The Impact of Midwife-Led Partnered Doula Support under the AIDET Communication Model on Coping Styles in Primiparous Women

【Objective】To explore the effect of midwife-led partnered doula support under the AIDET communication model on coping styles in primiparous women. 【Methods】 A total of 122 primiparous women admitted to our hospital from January 2023 to October 2024 were selected and divided into the control group (receiving routine care, n=57) and the observation group (receiving midwife-led partnered doula support under the AIDET communication model, n=65) based on different nursing approaches. The labor duration, incidence of postpartum complications, and levels of childbirth fear [Childbirth Attitudes Questionnaire (CAQ)] and coping styles [Simplified Coping Style Questionnaire (SCSQ)] before and after the intervention were compared between the two groups.【Results】After the intervention, the observation group showed significantly shorter durations in the first, second, third, and total stages of labor compared to the control group (P<0.05). Post-intervention, the observation group had lower scores in fears related to infant health, loss of control during delivery, pain and injury, and hospital interventions/environment (P<0.05). Additionally, the observation group exhibited higher positive coping scores (P<0.05) and lower negative coping scores (P<0.05) than the control group. The total incidence of postpartum complications in the observation group was significantly lower than that in the control group (P<0.05). 【Conclusion】Midwife-led partnered doula support under the AIDET communication model can significantly shorten labor duration, alleviate childbirth fear, improve coping styles, and reduce the incidence of postpartum complications in primiparous women, demonstrating positive clinical value in childbirth care.

Correlation of Toll-Like Receptor 4, Nuclear Factor-κB and Interleukin-17 Levels with Wheezing in Children with Mycoplasma Pneumonia

【Objective】To investigate the relationship of the levels of Toll-like receptor 4 (TLR4), nuclear factor-κB (NF-κB) and interleukin-17 (IL-17) with the occurrence of wheezing in children with mycoplasma pneumonia (MP). 【Methods】 A total of 82 children with MP were enrolled, Based on the presence of clinical symptoms of wheezing and auscultatory findings of wheezing sounds, the children with wheezing were assigned to the observation group, and those without wheezing were assigned to the control group. The serum levels of TLR4, NF-κB, and IL-17 were compared between the two groups. Logistic regression analysis was used to assess the relationship of levels of TLR4, NF-κB and IL-17 with the occurrence of wheezing in the children. The receiver operating characteristic (ROC) curve was employed to evaluate the predictive value of these markers for the occurrence of wheezing in the children with mycoplasma pneumonia. 【Results】 The levels of TLR4, IL-17, and NF-κB in the observation group were significantly higher than those in the control group (all P<0.05). Logistic regression analysis revealed that the levels of TLR4, NF-κB, and IL-17 were positively correlated with the risk of wheezing in the children (all P<0.05). The ROC curve indicated that the combined detection of serum TLR4, NF-κB, and IL-17 had significantly higher sensitivity, specificity, and area under the curve for predicting wheezing in children with MP compared with the detection of a single marker (all P<0.05). 【Conclusion】 Elevated levels of TLR4, NF-κB, and IL-17 are significantly associated with an increased risk of wheezing in children with MP. The combined detection of these three markers exhibits high predictive sensitivity and specificity.

Comparison of Clinicopathologic Features of Gastric Carcinoma in Shandong Han and Xinjiang Uygur

【Objective】To compare the clinicopathological characteristics of patients with gastric carcinoma in Xinjiang Uyghur and Shandong Han ethnic groups. 【Methods】A total of 144 specimens of primary gastric cancer surgically resected in Shandong Provincial Third Hospital and the Third Clinical Medical College of Xinjiang Medical University (Affiliated Cancer Hospital) from January 2018 to December 2020 were collected, including 94 cases of Han ethnicity in Shandong (Han group) and 50 cases of Uyghur ethnicity in Xinjiang (Uyghur ethnicity group). The clinicopathological characteristics and HER2 expression of gastric cancer in the two groups were compared. 【Results】 The average age of onset of gastric cancer in the Uyghur group was lower than that in the Han group, and the proportion of patients aged 40-65 in the Uyghur group was higher than that in the Han group. The proportion of male patients in both groups of gastric cancer patients was higher than that of female patients. The proportion of gastric cancer located at the gastroesophageal junction in the Uyghur group was higher than that in the Han group. The proportion of poorly differentiated gastric cancer in the Uyghur group was higher than that in the Han group, while the proportion of moderately differentiated gastric cancer was lower than that in the Han group. The proportion of diffuse and intestinal-type gastric cancer in the Han group was higher than that in the Uyghur group, while the proportion of mixed gastric cancer was lower than that in the Uyghur group. in gastric cancer tissues of the Uyghur group was lower than that of the Han group, while the expression rate of HER2 protein 2

was higher than the Han group. The number of HER2-negative intestinal and mixed gastric cancer patients in the Uyghur group was lower than in the Han group, while the number of HER2-positive mixed gastric cancer patients with han was higher than in the Uygur group. The difference in HER2 protein expression was correlated with ethnicity, tumor differentiation degree, and Lauren classification (P<0.05). 【Conclusion】Gastric cancer patients of Uyghur ethnicity in Xinjiang and Han ethnicity in Shandong have different clinicopathological characteristics. The differences in HER2 protein expression in gastric cancer with different racial genetic backgrounds may affect the prognosis of patients.

and IC in the observation group were lower than those in the control group (P<0.05); FEV

and IC in patients with grade Ⅳ were lower than those in patients with grade Ⅰ, Ⅱ and Ⅲ (P<0.05). Correlation analysis showed that FEV

and IC were negatively correlated with lung function grading (P<0.05). ROC analysis results showed that the sensitivity and specificity of FEV
1
combined with IC in diagnosing ACO patients were 84.6% and 72.3% respectively, and the area under the ROC curve was 0.806, which were better than those of single index detection. 【Conclusion】FEV
1
and IC are significantly decreased in patients with ACO, and are negatively correlated with lung function grading. The combined detection of FEV
1
and IC has certain diagnostic value for ACO, with good clinical application prospects.

Analysis of the Influencing Factors and Correlation between Self-Aversion and Self-Management Ability in Middle-Aged and Young Stroke Patients

【Objective】 To explore the influencing factors and correlation between self-aversion and self-management ability in middle-aged and young stroke patients. 【Method】 Statistical analysis was conducted to compare the differences in the scores of the Questionnaire for the Assessment of Self-Disgust(QASD) and the Stroke Self-Management Ability Scale (SSSMQ) among 228 middle-aged and young stroke patients with different demographic characteristics. Logistic regression analysis was used to identify the relevant factors that affect the scores of QASD and SSSMQ in patients and analyze the correlation between QASD and SSSMQ scores. 【Result】The QASD score of middle-aged and young stroke patients who were not the main source of income for their families, had no negative life events, and had no underlying diseases was significantly lower than that of the corresponding patients (P<0.05). The SSSMQ score of patients who were not the main source of income for their families, had no negative life events, were extroverted, and had no underlying diseases was significantly higher than that of the corresponding patients (P<0.05). The results of logistic regression analysis showed that the main source of income in the family, negative life events, and comorbidities were significant influencing factors (P<0.05) for patients' QASD and SSSMQ scores, and personality was also an influencing factor (P<0.05) for SSSMQ score. Pearson correlation analysis revealed a significant negative correlation (r=-0.105, P<0.05) between patients' QASD score and SSSMQ score.【Conclusion】 Multiple factors affect the self-aversion and self-management ability of middle-aged and young stroke patients, and there is a significant negative correlation between patients' self-aversion and self-management ability.

