2081 Study of the Correlation between the Distribution of Pathogens in Nasopharyngeal and Middle Ear Cavities in Children with Chronic Secretory Otitis Media

【Objective】To analyze the distribution of pathogens and its correlation with middle ear fluid (MEF) and nasopharyngeal secretion (NPS) in children with chronic secretory otitis media (SOM) .【Methods】MEF and NPS secretions were collected from SOM children. The pathogen distribution of NPS and MEF were observed by PCR technique for detection of A.otitidis, S.pneumonia, H.influenzae, M.catarrhails, S.aureus, and S.viridans. The correlation was evaluated.【Results】1) The presence of S.pneumonia or S.aureus in middle earfiuid was significantly positively correlation with the presence of the same species in the nasopharynx (S.pneumoniae, P=0.000, V=0.638; S.aureus, P= 0.000, V=0.761) . Higher prevalence (Sensitivity, specificity and positive predictive values) was found for S.pneumonia and S.aureu S.H.influenzae or M.catarrhails in middle earfiuid correlated positively with the presence of the same species in the nasopharynx, however, the correlation was low (H.influenzae, P=0.021, V=0.258 M.catarrhails, P=0.000, V=0.459) . 2) Alloiococcus otitidis and Streptococcus viridans were not significantly associated with MEF and NPS PCR result S.3) The highest prevalence in NPS was found for H.influenzae (79.63%) , followed by S.aureus (75.93%) and S.pneumonia (75.93%) . The highest prevalence in MEF was found for S.pneumonia (75.93%) and S.aureus (75.93%) .【Conclusion】The results indicate that PCR results from NPS are valuable for prediction of the pathogen presence in MEF in children with SOM.

2084 Effect of Fast Track Surgery on Postoperative Nutritional Support in Patients with Gastric Cancer

【Objective】To analyze the effect of fast track surgery on nutritional support after gastrectomy for gastric cancer, and summarize the application experience.【Methods】A total of 86 elderly patients with gastric cancer treated in our hospital from February 2014 to February 2017 were enrolled in this study and were prospectively analyzed. According to the random number table method, the patients were divided into observation group and control group, 43 cases eacH.The two groups were treated with radical gastrectomy, postoperative routine analgesia and nutritional support were comprehensively intervened. On this basis, the observation group was given nutritional support based on the concept of fast track surgery. The nutritional status, immune function, recovery and complications were compared between the two groups before and after intervention. The guiding value of the concept of fast track surgery in nutritional support was analyzed.【Results】At 7 d after operation, the total protein and albumin of the two groups increased compared with the preoperative 1 d; the total protein and albumin of 7 d after operation in the observation group were higher than those in the control group, the difference was statistically significant (P<0.05) . Compared with the preoperative 1 d, CD3+, CD4+ of 7 d after operation in the observation group increased, CD8+ decreased, the difference was statistically significant (P<0.05) . Postoperative recovery time of bowel sound, anal exhaust time, hospitalization time in the observation group were lower than the control group, the difference was statistically significant (P<0.05) . The incidence of postoperative complications in the observation group was 6. 98%, which was lower than that of the control group (30.23%) , and the difference was statistically significant (P<0.05) .【Conclusion】Postoperative nutritional support based on fast track surgery can significantly improve the nutritional status and immune function of elderly patients with gastric cancer after operation, which has a positive significance for shortening the recovery time and reducing the risk of postoperative complications.

2088 Expression and Significance of miR-375 and Bcl-2 Genes in Breast Cancer

【Objective】To investigate the expression and significance of miR-375 and Bcl-2 genes in breast cancer.【Methods】The expression of miR-375 and Bcl-2 mRNA in 48 cases of breast cancer and 48 cases of adjacent breast cancer tissues were detected by real-time quantitative PCR, The differential expression and correlation of miR-375 and Bcl-2 mRNA in different clinicopathological parameters of breast cancer were analyzed.【Results】Compared with adjacent breast cancer tissues, miR-375 and Bcl-2 mRNA showed significantly lower expression and high expression in breast cancer tissues (P<0.05) , respectively, and the expression of Bcl-2 mRNA was negatively correlated with the expression of miR-375 (r=-0.648, P<0.05) . In addition, the expression of Bcl-2, mRNA and miR-375 was significantly correlated with lymph node metastasis in breast cancer (P<0.05) .【Conclusion】MiR-375 may promote the progression of breast cancer by targeting Bcl-2 mRNA, and affect the prognosis of patients with breast cancer.

