Value of CRISPR-Cas9 Protein Kinase Libraries in Screening Common Targets for Multidrug Resistance in Pancreatic Cancer

【Objective】To analyze the value of CRISPR-Cas9 protein kinase library in screening common targets for multi-drug resistance in pancreatic cancer. 【Methods】 PDAC cell line TB32047 derived from KPC mouse model was transfected with lentivirus and treated with oxaliplatin, fluorouracil and ilictecan at IC90 concentration for 3 weeks. Next generation sequencing and MAGeCK VISPR analysis were performed to identify candidate genes through CRISPR-Cas9 function loss screening. The synergistic effect of cyclin-dependent kinase 7 (CDK7) depletion or targeted inhibition with FOLFIRINOX chemotherapy for pancreatic cancer was examined by MTT, flow cytometry, and colony formation assay.【Results】CDK7 ranked high in the screening of three chemotherapy drugs, and was a common target of multi-drug resistance in pancreatic cancer. After completely knocking out CDK7 with CRISPR-Cas9 or inhibiting CDK7 expression with THZ1,pancreatic cancer cell proliferation was slowed down, the cell cycle was blocked and apoptosis increased, while colony-forming ability decreased, and together with FOLFIRINOX, they promoted apoptosis of pancreatic cancer cells. 【Conclusion】CDK7 is a common target of multi-drug resistance in pancreatic cancer, and CDK7 is a new target to overcome chemotherapy resistance in pancreatic cancer.

Effects of Different Doses of Dexmedetomidine Combined with Dezocine on Postoperative Pain and Cognitive Function in Patients Undergoing Thoracoscopic Lung Cancer Radical Surgery

【Objective】 To investigate the effects of different doses of dexmedetomidine (Dex) combined with dezocine on postoperative pain and cognitive function in patients undergoing thoracoscopic lung cancer radical surgery. 【Methods】A total of 122 lung cancer patients were randomly divided into the observation group and the control group, with 61 cases in each group. The observation group received high-dose (1.2 μg/kg) Dex combined with dezocine anesthesia, while the control group received low-dose (0.8 μg/kg) Dex combined with dezocine anesthesia. The hemodynamic levels at different anesthesia time points, postoperative pain responses, levels of inflammatory factors [tumor necrosis factor-alpha (TNF-α) and interleukin-6 (IL-6)] after surgery, postoperative cognitive function, and incidence of adverse reactions were compared between the two groups. 【Results】 At pre-anesthesia (T

), there were no statistically significant differences in oxygen saturation (SpO

The pain scores at 6 hours and 12 hours postoperatively were lower than at 1 hour postoperatively in both groups (P<0.05), and the pain scores at 12 hours postoperatively were lower than at 6 hours postoperatively (P<0.05). Additionally, the observation group had lower pain scores at 1 hour, 6 hours, and 12 hours postoperatively compared to the control group (P<0.05). Postoperatively, TNF-α and IL-6 levels in both groups were lower than preoperatively (P<0.05), and the observation group was lower than the control group (P<0.05). There were no statistically significant differences in cognitive function scores between the two groups at 72 hours postoperatively compared to preoperatively (P>0.05). Cognitive function levels in both groups were higher at 72 hours postoperatively compared to preoperatively (P<0.05). 【Conclusion】 High-dose Dex combined with dezocine used in anesthesia for thoracoscopic lung cancer radical surgery is more advantageous in reducing postoperative pain levels and inflammation compared to low-dose Dex, minimizing cognitive function impairment with good safety.

Analysis of DSCC1 in Survival Prognosis, Immune Microenvironment, Immunotherapy and Cancer Drug Therapy in Cancer Patients

【Objective】To investigate the role of DNA replication and sister chromatid cohesion factor 1 (DSCC1) in survival prognosis, immune microenvironment, immunotherapy and cancer drug therapy in cancer patients.【Methods】Cancer Genome Atlas (TCGA), Genotypic Tissue Expression (GTEx), Human Protein Atlas (HPA), Tumor Immune Evaluation Resource 2.0 (TIMER2.0), single cell sequencing database (CancerSEA), Interactive gene/protein Search tool (STRING), GEO immunotherapy cohort and GDSC drug sensitive database agents were used to analyze the role of DSCC1 in survival prognosis, immune microenvironment, immunotherapy and cancer drug therapy in cancer patients.【Results】DSCC1 mRNA was highly expressed in 25 different types of cancer tissues and was associated with poor prognosis in patients with 11 types of cancer. The results of immunomicroenvironment research showed that the mRNA expression of DSCC1 was significantly correlated with a variety of immune cells, including various types of T helper cells, eosinophils, macrophages, mast cells, etc. Further studies predicted that patients with abnormal DSCC1 expression were expected to achieve better outcomes of immunotherapy. Enrichment analysis showed that the mRNA expression of DSCC1 was significantly correlated with cell cycle and p53 signaling pathway.【Conclusion】DSCC1 is a key molecule in the occurrence, development, diagnosis and treatment of cancer, and it affects the therapeutic effect of lung adenocarcinoma. DSCC1 plays an important role in a variety of cancers.

