164 Expression and Significance of Girdin and BCL-2 Protein in Glioma Tissue

【Objective】To investigate the expression and significance of Girdin and B-lycytoma-2(BCL-2) proteins in glioma tissues.【Methods】From March 2010 to January 2013, 53 patients with gliomas diagnosed and treated in our hospital were selected. The expression of Girdin and BCL-2 protein in gliomas and normal tissues adjacent to gliomas were detected by immunohistochemistry. The relationship between the expression level and the clinicopathological characteristics of patients and the total survival period was analyzed.【Results】The positive rates of Girdin and BCL-2 protein in gliomas were 60.38% (32 / 53) and 41.51% (22 / 53), which were significantly higher than those in normal tissues 5.66% (3 / 53) and 1.89% (1 / 53). The differences were statistically significant (P<0.05). The expression level of Girdin protein in gliomas was related to pathological grade, peritumoral edema and gross diameter of tumor (P<0.05); the expression level of Bcl-2 protein in gliomas was related to gender (P<0.05). The survival rate of patients with high expression of Girdin protein was significantly lower than that of patients with low expression (P<0.05), and there was no significant difference between patients with high expression of BCL-2 protein and patients with low expression of BCL-2 protein (P>0.05).【Conclusion】Girdin and BCL-2 protein are highly expressed in gliomas. Girdin protein is involved in the development of gliomas and has a negative impact on their prognosis.

167 Prenatal Ultrasound Imaging Features and Prognosis of Hepatic Hemangioma

【Objective】To investigate the prenatal sonographic characteristics and prognosis of the hepatic hemangiomas.【Methods】We retrospectively reviewed the sonographic images of 15 patients with hepatic hemangiomas from January 2014 to October 2018 during our hospital. Prenatal liver hemangiomas ultrasound imaging features and prognosis were analyzed.【Results】Of the 15 cases, 13 were diagnosed as hepatic hemangioma by prenatal ultrasound, and 2 were misdiagnosed as hepatoblastoma (1 case was induced and 1 case was delivered by live birth). The diagnostic accuracy of prenatal ultrasound was 86.7% (13/15), and the misdiagnosis rate was 13.3% (2/15). The echogenicity of hemangioma included hyperechoic(2/15), isoechoic(1/15), hypoechoic(5/15), mixed echo (7/15).The 13 live births were confirmed to have hepatic hemangioma by color Doppler ultrasound or enhanced CT scan or MRI; while 2 induced fetuses were confirmed hepatic hemangiomas by autopsy and pathology on the premise of informed consent of pregnant women. All 13 live-born infants were treated conservatively and performed color Doppler ultrasound during treatment with good prognosis for 6~36 months follow-up.【Conclusion】 Fetal hepatic hemangioma has certain characteristics such as boundary, internal echo, and blood flow signals, however, it lacks specificity. Prenatal ultrasound can provide some values in the diagnosis of fetal hepatic hemangioma. Children with hepatic hemangioma have a good prognosis after conservative treatment.

171 Study on Clinical Characteristics and Influencing Factors of Bipolar Disorder Comorbid with Alcohol Use Disorder

【Objective】To investigate the clinical characteristics and influencing factors of bipolar disorder comorbid with alcohol use disorder.【Methods】Patients with bipolar disorder who were hospitalized at the Hebei Mental Health Center were divided into comorbid groups (n=51) and non-comorbid groups (n=97) according to the status of comorbid alcohol use disorders. The diagnostic criteria were based on the Diagnostic and Statistical Manual of Mental Disorders in the United States (the fifth revised edition, DSM-5). At the same time, general situation questionnaire and response questionnaire were used to survey the subjects and analyze the relevant factors.【Results】There were differences between the comorbid group and the non-comorbid group in terms of gender, education level, family relationship, occupation, pre-disease personality, family history of alcoholism, and the onset of disease (P< 0.05). In various risk assessments, impulsive behavior and self-injury behavior of the comorbid group were higher than those of the non-comorbid group, and the difference was statistically significant (P<0.05). The score of problem-solving approach style in the comorbid group was lower than that in the non-comorbid group(P<0.05), while the self-blame coping style of the comorbid group was higher than that of the non-comorbid group(P<0.05). Unconditional multivariate Logistic regression analysis showed that male, impulsive behavior, acute and subacute onset were risk factors for comorbid alcohol use disorder(P<0.05), while elementary education and below, high school / secondary education, workers, farmers, and introverted personality before illness were protective factors for comorbid alcohol use disorder (P<0.05).【Conclusion】The proportion of comorbid alcohol use disorder in hospitalized patients with bipolar disorder is not low at all. The comorbid patients are less likely to adopt problem-solving approaches, instead they have a higher risk of impulsive behavior.

174 A Clinical Study on Abscopal Effect after Radiotherapy in Metastatic Non-small Cell Lung Cancer Patients

【Objective】To investigate the abscopal effects and associated clinical factors after local radiotherapy (RT) in patients with metastatic non-small cell lung cancer (NSCLC).【Methods】All eligible patients were treated with radiotherapy on primary or metastatic lesions concurrent with GM-CSF (granulocyte macrophage colony-stimulating factor). The tumor response, adverse events and progression-free survival (PFS) were recorded after follow-up. SPSS 18.0 software was used for statistical analysis.【Results】A total of 39 patients were enrolled in this study, in which abscopal effects after RT occurred in 19 cases, accounting for 48.7%. Of 15 patients who underwent stereotactic radiotherapy (SBRT), 10 had abscopal effects (10/15, 66.7%) after receiving RT on liver metastases. Univariate analysis showed that pathological type, RT target, RT mode, BED10 (radiation-equivalent biological dose) and single dose were significantly correlated with abscopal effects after RT. Multivariate analysis showed that RT on liver lesion, SBRT and single dose of radiatoin ≥ 8Gy were more likely to have abscopal effects. Generally, the median PFS was 10.43 (3.97~16.89) months and the 1-year PFS rate was 43.5% with 95% CI. Among them, the median (95% CI) PFS of patients with and without abscopal effects after radiotherapy were 21.33 (10.79 to 31.9) months and 7.97 (7.07 to 8.87) months, respectively. There was statistically significant (P<0.05). The incidence of adverse events for all levels was 33.3% for all the patients in this study.【Conclusion】SBRT concurrent with GM-CSF is a better treatment for metastatic NSCLC. A single large dose of radiation therapy and radiation therapy for liver lesions can stimulate more immune effects and more easily produce abscopal effects, thereby prolonging the survival time of patients.