To Construct and Validate a Risk Model for Corneal Epithelial Damage Related to Dry Eye after Cataract Surgery in Patients with Cataract and Dry Eye

【Objective】To construct and validate a risk model for corneal epithelial damage related to dry eye after cataract surgery in patients with cataract and dry eye. 【Methods】 A total of 112 patients with cataract and dry eye who underwent surgery in our hospital from July 2020 to July 2023 were randomly divided into a training set (n=90) and a validation set (n=22) according to the 80/20 rule. All patients underwent intraocular lens implantation after enrollment. They were divided into the damage group and the non-damage group based on whether corneal epithelial damage related to dry eye occurred 4 weeks after surgery. The influencing factors of corneal epithelial damage related to dry eye after cataract surgery in patients with cataract and dry eye were screened, and a predictive model was constructed and validated. 【Results】Among the 90 patients with cataract and dry eye, 34 (37.78%) developed corneal epithelial damage related to dry eye. The proportion of patients with diabetes, corneal edema, elevated intraocular pressure, Ocular Surface Disease Index (OSDI), and corneal fluorescein staining score in the damage group was higher than that in the non-damage group (P<0.05). Diabetes, corneal edema, elevated intraocular pressure, high OSDI, and high corneal fluorescein staining score were risk factors for corneal epithelial damage related to dry eye after cataract surgery in patients with cataract and dry eye (P<0.05). The sensitivity, specificity, and area under the curve of the nomogram model for predicting corneal epithelial damage related to dry eye were 0.801, 0.739, and 0.828, respectively. The sensitivity, specificity, and area under the curve of the nomogram model for validating corneal epithelial damage related to dry eye in patients with cataract and dry eye after surgery were 0.799, 0.698, and 0.810, respectively. 【Conclusion】Diabetes, corneal edema, elevated intraocular pressure, high OSDI, and high corneal fluorescein staining score are associated with corneal epithelial damage related to dry eye after cataract surgery in patients with cataract and dry eye. The construction of a risk model is helpful for predicting corneal epithelial damage caused by dry eye.

Effect of Tirofiban Local Intra-Arterial Catheter Thrombolysis on Platelet Parameters, Serum S100B and NSE in Patients with Cerebral Infarction

【Objective】 To investigate the effects of tirofiban local intra-arterial catheter thrombolysis (TTT) on platelet parameters and serum levels of S100B protein and neuron-specific enolase (NSE) in patients with cerebral infarction.【Methods】 A total of 110 patients with cerebral infarction were randomly divided into an observation group and a control group, with 55 cases in each group. The control group received standard drug therapy, while the observation group received additional TTT treatment with tirofiban. Short-term efficacy, pre-and post-treatment scores on the National Institutes of Health Stroke Scale (NIHSS), Fugl-Meyer Assessment (FMA), and Barthel Index (BI), cerebral blood flow indices, platelet parameters [platelet count (PLT), mean platelet volume (MPV), and platelet distribution width (PDW)], as well as serum levels of S100B and NSE, were compared between the two groups.【Results】 The total effective rate in the observation group was significantly higher than that in the control group (P<0.05). After treatment, NIHSS scores decreased and FMA and BI scores increased in both groups (all P<0.05), with the observation group showing lower NIHSS scores and higher FMA and BI scores compared to the control group (P<0.05). Cerebral artery blood flow indices improved in both groups, with greater improvements in the observation group (P<0.05). PLT increased and both MPV and PDW decreased in the two groups (P<0.05), with the observation group showing higher PLT and lower MPV and PDW than the control group (P<0.05). Serum levels of S100B and NSE decreased in both groups, with lower levels observed in the observation group (P<0.05).【Conclusion】 TTT with tirofiban can effectively enhance therapeutic outcomes in patients with cerebral infarction, promote recovery of neurological, motor, and self-care functions, improve cerebral blood circulation, and regulate platelet parameters and serum levels of S100B and NSE.

Diagnostic Value of Combined Detection of TgAb and TRAb in Thyroid Cancer

【Objective】To explore the diagnostic value of combined detection of thyroglobulin antibody (TgAb) and thyrotropin receptor antibody (TRAb) in thyroid cancer.【Methods】 A total of 85 patients with thyroid cancer were included in the observation group, and 92 patients with benign thyroid lesions served as the control group. Basic clinical data, imaging results, and serum levels of thyroglobulin (TG), thyroid-stimulating hormone (TSH), TgAb, and TRAb were compared between the two groups. Logistic regression was used to analyze the risk factors for thyroid cancer. The diagnostic value of TgAb and TRAb was assessed using the receiver operating characteristic (ROC) curve.【Results】 In the observation group, the proportions of patients with irregular tumor morphology, aspect ratio ≥1, microcalcifications, blurred margins, and hypoechoic patterns on imaging, as well as higher peak systolic velocity (PSV) and resistance index (RI), were significantly greater than those in the control group (P<0.05). The levels of TG, TSH, TgAb, and TRAb were also significantly higher in the observation group (P<0.05). Logistic regression analysis indicated that imaging features, TG, TSH, TgAb, and TRAb were all significant risk factors for thyroid cancer (P<0.05). The ROC curve analysis showed that the combined detection of TgAb and TRAb had an area under the curve (AUC) of 0.926, indicating high diagnostic accuracy.【Conclusion】 Combined detection of TgAb and TRAb has significant value in the diagnosis of thyroid cancer.

Clinical Value of Different Sampling Methods under Electronic Bronchoscopy in Diagnosing Lung Cancer

【Objective】To explore the clinical value of different sampling methods under electronic bronchoscopy in the diagnosis of lung cancer. 【Methods】A retrospective analysis was conducted on the clinical data of 93 suspected lung cancer patients admitted to our hospital from January 2023 to December 2023. All patients were treated with brushing and/or clamping techniques to obtain lung tissue. The lung tissues obtained by brushing technique were classified as Group A, the lung tissues obtained by clamping technique were classified as Group B, and the lung tissues obtained by brushing and clamping technique were classified as Group C. Using pathological examination results as the gold standard, compare the detection rates of lung cancer, detection rates of different pathological types of lung cancer, and diagnostic efficacy among the three groups. 【Results】The detection rates of lung cancer and benign lesions in Group C were higher than those in Group A and Group B, and the differences were statistically significant (both P<0.05). In Group C, the detection rate of pathological types of lung cancer, such as intraductal hyperplasia, wall infiltration, extracorporeal pressure, and intraductal normalwas higher than that in Group A and Group B, however, the difference was not statistically significant (P>0.05). The sensitivity, specificity, accuracy, positive predictive accuracy, and negative predictive accuracy of Group C in diagnosing lung cancer were higher than those of Group A and Group B, and the difference was statistically significant (P<0.05). 【Conclusion】The combined use of brushing and clamping techniques for sampling and diagnosing lung cancer has high clinical value, improving the detection rate of lung cancer and different pathological types. The diagnostic efficiency of lung cancer with both brushing and clumping sampling is also high, which helps to guide the rational diagnosis and treatment in clinical practice.

