Construction of a Follow-up Nursing Problem Assessment System for Liver Transplant Patients Based on the Omaha Classification System

【Objective】 To construct a follow-up nursing problem assessment system for liver transplant patients using the Omaha Classification System(OCS), so as to provide evidence-based support for clinical nurses' nursing decision-making.【Methods】 With OCS as the theoretical framework, the indicator system was constructed through literature analysis, qualitative interviews, group brainstorming, and the Delphi method. An expert consultation questionnaire was developed, expert consultation was conducted, and the expert opinions were collected and statistically analyzed.【Results】 A total of two rounds of consultation were conducted in this study, with a questionnaire recovery rate and effective rate of 100%. The rates of experts putting forward opinions were 56.25% and 25.00% respectively. The expert authority coefficients(Cr) in the two rounds were 0.95 and 0.912 respectively. The coordination coefficients of experts for the first, second, and third-level indicators in the two rounds were 0.385, 0.236, 0.244(all P<0.05) and 0.272, 0.289, 0.274(all P<0.05) respectively. After two rounds of expert consultation, the final assessment form had the following number of indicators at each level:the first level(4 items), the second level(21 items), and the third level(45 items).【Conclusion】 The follow-up nursing problem assessment system for liver transplant patients based on OCS is scientific and reliable. It is suitable for assessing the nursing problems of liver transplant patients and provides valuable reference for clinical nursing diagnosis and intervention.

Expression Levels and Significance of Leptin and Glucagon in Colorectal Cancer Tissues

【Objective】 To investigate the expression levels of leptin(LEP) and glucagon(GCG) in colorectal cancer(CRC) tissues and their clinical significance.【Methods】 Tissue samples of CRC were collected from 34 patients who underwent surgical treatment in our hospital from January 2023 to January 2024. The expression levels of LEP and GCG were detected, and their correlations with the clinicopathological features of CRC patients were analyzed.【Results】 Among the 34 CRC patients, the age ranged from 44 to 80 years, with an average of(66.82±12.36) years; 58.82%(20/34) were male; the prevalence of diabetes was 52.94%(18/34); the body mass index(BMI) ranged from 19.6 to 37.3 kg/m
2
, with an average of(24.86±3.88) kg/m
2
. Among them, 35.29%(12/34) were overweight(23.5 kg/m
2
≤BMI<25.0 kg/m
2
) and 11.76%(4/34) were obese(BMI≥25.0 kg/m

). Immunohistochemical analysis showed that the positive expression of LEP in CRC tissues was correlated with tumor diameter(P=0.014), differentiation grade(
P
=0.012), TNM stage(P=0.014) and the presence of diabetes(P=0.023), while the positive expression of LEP in paracancerous tissues of CRC had no correlation with clinicopathological features(P>0.05). GCG showed characteristic deletion in CRC tissues, and there was a significant difference in the expression of GCG in paracancerous tissues between patients with stage Ⅰ/Ⅱ and stage Ⅲ/Ⅳ(P<0.05).【Conclusion】 The expression of LEP in CRC tissues and the expression of GCG in paracancerous tissues are significantly correlated with tumor progression and metabolic characteristics, which provides a new direction for the targeted therapy of metabolism-related CRC.

Efficacy of Aumetinib Combined with Gemcitabine and Cisplatin Chemotherapy in the Treatment of Advanced Non-Small Cell Lung Cancer

【Objective】 To investigate the efficacy of aumetinib combined with gemcitabine and cisplatin chemotherapy in the treatment of advanced non-small cell lung cancer(NSCLC).【Methods】 A total of 194 patients with advanced NSCLC were randomly divided into the observation group(98 cases) and the control group(96 cases). The control group received gemcitabine and cisplatin chemotherapy, while the observation group was additionally treated with aumetinib in addition to the control regimen. The efficacy, drug safety, one-year survival, as well as changes in tumor markers, pulmonary function, and immune function before and after treatment were compared between the two groups.【Results】 The objective response rate and disease control rate in the observation group were significantly higher than those in the control group(P<0.05). After treatment, the levels of tumor biomarkers in the observation group were significantly lower than those in the control group(P<0.05). Forced vital capacity(FVC) and the ratio of forced expiratory volume in one second to FVC(FEV
1
/FVC) were also higher in the observation group(P<0.05). The ratios of Th1/Th2 and Th17/Treg were significantly lower, while the CD4
+
/CD8
+
ratio was higher in the observation group compared to the control group(P<0.05). There was no statistically significant difference in the overall incidence of adverse drug reactions between the two groups(P>0.05). Kaplan-Meier survival analysis showed better survival outcomes in the observation group(P<0.05).【Conclusion】 Aumetinib combined with gemcitabine and cisplatin chemotherapy significantly improves short-term treatment efficacy in patients with advanced NSCLC, effectively reduces tumor marker levels, helps restore pulmonary ventilation function, modulates immune response, and demonstrates good safety.

Correlation Analysis of Pregnant Women's Thrombophilia Three Indices Activity, D-Dimer, and Homocysteine Levels with Fetal Growth Restriction

【Objective】 To explore the correlation between the activity of thrombophilia three indices [protein C(PC), protein S(PS), antithrombin-Ⅲ(AT-Ⅲ)], D-dimer, homocysteine(Hcy) levels in pregnant women and fetal growth restriction(FGR).【Methods】 A total of 38 pregnant women with FGR admitted to our hospital from January 2023 to January 2024 were selected as the FGR group, and 40 healthy pregnant women who delivered in our hospital during the same period were selected as the control group. Enzyme-linked immunosorbent assay(ELISA) was used to detect the activity of thrombophilia three indices, D-dimer, and Hcy levels in both groups. Logistic multivariate regression analysis was used to analyze their correlation with FGR.【Results】 The PC activity and AT-Ⅲ activity in the FGR group were lower than those in the control group, while the levels of D-dimer and Hcy were higher than those in the control group. The fetal tibial length, biparietal diameter, and head circumference in the FGR group were shorter/smaller than those in the control group, with statistically significant differences(P<0.05). Correlation analysis showed that PC activity, AT-Ⅲ activity, D-dimer, and Hcy levels were significantly correlated with FGR(P<0.05). Logistic multivariate regression analysis showed that low PC activity, low AT-Ⅲ activity, high D-dimer level, and high Hcy level were risk factors for FGR in pregnant women(P<0.05).【Conclusion】 Low PC activity, low AT-Ⅲ activity, high D-dimer level, and high Hcy level in pregnant women are significantly correlated with the occurrence of FGR. PC activity, AT-Ⅲ activity, D-dimer, and Hcy can be used as important markers to predict FGR in pregnant women, which is helpful for early diagnosis and prevention of FGR.

