329 Effect of Human Amniotic Fluid Stem Cell Transplantation on Carbon Tetrachloride Induced Cirrhosis in Rats and its Mechanism

【Objective】To investigate the therapeutic effect and mechanism of human amniotic fluid stem cell transplantation on carbon tetrachloride induced cirrhosis in rats. 【Methods】 Cirrhotic rats were made by injecting 60% carbon tetrachloride vegetable oil subcutaneously for 7 weeks, and then randomly divided into control group (injecting PBS of equal volume),, hAFSC transplantation group and the intervention group of praxafu (AMD3100), a specific blocker of CXCR4 receptor of stromal cell-derived factor-1 (SDF-1) after the transplantation of AFSC.After 3 weeks of treatment, all rats were killed, serum ALP, ALB and CH were measured, and cirrhosis was evaluated according to Ishak scoring standard. In vitro experiments were divided into three groups: control group (PBS), AMD3100 group and AMD3100 + SDF-1 group. The effects of different treatment factors on the survival, migration and adhesion of HFSC were observed. 【Results】Compared with the control group, the serum ALB and Ch in the transplantation group were significantly higher (P<0.05), and ALP had no significant change (P>0.05), while ALP, ALB and Ch in the intervention group were not significantly different from those in the control group (P>0.05). Liver tissue fibrosis scores were statistically significant between the transplant and control groups (P=0.005), and significant differences between the transplant and the intervention groups (P=0.022).The results The results of in vitro experiments showed that SDF-1 could alleviate the inhibition of AMD3100 on the migration, survival and adhesion of hAFSC (P<0.05).【Conclusion】hAFSC transvenous intrahepatic transplantation reduces the extent of carbon tetrachloride-induced cirrhosis in rats and its mechanism is related to SDF-1/CXCR4 regulation.

333 Significance of Vitamin A and E Levels in the Pathogenesis of Herpetic Isthmus in Children

【objective】To explore the association between serum vitamin A、E levels and herpetic isthmus in children.【Methods】 Sixty children with herpes isthmus treated in our hospital from october 2018 to september 2019 were selected as observation group, and 60 children with healthy physical examination were selected as control group in the same period to compare the serum vitamin A、E levels of children in the two groups.【Results】There was no significant difference in leukocyte count, neutrophil percentage, lymphocyte percentage and CRP between the two groups (P>0.05). The proportion of normal serum vitamin a content, marginal type deficiency and clinical deficiency in the observation group was significantly lower than that in the control group (P<0.05); the serum vitamin a level in the observation group was significantly lower than that in the control group (P<0.05). Compared with the control group, there was no significant difference in the proportion of normal, deficient and excessive vitamin E in the serum of the observation group (P>0.05); the level of vitamin E in the serum of the observation group was slightly lower than that of the control group, but there was no significant difference (P>0.05).【Conclusion】 Vitamin A clinical deficiency and marginal type deficiency in children with herpetic isthmus are higher, and the average serum vitamin A content is significantly lower than that in normal healthy children, which may be one of the predisposing factors of herpes isthmus.

336 Experimental Study of Individualized Bionic Artificial Lamina Based on 3D Printing Technique

【Objective】The individualized bionic artificial lamina of titanium alloy was made by 3D printing technique. The feasibility of implantation was discussed through the model of lumbar laminectomy. 【Methods】A sample of adult lumbar vertebrae was selected, and the titanium alloy bionic artificial plate was printed in 3D. The lumbar vertebral laminectomy and implantation were performed and the cortical bone trajectory screw and locking screw technique were used to fix it. Afterwards the stability was observed. Finally, CT scan was applied to understand the condition of artificial lamina and screws. 【Results】The morphology of bionic spinal lamina designed in this study was similar to the original lamina structure. It was easy to install and matched well in simulated surgery. The use of cortical bone trajectory screw technique to fix the bionic lamina showed good stability. The retained spine and interspinous ligaments were reconstructed to the suture hole of artificial lamina spinous process. The results of CT scan showed that the designed artificial lamina was similar to the original spinal spinous process lamina, with good matching, ideal position of fixation screw and significant increase in the volume and anteroposterior diameter of osseous spinal canal after surgery. 【Conclusion】The individualized bionic artificial lamina designed by 3D printing technique can increase the volume of spinal canal after lumbar laminectomy, whose compatibility and stability are good. It has the advantages of retaining the motion of spinal segment and providing posterior ligament attachment point reconstruction.

340 Influence of Mother-infant Bedside Intervention on Mastery of Breastfeeding Knowledge and Exclusive Breastfeeding Rate in Primiparas

【Objective】To explore the influence of mother-infant bedside intervention on the rate of exclusive breast-feeding and mastery of breast-feeding knowledge. 【Methods】A total of 210 primiparas in the obstetrics department of Shanghai First Maternity and Infant Hospital from July 2018 to July 2019 were randomly divided into the observation group(n=105 cases received mother-infant bedside intervention)and the control group(n=105 cases received routine bedside intervention).Observation and comparison of breastfeeding rate, health knowledge, psychological status and nursing satisfaction in the two groups after different interventions. 【Results】After intervention, the breastfeeding rate, health knowledge, psychology and nursing satisfaction in the observation group were better than those in the control group. The differences were statistically significant (P<0.05). 【Conclusion】The application of mother-infant bedside intervention in primipara can effectively increase the breastfeeding rate and improve mothers' health knowledge. At the same time, it is beneficial to the relationship between nurses and patients to improve the nursing satisfaction. So it is worthy of clinical popularization.

343 Clinical Value of Mobile Medicine in Standardized Management of Gestational Diabetes Mellitus

【Objective】To analyze the clinical value of mobile medicine in the standardized management of gestational diabetes mellitus (GDM). 【Methods】A total of 120 GDM patients admitted to Beijing Pinggu District Hospital from January 2018 to September 2019 were randomly divided into the observation group and the control group, with 60 cases in each group. Both groups were given routine GDM standardized management, while the observation group was added with WeChat-based mobile medical management. The management effect, pregnancy outcome and delivery outcome of the two groups were compared. 【Results】Two weeks and 4 weeks after management, the Fasting plasma glucose (FPG) and 2-hour postprandial blood glucose (2hPG) of the two groups were lower than before the management. The FPG and 2hPG compliance rates were higher than before the management. Both the 2hPG compliance rate after 2 weeks of management and the FPG and 2hPG compliance rates after 4 weeks of management in the observation group were higher than those of the control group (P<0.05). The incidence of preeclampsia, the rate of cesarean section and the total time of labor in the observation group were lower than those in the control group; the difference was statistically significant (P<0.05). The incidence of hypoglycemia in newborns of the observation group was lower than that of the control group (P<0.05). 【Conclusion】The application of mobile medicine in the standardized management of GDM can significantly improve the effect of blood glucose control, improve the outcome of pregnancy and delivery.

