177 Research on the Current Situation of Refine Drug Management in Hospitals Based on CiteSpace Analysis

【Objective】To analyze the current status of refined drug management in hospitals based on CiteSpace software analysis.【Methods】CiteSpace software was used to visually analyze relevant domestic literature, explore the trends and hotspots of hospital drug refinement management under the background of the new medical reform. The five aspects: authors, research institutions, high-frequency keywords, keyword clustering, and keyword emergence diagram were analyzed.【Results】Refined management of drugs improved the quality and safety of drugs, reduce medical costs, and improve the efficiency and quality of medical services. At the same time, it also enhanced patients' trust and satisfaction with medical services and promoted the harmonious development of doctor-patient relationships.【Conclusion】Through literature analysis of the past 14 years in the field of refined drug management in China, drug management has evolved from an initial focus on economic benefits, cost accounting, and drug procurement strategies, to a gradual focus on outpatient pharmacy management and drug inventory management, as well as a later focus on antimicrobial drug management, financial management, and overall hospital operation at multiple levels of research and hot topics. At the same time, it is predicted that in the future, refined drug management will pay more attention to controlling medical expenses, optimizing hospitalization expenses, conducting personalized treatment strategies based on disease diagnosis, and improving work efficiency.

181 Reliability and Validity Evaluation in Obese Adolescents of Chinese Version of Depression-Anxiety-Stress Scale

【Objective】To evaluate the reliability and validity of the Chinese version of depression, anxiety and stress scale (DASS-42) in obese adolescents.【Methods】A convenient sampling method was adopted to select 120 obese adolescents who visited the nutrition clinic of our hospital from January to September 2024. The Chinese version of DASS-42 was used to assess the depression, anxiety or stress of obese adolescents. 20 adolescents were randomly selected for re-evaluation 2 weeks later.【Results】The Cronbach's α coefficients of depression, anxiety and stress in Chinese version of DASS-42 were 0.966, 0.974 and 0.974, respectively, and the retest reliability of the scale was 0.891. Confirmatory factor analysis showed that the Chinese version of DASS-42 had a good fit index (χ2/df=3.048, CFI=0.988, TLI=0.980, RMSEA=0.058, SRMR=0.009).【Conclusion】The Chinese version of DASS-42 has good reliability and validity in adolescent obese patients, and can be used as an evaluation tool related to depression, anxiety and stress.

184 Expression of LncRNA SNHG3 and LncRNA SNHG15 in Non-Small Cell Lung Cancer Tissues and Relationship with Pathological Features and Prognosis

【Objective】To investigate the relationship of the expression of long non-coding RNA (LncRNA) small nucleolar RNA host gene 3 (SNHG3) and LncRNA SNHG15 in non-small cell lung cancer (NSCLC) tissues with the pathological characteristics and prognosis of patients.【Methods】The clinical data of 179 patients with NSCLC who underwent radical resection for lung cancer in our hospital from January 2017 to September 2021 were retrospectively analyzed, and the disease-free survival of the patients was recorded after 3 years of postoperative follow-up. Real-time fluorescent quantitative PCR was used to detect and compare the expression of LncRNA SNHG3 and LncRNA SNHG15 in cancer tissues and adjacent tissues of patients, and analyze the relationship of the expression of LncRNA SNHG3 and LncRNA SNHG15 in cancer tissues with the pathological characteristics and prognosis of patients with NSCLC.【Results】The relative expression levels of LncRNA SNHG3 and LncRNA SNHG15 in cancer tissues were higher than those in cancer adjacent tissues (P<0.05). Cox regression analysis showed that pathological stage, differentiation degree, and levels of LncRNA SNHG3 and LncRNA SNHG15 expression were prognostic factors of NSCLC patients (P<0.05). The results of receiver operating characteristic (ROC) curve analysis showed that the sensitivity, specificity and area under ROC curve (AUC) of the combined relative expression level for both LncRNA SNHG3 and LncRNA SNHG15 in predicting the prognosis of NSCLC patients were higher than those predicted by LncRNA SNHG3 and LncRNA SNHG15 alone. The survival curves of LncRNA SNHG3 and LncRNA SNHG15 patients with low expression were better than those with high expression (P<0.05).【Conclusion】The expression of LncRNA SNHG3 and LncRNA SNHG15 in cancer tissues of patients with NSCLC is related to pathological characteristics and prognosis. The combination of both LncRNA SNHG3 and LncRNA SNHG15 is more effective in predicting prognosis of patients with NSCLC.Those with low expression of LncRNA SNHG3 and LncRNA SNHG15 have lower risk of recurrence and metastasis after treatment.

189 Analysis of Clinical Characteristics of 15 Cases of Neonatal Fungal Meningitis

【Objective】To explore the clinical characteristics, diagnosis, treatment and prognosis of neonatal fungal meningitis.【Methods】A retrospective analysis was conducted on the clinical data of 15 neonates with fungal meningitis admitted to the Neonatal Department of Hunan Children's Hospital from January 2018 to December 2022. The onset characteristics, clinical manifestations, laboratory tests, treatment and prognosis were summarized.【Results】Among the 15 cases of neonatal fungal meningitis, 11 cases were preterm infants (73.3%), 1 case was extremely low birth weight infant (6.7%), 4 cases were very low birth weight infants (26.7%), and 7 cases were low birth weight infants (46.7%). The age of onset was ≥ 9 days. All 15 cases had received broad-spectrum antibiotics before diagnosis. Twelve cases (80%) received peripherally inserted central catheter (PICC) catheterization, 11 cases (73.3%) received intravenous nutrition, 5 cases (33.3%) received invasive mechanical ventilation, and 10 cases (66.7%) received non-invasive ventilation. Another 5 cases were diagnosed based on positive blood fungal culture and cerebrospinal fluid white blood cell count > 20×106/L. The main treatment was antifungal therapy with fluconazole, amphotericin B, and flucytosine. Eight cases were cured (53.3%), five cases improved (33.3%), and two cases died.【Conclusion】Fungal meningitis in neonates is common in premature infants and low birth weight infants, mainly secondary to the irrational use of broad-spectrum antibiotics and various invasive procedures. The clinical symptoms are similar to those caused by other pathogenic bacteria. Fluconazole can be the first choice for antifungal drugs, and amphotericin B combined with flucytosine can be used when the treatment effect is poor.

