Dosimetric Study of Intensity Modulated Radiation Therapy and Body Gamma-Knife Radiosurgery for Peripheral Non-small-cell Lung Cancer

【Objective】To investigate the dosimetric characteristic of radiotherapy for peripheral non-small-cell lung cancer (NSCLC) with body γ-knife (MASEP body γ-knife) and intensity modulated radiation therapy (IMRT).【Methods】A total of 11 patients with peripheral non-small-cell lung cancer were selected in the study. Two treatment plans were designed for each patient, including γ-knife and IMRT. The total prescribed dose was 66 Gy for each patient. The planning standard was that the prescribed dose covered at least 95% of the planned target volume (PTV), and each dangerous organ dose was required to be within the tolerable dose range. The difference between the tumor target dose and the normal tissue exposure dose was compared by the isodose distribution of the target area, dose volume histogram (DVH), conformity index (CI), and heterogeneity index (HI).【Results】CI and HI of the IMRT plan were superior to the γ-knife plan . The V5, V10, V20, and V30 doses of surrounding normal tissues of the γ-knife plan were significantly lower than those of the IMRT plan (P<0.05). The maximal dose (Dmax), mean dose (Dmean), and minimum dose (Dmin) of gamma-knife PTV were better than the corresponding IMRT indicators, and the target dose was higher than the IMRT plan (P<0.05).【Conclusion】Body γ-knife treatment of peripheral non-small-cell lung cancer can significantly increase the target volume dose as well as protect the surrounding normal tissue. It has the potential to improve efficacy of comprehensive treatment.

Analysis of the Relationship between the Recovery of Thyroid Function and the Growth and Development of Children with Newly Screened High TSH Hyperthyroidemia

【Objective】To explore the relationship between the recovery of thyroid function and the growth and development of children with newly screened TSH (thyroid stimulating hormone) hyperthyroidemia, and to provide references for the formulation of therapeutic indications and treatment goals. 【Methods】 A total of 281 children with newly screened high TSH hyperthyroidemia who were admitted to our hospital from February 2014 to February 2017, according to the recovery of thyroid function during follow-up , were divided into the TSH natural recovery group and the drug treatment group. In the same time period, 60 healthy children of similar age were selected as the control group. We compared the intelligence and growth development of the three groups at the age of one year, and analyzed the effects of serum TSH on children's growth and development.【Results】Of the 281 cases of high TSH hyperthyroidemia patients, 237 cases (84.34%) showed their TSH returned to normal level naturally (without any treatment) during the follow-up period, of which 176 cases returned to normal within 3 months, 55 cases return to normal within 3~6 months, and 6 cases returned to normal within 6~9 months. The remaining 44 patients with high serum TSH more than 15 mIU/mL during the initial or follow-up period received levothyroxine sodium tablets. Of these, 35 cases returned to normal serum TSH level after treatment. However, the remaining 9 patients still maintained at high TSH hyperthyroidemia even after receiving the treatment. There were no significant differences between the MDI (mental development index) and PDI (psychological development index) scores of the three groups of children at the age of one year (P>0.05). The children's height of the three groups at the age of one year did not show significant difference (P>0.05).【Conclusion】The thyroid function of most children with newly screened high serum TSH hyperthyroidemia can return to normal in a short time, and high TSH hyperemia does not have a significant influence on the growth and development of children.Clinical treatment indications should be carefully judged to avoid overtreatment.

Relationship between Cystatin C and Cognitive Dysfunction in Elderly Patients with Type 2 Diabetes Mellitus

【Objective】To investigate the relationship between serum cystatin C(Cys C) level and cognitive impairment in elderly patients with Type 2 Diabetes Mellitus. 【Methods】A total of 100 elderly patients with type 2 diabetes were selected and divided into the high Cys C group and low Cys C group according to their levels of cystatin C. The differences of clinical characteristics and the scale of elderly cognitive function (SECF) scores between the two groups were compared. Logistic regression was used to analyze the correlation between the level of Cys C and cognitive dysfunction. 【Results】There were statistically significant differences between the two groups in the total score of SECF and its subdivisions: orientation, width, animal name, calculation, classification, copying, speech and memorization (P<0.05). Comparing the ratio of abnormal case numbers in each item, the differences in total score, orientation, memorization, animal name, calculation, copying and speech were statistically significantly different (P<0.05). Univariate logistic regression analysis showed that the OR value of cognitive dysfunction in high Cys C group was 2.538 compared with that in low Cys C group. However, multivariate logistic regression analysis showed age, male, uric acid were independent risk factors for cognitive impairment; Cys C did not enter the equation. 【Conclusion】 The level of serum Cys C may be associated with cognitive dysfunction in elderly patients with type 2 diabetes, but is not independently related.

Clinical Efficacy of GEMOX and GDP Regimens in the Treatment of Relapsed Refractory Non-Hodgkin's Lymphoma

【Objective】To investigate the efficacy of gemcitabine plus oxaliplatin (GEMOX) and gemcitabine plus cisplatin, dexamethasone (GDP) in the treatment of relapsed refractory non-Hodgkin's lymphoma(NHL). 【Methods】Retrospective analysis was conducted on a total of 80 cases of relapsed refractory NHL.. There were 40 patients who were treated with GEMOX regimen (A group) and 40 patients who were treated with GDP regimen (B group). Every cycle of treatment lasted 21 days. Efficacy, toxicity and Karnofsky scores were evaluated every two cycles of treatment.. 【Results】The total effective rates of patients with relapsed and refractory B-cell and T-cell lymphoma in group A were 62.96% (17/27) and 30.77% (4/13) respectively, significantly higher than those in group B (39.29%,11/28 and 16.67%,2/12) respectively (P<0.05); The incidence of nausea and vomiting in group Ⅲ~Ⅳ was significantly lower in group A than in group B (P<0.05). There was no significant difference in the other adverse reactions between the two groups (P>0.05). Karnofsky score decreased significantly after treatment in both groups, and Karnofsky score of the group B was significantly lower than that of the group A (P<0.05). 【Conclusion】The efficacy of GEMOX and GDP regimens in the treatment of relapsed and refractory B-cell lymphomas is significantly higher than that of T-cell lymphomas, and the short-term efficacy of GEMOX regimen in the treatment of relapsed and refractory NHL is significantly better than that of GDP. It is worthy of clinical promotion.