179 Effect of Etomidate Emulsion on Anesthesia Recovery and changes of MAP and HR in Patients undergoing Laparoscopic Hysterectomy

【Objective】To investigate the effects of etomidate emulsion on anesthesia recovery and changes of mean arterial pressure (MAP) and heart rate (HR) in patients undergoing laparoscopic uterine surgery.【Methods】A total of 120 patients undergoing laparoscopic hysterectomy were divided into two groups according to anesthesia methods. Patients in the control group received propofol combined with remifentanil by intravenous anesthesia, while patients in the observation group received etomidate emulsion combined with remifentanil by intravenous anesthesia. The anesthesia and operation procedure of the two groups were compared.【Results】MAP, HR level, PI (pulsatility index) and MCA-PSV (peak systolic velocity of middle cerebral artery) showed no significant differences between the two groups before anesthesia (T0), immediately after endotracheal intubation (T1),10 min after no pneumoperitoneum in supine position (T3) (P >0.05). The MAP level at T2 (low head position with pneumoperitoneum) in the observation group was lower than that in the control group, however, the HR, MCA-PSV and PI in the observation group were not significant from the control group (P>0.05). Eye opening time, tracheal extubation time and disorientation timer in observation group was shorter than those in the control group; and the differences between the two groups were statistically significant (P<0.05). The incidence of nausea and vomiting in the observation group was 1.67% (1/60) and the incidence of muscle fibrillation was 1.67% (1/60), which were not significant difference with those in the control group (P> 0.05). The incidence of injection pain in the observation group was 0.00% (0/60), which was lower than that in the control group (10.00%, 6/60) (P<0.05).【Conclusion】The application of etomidate emulsion in laparoscopic hysterectomy can promote the recovery of anesthesia. It has little effect on the cerebral blood flow, MAP and HR.

182 Comparison of Sun's Tip-flexible Ureterorenoscopy and Minimally Invasive Percutaneous Nephrolithotomy in the Treatment for 2-3cm Lower-pole Stones in the Elderly

【Objective】 To compare the therapeutic effect of Sun's tip-flexible ureterorenoscopy and minimally invasive percutaneous nephrolithotomy for the treatment of 2-3cm renal inferior calyx stone in the elderly.【Methods】From June 2016 to July 2018, 80 interns were randomly divided into Sun's tip-flexible ureterorenoscopy group (40 cases) and minimally invasive percutaneous nephrolithotomy group (40 cases). They were treated by retrograde intrarenal surgery and minimally invasive percutaneous nephrolithotomy. The one-time success rate and stone clearance rate, operative time, intraoperative blood loss, postoperative hospitalization days, postoperative complications and trauma indicators were compared.【Results】There was no significant difference between the two groups (92.5% vs 95.0%,χ2=0.213, P>0.05). The operation time of the Sun's tip-flexible ureterorenoscopy group was longer than that of the mPCNL group, and the hospitalization time and intraoperative blood volume were lower than that of the mPCNL group (P<0.05). The serum CRP, TNF-α and IL-6 levels in the two groups were higher than those in the immediate and 72 hours postoperatively, and the serum CRP, TNF-α and IL-6 levels in the mPCNL group were higher than those in the Sun's tip-flexible ureterorenoscopy group (P<0.05). The incidence of postoperative complications in the Sun's tip-flexible ureterorenoscopy group was 5% (2 / 40) lower than that in the mPCNL group 25.0% (10 / 40), the difference was statistically significant (P<0.05). 【Conclusion】Sun's tip-flexible ureterorenoscopy is better in the treatment of 2~3cm lower calyceal calculi in the elderly, which is similar to that of mPCNL. At the same time, it has the advantages of quick recovery, less complications, small wound and less bleeding.

185 Effects of Different End-tidal Carbon Dioxide Partial Pressure during Anaesthesia on Cerebral Blood Perfusion and Cognitive Function in Patients undergoing Laparoscopic Inguinal Hernia Repair

【Objective】To investigate the effects of different end-tidal carbon dioxide partial pressure (PETCO2) during anaesthesia on cerebral blood perfusion and cognitive function in patients undergoing laparoscopic inguinal hernia repair (LIHR).【Methods】A total of 98 patients with LIHR under general anesthesia were randomly divided into normal PETCO2 group (L group) and mild-high PETCO2 group (H Group), with 49 cases in each group. The PETCO2 was controlled at 35-39 mmHg and 40~45 mmHg in the group L and the group H, respectively. The effects of PETCO2 through regulating tidal volume (VT) and respiratory rate (RR) on cerebral blood perfusion and postoperative cognitive function were analyzed. 【Results】 The VT, RR, Ppeak (peak airway pressure) and PETCO2 of T1(5min), T2(15min), T3(30min), T4(60min) and T5(60min) after pneumoperitoneum in the group L were higher than T0 (5min before pneumoperitoneum) in the group L; Also, they were higher than those at the same time point of the group H (P<0.05). The Da-jvO2 (arterial-venous blood oxygen content difference ) and CERO2 (cerebral oxygen uptake rate ) at T1, T2, T3,T4,T5 in the group L were higher than those at T0 of the group L and at T3, T4 and T5 of the group H (P<0.05). Ppeak, PETCO2 ,Da-jvO2 and CERO2 at T1, T2, T3,T4,T5 and RR at T2, T3, T4, T5 were higher than those at T0 in the group H (P<0.05). The Vm (average flow rate),Vs (peak cerebral systolic flow rate) and Vd (end-diastolic flow rate ) of the group H (45 mmHg for PETCO2) were higher than those before anesthesia and the group L (35 mmHg for PETCO2) (P<0.05). The PI (pulsatility index) and RI (resistance index) of the group H were lower than before the anesthesia; and they were also lower than those of the group L (P<0.05). Compared to 1 day before operation, the number span (inverse), number symbol and MMSE in both groups at 3 days after operation decreased and the trace line increased. In group L, the number span (sequential), vocabulary fluency and word recognition decreased (P<0.05). At 3 days after operation, the word recognition and MMSE of group L were lower than that those of group H (P<0.05).【Conclusion】 In the general anesthesia of laparoscopic inguinal hernia repair, increasing PETCO2 can properly improve cerebral blood perfusion and effectively reduce the incidence of postoperative cognitive impairment.

189 Correlation between Liver Fat Content and Metabolic Indexes in Patients with Non-alcoholic Fatty Liver Complicated with Type 2 Diabetes

【Objective】To evaluate the application of quantitative measurement of liver fat content (LFC) in patients with nonalcoholic fatty liver disease(NAFLD)complicated with type 2 diabetes mellitus(T2DM)by ultrasound, and to explore the correlation between quantitative results and metabolic indexes.【Methods】A total of 105 patients with NAFLD complicated with T2DM were selected as the observation group, and 43 T2DM patients without fatty liver disease were enrolled as the control group. The FLC was estimated by computer assisted ultrasound. Fully automatic biochemical analyzer detected various metabolic indexes such as serum uric acid (SUA), glutamic acid transaminase (ALT), aspartate aminotransferase (AST), total cholesterol (TC), triacylglycerol (TG), high-density lipoprotein cholesterol (HDL-C), Low-density lipoprotein cholesterol (LDL-C), blood urea nitrogen (BUN), free fatty acids (FFA), glycated hemoglobin (HbA1C), fasting blood glucose (FPG), fasting insulin (FINS), urine microalbumin (ALB ) and Urine creatinine (CREA). Insulin resistance index was calculated (HOMA-IR = FPG × FINS / 22.5). The differences in metabolic indexes between the two groups were compared and the correlation between LFC and each index was analyzed.【Results】The levels of ALT, TG, LDL-C, FPG, CREA, HOMA-IR, and LFC in the observation group were higher than those in the control group, while the level of HDL-C was lower than that in the control group; the differences were statistically significant (P<0.05). Correlation analysis showed that LFC was positively correlated with ALT and TG in the observation group; and LFC was positively correlated with ALT, AST, and TG in the control group (P<0.05).【Conclusion】 As a common complication of T2DM, NAFLD is used to quantitatively determine LFC by ultrasound. It is simple, low-cost, non-invasive and convenient, and can reflect changes in related indexes such as liver function and lipids. It can be used as a routine screening method for admission and provide a basis for early detection of liver fatty changes.