2091 CyclinD1 and P16 Expression and the Clinicopathological Significance in Human Nasopharyngeal Carcinoma

【Objective】To investigate the expression of CyclinD1 and P16 in patients with nasopharyngeal carcinoma (NPC) .【Methods】A total of 70 patients with NPC and 55 patients with chronic rhinitis in our hospital from Jan. 2009 to Dec. 2015 were selected. The expression of P16 and CyclinD1 protein in NPC and chronic rhinitis tissue were detected by SP immunohistochemistry, and the gene loss of p16 in patients of NPC was detected by polymerase chain reaction (PCR) .【Results】Positive expression rate of CyclinD1 in the NPC group was higher than that of the chronic rhinitis group; and the difference between the two groups was statistically significant (P<0.05) . Positive expression rate of P16 in the NPC group was lower than that of the chronic rhinitis group; and the difference was statistically significant (P<0.05) . Positive expression rate of CyclinD1 in the T3~T4 stage patients was significantly higher than that of the T1~T2 stage patients (P<0.05) . Positive expression rate of CyclinD1 in the N1~3 stage patients was significantly higher than that of the N0 stages patients (P<0.05) . Positive expression rate of CyclinD1 in the Ⅲ~Ⅳ stage patients was significantly higher than that of the Ⅰ~Ⅱ stage patients (P<0.05) . Positive expression rate of P16 in the T3~T4 stage patients was significantly lower than that of the T1~T2 stage patients (P<0.05) . Positive expression rate of P16 in the N1~3 stage patients was significantly lower than that of the N0 stage patients (P<0.05) . Positive expression rate of P16 in the Ⅲ~Ⅳ stage patients was significantly lower than that of the Ⅰ~Ⅱ stage patients (P<0.05) . The rate of p16 gene loss in NPC (34.3%) was significantly higher than that of the chronic rhinitis group (1.8%) ; and the difference was statistically significant (P<0.05) .【Conclusion】Higher expression of CyclinD1, the loss of p16 gene and the lower expression of P16 were associated with the occurrence and prognosis of NPC.

2095 Effect of Cinobufacini tablets, Erbitux Injection Combined with Chemotherapy on Patients with Advanced Non-small Cell Lung Cancer

【Objective】To investigate the clinical efficacy and quality of life of patients with advanced non small cell lung cancer (NSCLC) treated with Cinobufacini tablets and Erbitux injection combined TP chemotherapy.【Methods】Seventy-three cases of patients with advanced NSCLC were divided into the observation group (group A) and the control group (group B) . The group B was treated with TP (paclitaxel and cisplatin) chemotherapy, and the group A was treated with cinobufacini tablets and Erbitux injection on the basis of treatment in the group B. The short-term efficacy, adverse reactions, and related changes before and after treatment were compared between the two groups.【Results】The total effective rate of the observation group was 59.46% (22/37) , which was significantly higher than 33.33% (12/36) of the control group (P<0.05) , but there was no significant difference in the disease control rate between the two groups (P>0.05) ; the levels of serum interleukin -6 (IL-6) , IL-10, CA125, carcinoembryonic antigen (CEA) and vascular endothelial growth factor (VEGF) in the observation group were significantly lower than those in the observation group after treatment; IL-17, TNF-αlevel was significantly higher than that of the control group, and the difference was significant (P<0.05) ; the improvement rate of survival quality in the observation group was 48.65% (18/37) , which was significantly higher than 25% (9/36) in the control group (P < 0.05) ; the incidence of nausea and vomiting, white blood cell reduction and anemia in the observation group were lower than those in the control group (P<0.05) .【Conclusion】Cinobufacini tablets, Erbitux injection combined with TP chemotherapy can effectively improve the short-term efficacy, regulate immune abnormalities, enhance immune function, and significantly improve the quality of life in patients with advanced NSCLC.

