LI Xiaoxia, XIA Huan, YAN Run, et al
DOI: 10.3969/j.issn.1671-7171.2025.08.003
【Objective】 To compare the efficacy and safety of levodopa-benserazide and compound carbidopa in the treatment of patients with early Parkinson's disease (PD).【Methods】 A total of 180 PD patients were randomly divided into the observation group and the control group, with 90 cases in each group. The control group was treated with levodopa-benserazide, while the observation group was treated with compound carbidopa. The efficacy, incidence of adverse reactions, and changes in scores of Part Ⅱ (daily activities, UPDRSⅡ) and Part Ⅲ (motor function, UPDRSⅢ) of the Unified Parkinson's Disease Rating Scale (UPDRS) and Parkinson's Disease Questionnaire-39 (PDQ-39) before and after treatment were compared between the two groups.【Results】After 12 weeks of treatment, there was no significant difference in the total effective rate between the two groups (P>0.05). From pre-treatment to 12 weeks after treatment, the scores of UPDRSⅡ, UPDRSⅢ, and PDQ-39 in both groups gradually decreased with the extension of time, and the differences at each time point were statistically significant (P<0.05). There were no significant differences in UPDRSⅡ, UPDRSⅢ, and PDQ-39 scores between the two groups before and at each time point after treatment (P>0.05). The total incidence of adverse reactions in the observation group was lower than that in the control group (P<0.05).【Conclusion】The efficacy of levodopa-benserazide tablets and compound carbidopa in the treatment of early PD is comparable, however, compound carbidopa has higher safety.

DOI: 10.3969/j.issn.1671-7171.2025.08.005
【Objective】 To investigate the efficacy of personalized 3D-printed surgical guide plate-assisted localization of hematoma puncture and drainage in patients with cerebral hemorrhage.【Methods】A total of 92 patients with cerebral hemorrhage were randomly divided into two groups, with 46 cases in each. The observation group underwent hematoma puncture and drainage with the assistance of a personalized 3D-printed surgical guide plate, while patients in the control group received CT-guided hematoma puncture and drainage. Surgical parameters, postoperative quality of life (measured by the SF-36 scale), neurological function [National Institutes of Health Stroke Scale (NIHSS) score], and incidence of postoperative rebleeding and secondary cerebral edema were compared between the two groups.【Results】The observation group had a shorter localization time and smaller residual hematoma volume compared with the control group (P<0.05). The rates of first-attempt puncture success, hematoma clearance, and correct drainage tube placement in the observation group were significantly higher than those in the control group (P<0.05). One week after surgery, the SF-36 total score in the observation group was higher than that in the control group (P<0.05), and at six months postoperatively, all SF-36 dimensions and total scores were significantly higher in the observation group (P<0.05). Six months after surgery, the NIHSS score of the observation group was lower than that of the control group (P<0.05). The incidences of postoperative rebleeding and secondary cerebral edema were significantly lower in the observation group (P<0.05).【Conclusion】Personalized 3D-printed surgical guide plate-assisted localization in hematoma puncture and drainage can effectively improve intraoperative localization accuracy, shorten localization time, increase first-attempt success rate, enhance neurological function and quality of life in patients with cerebral hemorrhage, and reduce postoperative complications. It is worthy of clinical application and promotion.