Effect of Medical-Education Integrated Training on Family Burden of Hospitalized Children after RSV Infection in Children

【Objective】 To explore the impact of medical-education integrated training on the family burden of hospitalized children with respiratory syncytial virus(RSV) infection.【Methods】 A total of 560 children hospitalized in the general pediatrics department of our hospital due to RSV infection from January 2022 to December 2023 were selected. The caregivers were trained through a medical-education integrated approach. The awareness rate of RSV infection knowledge among the caregivers before and after the training was compared, as well as the impact of the medical-education integrated training on the family burden of the children.【Results】 After the medical-education integrated training, the awareness rate of RSV infection knowledge among the caregivers was 81.07%(454/560), significantly higher than the pre-training rate of 39.82%(223/560) with a statistically significant difference(P<0.05). The mental health scores of family members of RSV-infected children decreased more significantly after the medical-education integration training(P<0.05), and the time-dependent burden decreased most significantly after the training(P<0.05).【Conclusion】 RSV infection has a significant impact on the family burden of hospitalized children. Medical-education integrated training can significantly improve the awareness of RSV infection knowledge among caregivers, and significantly reduce the psychological burden and time-dependent burden of caregivers.

Comparison of the Effects of Single-Port Thoracoscopic Segmentectomy and Single-Port Thoracoscopic Wedge Resection in Elderly Patients with Early-Stage NSCLC

【Objective】 To compare the clinical effects of single-port thoracoscopic segmentectomy and single-port thoracoscopic wedge resection in the treatment of elderly patients with early-stage non-small cell lung cancer(NSCLC).【Methods】 The clinical data of 98 elderly patients with early-stage NSCLC admitted to the department of thoracic surgery of Lankao County Central Hospital from June 2022 to April 2024 were prospectively selected. According to different surgical methods, the patients were divided into the observation group(treated with single-port thoracoscopic segmentectomy) and the control group(treated with single-port thoracoscopic wedge resection), with 49 cases in each group. The results of rapid pathological examination, perioperative indicators, postoperative pain and incidence of complications were compared between the two groups.【Results】 There was no statistically significant difference in the results of rapid lymph node pathological examination between the two groups(P>0.05). The operation time and postoperative drainage tube indwelling time in the observation group were longer than those in the control group, and the postoperative drainage volume and intraoperative blood loss were greater than those in the control group, with statistically significant differences(P<0.05). The Visual Analogue Scale(VAS) scores of the two groups at 9 h and 18 h after surgery were higher than those at 3 h after surgery, and the VAS scores at 36 h after surgery were lower than those at 9 h and 18 h after surgery; the VAS scores of the observation group at 9 h, 18 h and 36 h after surgery were lower than those of the control group, with statistically significant differences(P<0.05). The forced vital capacity and peak expiratory flow of the observation group at 3 months and 6 months after surgery were higher than those of the control group(P<0.05). There was no statistically significant difference in the total incidence of complications between the two groups(P>0.05).【Conclusion】 Compared with single-port thoracoscopic wedge resection, single-port thoracoscopic segmentectomy is more conducive to improving lung function in NSCLC patients, can better alleviate postoperative pain, and is safe and reliable

Effects of Different Position Fixation Methods on Margins, Positioning Errors, and Patient Comfort during Radiotherapy for Head and Neck Tumors

【Objective】 To investigate the effects of different position fixation methods on the determination of margins, control of positioning errors, and patient comfort during radiotherapy for head and neck tumor patients.【Methods】 A total of 256 patients with head and neck tumors undergoing radiotherapy were divided into two groups according to the fixation methods:128 patients received thermoplastic mask fixation(the control group), and 128 patients received thermoplastic mask combined with foam cushion fixation(the observation group). Positioning errors, margins, and treatment comfort were compared between the two groups.【Results】 In the observation group, positioning errors of the first and last treatments in the X-axis, Y-axis, and Z-axis directions were significantly lower than those in the control group(P<0.05). Margins in the X-axis, Y-axis, and Z-axis directions in the observation group were also significantly smaller than those in the control group(P<0.05), with greater uniformity. There was no statistically significant difference between the two groups in social, cultural, and environmental comfort scores(P>0.05). However, psychological comfort, physical comfort, and total comfort scores in the observation group were significantly higher than those in the control group(P<0.05).【Conclusion】 In radiotherapy for patients with head and neck tumors, combining thermoplastic mask with foam cushion for position fixation can significantly reduce positioning errors and margins, and enhance the comfort experience of patients throughout the treatment period.

Anesthetic Effects of Ultrasound-Guided Lumbosacral Plexus Nerve Block Combined with Propofol in Patients Undergoing Lower Limb Orthopedic Surgery

【Objective】 To investigate the anesthetic effects of ultrasound-guided lumbosacral plexus nerve block(PNS) combined with propofol in patients undergoing lower limb orthopedic surgery, and its influence on serum level of neurospecific protein S-100β.【Methods】 Ninety patients scheduled for lower limb orthopedic surgery were randomly divided into two groups:the observation group(n=45), which received ultrasound-guided PNS combined with propofol, and the control group(n=45), which received combined spinal-epidural anesthesia. Anesthetic effects, postoperative adverse reactions, and serum S-100β protein levels at different time points were compared between the two groups.【Results】 At 3, 6, 12, and 24 hours postoperatively, the visual analogue scale(VAS) pain scores in the observation group were significantly lower than those in the control group(P<0.05). The onset time of sensory nerve block was shorter in the observation group(P<0.05), while the duration of both sensory and motor nerve block in the observation group was longer than that in the control group(P<0.01). Mean arterial pressure at 10 minutes after anesthesia and 10 minutes after surgery were higher in the observation group(P<0.05). The incidence of adverse reactions was lower in the observation group compared to the control group. Serum S-100β protein levels at 30 minutes during surgery, and at 1 and 6 hours postoperatively, were significantly lower in the observation group(P<0.01).【Conclusion】 Ultrasound-guided PNS combined with propofol provides effective anesthesia for lower limb orthopedic surgery, with fewer adverse reactions and neurological complications. It is therefore worthy of clinical promotion.