Application of Laryngeal Mask Airway General Anesthesia Combined with Brachial Plexus Block Anesthesia in Upper Limb Surgery of Elderly Patients with Cardiovascular Diseases

【Objective】To study the application value of laryngeal mask airway (LMA) general anesthesia combined with brachial plexus block anesthesia in upper limb surgery of elderly patients with cardiovascular diseases. 【Methods】A total of 74 elderly patients with cardiovascular diseases who underwent upper limb surgery were randomly divided into two groups with 37 cases each. Patients in the LMA group were given LMA general anesthesia combined with brachial plexus block anesthesia while patients in the control group were given general anesthesia by tracheal intubation combined with brachial plexus block anesthesia. Comparison of the Effect of bBood Transfusion of the Different Ratio of Plasma and Red Blood Cells on the Prognosis of Patients with Acute Upper Gastrointestinal Bleeding

【Objective】To explore optimal proportion of plasma (FFP) and red blood cell (RBC) in patients with acute upper gastrointestinal bleeding and to evaluate the prognosis.【Methods】The clinical data of 120 patients with acute upper gastrointestinal bleeding in our hospital from June 2014 to June 2017 were retrospectively analyzed. According to the proportion of blood transfusion in FFP and RBC, 37 cases of FFP:RBC > 1:1 were recorded as group A, 45 cases of FFP:RBC more than or equal to 1:2, and less than or equal to 1:1 were recorded as B group, and the other 38 cases of FFP:RBC < 1:2 were recorded as group C. The use of FFP and RBC, blood routine, blood coagulation, changes in electrolyte indicators, adverse effects of blood transfusion and prognosis were compared. 【Results】The differences of the use of FFP and RBC in the 3 groups was statistically significant (P< 0.05), which were also statistically significant between each two groups (P<0.05). The use of FFP in the group A was the highest, the B group was a little lower and the C group was the lowest. The use of RBC in the group B was the highest, the A group was a little lower, and the C group was the lowest. The differences of blood routine, blood coagulation function index and Ca
2+
levels among the 3 groups after blood transfusion were statistically significant (P<0.05). Compared among the groups, Hb, PLT and INR in group C were significantly higher than those in the group A and the group B (
P
<0.05), and the PT and APTT were significantly longer than those of the group A and B (P<0.05), and the INR in the group A was significantly higher than that in the group B (P<0.05), and the levels of PT and APTT were significantly longer than those in the group B (P<0.05). The level of Ca

in the group B was significantly lower than those of the group A and C group (P<0.05). After transfusion, the levels of Hb and Ca
2+
in the 3 groups were all decreased (P<0.05), and the levels of PLT in the group A and C decreased (P<0.05), the levels of PT and APTT were prolonged (P<0.05), INR increased (P<0.05). The incidences of total adverse reactions of blood transfusion in the group A, the group B and the C group were 5.41%, 0.00%, 2.63%, respectively, without statistically significant differences (P> 0.05). The incidences of total poor prognosis in the 3 groups was 21.62%, 2.22%, 15.79%, respectively, of which the group B was significantly lower than that of the group A and C (P<0.05). 【Conclusion】Patients with different FFP and RBC ratios can affect the function of blood clotting and electrolyte of acute upper gastrointestinal bleeding, and it is recommended to maintain FFP:RBC more than or equal to 1:2 and less than or equal to 1:1, which can significantly reduce adverse events and improve the prognosis.

Effect of Benzamide on Inflammatory Cells and Inflammatory Factors in Periodontitis in Experimental Rats

【Objective】To study the effects of poly ADP-ribose polymerase (PARP) inhibitor Benzamide on the levels of inflammatory cells and inflammatory cytokines in experimental periodontitis of rats. 【Methods】 Fourty male Wistar rats ( SPF grade,3-month-old) of periodontitis model were randomly divided into periodontitis group, periodontitis intervention group, drug control group and blank group, with 10 mice in each group. The rat model of periodontitis was established by ligature of the first maxillary molar with stainless steel wire. At the fifth week after modeling, the periodontitis intervention group and the drug control group were intraperitoneally injected with 0.3 mg / kg PARP inhibitor Benzamide, while the periodontitis group and the blank group were intraperitoneally injected with equal doses of normal saline. Three rats in each group were randomly sacrificed at the end of 4,5 and 6 weeks. The histopathological changes of periodontal tissue were observed with H&E staining.The levels of serum hs-CRP、IL-1β、IL-6 and TNF-α were measured by ELISA.Results were analyzed by using SPSS 17.0, and