2098 Effect of Roxithromycin on the Proliferation of HaCaT Keratinocytes Induced by IL-22

【Objective】To explore the inhibition of roxithromycin on the proliferation of HaCaT keratinocyte cells induced by interleukin-22 (IL-22) .【Methods】Firstly, HaCaT cell viability was detected by MTT after cells were cultured with IL-22 at concentrations of 0, 12. 5, 25, 50, 100 and 200 μg/mL for 48h. Secondly, HaCaT cell viability was also detected by MTT after cells were cultured with roxithromycin at concentrations of 0, 10, 20, 40, 80, 160 and 320 μg/mL for 48h. Lastly, experiments were divided into the control group (vehicle treatment) , the model group (100 μg/mL IL-22) and roxithromycin groups at low, medium and high-dose groups (100, 300, 1000 μg/mL+100 μg/mL IL-22, respectively) . The cell viability was measured by MTT methodand expressions of cytokeratin16 (K16) , K17, Bax and Bcl-2 were detected by Western blot.【Results】Cell viability of HaCaT was increased by IL-22 at concentrations of 12. 5, 25, 50 and 100 μg/mL. However, the cell viability was decreased by IL-22 at a concentration of 200 μg/mL. Similarly, cell viability was increased by roxithromycin at concentrations of 10、20 μg/mL, but the cell viability was decreased by roxithromycin at concentrations of 40、80、160、320 μg/mL. Compared to the vehicle control group, while the cell viability of IL-22 treated model group was increased, the expression of K16, K17 and Bcl-2 was up-regulated (P<0.01) , the expression of Bax was down-regulated. Compared to the model group, the cell viability in low, medium and high-dose of roxithromycin groups was decreased, and the expression of K17 was down-regulated. The expression of Bax was up-regulated (P<0.01) . The expression of K16 and Bcl-2 was down-regulated in roxithromycin medium and high-dose groups.【Conclusion】Roxithromycin can inhibit IL-22-induced proliferation of HaCaT keratinocyte cells by down-regulating the expression of cytokeratin and regulating the expression of cell apoptotic related proteins.

2101 Clinical Effect of Spermatic Vein Ligation on Varicocele under Modified Outer Ring

【Objective】To analyze the clinical effect of spermatic vein ligation on varicocele under microscopically modified outer ring.【Methods】The clinical data of 100 patients with varicocele from January 2015 to December 2010 in our hospital were analyzed retrospectively. According to the patients' surgical procedure, they were divided into microscope surgery group (modified outer ring) and traditional surgery group (trans-rectal spermatic cord suture) , with 50 cases in each group. The surgical outcomes, the incidence of complications, the recurrence rate and the semen density and viability were compared between the two groups.【Results】The surgeries in all patients of the two groups were successfully completed. The operation time of the microscope group was significantly longer than that of the traditional group (P<0.05) , but the hospitalization time was significantly lower than that of the traditional operation group (P<0.05) . The postoperative VAS score was significantly lower in the microscope group than in the traditional group; the difference was statistically significance (P<0.05) . The recurrence rate of the microscope group was significantly lower than that of the conventional operation group (P<0.05) . The postoperative sperm and viability of the two groups were statistically higher than those before operation (P<0.05) , and the improvement rate of the microscope group was greater than that of the traditional group (P<0.05) . The incidence of postoperative complications in the microscope group was significantly lower than that in the traditional group (4.0% vs 32.0%, respectively; χ2=12. 53, P<0.05) .【Conclusion】The modified spermatic vein ligation in the outer ring has good clinical effect in the varicocele, which can effectively reduce the postoperative complications and recurrence rate, improve the semen density and vitality of the patients, and have the advantages of convenient operation. It is worthy of promotion in clinical use.

2104 Application Value of Transthoracic Echocardiography in Perioperative and Postoperative Infants with Interventional Therapy of Coronary Artery Fistulae

【Objective】To explore the clinical value of transthoracic echocardiography (TTE) in perioperative and postoperative infants with interventional therapy of coronary artery fistulae (CAF) to improve its therapeutic effects.【Methods】Fifteen cases of infant CAF from August 2012 to July 2016 underwent interventional treatment with transcatheter closure techniques in our hospital. TTE was performed before and after the treatmentS.The clinical data were analyzed retrospectively.【Results】Right CAFs were detected in seven patients, among which four had drains to the right ventricles and three to right atrium. Left CAFs were found in eight patients, among which one had drains to the right ventricles and seven to right atrium. Preoperative TTE could show the origin and hole size of CAF to evaluate the heart function. Postoperative TTE could show the location of the closure, whether or not there were residual shunts, blood clots in coronary artery and heart function improvement.【Conclusion】Echocardiography has high application value in preoperative screening of infant CAF, timing of surgery and the postoperative evaluation.