Correlation Analysis of Serum hs-CRP, SAA, and D-D Levels with the Risk of Infection in Elderly Patients with Uremia

【Objective】To explore the relationship of serum levels of high-sensitivity C-reactive protein (hs-CRP), serum amyloid A (SAA), and D-dimer (D-D) with the risk of infection in elderly patients with uremia.【Methods】A total of 92 elderly uremic patients were enrolled and divided into a non-infection group (n=70) and an infection group (n=22) based on bacterial culture results. General clinical data and serum levels of hs-CRP, SAA and D-D were compared between the two groups. Multivariate logistic regression was used to analyze the risk factors for infection in elderly uremic patients. The predictive value of these serum markers for infection was assessed using receiver operating characteristic (ROC) curve analysis.【Results】 There were no statistically significant differences between the two groups in terms of gender, age, duration of hemodialysis, or underlying kidney disease (P>0.05). Serum levels of hs-CRP, SAA and D-D were significantly lower in the non-infection group compared to the infection group (P<0.05). Multivariate logistic regression analysis showed that hs-CRP ≥ 31.780 mg/L, SAA ≥ 105.285 mg/L, and D-D ≥ 301.025 ng/mL were independent risk factors for infection in elderly patients with uremia (all P<0.05). ROC curve analysis demonstrated that serum hs-CRP, SAA, and D-D levels could predict the occurrence of infection in these patients (all P<0.05).【Conclusion】 Infection in elderly uremic patients is influenced by multiple factors. Elevated levels of serum hs-CRP, SAA, and D-D may indicate the presence of infection.

Investigation of the Effect of Minimally Invasive Arch Nailing Technique Access Selection on Vertebral Fractures in the Thoracolumbar Segment

【Objective】To explore the efficacy of percutaneous and trans-paraspinal muscle space approaches for pedicle screw fixation in the treatment of thoracolumbar vertebral fractures and their impact on surgical traumatic stress.【Methods】A total of 124 patients with thoracolumbar vertebral fractures treated in our hospital from January 2019 to October 2022 were selected and randomly divided into the paraspinal muscle group (treated pedicle screw fixation via paraspinal muscle space approach) and the percutaneous group (treated pedicle screw fixation via percutaneous approach), with 62 cases in each group. The surgical indicators, visual analogue scale (VAS) scores, traumatic factors [cortisol (Cor), interleukin-8 (IL-8), C-reactive protein (CRP)], radiological imaging indicators (kyphotic Cobb angle and sagittal vertebral index), functional recovery parameters [Oswestry Disability Index (ODI) and Barthel Index (BI)], and complication rates were compared between the two groups.【Results】The operation time of the percutaneous group was longer than that of the paraspinal muscle group, and the number of fluoroscopies was more than that of the paraspinal muscle group. The incision length, intraoperative blood loss, and hospital stay of the percutaneous group were better than those of the paraspinal muscle group (P<0.05). The vertebral body reduction rate was 100.00% in both groups one week after surgery. At 3 days and 1 week after surgery, the VAS score of the percutaneous group was lower than that of the paraspinal muscle group (P<0.05), and the levels of serum Cor, IL-8, and CRP were lower than those of the paraspinal muscle group (P<0.05). At 3 days, 3 months, and 1 year after surgery, there was no significant difference in the sagittal vertebral index and kyphotic Cobb angle between the two groups (P>0.05). At 1 month after surgery, the ODI of the percutaneous group was lower than that of the paraspinal muscle group (P<0.05), and the BI was higher than that of the paraspinal muscle group (P<0.05). At 1 year after surgery, there was no significant difference in both ODI and BI between the two groups (P>0.05). The total complication rate of the percutaneous group was 9.68%, which was not significantly different from 14.52% of the paraspinal muscle group (P>0.05).【Conclusion】Both percutaneous and paraspinal muscle space approaches for pedicle screw fixation in the treatment of thoracolumbar vertebral fractures are effective, safe, and reliable. However, the former has less trauma, higher surgical precision, and greater difficulty, with longer operation time and more fluoroscopy. The operator requires rich experience in open surgery and minimally invasive surgery. The latter has simpler surgical operation, but longer incision and relatively slower early postoperative recovery.

Relationship of Serum Neutrophil Extracellular Traps with Short-Term Prognosis in Patients with Unstable Angina Pectoris after Percutaneous Coronary Intervention

【Objective】To explore the correlation between neutrophil extracellular traps (NETs) and short-term prognosis in patients with unstable angina pectoris (UAP) undergoing percutaneous tubular intervention (PCI). 【Methods】A total of 215 patients with UAP who underwent PCI treatment in our hospital from September 2019 to September 2023 were selected. Based on the prognosis within 30 days after PCI, the patients were divided into a good prognosis group (n=138) and a poor prognosis group (n=77). The serum dsDNA/NETs, C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), interleukin-10 (IL-10), myeloperoxidase (MPO), creatine kinase isoenzyme MB (CK-MB), cardiac troponin I (cTnI), as well as platelet count, white blood cell count, and mean platelet volume (MPV) were compared between the two groups. Pearson method was used to analyze the relationship of serum dsDNA/NETs with various indicators. Logistic regression model was used to analyze the risk factors for adverse cardiovascular events in UAP patients after PCI. Receiver operating characteristic (ROC) curve was used to analyze the predictive value of 24-hour serum dsDNA/NETs level for adverse cardiovascular events in UAP patients. 【Results】The serum levels of dsDNA/NETs, CRP, CK-MB, cTnI, TNF-α, IL-10, and MPO at 24 hours after surgery in both groups were higher than those before surgery, while these parameters in the poor prognosis group were higher than those in the good prognosis group. The levels of MPV and PLT in both groups were lower than those before surgery, and the MPV and PLT levels of the poor prognosis group were lower than those of the good prognosis group, with statistical significance (P<0.05). The serum dsDNA/NETs level in UAP patients was negatively correlated with MPV and PLT levels at 24 hours after surgery (r=-0.382, -0.527, both P<0.05). Multivariate logistic regression analysis showed that high levels of dsDNA/NETs, CRP, CK-MB, and low level of MPV were risk factors for poor prognosis after PCI in UAP patients (P<0.05). Effects of Simotang Combined with Bifidobacterium BB-12 on Intestinal Flora and Digestive Function in Children with Constipation