<0.05 was considered statistically significant. 【Results】Periodontitis intervention group showed that, at the 5th and 6th weeks, there were slightly migration of cells from the gingival epithelium toward the root, a shallow periodontal pocket, and a small amount of inflammatory cell infiltration in the gingival epithelium, the groove epithelium and the lamina propria. In the same time period, the periodontitis group showed periodontal vascular dilatation and hyperemia, migration of gingival epithelium cells to the root, a deep periodontal pocket, and a large number of inflammatory cell infiltration in the gingival epithelium, under the groove epithelium and underlying lamina propria. The drug control group and the blank group showed that the epithelial cells were located at the border of glazed cementum and that the periodontal membrane fibers were arranged in an orderly manner without inflammatory cell infiltration. Serum levels of hs-CRP, IL-1β, IL-6 and TNF-α in the drug control group and the blank group at week 5 and 6 were significantly lower than those in periodontitis intervention group and periodontitis group (P<0.05). The periodontitis intervention group was significantly lower than the periodontitis group, but there was no significant difference between the drug control group and the blank group (P>0.05).【Conclusion】 Activation of PARP aggravates the symptoms of periodontitis in the pathogenesis of periodontitis. Benzoamide relieves periodontal lesions by inhibiting PARP, which reduces the inflammatory cells and inflammatory cytokines in the periodontium in experimental rats.

Study on the Effects of Remifentanil Combined With Propofol or Sevoflurane for Myocardial Ischemia-reperfusion in Rats

【Objective】To investigate the effects of propofol or sevoflurane combined with remifentanil on myocardial ischemia-reperfusion injury (IRI) in rats.【Methods】 Sixty clean rats were randomly divided into 5 groups: ischemia grougroup, sevoflurane group, propofol group, ischemia reperfusion group (I/R group) and sham operation group (SO group) with 12 in each group. The IRI of rats in vitro was established according to the classical method.In each group, myocardial infarction area index, apoptosis index Toll-like receptor 2 (TLR-2) Toll-like receptor 4 (TLR-4), tumor necrosis factor-α (TNF-α), nuclear transcription factor- κ B (NF-κB), ribosomal protein s6 kinase (p-p70S6K) and cardiac function indexes left ventricular end-diastolic pressure (LVEDP), left ventricular maximum rate of decline (-dp/dt), left ventricular maximum rate of rise] (+dp/dt), heart rate (HR), left ventricular progressive pressure (LVDP) were compared.【Results】 In the propofol group and the sevoflurane group, there was no significant difference in LVEDP, LVDP and HR compared with those in the SO group (P>0.05), the +dp/dt, -dp/dt, LVDP in the propofol group and the sevoflurane group was significantly higher than those in the ischemic group. but lower than those of the I/R group; LVEDP was significantly lower than that in the ischemia group and the I/R group (P<0.05), and the HR was significantly higher than that in the ischemia group and the I/R group (P<0.05),and there was significant difference between propofol group and sevoflurane group (P<0.05). There was no significant difference in the TLR-2, TLR-4, TNF-α, NF-κB protain, p-p70S6K protein expression between the propofol groeup, the sevoflurane group and the SO group (mean P>0.05). The indexes of the propofol group and the sevoflurane group were significantly higher than that of the ischemia group and lower than that of the I/R group (P<0.05),there was significant difference between the propofol group and the sevoflurane group (P<0.05). The myocardial infarction area index and apoptosis index in the propofol group and the sevoflurane group were significantly higher than those in the SO group and lower than that in the ischemia group and the I/R group (P<0.05).【Conclusion】For myocardial IRI rats, propofol combined with remifentanil anesthesia could protect cardiac function, decrease myocardial infarction area index, ameliorate the imbalance of mediators, promote the p-p70S6K,NF-κB, TLR-2, TNF-α, TLR-4 expression and decrease the apoptosis index of rat heart in vitro.

Effect of Cooperative Nursing Combined with Psychological Intervention on Self- Nursing Ability and Negative Emotion and Quality of Life in First-episode Stroke Patients

【Objective】To analyze the effect of collaborative nursing combined with psychological intervention on self-care ability, negative emotions and quality of life in patients with first stroke, and to explore its clinical application value.【Methods】A total of 150 cases of first stroke patients admitted to our hospital from March 2014 to May 2017 were randomly divided into observation group and control group, 75 cases in each group. The changes of self-nursing ability, negative emotion and quality of life were compared before and after the intervention of the two groups, and the clinical application value of cooperative nursing combined with psychological intervention was summarized.【Results】After 3 months' intervention, the scores and total scores of the ESCA scale in the two groups were all higher than those before the intervention. After 3 months' intervention, the ESCA score in the observation group was higher than that in the control group, the difference was statistically significant (P<0.05). After 3 months' intervention, the HAMA scores and HAMD scores of the two groups were lower than those before the intervention. After 3 months' intervention, the HAMA scores and HAMD scores in the observation group were all lower than those in the control group, the difference was statistically significant (P<0.05). After 3 months' intervention, the SF-36 scale scores of the two groups were all higher than those before the intervention. After 3 months' intervention, the scores of SF-36 scale in the observation group were all higher than those in the control group, the difference was statistically significant (P<0.05).【Conclusion】Cooperative nursing combined with psychological intervention can effectively improve the self-care ability, improve the negative emotions and quality of life of the first stroke patients, and is worthy of wide application.