2107 Clinical Value of Two-dimensional Combined with Four-dimensional Ultrasound in Prenatal Diagnosis of Fetus Congenital Heart Malformation

【Objective】To explore the clinical value of two-dimensional combined with four-dimensional ultrasound in prenatal diagnosis of fetus congenital heart malformation.【Methods】From September 2011 to September 2015, 4642 cases of the middle and late pregnant women who received prenatal examination were randomly selected from in our hospital. Prenatal examination of pregnant women was performed by two-dimensional ultrasound at first, then, the fetuses with suspected malformation under two-dimensional ultrasound were examined by four-dimensional ultrasonography again. By analyzing and comparing the two methods of two-dimensional ultrasound and two-dimensional combined with four-dimensional ultrasound, the clinical value of the two methods was determined by the different accuracy of fetal malformation diagnosis.【Results】By follow-up, congenital heart malformations were found in 89 fetuses out of 4681 fetuses, the incidence rate was 1.9%. In 89 fetal malformations, 70 fetuses with congenital heart malformations were examined by two-dimensional ultrasound. Among them, 16 cases were missed or misdiagnosed; a total of 78 fetuses with congenital heart malformations were examined by two-dimensional ultrasound combined with four-dimensional ultrasound, and 11 cases were missed or misdiagnosed. The diagnostic accuracy of two-dimensional ultrasound combined with four-dimensional ultrasonography was significantly higher than that of two-dimensional ultrasound (P<0.05) .【Conclusion】Ultrasonography is an effective method for prenatal diagnosis of fetal congenital heart malformations. Four dimensional ultrasonography can be a useful supplement to two-dimensional ultrasound. Combination of two kinds of ultrasound examination can significantly improve the accuracy of diagnosis of fetal congenital heart malformations, and effectively reduce the incidence of misdiagnosis and leakage.

2110 Synergistic Effect of Dexmedetomidine (Dex) Combined with Flurbiprofen Axetil on Postoperative Hyperalgesia Induced by Remifentanil

【Objective】To analyze the synergistic effect of dexmedetomidine (Dex) combined with flurbiprofen axetil on postoperative hyperalgesia induced by remifentanil.【Methods】One hundred patients scheduled for gynecological laparoscopic surgery were randomly divided into three groups. Remifentanil anesthesia was used in all patients. Before anesthesia induction, the group A was not given drugs, the group B received Dex infusion and the group C received Dex combined with flurbiprofen intravenous injection. Mechanical pain threshold around umbilicus, the static visual analogue scale (VAS) , 24h sensitive area around the umbilicus after operation, the dosage of sufentanil and the incidence of adverse reactions at different time points were compared among the groups.【Results】The mechanical pain thresholds around umbilicus of 1h, 6h and 24h in the group and B and the group C were significantly higher than those in the group A after operation (P<0.05) . At postoperative 0.5h, 1h, 6h, 24h, resting VAS, seven halothane concentration, postoperative 24h sufentanil dosage and the sensitive area of umbilicus pain were significantly lower than those in the group A (P<0.05) ; the above indexes in the group C were significantly better than those in the group B (P<0.05) . The incidence of postoperative hypotension and bradycardia in the group B and C were significantly higher than those in the group A; nausea, vomiting and the incidence of postoperative shivering were significantly lower than those in the group A (P<0.05) , but there was no significant difference between the group B and C (P>0.05) .【Conclusion】Dex is good at preventing postoperative hyperalgesia induced by remifentanil. It can significantly reduce the dosage of remifentanil, but is prone to cause hypotension and bradycardia. Its combination with flurbiprofen can not only enhance the synergistic effects in reducing postoperative hyperalgesia but also decrease the adverse reactions.