193 Clinical Pathological Characteristics and Prognostic Impact of Primary TP53 Mutated Gastric Cancer

【Objective】To investigate the clinical pathological characteristics and prognostic impact of patients with primary TP53 mutated gastric cancer.【Methods】This retrospective study analyzed the clinical data of 68 patients with TP53 mutated gastric cancer (mutation group) and 102 patients with TP53 wild-type gastric cancer (wild-type group). The clinical pathological characteristics of the two groups were compared. Survival curves were plotted using the Kaplan-Meier method, and overall survival (OS) and disease-free survival (DFS) were compared using the Log-rank test. Univariate analysis and Cox regression models were employed to explore the independent factors affecting the prognosis of gastric cancer patients.【Results】Patients in the mutation group with age≥55 a significantly higher proportion of poorly differentiated tumors, advanced (Stage Ⅲ+Ⅳ) TNM staging, lymph node metastasis, and larger tumor size compared to the wild-type group (P<0.05). Survival analysis showed that the 3-year OS and DFS rates of the TP53 mutation group were significantly lower than those of the wild-type group (P<0.05). Univariate Cox regression analysis indicated that differentiation degree, TNM stage, lymph node metastasis, tumor diameter, and TP53 mutation were adverse prognostic factors for gastric cancer; multivariate Cox regression analysis revealed that age, differentiation degree, TNM stage, lymph node metastasis, tumor diameter, and TP53 mutation were independent risk factors for poor prognosis in gastric cancer.【Conclusion】Patients with TP53 mutated gastric cancer exhibit poorer clinical characteristics and prognosis. Age, differentiation degree, TNM stage, lymph node metastasis, tumor diameter, and TP53 mutation are independent prognostic risk factors.

197 Clinical Imaging Analysis of Congenital Granular Cell Epulis

【Objective】To analyze the clinical and imaging finding of in congenital granular cell epulis, and improve the recognition of the disease.【Methods】The clinical data of 6 children diagnosed with CGCE by surgical pathology from June 2017 to February 2023 in our hospital were retrospectively analyzed, and their clinical and imaging characteristics were analyzed.【Results】The maximum tumor diameter of 6 cases was 1.3-3.4 cm, and 4 cases had multiple lesions, of which 3 cases involved both maxillary and mandibular alveolar crista, and 1 case involved both maxillary alveolar crista and sublingual. 2 cases had single lesions involving maxillary anterior alveolar ridge and mandibular alveolar ridge, respectively. Necrosis、cystic change and hemorrhage was not seen in 6 cases. Two cases showed filling enhancement from the edge to the center, and 3 cases showed gradual enhancement. The tumors had high signal on DWI, and low signal on ADC map. No adjacent bone was involved in 6 cases.【Conclusions】The imaging findings of CGCE have certain characteristics, and the diagnosis needs to be confirmed by pathological and immunohistochemical examination.

200 Effect of Antiandrogen Withdrawal Therapy on Cognitive Function in Castration-resistant Prostate Cancer Patients

【Objective】To investigate the effect of antiandrogen withdrawal therapy on cognitive function in castration-resistant prostate cancer (CRPC).【Methods】The clinical data of 100 patients with castration-resistant prostate cancer admitted to our hospital from February 2018 to March 2023 were retrospectively analyzed, and they were divided into the observation group and the control group according to whether the patients were treated with antiandrogynous drug withdrawal, with 50 cases in each group. The score of Montreal Cognitive Assessment Scale (MoCA), Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD) and neuropsychological cognitive function test [Auditory Word Learning Test (AVLT), Number Range Test (DS), Connected Task Test (TMT), Verbal Fluency Test (VF), Classic Stroop paradigm test] were compared between the two groups. Pearson correlation was used to analyze the correlation between the outcome of antiandrogenic drug withdrawal and the degree of cognitive impairment.【Results】MoCA scores in the observation group were significantly lower than those in the control group, and HAMA and HAMD scores were higher than those in the control group, with statistical significance (P<0.05). The recognition memory score and anteroposteric number score in the observation group were lower than those in the control group, and the time of classic Stroop paradigm C test, TMT A test and TMT B test were longer than those in the control group, with statistical significance (P<0.05). Pearson correlation analysis showed that the results of antiandrogant withdrawal treatment were positively correlated with cognitive function, anxiety state and recognition memory ability of patients (P<0.05), and negatively correlated with TMT A test time and TMT B test time (P<0.05).【Conclusion】CRPC patients treated with antiandrogen withdrawal showed decreased cognitive function, recognition memory ability, anterograde digital breadth, executive function, information processing ability, and increased anxiety and depression. For CRPC patients treated with antiandrogen withdrawal, intervention measures should be paid attention to before initiation and during treatment, which can improve the cognitive function of patients.

204 Correlation Analysis of Peripheral Venous Pressure and Central Venous Pressure in Patients with Severe Heart Failure with Invasive Ventilation

【Objective】To investigate the correlation and consistency between peripheral venous pressure and central venous pressure in patients with severe heart failure with invasive ventilation.【Methods】Patients with severe heart failure who received invasive ventilation in the intensive care unit from January to December 2021 were selected as the study subjects. When central venous pressure (CVP) was measured for the first time in each patient, peripheral venous pressure (PVP) was measured simultaneously through the median of the elbow or the main vein. Pearson correlation analysis was used to analyze the correlation between CVP and PVP. Bland-Altman method was used to analyze the consistency of CVP and PVP.【Results】Pearson correlation analysis showed that there was significant linear correlation between CVP and PVP (r=0.93, P<0.05). The linear regression equation was y=0.98x-0.44 (y was CVP, x was PVP; F=846.02, P<0.05), calibration determination coefficient R2=0.87. In 138 patients, the measured values of CVP and PVP were (11.53±5.70) mmHg and (12.28±5.46) mmHg, and the 95% conjunctive limit was from -4.8 mmHg to 3.3 mmHg. Bland-Altman analysis showed that CVP had good consistency with PVP.【Conclusion】PVP and CVP have good correlation and consistency. For patients with heart failure receiving invasive ventilation, PVP and CVP are a rapid, reliable and non-invasive assessment index of volume status.