Effect of Digital Subtraction Angiography-Guided Neurointerventional Thrombolysis on Vascular Endothelial Function in Patients with Acute Cerebral Infarction

【Objective】 To investigate the effect of digital subtraction angiography(DSA)-guided neurointerventional thrombolysis on vascular endothelial function in patients with acute cerebral infarction(ACI).【Methods】 A total of 86 patients with ACI undergoing thrombolytic therapy were randomly divided into two groups, with 43 patients in each group. The control group received intravenous thrombolysis, while the observation group was treated with DSA-guided neurointerventional thrombolysis. Vascular endothelial function [flow-mediated dilation(FMD), nitric oxide(NO), endothelin-1(ET-1), thromboxane A2(TXA2)], hemodynamic parameters [mean blood flow(Qm), pulse wave velocity(Wv), characteristic impedance(ZCV), peripheral resistance(RV)], serum neuronal injury markers [homocysteine(Hcy), nerve growth factor(NGF), neuron-specific enolase(NSE), brain-derived neurotrophic factor(BDNF)], and neurological function [Chinese Stroke Scale of Slinical Neurological Deficit Grade(CSS) and National Institutes of Health Stroke Scale(NIHSS) scores] were compared between the two groups before and after treatment.【Results】 After treatment, FMD and NO levels in the observation group were significantly higher, while ET-1 and TXA2 levels were significantly lower than those in the control group(all P<0.05). The observation group also showed higher Qm and lower Wv, ZCV, and RV(all P<0.05)than the control group. Levels of Hcy and NSE were lower, while NGF and BDNF were higher in the observation group compared to the control group(all P<0.05). Additionally, CSS and NIHSS scores in the observation group were significantly lower than those in the control group(P<0.05).【Conclusion】 DSA-guided neurointerventional thrombolysis in ACI patients can effectively improve cerebral blood flow parameters and vascular endothelial function, thereby promoting neurological recovery.

The Application Effect of Interesting Guidance Combined with Multidimensional Intervention in Children with Bronchopneumonia

【Objective】 To explore the application effect of interesting guidance combined with multidimensional intervention in children with bronchopneumonia.【Methods】 A total of 88 children with bronchopneumonia admitted to our hospital from January 2022 to December 2024 were selected. They were divided into the observation group and the control group by random number table method, with 44 cases in each group. The control group received routine intervention, while the observation group received interesting guidance combined with multidimensional intervention on the basis of routine intervention. The recovery status(antipyretic time, lung rales disappearance time, cough and wheeze disappearance time, hospital stay), pulmonary function indicators [vital capacity(VC), tidal volume(VT), maximum voluntary ventilation(MVV), functional residual capacity(FRC)], medical compliance behavior, and emotional status [Screen for Child Anxiety Related Emotional Disorders(SCARED) score] were compared between the two groups.【Results】 The antipyretic time, lung rales disappearance time, cough and wheeze disappearance time, and hospital stay in the observation group were shorter than those in the control group(P<0.05). After intervention, the VC, VT, MVV, and FRC in the observation group were higher than those in the control group(P<0.05); the scores of all items of medical compliance behavior in the observation group were higher than those in the control group(P<0.05); the scores of all dimensions of SCARED in the observation group were lower than those in the control group(P<0.05).【Conclusion】 Interesting guidance combined with multidimensional intervention can promote the rehabilitation of children with bronchopneumonia, improve pulmonary function, enhance medical compliance behavior, and alleviate negative emotions, which has good clinical practical value.

The Application Effect of Multidimensional Psychological Crisis Intervention Combined with Time-Dependent and Demand-Oriented Intervention in Perioperative Patients with Ovarian Cancer

【Objective】 To explore the application effect of multidimensional psychological crisis intervention combined with time-dependent and demand-oriented intervention in perioperative patients with ovarian cancer.【Methods】 A total of 118 perioperative patients with ovarian cancer treated in our hospital from January 2021 to December 2023 were selected. They were divided into the control group and the observation group according to the random number table method, with 59 cases in each group. The control group received routine intervention, while the observation group received multidimensional psychological crisis intervention combined with time-dependent and demand-oriented intervention. The perioperative indicators(indwelling time of urinary catheter, postoperative exhaust time, length of hospital stay) of the two groups were compared. Before intervention and 1 month after intervention, the stigma [Social Impact Scale(SIS) score for patients with chronic diseases], self-identity [Wallace Self-Concept Scale(WSCS) score], and quality of life [Functional Assessment of Cancer Therapy-Ovary(FACT-O) score] were compared between the two groups.【Results】 The indwelling time of urinary catheter, postoperative exhaust time, and length of hospital stay in the observation group were significantly shorter than those in the control group, with statistically significant differences(P<0.05). One month after intervention, the SIS scores of both groups were lower than those before intervention, while the WSCS scores and FACT-O scores were higher than those before intervention. Moreover, the SIS score of the observation group was lower than that of the control group, and the WSCS score and FACT-O score were higher than those of the control group, with statistically significant differences(P<0.05).【Conclusion】 Multidimensional psychological crisis intervention combined with time-dependent and demand-oriented intervention can optimize perioperative indicators, effectively promote the rehabilitation of perioperative patients with ovarian cancer, improve their quality of life, significantly reduce stigma, and enhance self-identity.

The Application Effect of Nasal Three-Needle Acupuncture Combined with Lung-Dispersing and Spleen-Strengthening Tuina in Children with Allergic Rhinitis

【Objective】 To explore the application effect of nasal three-needle acupuncture combined with lung-dispersing and spleen-strengthening Tuina in children with allergic rhinitis.【Methods】 A total of 92 children with allergic rhinitis admitted to our hospital from August 2023 to August 2024 were selected and grouped by the ball-touching method. Those with odd numbers were included in the control group(n=46), and those with even numbers were in the observation group(n=46). The control group received conventional Western medicine treatment, while the observation group was treated with nasal three-needle acupuncture combined with lung-dispersing and spleen-strengthening Tuina. The clinical efficacy, TCM syndrome scores, nasal physiological functions, and levels of inflammatory factors [tumor necrosis factor-α(TNF-α), C-reactive protein(CRP), interleukin-6(IL-6), interleukin-8(IL-8)] were compared between the two groups.【Results】 After treatment, the total effective rate of the observation group was higher than that of the control group(P<0.05). The TCM syndrome scores of nasal itching, nasal congestion, sneezing, and runny nose in the observation group were lower than those in the control group(P<0.05). The total nasal resistance of the observation group was lower than that of the control group(P<0.05), and the nasal ventilation area was larger than that of the control group(P<0.05). The levels of TNF-α, CRP, IL-6, and IL-8 in the observation group were lower than those in the control group(P<0.05).【Conclusion】 Nasal three-needle acupuncture combined with lung-dispersing and spleen-strengthening Tuina in the treatment of allergic rhinitis in children can significantly improve the total clinical effective rate, relieve clinical symptoms, reduce total nasal resistance and inflammatory factor levels, and enhance nasal mucosa function.

