摘要【目的】评估影响胆道闭锁患儿父母心理健康状况的因素、降低胆道闭锁家庭的疾病心理负担,提高胆道闭锁患儿的整体疗效。【方法】采用便利抽样方法,抽取2017年11月至2019年11月于深圳市儿童医院就诊的胆道闭锁患儿父母共100例,采用症状自评量表(SCL-90)对其心理健康状况进行评估,评估数据与全国常模相比较,并采用单因素方差分析及多元线性回归分析影响其心理状况的因素。【结果】胆道闭锁患儿父母的SCL-90评估得出的总均分及躯体化、焦虑、敌对、恐怖、精神病性评分均与全国常模相比,差异有统计学意义( P <0.05),一定程度上说明患儿父母的心理健康状况不容乐观。对患儿父母心理健康状况总分、抑郁及焦虑评分进行单因素方差分析,可见在性别、家庭支持情况、患儿出生史方面均存在统计学差异( P <0.05),同时患儿父母的总分、抑郁心理状况在不同年龄组、文化程度、职业、民族等方面存在统计学差异( P <0.05)。对有意义的因素进一步进行多元线性回归结果显示,患儿父母心理健康SCL-90评分的总分与年龄、性别均呈负相关,与家庭支持与否呈正相关;抑郁状况与性别呈负相关,与年龄、家庭支持与否呈正相关;焦虑状况与家庭支持与否呈正相关。【结论】胆道闭锁患儿父母心理健康状况普遍较差,年龄、性别、家庭支持情况与患儿父母心理健康状况存在线性相关关系。
Abstract:【Objective】 To evaluate the factors that affect the mental health of parents who have children with biliary atresia, reduce the psychological burden of diseases in families with biliary atresia, and improve the overall efficacy of children with biliary atresia. 【Methods】Using the convenience sampling method, a total of 100 parents with biliary atresia children, who were admitted to Shenzhen Children's Hospital from November 2017 to November 2019, were selected. The mental health status was assessed by the Symptom Self-Rating Scale (SCL-90). The evaluation data were compared with the national norm. Single factor analysis of variance and multiple linear regression were used to analyze the factors that affected the psychological status. 【Results】The total mean score and scores of somatization, anxiety, hostility, terror and psychosis obtained by the SCL-90 assessment of the parents with biliary atresia children were compared with the national norm. The difference was statistically significant ( P <0.05). The results showed that the mental health of the parents with biliary atresia children was not optimistic to a certain extent. A one-way analysis of variance was performed on the total scores of the parents, depression and anxiety. It showed that there were statistical differences in gender, family support, and birth history of the children ( P <0.05). At the same time, there were statistical differences in the total scores and depressive psychological status of the parents who had biliary atresia children in different age, education levels, occupations, nationalities, etc. ( P <0.05). Further multivariate linear regression results from meaningful factors showed that the status of parents' mental health total SCL-90 scores was negatively correlated with age and gender, and positively correlated with family support. The depression status was negatively correlated with gender, while it was positively correlated with age and family support. The anxiety status was positively correlated with family support. 【Conclusion】The mental health status of parents who have children with biliary atresia is generally not so good. There is a linear correlation of parents' mental health status with age, gender and family support.