193 Expression and Clinical Significance ofα1-antitrypsin in Serum of Patients with Stable Chronic Obstructive Pulmonary Disease Complicated with Bronchiectasis

【Objective】To investigate the level and significance of serum α1-antitrypsin (α1-AT) in patients with chronic obstructive pulmonary disease (COPD) complicated with bronchiectasis.【Methods】From January 2017 to December 2018, 64 stable COPD patients with bronchiectasis (combined group) were selected from the respiratory department of our hospital. In addition, 50 stable COPD patients who were treated in our hospital at the same time were selected as the COPD group and 50 healthy people in our hospital as the control group. The expression level ofα1-AT in serum of three groups was detected. The rs1243166 single nucleotide polymorphism of α 1-AT gene was detected by sequenom method and compared.【Results】There was no significant difference in age, gender, smoking history, drinking history, systolic blood pressure and diastolic blood pressure among the three groups (P>0.05). The levels ofα1-AT in the control group, COPD group and combined group were (1.79±0.56) mg / L, (1.45±0.45) mg / L, (0.81±0.31) mg / L, respectively, which decreased in turn, and there was a significant difference between the two groups (P<0.05). ROC curve showed that the best critical point ofα1-AT level diagnosis of COPD combined with bronchiectasis was 3.21 mg / L. at this time, the sensitivity ofα1-AT level diagnosis of COPD combined with bronchiectasis was 71.5%, the specificity was 79.4%; the area under AUC curve was 0.79. There was significant difference in rs1243166 single nucleotide polymorphism among the three groups (P<0.05). The frequencies of GG and GA genotype in the combined group were significantly higher than those in the COPD group and the control group. Compared with AA, the risk of COPD with bronchiectasis in GG and GA was 1.154 and 1.697.【Conclusion】 Alpha 1-AT level is more sensitive and specific in diagnosing COPD combined with bronchiectasis, and incidence rate ofα1-AT gene rs1243166 GG type and GA type COPD combined with bronchiectasis is high.

196 Expression and Significance of GAS5 in Serum and Placenta of Preeclampsia Patients

【Objective】To investigate the expression and significance of growth arrest specific protein 5 (gas5) in serum and placenta of preeclampsia patients.【Methods】From January 2015 to December 2017, 43 preeclampsia patients (observation group) and 43 normal pregnant women (control group) were selected. Serum samples were collected before delivery and placenta samples were collected after delivery. Enzyme linked immunosorbent assay was used to detect the level of serum GAS5, real-time PCR was used to detect the level of GAS5 mRNA, SPSS Pearson correlation software was used to analyze the correlation between the incidence of preeclampsia and the level of serum GAS5 and placental GAS5.【Results】The levels of serum GAS5 and placental GAS5 mRNA were 0.185±0.121 and 0.12±0.05 in the observation group, respectively, which were significantly lower than those in the control group (0.332±0.036 and 0.452±0.019, P<0.05). The correlation analysis showed that the levels of serum GAS5 and placental GAS5 mRNA were negatively correlated with the incidence of preeclampsia (r=-0.693,P=0.038;r=-0.712,P=0.027).【Conclusion】 Serum GAS5 and placental GAS5 mRNA in preeclampsia patients show low expression, its expression is negatively related to the pathogenesis of preeclampsia, and may be involved in the occurrence and development of preeclampsia.

199 Effect of Magnesium Isoglycate Combined with Ademetionine Succinate on Liver Function and Prognosis after Interventional Therapy for Primary Liver Cancer

【Objective】To explore the effect of magnesium isoglycate combined with ademetionine succinate on liver function and prognosis after interventional therapy for primary liver cancer.【Methods】One hundred and eighty patients with primary liver cancer who were treated in our hospital from August 2014 to may 2017 were selected as the study objects. According to the different schemes of liver protection after TACE, it was divided into two groups: group A (reduced glutathione + magnesium isoglycate injection) and group B (magnesium isoglycate + adenosylmethionine succinate). Serum ALT, AST, DBIL and TBIL levels were compared between the two groups. The adverse reactions of the two groups were observed to analyze the prognostic factors of patients with primary liver cancer.【Results】After treatment, the levels of ALT, AST, DBIL and TBIL in the two groups were significantly higher than those before treatment (P<0.05). The levels of ALT, AST, DBIL and TBIL in the group B were lower than those in the group A (P<0.05). The incidence of gastrointestinal reaction, leucopenia, hemoglobin reduction and thrombocytopenia in the group A were 26.44%, 25.29%, 20.69% and 2.30% respectively; the incidence of gastrointestinal reaction, leucopenia, hemoglobin reduction and thrombocytopenia in the group B were 24.73%, 20.43%, 16.13% and 2.15%, respectively. There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). The mean survival time of the group A was 27.40 (95% CI: 25.22~29.59) months, and that of the group B was 30.27 (95% CI: 28.40~32.14) months.There was significant difference between the two groups in the 3a survival time (P<0.05).Cox single factor and multi factor analysis showed that tumor number, vascular invasion and liver protection were closely related to the prognosis of patients with primary liver cancer.【Conclusion】Magnesium isoglycate combined with ademetionine succinate can improve the liver function and prognosis of patients with primary liver cancer after TACE treatment, which is worthy of clinical promotion.