【Objective】To explore the effects of Simotang combined with Bifidobacterium BB-12 on intestinal flora and digestive function in children with constipation. 【Methods】A total of 100 children with constipation admitted to our hospital from July 2021 to July 2023 were selected and divided into the control group and the observation group by random number table method, with 50 cases in each group. The control group was treated with Simotang Oral Liquid, while the observation group was treated with Simotang combined with Bifidobacterium BB-12. The symptom improvement, anorectal manometry indices, gastrointestinal hormone levels and the number of intestinal flora were compared between the two groups before and after treatment. 【Results】After treatment, the defecation interval, defecation time, defecation pain score, abdominal pain and bloating symptom scores in both groups were lower than those before treatment, and the observation group was lower than the control group, with statistically significant differences (P<0.05). After treatment, the rectal maximum volume sensory threshold, maximum anal squeeze pressure and simulated defecation anal pressure in both groups were lower than those before treatment, and the observation group was lower than the control group, with statistically significant differences (P<0.05); there was no significant difference in rectal resting pressure between the two groups before and after treatment (P>0.05). After treatment, the serum levels of gastrin and motilin in both groups were lower than those before treatment, and the observation group was lower than the control group, with statistically significant differences (P<0.05); there was no significant difference in serum somatostatin and vasoactive intestinal peptide levels between the two groups before and after treatment (P>0.05). After treatment, the numbers of lactobacilli and bifidobacteria in both groups were higher than those before treatment, and the observation group was higher than the control group; the numbers of enterobacteria and staphylococci in both groups were lower than those before treatment, and the observation group was lower than the control group, with statistically significant differences (P<0.05). 【Conclusion】 Simotang combined with Bifidobacterium BB-12 can effectively improve intestinal flora imbalance in children and promote the recovery of gastrointestinal digestive function, which is worthy of clinical reference.

【Objective】To explore the effects of exercise intervention on muscle strength, physical function and quality of life in patients with acute pancreatitis (AP) complicated with sarcopenia. 【Methods】A total of 72 AP patients complicated with sarcopenia admitted to Hunan Provincial People's Hospital from January 2022 to June 2023 were selected and divided into control group and observation group by random number table method, with 36 cases in each group. The control group received conventional treatment and nursing, and the observation group received exercise intervention on the basis of the control group. Grip strength, 5-time sit-to-stand test time, timed up-and-go test (TUG) time, Activities of Daily Living Scale (ADL) score and Short Form 36-Item Health Survey (SF-36) score were compared between the two groups. 【Results】After intervention, grip strength and ADL scores of both groups were higher than those before intervention, and the observation group was higher than the control group (P<0.05); the 5-time sit-to-stand test time and TUG time of both groups were shorter than those before intervention, and the observation group was shorter than the control group (P<0.05). After intervention, the total SF-36 score, and scores of physical function (PF), role-physical (RP), general health (GH), vitality (VT), social function (SF), mental health (MH), bodily pain (BP), role-emotional (RE) in both groups were higher than those before intervention, with statistically significant differences (P<0.05); the total SF-36 score, and scores of PF, RP, GH, VT, SF, MH in the observation group were higher than those in the control group, with statistically significant differences (P<0.05); there were no significant differences in BP and RE scores between the observation group and the control group (P>0.05). 【Conclusion】Exercise intervention is beneficial to improve muscle strength, physical function and quality of life in AP patients complicated.

Application Effect of Psychological Intervention Guided by Need Satisfaction Theory in Patients with Recurrent Implantation Failure in Assisted Reproduction

【Objective】To explore the application effect of psychological intervention guided by need satisfaction theory in patients with recurrent implantation failure (RIF) in assisted reproduction. 【Methods】A total of 82 RIF patients admitted to our center from April 2024 to February 2025 were selected and divided into the observation group (receiving psychological intervention guided by need satisfaction theory on the basis of routine nursing) and the control group (receiving routine nursing) according to the random number table method, with 41 cases in each group. The mental health status [assessed by Symptom Checklist (SCL-90)], psychological resilience [assessed by Connor-Davidson Resilience Scale (CD-RISC)], fertility-related quality of life [assessed by Fertility Quality of Life Questionnaire (FertiQoL) scores] before and after intervention, as well as the number of retrieved oocytes, number of high-quality embryos, β-hCG positive rate and fetal heart confirmation rate were compared between the two groups. 【Results】 After intervention, the scores of all dimensions of SCL-90 in the observation group were lower than those in the control group, and the scores of all dimensions of CD-RISC and FertiQoL in the observation group were higher than those in the control group, with statistically significant differences (P<0.05). The number of retrieved oocytes, high-quality embryo rate, β-hCG positive rate and fetal heart confirmation rate in the observation group were higher than those in the control group, with statistically significant differences (P<0.05). 【Conclusion】 Psychological intervention guided by need satisfaction theory can effectively improve the mental health status, enhance the level of psychological resilience and fertility quality of life of RIF patients, and increase the embryo quality and early pregnancy success rate, which provides a new idea for the comprehensive management of RIF patients.

Prognostic Value of Combined D-Dimer, Procalcitonin, Blood Glucose Levels, and CTSI Score in Acute Pancreatitis

【Objective】 To investigate the prognostic prediction value of combined serum D-dimer (D-D), procalcitonin (PCT), fasting blood glucose (FBG), and computed tomography severity index (CTSI) score in patients with acute pancreatitis (AP).【Methods】 A total of 102 patients with AP were enrolled and divided into two groups based on clinical outcomes: a favorable prognosis group (n=90) and a poor prognosis group (n=12). Clinical data were compared between the groups. Logistic multivariate regression analysis was used to identify factors associated with poor prognosis, and receiver operating characteristic (ROC) curves were used to evaluate predictive performance.【Results】 Levels of D-D, PCT, FBG, and CTSI score were significantly higher in the poor prognosis group than in the favorable prognosis group (all P<0.05). Logistic regression analysis indicated that elevated D-D, PCT, FBG, and CTSI score were independent risk factors for poor prognosis in AP patients (all P<0.05). ROC analysis showed that combined detection of D-D, PCT, FBG, and CTSI score had superior predictive value compared to each marker alone (all P<0.05).【Conclusion】 Patients with poor prognosis in AP exhibit higher levels of D-D, PCT, FBG, and CTSI scores. Combined detection of these markers significantly enhances the accuracy of prognostic assessment in AP and provides a valuable evidence for early clinical intervention.