Clinical Analysis of 6 Cases of milk Protein Allergic Proctocolitis Induced by Milk Protein in Premature Infants with Blood Stool as the Main Symptom

【Objective】To analyze the clinical characteristics of milk protein allergic proctocolitis (AP) in premature infants with bloody stools as the main symptom. 【Methods】A retrospective analysis was conducted on the clinical data of 6 premature infants who were diagnosed with AP and admitted to the neonatology department of our hospital from January 2023 to September 2023. The clinical characteristics of AP in premature infants were analyzed and summarized. 【Results】All 6 premature infants were initially considered to have necrotizing enterocolitis due to varied degrees of bloody stools appeared within the first 4 to 49 days of birth. Among them, 4 cases were accompanied by abdominal distension, 2 cases were accompanied by elevated eosinophil count, 3 cases had abnormal infection indicators, 1 case had intestinal wall gas accumulation on abdominal plain film, 1 case had portal vein gas accumulation, 2 cases had mild intestinal dilation, but both abdominal ultrasound and bowel sounds were normal. Six premature infants had a good prognosis after being fed with a deeply hydrolyzed formula. 【Conclusion】For premature infants with rectal bleeding, attention should be paid to AP, clinicians need to repeatedly assess and avoid misdiagnosis.

The Expression Level and Clinical Value of

Gene in Colon Cancer Based on TCGA Database

gene in colon cancer based on TCGA database. 【Methods】In this study, TCGA RNA-seq data from the Tumor Immune Evaluation Database (TIMER) were collected to analyze the expression of

in colon cancer and the impact of

on the prognosis of colon cancer patients. The TIMER database was used to detect the infiltration of immune cells. The correlation between
CDKN2A
and clinical features was analyzed by R packet, and the protein interaction network information in
CDKN2A
gene was analyzed by STRING database. 【Results】
CDKN2A
gene was found to be overexpressed in 16 tumors, especially in colon cancer. The prognosis of colon cancer patients with higher
CDKN2A
expression level was worse, and the expression level of
CDKN2A
was significantly correlated with the T stage, N stage and pathological stage of the tumor (P<0.05). ROC curve analysis showed that
CDKN2A
was more effective in the diagnosis of colon cancer, and the area under the curve was 0.918.
CDKN2A
and immune checkpoint co-expression showed that
CDKN2A
was positively correlated with CD274, CTLA4, HAVCR2, LAG3, PDCD1, PDCD1LG2, SIGLEC15 and TIGIT (P<0.05). Spearman correlation analysis showed that CDKN2A expression was positively correlated with CCND1 expression (P<0.05). 【Conclusion】
CDKN2A
has high efficacy in the diagnosis and prognosis assessment of colon cancer patients, which is a potential target for the treatment of colon cancer, and has important significance in improving the diagnostic accuracy and prognosis assessment of colon cancer.

The Application Value of 5-min Nursing Knowledge Output Training Mode Based on Feynman Learning Theory in the Training of Foreign Aid Medical Staff

【Objective】To explore the application value of 5-min nursing knowledge output training mode based on Feynman learning theory in the training of foreign medical aid workers. 【Methods】A total of 57 foreign aid medical staff in emergency mixed intensive care unit were selected as the research subjects to understand their demand for training content, and a 5-min nursing knowledge output training mode based on Feynman learning theory was adopted for training. The theoretical and operational achievements of foreign aid medical staff before and after training were compared, and the satisfaction of the training mode was evaluated. 【Results】After the training, the scores of foreign aid medical workers in airway management module, nosocomial infection control module, nutrition support module, intravenous therapy support module, sedation and analgesia module, hemodynamic monitoring module, cardiopulmonary support module, critical patient reception and transport module were higher than those before the training, with statistical significance (P<0.05). The results of the questionnaire showed that 56 foreign aid medical workers were satisfied with the content of the training teacher, and the overall satisfaction rate was 98.2%(56/57). 【Conclusion】The 5-min nursing knowledge output training mode based on Feynman learning theory can help to improve the competence of foreign aid medical posts quickly, and the training mode has a high degree of satisfaction, which is worth promoting.