347 Effect of Stachydrine on Tumor Growth and Gene Expression in Gastric Xenograft Cancer in Mice

【Objective】 To study the regulation effect of stachydrine on tumor growth and the expression of CyclinD1, c-myc, Bcl-2, c-Met, p53, p16, Bax, Caspase-3 in gastric cancer xenograft tumors mice. 【Methods】After cultured SGC-7901 cell line of human gastric cancer, the cell suspension was inoculated into BALB / C nude mice subcutaneously to obtain the transplanted tumor model of gastric xenograft cancer. The mice were randomly divided into two groups: the control group treated with normal saline and the STA group treated with stachydrine . The volume and weight of the transplanted tumor and the expression of these oncogenes were measured after 21 days.【Results】The volume and weight of the transplanted tumor in the STA group were significantly lower than those in the control group; the protein expression of CyclinD1, c-myc, Bcl-2 and c-Met in the transplanted tumor was significantly lower than that in the control group, while the protein expression of p53, p16, Bax and Caspase-3 were significantly higher than that in the control group. 【Conclusion】Stachydrine can inhibit the growth of gastric cancer transplanted tumor mice, down regulate the expression of proto oncogene and up regulate the expression of tumor suppressor gene.

350 Effects of Danhong Injection on Coagulation and Fibrinolysis Function, Platelet Activation, Inflammatory Factors and Oxidative Stress after Emergency PCI

【Objective】 To investigate the effects of Danhong injection on coagulation and fibrinolysis function, platelet activation, inflammatory factors and oxidative stress after emergency percutaneous coronary intervention (PCI) in patients with acute myocardial infarction (AMI). 【Methods】A total of 86 patients with AMI who underwent emergency PCI treatment in our hospital were randomly divided into two groups: the observation group and the control group. The patients in the two groups were continuously treated for 14 days with conventional western medicine, while patients in the observation group received Danhong injection. The coagulation and fibrinolytic function: thrombin time (PT), activated partial thromboplastin time (APTT), fibrinogen (FIB), and D-dimer (DD), platelet activation function: Lysosomal membrane glycoprotein (CD63), platelet alpha granule surface membrane glycoprotein (CD62p), monocyte platelet aggregates (MPA), inflammatory factor indicators: C-reactive protein (CRP), interleukin-6 (IL-6) ) and oxidative stress indicators: peroxidase dismutase (SOD) and malondialdehyde (MDA) were observed before PCI and 14 days after PCI and compared between the two groups. 【Results】 ①The total effective rate of the observation group was higher than that of the control group (90.70% vs 72.09%, respectively, P<0.05).②There was no significant difference in coagulation and fibrinolysis function between the two groups before treatment (P>0.05).After treatment, the coagulation and fibrinolysis function (APTT,PT,FIB and D-Dimer) of the two groups was improved obviously; and the improvement degree of the observation group was better than that of the control group (P<0.05).③After treatment, the platelet activation function (CD63,CD62p and MPA) in the two groups was significantly lower than that before treatment; and the decrease in the observation group was more significant than that in the control group (P<0.05).④After treatment, the levels of inflammatory factors (CRP and IL-6) in the two groups were both lower than before; and the decrease in the observation group was more significant than that in the control group at the same time (P<0.05).⑤After treatment, the index of oxidative stress (SOD and MDA) in the two groups was obviously improved; and the degree of improvement in the observation group was better than that in the control group (P<0.05). 【Conclusion】Application of routine western medicine combined with Danhong injection after emergency PCI can significantly improve clinical efficacy, reduce blood viscosity, improve hemodynamics, inhibit immune inflammatory and oxidative stress response, and thus stabilize atherosclerotic plaques and improve the prognosis.

354 Analysis of Disinfection Effect and Safety on Anesthesia Machine Using Circuit Disinfection Device

【Objective】To investigate the disinfection effect and safety on anesthesia machine using circuit disinfection device. 【Methods】Our hospital had 38 close-circulated anesthesia machines that normally run during June 2017 to May 2018. Those anesthesia machines were sterilized by the device called anesthesia machine circuit loop sterilizer. The total number of colonies, the contaminated sites of the anesthesia machine, the incidence of patient infection and the metal corrosion at 3 months before and after anesthesia machine disinfection were recorded and compared. 【Results】The rate of anesthesia machine contamination was 28.95% (11/38) 3 months before disinfection, which was significantly higher than the contamination rate of 7.79% (3/38) 3 months after disinfection. The difference was statistically significant (P<0.01).There were no significant differences in the contamination rates between the inhalation end of the anesthesia machine, the outlet of the sodium-lime tank and the inlet of sodium-lime tube at 3 months before disinfection and 3 months after disinfection (P>0.05).The exhalation contamination rate of the anesthesia machine 3 months before disinfection was 18.42% (7/38), which was significantly higher than that of 5.26% (2/38) showed at 3 months after disinfection (P<0.01).The lung infection rate in patients 3 months before disinfection was 9.00% (9/100) and the respiratory infection rate was 14.00% (14/100), which was significantly higher than 2.00% (2/100) and 3.00% (3/100) , irrespectively, at 3 months after disinfection (P<0.05). The weight of carbon steel 72h after disinfection was significantly lower than that before disinfection (P<0.05). However, here was no significant difference in the weight of aluminum, copper and stainless steel 72h before and after disinfection (P>0.05). 【Conclusion】The use of 10% hydrogen peroxide by ultrasonic atomization disinfection of anesthesia machine can obtain a reliable disinfection effect, thereby reducing the contamination rate of anesthesia machine and patient respiratory infection, while improving the safety of anesthesia machine usage.

