Abstract:【Objective】To investigate the diagnostic value of the detection of anaplastic lymphoma kinase (ALK) in uterine inflammatory myofibroblastic tumor (IMT). 【Methods】A total of 2000 cases of uterine smooth muscle tumor were screened by ALK immunohistochemistry. ALK staining in the cytoplasm and/or cell membrane of tumor cells was considered as ALK positive. The diagnostic value of ALK protein detection in uterine IMT was discussed. 【Results】The positive rate of ALK protein in patients with uterine IMT was significantly higher than that in patients with benign leiomyoma (P<0.05). Using pathological results as gold standard, ALK protein expression was calculated by four grid table to diagnose uterine IMT. Results showed that the sensitivity, specificity, accuracy, positive predictive value and negative predictive value were 72.00%, 99.85%, 99.50%, 85.71% and 99.65%, respectively. In 2000 cases of uterine smooth muscle tumor, 25 cases were diagnosed as uterine IMT by pathological examination. Of which, 18 cases were diagnosed with uterine IMT by using ALK protein positive: 14 cases (14/18, 77.78%) were strongly positive for ALK expression (+++), 3 cases (3/18, 16.67%) were moderately positive (++), and 1 case (1/18, 5.56%) was weakly positive (+). In the 18 uterine IMT patients with ALK positive stainning, the FISH results showed that 88.89% (16/18) patients had ALK rearrangement, and 11.11% (2/18) had no ALK rearrangement. 【Conclusion】Immunohistochemical staining of ALK protein expression in uterine smooth muscle tumors has a good diagnostic value for the detection of IMT with high specificity and accuracy.