Efficacy of Anterior-Posterior Combined Approach Decompression, Reduction, Fusion and Internal Fixation in the Treatment of Cervical Fracture-Dislocation Complicated with Spinal Cord Injury

【Objective】To explore the efficacy of anterior-posterior combined approach decompression, reduction, fusion and internal fixation in the treatment of cervical fracture-dislocation (FDLCS) complicated with spinal cord injury (SCI).【Methods】A retrospective analysis was conducted on 100 patients with FDLCS complicated with SCI who underwent decompression, reduction, fusion and internal fixation in our hospital from January 2019 to January 2023. According to the surgical approach, they were divided into the anterior approach group (anterior approach surgery, n=48) and the combined group (anterior-posterior combined approach surgery, n=52). The perioperative indicators, imaging indicators, bone graft fusion, therapeutic effect, spinal nerve recovery and complications were compared between the two groups.【Results】The operation time and hospital stay in the combined group were longer than those in the anterior approach group, and the intraoperative blood loss and postoperative drainage volume were higher than those in the anterior approach group (P<0.05). After surgery, the average Cobb angle and intervertebral slip distance in the combined group were smaller than those in the anterior approach group (P<0.05). There was no significant difference in the bone graft fusion rate and average fusion time between the combined group and the anterior approach group (P>0.05). The total effective rate of the combined group was higher than that of the anterior approach group (P<0.05). After surgery, the Japanese Orthopaedic Association (JOA) score in the combined group was higher than that in the anterior approach group (P<0.05). There was no significant difference in the total incidence of complications between the two groups (P>0.05). 【Conclusion】The anterior-posterior combined approach decompression, reduction, fusion and internal fixation for patients with FDLCS complicated with SCI has greater trauma, higher operational difficulty and slower postoperative recovery, but the combined approach can better restore the cervical anatomical structure, with better postoperative neurological function recovery and better curative effect, and higher safety. Clinicians can choose the appropriate approach according to the individual situation of patients.

Effects of Different Sentinel Lymph Node Incisions on Lymph Node Detection and Subcutaneous Effusion in Breast-Conserving Surgery for Breast Cancer

【Objective】To explore the effects of different sentinel lymph node (SLN) incisions on lymph node detection and subcutaneous effusion in breast-conserving surgery for breast cancer. 【Methods】A total of 135 female breast cancer patients who underwent breast-conserving surgery from June 2017 to April 2023 were selected and divided into the observation group (n=67) and the control group (n=68) according to different SLN incision methods. The control group received axillary incisions, while the observation group received periareolar incisions. The two groups were compared in terms of SLN detection, subcutaneous effusion, quality of life, surgery-related indicators, and incidence of complications. 【Results】There was no statistically significant difference in SLN detection between the observation group and the control group (P>0.05). The operation time of the observation group was shorter than that of the control group, and the intraoperative blood loss was less than that of the control group, with statistically significant differences (P<0.05). At 3 months after surgery, the ROETC QLQ-BR45 scores and FACT-B scores of both groups were higher than those before surgery, and the observation group had higher scores than the control group, with statistically significant differences (P<0.05). There were no statistically significant differences in the incidence of complications and tumor recurrence rate between the two groups (P>0.05). 【Conclusion】Different SLN incisions have good effects in breast-conserving surgery for breast cancer, with a high number of detected SLNs, low tumor recurrence rate and subcutaneous effusion rate. However, the periareolar incision has obvious advantages in improving patients' postoperative quality of life.

Prognostic Prediction Value of Serum Procalcitonin Combined with Lactate Dehydrogenase in Children with Severe Pneumonia

【Objective】To explore the predictive value of procalcitonin (PCT) combined with lactate dehydrogenase for the prognosis of children with severe pneumonia. 【Methods】A retrospective analysis was conducted on the clinical data of 70 children with severe pneumonia treated in Taiqian New District Hospital from January 2019 to January 2022. The patients were divided into a poor prognosis group and a good prognosis group based on whether they had a recurrence of pulmonary infection after treatment. The clinical data of the two groups were compared. Logistic multivariate regression analysis was used to identify the influencing factors of poor prognosis in children with severe pneumonia. The area under the receiver operating characteristic (ROC) curve (AUC) was used to evaluate the predictive value of PCT, lactate dehydrogenase levels, and their combination for poor prognosis in children with severe pneumonia. 【Results】The incidence of poor prognosis in 70 children with severe pneumonia after treatment was 14.29% (10/70). The levels of PCT and lactate dehydrogenase in the poor prognosis group were higher than those in the good prognosis group (P<0.05), and the proportion of agranulocytosis was higher than that in the good prognosis group (P<0.05). Logistic multivariate regression analysis showed that agranulocytosis and high levels of lactate dehydrogenase and PCT were risk factors for poor prognosis in children with severe pneumonia (P<0.05). ROC curve analysis showed that the AUC values of PCT, lactate dehydrogenase, and their combination for predicting poor prognosis in children with severe pneumonia were 0.870, 0.859, and 0.936, respectively. 【Conclusion】The proportion of poor prognosis in children with severe pneumonia is relatively high. PCT and lactate dehydrogenase have certain predictive value for poor prognosis in children with severe pneumonia, and their combination has a higher predictive value.

【Objective】To explore the application effect of Runfei Tongluo Decoction combined with azithromycin in children with persistent cough due to mycoplasma pneumonia of Yinxu Feire type. 【Methods】A total of 92 children with persistent cough due to mycoplasma pneumonia admitted to our hospital from October 2021 to June 2024 were selected and divided into the control group and the observation group according to the random number table method, with 46 cases in each group. The control group was treated with azithromycin, and the observation group was treated with Runfei Tongluo Decoction on the basis of the control group, with continuous treatment for 3 weeks. The TCM syndrome scores, clinical efficacy, incidence of adverse reactions, disappearance time of cough and lung rales, and serum levels of leukotriene and high-sensitivity C-reactive protein (hs-CRP) were compared between the two groups. 【Results】After treatment, the scores of primary and secondary syndromes in both groups were lower than those before treatment, and the observation group was lower than the control group, with statistically significant differences (P<0.05). After treatment, the disappearance time of lung rales, cough and hospital stay in the observation group were shorter than those in the control group, with statistically significant differences (P<0.05). The total clinical effective rate of the observation group was higher than that of the control group, with a statistically significant difference (P<0.05). After treatment, the serum levels of leukotriene and hs-CRP in both groups were lower than those before treatment, and the observation group was lower than the control group, with statistically significant differences (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the observation group and the control group (P=0.503).【Conclusion】 Runfei Tongluo Decoction combined with azithromycin has a good clinical effect in the treatment of children with persistent cough due to mycoplasma pneumonia of Yinxu Feire type, which can reduce inflammatory response with good safety.