357 Comparison of 3D ASL and SWI in Assessment of Ischemic Penumbra in Acute Ischemic Stroke

【Objective】To investigate the differential diagnosis of ischemic penumbra (IP) in acute ischemic stroke (AIS) by susceptibility weighted imaging (SWI) and three-dimensional arterial spin labeling (3D ASL).【Methods】 Seventy-eight AIS patients admitted to our hospital from October 2016 to may 2019 were selected. Through the display of prominent cortical vein (PCV) in AIS patients by SWI and the decreased cerebral blood flow (CBF) area of AIS patients to evaluate IP, the modified alberta stroke program early CT score (ASPECTS) was used to score the IP displayed by the two methods. the results of the IP scores displayed by the two methods were compared and the clinical data of different degrees of IP were compared and analyzed.SWI showed that there was no significant difference in gender, age, onset time, diabetes mellitus, hypertension and baseline NIHSS scores between different PCV groups (P>0.05). ASL showed that there was no significant difference in gender, age, onset time, diabetes mellitus, hypertension and baseline NIHSS scores between groups in different CBF reduction areas (P>0.05). The PCV score of 78 patients was (5.06 ± 2.28) and that of CBF decreased area was (6.03 ± 2.33). The PCV score was lower than that of CBF decreased area (t=2.46,P=0.02<0.05).【Results】 There were no significant differences in clinical datum between IP groups at different degrees (P>0.05).The score of PCV for IP was lower than that of CBF reduction area, and the difference was statistically significant (t=2.46, P<0.05).【Conclusion】 The PCV sign of SWI can indirectly indicate the presence of brain IP in AIS, while the decreased CBF area displayed by 3D ASL can more directly and comprehensively show the cerebral ischemia, and 3D ASL has more advantages than SWI in displaying IP.

361 Effects of 0.5% Ropivacaine Stellate Ganglion Block on Stress Response, Immune Function and Gastrointestinal Function after Abdominal Operation

【Objective】To investigate the effects of 0.5% ropivacaine stellate ganglion block (SGB) on stress response, immune function and gastrointestinal function after abdominal operation. 【Methods】A total of 80 patients who underwent abdominal surgery in our hospital were randomly divided into two groups, with 40 cases in each group. Patients in the observation group received 0.5% ropivacaine SGB before induction of anesthesia, while patients in the control group received the same amount of normal saline. The levels of TNF-α, IL-6 and Cor (cortisol) were compared between the two groups at 5 min before SGB and at 2 h, 4 h and 24 h after SGB. The levels of CD3, CD4, CD8 and CD4/CD8 were compared at 1 day before surgery and 3 days after operation. The recovery time of postoperative bowel sounds, anal exhaust time and bloating occurred in the groups were recorded. 【Results】(1) Compared to 5 min before SGB, the levels of TNF-α and IL-6 in the observation group increased significantly at 2h, 4h and 24h after SGB, which showed a trend of increasing first and then decreasing. Meanwhile levels of TNF-α and IL-6 in the observation group were higher than those in the control group (P<0.05). The levels of Cor at 2 h, 4 h, and 24 h after SGB in the observation group decreased gradually if compared to 5 min before SGB. The levels of Cor in the observation group were lower than those in the control group (P<0.05). (2) The levels of CD3, CD4 and CD4/CD8 in the two groups decreased first and then increased, while CD8 increased first and then decreased. CD3, CD4 and CD4/CD8 in the observation group at 1 and 3 days after operation were significantly higher than those in the control group, while CD8 in the observation group was significantly lower than that in the control group (P<0.05). (3) The recovery time of bowel sounds, anal exhaust time and incidence of abdominal distension in the observation group were significantly lower than those in the control group (P<0.05).【Conclusion】0.5% ropivacaine SGB can reduce the stress response and immune function of patients after abdominal surgery, and effectively promote the recovery of gastrointestinal function.

364 Effect of Serum Thyroid Stimulating Hormone Level on Risk of Cardiovascular Disease in Patients with Type 2 Diabetes Combined with Subclinical Hypothyroidism

【Objective】To investigate the effect of serum thyroid stimulating hormone levels on the risk of cardiovascular disease in type 2 diabetes patients with subclinical hypothyroidism. 【Methods】One hundred T2DM patients with subclinical hypothyroidism who were treated in our hospital from January 2014 to January 2017 were divided into two groups according to the TSH level at the time of admission: the TSH increased group (TSH > 5.6 IU/mL, n=28) and the TSH normal group (TSH ≤ 5.6 IU/mL, n=72). The incidence rate of cardiovascular disease in two groups was compared, and the correlation between TSH level and clinical index was analyzed. Multivariate Logistic regression analysis was performed to analyze the risk factors of cardiovascular events.【Results】The levels of free triiodothyronine (FT3) and free thyroxine (FT4) in the TSH increased group were significantly lower than those in the TSH normal group (P<0.05); the incidence of cardiovascular events in the TSH increased group was 39.29% (11 / 28), higher than 13.89% (10/72) in the TSH normal group (P<0.05). TSH level was positively correlated with systolic blood pressure at admission (P<0.05), and negatively correlated with FT3 and FT4 (P<0.05). Multiple logistic regression analysis showed that: uric acid, systolic blood pressure, TSH, FT3, triglyceride (TG) were the risk factors of cardiovascular disease.【Conclusion】Serum TSH levels are significantly associated with the risk of cardiovascular disease in type 2 diabetes patients with subclinical hypothyroidism.

367 Correlation between Thyroid Hormone and Blood Lipid Level in the Second Trimester of Pregnancy and its Effect on the Birth Weight of Newborns

【Objective】 To investigate the correlation of free thyroxine (FT4), free triiodothyronine (FT3) , thyrotropin (TSH), triglyceride (TG), total cholesterol (TC) and their effects on the birth weight of the newborn in the second trimester (12~16 weeks).【Methods】 A total of 10,574 pregnant women was selected from the International Peace Maternal and Child Health Center Affiliated to Shanghai Jiaotong University Medical College and Songjiang District Maternal and Child Health Center of Shanghai from January to December 2018. The correlation between thyroid hormone (FT4, FT3, TSH) and blood lipid level (TG, TC) at the 12th to 16th weeks of pregnancy was analyzed to determine the effect on the birth quality of newborns. 【Results】The level of FT4 in pregnant women was negatively correlated with TG and TC, the level of FT3 in pregnant women was negatively correlated with TG and TC (P<0.05), TSH was negatively correlated with TG (P<0.05), and TSH was not correlated with TC (P=0.79). FT4 level decreased, TG level increased more than the risk of gestational age infants (LGA); FT4 level increased, TG level decreased increased the risk of SGA in female infants; the combination of FT4 level decreased in pregnant women and TG level increased had an interactive effect on newborn birth weight, LGA risk increased in male infants [OR=1.79, 95% CI (1.38, 2.33)].【Conclusion】The risk of LGA of male baby is increased when FT4 level of pregnant women decreases and TG level increases. Monitoring thyroid hormone and lipid level and taking corresponding measures could reduce the risk of SGA and LGA.