XIAN Yaoyao, OU Jianghong, HUANG Wei, et al
DOI: 10.3969/j.issn.1671-7171.2025.08.007
【Objective】To analyze the clinical and chest high-resolution computed tomography (HRCT) features of rheumatoid arthritis (RA) patients complicated with Mycobacterium tuberculosis infection.【Methods】The clinical data of 68 RA patients complicated with Mycobacterium tuberculosis infection who were diagnosed and treated in Changsha Central Hospital from August 2023 to August 2024 were retrospectively analyzed, and their clinical and chest HRCT features were analyzed. 【Results】The main clinical manifestations of the 68 RA patients complicated with Mycobacterium tuberculosis infection were cough (41.18%), joint pain (38.24%), fever (27.94%), weight loss and fatigue (8.82%). Among them, 44 cases (64.71%) had active tuberculosis, and 24 cases (35.29%) had latent Mycobacterium tuberculosis infection. In RA patients complicated with Mycobacterium tuberculosis infection, the infection was mainly in the lungs (pulmonary tuberculosis), accounting for 83.82% (57/68); extrapulmonary tuberculosis was mainly joint tuberculosis (5.88%, 4/68); the remaining 7 cases were Mycobacterium tuberculosis-infected patients without detectable infection foci. The HRCT of 57 patients with lung involvement showed that the lesions of 6 patients were located in the typical predilection sites of pulmonary tuberculosis (i.e., the apical-posterior segment of the upper lobe or the dorsal segment of the lower lobe), the lesions of 31 patients were scattered in both lungs, the lesions of 9 patients were located in the upper lobes of both lungs, the lesions of 4 patients were located in the middle-upper lobes of both lungs, the lesions of 2 patients were located in the lower lobes of both lungs, the lesions of 2 patients were located in the upper and lower lobes of the right lung, the lesions of 1 patient were located in the three lobes of the right lung, and the lesions of 2 patients were located in the upper and lower lobes of the left lung. The chest HRCT features of the 57 patients with lung involvement were diverse. Only 4 patients showed a single HRCT feature, while most patients presented multiple HRCT features. Among them, the proportion of patients with patchy shadows, fibro-cord shadows and nodular shadows was relatively high. 【Conclusion】RA patients complicated with Mycobacterium tuberculosis infection have diverse clinical manifestations, and the infection site is mainly in the lungs. In addition, patients with lung involvement show diverse features on chest HRCT, which are prone to misdiagnosis and missed diagnosis.

Predictive Value of IL-6/STAT3 Signaling Pathway-Related Factors for Restenosis after Endovascular Therapy in Lower Extremity Arteriosclerosis Obliterans
YANG Fan, ZHANG Yu, ZANG Sheng
DOI: 10.3969/j.issn.1671-7171.2025.08.013
【Objective】To investigate the predictive value of interleukin-6 (IL-6)/signal transducer and activator of transcription 3 (STAT3) signaling pathway-related factors for restenosis after endovascular therapy in patients with lower extremity arteriosclerosis obliterans (LEASO). 【Methods】A total of 125 LEASO patients admitted to our hospital between January 2020 and January 2023 were enrolled. Patients were divided into poor-prognosis group (vascular restenosis >50%, n=41) and good-prognosis group (n=84) based on follow-up outcomes. Serum levels of IL-6, IL-6 receptor (IL-6R), Janus kinase 2 (JAK2), STAT3, vascular endothelial growth factor (VEGF), and nitric oxide (NO) were measured. Logistic regression analysis was used to identify risk factors for restenosis. Receiver operating characteristic (ROC) curves evaluated the predictive efficiency of the IL-6/STAT3 pathway. 【Results】 Poor-prognosis patients showed higher total cholesterol (TC) levels and lower ankle-brachial index (ABI) compared to good-prognosis patients (all P<0.05). Serum IL-6, IL-6R, JAK2, STAT3, and VEGF levels were significantly elevated, while NO levels were decreased in the poor-prognosis group (mean P<0.05). Multivariate analysis revealed high IL-6, IL-6R, JAK2, STAT3, VEGF levels and low NO levels as independent risk factors for restenosis (P<0.05). The ROC curve showed that the combined detection of IL-6, IL-6R, JAK2, and STAT3 had high diagnostic efficacy for the prognosis of patients with lower extremity arteriosclerosis obliterans who underwent interventional therapy.【Conclusion】IL-6/STAT3 pathway-related factors are closely associated with restenosis risk after endovascular therapy in LEASO patients. Elevated IL-6, IL-6R, JAK2, and STAT3 levels indicate higher restenosis probability.