【Objective】To explore the effects of mesalazine enteric-coated tablets combined with vitamin D on intestinal barrier function and inflammatory factors in patients with ulcerative colitis (UC). 【Methods】 A total of 96 UC patients admitted to our hospital from March 2020 to March 2023 were selected and divided into the control group and the observation group according to the random number table method, with 48 cases in each group. The control group was treated with mesalazine enteric-coated tablets, and the observation group was treated with mesalazine enteric-coated tablets combined with vitamin D on the basis of the control group. The clinical efficacy, intestinal mucosal barrier function indexes [serum diamine oxidase (DAO), D-lactic acid (D-LA), intestinal fatty acid binding protein (I-FABP)], intestinal flora (lactobacillus, bifidobacterium, enterobacterium), inflammatory factors [tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), interleukin-6 (IL-6)], endoscopic lesion indexes (Baron score, Mayo score) and incidence of adverse reactions were compared between the two groups. 【Results】The total effective rate of the observation group was 97.92% (47/48), which was higher than 79.17% (38/48) of the control group, with a statistically significant difference (

【Objective】To explore the clinical efficacy of Renshen Yangrong Decoction combined with Taohong Siwu Decoction in combination with conventional western medicine in the treatment of angina pectoris after percutaneous coronary intervention (PCI) in patients with coronary heart disease. 【Methods】A total of 80 patients with angina pectoris after PCI for coronary heart disease admitted to our hospital from August 2022 to December 2024 were selected and divided into an observation group and a control group according to the random number table method, with 40 cases in each group. The control group received conventional western medicine treatment, and the observation group was given Renshen Yangrong Decoction combined with Taohong Siwu Decoction on the basis of the control group. The efficacy, TCM syndrome scores, myocardial injury markers [creatine kinase isoenzyme (CK-MB), cardiac troponin I (cTnI), creatine kinase (CK)], and inflammatory factors [interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), high-sensitivity C-reactive protein (hs-CRP)] were compared between the two groups. 【Results】The total clinical effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). After treatment, the scores of chest pain, dyspnea, palpitations and spontaneous sweating, and fatigue and weakness in both groups were lower than those before treatment, and the observation group was lower than the control group, with statistically significant differences (P<0.05); the levels of CK-MB, cTnI, CK, IL-6, TNF-α, and hs-CRP in both groups were lower than those before treatment, and the observation group was lower than the control group, with statistically significant differences (P<0.05). 【Conclusion】Renshen Yangrong Decoction combined with Taohong Siwu Decoction in combination with conventional western medicine has a significant clinical efficacy in the treatment of patients with angina pectoris after PCI for coronary heart disease, which can effectively improve myocardial injury and reduce the levels of inflammatory factors.

Predictive Efficacy of Bishop Score Combined with Modified VBAC Score for Vaginal Birth after Cesarean Delivery Failure

【Objective】 To explore the predictive efficacy of cervical maturity score (Bishop score) combined with modified vaginal birth after cesarean (VBAC) score for VBAC failure. 【Methods 】A total of 400 parturients undergoing trial of labor after cesarean section who delivered in our hospital from January 2020 to May 2023 were selected. According to whether vaginal delivery was successful, the parturients were divided into the success group (n=295) and the failure group (n=105). The Bishop score, modified VBAC score and general data were compared between the two groups. Multivariate stepwise Logistic regression model was used to analyze the influencing factors of VBAC failure. Receiver operating characteristic (ROC) curve was used to analyze the predictive efficacy of Bishop score, modified VBAC score and their combination for VBAC failure. 【Results 】The Bishop score and modified VBAC score in the success group were higher than those in the failure group (P<0.05), the previous vaginal delivery rate was higher than that in the failure group (P<0.05), the time since the last cesarean section was longer than that in the failure group (P<0.05), and the estimated fetal weight was lower than that in the failure group (P<0.05). Multivariate stepwise Logistic regression model analysis showed that previous vaginal delivery history, time since last cesarean section, estimated fetal weight, Bishop score and modified VBAC score were the influencing factors of VBAC failure (P<0.05). ROC curve analysis showed that the AUC of Bishop score and modified VBAC score for predicting VBAC failure was 0.831 and 0.832, respectively, and the AUC of their combination for predicting VBAC failure was 0.916. 【Conclusion】The combination of Bishop score and modified VBAC score has high predictive efficacy for VBAC failure.

【Objective】 To investigate the diagnostic value of triglyceride (TG)/cystatin C (cys C) ratio combined with urine N-acetyl-β-D-glucosaminidase (UNAG) for the early diagnosis of diabetic nephropathy (DN).【Methods】 A total of 182 patients with Type 2 diabetes mellitus (T2DM) were included in this study, with 69 cases in the DN group and 113 cases in the non-DN group. The baseline data were compared between the two groups. Logistic regression was used to identify factors affecting the early diagnosis of DN, and receiver operating characteristic (ROC) curve analysis was performed to evaluate the diagnostic value of related indicators. The UNAG and TG/cys C ratio were also analyzed in patients with different stages of DN.【Results】 Logistic multivariate regression analysis revealed that serum creatinine, UNAG, and TG/cys C were influencing factors for the early diagnosis of DN (all P<0.05). The ROC curve demonstrated that the combination of UNAG and TG/cys C had higher diagnostic sensitivity, specificity, and area under the curve (AUC) than each indicator used individually (all P<0.05). The UNAG level was higher in the clinical DN group than in the early DN group, while the TG/cys C ratio in the clinical DN group was lower in the clinical DN group (both P<0.05).【Conclusion】 The combination of the TG/cys C ratio and UNAG can improve the diagnostic accuracy for diabetic nephropathy.

Effect of Ticagrelor or Clopidogrel on Circulating MicroRNA Levels after Stent Implantation in Patients with Acute Myocardial Infarction

【Objective】To investigate the effects of ticagrelor or clopidogrel on the levels of miR-150, miR-133a, and miR-192 in patients with acute myocardial infarction (AMI) after percutaneous coronary intervention (PCI).【Methods】A total of 84 AMI patients who underwent PCI in two hospitals from March 2022 to December 2023 were enrolled. They were divided into the ticagrelor group and clopidogrel group according to the drugs used, with 42 cases in each group. The ticagrelor group received aspirin combined with ticagrelor after surgery, and the clopidogrel group received aspirin combined with clopidogrel, both for 3 months. Inflammatory factors [tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), high-sensitivity C-reactive protein (hs-CRP)], cardiac function indices [left ventricular end-diastolic diameter (LVDD), left ventricular ejection fraction (LVEF)], incidence of adverse reactions, major adverse cardiovascular events (MACEs), and serum levels of miR-150, miR-133a, and miR-192 were compared between the two groups.【Results】 After treatment, serum levels of hs-CRP, IL-6, and TNF-α decreased in both groups, with the ticagrelor group significantly lower than the clopidogrel group (P<0.05). LVEF increased and LVDD decreased in both groups, with the ticagrelor group showing higher LVEF and lower LVDD than the clopidogrel group (P<0.05). The incidence of MACEs was significantly lower in the ticagrelor group (P<0.05), while there was no significant difference in adverse reaction rates between the two groups (P>0.05). After treatment, miR-133a and miR-192 were lower in both groups (with the ticagrelor group lower than the clopidogrel group), and miR-150 was higher (with the ticagrelor group higher than the clopidogrel group) (P<0.05).【Conclusion】Ticagrelor improves cardiac function and miRNA expression levels in AMI patients after PCI, with a lower incidence of MACEs.