371 Study on Predictive Role of Circulating Tumor Cells in Lung Cancer Metastasis and Chemotherapy Effect

【Objective】To investigate the effect of circulating tumor cells (CTC) on lung cancer metastasis and chemotherapy effect prediction. 【Methods】A total of 68 patients with lung cancer who were treated in our hospital from April 2015 to October 2016 were enrolled in the observation group. In the same time period, 57 patients with benign pulmonary diseases were selected as the control group and 63 healthy volunteers were enrolled as the blank control group. The positive rate of CTC in the vein was determined by immunomagnetic beads negative enrichment and cell immunofluorescence staining. All patients completed 4 cycles of chemotherapy. After the treatment, the patients were followed for 2 years. CTC positive rate at different treatment cycles were recorded. 【Results】The positive rate of CTC in the observation group was 60.29% (41/68), which was higher than that in the control group 5.26% (3/57) and the blank control group 7.94 (5/63). The difference between the observation group and control groups was statistically significant (P<0.05), however, there was no significant difference in the positive rate of CTC between the control group and the blank control group (P>0.05). Chemotherapy was completed and the patients in the observation group were followed up for 2 years. The follow-up results showed that 23 patients had metastasis (33.82%) and 41 patients with CR/PR (60.29%). There were statistically significant differences in terms of the positive rate of CTC in lung cancer patients with metastasis, non-metastasis and different effects of chemotherapy (P<0.05). 【Conclusion】CTC is highly expressed in lung cancer patients, which can predict metastasis. It can help to evaluate the effect of chemotherapy and provide reference for the adjustment of patients' chemotherapy plans.

【Objective】To investigate the efficacy of thoracoscopic single-hole and three-hole pneumonectomy in patients with early non-small cell lung cancer (NSCLC) and their effects on interleukin-8 (IL-8), irregular chemokines (Fractalkine) and human macrophage inflammatory protein 3α (MIP-3α) levels. 【Methods】Patients with early non-small cell lung cancer who underwent thoracoscopic pneumonectomy at our hospital were selected. The surgical methods were thoracoscopic single-hole and three-hole pneumonectomy. The general surgical information, lung function, IL-8, Fractalkine, MIP-3α, postoperative recovery and 1-year survival rate were compared between the two groups. 【Results】 The intraoperative blood loss and the time to get out of bed after operation in patients of the single-hole operation group were lower than those of the three-hole operation group (P<0.05). There were no significant differences in the operation time, number of lymph nodes dissipated, and time to remove the thoracic drainage tube between the two groups (P>0.05). Levels of IL-8, Fractalkine and MIP-3α before surgery did not show significant differences between the two groups (P>0.05). After surgery, the levels of IL-8, Fractalkine, and MIP-3α in the single-hole operation group level was lower than those in the three-hole operation group; and the differences were statistically significant (P<0.05). There were no significant differences of lung function-related indicators: forced expiratory velocity at the first second (FEV1), forced vital capacity (FVC), and maximum voluntary ventilation (MVV) levels before and after surgery between the two groups of patients (P>0.05). The one-year survival rates of the two groups were 75.00% and 72.50%, respectively, which had no significant difference (P<0.05). The number of postoperative lung infections, nerve injuries and chest pain in the two groups were no significant difference as well (P>0.05). 【Conclusion】Thoracoscopic single-hole and three-hole pneumonectomy are similarly effective for patients with early non-small cell lung cancer. However, single-hole surgery has less trauma and can lower levels of IL-8, Fractalkine and MIP-3α, which has good application value.

378 Effect of Curcumol on Proliferation and Migration of Mouse Melanoma B16 Cells

【Objective】To investigate the effect and mechanism of curcumol on proliferation and migration of mousemelanoma B16 cell. 【Methods】B16 cells were cultured in RPMI-1640 medium with 10% fetal bovine serum. The proliferation, migration and mir-7-5p content of B16 cells treated with Curcumol (50, 100, 200μmol / L) were detected. B16 cells were pretreated with mir-7-5p inhibitor, then treated with Curcumol (100 μ mol / L) to detect cell proliferation and migration.【Results】The 490 nm absorbance of B16 cells treated with different doses of Curcumol (50, 100, 200μmol / L) was lower than that of the control group, and with the increase of Curcumol Concentration, the 490 nm absorbance of the cells decreased significantly, the difference was statistically significant (P<0.05). After treatment of B16 cells with different doses of Curcumol (50, 100, 200 μ mol / L), the number of transmembrane cells was lower than that of the control group, and with the increase of Curcumol Concentration, the number of transmembrane cells decreased significantly, the difference was statistically significant (P<0.05).The inhibitory effect of miR-7-5p inhibitor on B16 cells was verified by real-time PCR. After NC inhibitor and miR-7-5p inhibitor were transfected, the miR-7-5p of the cells were (1.02±0.10) and (0.41±0.14), respectively, with statistical significance (P<0.05). After inhibiting miR-7-5p of B16 cells, the regulation of Curcumol on cell proliferation was observed, compared with NC inhibitor (1.413) and the difference was statistically significant (P<0.05). Compared with NC inhibitor (1.413±0.108), miR-7-5p inhibitor increased the absorbance value (1.697±0.089) of B16 cells treated with Curcumol,the difference was statistically significant (P<0.05).【Conclusion】Curcumol could inhibit cell proliferation and migration of mouse melanoma B16 cells, and its mechanism might be related to the increased expression of miR-7-5p.

