Abstract：【Objective】To explore the clinical effect of Xiaoshui Decoction on communicating hydrocephalus (aSAH-CHP) after spontaneous subarachnoid hemorrhage. 【Methods】From October 2016 to August 2018, 60 patients with aSAH-CHP admitted to the Department of Neurosurgery, Affiliated Hospital of Shandong University of Traditional Chinese Medicine and Neurosurgery, Weifang Hospital of Traditional Chinese Medicine were randomly divided into observation group and control group with 30 cases in each group. Patients in the control group were given conventional western medicine treatment, while patients in the observation group were given Xiaoshui drink on the basis of the same treatment in the control group. Two groups of patients were treated for 6 courses, followed by 3 months of observation and follow-up. After the end of the clinical study, we observed and recorded the improvement of clinical symptoms of TCM, the change of neurological deficit score, the disappearance of hydrocephalus and the adverse reactions during the medication.【Results】The effective rate of the observation group was 83.3% (25/30) significantly higher than that of the control group (56.7%) (17/30), the difference was statistically significant (χ2=-2.180,P=0.029<0.05). After treatment, the scores of main symptoms, secondary symptoms and total symptoms in the two groups were lower than those before treatment, and those in the observation group were lower than those in the control group, the difference was statistically significant (P<0.05). There were 26 cases (86.6%) with Evans index < 0.3 in the observation group and 24 cases (80.0%) in the control group. There was no significant difference in the disappearance rate of hydrocephalus between the two groups (P>0.05); 2 cases of diarrhea and 1 case of electrolyte abnormality in the observation group; 2 cases of abnormal liver and kidney function and 2 cases of electrolyte disturbance in the control group. There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).【Conclusion】On the basis of Western medicine, Xiaoshui Decoction can reduce the scores of TCM symptoms and improve clinical symptoms in patients with aSAH-CHP.