YANG Yuchen, FANG Wei
DOI: 10.3969/j.issn.1671-7171.2025.08.017
【Objective】To evaluate the predictive value of serum creatinine (Scr), cystatin C (cys C), and blood urea nitrogen (BUN) for acute kidney injury (AKI) in patients with severe acute pancreatitis (SAP).【Methods】 A total of 89 patients with SAP were enrolled and divided into two groups based on the occurrence of AKI: the AKI group (33 cases) and the non-AKI group (56 cases). Clinical data, Scr, cys C, and BUN levels were compared between the two groups. Univariate and multivariate logistic regression analyses were performed to identify risk factors for SAP complicated by AKI. Receiver operating characteristic (ROC) curves were used to analyze the predictive efficiency of Scr, cys C, and BUN.【Results】 Levels of Scr, cys C, and BUN were significantly higher in the AKI group compared to the non-AKI group (P<0.05). Univariate analysis showed that APACHE Ⅱ score, blood lactate, Ranson score, abdominal compartment syndrome (ACS), systemic inflammatory response syndrome (SIRS), shock, mechanical ventilation, C-reactive protein, procalcitonin, and serum calcium were associated with AKI occurrence (P<0.05). Multivariate logistic regression revealed that high APACHE Ⅱ score, high Ranson score, low serum calcium, elevated Scr, cys C, and BUN were independent risk factors for AKI in SAP patients (P<0.05). ROC curve analysis demonstrated that the combination of APACHE Ⅱ score, Ranson score, serum calcium, Scr, cys C, and BUN had a higher predictive value than any single indicator.【Conclusion】 Scr, cys C, and BUN levels are elevated in SAP patients with AKI. When the levels of Scr, cys C, and BUN combined with APACHE Ⅱ score, Ranson score, and serum calcium, they offer an even better predictive value for early identification of AKI in SAP.

DONG Jin, SHAN Fuqiang, LI Huimin
DOI: 10.3969/j.issn.1671-7171.2025.08.021
【Objective】To investigate the prognostic value of combined detection of plasma D-dimer (D-D) and peripheral blood neutrophil-to-lymphocyte ratio (NLR) in patients with community-acquired bacterial liver abscess (CABHA).【Methods】 Plasma D-D and peripheral blood NLR levels were measured in 94 CABHA patients. Based on 6-month follow-up outcomes, patients were divided into a good prognosis group and a poor prognosis group. Factors influencing prognosis were analyzed, and the predictive value of combined D-D and NLR detection was evaluated.【Results】Among the 94 patients, 75 (79.79%) had good prognosis and 19 (20.21%) had poor prognosis. Logistic regression analysis showed that a history of diabetes, multiple abscesses, maximum abscess diameter >5 cm, and elevated serum D-D and NLR levels were all risk factors for the poor prognosis in CABHA patients (P<0.05). Receiver operating characteristic (ROC) curve analysis showed that the area under the curve (AUC) for predicting prognosis using D-D alone, NLR alone, and combined both D-D and NLR detection was 0.734, 0.726, and 0.797, respectively, with combined detection showing significantly better predictive performance than single indicator(Z=7.104, P<0.05).【Conclusion】 Plasma D-D and peripheral blood NLR levels can be used to evaluate the prognosis of patients with CABHA.

【Objective】To explore the efficacy of two different approaches of hip arthroplasty in the treatment of patients with ankylosing spondylitis (AS) complicated by hip joint lesions and their impact on serum inflammatory factor levels. 【Methods】A total of 86 patients with AS complicated by hip joint lesions who underwent hip arthroplasty in our hospital from January 2020 to January 2024 were selected. According to the different surgical approaches, they were divided into the observation group (anterior approach) and the control group (posterolateral approach), with 43 cases in each group. The Visual Analogue Scale (VAS) score, hip function score [Harris Hip Score (HHS) system], Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score, serum inflammatory factor [interleukin-1 (IL-1), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α)] levels, and hip abduction and flexion angles were compared between the two groups before and after treatment. 【Results】After treatment, the WOMAC score and VAS score of the observation group were lower than those of the control group (P<0.05). After treatment, the levels of inflammatory factors in the observation group were lower than those in the control group, and the HHS score was higher than that in the control group (P<0.05). At 1 month and 3 months after treatment, the hip abduction angle and hip flexion angle of the observation group were larger than those of the control group, and the differences were statistically significant (P<0.05). 【Conclusion】Compared with posterolateral approach hip arthroplasty, anterior approach hip arthroplasty has a more significant effect on improving hip function, reducing pain scores and lowering serum inflammatory factor levels in patients with AS complicated by hip joint lesions.