382 Expression of BMP-15, sRAGE and MMP9 in the Follicular Fluid of Patients with PCOS and Their Correlation with Egg Quality

【Objective】To investigate the expression of BMP-15, sRAGE and MMP9 in the follicular fluid of patients with PCOS and their correlation with egg quality. 【Methods】A total of 96 patients undergoing in vitro fertilization-embryo transfer (IVF-ET) in our hospital from January 2015 to February 2018 were selected. Among them, 63 patients with PCOS were divided into 35 cases with insulin-resistant PCOS (IR group) and 28 cases with non-insulin resistant PCOS (non-IR group) according to the insulin resistance index (HOMA-IR). In addition, 33 infertility (normal ovulation) cases caused by fallopian tube or male factors were used as the control group. The levels of BMP-15, sRAGE, MMP9, FSH, LH, E2 and T (testosterone) were detected from the follicular fluid of the three groups. The correlation between those index and egg quality was analyzed. 【Results】The levels of sRAGE and MMP9 in the follicular fluid of the IR and non-IR groups were significantly higher than those in the control group (P<0.05), while the level of BMP-15 was significantly lower than the control group (P<0.05). The level of sRAGE in the follicular fluid of the IR group was higher than that of the non-IR group (P<0.05). There was no significant difference in serum FSH and E2 levels among the three groups (P>0.05). The levels of serum LH and T in the IR and non-IR groups were higher than those in the control group (P<0.05). However, there was no significant difference in serum LH level between the IR and non-IR groups (P>0.05).The egg maturation rate and fertilization rate did not show any significant difference between the IR group, the non-IR group and the control group (P> 0.05). The number of eggs obtained in the IR group and non-IR group was higher than that of control group (P<0.05), while the rate in the IR group and non-IR group was lower than that in the the control group (P<0.05). The levels of BMP-15 and sRAGE in the follicular fluid of patients with PCOS were positively related to the fertilization rate and high-quality embryo rate (P<0.05), while MMP9 was negatively related to the fertilization rate and high-quality embryo rate (P<0.05).【Conclusion】PCOS patients have lower level of BMP-15 and higher levels of sRAGE and MMP9, which are related to the quality of egg development.

385 Meta Analysis to Valuate the Efficacy and Safety of Reduning Injection Combined with Antibiotics in the Treatment of Community Acquired Pneumonia in Children

【Objective】 To evaluate the clinical efficacy and safety of Reduning injection combined with antibiotics in the treatment of children community acquired pneumonia by Meta-analysis. 【Methods】Randomized controlled trials were retrieved in Pubmed,Embase,CBM,CNKI,VIP and Wanfang database.Quality of the RCTs was evaluated strictly by using the Jadad quality evaluation method,statistical analysis was performed using Meta-analysis.【Results】 Finally, 8 articles were included in the study, the total number of cases was 2451. The results of meta analysis showed that: compared with the single antibiotic group, the combined drug group had more advantages in the total effective rate[WMD=-2.64,95%CI (-2.75,-2.54),P<0.0001], the disappearance time of cough [WMD=-0.96,95%CI (-1.18,-0.74),P<0.00001], asthma and sputum, the disappearance time of lung rale[WMD=-0.96,95%CI(-1.18,-0.74),P<0.05], the shadow absorption time of X-ray focus[WMD=-2.88,95%CI(-3.05,-2.71),P<0.00001], the disappearance time of respiratory shortness of breath[WMD=-1.26,95%CI(-1.32,-1.20),P<0.05] and the difference was statistically significant (P<0.05).【Conclusion】Reduning combined with antibiotics is safe and effective in the treatment of children's community-acquired pneumonia, but more high-quality large sample, multicenter, randomized, double-blind, controlled clinical trials are needed to further confirm.

390 Investigation and Analysis of Influencing Factors of Psychological Resilience of Outpatients Based on Multicenter

【Objective】To analyze the psychological resilience of outpatients and its influencing factors. 【Methods】With the method of cluster sampling, 247 outpatient nurses from 9 Grade-A general hospitals in Changsha,Zhuzhou and Xiangtan of Hunan Province were investigated by using general data questionnaire, simple coping style scale (SCSQ), social support rating scale (SSRs) and Chinese Revised Psychological Resilience Scale (CD-RISC). Single factor analysis of variance and multiple stepwise regression analysis were used to analyze the psychological resilience status and influencing factors of outpatients. 【Results】Therhe was significant difference in psychological resilience among nurses of different age, education level and working age (P<0.05). Pearson correlation analysis showed that the total score of psychological resilience of outpatient nurses was positively correlated with the total score of social support, subjective support, objective support and utilization of support (P<0.05), positively correlated with positive response (P<0.05), and not positively correlated with negative response (P>0.05). Positive coping, social support and education level were the related factors (P<0.05). 【Conclusion】The psychological resilience of out-patient nurses is at a low level. Positive coping style, social support and educational level are the important factors affecting the psychological resilience of out-patient nurses.

393 Effect of Spontaneous Respiratory Thoracic Anesthesia and Double Lumen Tracheal Catheter Anesthesia on Persistent Cough after Pulmonary Resection in patients with Non-small Lung Cancer

【Objective】To investigate the effects of spontaneous respiratory thoracic anesthesia and double lumen tracheal (DLT) catheter anesthesia on persistent cough after pulmonary resection (CAP) in patients with non-small cell lung cancer (NSCLC) . 【Methods】A total of 482 patients with NSCLC who underwent surgery from February 2012 to May 2017 were enrolled. Patients were divided into the double-lumen tracheal catheter anesthesia group (T group, n=255) and spontaneous respiratory thoracic anesthesia group (S group, n=227) according to the anesthesia method. The S group was further divided into two sub-groups: intravenous intercostal nerve block anesthesia group (SB group, n=93) and intravenous epidural anesthesia group (SE group, n=134). The cough probabilities of the patient on the first day (T1), the second day (T2), the third day (T3), the first month (T4), and the third month (T5) after thoracoscopic surgery were recorded. Leicester cough questionnaire (LCQ) survey results, visual analogue score (VAS) and cough symptom scores were measured as well. Logistic regression was used to analyze the independent influencing factors of persistent cough after pulmonary resection (CAP) in patients. 【Results】After operation, the incidence of cough in the three groups decreased with time (P<0.05). The incidence of cough in the SE group and the SB group was significantly lower than that in the T group (P<0.05). There was no significant difference in the incidence of cough between the SE group and the SB group (P>0.05). After operation, the cough symptom score and VAS score in the T group were higher than those in the SE group and the SB group (P<0.05). The LCQ scores of patients in the T group were significantly lower than those in the SE group and the SB group; and the difference was statistically significant (P<0.05). However, there were no significant differences in cough symptoms scores, VAS scores, and LCQ scores between the SE group and the SB group (P>0.05). Multivariate analysis showed that age, smoking history, lymph nodes around the trachea, anesthesia time, and anesthesia mode were the risk factors for postoperative CAP. 【Conclusion】There is a certain correlation between anesthesia of lung surgery and postoperative CAP. Spontaneous respiratory anesthesia can significantly reduce the incidence of postoperative CAP, improve postoperative recovery and postoperative quality of life. Since age, smoking history, lymph nodes around the tracheal tree, anesthesia time and anesthesia mode are risk factors for postoperative CAP, special attentions should be paid.