Effect of Low Airway Bacterial Colonization on the Outcomes and Prognosis of Elderly COPD Patients at Stable Stage

【Objective】To investigate the effect of low airway bacterial colonization (LABC) on the outcomes and prognosis of senile patients with chronic obstructive pulmonary disease (COPD) at stable stage, and to provide reference for assessing the severity of disease and treatment. 【Methods】A total of 85 elderly patients with stable COPD from May 2016 to August 2017 were selected as the study subjects and quantitative bacterial cultures were performed on their induced sputum. According to whether or not LABC occurred, they were divided into colonization group (53 cases) and non-colonization group (32 cases). The levels of serum inflammatory factors, symptoms, and lung function grading were compared between the two groups. 【Results】A total of 83 elderly patients with stable COPD contained 53 cases of LABC; the incidence rate was 62.35%. The main bacteria were haemophilus influenzae (30.19%), Streptococcus pneumoniae (16.98%) and Moraxella catarrhalis (13.21%). The proportion of colonizing strains and the amount of colonization were positively correlated. The levels of serum interleukin-8 (IL-8), high-sensitivity C-reactive protein (hs-CRP), the average number of acute exacerbations per year, and the rate of combined lower respiratory tract infection were higher in the colonization group than in the non-colonization group (t/χ

【Objective】To explore the influence of short-term intensive insulin therapy (CSII) on toll like receptor-4 (TLR4), interleukin-6 (lL-6), and interleukin-lβ (IL-lβ) expressions in peripheral blood mononuclear cells of patients with type-2 diabetes mellitus (T2DM). 【Methods】A total of 86 cases of patients with T2DM treated in our hospital were selected as the observation group, and 80 cases of healthy volunteers with normal physical examination were selected as the control group. The observation group were given CSII treatment for two weeks. Clinical data such as the peripheral blood glucose (FPG), postprandial 2h blood glucose (2hPG), insulin resistance index (HOMA-IR), triglyceride (TG), total cholesterol (TC), glycosylated hemoglobin (HbAlc) were measured before and after treatment in the observation group. High Density Lipoprotein (HDC-L), Low Density (LDC-L)/High Density Lipoprotein (HDC-L) of the two groups before and after treatment were measured, and TLR4, lL-6, and IL-lβ levels were detected by using reverse transcription polymerase chain reaction (RT-PCR) as well.【Results】After treatment, FPG, 2hPG, HOMA-IR, TG, TC, and HbAlc were all lower than those before the treatment, and the differences were statistically significant (P<0.05). However, compared to the control group, FPG, 2hPG, HOMA-IR, TG, TC, HbAlc levels did not change significantly after treatment (P>0.05). The results of RT-PCR showed that the expressions of TLR4, IL-l beta and lL-6 mRNA in peripheral blood mononuclear cells after treatment were lower than those before treatment (P<0.05);however, compared to the control group, the difference was not significant (P>0.05). 【Conclusion】Short-term intensive insulin therapy (CSII) in the treatment of patients with T2DM can control blood sugar (glucose) and increase insulin sensitivity. It may be related to the inhibition of TLR4, lL-6, and IL-lβ expressions in peripheral blood mononuclear cells.

Correlation between Endometriosis Fertility Index and Pregnancy Rate after Laparoscopic Surgery in Patients with Endometriosis-Associated Infertility

【Objective】To investigate the correlation between endometriosis fertility index (EFI) and pregnancy rate after laparoscopic surgery in patients with endometriosis (EMT) associated infertility.【Methods】A total of 125 EMT-associated infertility patients admitted to our department were selected in this study. There were 38 cases of ovarian endometriotic cysts, 47 cases of peritoneal ectopic lesions, 32 cases of ovarian endometriotic cysts with peritoneal ectopic lesions, and 8 cases of adenomyosis. Laparoscopic surgery was performed and EFI scores were recorded after surgery. Follow-up was conducted for three years to obtain the annual pregnancy rate. The correlation between EFI and pregnancy rate was analyzed. The possible influencing factors of postoperative pregnancy outcomes were conducted by logistic analysis as well. 【Results】 The pregnancy rate was 30.40% in the first year after surgery, 14.40% in the second year, and 4.00% in the third year. The cumulative pregnancy rate after three years was 48.80%. There were no significant differences in the three-year cumulative pregnancy rate among the four different clinical types of EMT (P>0.05). The higher the EFI score, the higher the pregnancy rate was. Logistic regression analysis showed that the risk factors of postoperative pregnancy rate were uterine benign lesion, fallopian tubal occlusion, history of pelvic surgery, and EFI score. 【Conclusion】Laparoscopic EFI is an important index to evaluate the fertility of patients with EMT associated infertility. The risk factors of pregnancy rate after laparoscopic surgery are uterine benign lesion, tubal occlusion, history of pelvic surgery, and EFI score.

Effects of Intrauterine Device Combined with Different Doses of Estrogen on Uterine Adhesion, Menstrual Recovery and Pregnancy Rate after Laparoscopic Intrauterine Adhesions Separation

【Objective】To investigate the effects of intrauterine device(IUD) combined with different doses of estrogen on intrauterine adhesion (IUA), menstrual recovery and pregnancy rate after laparoscopic intrauterine adhesions separation by trans-cervical resection of adhesion (TCRA).【Methods】The clinical data of 60 patients with intrauterine adhesions in our hospital from January 2014 to January 2017 were analyzed retrospectively. All patients underwent trans-cervical resection of adhesion at 3~7 days after menstruation, and were given O-type metal IUD combined with estrogen after operation. According to the different doses of estrogen received, patients were divided into the low-dose group (3 mg/d) with 25 cases, the middle-dose group (6mg/d) with 19 cases, and the high-dose group (9mg/d) with 16 cases. Patients were followed up with three months after operation, and changes in endometrial thickness, incidence of intrauterine adhesions and recovery of menstruation were examined. Based on the requirements of the American Fertility Association (AFS), the intrauterine adhesions score was determined. For patients who had fertility requirements, conditions for pregnancy within 1 year after operation were noted. 【Results】The incidence of intrauterine re-adhesions in the low-dose group was less than that in the middle-and high-dose groups, and the percentile of menstruation recovery was higher than that in the middle-and high-groups. The differences were statistically significant (P<0.05). After treatment, the AFS score in the low-dose group decreased more obviously than that in the middle- and high-dose groups, and the difference was statistically significant (P<0.05). The thickness of the endometrium after treatment in the low-dose group increased more obviously than that in the middle- and high-dose groups and the difference was statistically significant (P<0.05). The pregnancy rates within 1 year after operation in the three groups were 54.5% (6/11), 50% (8/16), and 55.6% (5/9) respectively but the difference was not statistically significant (P>0.05).【Conclusion】Uterine cavity indwelling IUD combined with low dose estrogen (3mg/d) is conducive to the recovery of patients with intrauterine adhesions. It can reduce intrauterine re-adhesion, improve menstrual recovery, and have no significant effect on postoperative pregnancy rate.