207 Application Effect of Evidence-Based Nursing Supported by Risk Identification Concept in PICC Catheterization of Newborns

【Objective】To explore the application effect of evidence-based nursing supported by the concept of risk identification in PICC catheterization of newborns.【Methods】A total of 104 neonates who received PICC catheterization in our hospital from October 2022 to March 2024 were selected and divided into the observation group and the control group by random number table method, with 52 cases in each group. The observation group was treated with evidence-based nursing supported by the concept of risk identification, while the control group was treated with routine nursing according to hospital SOP. The catheter placement situation, family satisfaction and complication rate of newborns in the two groups were compared.【Results】The success rate of puncture and weight gain during catheterization in the observation group were higher than those in the control group, and the catheterization time and hospital stay in the observation group were shorter than those in the control group, with statistical significance (P<0.05). Family support, family education, information communication, timeliness, overall satisfaction scores and total scores of the observation group were higher than those of the control group, and the differences were statistically significant (P<0.05). The incidence of complications in the observation group was 7.69% (4/52), lower than that in the control group [30.77% (16/52)], with statistical significance (χ2=8.914, P<0.05).【Conclusion】Evidence-based nursing supported by the concept of risk identification is helpful to improve the success rate of PICC catheterization, promote the weight gain and overall health status of newborns, and reduce the incidence of complications of PICC catheterization in children.

211 The Effect of Otilonium Bromide Combined with Helicobacter Pylori Eradication Therapy on Gastrointestinal Hormone Levels in Patients with Functional Dyspepsia

【Objective】To explore the effect of otilonium bromide combined with Helicobacter pylori (HP) eradication therapy on gastrointestinal hormone levels in patients with functional dyspepsia (FD).【Methods】A total of 88 FD patients were randomly divided into the observation group and the control group, with 44 cases in each group. Patients in the control group received HP eradication therapy, while those in the observation group received otilonium bromide combined with HP eradication therapy. The clinical efficacy, adverse reactions, gastrointestinal hormone levels, gastric motility indicators, and changes in quality of life before and after treatment were compared between the two groups.【Results】The total effective rate in the observation group was 95.45%, which was higher than that of the control group (79.54%,P<0.05). After treatment, the gastrointestinal hormone levels and gastric motility in both groups significantly improved compared to before treatment, and the improvement in the observation group was better than that in the control group (P<0.05). After treatment, the quality of life scores in both groups increased compared to before treatment, while the score of quality of life in the observation group was higher than that in the control group (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05).【Conclusion】Otilonium bromide combined with HP eradication therapy significantly improves gastrointestinal hormone levels, gastric motility indicators, clinical efficacy, and quality of life in FD patients, with a certain level of safety.

214 The Effect of Nalbuphine in Assisting the Suppression of Sufentanil-Induced Cough Reflex during Anesthesia Induction for Laparoscopic Cholecystectomy

【Objective】To investigate the effect of nalbuphine in suppressing the cough reflex induced by sufentanil during anesthesia induction for laparoscopic cholecystectomy (LC).【Methods】A total of 82 patients undergoing LC were randomly divided into the observation group and the control group, with 41 patients in each group. Both groups were administered sufentanil for anesthesia induction. Ten minutes before induction, the control group received 0.01 mg/kg of penehyclidine hydrochloride intravenously, while the observation group received 0.01 mg/kg of penehyclidine hydrochloride plus 0.1 mg/kg of nalbuphine intravenously. The incidence of cough reflex, hemodynamic and stress marker changes, pain levels, and adverse reactions were compared between the two groups.【Results】The incidence of cough reflex was lower in the observation group than in the control group (P<0.05). There was no significant difference in the duration of cough reflex between the two groups (P>0.05). At 2 minutes after sufentanil injection (T2), the mean arterial pressure (MAP) and heart rate (HR) were lower in both groups compared to 10 minutes before injection (T0) and 1 minute after injection (T1) (P<0.05), and they were lower at T1 than at T0 (P<0.05). MAP and HR at T1 and T2 were higher in the observation group compared to the control group (P<0.05). The numerical rating scale (NRS) pain scores at 4 hours (T4) and 24 hours (T5) postoperatively were lower than at T3 in both groups (P<0.05), and the scores at T5 were lower than at T4 (P<0.05); however, there was no significant difference in NRS scores between the two groups at any time point (P>0.05). At 24 hours postoperatively, adrenaline and norepinephrine levels were elevated compared to preoperative levels in both groups (P<0.05), but there was no significant difference between the groups (P>0.05). The overall incidence of adverse reactions was not significantly different between the two groups (P>0.05).【Conclusion】Nalbuphine effectively suppresses sufentanil-induced cough reflex during anesthesia induction for LC, with minimal impact on hemodynamics. It alleviates postoperative pain and is safe and reliable.

218 Prognostic Value of Dual-source CT Whole-Brain Perfusion Imaging in Patients with Acute Ischemic Stroke

【Objective】To investigate the clinical value of dual-source CT whole brain perfusion imaging (CTP) in predicting the prognosis of patients with acute ischemic stroke (AIS).【Methods】A total of 100 patients with AIS admitted to our hospital were divided into poor prognosis group (mRS scores>2 points, n=31) and good prognosis group (mRS scores≤2 points, n=69) according to mRS Scores on the 90th day after discharge. Baseline mRS and National Institutes of Health Stroke Scale (NIHSS) scores of all patients were compared. Cerebral blood volume (CBV), time to peak (TTP), mean transit time (MTT), and time of delay(TD) were calculated by dual-source CTP. The correlation of baseline NIHSS score and mRS Score with CBV, TTP, MTT and TD was discussed, and the influence of these parameters on the 90d prognosis of AIS patients was analyzed.【Results】There was no statistical significance in the basic clinical data between the good prognosis group and the poor prognosis group (P>0.05).Compared with the good prognosis group, CBV was decreased in the poor prognosis group, but TTP, MTT and DLT were increased (P<0.05). Pearson correlation analysis showed that mRS and NIHSS scores were negatively correlated with CBV and positively correlated with TTP, MTT and TD, respectively (P<0.05).Multivariate Logistic regression showed that CBV was an independent protective factor affecting the prognosis of AIS patients within 90 days, and TTP was an independent risk factor affecting the prognosis of AIS patients within 90 days (P<0.05).【Conclusion】Dual-source CTP related parameters are potential prognostic factors in AIS patients within 90 days, in which CBV is a protective factor affecting prognosis, while TTP is a risk factor affecting prognosis. CBV, TTP, MTT and TD are significantly correlated with baseline mRS And NIHSS scores. Helps to identify high-risk patients early and guide personalized treatment.

