Abstract  【Objective】To explore effect and safety of laparoscopic single-hole surgery (LESS), on ovarian cystectomy. 【Methods】A total of 150 patients who underwent laparoscopic ovarian cystectomy were enrolled in our hospital. Patients were randomly divided into the single-hole group (using LESS surgery) and the three-hole group (routine three-hole laparoscopic surgery), with 75 cases in each group. The intraoperative and postoperative indexes (operation time, bleeding volume, time of first anal exhaust, hospitalization time and surgical complications) of the two groups, the changes of reproductive hormones and ovarian reserve follicles before and after surgery, and surgical complications were recorded and compared. 【Results】 There were no significant differences in the operation time, the first exhaust time of the anus, and the hospitalization time between two groups (P>0.05). The bleeding volume of the single-hole group was lower than that of the three-hole group (P<0.05). The VAS scores at 2h, 6h, 12h and 24h were statistically significant between the two groups (P<0.05); and the VAS scores of the single-hole group were significantly lower than those of the three-hole group. before and after surgery, There were no statistically significant differences in serum estrogen (E2), follicle stimulating hormone (FSH), luteinizing hormone (LH) and the number of reserve follicles between the two groups before and after surgery (P>0.05). 【Conclusion】Laparoscopic single-hole surgery of ovarian cystectomy has the advantage of less trauma, less pain in postoperative patients and no significant level changes of sex hormones.
