients with hypertension.【Methods】A total of 191 patients with hypertension in our hospital from April 2015 to August 2016 were selected as study subjects. According to the insulin resistance index, patients were divided into the simple hypertension group (the control group) and insulin-resistant hypertension group (the observation group). The systolic blood pressure, diastolic blood pressure, glucose and lipid metabolism, high sensitivity C reactive protein, and adiponectin were detected for all patients and insulin resistance indexes were calculated. The incidence of cardiovascular events were recorded through one year of follow-ups.【Results】 (1) There were statistically significant differences in blood pressure, glucose metabolism, lipid metabolism, high sensitivity C reactive protein, adiponectin, and insulin resistance indices of the two groups (P<0.05); (2) In the control group, patients with metabolic syndrome had significantly higher levels of fasting glucose and adiponectin levels than those with non-metabolic syndrome. The fasting blood glucose, adiponectin, high-sensitivity C-reactive protein and insulin resistance index of metabolic syndrome patients in the observation group were significantly higher than those in non-metabolic syndrome patients, and the levels of high-density lipoprotein cholesterol were significantly lower than those in metabolic syndrome patients (P<0.05). (3) With regards to the relationship between metabolic syndrome and metabolic risk factors; the high density lipoprotein cholesterol was negatively correlated with metabolic syndrome. Uric acid, high-sensitivity C reactive protein and insulin resistance index were positively correlated with metabolic syndrome. (4) A 1-year follow-up of patients with hypertension showed that high insulin resistance index, high triglyceride, high sensitive C reactive protein and low high density lipoprotein cholesterol had a higher probability of cardiovascular events (P<0.05).【Conclusion】Insulin resistance is significantly higher in hypertensive patients than in normal subjects. Insulin resistance is significantly associated with metabolic syndrome; insulin resistance index potentially becomes a new predictor after traditional cardiovascular risk factors such as high-density lipoprotein cholesterol, triacylglycerol, and high-sensitivity C-reactive protein.

Predictive Intervention in Preventing Aspiration of Enteral Nutrition in Critical Newborns

【Objective】To investigate the effect of preventive intervention on the prevention of enteral nutrition aspiration in critical newborns.【Methods】A total of 102 cases of critical newborns who received enteral nutrition in the neonatal department of our hospital from February 2014 to February 2016 were selected as the research subjects. They were divided into the observation group and the control group according to the random number table method, with 51 cases in each group. The newborns in the control group were given routine nursing for critical ill children, and the newborns in the observation group were given preventive intervention based on the risk assessment of aspiration. The aspiration rate, mortality rate, survival rate, complications, hospitalization time and hospitalization cost were compared between the two groups.【Results】The aspiration rate and mortality rate in the observation group were significantly lower than those in the control group, and the survival rate of the observation group was significantly higher than that of the control group. The incidence of complications such as vomiting was significantly lower than that of the control group (P<0.05). Days of hospitalization in the neonatal intensive care unit (NICU) and total length of hospital stay in the observation group were significantly shorter than those in the control group. Therefore, NICU hospitalization expenses and total hospitalization costs were significantly lower than those in the control group (P<0.05).【Conclusion】The preventive intervention based on aspiration risk assessment can help to reduce the aspiration rate, mortality and complications of enteral nutrition in critical newborns, shorten the hospital stays and cut the hospitalization expenses.

Effects of Microneurosurgical Techniques Combined with Gamma Knives on Parasagittal Meningioma and Analysis of Postoperative Recurrence Rate