221 Clinical Value of Head MSCTA in Diagnosis of Intracranial Aneurysms

【Objective】To evaluate the clinical value of head multislice spiral computed tomography angiography(MSCTA) in the diagnosis of intracranial aneurysm (IA).【Methods】The clinical data of 85 patients with suspected IA admitted to our hospital from February 2019 to February 2023 were retrospectively analyzed. All patients underwent digital subtraction angiography (DSA) and MSCTA. With surgical pathological diagnosis as the gold standard, the diagnostic results and diagnostic efficacy of the two diagnostic methods for IA were compared, and the detection values of the maximum diameter of the tumor neck and the maximum diameter of the tumor body were compared between MSCTA and DSA.【Results】48 patients were confirmed as IA by operation. The Kappa values of MSCTA and DSA for diagnosis of IA were 0.880 and 0.881, respectively, which were in good agreement with pathological diagnosis results (P<0.05). There was no significant difference in accuracy, specificity and sensitivity between MSCTA and DSA in the diagnosis of IA (P>0.05). There was no significant difference between MSCTA and DSA in the maximum diameter of tumor neck and the maximum diameter of tumor body (P>0.05).【Conclusion】MSCTA can effectively diagnose IA, and its diagnostic value is similar to that of traditional DSA, and it can be used as an auxiliary diagnostic method for DSA in clinic.

225 Diagnostic Value of Serum sCD14 Combined with sCD30 in Elderly Patients with Chronic Obstructive Pulmonary Disease Complicated with Pulmonary Fungal Infection

【Objective】To investigate the diagnostic value of serum soluble 14 (sCD14) combined with sCD30 in elderly patients with chronic obstructive pulmonary disease (COPD) complicated with pulmonary fungal infection.【Methods】A total of 80 elderly patients with COPD admitted to our hospital from April 2021 to June 2023 were selected and divided into group A (n=23, combined with pulmonary fungal infection) and group B (n=57, not combined with pulmonary fungal infection), using fungal culture as the gold standard. Serum sCD14 and sCD30 levels were detected by ELISA on the second day of admission in the two groups, and the influencing factors of senile COPD combined with pulmonary fungal infection and the efficacy of serum sCD14 and sCD30 in diagnosis of senile COPD combined with pulmonary fungal infection were analyzed.【Results】The Acute physiology and Chronic Health status Scoring System Ⅱ (APACHE Ⅱ) scores and the levels of sCD14, sCD30 and C-reactive protein (CRP) in group A were higher than those in group B (P<0.05). Binary Logistic regression analysis showed that APACHE Ⅱ score and high levels of sCD14, sCD30 and CRP were risk factors for pulmonary fungal infection in elderly patients with COPD (P<0.05). APACHE Ⅱ score combined with serum sCD14, sCD30 and CRP were more effective than single index in predicting pulmonary fungal infection in elderly patients with COPD.【Conclusion】APACHEⅡ score combined with serum sCD14, sCD30 and CRP has a high predictive value for pulmonary fungal infection in elderly patients with COPD.

228 Relationship between Circadian Rhythm, Blood Pressure Variability and Peripheral Atherosclerosis in Patients with Essential Hypertension

【Objective】To investigate the relationship between blood pressure circadian rhythm, blood pressure variability and ankle-brachial index (ABI) in patients with essential hypertension.【Methods】A total of 176 patients with essential hypertension admitted to our hospital from September 2021 to December 2023 were selected and divided into sclerotic group (n=90) and non-sclerotic group (n=86) according to the detection results of anklebrachial index (ABI). Patients were divided into arytenoid group (n=48), non-arytenoid group (n=94) and reverse arytenoid group (n=34) according to circadian rhythm of blood pressure. The non-sclerotic group and the sclerotic group were compared in terms of the ambulatory blood pressure index [24 h systolic blood pressure (24hSBP), daytime systolic blood pressure (dSBP), night systolic blood pressure (nSBP), and 24 h diastolic blood pressure (24hDBP), daytime diastolic blood pressure (dDBP), night diastolic blood pressure (nDBP)] and the indicators of blood pressure variability [24 h standard deviation of systolic blood pressure (24hSSD), 24 h standard deviation of diastolic blood pressure (24hDSD), standard deviation of diastolic blood pressure (dSSD), standard deviation of diastolic blood pressure (dDSD), standard deviation of nocturnal systolic blood pressure (nSSD), standard deviation of nocturnal diastolic blood pressure (nDSD)]. The decreasing rate of systolic blood pressure (ΔSBP%) and the decreasing rate of diastolic blood pressure at night (ΔDBP%) were calculated. Pearson was used to analyze the correlation between circadian rhythm of blood pressure and peripheral atherosclerosis, and multiple Logistic regression was used to analyze the influencing factors of peripheral atherosclerosis.【Results】24hSBP, dSBP, nSBP, 24hSSD, 24hDSD, dSSD, nSSD, nDSD in hardening group were higher than those in non-hardening group, and ΔSBP% was lower than that in non-hardening group, the difference was statistically significant (P<0.05). ABI of non-arytenoid group and reverse arytenoid group was significantly lower than that of arytenoid group, and the difference was statistically significant (P<0.05). Pearson correlation analysis showed that ABI was negatively correlated with ΔSBP%, the difference was statistically significant (r=-0.406,P=0.044). Logistic regression analysis showed that the increase of 24hSSD and nSSD was a risk factor for peripheral atherosclerosis (P<0.05).【Conclusion】Abnormal circadian rhythm and variability of blood pressure are important factors leading to atherosclerosis, and correct analysis of blood pressure variation is an effective measure to prevent and intervene atherosclerosis.