Clinical Value of RDW and NLR in Predicting Recurrence Risk of Glandular Cystitis Patients after Transurethral Plasmakinetic Resection

【Objective】 To explore the clinical value of red blood cell distribution width(RDW) and neutrophil-to-lymphocyte ratio(NLR) in predicting the recurrence risk of patients with glandular cystitis after transurethral plasmakinetic resection.【Methods】 A total of 184 patients with glandular cystitis who underwent transurethral plasmakinetic resection in our hospital from July 2019 to July 2022 were selected. According to the recurrence status, the patients were divided into the recurrence group(n=27) and the non-recurrence group(n=157). The levels of RDW and NLR before and after treatment were compared. Logistic multivariate regression analysis was used to identify the risk factors for postoperative recurrence of glandular cystitis. Receiver operating characteristic(ROC) curve was used to evaluate the clinical value of RDW and NLR in predicting postoperative recurrence of glandular cystitis.【Results】 After treatment, the levels of RDW and NLR in patients were lower than those before treatment, with statistically significant differences(P<0.05). The proportions of urinary tract infection, urinary calculi, high-risk clinical type, diffuse lesion location, and the levels of RDW and NLR in the recurrence group were higher than those in the non-recurrence group, with statistically significant differences(P<0.05). Logistic multivariate regression analysis showed that high-risk clinical type, high RDW level, and high NLR level were risk factors for recurrence of glandular cystitis after transurethral plasmakinetic resection(P<0.05). ROC curve results showed that the sensitivity and specificity of combined detection of RDW and NLR in predicting postoperative recurrence of glandular cystitis were 77.8% and 86.6% respectively, and the area under the ROC curve(AUC) was 0.865, which were better than those of either single detection(P<0.05).【Conclusion】 High levels of RDW and NLR are closely related to the increased recurrence risk of glandular cystitis. The combined detection of RDW and NLR has high sensitivity and specificity for predicting recurrence, and can be used as an effective predictive tool.

Clinical Efficacy of Sintilimab Combined with Bevacizumab in the Treatment of Patients with Advanced NSCLC and its Impact on Tumor Marker Levels

【Objective】 To explore the clinical efficacy of Sintilimab combined with bevacizumab in the treatment of patients with advanced non-small cell lung cancer(NSCLC) and its impact on the levels of tumor markers in patients.【Methods】 A total of 86 patients with advanced NSCLC treated in our hospital from January 2018 to October 2022 were selected. According to different treatment regimens, the patients were divided into the control group(treated with bevacizumab alone, n=38) and the observation group(treated with Sintilimab combined with bevacizumab on the basis of the control group, n=48). The efficacy, tumor marker levels, immune function indicators, quality of life scores, adverse reactions and survival status were compared between the two groups.【Results】 After treatment, the overall response rate(ORR) and disease control rate(DCR) of the observation group were higher than those of the control group(P<0.05). Effect of Qinqiao Oral Liquid Combined with Montelukast Sodium on the Treatment of Asthmatic Bronchitis in Children

【Objective】 To investigate the efficacy of Qinqiao oral liquid combined with Montelukast Sodium in the treatment of childhood asthmatic bronchitis.【Methods】 A total of 82 children with asthmatic bronchitis were randomly divided into a control group and a combination group, with 41 cases in each group. The control group received Montelukast Sodium, while the observation group received Montelukast Sodium plus Qinqiao oral liquid. The treatment time in both groups was 4 weeks. The clinical efficacy, improvement time of blood gas [Partial pressure of carbon dioxide(PCO

The Clinical Efficacy of Sacubitril/Valsartan in the Treatment of Patients with HFrEF and its Impact on Cardiac Function and Quality of Life

【Objective】 To explore the clinical efficacy of sacubitril/valsartan in the treatment of patients with heart failure with reduced ejection fraction(HFrEF) and its impact on patients' cardiac function and quality of life.【Methods】 The clinical data of 90 HFrEF patients admitted to Xuchang Central Hospital from May 2023 to May 2024 were selected. According to different treatment regimens, the patients were divided into the control group(routine anti-heart failure treatment) and the observation group(valsartan was discontinued on the basis of the control group, and sacubitril/valsartan sodium tablets were added), with 45 cases in each group. The clinical efficacy, cardiac function indicators [left ventricular ejection fraction(LVEF), left atrial diameter(LAD), left ventricular end-diastolic diameter(LVEDD)], ventricular remodeling indicators [interventricular septal thickness(IVST), left ventricular posterior wall thickness(LVPWT), left ventricular mass index(LVMI)], laboratory indicators [high-sensitivity C-reactive protein(hs-CRP), B-type natriuretic peptide(BNP)], quality of life [Minnesota Living with Heart Failure Questionnaire(MLHFQ) score] and exercise tolerance [6-minute walk distance(6MWD)] were compared between the two groups.【Results】 The clinical effective rate of the observation group was higher than that of the control group, with a statistically significant difference(P<0.05). After treatment, the LVEF of both groups was higher than that before treatment, while LAD and LVEDD were lower than those before treatment; the LVEF of the observation group was higher than that of the control group, and LAD and LVEDD were lower than those of the control group, with statistically significant differences(P<0.05). After treatment, the IVST, LVPWT and LVMI of both groups were lower than those before treatment, and the observation group was lower than the control group, with statistically significant differences(P<0.05). After treatment, the serum levels of hs-CRP and BNP in both groups were lower than those before treatment, and the observation group was lower than the control group, with statistically significant differences(P<0.05). After treatment, the MLHFQ scores of both groups were lower than those before treatment, and 6MWD was longer than that before treatment; the MLHFQ score of the observation group was lower than that of the control group, and 6MWD was longer than that of the control group, with statistically significant differences(P<0.05).【Conclusion】 Sacubitril/valsartan sodium tablets have good clinical efficacy in the treatment of HFrEF patients. They can effectively reduce the cardiac load of patients, inhibit pathological ventricular remodeling, and improve patients' quality of life and exercise tolerance.