【Objective】To study effects of microneurosurgical techniques combined with gamma knives on parasagittal meningioma and analyze the postoperative recurrent rate.【Methods】A total of 50 patients with parasagittal meningioma treated in our hospital from April 2012 to April 2015 were selected as study subjects. With regards to tumor location, there were 15 cases in the anterior third, 22 cases in middle third and 13 cases in posterior third of the sagittal sinus. Retrospective analysis was conducted on all clinical data. Microneurosurgical techniques were employed as the main treatment, and gamma knife was adopted to handle tumor remnants after surgery. Tumor resection status (classified as benign or malignant through pathological examination) were recorded for all patients.Patients were surveyed for incidence and recurrence rates of various complications through 2 years of follow-ups. KPS scores of quality of life were compared before and after treatment.【Results】According to the Simpson resection classification criteria, there were 16 cases of grade Ⅰ (32%), 8 cases of grade Ⅱ (30%) and 15 cases of grade Ⅲ. The total resection rate of tumors was 78%. There were 11 cases (22.00%) of Simpson grade Ⅳ with a subtotal resection rate of 22.00%. The gamma knife treatment was performed on the Simpson grade Ⅱ, Ⅲ, and Ⅳ resections. After 2 years of follow-up, the clinical symptoms nearly disappeared, and there were no cases of death. Postoperative tumor recurrence occurred in 4 patients (8.00%), including 1 case in both grade Ⅱ and grade Ⅲ and 2 cases in grade Ⅳ. Based on pathological diagnosis, there were 44 benign tumors (88.00%) and 6 malignant tumors (12.00%). The KPS scores before operation was 72.67±3.32 points, which was lower than that of KPS scores after operation of 86.43±4.02 points; the difference was statistically significant (P<0.05). A total of 18 cases (36.00%) had postoperative complications: 9 cases (18.00%) of neurological impairment, 3 cases (6.00%) of spasms, 4 (8.00%) of hematoma, and 2 cases (4.00%) of brain swelling.【Conclusion】Microneurosurgical treatment of sagittal sinus paraneoplastic meningioma has great clinical effects. After resection of residual tumors by gamma knife treatment, it can effectively control the recurrence of tumors. Microneurosurgical techniques combined with gamma knives can significantly improve the quality of life of patients and is worthy of clinical application.

Correlation Analysis of Insulin Pump Base Amount and BMI,HbA1c in Patients with Newly Diagnosed Type 2 Diabetes

【Objective】To investigate the correlation between the basic amount of initial intensive insulin pump therapy and body mass index (BMI), glycosylated hemoglobin (HbA1c) in patients with newly diagnosed type 2 diabetes mellitus (T2DM).【Methods】From January 2015 to June 2017, 93 newly diagnosed T2DM patients in Department of Endocrinology, Guangdong Provincial Hospital of traditional Chinese Medicine, receiving intensive insulin pump therapy, were selected, and the general data of the patients were collected. BMI and WHR were calculated. Serum biochemical indexes were detected by automatic biochemical analyzer, whole blood HbA1c was measured by HPLC, fasting blood glucose (FBG) and 2 h postprandial blood glucose (2hPGG) were monitored during treatment. The correlation between the basic quantity of insulin pump and clinical index was analyzed.【Results】After intensive treatment with insulin pump, 72 patients reached the standard of blood glucose and 21 patients failed to reach the standard. The BMIs, the total amount of insulin pump and the basic quantity of insulin pump were significantly lower than those in the substandard group (P<0.05). There was a positive correlation between the basal insulin pump level and the course of BMI, FPG, 2hPG and HDL-C (mean P<0.05), and a negative correlation with low density lipoprotein cholesterol (LDL-C) (P<0.05), and no correlation with age, WHR, HbA1c, TC, TGs (mean P>0.05).【Conclusion】For patients newly diagnosed with type 2 diabetes, BMI is associated with the basal volume of initial insulin pump therapy and can be used as a reference indicator of basal insulin pump volume.

Analysis of the Relationship Between Social Discrimination, Stigma and Quality of Life in Schizophrenic Patients

【Objective】To explore the relationship between social discrimination, stigma and quality of life of outpatients and inpatients with schizophrenia.【Methods】The clinical data of 96 schizophrenic patients from April 2014 to February 2017 were selected and divided into outpatient group and inpatient group according to whether they were hospitalized with 48 cases in each group. The social discrimination, stigma and quality of life of patients with schizophrenia were evaluated and compared with the Discrimination against Psychiatric patients Assessment scale / Link's stigma scale and the schizophrenia quality of Life scale (SQLS). Spearman correlation analysis was used also to analyze the relationship between social discrimination, stigma and quality of life.【Results】The incidence of stigma and social discrimination in the inpatient group was 100.0% (48/48), 95.83% (46/48)respectively, while in the outpatient group it was 81.25% (39/48), 72.92% (35/48), respectively, the incidence of stigma and social discrimination in outpatient group was significantly lower than that in inpatient group, the difference was statistically significant (P<0.05). Compared with the outpatient group, the scores and total scores of the inpatients' stigma scale, social discrimination scale and quality of life scale were significantly higher than those of the outpatient group, the difference was statistically significant (P<0.05).The stigma and social discrimination were positively correlated with the total scores of psycho-social, motivation and energy, quality of life (P<0.05), but not with symptoms and adverse drug reactions (P>0.05).【Conclusion】Schizophrenia patients generally have a sense of social discrimination and stigma, especially inpatients, and the higher the sense of social discrimination and stigma, the worse the quality of life.