397 Effectiveness and Safety of Single-port Laparoscopy Combined with Appendix Suspension in the Treatment of Appendicitis in Children

【Objective】To investigate the efficacy and safety of single-port laparoscopy combined with appendix suspension in the treatment of appendicitis in children. 【Methods】A total of 230 children with appendicitis admitted to our hospital from December 2016 to August 2018 were divided into observation group (n=115) and control group (n=115). The observation group was treated with single-port laparoscopy combined with appendix suspension, while the control group was treated with routine laparoscopic surgery. The general conditions of operation (including incision length, operation time, intraoperative bleeding volume.) and the occurrence of complications were compared between the two groups. Visual analogue scale (VAS) was used at 12, 24, 36 and 72 hours after operation to evaluate the pain degree of the two groups. The levels of serum inflammatory stress factors (IL-8, TNF-α) were measured before operation and 1 and 7 days after operation. 【Results】The general conditions of operation in the observation group were significantly better than those in the control group (except operation time, incision length, intraoperative bleeding volume, postoperative exhaust time, hospitalization time, and the use time of analgesics after operation) (P<0.05). The VAS scores of pain in the observation group at 12, 24, 36 and 72 hours after operation were significantly lower than those in the control group at the same time point (P<0.05). The serum levels of IL-8 and TNF-alpha were significantly higher in the two groups at 1 day after operation than those before operation (P<0.05), and gradually decreased at 7 days after operation. The serum levels of IL-8 and TNF-alpha in the observation group were significantly lower than those in the control group at 1, 7 days after operation (P<0.05). There were no short-term and long-term complications such as incision infection, subcutaneous emphysema, residual abdominal infection, adhesive intestinal obstruction and umbilical hernia in both groups. 【Conclusion】Single-port laparoscopy combined with appendix suspension in the treatment of appendicitis in children is safe and feasible. It can significantly alleviate inflammation stress reaction in children. It has advantages of less trauma, faster recovery and fewer complications after operation, and has more aesthetic value than traditional laparoscopic appendectomy.

400 Effect of ERAT on the Level of PCT and sICAM-1 in Patients with Appendicitis and its Safety Analysis

【Objective】To analyze the effect and safety of endoscopic retrograde appendicitis therapy (ERAT) on the levels of procalcitonin (PCT) and soluble intercellular adhesion molecule-1 (sICAM-1) in patients with appendicitis. 【Methods】The clinical data of 128 patients with appendicitis admitted to our hospital from October 2016 to March 2018 were analyzed retrospectively. According to the different operation methods, the patients were divided into control group (treated with laparoscopic appendectomy) and observation group (treated with ERAT operation), 64 cases in each group. The changes of PCT, sICAM-1, lipoxygenase A4 (LXA4) and soluble P-selectin (sP-s) in the two groups were observed, and the operation time, bed time, time of leukocyte recovery, hospitalization time and postoperative complications were compared.【Results】 The operation time, bed time, WBC recovery time and hospitalization time in the observation group were shorter than those in the control group, and the hospitalization cost was less than that in the control group, with a statistically significant difference (P<0.05). Before operation, there was no significant difference in PCT, sICAM-1, LXA4 and sP-s between the two groups (P>0.05). 72 hours after operation, the levels of serum PCT, sICAM-1, LXA4 and sP-s in the observation group were lower than those in the control group, the difference was statistically significant (P<0.05).There was 1 case of gastrointestinal hemorrhage in the observation group, 1 case of gastrointestinal hemorrhage, 1 case of abdominal abscess and 1 case of intestinal obstruction in the control group. The incidence of complications in the observation group was 1.56% (1/64) lower than that in the control group (4.69% (3/64). The difference was not statistically significant (χ2=1.032,P=0.310>0.05).【Conclusion】 ERAT treatment of appendicitis has the advantages of less trauma, less inflammatory reaction, quick postoperative recovery, less complications and high safety, which is worthy of clinical application.

403 Effect of Single-hole Laparoscopic Surgery on Ovarian Cysts

【Objective】To explore effect and safety of laparoscopic single-hole surgery (LESS), on ovarian cystectomy. 【Methods】A total of 150 patients who underwent laparoscopic ovarian cystectomy were enrolled in our hospital. Patients were randomly divided into the single-hole group (using LESS surgery) and the three-hole group (routine three-hole laparoscopic surgery), with 75 cases in each group. The intraoperative and postoperative indexes (operation time, bleeding volume, time of first anal exhaust, hospitalization time and surgical complications) of the two groups, the changes of reproductive hormones and ovarian reserve follicles before and after surgery, and surgical complications were recorded and compared. 【Results】 There were no significant differences in the operation time, the first exhaust time of the anus, and the hospitalization time between two groups (P>0.05). The bleeding volume of the single-hole group was lower than that of the three-hole group (P<0.05). The VAS scores at 2h, 6h, 12h and 24h were statistically significant between the two groups (P<0.05); and the VAS scores of the single-hole group were significantly lower than those of the three-hole group. before and after surgery, There were no statistically significant differences in serum estrogen (E2), follicle stimulating hormone (FSH), luteinizing hormone (LH) and the number of reserve follicles between the two groups before and after surgery (P>0.05). 【Conclusion】Laparoscopic single-hole surgery of ovarian cystectomy has the advantage of less trauma, less pain in postoperative patients and no significant level changes of sex hormones.