Risk Factors and Nursing Interventions for Supine Hypotensive Syndrome during Cesarean Section

【Objective】 To explore the risk factors and nursing interventions for supine hypotensive syndrome(SHS) during cesarean section.【Methods】 A total of 80 parturients who underwent cesarean section in our hospital from January 2023 to January 2025 were selected as the research subjects. They were divided into the non-SHS group(n=59) and the SHS group(n=21) according to whether SHS occurred during the operation. Univariate analysis and Logistic multivariate regression analysis were used to identify the risk factors for SHS in parturients during cesarean section.【Results】 Among the 80 parturients undergoing cesarean section, 21 cases developed SHS. Univariate analysis showed that age, fear of childbirth, surgical position, and oxygenation index(OI) were influencing factors for SHS in parturients during cesarean section(P<0.05). Logistic multivariate regression analysis revealed that elderly parturients, supine surgical position, fear of childbirth, and low OI were independent risk factors for SHS in parturients during cesarean section(P<0.05).【Conclusion】 There are multiple influencing factors for SHS in parturients during cesarean section. Among them, elderly parturients, supine surgical position, fear of childbirth, and low OI are independent risk factors. Nurses should strengthen the monitoring of parturients' vital signs, assist parturients in adopting the left lateral position during surgery, alleviate their fear of childbirth, and maintain normal OI of the body, so as to effectively prevent SHS during cesarean section.

Diagnostic Value of Quantitative Detection of EBV-DNA in Bronchoalveolar Lavage Fluid for Transplant-Related Pulmonary Infection

【Objective】 To explore the diagnostic value of quantitative detection of Epstein-Barr virus(EBV)-DNA in bronchoalveolar lavage fluid(BALF) for transplant-related pulmonary infection.【Methods】 A retrospective analysis was conducted on 912 clinical samples from patients with transplant-related pulmonary infection admitted to our hospital from May 2023 to October 2024, including 624 whole blood samples and 288 BALF samples. Among them, 163 patients submitted both whole blood samples and BALF samples for testing. The EBV positive rate, EBV-DNA load of the two types of samples, as well as the EBV positive rate and EBV-DNA load in different clinical groups(hematological disease transplantation group, hematological disease non-transplantation group, non-hematological disease group) were compared.【Results】 The EBV positive rate and EBV-DNA load of BALF samples were significantly higher than those of whole blood samples(P<0.01). The EBV positive rate of BALF in the hematological disease transplantation group(59.13%) was significantly higher than that in the non-transplantation group(28.95%) and the non-hematological disease group(50.00%)(P=0.0054).【Conclusion】 The detection of EBV-DNA in BALF samples has higher diagnostic sensitivity than that in whole blood samples. It is especially suitable for the early identification and dynamic monitoring of pulmonary infection after hematological disease transplantation, and can provide an important basis for clinical precise intervention.

The Therapeutic Effects of Amlodipine Combined with Atorvastatin Calcium in Managing Hypertension Concurrent with Coronary Heart Disease

【Objective】 This study aimed to assess the therapeutic efficacy of combining amlodipine with atorvastatin calcium in patients with concurrent hypertension and coronary heart disease(CHD).【Methods】 A total of 300 patients with coexisting hypertension and CHD, treated at our hospital from January 2022 to December 2024, were selected. They were randomly assigned into two groups of 150 patients each. The control group received standard treatment with amlodipine and fluvastatin sodium, whereas the intervention group was treated with amlodipine in combination with atorvastatin calcium. Key clinical indicators, including blood pressure [systolic blood pressure(SBP), diastolic blood pressure(DBP)], lipid profiles [total cholesterol(TC), triglycerides(TG), high-density lipoprotein cholesterol(HDL-C), low-density lipoprotein cholesterol(LDL-C)], cardiac function [cardiac index(CI), cardiac output(CO), left ventricular ejection fraction(LVEF)], endothelial function [nitric oxide(NO), endothelin-1(ET-1)], and inflammatory markers [C-reactive protein(CRP), homocysteine(Hcy)], were compared between the two groups.【Results】 After treatment, SBP and DBP in both groups were lower than before treatment, with statistically significant differences(P<0.05). And the intervention group showed significantly lower systolic and diastolic blood pressure levels compared to the control group(P<0.05). Serum TC, TG, and LDL-C levels in both groups were lower than before treatment, while the intervention group demonstrated lower levels of TC, TG, and LDL-C, and higher levels of HDL-C(P<0.05) compare to the control group. After treatment, LVEF, CI, and CO levels in both groups were higher than before treatment, with the observation group having a higher level than the control group(P<0.05). After treatment, serum NO levels in both groups were higher than before treatment, with the observation group having a higher level than the control group. Serum ET-1 levels and IMT were lower than before treatment, with the observation group having a lower level than the control group(P<0.05). Serum CRP and Hcy levels in both groups were lower than before treatment, with the observation group having a lower level than the control group(P<0.05).【Conclusion】 The combined use of amlodipine and atorvastatin calcium offers superior benefits in managing both hypertension and lipid levels, while also improving cardiac and endothelial function and attenuating systemic inflammation. This regimen presents a promising approach in the clinical management of patients with coexisting hypertension and CHD.

【Objective】 To explore the clinical efficacy of low-frequency repetitive transcranial magnetic stimulation(rTMS) on bilateral dorsolateral prefrontal cortex(DLPFC) in the treatment of patients with insomnia after ischemic stroke.【Methods】 A total of 90 patients with post-ischemic stroke insomnia treated in our hospital from August 2022 to October 2023 were selected and randomly divided into the observation group and the control group by envelope method, with 45 cases in each group. The control group received routine basic treatment for stroke and rehabilitation nursing, while the observation group was additionally given low-frequency rTMS treatment on the basis of the control group. The sleep quality [Pittsburgh Sleep Quality Index(PSQI) score] and polysomnography monitoring indicators [total sleep time(TST), sleep efficiency(SE), sleep latency(SL), rapid eye movement sleep(REM), non-rapid eye movement sleep stage 1(N1), non-rapid eye movement sleep stage 2(N2), non-rapid eye movement sleep stage 3(N3)] were compared between the two groups before and after treatment.【Results】 After treatment, the PSQI score of the observation group was significantly lower than that of the control group(P<0.05). The TST, REM, N2, and N3 of the observation group were longer than those of the control group; the SL and N1 were shorter than those of the control group; the SE was higher than that of the control group, with statistically significant differences(P<0.05).【Conclusion】 Low-frequency rTMS on bilateral DLPFC in the treatment of patients with post-ischemic stroke insomnia can significantly improve the patients' sleep status.