Value of Color Doppler Ultrasound in Evaluating the Curative Effect of Neoadjuvant Chemotherapy in Patients with Breast Cancer

【Objective】To evaluate the value of color Doppler ultrasound (CDU) in evaluating the curative effect of neoadjuvant chemotherapy in patients with breast cancer.【Methods】A total of 200 patients were examined with CDU before and after neoadjuvant chemotherapy. The tumor volume, two-dimensional ultrasonic findings, blood flow grade of lesions and resistance index (RI) were compared between patients with different curative effect.【Results】CDU showed CR+PR in 130 cases and SD+PD in 70 cases. With the pathological results as the gold standard, the sensitivity and accuracy of CDU in evaluating the curative effect of neoadjuvant chemotherapy were 85.29% and 71.00%, respectively. The tumor volume of CR+PR group was significantly smaller than that of the same group before chemotherapy and SD+PD group after chemotherapy (P<0.05). After chemotherapy, rates of clear boundary, regular shape, clear internal echo and normal rear echo in the PR group were significantly higher while rate of rich blood flow and RI value were significantly lower than those before chemotherapy (P<0.05). There was no significant differences of above indexes in the SD+PD group before and after chemotherapy (P>0.05).【Conclusion】CDU can be used to objectively and effectively evaluate the curative effect of chemotherapy in treatment of breast cancer through two-dimensional ultrasonic findings and blood flow signals.

Changes of Serum Tumor Markers CEA, CA72-4 and CA153 in Patients with Postmenopausal Diabetes Mellitus

【Objective】To investigate the changes in serum tumor markers carcinoembryonic antigen (CEA), carbohydrate antigen 72-4 (CA72-4) and CA153 in patients with postmenopausal type 2 diabetes mellitus (T2MD).【Methods】A total of 105 postmenopausal women with T2MD were selected as the observation group, and 94 healthy women of the same age were selected as the control group. The fasting plasma glucose (FPG), postprandial 2h blood glucose (2hPG), glycosylated hemoglobin A1c (HbA1c) and levels of CEA, CA72-4 and CA153 were compared between the two groups.【Results】The levels of FPG, 2hPG, HbA1c, CEA, CA72-4 and CA153 in the observation group were higher than those in control group, and the differences were statistically significant (P<0.05). Levels of CEA, CA72-4 and CA153 were lower in T2MD patients<65 years old than those patients ≥ 65 years old, and the difference was statistically significant (P<0.05). The CEA, CA72-4, and CA153 in HbA1c<7% T2MD patients were lower than those in HbA1c 7% ~ 9% and HbA1c>9% T2MD patients, and CEA, CA72-4 and CA153 levels were lower in T2MD patients with HbA1c of 7% ~ 9% than those with HbA1c>9%; the differences were statistically significant (P<0.05). The levels of serum CEA, CA72-4 and CA153 were positively correlated with FPB, 2hPG, HbA1c and age (P<0.05).【Conclusion】Compared to postmenopausal women with normal glucose metabolism, levels of serum CEA, CA72-4 and CA153 are higher in postmenopausal women with T2MD, which may be related to the fluctuation of blood glucose.

The control group was given routine treatment for asthma, and the observation group was given vitamin D intervention for 3 months on the basis of routine treatment. Clinical Efficacy of Continuous Vena-venous Hemofiltration Combined with Invasive Mechanical Ventilation on Refractory Heart Failure (24 Cases of Report)

【Objective】To explore the clinical effect of continuous Vena-venous hemofiltration (CVVH) combined with invasive mechanical ventilation for refractory heart failure.【Methods】A total of 46 patients with refractory heart failure were selected in the ICU of the Zhuzhou Center Hospital from March 2015to April 2017, and they were divided into observation group ( n = 24) and control group ( n = 22 ) by using the random number table method. The control group was treated with routine therapy and invasive mechanical ventilation, while the observation group was treated with continuous hemofiltration CVVH on the basis of the control group. The improvement of cardiac function, renal function and related respiratory function was compared between the two groups before and after treatment.【Results】After treatment, the improvement of left ventricular ejection fraction, cardiac output and oxygenation index in the observation group was significantly better than that in the control group, and the levels of blood urea nitrogen and creatinine in the observation group were lower than those in the control group (P<0.05). Compared with the control group, the number of B-line ultrasound in the observation group was significantly lower than that in the control group (P<0.05), and the time of invasive mechanical ventilation was shorter than that in the control group, the difference was statistically significant (P<0.05).【Conclusion】For refractory heart failure, invasive mechanical ventilation combined with continuous hemofiltration therapy can improve the heart and kidney function of patients, remove the machine early, and have high clinical application value.