Application Value of Core Muscle Group Exercise in Set Suspension System for Early Rehabilitation after Artificial Hip Replacement

【Objective】 To study and analyze the early rehabilitation effects of core muscle group exercises in the set suspension system after artificial hip joint replacement. 【Methods】A total of 122 patients undergoing artificial hip arthroplasty admitted to our hospital from March 2014 to March 2015 were selected as the study subjects. Patients were randomly divided into the observation group (n=62) and the control group (n=60). The patients in the control group received routine rehabilitation training 24 hours after operation, and patients in the observation group performed the core muscle training in the set suspension system on the basis of routine exercises. The rehabilitative effects of the two groups were compared. 【Results】The "excellent and good" rate of postoperative joint level was 91.94% (57/62) in the observation group, which was significantly higher than that of the control group of 76.67% (46/60). In the observation group, the Harris hip score was 92.4 ± 7.1 points over the course of the 6 month treatment, which was higher than that of the control group's 83.2 ± 5.8 points. The difference was statistically significant (P<0.05). The scores of MBI (modified Barthel index) in the two groups before rehabilitation training showed no significant differences (P>0.05). The scores of MBI in the observation group at two weeks, one month, and three months after rehabilitation training were higher than those of the control group (48.27 ± 7.48 vs 39.18 ± 6.29),(71.54 ± 10.35 vs 59.15 ± 9.18), and (92.48 ± 7.22 vs 86.24 ± 6.99), respectively (P<0.05). One year follow-up showed that the patients' complications mainly included pain, claudication, and deep venous thrombosis of lower extremity. The incidence of complications in the observation group was 4.84% (3/62), which was lower than the control group 26.66%(16/60). The difference was statistically significant (P<0.05). 【Conclusion】The use of the set suspension system core muscle group exercises after hip replacement can promote hip function recovery, reduce the incidence of pain and complications, improve the quality of life of patients, and is worthy of clinical application.

【Objective】To compare the effects of adjustable sodium curve and sequential dialysis on the treatment of end-stage renal disease (ESRD) with refractory hypertension (RH) and their effects on left ventricular function. 【Methods】A total of 40 cases of patients with ESRD and RT were randomly divided into an adjustable sodium curve group (A group) and a sequential dialysis group (B group), with 20 cases in each. The levels of C reactive protein (CRP), IL-6, TNF- α, angiotensin II (Ang II), systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), left ventricular ejection fraction (LVEF), left ventricular mass index (LVMI), and left ventricular end diastolic diameter (LVEDd) in both groups were determined before dialysis and 3 months after dialysis. 【Results】After dialysis, the levels of Ang II, CRP, IL-6, TNF-α ; MAP, SBP, DBP, LVMI, and LVEDd of group A were lower than those of group B (P<0.05). The LVEF of group A was higher than that of group B (P<0.05). 【Conclusion】 The curative effects of ESRD with RH and the improvements of left ventricular function in patients receiving adjustable sodium curve are superior to sequential dialysis treatment.

Value of MicroRNA-499 on the Diagnostic and Prognosis in Patients with ST-elevation Myocardial Infarction

【Objective】To detect the value of microRNA-499 on the diagnostic and prognosis in patients with ST-elevation myocardial infarction.【Methods】From January 2014 to March 2016, 74 STEMI patients admitted to our hospital from January 2014 to March 2016 were selected as clinical data (observation group) and 80 healthy volunteers as control group. In all subjects, venous blood was taken immediately after admission for the detection of miR-499 concentrations. All patients were followed up for 6 months to record major adverse cardiovascular events (Mace). By Persean correlation analysis, the linear relationship between miR-499 and CK-MB and cTnI was studied, and the difference of miR-499 concentration between STEMI patients with Mace and those without Mace was compared.【Results】 The level of miR-499 in peripheral blood circulation in STEMI patients was significantly higher than that in healthy controls (P<0.01). The level of circulating miR-499 was positively correlated with troponin I (cTnI) (r=0.475,P<0.01) and CK-MB (r=0.526,P<0.01). 31 STEMI patients were classified as Mace group, while the remaining STEMI patients were not classified as non-MACE group. It was found that the concentration of miR-499 in peripheral blood circulation in Mace group was significantly higher than that in non-MACE group (P<0.01). 【Conclusion】Circulating miR-499 can be used as a potential marker for the diagnosis of STEMI and the evaluation of its clinical prognosis.