Comparison of Mesh Fixation Versus Non-Fixation in Laparoscopic Transabdominal Preperitoneal Inguinal Hernia Repair

【Objective】 To compare the clinical outcomes of mesh fixation versus non-fixation in laparoscopic transabdominal preperitoneal(TAPP) inguinal hernia repair.【Methods】 A total of 88 patients with inguinal hernia treated at Xinye County Health Vocational Secondary School Affiliated Hospital from January 2021 to October 2024 were included in this study. All patients underwent laparoscopic TAPP inguinal hernia repair and were randomly assigned into two groups using a random number table:the observation group(non-fixation of mesh, n=44) and the control group(mesh fixation, n=44). The two groups were compared in terms of mesh placement time, peritoneal suturing time, blood loss, operative time, length of hospital stays, Visual Analog Scale(VAS) scores, and the incidence of postoperative complications.【Results】 The bleeding volume of the observation group was lower than that of the control group, and the operation time and hospitalization time were shorter than those of the control group, with statistical significance(P<0.05); There was no statistically significant difference(P>0.05) in the placement time and peritoneal suturing time between the two networks. The VAS scores of the observation group were lower than those of the control group on the 2nd day, 1st month, and 3rd month after surgery, and the difference was statistically significant(P<0.05). The total incidence of complications within 3 months after surgery in the observation group and control group was 22.73%(10/44) and 18.18%(8/44), respectively. There was no statistically significant difference in the incidence of postoperative complications between the two groups(χ

=0.279,P>0.05).【Conclusion】 Non fixed mesh laparoscopic TAPP treatment for inguinal hernia has a good effect, which can effectively shorten the operation time and hospitalization time, as well as reduce postoperative pain. It has high safety and is worthy of clinical reference.

The Impact of Olanzapine Combined with Cognitive Behavioral Therapy on Quality of Life and Clinical Efficacy in Patients with Schizophrenia

【Objective】 To explore the impact of olanzapine combined with cognitive behavioral therapy(CBT) on the quality of life and clinical efficacy in patients with schizophrenia.【Methods】 A total of 106 patients with schizophrenia admitted to our hospital from January 2023 to January 2025 were selected. According to different treatment methods, they were divided into the observation group(olanzapine combined with CBT) and the control group(olanzapine alone), with 53 cases in each group. Both groups received treatment for 12 weeks. The personality traits and psychological status [Minnesota Multiphasic Personality Inventory(MMPI) scores], rehabilitation efficacy [Inpatient Psychiatric Rehabilitation Outcome Scale(IPROS) scores], quality of life [World Health Organization Quality of Life-Brief Version(WHOQOL-BREF) scores], and symptom improvement [Positive and Negative Syndrome Scale(PANSS) scores] were compared between the two groups before and after treatment.【Results】 After treatment, the scores of negative symptoms, general symptoms, and positive symptoms in both groups were lower than those before treatment; the scores of negative symptoms and general symptoms in the observation group were lower than those in the control group, with statistically significant differences(P<0.05). After treatment, the scores of work therapy, life ability, social ability, care and interest, and hygiene ability in both groups were lower than those before treatment; the scores in the observation group were lower than those in the control group, with statistically significant differences(P<0.05). After treatment, the MMPI scores of both groups were lower than those before treatment; the score in the observation group was lower than that in the control group, with a statistically significant difference(P<0.05). After treatment, the scores of physical health, social relations, environmental dimension, and mental health in both groups were higher than those before treatment; the scores in the observation group were higher than those in the control group, with statistically significant differences(P<0.05).【Conclusion】 Olanzapine combined with CBT has a significant effect on relieving negative symptoms and general symptoms of schizophrenia patients, achieves good rehabilitation efficacy, can improve patients' personality traits and psychological status, and enhance their quality of life.

Application Value of Intravenous Thrombolysis in Patients with Non-Disabling Mild Acute Ischemic Stroke

【Objective】 To explore the application value of intravenous thrombolysis in patients with non-disabling mild acute ischemic stroke.【Methods】 A total of 61 patients with non-disabling mild acute ischemic stroke were divided into an observation group(intravenous thrombolysis combined with dual antiplatelet therapy, n=33) and a control group(dual antiplatelet therapy, n=28) according to different treatment methods. The National Institutes of Health Stroke Scale(NIHSS) scores at 24 h, 72 h, and 7 days after treatment were compared between the two groups. At 3 months after treatment, the modified Rankin Scale(mRS) score, Extended Barthel Index(EBI) score, hematological indices [interleukin-6(IL-6), high-sensitivity C-reactive protein(hs-CRP), and lipoprotein-associated phospholipase A2(Lp-PLA2)], as well as the incidences of symptomatic intracranial hemorrhage, neurological deterioration, and stroke recurrence, were evaluated.【Results】 The NIHSS scores at 24 h, 72 h, and 7 days after treatment in the observation group were significantly lower than those in the control group(P<0.05). After 3 months of treatment, the mRS score, IL-6, and hs-CRP levels in the observation group were lower than those in the control group, while the EBI score and Lp-PLA2 levels were higher than those in the control group(all P<0.05). There were no statistically significant differences in the total incidence of symptomatic intracranial hemorrhage, neurological deterioration, or stroke recurrence between the two groups within 3 months(P>0.05).【Conclusion】 Intravenous thrombolysis can significantly improve early neurological function, promote long-term functional recovery, and reduce systemic inflammatory responses in patients with non-disabling mild acute ischemic stroke, with good safety.