232 Value of Endoscopic Ultrasonography in the Diagnosis of Gastric Stromal Tumor and Gastric Leiomyoma

【Objective】To investigate the value of endoscopic ultrasonography in the diagnosis of gastric stromal tumor and gastric leiomyoma.【Methods】A total of 59 patients with gastric stromal tumor (the observation group) and 44 patients with gastric leiomyoma (the control group) admitted to our hospital from July 2017 to July 2023 were selected. All patients underwent endoscopic ultrasonography before surgery. General data of the two groups were compared with endoscopic features (lesion site, size, boundary, origin level, echo, presence or absence of mucosal bridge, liquefaction, calcification, and ulcer), and the risk factors of gastric stromal tumor were determined by multivariate Logistic regression analysis.【Results】There was no significant difference in lesion site, boundary, mucosal bridge, liquefaction and ulcer between the two groups (P>0.05). The proportion of uniform echo, no calcification and clear boundary in control group was higher than that in observation group, and the difference was statistically significant (P<0.05). Multivariate Logistic regression analysis showed that the risk factors of gastric stromal tumor were uneven echo, calcification and unclear boundary (P<0.05).【Conclusion】Endoscopic ultrasound can be used in the differential diagnosis of gastric stromal tumor and gastric leiomyoma. Patients with uneven echo, calcification and unclear boundary should be paid close attention to avoid misdiagnosis and missed diagnosis.

235 The Detection Rate and Influencing Factors of TI-RADS Grade 4 Nodules ≤1 cm in the Diagnosis of Suspected Malignant Calcifications via Ultrasound-Guided Fine Needle Aspiration

【Objective】To investigate the detection rate of grade 4 nodules within 1 cm and the influencing factors in the diagnosis of suspected malignant calcification by ultrason-guided thyroid fine needle puncture in the Thyroid Imaging Reporting and Data System (TI-RADS).【Methods】A total of 213 cases of TI-RADS grade 4 patients with small nodules less than 1 cm were prospectively selected in our hospital from January 2020 to December 2023. All of them underwent ultrasound-guided thyroid fine needle puncture diagnosis, and the postoperative histopathological examination results were used as the gold standard. The diagnostic value of ultrasound-guided thyroid fine needle puncture in the small nodules within 1 cm of grade 4 TI-RADS was analyzed, and the influencing factors of the accuracy of ultrasound-guided thyroid fine needle puncture in diagnosing the small nodules within 1 cm of grade 4 TI-RADS were analyzed.【Results】Postoperative pathological diagnosis showed that 184 cases were malignant nodules and 29 cases were benign nodules. Ultrasound-guided thyroid fine needle puncture was used to diagnose grade 4 small nodules within 1 cm of TI-RADS accurately in 192 cases, and under-diagnosed in 21 cases. The sensitivity, specificity and accuracy of ultrasus-guided thyroid fine needle puncture in diagnosing the benign and malignant nodules within 1 cm of grade 4 TI-RADS were 88.59%, 100.00% and 90.14%, respectively. Logistic regression analysis showed that the maximum nodule diameter, nodule composition, calcification, and blood supply were the influencing factors for the accuracy of ultrasound-guided thyroid fine needle puncture in diagnosing small nodules within 1 cm of grade 4 TI-RADS (P<0.05).【Conclusion】The risk of malignant nodules within 1 cm of grade 4 TI-RADS with suspected malignant calcification is higher. The maximum diameter of nodules, nodule composition, calcification and blood supply are the factors influencing the accuracy of diagnosing the benign and malignant nodules within 1 cm of grade 4 TI-RADS with ultrasound-guided thyroid fine needle puncture.

239 Efficacy and Influencing Factors of Dorsal Root Ganglion Pulsed Radiofrequency Combined with Pregabalin in the Treatment of Postherpetic Neuralgia

【Objective】To investigate the efficacy of dorsal root ganglion pulsed radiofrequency combined with pregabalin in the treatment of postherpetic neuralgia (PHN) and identify related influencing factors.【Methods】A total of 90 patients with PHN, who were treated at the Third People's Hospital of Nantong between May 2020 and May 2023, were enrolled. All patients received dorsal root ganglion pulsed radiofrequency combined with pregabalin treatment. Pain intensity was assessed before and after treatment using the Visual Analog Scale (VAS), and inflammatory markers including calcitonin gene-related peptide (CGRP), β-endorphin (β-EP), and prostaglandin E2 (PGE2) were measured as well. After 6 months of follow-up, patients were divided into the effective group and the ineffective groups based on the treatment outcome. Clinical data were collected, and multivariate Logistic regression analysis was performed to determine the factors influencing the treatment efficacy.【Results】After treatment, VAS scores significantly decreased (P<0.05). Levels of CGRP and PGE2 were lower, while β-EP levels were higher compared to baseline before treatment (P<0.05). There were significant differences between the two groups in terms of age, disease duration, diabetes, antiviral treatment, and the duration of analgesic use (P<0.05). Multivariate analysis showed that age >65 years, disease duration >6 months, diabetes, and prolonged analgesic use (>2 weeks) were risk factors affecting treatment efficacy (P<0.05).【Conclusion】Dorsal root ganglion pulsed radiofrequency combined with pregabalin significantly alleviates pain and reduces inflammatory markers in patients with PHN. Key risk factors influencing treatment efficacy include age >65 years, disease duration >6 months, diabetes, and prolonged use of analgesics for more than 2 weeks.

243 Evaluation and Application Prospect of Iron Protein Succinate Oral Solution Combined with Nutrition and Health Care in the Treatment of Children with NIDA

【Objective】To investigate the clinical effect of iron protein succinate oral solution combined with nutritional health intervention in the treatment of children with nutritional iron deficiency anemia(NIDA).【Methods】A total of 82 children with NIDA treated in the Ankang Central Hospital of Shaanxi Province from June 2020 to June 2023 were selected and divided into observation group and control group according to treatment methods, with 41 cases in each group. Both groups were given nutrition and health care interventions, the control group was given ferrous sulfate tablets, and the observation group was given iron protein succinate oral solution. Hemoglobin (Hb) and red blood cell count (RBC) were compared between the two groups, and the clinical efficacy and incidence of adverse reactions were compared between the two groups.【Results】Before treatment, there was no significant difference in Hb and RBC levels between the two groups (P>0.05). After treatment, Hb and RBC levels in 2 groups were higher than before treatment, and the observation group was higher than the control group, the difference was statistically significant (P<0.05). The total effective rate of the observation group was 92.68% (38/41), higher than that of the control group 75.61%(31/41), and the difference was statistically significant (χ2=4.479, P<0.05). There was statistical significance in the incidence of adverse reactions between the two groups (P<0.05).【Conclusion】Iron protein succinate oral solution combined with nutritional intervention in the treatment of children with NIDA has good clinical efficacy and high safety, and is worthy of promotion.