Predictive Value of Right Ventricular Myocardial Perfomance Index Combined with NT-proBNP in COPD Associated Pulmonary Arterial Hypertension

【Objective】To explore the predictive value of right ventricular myocardial performance index (RVMPI) combined with N-terminal pro-brain natriuretic peptide (NT-proBNP) for chronic obstructive pulmonary disease (COPD)-associated pulmonary arterial hypertension (PH).【Methods】NT-proBNP, RVMPI, and pulmonary artery systolic pressure (PASP) levels were measured in 144 COPD patients within 24 hours of admission and PASP levels were reassessed after one month of treatment. Patients were divided into a PH group and a normal pulmonary artery group based on their PASP levels. Logistic multivariate regression was used to identify factors associated with secondary PH, and receiver operating characteristic (ROC) curves were used to assess the predictive value of RVMPI and NT-proBNP for secondary PH.【Results】 Of the 144 patients, 49 had PH (34.03%) and 95 had normal pulmonary artery pressure (65.97%). The PH group had a higher proportion of smoking history, Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ) scores, NT-proBNP level, and RVMPI level compared to the normal group, while their 6-minute walk distance(6MWD) distance and FEV
1
/FVC levels were lower (P<0.05). Multivariate regression analysis showed that smoking history, lower 6MWD distance, higher APACHE Ⅱ scores, elevated NT-proBNP level, and increased RVMPI were associated with secondary PH in COPD patients (P<0.05). ROC analysis indicated that the area under the curve (AUC) for predicting secondary PH using NT-proBNP, RVMPI, and their combination were 0.764, 0.746, and 0.854, respectively, with the combined prediction being superior to either alone (P<0.05).【Conclusion】 COPD patients with secondary PH have elevated NT-proBNP and RVMPI levels, and both markers provide high predictive efficacy for PH.

Mechanism of lncRNA IGF2-AS in Osteogenic Differentiation of BMSCs

【Objective】To investigate the mechanism of long non-coding RNA insulin-like growth factor 2 antisense transcript (lncRNA IGF2-AS) in the osteogenic differentiation of bone marrow mesenchymal stem cells (BMSCs). 【Methods】 Fifty 6-month-old SD female rats were selected as the research objects and divided into 5 groups with 10 rats in each group by random number table method. Group A was sham operation group, group B, group C and group D underwent surgical resection of both ovaries to construct the ovariectomized (OVX) rat model (the success rate of each group was 100%), and the remaining 10 were control group after successful construction. After the OVX rat model was successfully constructed, the rats in group C were given lncRNA IGF2-AS simulants, the rats in group D were given lncRNA IGF2-AS inhibitors, and the other groups were given equal volume normal saline. The rats in all groups were routinely fed for 8 weeks. Bone mineral density, body weight, blood urea nitrogen level, femur biomechanical strength, serum index level, lncRNA IGF2-AS, miR-3126-5p and KLK4 expression levels, and BMSCs osteogenic differentiation level were compared in all groups after feeding for 8 weeks. 【Results】 There was no significant difference in body weight among all groups (P>0.05). and femur of rats in groups B and D were lower than those in group A and the control group, the bone mineral density of L

and femur of rats in group C was higher than that in group A and the control group, and the bone mineral density of L

and femur of rats in group B was higher than that in group D, with statistical significance (P<0.05). The blood urea nitrogen levels of rats in group B, C and D were higher than those in group A and the control group, the blood urea nitrogen level of rats in group C was lower than that in group B and D, and the blood urea nitrogen level of rats in group B was lower than that in group D, all difference were statistical significance (P<0.05). The maximum femur load and Young's modulus of rats in group B, C and D were lower than those in group A and the control group, and the maximum femur load and Young's modulus of rats in group C were higher than those in group B and D, and those in group B were higher than those in group D, with all statistical significance (P<0.05). Serum Ca and P levels of rats in group B, C and D were lower than those in group A and the control group, while serum ALP, PⅠNP and CTX-Ⅰ levels were higher than those in group A and control group, the difference were significant (P<0.05). The serum Ca and P levels of rats in group C were higher than those in group B and D, and levels of Ca and P in group B was higher than those in group D; While serum ALP, PⅠNP and CTX-Ⅰ levels of rats in group C were lower than those in group B and D, and those in group B were lower than those in group D. All differences were statistically significant (P<0.05). There was no significant difference in the expression levels of lncRNA IGF2-AS, miR-3126-5p and KLK4 in group A, group B and the control group (P>0.05). The expression levels of lncRNA IGF2-AS, miR-3126-5p and KLK4 in group C were higher than those in groups A, B, D and control group, and the differences were statistically significant (P<0.05). There was no statistically significant difference in the distribution of BMSCs osteoblasts between group A and control group. The distribution of BMSCs osteoblasts in groups B, C and D was less than that in groups A and control group, and the distribution of BMSCs osteoblasts in group C was more than that in groups B and D, and group B has more distribution of BMSCs osteoblasts than that in group D. 【Conclusion】 Overexpression of lncRNA IGF2-AS can promote osteogenic differentiation of BMSCs by regulating miR-3126-5p/KLK4 signaling pathway, improve bone mineral density, blood urea nitrogen levels and serum levels of Ca, P, ALP, PⅠNP and CTX-Ⅰ, and enhance the biomechanical strength of femur.