Therapeutic Effect of Low-Frequency Neuromuscular Electrical Stimulation Combined with Robot-Assisted Rehabilitation Training in Patients with Post-Stroke Hemiplegia

【Objective】 To investigate the therapeutic effect of low-frequency neuromuscular electrical stimulation(NMES) combined with robot-assisted rehabilitation training in patients with post-stroke hemiplegia.【Methods】 A total of 118 patients with post-stroke hemiplegia were divided into a control group(n=62, treated with robot-assisted rehabilitation training alone) and an observation group(n=56, treated with low-frequency NMES combined with robot-assisted rehabilitation training) according to different treatment methods. The efficacy, changes in muscle tone [Modified Ashworth Scale(MAS)], neuro-electrophysiological parameters, neurological function, motor function, and quality of life before and after treatment were compared between the two groups.【Results】 The total effective rate was 94.64%(53/56) in the observation group, which was significantly higher than 83.87%(52/62) in the control group(P<0.05). After treatment, MAS scores and levels of central nervous system-specific protein S-100β in both groups were significantly lower than those before treatment, with even lower values in the observation group compared to the control group(P<0.05). Fugl-Meyer Assessment(FMA) scores, Berg Balance Scale(BBS) scores, Modified Barthel Index(MBI), sensory and motor nerve conduction velocities of the peroneal and tibial nerves, as well as levels of brain-derived neurotrophic factor and nerve growth factor, were significantly increased in both groups after treatment(P<0.05). After treatment, the above indicators in the observation group were higher than those in the observation group(P<0.05).【Conclusion】 Low-frequency NMES combined with robot-assisted rehabilitation training can effectively improve muscle tone, neuro-electrophysiological parameters, neurological and motor functions in patients with post-stroke hemiplegia, thereby enhancing treatment efficacy and improving quality of life.

【Objective】 To explore the intervention effect of responsibility-based holistic intervention combined with picture-text communication on children with pneumonia.【Methods】 A total of 82 children with pneumonia who were treated in our hospital from January 2022 to December 2024 were selected and divided into a control group and an observation group by convenience sampling, with 41 cases in each group. The control group received responsibility-based holistic intervention, while the observation group received the same intervention combined with picture-text communication. The symptom relief status, treatment compliance, medical fear(Children's Medical Fear Scale score), and quality of life(Children's Quality of Life Scale score) of the two groups were compared.【Results】 The cough cessation time, wheezing disappearance time, and hospitalization time of the observation group were shorter than those of the control group(P<0.05). The treatment compliance of the observation group was higher than that of the control group(P<0.05). After the intervention, the scores of medical fear in the observation group were lower than those in the control group(P<0.05), and the scores of quality of life were higher than those in the control group(P<0.05).【Conclusion】 Responsibility-based holistic intervention combined with picture-text communication can promote the relief of symptoms in children with pneumonia, improve treatment compliance, reduce medical fear, and improve quality of life, which is worthy of clinical promotion and application.

Effect of Injection of Tirofiban through Thrombus Aspiration Catheter on TIMI Blood Flow Grad and Cardiac Function in Acute STEMI Patients Undergoing Emergency PCI

【Objective】 To investigate the effects of tirofiban injected via thrombus aspiration catheter on thrombolysis in myocardial infarction(TIMI) flow grade and cardiac function in patients with acute ST-segment elevation myocardial infarction(STEMI) undergoing primary percutaneous coronary intervention(PCI).【Methods】 A total of 82 patients with acute STEMI admitted to our hospital from January 2022 to January 2024 were selected. All patients underwent emergency PCI and were divided into an observation group and a control group based on different treatment methods, with 41 groups in each group. All patients underwent thrombus aspiration first. The control group underwent PCI surgery directly, while the observation group underwent PCI surgery after injection of tirofiban through the aspiration catheter. Laboratory indicators [creatine kinase isoenzyme(CK-MB), C-reactive protein(CRP)], cardiac function indicators [left ventricular ejection fraction(LVEF), left ventricular end diastolic diameter(LVEDD)], and myocardial infarction thrombolysis test(TIMI) blood flow grading before PCI procedure and at 7 days after PCI surgery were compared between the two groups. The incidence of major adverse cardiovascular events(MACE) within 6 months after surgery were compared between two groups of patients.【Results】 Seven days after surgery, the serum CK-MB and CRP levels in both groups were lower than before surgery, and the levels of CK-MB and CRP in the observation group was lower than those in the control group, with a statistically significant difference(P<0.05). After surgery, both groups of patients had higher LVEF and lower LVEDD than before surgery, while patients in the observation group had higher LVEF and lower LVEDD than those in the control group, with statistical significance(P<0.05). The proportion of patients with TIMI blood flow grade 3 in the observation group was significantly higher than that in the control group, and the difference was statistically significant(P<0.05). After a 6-month follow-up, the incidence of MACE in the observation group was 9.76%(4/41), which was lower than 31.71%(13/41)in the control group, and the difference was statistically significant(P<0.05).【Conclusion】 Injection of tirofiban through thrombus aspiration catheter can effectively inhibit the inflammatory response of acute STEMI patients undergoing emergency PCI, reduce the levels of myocardial injury markers, restore blood flow perfusion to ischemic myocardial tissue, improve patient cardiac function, and reduce the incidence of MACE.

【Objective】 To explore the clinical efficacy of Shenshu Tiaozhong Decoction in the treatment of elderly patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD) complicated with multidrug resistant bacterial infection.【Methods】 A total of 82 elderly AECOPD patients with multi-drug resistant bacterial infection, who were treated in our hospital from January 2022 to April 2024, were selected and they were randomly divided into an observation group(treated with Shenshu Tiaozhong Decoction in addition to Western medicine) and a control group(treated with Western medicine alone) according to the random number table method, with 41 cases in each group. Correlation of Serum ANGPTL8, sICAM-1, and sFlt-1 with Cerebrovascular Vasospasm after Interventional Therapy in Patients with Intracranial Aneurysm