203 Prognostic Value of Procalcitonin, BNP, Cardiac Troponin I and D-dimer in Patients with Chronic Heart Failure

【Objective】To investigate the prognostic value of procalcitonin (PCT), B-type natriuretic peptide (BNP), cardiac troponin I (cTnI), and D-dimer (D-D) in patients with chronic heart failure (CHF).【Methods】A total of 70 patients with CHF patients in our hospital of year 2016 were selected in the CHF group. Patients in the CHF group was further divided into three groups according to New York Heart Association's cardiac function classification criteria (NYHA): cardiac function Ⅱ group (n=33), Ⅲ group (n=26) and Ⅳ group (n=11). At the same time period, 70 healthy volunteers were enrolled as the control group. Serum PCT, BNP, cTnI, and D-dimer levels were measured at the admission day and one month later. Then the statistical analysis was performed as well.【Results】Serum levels of PCT, BNP, cTnI, and D-D in the CHF group were higher than those in the control group (P<0.05). The levels of serum PCT, BNP, cTnI, and DD in the cardiac function level Ⅱ, Ⅲ and Ⅳ in the CHF group gradually elevated with the increase of the degree of heart failure. The differences among the three groups were statistically significant(P<0.05). There was a decreasing trend in serum levels of PCT, BNP, cTnI, and D-D after 1 month of treatemnt (P<0.01). Left ventricular ejection fraction (LVEF) of CHF patients after one treatment was higher than that before treatment (P<0.01). NYHA cardiac function level of the CHF patients after 1 month treatment was significantly lower than before treatment; and the difference was statistically significant (P<0.01). Serum levels of PCT, BNP, cTnI, and D-D were positively correlated with NYHA cardiac function level (P<0.05) and negatively correlated with LVEF (P<0.05).【Conclusion】The levels of PCT, BNP, cTnI, and D-D in serum reflect severity of the CHF patients, which can provide important predictive value for disease monitoring.

206 Effect of Runx2 Overexpression Lentivirus on Proliferation and Osteogenesis of Human Periodontal Ligament Cells in Vitro

【Objective】To investigate the effect of Lentiviral-mediated Runx2 on proliferation and osteogenic potential of human periodontal ligament cells (PDLCs) in vitro.【Methods】PDLCs were cultured in vitro. The logarithmic cells were divided into blank control group (BC group, without any treatment), negative control group (NC group, infected cells without Runx2 gene with MOI = 50) and Runx2 group (transfected cells with Runx2 overexpression of MOI = 50). After 48 hours of transfection, the expression of GFP was observed under fluorescence microscope, and the transfection efficiency was calculated. MTT was used to detect cell proliferation in vitro, and real-time fluorescent quantitative PCR and Western blot were used to detect the expression of Runx2, BMP-2, ALP and OPN.【Results】After 48 hours of transfection, there was no green fluorescent protein in BC group, and a large number of cells in Runx2 group and NC group expressed green fluorescent protein. The transfection efficiency of Runx2 group and NC group was (88.57±3.07)% and (89.43±2.09)% respectively, there was no significant difference between the two groups (P>0.05). The odvalue of the number of living cells in Runx2 group was significantly higher than that in NC group and BC group at different time points, and the difference was statistically significant (mean P<0.05); the odvalue of the number of living cells in NC group and BC group was not statistically significant (P>0.05); the mRNA and protein expression of Runx2, BMP-2, ALP and OPN in Runx2 group were significantly higher than that in NC group and BC group, and the difference was statistically significant (all P<0.05); while in NC group There was no significant difference in the mRNA expression of Runx2, BMP-2, ALP and OPN between the two groups (P>0.05).【Conclusion】Runx2 overexpression of lentivirus can promote the proliferation of PDLCs in vitro and enhance their osteogenic ability, which has an important application prospect in periodontal tissue repair and regeneration.

【Objective】To investigate the effect of intravitreal injection of Conbercept combined with retinal laser photocoagulation on the long-term vision and prognosis of patients with macular edema secondary to branch retinal vein occlusion (BRVO).【Methods】Retrospective analysis of 45 cases (45 eyes) of macular edema secondary to ischemia-type BRVO treated in our hospital from October 2016 to June 2017 was performed, all patients were treated with Conbercept vitreous injection combined with retinal laser photocoagulation; and patients' macular center retinal thickness (CMT), best corrected visual acuity (BCVA) changes were compared1 week,1 month,3 months,6 months,9 months and 12 months pre-treatment and post-treatment , and patient complications and adverse reactions were recorded .【Results】BCVA was 0.44±0.07, 0.45±0.05, 0.52±0.07, 0.54±0.06, 0.62±0.05, 0.90±0.10 at 1 week,3 months,6 months,9 months and 12 months after treatment, respectively, which was significantly higher than 0.39±0.07 before treatment. CMT was (295.14±63.51) mm,(275.39±60.14) mm,(249.63±61.50) mm,(238.75±62.55) mm,(232.64±64.71) mm,(210.08±60.09) mm, respectively, significantly lower than (679.24±62.31) mm before treatment (P<0.05). There was no significant difference in CMT at different time points after treatment (P>0.05). There were no ocular complications such as iatrogenic cataract, high intraocular pressure, retinal tear, vitreous hemorrhage and endophthalmitis.【Conclusion】Conbercept combined with retinal laser photocoagulation can significantly improve BCVA and reduce CMT in patients with macular edema secondary to BRVO, and it has high safety, which is worthy of clinical application.

213 Effect of Transvaginal Sacrospinous Ligament Suspension on Pelvic Organ Prolapse

【Objective】To investigate the clinical effect of transvaginal sacrospinous ligament suspension and pelvic funnel ligament suspension in the treatment of pelvic organ prolapse.【Methods】A total of 47 patients with pelvic organ prolapse (POP) treated in our hospital from January 2016 to November 2018 were selected and divided into the observation group (n=27) and the control group (n=20) according to the selected surgical scheme. Patients in the observation group were treated with vaginal sacrospinous ligament suspension, while patients in the control group were treated with pelvic funnel ligament suspension. The operation time and bleeding volume were recorded. All patients underwent POP-Q measurement at 3 months after operation. In the measurement, Ba referred to the anterior wall of the vagina at a distance of 3 cm from the edge of the hymen, C referred to the stump of the vagina and Bp referred to the reflex of the vaginal fornix and the stump of the vagina to the midline of the vaginal hymen. The pelvic floor dysfunction questionnaire-20 (PFDI-20) and the pelvic organ prolapse/urinary incontinence sexual questionnaire-12 (PISQ-12) were used to evaluate the quality of life and sexual life of patients.【Results】The operation time, intraoperative bleeding volume and indwelling time of urinary catheter in the observation group were (64.02±13.22) min, (87.20±12.23) ml and (87.92 ±12.21) h, irrespectively, which were significantly shorter than those in the control group (P<0.05). Ba, C, and Bp in the observation group and the control group were improved at 3 months after surgery (P<0.05), however, there was no significant difference in Ba, C, and Bp at 3 months after surgery in the two groups (P> 0.05). One year after operation, the PDFI-20 score of the observation group was (20.03±2.19), which was significantly lower than that of the control group (P<0.05); while the PISQ-12 score was (43.30±5.10), which was significantly higher than that of the control group (P<0.05). There was no significant difference in the incidence of complications between the observation group and the control group (P>0.05); No recurrence occurred during the follow-up period in the observation group and the control group.【Conclusion】Transvaginal sacrospinous ligament suspension has significant curative effect on pelvic organ prolapse. It can reduce the operation time and the amount of intraoperative bleeding, and improve the quality of patients' lives and sexual life, which is worthy of clinical promotion and application.

