Abstract:【Objective】To explore the clinicopathological features of patients with different subtypes of papillary thyroid micro carcinoma (PTMC). 【Methods】A total of 367 patients with different subtypes of PTMC who were treated in our hospital from January 2010 to March 2018 were enrolled. The patients were observed for lesion size, calcification, extra-glandular microinfiltration, lymph node metastasis and Hashimoto's thyroiditis. Univariate and multivariate logistic regression analysis evaluated the influencing factors of lymph node metastasis in patients with PTMC. 【Results】Among all patients, there were 166 cases of columnar cell subtype PTMC, accounting for 45.2%; 152 cases of follicular subtype PTMC, accounting for 41.4%; 49 cases of other subtypes, containing 35 cases of intracapsular subtype, 8 cases of Warthin tumor-like subtype, 1 case of acidophilic subtype, 1 case of trabecular subtype and 4 cases of high cell subtype PTMC. Follicular subtype PTMC had the largest lesion diameter. Other subtypes of PTMC had the highest calcification rate and no extra-glandular infiltration. The lymph node metastasis rate of columnar cell subtype PTMC patients was significantly higher than that of follicular subtypes and other subtypes. Multivariate logistic regression analysis showed that males, younger than 50 years old, lesion calcification and extra-glandular infiltration were independent risk factors for PTMC lymph node metastasis. 【Conclusion】PTMC is mainly composed of columnar cell subtypes and follicular subtypes. The clinicopathological characteristics of PTMC in different subtypes are varied. Being male, age less than 50 years old, lesion calcification and extra-glandular infiltration are independent risk factors for PTMC lymph node metastasis.