Effect of Chloroprocaine Combined Epidural Anesthesia in Cesarean Section on Inflammatory Factors and Pain Levels in Patients

【Objective】This study aims to explore the effect of chloroprocaine combined epidural anesthesia in cesarean section on maternal inflammatory factors and pain intensity. 【Methods】A total of 100 pregnant women undergoing cesarean section from two hospitals were randomly divided into an observation group and a control group, with 50 cases in each. The observation group received an intravenous injection 5 mL of 3% chloroprocaine 10 minutes after morphine injection, while the control group received an intravenous injection 5 mL of 0.9% saline solution 10 minutes before morphine injection. The levels of inflammatory factors [interleukin-2(IL-2), interleukin-10(IL-10), tumor necrosis factor-α (TNF-α)] before surgery and 4, 12, 24, 48 hours after surgery were compared between the two groups, as well as the Visual Analog Scale (VAS) and Ramsay Sedation Scores before surgery and 2, 6, 12, 24, 48 hours after surgery, and the incidence of adverse reactions in both groups. 【Results】 The levels of IL-2 at 4, 12, 24, 48 hours after surgery in both groups were significantly lower than those before surgery (P<0.05), while the level of IL-10 was significantly higher than before surgery (P<0.05). IL-2 levels at all time points in the observation group were significantly lower and IL-10 levels of the observation group at 12, 24 hours after surgery were higher if compared to the control group (P<0.05). TNF-α levels in the observation group at all time points did not significantly change compared to before surgery (P>0.05), while in the control group, TNF-α levels at 4, 12, 24 hours after surgery were significantly higher than before surgery (P<0.05), and TNF-α levels at 4, 12, 24 hours in the observation group were lower than those in the control group (P<0.05). VAS scores and sedation scores of both groups at all time points after surgery increased compared to before surgery (P<0.05), with lower VAS scores and higher sedation scores in the observation group at all time points after surgery (P<0.05) if compared to those in the control group. There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】 In cesarean section, chloroprocaine combined with epidural anesthesia can effectively reduce maternal inflammatory factors and pain intensity, with good sedation effects and few adverse reactions, making it worthy of clinical promotion and application.

Effects of Individualized Nutritional Management Strategy Combined with Comprehensive Intervention Measures on Nutritional Status, Liver Function and Incidence of Complications in Patients with Liver Cirrhosis

【Objective】To explore the effects of individualized nutritional management strategy combined with comprehensive intervention measures on nutritional status, liver function and incidence of complications in patients with liver cirrhosis.【Methods】A total of 92 patients with liver cirrhosis admitted to our hospital from January 2023 to January 2024 were selected and divided into the observation group and the control group according to random number table method, with 46 cases in each group. The control group was given routine intervention, and the observation group was given individualized nutritional management strategy combined with comprehensive intervention measures on the basis of routine intervention. The liver function indexes [total bilirubin (TBIL), alanine aminotransferase (ALT), aspartate aminotransferase (AST)], nutritional indexes [body mass index (BMI), mid-arm muscle circumference (MAMC), albumin (ALB), prealbumin (PAB)], incidence of complications (gastrointestinal bleeding, hepatic encephalopathy, hepatorenal syndrome, ascites), and quality of life [Chronic Liver Disease Questionnaire (CLDQ) score] were compared between the two groups before intervention and 1 month after discharge. 【Results】After intervention, the serum levels of TBIL, ALT and AST in both groups were lower than those before intervention, and the observation group was lower than the control group, with statistically significant differences (P<0.05). After intervention, BMI, MAMC, ALB and PAB in both groups were significantly higher than those before intervention, and the observation group was higher than the control group, with statistically significant differences (P<0.05). The total incidence of complications in the observation group was lower than that in the control group, with a statistically significant difference (P<0.05). After intervention, the scores of fatigue, abdominal symptoms, systemic symptoms, emotional function, social function and vitality in both groups were higher than those before intervention, and the observation group was higher than the control group, with statistically significant differences (P<0.05). 【Conclusion】 Individualized nutritional management strategy combined with comprehensive intervention measures can improve liver function indexes and nutritional status of patients with liver cirrhosis, enhance their quality of life, and reduce the incidence of complications.

Comparison of the Effects of Jejunal Interposition Anastomosis and Esophago-Jejunal Roux-en-Y Anastomosis in Digestive Tract Reconstruction after Radical Gastrectomy

【Objective】 To compare the effects of jejunal interposition anastomosis and esophago-jejunal Roux-en-Y anastomosis (EJRY) in digestive tract reconstruction after radical gastrectomy. 【Methods】 Eighty patients who underwent radical gastrectomy for gastric cancer were randomly divided into two groups. The control group was given EJRY, and the observation group was given jejunal interposition anastomosis for digestive tract reconstruction. The clinical effects of the two groups were compared. 【Results】 There was no significant difference in the time of digestive tract reconstruction and hospital stay between the observation group and the control group (P>0.05). Before treatment, there were no significant differences in serum gastrin (GAS), motilin (MTL), and somatostatin (SS) levels between the two groups (P>0.05). At 7 days after surgery, the GAS level in the observation group was higher than that in the control group, while there was no significant difference in MTL and SS levels between the two groups (P>0.05). At 6 months after surgery, the GAS and SS levels in the observation group were higher than those in the control group, and there was no significant difference in MTL level between the two groups (P>0.05). The increases in hemoglobin, albumin, and body weight at 1 year after surgery in the observation group were all higher than those in the control group (P>0.05). The total incidence of postoperative complications in the observation group was lower than that in the control group (P<0.05). 【Conclusion】 Jejunal interposition anastomosis applied in digestive tract reconstruction after radical gastrectomy can improve the postoperative nutritional status and promote the recovery of digestive function.

Incidence and Influencing Factors of Preoperative Gastric Retention in Elderly Patients Undergoing Endoscopic Retrograde Cholangiopancreatography

【Objective】To investigate the incidence and influencing factors of preoperative gastric retention in elderly patients undergoing endoscopic retrograde cholangiopancreatography (ERCP). 【Methods】A total of 174 elderly patients who planned to undergo ERCP were divided into retention group and non retention group according to whether gastric retention occurred before ERCP. The baseline data and drug use of the two groups were compared, and the risk factors of gastric retention in elderly patients before ERCP were analyzed by multivariate logistic. 【Results】There were 56 cases of gastric retention in 174 patients. The sex, age, body mass index (BMI), the proportion of patients with cirrhosis, hyperlipidemia, hypertension, acidosis, low blood sodium and taking calcium antagonists in retention group and non retention group had no statistical significance (P>0.05). The proportion of patients with retention combination and diabetes, gastric ulcer, jaundice, low blood potassium was significantly higher than that in non retention group, The proportion of patients taking opioids and somatostatin in retention group was significantly higher than that in non retention group, and the proportion of patients taking glutamine in retention group was significantly lower than that in non retention group (P<0.05). Multivariate logistic analysis confirmed that diabetes, gastric ulcer, jaundice, hypokalemia, opioids and somatostatin were risk factors for gastric retention in elderly patients before ERCP (P<0.05), and glutamine was an independent protective factor for gastric retention in elderly patients before ERCP (P<0.05). 【Conclusion】The risk factors of gastric retention in elderly patients before ERCP include diabetes, gastric ulcer, preoperative jaundice, taking opioids, taking somatostatin drugs, hypokalemia, and taking glutamine as its protective factor, which should be comprehensively evaluated before ERCP and early intervention.