216 Analysis of Effects of Myocardial Dilatation and Restrictive Changes on Cardiac Structure and Function in Patients with Hypertrophic Cardiomyopathy and Heart Failure

【Objective】To investigate the effects of dilatation and restrictive changes of myocardiumon on cardiac structure and functional characteristics in patients with hypertrophic cardiomyopathy and heart failure.【Methods】A total of 82 patients with hypertrophic cardiomyopathy who were admitted to our hospital from February 2013 to February 2018 were enrolled in this study. All patients underwent thoracic echocardiography and magnetic resonance imaging. According to the results of ultrasound examination, the patients were divided into the dilated group (n=56) and the restricted group (n=26). The left atrial anteroposterior diameter (LAD), left ventricular ejection fraction (LVEF), maximum left ventricular wall thickness (LVWT), left ventricular end diastolic diameter (LVEDD), left atrial enlargement, ventricular wall hypertrophy and delayed ventricular wall enhancement were recorded and analyzed.【Results】The average age of the dilated group was younger than that of the restricted group, and the proportion of males was significantly higher than that of the restricted group. The difference was statistically significant (P<0.05).There was no significant difference in heart rate and plasma NT-proBNP levels between the two groups (P>0.05). Left atrial anteroposterior diameter, mitral regurgitation ratio, left atrial enlargement ratio and right ventricular enlargement ratio did not show any significant difference between the two groups (P>0.05). The proportions of LVEF, LVWT, tricuspid regurgitation, left and right atrial enlargement were significantly lower in the dilated group than in the restricted group. The ratios of LVEDD, LVWT<11mm and LVEF <50% in the dilated group were significantly greater than those in the restricted group; the differences were statistically significant (P<0.05). The maximum ventricular wall thickness, interventricular septum, partial left ventricular free wall hypertrophy ratio and ratio of delayed enhancement of ventricular septal or apical ventricular wall in the dilated group were lower than those in the restricted group. The proportions of simple interventricular septum hypertrophy, interventricular septum and left ventricular free wall delayed enhancement in the dilated were higher than those in the restricted group (P<0.05).【Conclusion】 Restrictive and dilated cardiomyopathy has a significant effect on cardiac structure and function in patients with hypertrophic cardiomyopathy complicated with heart failure. The degree of myocardial fibrosis and the hypertrophic site of the ventricular wall are important factors leading to changes in restrictive and dilated cardiomyopathy.

220 Clinical Evaluation of Unilateral Craniotomy for Frontal Lobe Contusion and Laceration Combined with Intracranial Hematoma

【Objective】To explore the clinical effect of unilateral craniotomy in the treatment of frontal lobe contusion and laceration combined with intracranial hematoma.【Methods】From June 2015 to June 2017, 110 patients with frontal lobe contusion and laceration combined with intracranial hematoma in our hospital were divided into unilateral craniotomy (observation group,n=60) and bilateral craniotomy (control group, n=50). The operation time, blood transfusion volume, hospitalization time, Glasgow Coma score (GCS) and Glasgow pre-treatment were compared between the two groups The changes of serum NSE, S100 β protein and lactate were observed before and after operation. 【Results】 The operation time, blood transfusion volume and hospitalization time in the observation group were significantly lower than those in the control group (P<0.05). One week after operation, the GCS score of the observation group was significantly higher than that of the control group (P<0.05). Three months after operation, the prognosis of the patients in the observation group was 66.67%, which was significantly higher than that in the control group (46.00%) (P<0.05). There was no significant difference in serum NSE, S100 β protein and blood lactate concentration between the two groups on the first day after operation (P>0.05); the serum NSE, S100 β protein and blood lactate concentration in the observation group were significantly lower than those in the control group on the first week after operation (P<0.05).【Conclusion】Compared with bilateral craniotomy, the patients with frontal lobe contusion combined with intracranial hematoma have less trauma and better postoperative recovery.

223 Effect of Ultrasound Combined with PNS on Anesthesia and Cognitive Function in Elderly Lower Extremity Orthopedic Surgery

【Objective】To explore the application value of ultrasound combined with peripheral nerve stimulator (PNS) in lumbar plexus-sciatic nerve block anesthesia for elderly lower extremity orthopedics.【Methods】A total of 160 elderly patients undergoing lumbar plexus-sciatic nerve block anesthesia orthopedic surgery from our hospital were selected and randomly divided into two groups with 80 cases each. The observation group was anesthetized with lumbar plexus-sciatic nerve block under the guidance of ultrasound combined PNS; the control group was anesthetized with lumbar plexus-sciatic nerve block by PNS guidance. Hemodynamic indexes, MMSE scores and anesthesia-related indexes were compared between the two groups at different time points.【Results】There was no significant difference in the systolic blood pressure (SBP), diastolic blood pressure (DBP), and heart rate (HR) between the two groups of patients before (T0) to 60min (T3) after block (P> 0.05) . The changes in SBP, DBP, and HR monitoring values of patients in the two groups over time showed significant fluctuations compared to the time of T0 in this group, and the differences were statistically significant (P<0.05). There were no significant difference in the MMSE scores between the two groups before anesthesia and at 12h, 24h and 72h after anesthesia (P>0.05). However, compared to the same group before anesthesia, the changes in MMSE scores of the two groups at 12h and 24h after anesthesia showed the pattern that decreased first and then increased (P<0.05). The onset time of sensory nerve and motor nerve block in the observation group was significantly lower than that in the control group (P<0.05). The maintenance time of sensory nerve and motor nerve block in the observation group was significantly longer than that in the control group (P<0.05).【Conclusion】In the elderly patients with lower extremity orthopedic surgery, the use of ultrasound combined with external PNS guidance in lumbar plexus-sciatic nerve block anesthesia is helpful to improve the rapid accuracy of positioning and prolong never block time. It does not show any impact on hemodynamic fluctuation and cognitive function comparing to PNS positioning alone.

226 Expression and Significance of CRP, PCT and ESR in Patients with Incision Infection after Posterior Pedicle System Internal Fixation

【Objective】To investigate the expression and significance of C-reactive protein (CRP), procalcitonin (PCT) and erythrocyte sedimentation rate (ESR) in the serum of patients with incision infection after posterior pedicle system internal fixation.【Methods】From September 2016 to April 2018, 42 patients with incision infection after posterior pedicle system internal fixation were selected as the observation group (17 cases with superficial infection and 25 cases with deep infection), and 78 patients without infection after posterior pedicle system internal fixation were selected as the control group. The serum levels of CRP, PCT and ESR were compared before and 1,3,5,7 days after operation, and the correlation between them and infection was analyzed.【Results】There was no significant difference in the levels of CRP, PCT and ESR between the two groups (P>0.05); on the 1st, 3rd, 5th and 7th day after operation, the levels of CRP, PCT and ESR in the observation group were significantly higher than those in the same group before surgery and those of the control group at the same time (P<0.05). There was no significant difference in the levels of serum CRP, PCT and ESR between patients with Asabu infection and those with deep infection before and 1 d after operation (P>0.05). The levels of serum CRP, PCT and ESR in patients with deep infection were significantly higher than those in patients with shallow infection at 3, 5 and 7 d after operation (P<0.05).Pearson correlation analysis showed that serum CRP, PCT and ESR levels were positively correlated with leukocyte count in patients with incision infection after posterior pedicle system internal fixation. In the observation group, 67 strains of pathogenic bacteria were isolated from 42 infected patients, of which 44 were Gram-positive bacteria (65.67%), 19 were Gram-negative bacteria (28.36%) and 4 were fungi (5.97%).【Conclusion】The levels of CRP, PCT and ESR in patients with incision infection after posterior pedicle system internal fixation are closely related to the infection, and the main infection is Gram-positive bacteria.