406 Expression and Significance of Livin and PTEN Proteins in EMs

【Objective】To in vestigate the expression and significance of Livin protein and PTEN protein in endometriosis (EMs) tissues. 【Methods】From January 2016 to may 2018, 80 cases of postoperative EMs endometrial tissue (EMs group) and 80 cases of normal endometrial tissue obtained from hysteromyoma operation were selected as the control group. Livin and PTEN protein levels in the two groups were detected and compared by immunohistochemistry, and their clinical significance was analyzed.【Results】The positive expression rate of PTEN protein in the EMs group was significantly lower than that in the control group (P<0.05) positive expression rate of Livin protein was significantly higher than that in control group (P<0.05); the positive expression rate of Livin protein and PTEN protein in patients with stage Ⅲ~Ⅳ of AFS stage was not significantly different from that of stage Ⅰ~Ⅱ(P>0.05); the positive expression rate of PTEN protein was higher than that of ectopic intima (P<0.05) in EMs patients, and the positive expression rate of Livin protein was lower than that of ectopic intima (P<0.05). 【Conclusion】High expression of Livin protein and low expression of PTEN protein in EMs endometrial tissue may be related to the occurrence of EMs.

409 Characteristics and Diagnosis of Renal Perivascular Epithelioid Cell Tumor

【Objective】To investigate the clinical characteristics and the latest treatment of renal perivascular epithelioid cell tumor. 【Methods】The clinical data of 3 patients with perivascular epithelioid cell tumor in our department from January 2019 to September 2019 were analyzed retrospectively, and the latest diagnosis and treatment of this disease were summarized by referring to relevant literature. 【Results】All the patients were female, with an average age of 51.3 years. Before operation, B-ultrasonography and CT were performed in all the patients. Two cases were diagnosed as renal angiomyolipoma before operation, including one case of bilateral renal angiomyolipoma, one case of tuberous sclerosis to be excluded and one case of renal malignant tumor. All of the 3 patients underwent partial nephrectomy, and all of them were diagnosed as perivascular epithelioma. One of them could not completely exclude certain malignant potential. TSC2 mutation was found by gene detection, with a mutation rate of 28.96%. It was sensitive to targeted drugs of everolimus and sirolimus. The patient requested temporary follow-up, but did not receive drug treatment. There was no obvious recurrence of tumor and enlarged lymph nodes in 1~3 months follow-up. 【Conclusion】It is difficult to diagnose the perivascular epithelioid cell tumor of kidney before operation. The treatment plan is still to resect the tumor completely. For the recurrence or metastasis of the tumor, targeted therapy and immunotherapy are expected to be the effective treatment.

412 Expression and Clinical Significance of PDGF and HMGB1 in Serum of Patients with Endometriosis

【Objective】To investigate the expression of platelet-derived growth factor (PDGF) and high mobility group protein B1 (HMGB1) in the serum of patients with endometriosis (EMs) and its clinical significance.【Methods】From March 2016 to March 2019, 80 EMS patients in our hospital were selected as the observation group and 68 healthy women were included as the control group. The expression of PDGF and HMGB1 in serum of the two groups were detected and compared, and the correlation between PDGF and HMGB1 and clinical stage, lymph node metastasis and deep infiltration of EMS patients were analyzed. The influencing factors of EMs were analyzed by logistic regression.【Results】The levels of PDGF and HMGB1 in the observation group were significantly higher than those in the control group (P<0.05); the serum levels of PDGF and HMGB1 in EMs patients with clinical stage Ⅲ~Ⅳ, lymph node metastasis and deep infiltrating focus were higher than those in patients with clinical stageⅠ~Ⅱ, no lymph node metastasis and no deep infiltrating focus (P<0.05);Logistic regression analysis showed that serum PDGF and HMGB1 were risk factors of EMs (P<0.05); the results of Spearman correlation analysis showed that the expression of PDGF and HMGB1 in serum was positively correlated with stage Ⅲ~Ⅳ, lymph node metastasis and deep infiltrating lesions in EMs patients (all P<0.05).【Conclusion】The serum PDGF and HMGB1 of patients with EMs are abnormally high expression, which are closely related to stage Ⅲ~Ⅳ, lymph node metastasis and deep infiltrative lesions. It can provide basis for disease monitoring and prognosis evaluation, and provide new targets for follow-up treatment.

415 Clinical Effects of Clopidogrel Combined with Aspirin in Patients with Type 2 Diabetes Complicated with Acute Cerebral Infarction

【Objective】To investigate the clinical effect of clopidogrel combined with aspirin in patients of type 2 diabetes mellitus (T2DM) with acute cerebral infarction. 【Methods】Retrospectively, a total of 82 patients with T2DM complicated with acute cerebral infarction from August 2016 to October 2018 in our hospital were divided into the observation group and the control group according to the treatment methods. Patients in the control group received conventional aspirin, while those in the observation group received clopidogrel and aspirin. The clinical effects of the two groups after treatment were analyzed. 【Results】The fasting blood glucose (FPG), postprandial 2h blood glucose (2hPG), glycosylated hemoglobin (HbA1c), vascular endothelial growth factor (VEGF), matrix metalloproteinase-9 (MMP-9), angiotensin-ii (Ang-Ⅱ), mean platelet volume (MPV), large platelet ratio (P-LCR) level, carotid intima-media thickness (IMT), plaque number and plaque area between the two groups were not statistically significant (P>0.05). After treatment, the levels of FPG,2hPG, HbAlc, MMP-9,Ang-II,MPV,P-LCR,IMT,plaque number and area in the observation were lower than those in the control group (P<0.05), while VEGF level in the observation group was higher than that in the control group (P<0.05). Before treatment, there was no significant difference in the scores of neurological impairment (NIHSS) between the two groups (P>0.05), however, the scores of NIHSS in the observation group at 6 and 12 days after treatment were significantly lower than those in the control group (P<0.05). 【Conclusion】Clopidogrel combined with aspirin in the treatment of T2DM with acute cerebral infarction can effectively improve the status of blood glucose and nerve function of patients, which is worthy of clinical application.