246 Effect of Tirofiban on Percutaneous Coronary Intervention in Patients with Acute Coronary Syndrome

【Objective】To investigate the effect of Tirofiban on percutaneous coronary intervention (PCI) in patients with acute coronary syndrome (ACS).【Methods】A total of 100 patients with ACS who underwent PCI treatment in our hospital from March 2020 to March 2023 were selected and divided into observation group and control group according to random number table method, with 50 cases in each group. The control group was given routine treatment, the observation group was given intravenous tirofiban during PCI. TIMI blood flow grading, myocardial enzyme profile indexes [creatine kinase isoenzyme (CK-MB), troponin I(cTnI), lactate dehydrogenase (LDH)], major adverse cardiovascular events (MACE) and bleeding complications were compared between the two groups.【Results】After treatment, TIMI blood grading in both groups was significantly improved compared with that before surgery, and TIMI blood grading in the observation group was better than that in the control group, the difference was statistically significant (P<0.05). After treatment, serum CK-MB, cTnI and LDH levels in the 2 groups were lower than before treatment, and the observation group was lower than the control group, the difference was statistically significant (P<0.05). After 3 months of follow-up, the incidence of MACE in the observation group was significantly lower than that in the control group, with statistical significance (P<0.05). No serious bleeding occurred in all patients during treatment, and there was no significant difference in the incidence of bleeding complications between the two groups (P>0.05).【Conclusion】The use of Tirofiban during perioperative PCI can effectively improve the coronary artery flow and myocardial injury in ACS patients, reduce the incidence of MACE, and do not increase the risk of bleeding.

249 Ultrasonic Characteristics of Hashimoto Thyroiditis Complicated with TI-RADS Type 4 Benign and Malignant Nodules

【Objective】To investigate the ultrasonic characteristics of Hashimoto's thyroiditis (HT) combined with TI-RADS in the 4 types of benign and malignant nodules.【Methods】From January to May 2024, 136 patients clinically diagnosed as HT with TI-RADS type 4 nodules in our hospital were examined by ultraso-guided fine needle aspiration cytology(US-FNAC) to analyze the ultrasonic characteristics of nodules and compare them with Bethesda classification results. According to the results of Bethesda classification, they were divided into the benign group (Bethesda grade Ⅱ) and the malignant group (grade Ⅴ+Ⅵ). The correlation between the ultrasonic features of nodules and the benign and malignant nodules was analyzed, and the independent risk factors for malignant nodules of type 4 of HT combined with TI-RADS were predicted.【Results】Univariate analysis showed that the type of nodule echo, edge and strong echo were correlated with the malignancy of nodule (P<0.05), but the solid structure of nodule, whether there was strong focal echo, shape and location were not correlated with the malignancy of nodule (P>0.05). The results of multi-factor analysis showed that low echo and strong focal echo type HT combined with TI-RADS type 4 nodules were independent risk factors for malignancy. The proportion of coarse calcification in the malignant group was lower than that in the benign group, and the proportion of microcalcification was higher than that in the benign group, with statistical significance (P<0.05).【Conclusion】Among the 4 types of TI-RADS nodules with HT, the malignant risk of low echo and dot strong echo nodules is higher.

252 The Effect of Olanzapine on Glycolipid Metabolism and Blood Routine Results in Schizophrenia Patients

【Objective】To investigate the impact of olanzapine treatment on glycolipid metabolism and blood routine test results in patients with schizophrenia (SCZ).【Methods】A total of 230 SCZ patients were divided into the observation group (118 patients, treated with olanzapine) and the control group (112 patients, treated with risperidone) based on different treatment plans. The efficacy and the changes of glycolipid metabolism and blood routine indicators before and after treatment were compared between the two groups.【Results】The clinical efficacy in the observation group was higher than that in the control group, however, the difference was not statistically significant (P>0.05). After treatment, the scores of the Positive and Negative Syndrome Scale (PANSS) on all dimensions in the observation group were significantly lower than those in the control group (P<0.05). After treatment, the fasting plasma glucose (FPG), total cholesterol (TC), and low-density lipoprotein cholesterol (LDL-C) levels in both groups were significantly lower than before (P<0.05), while triglyceride (TG) level significantly increased (P<0.05). Moreover, the levels of these indicators were significantly lower in the observation group than those in the control group (P<0.05). There were no statistically significant differences in blood routine test results between the two groups before and after treatment (P>0.05).【Conclusion】Olanzapine and risperidone have similar overall therapeutic effects on SCZ patients. However, olanzapine has a more significant advantage in improving glycolipid metabolism and it does not have adverse effects on blood routine indicators.

255 Clinical Efficacy of Vitamin B12 and Folic Acid in the Treatment of Megaloblastic Anemia Patients

【Objective】To explore the clinical efficacy of vitamin B12 and folic acid in the treatment of megaloblastic anemia (MA) patients.【Methods】Clinical data of 50 MA patients treated with vitamin B12 and folic acid supplementation were collected. The patients' clinical manifestations, changes in hematological indicators, and treatment outcomes were analyzed.【Results】After treatment, the mean corpuscular volume (MCV) of the patients decreased, while hemoglobin (Hb), vitamin B12, and folic acid levels increased significantly compared to before treatment, with differences being statistically significant (P<0.05). The proportion of patients with symptoms such as fatigue, pallor, loss of appetite, weight loss, and neurological symptoms significantly decreased after treatment, with differences being statistically significant (P<0.05). Correlation analysis showed that vitamin B12 had the closest association with symptom improvement. Vitamin B12, folic acid, and Hb levels were negatively correlated with fatigue, pallor, loss of appetite, weight loss, and neurological symptoms (all P<0.05). MCV was positively correlated with the above symptoms (P<0.05).【Conclusion】Supplementation of vitamin B12 and folic acid can improve hematological indicators and clinical symptoms in MA patients, which vitamin B12 plays a key role in symptom improvement. When diagnosing and treating MA clinically, attention should be paid to the detection and supplementation of vitamin B12 levels.