The Efficacy of Hyperbaric Oxygen Combined with Edaravone in the Treatment of Acute Cerebral Infarction and its Effect on Hemodynamics and Neurological Function

【Objective】 To explore the therapeutic efficacy of hyperbaric oxygen therapy combined with edaravone on acute cerebral infarction and its impacts on cerebral hemodynamics and neurological function of patients.【Methods】A total of 106 patients with acute cerebral infarction were randomly divided into the observation group and the control group, with 53 patients in each. The control group received conventional treatment, while the observation group was treated with hyperbaric oxygen combined with edaravone on top of the conventional treatment. Expression of CK20 and p53 and their Relationship with Clinicopathological Features and Prognosis in Gastric Cancer Patients

【Objective】 To explore the relationship of the clinicopathological features and prognosis with the expression of cytokeratin 20 (CK20) and p53 in gastric cancer patients.【Methods】 Seventy patients who were diagnosed with gastric cancer for the first time and underwent gastric cancer resection were selected for the study. The levels of CK20 and p53 in the cancerous tissues and para-cancerous tissues of the patients were analyzed to investigate the association of the expression of the two with the clinicopathological features, further the effects of the two on the prognosis of the patients were analyzed by Cox.【Results】 The percentage of CK20 and p53 protein positivity in cancer tissues of gastric cancer patients was higher than that in para-cancerous tissues (P<0.05). The percentage of CK20 and p53 protein positive expression in cancer tissues of patients with stage Ⅲ, low differentiation, and lymph node metastasis was higher than that of patients with stage Ⅰ/Ⅱ, middle/high differentiation, and lymph nodes without metastasis (P<0.05). After 1 year of follow-up, 3 out of 70 patients were lost, and 15 out of the remaining 67 patients died, with a case-fatality rate of 22.39%(15/67).The results of Cox analysis showed that stage Ⅲ, poorly differentiated, lymph node metastasis, p53 protein positivity, and CK20 protein positivity were all influencing factors in the prognosis of patients with gastric cancer (P< 0.05).【Conclusion】 The expression levels of p53 and CK20 proteins in cancer tissues of gastric cancer patients showed a close correlation with the degree of differentiation of the tumor, the presence of lymph node metastasis, the clinical stage, and the prognosis of the patients.

Observation on the Therapeutic Effect of Group Painting Therapy Combined with Clozapine in the Treatment of Chronic Schizophrenia

【Objective】To explore the therapeutic effect of group painting therapy combined with clozapine in the treatment of chronic schizophrenia. 【Methods】A total of 100 patients with chronic schizophrenia who were hospitalized in our hospital from February 2019 to February 2023 were selected as the study subjects. They were randomly divided into the observation group and the control group using a random number table method, with 50 cases in each group. The control group received treatment with clozapine, while the observation group received a combination of group painting therapy on the basis of the control group. We compared the clinical efficacy of two groups and evaluated their positive and negative symptom scale (PANSS), quality of life scale for schizophrenia patients (SQLS), and social function assessment scale for hospitalized psychiatric patients (SSPI). We also compared the incidence of adverse reactions between the two groups. 【Results】The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). After treatment, the positive factor score, negative factor score, general psychopathological score, and total score of the two groups were significantly lower than before treatment, and the observation group was lower than the control group, with statistical significance (P<0.05); The scores and total scores of psychosocial, energy and motivation, symptoms and side effects in the two groups of patients were lower than before treatment, and the observation group was lower than the control group, with statistical significance (P<0.05); The daily life function, motility and social interaction, social activity skill scores, and total scores of the two groups were lower than before treatment, and the observation group was lower than the control group, with statistical significance (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups of patients (P>0.05).【Conclusion】The combination of group painting therapy and clozapine has significant clinical efficacy in the treatment of chronic schizophrenia, which can effectively improve the positive and negative symptoms of patients, as well as improve their quality of life and social function, with high safety.

