Prognostic Value of Procalcitonin, BNP, Cardiac Troponin I and D-dimer in Patients with Chronic Heart Failure

Abstract  【Objective】To investigate the prognostic value of procalcitonin (PCT), B-type natriuretic peptide (BNP), cardiac troponin I (cTnI), and D-dimer (D-D) in patients with chronic heart failure (CHF).【Methods】A total of 70 patients with CHF patients in our hospital of year 2016 were selected in the CHF group. Patients in the CHF group was further divided into three groups according to New York Heart Association's cardiac function classification criteria (NYHA): cardiac function Ⅱ group (n=33), Ⅲ group (n=26) and Ⅳ group (n=11). At the same time period, 70 healthy volunteers were enrolled as the control group. Serum PCT, BNP, cTnI, and D-dimer levels were measured at the admission day and one month later. Then the statistical analysis was performed as well.【Results】Serum levels of PCT, BNP, cTnI, and D-D in the CHF group were higher than those in the control group (P<0.05). The levels of serum PCT, BNP, cTnI, and DD in the cardiac function level Ⅱ, Ⅲ and Ⅳ in the CHF group gradually elevated with the increase of the degree of heart failure. The differences among the three groups were statistically significant(P<0.05). There was a decreasing trend in serum levels of PCT, BNP, cTnI, and D-D after 1 month of treatemnt (P<0.01). Left ventricular ejection fraction (LVEF) of CHF patients after one treatment was higher than that before treatment (P<0.01). NYHA cardiac function level of the CHF patients after 1 month treatment was significantly lower than before treatment; and the difference was statistically significant (P<0.01). Serum levels of PCT, BNP, cTnI, and D-D were positively correlated with NYHA cardiac function level (P<0.05) and negatively correlated with LVEF (P<0.05).【Conclusion】The levels of PCT, BNP, cTnI, and D-D in serum reflect severity of the CHF patients, which can provide important predictive value for disease monitoring.