230 Relationship of Hcy, lipoprotein a and L-FABP Levels with Early Renal Injury in Diabetic Nephropathy Patients

【Objective】To investigate the changes and value of homocysteine (Hcy), lipoprotein a and liver fatty acid binding protein (L-FABP) in early renal injury of diabetic nephropathy (DN).【Methods】The study contained patients with type 2 diabetes mellitus (T2DM group,n=60), type 2 diabetes mellitus with microalbuminuria (MA group,n=60), type 2 diabetic kidney injury (CN group,n=60) and healthy volunteers as the control group (n=60). The levels of Hcy, lipoprotein a, L-FABP and renal function were compared in the four groups. The relationship of levels of Hcy, lipoprotein a and L-FABP with changes in renal function of patients was investigated by a linear correlation method.【Results】The levels of Hcy, lipoprotein a and L-FABP in the T2DM group, MA group and CN group were significantly higher than those in the control group (P<0.05). Hcy, lipoprotein a and L-FABP levels in the MA group and CN group were higher than those in the T2DM group. The levels of Hcy, lipoprotein a and L-FABP in the CN group was significantly higher than those in the MA group (P<0.05). The levels of BUN and SCr in the CN group were significantly higher than those in the T2DM group, the MA group and the control group (P<0.05). The UAER values of the MA group and the CN group were higher than those of the T2DM group (P<0.05); and the UAER value of the CN group was higher than that of the MA group (P<0.05); The levels of Hcy, lipoprotein a and L-FABP in the T2DM patients with >20μg/min were significantly positively correlated with SCr and UAER (P<0.05).【Conclusion】The levels of Hcy, lipoprotein a, and L-FABP in patients with DN were significantly elevated and closely correlated with renal dysfunction in patients.

233 Effect of Prone Position with Hands and Knees in the Second Stage of Labor on Delivery Control and Outcomes of Delivery in Senile Primipara

【Objective】To investigate the effects of prone position with hands and knees in the second stage of labor on the control of delivery control and outcomes of delivery in elderly primipara.【Methods】A total of 148 elderly primiparas who were admitted in our hospital from July 2016 to December 2017 were enrolled. All primiparas were randomly divided into the observation group and the control group, with 74 cases in each. In the control group, the conventional supine position was performed in the second stage of labor and the treatment group, while the prone position of the hands and knees was placed in the observation group. The maternal Labour Agentry Scale (LAS), the Visual Analogue Scale (VAS), the Visual Analogue Scale-Anxiety (VAS-A) and the delivery outcome were compared between the two groups.【Results】The scores of LAS, VAS and VAS-A in the observation group were significantly lower than those in the control group (t=12.686, 11.322, 15.078, P<0.01). The second stage of labor was significantly shorter in the observation group than that in the control group; and the amount of bleeding was significantly lower than that of the control group at 2 hours postpartum (t=9.496, 8.174, P<0.05, P<0.01). The natural labor and perineal integrity in the observation group were significantly higher than those in the control group (90.54% vs 75.68%, 20.27% vs 8.11%) (χ2=5.824,4.495,P<0.05).【Conclusion】The prone position with hands and knees in the second stage of labor can alleviate the anxiety and pain level, improve the sense of delivery control, and improve the delivery outcome in the elderly primiparas.

236 Effects of high-throughput Low-calcium Dialysis on Blood Phosphorus, Calcium and iPTH Levels in Patients with Uremia

【Objective】To observe the effect of high-throughput low-calcium dialysis on uremia treatment and on blood phosphorus, blood calcium and iPTH.【Methods】The patients with uremia who were treated in our hospital from August 2016 to August 2017 were selected as the research subjects. According to the dialysis methods, the patients were divided into high-throughput dialysis group and high-throughput low-calcium dialysis group. The levels of patients' nutrition evaluation score (SGA), blood phosphorus, blood calcium and iPTH were observed before and after dialysis. The blood biochemical indexes and the levels of inflammatory cytokines in the two groups were compared before and after the treatment.【Results】There were no differences in the score of SGA and levels blood phosphorus, blood calcium and iPTH between the two groups before treatment. After treatment, the SGA score of the patients in the observation group was higher than that of the control group, while the levels of blood phosphorus, blood calcium and iPTH were lower than those of the control group. The levels of BUN, SCr and GFR were not significant differences between the two groups before treatment. After treatment, the levels of BUN and SCr in the observation group were lower than those of the control group, while the level of GFR was higher than that of the control group. There was no difference in cytokine levels between the two groups before treatment. After treatment, the levels of IL-18, IL-6 and hs-CRP in the observation group were lower than those in the control group.【Conclusion】High-throughput low-calcium dialysis has a better therapeutic effect on uremia. It can obviously lower the levels of calcium, phosphorus, iPTH and improve renal function.

239 Correlation between Neutrophil to Lymphocyte Ratio and Contrast-induced Nephropathy after Coronary Angiography

【Objective】To investigate the correlation between neutrophil to lymphocyte ratio (NLR) and contrast-induced nephropathy (CIN) after coronary angiography (CAG).【Methods】A total of 125 patients who underwent CAG examination in our hospital were studied. According to the occurrence of CIN, 125 patients were divided into the CIN group and the non CIN group. Baseline data of the two groups were compared. Multivariate logistic stepwise regression analysis was used to screen the independent risk factors of CIN. Receiver operating curve (ROC) was used to analyze the value of NLR ratio in predicting CIN.【Results】CIN occurred in 23 out of 125 patients, with an incidence of 18.4%. Compared to the non-CIN group, patients in the CIN group were older with higher proportion of diabetes mellitus, urea nitrogen, serum creatinine, urinary β-2 microglobulin, C-reactive protein and NLR ratio (P<0.05), while the GFR was significantly decreased (P<0.05) and the dosage of contrast agent was significantly increased in the CIN group (P<0.05). Multivariate logistic stepwise regression analysis showed that age, diabetes mellitus, basic renal insufficiency, serum creatinine, C-reactive protein, NLR ratio and contrast agent dosage were independent risk factors for CIN (P<0.05). When the cut-off value was 6.5, the area under the ROC curve was 0.865 of NLR ratio in predicting CIN; the sensitivity and the specificity were 85.9% and 83.4%, respectively.【Conclusion】NLR ratio (≥6.5) is an independent risk factor for CIN and it has high value in predicting CIN.