The Impact of Integrated Traditional Chinese Medicine Nursing Intervention on Anorectal Dynamics and Symptom Improvement in Patients with Functional Constipation

【Objective】 To investigate the effects of integrated traditional Chinese medicine (TCM) nursing intervention on anorectal dynamics and symptom improvement in patients with functional constipation.【Methods】 A total of 82 patients with functional constipation were randomly divided into the control group and the observation group, with 41 patients in each group. The control group received Mibaokang treatment and routine nursing care, while the observation group received the same treatment as the control group with the addition of integrated TCM nursing intervention. The differences in anorectal dynamics [resting anal pressure (ARP), maximal anal contraction pressure (AMCP), longest anal contraction time (ALCT)], constipation symptom scores, TCM syndrome scores, and anxiety trait scores (STAI) before and after the intervention were compared between the two groups.【Results】 After the intervention, the ARP and AMCP levels in the observation group were lower than those in the control group (P<0.05), but there was no significant difference in ALCT levels between the two groups (P>0.05). The constipation symptom scores, TCM syndrome scores, and STAI scores in the observation group were all significantly lower than those in the control group (P<0.05).【Conclusion】 Integrated TCM nursing intervention can improve anorectal dynamics, alleviate symptoms, and reduce the severity of constipation and anxiety in patients with functional constipation, making it worthy of clinical promotion and application.

Establishment and Validation of A Nomogram Risk Model for the Failure of Corrective Treatment in Children with Amblyopia

【Objective】 To establish and validate the risk model of the failure of correction therapy in children with amblyopia. 【Methods】The clinical data of 129 children with amblyopia treated in our hospital from January 2020 to June 2023 were retrospectively analyzed and randomly divided into a training set (n=104) and a validation set (n=25) according to 8∶2. All the children were given routine corrective treatment. After 3 months of treatment, the refractive status was checked to evaluate the treatment effect. The effective and effective children were included in the effective group, and the ineffective children were included in the ineffective group. The factors influencing the effect of correction treatment in children with amblyopia were analyzed, and the prediction model of correction treatment ineffectiveness was established, and the model was validated and evaluated. 【Results】There were 23 cases (22.12%) in the training set and 7 cases (28.00%) in the verification set. Logistic regression analysis showed that the degree of disease, the difference of retinal nerve fiber layer (RNFL) and the difference of retinal thickness (MRT) in macular area were the factors affecting the effect of correction treatment in children with amblyopia (P<0.05). Using the above influencing factors as predictive variables, a nomogram prediction model was established. The C-index was 0.804 (95%

:0.756-0.828), and the correction curve for predicting the failure of correction treatment in children with amblyopia with abnormal refractive error was close to the ideal curve (P>0.05). ROC curve analysis of the training set showed that the sensitivity, specificity and AUC of the nomogram model were 78.26%, 81.48% and 0.844 to predict the failure of corrective treatment in the children with amblyopia. ROC curve results of validation set showed that the sensitivity, specificity and AUC of the nomogram model to predict the failure of correction therapy in children with abnormal refractive amblyopia were 71.43%, 88.89% and 0.873. 【Conclusion】The degree of illness, RNFL difference and MRT difference are the factors that affect the therapeutic effect of correction in children with amblyopia. The nematic model based on this construction can predict the ineffective therapeutic effect of correction in children with amblyopia.

Comparison of the Effects of Sodium Lactate Ringer's Solution, Sodium Bicarbonate Ringer's Solution, and Sodium Acetate Ringer's Solution in Fluid Resuscitation due to Trauma Hemorrhagic Shock

【Objective】 To compare the effects of sodium lactate Ringer's solution, sodium bicarbonate Ringer's solution, and sodium acetate Ringer's solution in fluid resuscitation caused by trauma hemorrhagic shock (THS).【Methods】 A total of 306 patients with multiple organ or tissue injuries undergoing fluid resuscitation caused by THS were randomly divided into three groups: Group A, Group B, and Group C, with 102 patients in each group. Group A was resuscitated with sodium lactate Ringer's solution, Group B with sodium bicarbonate Ringer's solution, and Group C with sodium acetate Ringer's solution. Blood serum inflammatory factors, blood gas indicators, coagulation function levels, incidence of complications, and in-hospital mortality rates were compared among three groups before and after resuscitation.【Results】 After resuscitation, levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in all three groups were elevated compared to pre-resuscitation levels, with the greatest change in Group A, followed by Group B, and then Group C (all P<0.05). Arterial blood lactate (Lac) levels were reduced, and arterial blood pH levels increased after resuscitation in all three groups, with the most significant improvement in Group B, followed by Group C then Group A (all P<0.05). Oxygen partial pressure (PaO
2
) and oxygen saturation (SaO
2
) increased after resuscitation in all three groups, but the differences among groups were not statistically significant (all P>0.05). After resuscitation, activated partial thromboplastin time (APTT) and prothrombin time (PT) levels were elevated in all groups, with the smallest increase in Group C, followed by Group B and Group A (P<0.05). The incidence of multiple organ dysfunction syndrome and in-hospital mortality rates in Group B and Group C were lower than that in Group A (P<0.05), while there were no significant differences between Group B and Group C (P>0.05).【Conclusion】 Sodium acetate Ringer's solution and sodium bicarbonate Ringer's solution were generally more effective than sodium lactate Ringer's solution in fluid resuscitation, with sodium acetate Ringer's solution showing the best ability to suppress the inflammatory response and maintain coagulation function, followed by sodium bicarbonate Ringer's solution. For patients at risk of liver or kidney dysfunction, sodium lactate Ringer's solution should be used cautiously.