419 Influence of Health Education with Feedback Method on Cognition, Behavior and Prognosis of Gestational Diabetes Mellitus

【Objective】To explore the effect of health education with feed-back method on cognition, compliance behavior and prognosis of pregnant women with gestational diabetes mellitus (GDM).【Methods】From January 2016 to December 2017, 106 GDM patients were randomly divided into two groups: the observation group (health education by feedback method) and the control group (routine health education), 53 cases in each group. After 12 weeks of intervention, the level of disease knowledge, compliance behavior, blood glucose index changes and maternal and infant outcomes were compared between the two groups.【Results】After 12 weeks of intervention, the awareness rate of disease knowledge and compliance of health behavior in the observation group were significantly higher than that in the control group (P<0.05). The levels of fasting blood glucose, 2 hours postprandial blood glucose and HbA1c in the observation group were significantly lower than those in the control group (P<0.05). The incidence of pregnancy hypertension, late abortion, premature delivery, postpartum hemorrhage and puerperal infection in the observation group was significantly lower than that in the control group (P<0.05).【Conclusion】Feed-back health education can enhance the cognition level of GDM patients, improve the compliance behavior, improve the effect of blood glucose control, and improve the prognosis of mothers and infants.

422 Clinical Effect of EnCor Operation on Mammary Abscess and its Effect on ICAM-1 Level

【Objective】To investigate the clinical effect of EnCor in the treatment of mammary abscess and the effect on the level of ICAM-1.【Methods】The clinical data of 120 patients with breast abscess admitted to our hospital from January 2017 to June 2018 were analyzed retrospectively. According to the operation methods, they were divided into observation group (EnCor minimally invasive surgery, n=86) and control group (conventional incision and drainage, n=34). The results of clinical treatment, breast cosmetology, serum ICAM-1, TNF - α, IL-1 β and complications were compared between the two groups.【Results】The operation time and incision healing time of the observation group were significantly shorter than those of the control group, the postoperative effective rate was higher than that of the control group, the postoperative recurrence rate and VAS score were significantly lower than that of the control group, the difference was statistically significant (P<0.05). The excellent rate of the observation group was 88.37% (76 / 86) higher than that of the control group (73.53% (25 / 34), the difference was statistically significant (P<0.05). On the 7th day after operation, the serum levels of ICAM-1, TNF -αand IL-1βin the two groups were lower than those before operation, and the difference between the observation group and the control group was statistically significant (P<0.05). The incidence of postoperative complications in the observation group was 3.49% (3 / 86) lower than that in the control group (14.71%) (5 / 34), the difference was statistically significant (χ2=5.3707,P=0.0417). 【Conclusion】EnCor has the advantages of small trauma, short postoperative healing time, high healing rate and good cosmetic effect.

425 Effect of Mobile WeChat Group Transitional Care on Prognosis and Knowledge, Belief and Practice in Older Patients with COPD

【Objective】 To explore the effect of mobile WeChat group transitional care on prognosis and knowledge, belief and practice in older patients with COPD. 【Methods】 A total of 100 older patients with COPD were selected as the observation subjects, they were divided into the control group and the observation group according to the random number table method, 50 cases in each group. The control group was given routine continuity nursing, and the observation group received mobile WeChat continuity nursing intervention on the basis of the control group. Prognosis, self-management, self-efficacy, drug compliance and knowledge, belief and practice between the two groups were compared and analyzed. 【Results】 After intervention, although there was no significant difference in lung function grading between the two groups (P>0.05), the difference in dyspnea grade was statistically significant (P<0.05), and the dyspnea difficulty was reduced in the intervention group; the levels of self-management and total score of the two groups were significantly improved, and the levels and total scores of self-management in the observation group were significantly lower than those in the control group (P<0.05); The self-efficacy level of the two groups increased significantly (P<0.05), and the self-efficacy level of the patients in the observation group increased significantly compared with the control group (P<0.05); After the intervention, the knowledge, belief, behavior and total score of the two groups increased significantly (P<0.05), and the knowledge, belief, behavior and total score of the patients in the observation group were significantly higher than that of the control group (P<0.05); The compliance score of the patients in the observation group was significantly higher than that in the control group (P<0.05), and the degree of drug compliance distribution in the observation group was significantly different from that in the control group (P<0.05). 【Conclusion】 Mobile WeChat group transitional care model can effectively improve the prognosis, enhance the compliance of drug use, improve the level of self-management and self-efficacy, optimize the ability of patients' knowledge, belief and practice, and it is worth promotion.

429 Etiological Characteristics of Infection in Elderly Patients with Cervical Cancer after Radical Resection

【Objective】To analyze the etiological characteristics of infection in elderly patients with cervical cancer after radical resection. 【Methods】A total of 163 elderly patients with cervical cancer who underwent surgical resection in Chengdu University Hospital from March 2014 to October 2018 were selected for the study. The incidence of postoperative infection, pathogenic characteristics and risk factors of postoperative infection were analyzed. 【Results】The incidence of postoperative infection was 28.22% (46/163), among which urinary tract infection (43.48%), surgical incision infection (23.91%) and vagina infection (10.87%) ranked the top three. A total of 57 pathogenic bacteria were detected, which contained 27 gram-negative bacteria (47.39%), 23 gram-positive bacteria (40.35%) and 7 fungi (12.28%). The top two pathogens were Escherichia coli (21.05%) and Staphylococcus aureus (15.79%). The susceptibility of Escherichia coli to imipenem, rifampicin and erythromycin was high (< 10%), while the resistance to sulfamethoxazole, piperacillin, levofloxacin and tetracycline was more than 80%. Staphylococcus aureus was highly sensitive to imipenem without any drug resistance, while its resistance to rifampin, penicillin G and levofloxacin was more than 70% and resistance to ceftriaxone, tetracycline and sulfamethoxazole was more than 40%. Single factor and logistic Univariate and multivariate logistic regression analysis showed that operation time ≥3h, urinary catheter retention time ≥14d, antibiotics usage ≥3 kinds, postoperative chemotherapy and anemia were independent risk factors for postoperative infection (P<0.05). 【Conclusion】The incidence of infection is high in the elderly patients with cervical cancer after radical resection. The main pathogens are Escherichia coli and Staphylococcus aureus showing high resistant to common antibiotics. In practical clinical work, antibiotics should be selected reasonably according to the results of drug resistance analysis, so relevant preventive measures should be taken against the risk factors of postoperative infection.