258 Correlation of CRP, LDH, and β2-MG Levels with Prognosis in Multiple Myeloma Patients

【Objective】To explore the relationship of prognosis with C-reactive protein (CRP), lactate dehydrogenase (LDH), and β2-microglobulin (β2-MG) levels in multiple myeloma (MM) patients.【Methods】A retrospective analysis was conducted on the clinical data of 96 MM patients, including age, gender, International Staging System (ISS) stage, immunoglobulin subtype, comorbidities, coagulation markers, lipid profiles, homocysteine (Hcy) levels, and CRP, LDH, and β2-MG levels. Factors influencing prognosis were analyzed, and Spearman's correlation test was used to analyze the correlation of prognosis with levels of Hcy, CRP, LDH and β2-MG.【Results】Among the 96 patients, 79 (82.29%) were alive and were categorized into the survival group, while 17 (17.71%) died and were categorized into the death group. The proportion of ISS stage Ⅱ and Ⅲ, as well as CRP, LDH, β2-MG, and Hcy levels in the death group, were significantly higher than those in the survival group (P<0.05). CRP, LDH, β2-MG, and Hcy were found to be significant prognostic factors for MM (P<0.05). Spearman's test showed that Hcy, CRP, LDH, and β2-MG levels were positively correlated with prognosis (P<0.05).【Conclusion】As the levels of Hcy, CRP, LDH, and β2-MG increase, the severity of the disease worsens, and the risk of poor prognosis in MM patients rises.

261 Diagnostic Value of Kyoto Classification of Gastritis Score Combined with Acetic Acid-Indigo Carmine Staining Magnification Endoscopy in Early Gastric Cancer

【Objective】To explore the diagnostic value of the Kyoto Classification of Gastritis (KCS) score combined with acetic acid-indigo carmine staining magnification endoscopy in early gastric cancer.【Methods】Ninety-eight early gastric cancer patients who underwent endoscopic submucosal dissection (ESD) were included in the observation group, and 57 non-cancer patients who were scheduled for endoscopy due to upper abdominal discomfort (such as bloating and pain) were included in the control group. According to the KCS, patients were divided into the low-score group (n=79, <4 points) and the high-score group (n=76, ≥4 points). Clinical data and KCS scores of the observation group and the control group were compared, and the proportion of gastric cancer cases in the high-score group and the low-score score groups were compared as well. Factors influencing early gastric cancer occurrence were screened, and receiver operating characteristic (ROC) curves were generated to assess the diagnostic value using the area under the curve (AUC).【Results】The KCS score in the observation group was significantly higher than that in the control group (P<0.05). The proportion of gastric cancer cases in the high-score group was significantly higher than that in the low-score group (P<0.05). Acetic acid-indigo carmine staining magnification endoscopy detected 81 cases of early gastric cancer and 17 non-cancer cases (6 cases with atypical hyperplasia and 11 cases with intestinal metaplasia). The pepsinogen (PG)Ⅰ/PGⅡ ratio in the observation group was lower than that in the control group (P<0.05). Helicobacter pylori (HP) urease antibody, carbohydrate antigen 125 (CA125), carbohydrate antigen 199 (CA199), carcinoembryonic antigen (CEA), PGⅠ/Ⅱ, and KCS were identified as factors related to early gastric cancer occurrence (P<0.05). The AUC values for KCS, acetic acid-indigo carmine staining magnification endoscopy, and their combined use in diagnosing early gastric cancer were 0.820, 0.852, and 0.882, respectively, with the combined approach showing a higher AUC value (P<0.05).【Conclusion】KCS and acetic acid-indigo carmine staining magnification endoscopy are effective in diagnosing early gastric cancer, while the combination of the two methods provides even higher diagnostic value.

265 Efficacy of Bevacizumab Combined with FOLFIRI Regimen in the Treatment of Fluorouracil-Resistant Advanced Colorectal Cancer

【Objective】To investigate the clinical efficacy of bevacizumab combined with the FOLFIRI regimen in treating fluorouracil (FU)-resistant advanced colorectal cancer (CRC) patients.【Methods】A total of 89 FU-resistant advanced colorectal cancer patients were randomly divided into two groups: the control group (44 patients, treated with the FOLFIRI regimen) and the observation group (45 patients, treated with bevacizumab combined with FOLFIRI regimen). The short-term and long-term efficacy, changes in tumor marker levels before and after treatment, and adverse reactions were compared between the two groups.【Results】The short-term overall response rate was significantly higher in the observation group than in the control group (P<0.05). After treatment, serum levels of carbohydrate antigen 19-9 (CA19-9), carbohydrate antigen 242 (CA242), carcinoembryonic antigen (CEA), and epidermal growth factor receptor (EGFR) were significantly lower than before treatment (P<0.05) in both groups, and the observation group had lower levels compared to the control group (P<0.05). At the end of the follow-up period, the median overall survival (OS) of the control group was 12.0 months, while the OS of the observation group was 17.0 months; the Kaplan-Meier curve comparison between the two groups showed a significant statistical difference (Log-rank χ2=5.624, P=0.018). There were no significant differences in the incidence of adverse reactions between the two groups (P>0.05).【Conclusion】Bevacizumab combined with FOLFIRI regimen treatment improves both short- and long-term efficacy in FU-resistant advanced colorectal cancer patients, reduces tumor marker levels, and it is safe and reliable.

【Objective】To investigate the clinical value of T lymphocyte subsets, hepatitis B surface antigen (HBsAg) and hepatitis Be antigen (HBeAg) in evaluating different pathological states of patients with hepatitis B.【Methods】A total of 85 cases of hepatitis B patients admitted to our hospital from May 2020 to May 2023 were selected. Pathological examination results showed that 48 cases had inflammation grade < 2, of which 37 cases had inflammation grade ≥2, of which 36 cases had fibrosis stage < 2, and 49 cases had fibrosis stage ≥2. The levels of HBsAg, HBeAg and T lymphocyte subsets in patients with different fibrosis stages and inflammatory grades were compared, and the clinical value of HBsAg,HBeAg and T lymphocyte subsets were analyzed by receiver operating characteristic (ROC) curve to evaluate the different pathological status of hepatitis B patients.【Results】The levels of HBsAg and HBeAg in patients with fibrosis stage ≥2 were significantly lower than those with fibrosis stage < 2, the difference was statistically significant (P<0.05). There was no significant difference in CD4+ and CD8+ levels among hepatitis B patients with different fibrosis stages (P>0.05). ROC curve analysis showed that HBsAg and HBeAg levels had better clinical efficacy in predicting fibrosis stage ≥2, and the area under the curve were 0.772 and 0.821, respectively (P<0.05). HBsAg, HBeAg and CD8+ levels in patients with inflammation grade ≥2 were significantly lower than those with inflammation grade < 2, and CD4+ levels were significantly higher than those with inflammation grade < 2, with statistical significance (P<0.05). ROC curve analysis showed that HBsAg, HBeAg, CD4+, CD8+ levels had better clinical efficacy in predicting inflammation grade ≥2, and the area under the curve was 0.785, 0.808, 0.644, 0.683, with statistical significance (P<0.05).【Conclusion】HBsAg and HBeAg levels and immune function are abnormal in patients with hepatitis B in different pathological states, and the levels of HBsAg, HBeAg and T lymphocyte subsets can be jointly detected in clinic to better evaluate the disease progression of patients.