Comparison of Efficacy and Side Effects between Oxaliplatin Combined with Capecitabine and ECF regimen in the Treatment of Advanced Gastric Cancer

【Objective】 To compare the curative effect and toxicity of two adjuvant chemotherapy regimens: oxaliplatin with capecitabine vs ECF regimen (epirubicin + cisplatin + leucovorin + fluorouracil) in the treatment of patients with advanced gastric cancer. 【Methods】A total of 100 patients with advanced gastric cancer were randomly divided into the control group who underwent the ECF regimen and the observation group who received oxaliplatin combined with capecitabine. There were 50 patients in each group. The efficacy and side effects of the two regimen treatments between the two groups were compared. 【Results】The total effective rate in the observation group was 78% (39/50), slightly higher than 70% (35/50) in the control group. However, there was no significant difference between the two groups (P>0.05). The incidence of toxic side effects was significantly lower in the observation group than in the control group and the difference was significant (P<0.05).【Conclusion】The efficacy of oxaliplatin combined with capecitabine regimen in the treatment of advanced gastric cancer is similar to that of the ECF regimen. However, the patient's tolerance is good, the hospitalization time is short, and the adverse reactions are mild. Patients have good compliance and high quality of life. It is worthy of clinical recommendation.

【Objective】To investigate the effect of cytochrome P450CYP2C19 gene polymorphism on antiplatelet effect of clopidogrel in patients with acute coronary syndrome (ACS).【Methods】From January 2015 to June 2017, 100 patients with ACS were selected. According to clopidogrel resistance, the patients were divided into two groups: resistance group (n=50) and non-resistance group (n=50). The platelet aggregation function and genotypes of the two groups were measured. The allele frequency, genotype frequency and distribution of two single nucleotide polymorphisms (SNPs) were analyzed.【Results】 The distribution rate of CYP2C19*2 (rs4244285) GG in resistant group was significantly lower than that in the non resistant group, and the distribution of GA+AA type was significantly higher than that in the non resistant group (P<0.05). The platelet aggregation test induced by adenosine diphosphate showed that the platelet aggregation rate of CYP2C19*2, CYP2C19*3 gene variant was significantly higher than that of the wild type (P<0.05).【Conclusion】Clopidogrel resistance is closely associated with CYP2C19*2 (rs4244285) GA+AA genotype in patients with ACS. The detection of clopidogrel genotype may provide a reference for clinical application of clopidogrel.

Effect of Cognitive Behavior Intervention in Combination with Kinesitherapy on Psychological Status and Living Quality of Patients with Depression

【Objective】To investigate the effect of cognitive behavior intervention in combination with kinesitherapy on the psychological status and living quality of patients with depression, and to provide reference for developing the nursing intervention method for depression patients. 【Methods】A total of 913 cases of depression patients admitted to hospital from January 2015 to December 2016 were selected as the research subjects. They were divided into the observation group (n=460 cases) and the control group (n=453 cases). Patients in the control group received oral administration of fluoxertine hydrochloride and routine nursing intervention, while patients in the observation group received cognitive behavior intervention in combination with kinesitherapy on the basis of the treatment in the control group. After 6 months of follow-up, the psychological status and living quality of the patients were assessed using symptom check list-90 (SCL-90) and short-form health survey questionnaire (SF-36). 【Results】The scores of depression, anxiety, bigotry (paranoid), obsessive-compulsive symptoms, psychoticism and interpersonal sensitivity in the observation group were significantly lower than those in the control group (t=19.737~72.760,P<0.01). The scores of physiological function, physiological role, body pain, general health, vitality, social function, emotional function and mental health were significantly higher in the observation group than in the control group (t=19.087~55.713,P<0.01). 【Conclusion】 Cognitive behavior intervention in combination with kinesitherapy can improve the psychological status and enhance the living quality of depression patients.

Diagnostic Value of IL-27 Combined with ADA for Tuberculous Pleurisy

【Objective】 To investigate the diagnostic value of interleukin -27 (IL-27) combi ned with adenosine deaminase (ADA) in the diagnosis of tuberculous pleurisy.【Methods】 Ninety cases of pleural effusion hospitalized in our hospital from May 2016 to October 2017 were selected, including 46 tuberculous pleural effusion patients (observation group) and 44 patients with other pleural effusion (control group). All of the subjects were performed pleural puncture after admission, collecting pleural effusion, separating pleural effusion supernatant, collecting peripheral venous blood at the early morning of admission, and separating serum. The content of ADA was measured by colorimetric method and the content of IL-27 was determined by enzyme linked immunosorbent assay (ELISA). The operation was determined strictly according to the specification standard of the kit. The changes of IL-27 and ADA content of pleural effusion, the changes of serum IL-27 and ADA, the combination of IL-27 and ADA, and the correlation between IL-27 and ADA were compared between the two groups. 【Results】Pleural effusion IL-27 and ADA content of the observation group were higher than those of the control group (P<0.05); the serum levels of IL-27 and ADA were higher than those of the control group (P<0.05); ADA IL-27 diagnostic sensitivity, specificity and accuracy were higher than those of IL-27 and ADA single; tuberculous pleural effusion and IL-27 ADA showed a linear positive correlation. 【Conclusion】IL-27 combined with ADA detection can improve the diagnostic efficiency of tuberculous pleurisy and can be used as an effective indicator for the diagnosis of tuberculous pleurisy.