【Objective】 To investigate the correlation and predictive value of serum angiopoietin-like protein 8(ANGPTL8), soluble intercellular adhesion molecule-1(sICAM-1), and soluble fms-like tyrosine kinase-1(sFlt-1) levels with the occurrence of cerebrovascular vasospasm(CVS) after interventional therapy in patients with intracranial aneurysm(ICA).【Methods】 A total of 90 ICA patients were enrolled and divided into the CVS group(38 cases) and non-CVS group(52 cases) according to whether CVS occurred after surgery. Serum levels of ANGPTL8, sICAM-1, and sFlt-1 were compared between the two groups. Logistic regression was used to analyze influencing factors for postoperative CVS. Receiver operating characteristic(ROC) curves were employed to assess the diagnostic value of ANGPTL8, sICAM-1, and sFlt-1 in predicting CVS after interventional therapy.【Results】 The proportions of patients with hypertension, Hunt-Hess grade Ⅲ-Ⅳ, and Glasgow Coma Scale(GCS) score of 3-8 were significantly higher in the CVS group compared with the non-CVS group(P<0.05). Serum ANGPTL8, sICAM-1, and sFlt-1 levels were also significantly higher in the CVS group(P<0.05). Logistic univariate analysis showed that hypertension history, Hunt-Hess grade, GCS score, ANGPTL8, sICAM-1, and sFlt-1 were all risk factors for postoperative CVS(all P<0.05). Multivariate analysis further indicated that hypertension history, Hunt-Hess grade, GCS score, VCAM-1, ANGPTL8, sICAM-1, and sFlt-1 were independent risk factors for postoperative CVS(P<0.05). ROC curve analysis demonstrated that combined detection of serum ANGPTL8, sICAM-1, and sFlt-1 achieved the highest diagnostic efficiency(P<0.05).【Conclusion】 Significant elevations in serum ANGPTL8, sICAM-1, and sFlt-1 are closely associated with the occurrence of CVS after interventional therapy in ICA patients. These biomarkers may help in the early identification of high-risk CVS patients.

Analgesic Effect of Ropivacaine Combined with Sufentanil on Intrathecal Anesthesia in Painless Delivery

【Objective】 To investigate the analgesic effect of ropivacaine combined with sufentanil on epidural anesthesia(CSEA) in women undergoing painless delivery.【Methods】 A total of 100 parturients undergoing painless delivery were randomly divided into a control group and an observation group, with 50 cases in each group. The control group received ropivacaine via CSEA, while the observation group received a combination of ropivacaine and sufentanil via CSEA. The following outcomes were compared between the two groups:duration of labor stages, analgesic effect [visual analogue scale(VAS) scores], serum biomarkers [prolactin(PRL), adiponectin(APN), tissue-type plasminogen activator(t-PA)], delivery outcomes, and incidence of adverse reactions.【Results】 The duration of the first, second, and third stages of labor was significantly shorter in the observation group than in the control group(P<0.05). The onset time of effective analgesia was shorter, and the duration of analgesia was longer in the observation group. VAS scores at 5, 15, 30, and 60 minutes after analgesia in the observation group were significantly lower than those in the control group(P<0.05). Serum levels of PRL and APN in the observation group were higher than those in the control group, while t-PA level was lower in the observation group compared to the control group(P<0.05). The rate of spontaneous vaginal delivery was higher in the observation group(P<0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).【Conclusion】 Ropivacaine combined with sufentanil for CSEA can significantly shorten labor duration, enhance analgesic efficacy, improve the rate of spontaneous delivery, and demonstrate good safety in painless labor.

Therapeutic Effect of Digital Subtraction Angiography Guided Neurointervention Combined with Alteplase in Patients with Ischemic Cerebrovascular Disease

【Objective】 To investigate the therapeutic effect of digital subtraction angiography(DSA)-guided neurointervention combined with alteplase in patients with ischemic cerebrovascular disease(ISCV).【Methods】 A total of 102 patients with ISCV were randomly assigned into the control group and the observation group, with 51 cases in each. The control group received intravenous thrombolysis with alteplase, while the observation group received DSA-guided neurointervention combined with alteplase. Clinical efficacy, vascular endothelial function, neurological function [National Institutes of Health Stroke Scale(NIHSS) score], quality of life [Activities of Daily Living(ADL) score], cerebral hemodynamic parameters, and incidence of complications were compared between the two groups before treatment and 12 weeks after treatment.【Results】 The total effective rate in the observation group was higher than that in the control group(P<0.05). After treatment, brachial-ankle pulse wave velocity(ba-PWV) and ankle-brachial index(ABI) levels in the observation group were lower, while flow-mediated dilation(FMD) was higher compared with the control group(P<0.05). The NIHSS score in the observation group was lower than that in the control group, and the ADL score in the observation group was higher than that in the control group(P<0.05). Peak systolic velocity(Vs) and end-diastolic velocity(Vd) in the observation group were lower than those in the control group(P<0.05). The overall incidence of complications was also lower in the observation group compared to the control group(P<0.05).【Conclusion】 DSA-guided neurointervention combined with alteplase has a superior therapeutic effect in ISCV patients. It can improve vascular endothelial function and neurological function, enhance quality of life, and reduce the incidence of complications.

Influencing Factors of Return of Spontaneous Circulation and Neurological Recovery in Out-of-Hospital Cardiac Arrest Patients

【Objective】 To investigate the factors influencing the return of spontaneous circulation(ROSC) and neurological recovery in patients with out-of-hospital cardiac arrest(OHCA).【Methods】 Clinical data of 90 OHCA patients were collected. Clinical characteristics were compared between the ROSC and non-ROSC groups, and risk factors for poor neurological recovery within 28 days were analyzed among ROSC patients.【Results】 Among the 90 patients, 69 achieved ROSC and 21 did not. The ROSC group had a higher proportion of initial shockable rhythms and epinephrine use, and a lower Acute Physiology and Chronic Health Evaluation Ⅱ(APACHEⅡ) score compared to the non-ROSC group(P<0.05). Multivariate logistic regression analysis showed that initial shockable rhythm and APACHEⅡ score were independent factors associated with ROSC(P<0.05). Among the 69 patients who achieved ROSC, 22 had good neurological outcomes(Group A) and 47 had poor neurological outcomes(Group B) after 28 days. Group A had higher proportions of initial shockable rhythms, antiarrhythmic drug use, and hemoglobin(Hb) level compared to Group B(P<0.05), while the use of epinephrine, vasopressors, and sodium bicarbonate in Group A was lower than those in Group B(P<0.05). Multivariate logistic regression analysis identified initial shockable rhythm and Hb level as independent risk factors for poor neurological outcomes in ROSC patients(P<0.05).【Conclusion】 The prognosis of OHCA patients after cardiopulmonary resuscitation is influenced by multiple factors. The main factors affecting ROSC are initial shockable rhythm and APACHEⅡ score, while independent risk factors for poor neurological recovery include initial shockable rhythm and hemoglobin level.