272 Effect of Corneal Protectants Combined with Meibomian Gland Massage in Patient after Cataract Surgery

【Objective】To investigate the effect of corneal protectants combined with meibomian gland massage in patients after cataract surgery.【Methods】A total of 108 patients who underwent phacoemulsification combined with intraocular lens implantation in our hospital from January 2021 to April 2022 were selected and randomly divided into the control group and the observation group, with 54 cases in each group. The control group was given meibomian gland massage 1 week before surgery, while the observation group was combined with corneal protectants in addition to the way of the control group. objective scatter index (OSI), schirmer test (SIt), signs and symptoms score, meibomian gland opening score and lacrimal river height were compared between the two groups.【Results】After treatment, OSI in observation group was lower than that in the control group, while SIt was higher than control group (P<0.05). The scores of sensory symptoms and ocular surface signs in the observation group were lower than those in the control group (P<0.05). The meibomian gland opening score of the observation group was lower than that of the control group, while the lacrimal river height was higher than that of the control group (P<0.05).【Conclusion】Meibomian gland massage combined with corneal protectants can effectively improve OSI, SIt, and dry eye symptoms and signs in patients treated with cataract surgery.

275 Comparison of the Efficacy of Unilateral Biportal Spinal Endoscopy and Posterior Lumbar Interbody Fusion in the Treatment of Lumbar Degenerative Diseases

【Objective】To compare the efficacy of unilateral biportal spinal endoscopic (UBE) and posterior lumbar interbody fusion (PLIF) in the treatment of lumbar degenerative diseases.【Methods】A total of 102 patients with lumbar degenerative diseases treated in our hospital from January 2022 to June 2024 were selected and divided into observation group (n=52, receiving UBE treatment) and control group (n=50, receiving PLIF treatment) according to different surgical methods. Preoperative and postoperative visual analogue scale (VAS) score, Japanese Orthopaedic Society (JOA) score, C-reactive protein (CRP) level and erythrocyte precipitation rate (ESR) were compared between the two groups.【Results】The intraoperative blood loss and postoperative drainage volume of the observation group were less than those of the control group, and the hospitalization time was shorter than that of the control group (P<0.05). 7 days after surgery, VAS score of the observation group was lower than that of the control group, JOA score was higher than that of the control group (P<0.05). The levels of CRP and ESR in the observation group were lower than those in the control group (P<0.05). There was no significant difference in the total incidence of complications between the two groups (P>0.05).【Conclusion】Compared with traditional PLIF therapy, UBE has less blood loss, shorter hospital stay, and more significant pain relief effect in the treatment of lumbar degenerative diseases. Moreover, the complication rates of the two surgical methods are similar, suggesting that UBE is a safe and effective treatment.

【Objective】To explore the application value of the motion sensing game exercise mode in postoperative rehabilitation of patients with TNM stage Ⅰ-Ⅲ lung cancer.【Methods】A total of 100 patients with lung cancer who underwent Video-assisted thoracoscopic surgery (VATS) were randomly divided into the control group and the observation group, with 50 cases in each group. The control group underwent routine rehabilitation exercise mode after operation, and the observation group implemented physical game sensory movement mode on the basis of the control group. Functional exercise compliance, lung function indexes and quality of life were compared 1 day after surgery and 12 weeks after surgery between the two groups.【Results】12 weeks after surgery, the functional exercise compliance of both groups was improved (P<0.05), and the observation group was significantly higher than the control group (P<0.05). The first forced expiratory volume in one second (FEV1), forced vital capacity (FVC), FEV1/FVC and other pulmonary function indexes in both groups were increased (P<0.05), and the observation group was higher than the control group (P<0.05).【Conclusion】The implementation of motion-sensing game exercise mode for lung cancer patients receiving VATS in TNM stage Ⅰ-Ⅲ can effectively improve their rehabilitation training compliance, improve lung function indexes, and enhance quality of life.

282 Clinical Effect of Non-stripping Dot Matrix Laser Combined with Finasteride and Minoxidil Tincture on Male Patients with Androgenic Alopecia

【Objective】To investigate the effect of non-exfoliating dot matrix laser combined with finasteride and minoxidil tincture in the treatment of androgenic alopecia.【Methods】A total of 80 male patients with androgenic alopecia admitted to our hospital from January 2022 to December 2023 were selected and divided into the control group (treated with finasteride and minoxidil tincture) and the observation group (treated with non-stripping dot matrix laser combined with finasteride and minoxidil tincture) according to random number table method, with 40 cases in each group. The clinical efficacy, hair growth (hair density, hair thickness, proportion of empty hair follicles), trichoscopic signs (hair diameter thinning > 20%, cellular pigmentation, local absence of hair, white scale, brown peri-hair sign, yellow dot sign, white dot sign) and adverse reactions were compared between the two groups.【Results】The total effective rate of the observation group was higher than that of the control group (P<0.05). After treatment, the hair density of the two groups was higher than before treatment, and the proportion of empty hair follicles was lower than before treatment (P<0.05). The hair density of the observation group was higher than that of the control group, and the proportion of empty hair follicles was lower than that of the control group (P<0.05). There was no statistical significance in hair coarseness between the two groups before and after treatment (P>0.05). After treatment, the proportion of hair diameter finer, local no hair sign and yellow dot in observation group was lower than that in control group (P<0.05). There was no statistical significance in the situation of honeycomb pigmentation, white scale, brown peri-hair sign and white dot sign between the two groups (P>0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05).【Conclusion】Non-stripping dot matrix laser combined with finasteride and minoxidil tincture in the treatment of male patients with androgenic alopecia has a good effect, can improve the physiological condition of hair follicles, promote hair growth, and has a good safety, worthy of clinical application.