Analysis of Leukopenia in DCD Recipients after Renal Transplantation

【Objective】To investigate the incidence of leukopenia in DCD (organ donation after cardiac death) recipients after renal transplantation. 【Methods】We retrospectively analyzed the clinical data of 83 DCD recipients from January 2012 to January 2017 who underwent renal transplantation in our hospital. According to whether or not ganciclovir was administered, patients were divided into the observation group (immunosuppressive agents + ganciclovir,n=62) and the control group (immunosuppressive group only,n=21). The occurrences of leukopenia in both groups were statistically analyzed. 【Results】In the observation group, leukopenia occurred in 22 cases (35.48%), including a mild 10 cases (16.13%), a moderate 6 cases (9.68%), a severe 4 cases (6.45%), and a critical 2 cases (3.23%). In the control group, neutropenia occurred in 3 patients (14.29%), including 1 mild case (4.76%), 1 moderate case (4.76%), 1 severe case (4.76%), and 0 critical cases. The incidence of leukopenia in the control group was significantly lower than that in the observation group (χ

=5.152,P=0.015<0.05). Except for one case of critical leukopenia with death, the white blood cells in all other cases returned to normal after treatment. 【Conclusion】The reduction of leukopenia after renal transplantation in DCD recipients is common, and the combined use of immunosuppressants with antiviral drugs is one of the common causes. Most patients can recover to normal levels after symptomatic treatment.

【Objective】 To study the effects of Tripterygium wilfordii multi glucoside tablets on the treatment of elderly IgA nephropathy and its effects on plasminogen activator inhibitor type-1 (PAI-1),Transforming growth factor beta 1 (TGF-β1). 【Methods】According to lottery drawing method, 90 elderly patients of IgA nephropathy were divided into two group, with 45 cases in each. Patients in the control group received routine treatment, while patients in the observation group were treated with glycosides of Tripterygium wilfordii (20mg each time with 3 times a day) in addition to routine treatment of the control group. The two groups were treated for one year. Afterwards, the therapeutic efficacy-levels of urinary protein, SCr (serum creatinine), Alb (plasma albumin) BUN (serum urea nitrogen), PAI-1, and TGF- beta 1-were compared between the two groups. 【Results】 The total effective rate of the observation group was significantly higher than that of the control group(P<0.05). The levels of urine protein content, SCr, BUN, PAI-1 and TGF- beta 1 in the observation group were significantly lower than those in the control group(P<0.05). The level of Alb of the observation group was significantly higher than that of the control group(P<0.05); There was no significant difference in adverse reactions between the two groups (P>0.05). 【Conclusion】Glycosides of Tripterygium wilfordii is effective for treatment of elderly IgA nephropathy. It can improve renal function, reduce urinary protein, delay disease progression and decrease the expression of PAI-1 and TGF- beta 1.

Effects of Ultra-early Alteplase Intravenous Thrombolysis in Patients with Acute Cerebral Infarction and the Influencing Factors of Deterioration after Improvement

【Objective】To analyze the effects of ultra-early alteplase (rt-PA) intravenous thrombolysis in patients with acute cerebral infarction (ACI) and the influencing factors of deterioration after improvement. 【Methods】A total of 70 patients with ACI treated by rt-PA intravenous thrombolysis in our hospital from January 2014 to December 2016 were selected as the observation group, and 90 patients with ACI in the same period who only received conventional treatment were selected as the control group. The status of neurological impairment was evaluated with the National Institutes of Health Stroke Scale (NIHSS) and the prognosis was evaluated with the Barthel index (BI) and modified Rankin scale (mRS). The incidence of complications was statistically analyzed. Furthermore, patients in the observation group were divided into the deterioration group (n=9) and the non-deterioration group (n=71). The clinical data were collected for univariate analysis and factors with significant differences were analyzed by logistic regression analysis. 【Results】Before treatment, there were no significant differences between the two groups in NIHSS score, BI and mRS scores, while after treatment, the above-mentioned indexes were significantly improved and the improvement was more obvious in the observation group than the control group. Multivariable logistic regression analysis showed that age, previous history of atrial fibrillation, late thrombolysis, high NIHSS score before thrombolysis, low white blood cell count and total anterior circulation infarction were independent risk factors for deterioration after improvement. 【Conclusion】Ultra-early rt-PA intravenous thrombolysis can quickly correct the status of cerebral ischemia and hypoxia and save the damaged neurological function in patients with ACI. Attention should be paid to the independent risk factors for deterioration after improvement.

), plasma atrial natriuretic peptide (ANP), brain natriuretic peptide (BNP) and N-terminal B-type brain natriuretic peptide precursor (NT-proBNP)were compared before treatment and 12 h ,24h and 36 h after treatment.【Results】 A total of 32 strains of pathogens was isolated, of which 21 were gram-negative bacilli (65.63%), 7 were gram-positive cocci (21.88%), 4 were fungi (12.50%). 12 hours, 24 hours and 36 hours after treatment, the serum levels of IgA, IgM and IgG were lower than those before treatment, and the level of IgA, IgM and IgG decreased to the lowest point at 12 hours after treatment, and then gradually increased, but still significantly lower than before the treatment, the difference was statistically significant (P<0.05). 12 hours and 24 hours and 36 hours after treatment, the levels of CD3

It can inhibit the immune function and plasma ANP、BNP and NT-proBNP level of patients.

Effect of Half-dose Reteprase Combined with Resuvastatin on Coagulation Status in Patients with Acute ST-segment Elevation Myocardial Infarction

【Objective】To investigate the effect of half-dose reteprase combined with resuvastatin on coagulation status in patients with acute ST-segment elevation myocardial infarction (STEMI).【Methods】A total of 86 patients with acute STEMI admitted in our hospital from October 2016 to October 2017 were randomly divided into two groups: control group (nong43, half dose of reteprase) and observation group (nyang43, half-dose of reteprase combined with resuvastatin), and the cardiac function of the two groups was observed. Coagulation factor and serum factor.【Results】After treatment, left ventricular end-diastolic diameter (LVEDD) and left ventricular end-systolic diameter (LVESD) in the observation group were significantly lower than those in the control group (P<0.05), and the cardiac output (SV) and left ventricular ejection fraction (LVEF) were significantly higher in the observation group than in the control group (P<0.05). After treatment, the levels of hs-CRP, homocysteine (Hcy) and complement 3 (C3) in the observation group were significantly lower than those in the control group (P<0.05). After treatment, the activated partial thromboplastin time (APTT), prothrombin time (PT) and thrombin time (TT) in the observation group were significantly longer than those in the control group (P<0.05), and the level of fibrinogen (FIB) was significantly lower than that in the control group (P<0.05).【Conclusion】Half-dose reteprase combined with resuvastatin in the treatment of acute STEMI can effectively improve the cardiac function and coagulation function of the patients, and reduce the level of serum hs-CRP,Hcy and C3in patients with acute STEMI.