【Objective】 To investigate the association between the Anisakis species-specific IgE and high-risk pregnancy.【Methods】 A total of 738 maternal who admitted to Wuhan fifth hospital from January 2017 to December 2019 diagnosed as high-risk pregnancy were selected as the study group, 500 healthy women were selected as the control group. The basic data of the maternal and the neonatal outcome were collected, with venous blood was collected and the presence of specific anti Anisakis IgE antibodies was detected. The differences in the maternal basic data and neonatal outcomes between two groups were compared and the related factors for the presence of specific anti Anisakis IgE antibodies were investigated.【Results】The proportion of IgE positive patients in the observation group was significantly higher than that in the control group [183 (24.8%) vs. 100 (20.0%), P=0.049]. The average age of women in the IgE-positive group was significantly older than that of the IgE-negative group, the proportion of women who received natural delivery was significantly lower and the rate of first birth was significantly lower in the IgE-positive group, the postpartum hospital stay was significantly longer and the times of inspection during pregnancy was significantly less in the IgE-positive group . Among the newborns who were delivered by IgE-positive women, the weight and gestational age were significantly less and the proportion of NICU was significantly higher than in the IgE-negative group. There was a significant correlation between IgE positive and high-risk pregnancy.【Conclusion】High-risk pregnancy is an independent risk factor for pregnant women infected by Anisakis with IgE antibody positive, which affects pregnancy outcomes.

【Objective】To investigate the clinical effect of traditional open reduction and percutaneous minimally invasive locking plate in the treatment of middle and lower tibial fractures.【Methods】The clinical data of 93 cases of middle and lower tibial fracture treated in our hospital from May 2014 to February 2018 were retrospectively analyzed. According to the different treatment methods, the patients were divided into minimally invasive group (n=54) and open group (n=39). The open group was treated with conventional open reduction and common plate internal fixation. The minimally invasive group was treated with percutaneous minimally invasive locking plate internal fixation. The perioperative conditions (operation time, incision length, intraoperative bleeding volume, hospital stay, fracture healing time), complications, ankle joint and knee joint function were compared between the two groups.【Results】The operation time, hospital stay and fracture healing time of the minimally invasive group were shorter than those of the open group, incision length was shorter than that of the open group, and the amount of bleeding during operation was less than that of the open group, the difference was statistically significant (P<0.05). The incidence of complications in the minimally invasive group was lower than that in the open group (P<0.05). The excellent and good rates of ankle and knee joint function in minimally invasive group were 83.33% and 90.74% respectively, which were higher than those in open group (69.23% and 74.36%, P<0.05 and P<0.05).【Conclusion】Compared with conventional open reduction and conventional plate internal fixation, percutaneous minimally invasive locking plate internal fixation has better clinical effect, fewer complications and better functional recovery of ankle and knee joint, which is worthy of clinical promotion.

249 Clinical Value of Thromboelastography(TEG) in the Evaluation of Patients with Unstable Angina and Antiplatelet Therapy after PCI

【Objective】To explore the clinical value of thromboelastography (TEG) in the evaluation of patients with unstable angina pectoris (UAP) and antiplatelet therapy after PCI.【Methods】One hundred and seventeen UAP patients in our hospital were divided into single vessel disease, double vessel disease and three vessel disease groups according to the number of coronary lesions. TEG parameters of each group were compared. According to the different antiplatelet treatment methods of UAP patients after PCI, they were divided into clopidogrel group and tegrerolol group. The platelet inhibition rate and TEG parameters of the two groups were analyzed.【Results】The response time (R) of UAP patients in the three vessel disease group and the two vessel disease group was significantly lower than that the in single vessel the disease group, and the R value of UAP patients in the three vessel disease group was significantly lower than that in the two vessel disease group; the α angle and Ma value of UAP patients in the three vessel disease group and the two vessel disease group were significantly higher than that in the single vessel disease group, and the α angle and Ma value of UAP patients in the three vessel disease group were significantly higher than that in the two vessel disease group (P<0.05). After PCI, the R value of the tegrilol group was higher than that of the clopidogrel group, while the α angle and MA value were lower than those of the clopidogrel group (P<0.05). After PCI, the platelet inhibition rate of AA pathway and ADP pathway in the tegrilol group was higher than that in the clopidogrel group (P<0.05).【Conclusion】TEG detection has a certain clinical value in evaluating the degree of coronary lesions in UAP patients and the effect of antiplatelet therapy after PCI.

252 Effect of Thoracic Paravertebral Nerve Block Combined with Urethral Mucosal Anesthesia on Urethral and Bladder Irritation in Patients with Urethral Catheterization

【Objective】To explore the value of ultrasound-guided thoracic paravertebral nerve block anesthesia (PVB) combined with urethral mucosal anesthesia in patients with renal calculi undergoing transurethral catheterization.【Methods】A total of 140 patients with kidney stones were randomly divided into group A and group B with 70 cases in each group. Patients in both groups received percutaneous nerve lithotripsy. Group A used ultrasound-guided PVB combined with urethral mucosal anesthesia, while group B used ultrasound-guided PVB only. The heart rate (HR) and mean arterial pressure (MAP) were compared between the two groups of patients before anesthesia (T0), 10 minutes after anesthesia (T1), after the establishment of a lithotripsy (T2), during lithotripsy (T3) and end of surgery (T4). Meanwhile, the degree of bladder irritation, postoperative hematuria and the incidence of various adverse reactions were recorded and compared during the intraoperative urethral catheterization. 【Results】At T0, T3 and T4, the HR values were not statistically significant difference between group A and group B (P>0.05). At T1 and T2, the HR values of group A were higher than those of group B (P<0.05). At T0 and T4, the difference of MAP between group A and group B was not statistically significant (P>0.05). At the time of T1, T2 and T3, the MAP value of group A was higher than that of group B (P<0.05). The incidence of bladder irritation during urethral catheterization in group A was lower in group than that in group B (P<0.05). The incidence of postoperative hematuria in group A was 31.43%, which was lower than that in group B (47.14%); the difference was statistically significant (P<0.05). The incidence of adverse reactions in group A was lower than that in group B (P<0.05).【Conclusion】Ultrasound-guided PVB combined with urethral mucosal anesthesia in patients with renal calculi undergoing transurethral catheterization is beneficial to reduce the irritating effect on the urethra and bladder of patients, maintain the stability of blood flow during surgery, and reduce various adverse reactions after operation.