Application Effect of Dapagliflozin in Elderly Patients with Type 2 Diabetic Nephropathy Complicated by Hypertension

【Objective】To explore the application effect of dapagliflozin in the treatment of elderly patients with type 2 diabetic nephropathy complicated with hypertension. 【Methods】A total of 92 elderly patients with type 2 diabetic nephropathy complicated with hypertension who were treated in our hospital from March 2021 to March 2024 were selected and divided into the conventional group (given conventional treatment) and the dapagliflozin group (with conventional treatment plus dapagliflozin treatment) according to their medication conditions, with 46 cases in each group. The clinical indicators, renal function indicators, inflammatory indicators, oxidative stress response indicators and adverse reactions before and after treatment were compared between the two groups. 【Results】After treatment, the body mass index (BMI), diastolic blood pressure (DBP), systolic blood pressure (SBP), fasting blood glucose (FPG), glycated hemoglobin (HbA1c), total cholesterol (TC), and triglycerides (TG) of both groups were lower than those before treatment. However, there was no statistically significant difference in the estimated glomerular filtration rate (eGFR) between the two groups (P>0.05). After treatment, the urine microalbumin/creatinine ratio (UACR), homocysteine (Hcy), malondialdehyde (MDA), FPG, HbA1c, C-reactive protein (CRP), interleukin-17 (IL-17), and interleukin-8 (IL-8) levels in the dapagliflozin group were lower than those in the conventional group (P<0.05), while the superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) levels were higher than those in the conventional group (P<0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】The application effect of dapagliflozin in the treatment of elderly patients with type 2 diabetic nephropathy complicated with hypertension is significant. It can effectively reduce blood glucose and BMI, improve lipid indicators, protect renal function, reduce inflammatory response, and enhance antioxidant capacity, thereby improving the clinical indicators of patients from multiple aspects and improving their prognosis.

Application Value of TBA/HDL-C and cys C in Short-Term Prognosis Evaluation of Patients with ACLF

【Objective】To explore the application value of the ratio of total bile acid (TBA) to high-density lipoprotein cholesterol (HDL-C) (TBA/HDL-C) combined with cystatin C (cys C) in evaluating the short-term prognosis of patients with acute-on-chronic liver failure (ACLF). 【Methods】The clinical data of 87 ACLF patients admitted to our hospital from January 2021 to October 2023 were retrospectively analyzed. According to the survival status, the patients were divided into the survival group (n=38) and the death group (n=49). General information of the patients was collected, and the levels of TBA, HDL-C and cys C at admission were recorded, with TBA/HDL-C calculated. Receiver operating characteristic (ROC) curve was used to analyze the efficacy of TBA/HDL-C and cys C in predicting the short-term prognosis of ACLF patients. Kappa consistency test was used to analyze the consistency between the prediction of short-term prognosis by TBA/HDL-C, cys C and the actual prognosis. 【Results】The serum levels of HDL-C and TBA in the survival group were higher than those in the death group, while the levels of TBA/HDL-C and cys C were lower than those in the death group, with statistically significant differences (P<0.05). The Kappa values of TBA/HDL-C, cys C, and their combination in predicting short-term prognosis compared with actual prognosis were 0.609, 0.626, and 0.716, respectively. The combination showed a higher degree of consistency with the actual prognosis. TBA/HDL-C and cys C had high sensitivity, specificity, accuracy, positive predictive value, and negative predictive value in predicting the short-term prognosis of ACLF, and their combination showed better predictive performance, especially in sensitivity and negative predictive value. ROC curve analysis showed that both TBA/HDL-C and cys C alone had good predictive efficacy, and the area under the curve (AUC) of their combination was 0.854, which was superior to either of them alone (P<0.05).【Conclusion】The combination of TBA/HDL-C and cys C can effectively predict the short-term prognosis of ACLF patients, and has important reference value for clinical treatment and prognosis evaluation of patients.

Effect of Thromboelastography Monitoring Combined with Risk Early Warning Nursing on Blood Viscosity and Quality of Life in Patients after Fracture Surgery

【Objective】To explore the effect of thromboelastography monitoring combined with risk early warning nursing on blood viscosity and quality of life in patients after fracture surgery. 【Methods】A total of 84 patients who underwent fracture surgery in our hospital from July 2020 to July 2023 were selected and divided into a control group (given routine nursing intervention) and an observation group (given thromboelastography monitoring combined with risk early warning nursing intervention on the basis of the control group) according to the random number table method, with 42 cases in each group. The incidence of deep vein thrombosis, blood viscosity, pain score and quality of life were compared between the two groups. 【Results】After intervention, the incidence of deep vein thrombosis in the observation group was lower than that in the control group (P<0.05); the levels of prothrombin time (PT), thrombin time (TT) and fibrinogen (FIB) in the observation group were better than those in the control group (P<0.05); the pain score in the observation group was lower than that in the control group, and the quality of life score was higher than that in the control group, with statistically significant differences (P<0.05). 【Conclusion】Thromboelastography monitoring combined with risk early warning system nursing for patients after fracture surgery can reduce the incidence of deep vein thrombosis, improve blood viscosity, reduce pain scores, and improve the quality of life of patients.

Application Effect of Stratified and Graded Continuous Intervention in Patients with Facial Burns

【Objective】To evaluate the clinical application effect of stratified and graded continuous nursing intervention in perioperative and post-discharge rehabilitation management of patients with facial burns. 【Methods】A total of 86 patients with facial burns who were treated in Henan Electric Power Hospital from January 2023 to December 2024 were selected and divided into the observation group (43 cases, receiving stratified and graded continuous intervention) and the control group (43 cases, receiving routine intervention) by coin toss method. The intervention lasted until 3 months after discharge. The differences in wound healing time, Body Image Rating Scale (BIRS) score, Self-Perceived Burden Scale (SPBS) score, Acceptance of Disability Scale-Revised (ADS-R) score and complication rate were compared between the two groups. 【Results】After intervention, the formation time of granulation tissue and the complete wound healing time in the observation group were significantly shorter than those in the control group (P<0.05); the BIRS score and SPBS score in the observation group were significantly lower than those before intervention and in the control group (P<0.05); the scores of all dimensions of ADS-R in the observation group were significantly higher than those before intervention and in the control group (P<0.05); the total complication rate in the observation group was 18.60%, which was significantly lower than 48.84% in the control group (P<0.05). 【Conclusion】 The stratified and graded continuous nursing intervention mode can significantly accelerate the tissue repair process of facial burn wounds, reduce body image disturbance and psychological burden, improve patients' acceptance of disability, and effectively reduce common complications during rehabilitation, which has high clinical promotion value.