Application of CT Cerebral Perfusion Imaging Combined with CTA Scan in Evaluating Carotid Artery Stenosis

【Objective】To explore the value of CT Perfusion Imaging (CTP) combined with CT Angiography (CTA) in the diagnosis of the degree of carotid artery stenosis. 【Methods】A retrospective analysis was conducted on the imaging data of 117 suspected carotid artery stenosis patients admitted to our hospital from February 2021 to February 2023. Based on the CTA detection results, they were divided into the mild stenosis group (n=41), moderate stenosis group (n=44), and severe stenosis and occlusion group (n=32). According to the degree of carotid artery stenosis, the patients' carotid arteries were divided into normal group (n=86), mild group (n=41), moderate group (n=44), and severe and occlusion group (n=63). All patients underwent both CTP and CTA examinations to compare the relative cerebral blood volume (sCBV), relative cerebral blood flow (sCBF), relative mean transit time (sMTT), relative time to peak (sTTP), peak blood flow velocity (PSV) and end diastolic blood flow velocity (EDV) of the internal carotid artery, as well as the pulse index (PI) and mean blood flow velocity (V

CT cerebral perfusion imaging combined with CTA scanning has significant value in the early diagnosis of cerebral ischemia. It can show the hemodynamic changes in patients with moderate and severe carotid stenosis or occlusion, which provides a reliable basis for early clinical treatment.

Evaluation Value of Pulmonary Ultrasonography Combined with Pulmonary Ultrasonography Score on the Severity of Neonatal NRDS

【Objective】To evaluate the severity of neonatal respiratory distress syndrome (NRDS) by pulmonary ultrasonography combined with pulmonary ultrasonography score. 【Methods】A total of 68 newborns with NRDS admitted to our hospital from January 2020 to December 2023 were selected as the observation group, while 42 newborns with other common lung diseases treated in our hospital during the same period were selected as the control group. The detection rate of lung ultrasound features and lung ultrasound score were compared between the two groups. Receiver operating characteristic (ROC) curve was used to analyze the value of lung ultrasound combined with lung ultrasound score in evaluating the severity of neonatal NRDS. 【Results】The detection rates of double lung points and isolated B line in the observation group were lower than those in the control group, but the detection rates of abnormal pleural line, fusion B line, diffuse B line, lung consolidation and pneumobronchial sign were higher than those in the control group, with statistical significance (P<0.05). The ultrasound scores of left lung, right lung, double lung, bilateral lung and double lung base in the observation group were higher than those in control group, and the difference was statistically significant (P<0.05). ROC curve analysis results showed that the sensitivity and specificity of lung ultrasound combined with lung ultrasound score in diagnosing the severity of neonatal NRDS were 95.6% and 97.6%, respectively, and the area under ROC curve (AUC) was 0.977. 【Conclusion】Pulmonary ultrasound combined with pulmonary ultrasound score has a high value in evaluating the severity of neonatal NRDS, with high sensitivity and specificity, and has important clinical application value.

Effect of Mechanical Ventilation During Cardiopulmonary Bypass on the Incidence of Postoperative Pulmonary Complications in Preschool Children Undergoing Correction of Congenital Heart Malformations

Curative Effect of Danhong Injection Combined with Edaravone Dextrocamphorol in the Treatment of Acute Cerebral Infarction

【Objective】 To investigate the efficacy of Danhong injection combined with Edaravone dextrocamphorol in the treatment of acute cerebral infarction (ACI). 【Methods】A total of 98 patients with ACI were randomly divided into the observation group and the control group, with 49 cases in each group. The control group was treated with Edaravone dexcamphorol, and the observation group was treated with Danhong injection combined with Edaravone dexcamphorol. The neurological function (NIHSS score) and cognitive function (Mini-mental State Examination, MMES) score, Ubiquitin carboxyl terminal hydrolase L1 (UCH-L1) ,hypersensitive C-reactive protein (hs-CRP) levels and living capacity and adverse reactions of the two groups were compared before and after treatment. 【Results】After treatment, NIHSS score, UCH-L1 level and hs-CRP level in the observation group were lower than those in the control group, and MMES score and life ability score were higher than those in the control group (P<0.05). The incidence of adverse reactions in the observation group was 10.20%, which was not statistically significant compared with 8.16% in the control group (P>0.05). 【Conclusion】Danhong injection combined with Edaravone dextrocamphorol is effective in the treatment of ACI patients, which can improve the neurological and cognitive functions of patients, reduce the level of inflammatory factors, and have fewer adverse reactions.