2113 A Clinical Efficacy Analysis of Traumatic Type C Distal Radius Fractures Treated by Double Column Universal Volar Locking Compression Plates

【Objective】To investigate the application of Double Column Universal Volar Locking Compression Plates in the treatment of traumatic type C fractures of the distal radius.【Methods】From June 2012 to April 2016, 96 patients with traumatic type C (AO type) and closed fractures of the distal radius were studied. The total selected study subjects included 38 cases of sub-type C1, 39 cases of sub-type C2 and 19 cases of sub-type C3. All type C fractures of the distal radius were treated with Double Column Universal Volar Locking Compression PlateS.The parameters were measured with imaging, and the Gartland-Werley Wrist Score was used to assess the operative effect.【Results】All of the patients healed during the follow-up period of 9~36 months (mean 21. 36 months) . Compared to the status of pre-operation, the parameters of volar tilt angle, ulnar inclination angle and radial shortening - all improved significantly. The postoperative functional excellence rate was 87.5% (84/96) . Postoperative complications included 10 cases of wrist pain tenderness, 5 cases of finger flexion and extension restriction, 4 cases of osteoarthritis and 2 cases of flexor tendon adhesion. There were no internal fixation failures, tendon raptures or nerve injuries.【Conclusion】Using the Double Column Universal Volar Locking Compression Plates in treatment of type C distal radius fractures has bettered the height of radius, reinforced fixative effects, decreased instances of complications, and allows for successful postoperative functional recovery.

2116 Predictive Value of Shear Wave Elastography and Serum Markers of Hepatic Fibrosis in Hepatic Fibrosis of Infantile Hepatitis Syndrome

【Objective】To investigate the value of shear wave elastography (SWE) and serum markers of hepatic fibrosis in predicting hepatic fibrosis in infantile hepatitis syndrome (IHS) .【Methods】Sixty infants with IHS diagnosed in our hospital from March 2016 to May 2017 were selected as the subjects, According to the staging criteria of chronic hepatitis fibrosis, it is divided into S1~4 stage, At the same time, 20 healthy infants were selected as control group. Liver stiffness (LS) was detected by SWE, and serum markers of liver fibrosis were detected in children.【Results】Age, hemoglobin (HB) , total bilirubin (TB) , hyaluronic acid (HA) , type Ⅳ collagen (Ⅳ -C) , LS were significantly correlated with the degree of liver fibrosis (P<0.05) , and HB was negatively correlated with liver fibrosis, and the others were significantly positively correlated. There was a significant positive correlation between HA, Ⅳ-C, LS and liver fibrosis (P<0.01) . Aspartate aminotransferase (AST) , alanine aminotransferase (ALT) and gamma glutamyl transferase (GGT) were not significantly correlated with liver fibrosis stages (P>0.05) . Compared with HA and Ⅳ-C, the detection of LS with SWE showed the highest curve area in mild fibrosis, moderate fibrosis and severe fibrosis analysis, followed by 0.866, 0.904 and 0.893. In the ROC curve analysis of mild fibrosis, the area under the HA curve (0.837) was greater than that of Ⅳ-C (0.716) , In the ROC curve of moderate fibrosis and high fibrosis, the area under the Ⅳ-C curve was 0.812 and 0.854, respectively, which were higher than those of HA (0.722 and 0.830) respectively.【Conclusion】SWE and serum markers of liver fibrosis (HA and Ⅳ-C) have good diagnostic efficacy in infants with IHS liver fibrosis, and SWE is superior to serum markers of liver fibrosis.

2120 Clinical Analysis and Review of 14 Cases with Sinonasal Neuroendocrine Carcinoma

【Objective】To investigate the clinical performance, pathological characteristics, treatment and prognosis of sinonasal neuroendocrine carcinoma.【Methods】A retrospectively study was carried out among 14 cases with this tumor treated in the First Affiliated Hospital of Army Medical University from October 2007 to August 2015 through a careful analysis on their clinical data, the clinical characteristics and treatment of the disease was studied on reviewing the relevant literature.【Results】All of the 14 cases were confirmed pathologically as small cell neuroendocrine carcinoma: 3 cases were lost to follow-up, 1 case died 1 month after treatment; 1 case had liver metastasis 5 months after treatment, no treatment, and died after 11 months of follow-up; Tumor bearing survival in 4 cases; Local recurrence occurred in 1 case after 11 months of treatment and had no treatment. The patient was followed up for 109 months and the condition was stable; Local recurrence occurred in 1 case 60 months after treatment, and was followed up for 67 months, now in the local radiotherapy; Brain metastasis occurred in 1 patient after 32 months of treatment, followed by cranial radiotherapy and chemotherapy. The patient was followed up for 44 months and the condition was stable; 1 case was followed up for 24 months, the condition was stable, no tumor survived in 5 cases, followed up for 24~89 months, and did not suggest local recurrence and distant metastasis of tumor.【Conclusion】The incidence of this disease is low, the diagnosis mainly depends on histopathologic manifestations, immunohistochemical results and ultrastructure under electron microscope characteristics; it should be differentiated from the poorly differentiated squamous carcinoma, melanoma and olfactory nerve blastoma and neurospongioma; Early diagnosis, tumor staging, extent of invasion, with or without surgery and combination of chemotherapy and radiotherapy are the prognostic factors of the disease.