Effects Analysis of Double-J Stent Indwelling Time in Treatment of Urinary Tract Calculi Complicated with Infection

【Objective】To discuss the therapeutic effect of double-J stent indwelling time in treatment of urinary tract calculi complicated with infection. 【Methods】From September 2012 to October 2016, the clinical data of 85 patients with urinary calculi and infection were analyzed. The patients were divided into 3 groups: group A (3~5 d,n=23 ), group B (6~7 d,n=28 ) and group C (>7 d,n=34 ) according to the time of double J catheter indwelling before operation. The patients were divided into two groups: group A (3~5 days), group B (n=28) and group C (>7 days). The clinical data, hematuria and postoperative complications of the three groups were compared and analyzed before and after operation.【Results】There was no significant difference among the three groups in the diameter of stone, the distribution of operation site, the time of operation, the time of stone drainage after operation and the proportion of percutaneous nephrolithotripsy (P>0.05). There was no significant difference in blood, urine white blood cell count (WBC) and positive proportion of urine culture between the three groups before indwelling double J tube (T

Clinical Effect of 30F Large Channel Percutaneous Nephrolithotomy with Holmium Laser Lithotripsy without Detaining Renal Fistulas for the Treatment of Staghorn Calculi

【Objective】To investigate the clinical effect of 30F large channel percutaneous nephrolithotomy with holmium laser lithotripsy without detaining renal fistulas for the treatment of staghorn calculi.【Methods】From April 2013 to November 2016, 60 patients with renal staghorn calculi were divided into observation group and control group with 30 cases in each group. The control group was treated with laparoscopic lithotripsy, the observation group was treated with 30 F large channel percutaneous nephrolithotripsy, and there was no indwelling nephrostomy tube after operation. Blood loss, operation time, hospital stay and pain score were recorded in both groups, and all patients were followed up for 1 month. Serum creatinine (SCR), glomerular filtration rate (GFR) and postoperative complications were compared between the two groups.【Results】The operative time, intraoperative bleeding volume, hospitalization days and pain score in the observation group were significantly lower than those in the control group (P<0.05). There was no significant difference in SCr,GFR between the two groups before operation (P>0.05), but the level of SCr in the observation group was significantly lower than that in the control group (P<0.05), and the level of GFR in the observation group was significantly lower than that in the control group (P<0.05). The stone clearance rate was 93.33% (28/30) in the observation group and 66.67% (20/30) in the control group. The difference was statistically significant (χ

Effect of Mindfulness-Based Cognitive Therapy on Symptoms of Depression and Quality of Life in Patients with Depression

【Objective】To investigate the effect of mindfulness-based cognitive therapy on symptoms of depression and quality of life in patients with depression. 【Methods】A total of 116 cases of patients with depression admitted to hospital from January 2016 to December 2016 were selected as the research subjects, and divided into an observation group and control group via a random number table, with 58 cases in each group. Patients in the control group were given conventional interventions such as drug therapy, health education, psychological intervention and complication prevention, while patients in the observation group received mindfulness-based cognitive therapy on the basis of conventional interventions. After 8 weeks of treatment, the symptoms of depression and quality of life of the patients were compared between the two groups. 【Results】The scores of Hamilton Depression Scale-17 (HAMD-17) and Self-rating Depression Scale (SDS) in the observation group were significantly lower than those in control group (P<0.05), while the scores of physiological function, emotional function, social function and general health were significantly lower than those in control group (P<0.05). 【Conclusion】Mindfulness-based cognitive therapy can relieve depressive symptoms and improve quality of life of patients with depression.

Analysis of IL-6 Levels in Patients with Pneumoconiosis, Tuberculosis, and Pneumoconiosis with Tuberculosis

【Objective】To investigate the levels of interleukin-6 (IL-6) in patients with pneumoconiosis, tuberculosis, and pneumoconiosis complicated with tuberculosis (TB). 【Methods】We selected 50 pneumoconiosis patients (group A), 50 pneumoconiosis patients with tuberculosis (group B), and 50 patients with tuberculosis (group C) who were diagnosed in this hospital from October 2015 to November 2016. At the same time, 50 healthy volunteers were selected as the control group (group D). All enrolled patients were male of age 45~75 years old. Serum IL-6 level were measured and compared among all 4 groups. 【Results】The levels of serum IL-6 in three groups (group A,B and C) were significantly higher than that in the control group (group D) (P<0.01). The levels of IL-6 in three groups of patients showed an increasing trend from group A to group C. There was a significant difference between the groups (P<0.01).【Conclusion】Serum IL-6 level was significantly associated with the pathogenesis of pneumoconiosis. The serum IL-6 level in patients with active tuberculosis is evidently higher than that in pneumoconiosis patients complicated with stable TB. The serum IL-6 level played an important role in the progress of TB and it has a certain auxiliary diagnostic value to determine the recurrence of TB and TB active status.