Effect of Family-friendly Interactive Nursing on Maternal Role Adaptability for Primipara Parturient

【Objective】To explore the effect of family-friendly interactive nursing on maternal role adaptability of primipara parturient . 【Methods】Forty primiparas who delivered in our hospital from January 2018 to January 2020 (the control group, with conventional nursing intervention) and 40 primiparas who delivered in our hospital from January 2021 to January 2023 (the observation group with affectionate family-friendly interactive nursing intervention) were selected. The scores of childbirth Attitude Questionnaire (CAQ), maternal role adaptation and breastfeeding self-efficacy (CBSEI-32) were compared between the two groups. 【Results】After intervention, the CAQ individual scores and CAQ total score of the observation group were lower than those of the control group, the difference was statistically significant (P<0.05). The rate of maternal role adaptability in observation group was higher than that in control group, and the difference was statistically significant (P<0.05). The individual and total scores of CBSEI-32 in observation group were higher than those in control group, and the difference was statistically significant (P<0.05). 【Conclusion】Family-friendly interactive nursing for parturient primipara parturient can improve the attitude towards childbirth, increase the rate of maternal role adaptation and improve breast-feeding self-efficacy, which is worthy of clinical promotion.

The Effect of Different Doses of Dexmedetomidine on Postoperative Cognitive Function and Inflammatory Cytokine Levels in Elderly Patients Undergoing Laparoscopic Gastric Cancer Resection

【Objective】 To explore the effects of different doses of dexmedetomidine (Dex) on postoperative cognitive function and inflammatory cytokine levels in elderly patients undergoing laparoscopic gastric cancer resection (LAGRP).【Methods】 A total of 128 elderly patients scheduled for LAGRP were randomly divided into a control group (C group) and three different doses of Dex groups (D1 group,D2 group,D3 group), with 32 patients in each group. All patients received general anesthesia. During anesthesia maintenance, the D1 group, D2 group, and D3 group were given dexmedetomidine via intravenous infusion at doses of 0.2 μg/(kg?h), 0.5 μg/(kg?h), and 0.6 μg/(kg?h), respectively, while the C group received an equal volume of saline. The perioperative indicators, pain severity, cognitive function, inflammatory cytokine levels, and incidence of adverse reactions were compared among the four groups.【Results】 There were no significant differences in surgery duration, intraoperative blood loss, or length of hospital stay among the four groups (P>0.05). The D3 group had a longer awakening time compared to the C group, D1 group, and D2 group (P<0.05). Postoperative 48-hour Visual Analog Scale (VAS) pain scores were lower in the D1 group, D2 group, and D3 group compared to the C group (P<0.05). The Mini-Mental State Examination (MMSE) scores on postoperative days 1 and 3 were higher in the D1 group, D2 group, and D3 group compared to the C group (P<0.05). C-reactive protein (CRP) and procalcitonin (PCT) levels on postoperative days 1 and 3 were lower in the D1 group, D2 group, and D3 group compared to the C group (P<0.05). There were no significant differences in the total incidence of adverse reactions among the four groups (P>0.05).【Conclusion】 Dexmedetomidine at different doses can improve postoperative cognitive function and inhibit inflammatory responses in elderly patients undergoing LAGRP, with good safety. A dose of 0.5 μg/(kg?h) showed the best overall effect.

Changes of Serum Procalcitonin Level and APACHE Ⅱ Score in Elderly Patients with Sepsis and its Influencing Factors

【Objective】To investigate the changes of serum procalcitonin (PCT) level, acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ) score and its influencing factors in elderly patients with sepsis.【Methods】 Clinical data of 90 elderly patients with sepsis were retrospectively analyzed and divided into high risk group (n=24), middle risk group (n=46) and low risk group (n=20) according to their APACHE Ⅱ scores. PCT level and APACHE Ⅱ score of the three groups were compared, Pearson test was used to analyze the correlation between PCT and APACHE Ⅱ score, and multiple Logistic regression was used to analyze the risk factors affecting senile sepsis. 【Results】The PCT level and APACHE Ⅱ score of the low-risk group were lower than those of the medium-risk group and high-risk group, and the medium-risk group was lower than the high-risk group (P<0.05). Pearson test showed that PCT level was positively correlated with APACHE Ⅱ score (r=0.857, P<0.05). Logistic regression analysis showed that PCT level and APACHE Ⅱ score were independent risk factors for senile sepsis (OR=5.078, 5.043, P<0.05). 【Conclusion】 The occurrence and progression of senile sepsis are related to the increase of serum PCT level and APACHE Ⅱ score, which should be paid attention to clinically.