2124 Relationship between 1h Post-Load Plasma Glucose Elevation and Carotid Intima-Media Thickness in Obese Women with Normal Glucose Tolerance

【Objective】To explore the relationship between 1h post-load plasma glucose (1hPG) elevation and carotid intima-media thickness (CIMT) in obese women with normal glucose tolerance (NGT) .【Methods】A total of 186 women who underwent physical examination in our hospital (May to December, 2014) were collected for the study. They were 18-57 years old with NGT. According to the body mass index (BMI) , all cases were divided into normal (n=82) , overweight (n=63) and obese (n=41) groups. All subjects underwent an oral glucose tolerance test (OGTT) and CIMT. The levels of blood glucose and insulin (INS) were compared among the three groups. Furthermore, the correlation between 1hPG and CIMT was analyzed.【Results】Systolic blood pressure (SBP) , triglyceride (TG) and CIMT in the obese group were higher than those in the normal group (P<0.05) . Insulin resistance index (HOMA-IR) in the obese group was higher than that in the overweight group (P<0.05) . Serum 1hPG, 0hINS and 1hINS levels in the obese group were significantly higher than those in the normal group, and serum 0hINS and 1hINS levels were significantly higher than those in the overweight group. All differences were statistically significant (P<0.05) . Other parameters (0.5hPG, 1 hPG, 2hPG, 3hPG, 0.5hINS, 2hINS and 3hINS) in the three groups were not statistically significant. CIMT was correlated with BMI (r=0.316, P=0.001) , fasting plasma glucose (FPG) (r=0.328, P<0.328) , and 1hPG (r=0.263, P=0.005) , (all P<0.05) . Multiple linear regression showed that 1hPG, age and BMI had a significant positive correlation with CIMT (P<0.05) .【Conclusion】Obese women with normal glucose tolerance have 1hPG, 1hINS and insulin resistance. 1hPG is an independent risk factor for carotid intima-media thickening (CIMT) .

2127 Comparison of Butorphanol and Oxycodone for Patient-controlled Intravenous Analgesia Effect after Transurethral Ureteroscopic Lithotripsy

【Objective】To compare the effects of butorphanol and oxycodone in patient-controlled intravenous analgesia after transurethral ureteroscopic lithotripsy (URL) .【Methods】Two hundred and fourteen patients undergoing URL were divided into butorphanol group (group A, n=107) and oxycodone group (group B, n=107) according to the different anestheticS.The analgesic effects were compared between the two groups.【Results】The excellent rate of anesthesia, operation time and duration of anesthesia showed no statistically significant difference between the two groups (P>0.05) . 3h, 6h, 12h and 24h after postoperative patient-controlled intravenous analgesia, the pain score of the group B was significantly lower than that of the group A, the sedation score was significantly higher than that of group A, and the difference was significant (P<0.05) . The anesthetic consumption, score of Post Operative Nausea And Vomitting (PONV) of the group B were significantly less or lower than those of the group A (P<0.05) . The anesthesia satisfaction of B group was 93.46% while of A group was 87.85% (P>0.05) . The anesthesia satisfaction of the group B was 93.46%, higher than that of the group A (87.85%) (P>0.05) ; the incidence of adverse reactions in the group A was 14.02% while that in the group B was 5.61% (P<0.05) .【Conclusion】General anesthesia effects of butorphanol and oxycodone for URL anesthesia are similar, but oxycodone has a relatively small amount and good analgesic effect with less adverse reaction, and is more suitable than butorphanol for URL intrathecal anesthesia.