255 Change of Blood Flow Density and No-vessel Area in Macular Fovea in dDiabetic Retinopathy and its Significance

【Objective】To investigate the changes of macular vascular density (MVD) and foveal avascular zone (FAZ) in diabetic retinopathy (DR).【Methods】From February 2015 to February 2017, 74 diabetic patients (DM group) and 40 healthy controls (CON) were selected. Diabetic patients were divided into no DR group, NPDR group and PDR group. The transverse, vertical and area of MVD and FAZ were measured and compared by optical coherence tomography (OCTA).【Results】There were 135 eyes in the DM group, 71 in the no-DR group, 53 in the NPDR group, 11 in the PDR group and 80 in the normal CON group. MVD of the PDR group and the NPDR group was significantly lower than that of con group (P<0.05).The MVD of the surface and inner retinas of the No-DR group was significantly lower than that of the CON (all P<0.05), and that of the PDR group and the NPD group was significantly lower than that of the No-DR group (all P<0.05). Compared with the CON, the vertical diameter, horizontal diameter and area of FAZ in the PDR group were significantly larger (P<0.05); the area of FAZ in the no-DR group and the NPDR group was significantly larger (P=0.000), but there was no significant difference in the vertical diameter and horizontal diameter of FAZ (P>0.05).【Conclusion】The expansion of FAZ area and the decrease of MVD in DR patients have important clinical value for early diagnosis of DR.

259 Effect of Heart Failure on the Survival of Patients with Septic Shock

【Objective】To explore the effect of heart failure on the survival of patients with septic shock.【Methods】 Eighty-two patients with septic shock admitted to our hospital from June 2016 to June 2018 were divided into the observation group (complicated with heart failure) and the control group (without heart failure) according to whether they had heart failure at the time of treatment, 41 in each group. The time of mechanical ventilation and hospitalization, the score of acute physiology and chronic health status Ⅱ (APACHE Ⅱ), the incidence and mortality of systemic inflammatory response syndrome (SIRS) and multiple organ dysfunction syndrome (MODS) were compared between the two groups.【Results】 The time of mechanical ventilation and hospital stay in the observation group were higher than those in the control group, and Apache Ⅱ score was lower than that in the control group, the difference was statistically significant (P<0.05). There were 22 cases of SIRS (53.66%), 23 cases of MODS (56.10%), 13 cases of death (31.70%), 13 cases of SIRS (31.71%), 15 cases of MODS (36.59%) and 6 cases of death (14.63%) in the observation group. The incidence of SIRS, the incidence and mortality of MODS in the two groups were statistically significant (P<0.05).【Conclusion】 Heart failure will affect the prognosis of patients with septic shock, increase the time of mechanical ventilation and hospitalization, increase the incidence and mortality of MODS, and threaten the survival of patients. Clinical attention should be paid to such patients.

262 Effect of Ultrasound-guided Microwave Ablation on Thyroid Hormone, Stress Response and Cosmetic Effect in Patients with Benign Thyroid Nodules

【Objective】To investigate the effect of ultrasound-guided microwave ablation on thyroid hormone, stress response and cosmetic effect in patients with benign thyroid nodules.【Methods】 The clinical data of 82 patients with benign thyroid nodules treated in our hospital from January 2017 to January 2018 were retrospectively analyzed. 33 patients receiving routine surgical treatment were divided into control group and 49 patients receiving ultrasound-guided microwave ablation were divided into observation group. The success rate of nodule treatment, nodule shrinkage, thyroid hormone level, inflammatory factor level, cosmetic effect and complications were compared between the two groups.【Results】One year follow-up showed that the treatment success rate of the control group was 95.7% (113 / 118) lower than that of the observation group 99.1% (114 / 115), the difference was not statistically significant (χ2=2.633, P>0.05). The cosmetic satisfaction rate of the observation group was 95.92% (47 / 49) which was significantly higher than that of the control group 75.76% (25/33). The difference was statistically significant (χ2=4.361, P<0.05). After treatment, the levels of free triiodothyronine (FT3) and free thyroxine (FT4) in the two groups were lower than those before treatment, and the levels of thyrotropin (TSH) were higher than those before treatment, and the differences between the two groups were statistically significant (P<0.05). After treatment, the level of serum TNF-α in the observation group was higher than that before treatment, the level of serum C-reactive protein (CRP), TNF-α and interleukin-6 (IL-6) in the control group was higher than that before treatment, and the difference was statistically significant (P<0.05). After treatment, the incidence of complications in the observation group was 10.20% (5 / 49), which was significantly lower than that in the control group 30.30%(10 / 33), the difference was statistically significant (χ2=5.423, P<0.05).【Conclusion】The effect of ultrasound-guided microwave ablation on benign thyroid nodule is similar to that of conventional operation, and it has little effect on thyroid function, little stress response to body and good cosmetic effect.

265 Analysis of Newly Diagnosed Occupational Disease Cases in Hunan Province during 2016~2018

【Objective】To analyze the specific situation of newly diagnosed occupational disease cases in Hunan Province in recent years in order to make recommendations for the focus of occupational disease prevention and control.【Methods】Obtain newly diagnosed and reported occupational disease report data of Hunan Province from 2016 to 2018 from the National Occupational Disease and Occupational Health Information Monitoring System. Statistical analysis software SPSS21 was used to analyze the distribution of different occupational disease types, different economic types, enterprise sizes, and occupational disease occurrence in the administrative city.【Results】A total of 6785 cases of various occupational diseases were newly diagnosed in Hunan Province from 2016 to 2018, of which 5490 cases were pneumoconiosis (80.91%). The new occupational diseases from 2016 to 2018 were mainly coal workers pneumoconiosis (4014 cases, 59.16%), followed by 1392 silicosis cases (20.52 %) and arsenic and its compound poisoning (1029 cases, 15.17%). Using the Kruskal-walis test, the results show that there were differences in terms of the working age for coal workers' pneumoconiosis, silicosis and other pneumoconiosis patients. The median working age of silicosis is less than that of coal workers' pneumoconiosis and other types of pneumoconiosis (P<0.01). There are 5045 cases of state-owned The new cases of occupational diseases from 2016 to 2018 were mainly state-owned enterprises (5045 cases, 74.35%), followed by private enterprises (1549 cases, 22.83%) and other enterprises (191 cases, 2.82%). From the perspective of enterprise scale, 382 cases (5.63%) were from large enterprises, 2475 cases (36.48%) from medium-sized enterprises, 3634 cases (53.56%) from small enterprises, 35 cases of micro-enterprises (0.52%) and 259 cases with unknown scale enterprises (3.82%). From the perspective of administrative city, Loudi had the largest number (1777 cases, 26.20%), followed by Changde (889 cases,13.10%) and Yueyang (776 cases,11.44%).【Conclusion】From 2016 to 2018 in Hunan Province, newly diagnosed occupational diseases are mainly caused by coal workers' pneumoconiosis, silicosis, arsenic and arsenic compounds poisoning. All cases come from state-owned enterprises. The main patients are workers from small medium enterprises. Loudi, Changde and Yueyang have more newly diagnosed and reported cases. Relevant departments should not only encourage enterprises to actively carry out occupational health protection and occupational health inspections, and provide conditions for the diagnosis of occupational diseases for sick employees, but also urge and supervise enterprises to do a good job of protecting occupational hazards.
