Application Value of Two-dimensional Strain Imaging in Evaluating Left Atrial Myocardial Function in Elderly Patients with Hypertension and Paroxysmal Atrial Fibrillation

Abstract:【Objective】To evaluate the value of two-dimensional strain imaging in the assessment of left atrial myocardial function in elderly patients who had hypertension complicated with paroxysmal atrial fibrillation.【Methods】A total of 86 patients with hypertension plus paroxysmal atrial fibrillation who were admitted to our hospital from January to December 2018 were enrolled. Patients were divided into different groups according to the number of echocardiographic signs of left ventricular diastolic dysfunction: zero sign group, single sign group and multiple signs group. Myocardial function and left atrial myocardial strain in different groups of patients were compared. 【Results】Based on the number of echocardiographic signs of left ventricular diastolic dysfunction in patients, there were 20 cases (23.3%) in the zero sign group, 35 cases (40.7%) in the single sign group and 31 cases (36.0%) in the multiple signs group. The results showed that LVEF of patients with the zero sign group was higher than that of the single sign and multiple signs group, while LVEDD, LVEDV and LAVI were lower than those of the single sign and multiple signs group. The differences were statistically significant (P<0.05). The strain and strain rate in the middle segment and the middle wall of the left atrial wall of the zero sign group were higher than those of the single sign and multiple signs group; the differences were statistically significant (P<0.05).【Conclusion】With the increase in the number of echocardiographic signs of left ventricular diastolic dysfunction, the left atrial myocardial function in hypertension patients complicated with paroxysmal atrial fibrillation was significantly reduced.