2130 Application of Pituitrin-Water Separation in Laparoscopic Ovarian Endometriosis Cystectomy

【Objective】To investigate the application of pituitrin-water separation in laparoscopic ovarian endometriosis cystectomy.【Methods】From March 2010 to September 2016, 167 cases of laparoscopic ovarian cystectomy in our hospital were selected as the research objects and were randomly divided into observation group (85 cases) and control group (82 cases) . Patients in the observation group were treated with pituitrin-water separation during the operation, and the patients in the control group were given routine surgery. The clinical efficacy of the two groups and the influence of operation on ovarian function were observed.【Results】The operation time and the amount of bleeding in the observation group were significantly less than those in the control group (P<0.05) ; compared with preoperative, the levels of follicle stimulating hormone (FSH) , luteinizing hormone (LH) , estradiol (E2) and antral follicle (Fo) were decreased 1 month after operation. At 6 months after operation, the levels of LH and E2 in the observation group were increased, and the levels of FSH, LH and E2 in the control group were all increased (P<0.05) ; the levels of FSH, LH, E2 and Fo in the observation group at 1 month after operation were lower than those in the control group 1 months after operation. At 6 months after operation, the FSH of the observation group was lower than that of the control group at 6 months after operation, and the levels of LH, E2 and Fo were higher than those of the control group at 6 months after operation (P<0.05) .【Conclusion】Laparoscopic resection of ovarian endometriotic cyst with diluted pituitrin-water separation reduces the difficulty of operation, shortens the operation time, reduces the amount of bleeding, and effectively protects the function of the ovary, which has certain clinical value.

2133 Morphology Changes of Menorrhagia Ovulatory Dysfunctional Uterine Patients in Hysteroscopy and Therapeutic Effect of Selective Endometrial Resection

【Objective】To investigate the morphology changes of menorrhagia ovulatory dysfunctional uterine patients in hysteroscopy and therapeutic effect of selective endometrial resection.【Methods】From January 2013 to January 2015, 120 cases of menorrhagia DUB patients in our hospital were selected as the observation group, and 80 normal menstrual volunteers were selected as the control group, hysteroscopy was used to observe the endometrial characteristics of the two groupS.After the examination, according to the random number table method, the observation group was divided into treatment group 1 and treatment group 2, 60 cases in each group. The groups 1 was treated with selective endometrial resection under hysteroscopy. And the group 2 was treated with traditional endometrial resection. The therapeutic effect and complication rate of the two groups were observed.【Results】In the observation group, the endometrium showed focal changes, and the average glandular area in irregular zone of endometrium was (l4567. 7 ± 3029. 1) μm2, which was significantly higher than that of the control group (9983. 6±3145. 5) μm2, and the difference was statistically significant (P<0.05) ; The average density of glandular cells in the irregular endometrium of the observation group was (1.41±0.79) , which was significantly lower than that of the control group (1.89±0.64) , and the difference was statistically significant (P<0.05) ; The cross-sectional area of spiral arterioles in the irregular endometrium was 398. 7 ± 84. μm2 in the observation group, which was significantly higher than that in the control group (321.8± 102. 8μm2) , and the difference was statistically significant (P<0.05) . The expression of estrogen receptor (ER) in the irregular endometrium of the observation group was (1.68 ±0.37) points, which was significantly higher than that of the control group (1.36±0.40) , and the difference was statistically significant (P<0.05) . Progesterone receptor (PR) expression score was (1.91±0.32) points, and compared with the control group (1.82 ±0.41) points, the difference was not statistically significant (P>0.05) . The two groups were treated by hysteroscopy, and the effective rate of the group 1 (selective endarterectomy) was 78.33%, the effective rate of the group 2 (traditional surgery) was 90%, and the effective rate of the group 1 was slightly lower than that of the treatment group 2 (χ2 = 3.0641, P=0.0800) , but there was no statistical significance. The average operation time, the number of postoperative amenorrhea, and the intrauterine adhesions after 6 months of treatment in the 1 groups were significantly better than those in the treatment group 2, the difference was statistically significant (P<0.05) .【Conclusion】Patients of menorrhagia ovulatory dysfunctional uterine show focal change in hysteroscopy, irregular thickening of endometrium is the main pathological change area causing menorrhagia. Hysteroscopic selective endometrial resection has the same therapeutic effect as traditional surgery, and the postoperative complications are low, which is an effective way to improve the traditional endometrial resection.
