Abstract:【Objective】 To exploring the diagnostic efficacy of four-dimensional ultrasound spatiotemporal correlation imaging and ultrasound tomography (STIC-TUI) technology for fetal congenital heart disease in the second trimester of pregnancy (early-middle pregance). 【Methods】 A total of 80 pregnant women who underwent ultrasound screening in our hospital from January 2020 to January 2022 were selected as the research subjects. All of them received both STIC-TUI and conventional two-dimensional ultrasound imaging examinations.Using the output review results as the gold standard,We used the Kappa to analyze the efficacy of STIC-TUI alone,conventional two-dimensional ultrasound alone,and the combination of the two methods in diagnosing fetal congenital heart disease in the second trimester. Time consumption and image quality scores between STIC-TUI and conventional two-dimensional ultrasound examination were also compared. 【Results】 Among 80 fetuses,7 cases were found to have cardiac abnormalities. Of which,6 cases were detected using STIC-TUI technology,and 1 was missed in the diagnosis of atrial septal defect; Routine two-dimensional ultrasound detected 5 cases,while missed diagnosis in 2 cases of atrial septal defect. Postpartum follow-up confirmed 7 children with heart disease. The postnatal follow-up result showed there was no significant difference between STIC-TUI technology and conventional two-dimensional ultrasound(P>0.05). However,the STIC-TUI examination time was significantly shorter than the conventional two-dimensional ultrasound examination time,and the difference was statistically significant (P<0.05). There was no significant difference in image quality scores between STIC-TUI and conventional two-dimensional ultrasound (P>0.05). After consistency analysis,the sensitivity of STIC-TUI in diagnosing fetal congenital heart disease was 0.714,the specificity was 0.986,the accuracy was 0.962,and the Kappa value was 0.748; The sensitivity,specificity,accuracy,and Kappa value of conventional two-dimensional ultrasound in diagnosing fetal congenital heart disease are 0.571,0.986,0.950,and 0.640,respectively; The sensitivity,specificity,accuracy,and Kappa value of the combination of STIC-TUI and conventional two-dimensional ultrasound in the diagnosis of fetal congenital heart disease are 0.857,0.986,0.975,and 0.843,respectively. 【Conclusion】 STIC-TUI can be used to diagnose fetal congenital heart disease in the second trimester of pregnancy with good diagnostic efficacy. Moreover,the combination of STIC-TUI and conventional two-dimensional ultrasound can improve the sensitivity of diagnosis.