Risk Factors Analysis and Prediction Model of Early Hypoglycemia in Newborns with Gestational Diabetes Mellitus

【Objective】To investigate the risk factors of early neonatal hypoglycemia in patients with gestational diabetes mellitus (GDM) and establish a predictive model. 【Methods】A total of 193 GDM patients and their neonates who gave birth in the obstetrics and Gynecology Department of our hospital from June 2020 to December 2023 were selected, and the neonates were divided into hypoglycemic group (n=45) and normal glycemic group (n=148) according to the early blood glucose level of the neonates. Univariate and multivariate Logistic regression were used to analyze the influencing factors of early neonatal hypoglycemia, and the predictive forest map model was built accordingly. Receiver operating characteristic (ROC) curve was used to evaluate the model performance, and the nomogram model was drawn with R4.3.0 software, and the differentiation of the model was evaluated by ROC curve. Bootstrap method was used to conduct 1000 repeated samples to verify the prediction efficiency of the forest map model. 【Results】The hypoglycemic group had poor prenatal body mass index and blood glucose control during pregnancy, the delivery mode was cesarean section, the proportion of neonatal low body weight was significantly higher than that of the normal blood glucose group, the proportion of family history of diabetes, and the Apgar score were lower than that of the normal blood glucose group. Logistic regression analysis results showed: High prenatal body mass index, poor glycemic control during delivery, cesarean section delivery and low birth weight were risk factors for early hypoglycemia in newborns (P<0.05), while family history of diabetes and high Apgar score were protective factors for early hypoglycemia in newborns (P<0.05). The model verification results showed that the model had high sensitivity and specificity.【Conclusion】This study identified the risk factors for early hypoglycemia in newborns with GDM, and constructed a risk prediction model with good predictive performance, which can be used as a clinical assessment tool to predict the risk of early hypoglycemia in newborns with GDM.

Efficacy of Herbal Fumigation Combined with Extracorporeal Radial Shock Wave Therapy in Patients with Chronic Plantar Fasciitis Induced Heel Pain

【Objective】 To observe the efficacy of herbal fumigation combined with extracorporeal radial shock wave therapy in patients with chronic plantar fasciitis induced heel pain, and its impact on recurrence. 【Methods】 A total of 94 patients with chronic plantar fasciitis and heel pain treated in our hospital were randomly divided into the control group and the observation group, with 47 cases in each group. The control group received radial extracorporeal shock wave therapy, while the observation group received additional herbal fumigation. Both groups performed plantar fascia and Achilles tendon stretching exercises under physician guidance. The clinical efficacy and recurrence rates of the two groups were compared as well. 【Results】 After treatment, the visual analog scale (VAS) pain scores of both groups decreased compared to before treatment (P<0.05), with the observation group showing lower scores than the control group (P<0.05). The American Orthopaedic Foot and Ankle Society (AOFAS) scale scores of both groups increased compared to before treatment (P<0.05), with the observation group showing higher scores than the control group (P<0.05). The recurrence rate in the observation group one year after treatment was 10.64%, which was lower than that in the control group's 27.66% (P<0.05). After treatment, Traditional Chinese Medicine syndrome scores in both groups decreased compared to before treatment (P<0.05), with the observation group showing lower scores than the control group (P<0.05). The total incidence of adverse reactions in the observation group was 8.51%, which was higher than the control group's 4.26%, but the difference was not statistically significant (P>0.05). 【Conclusion】 Herbal fumigation combined with extracorporeal radial shock wave therapy can alleviate pain symptoms in patients with chronic plantar fasciitis induced heel pain, improve foot and ankle function, help reduce recurrence, and has good safety.

Spearman was used to analyze the correlation of pregnancy hypertension with pre-pregnancy BMI and pregnancy weight gain. 【Results】The incidence rate of pregnancy-induced hypertension, mild preeclampsia and severe preeclampsia in group A was higher than that in group B and group C, with statistical significance (P<0.05). The incidence of hypertension, mild preeclampsia and severe preeclampsia in the excessive weight gain group were higher than those in the appropriate weight gain group and the insufficient weight gain group, and the difference was statistically significant (P<0.05). Pre-pregnancy overweight and obesity plus excessive weight gain during pregnancy were significantly associated with pregnancy-induced hypertension (P<0.05). 【Conclusion】Pre-pregnancy overweight, obesity and excessive weight gain during pregnancy are risk factors for pregnancy-induced hypertension in pregnant women. Obstetrical health care workers should attach great importance to and strengthen the popularization and education of pre-pregnancy and pregnancy weight management knowledge for women of childbearing age, especially overweight and obese women.

The observation group (n=48) was treated with Yiqi Weixue Capsule combined with Ferric Carboxymaltose, while the control group (n=49) was treated with Ferric Carboxymaltose only. The clinical efficacy, incidence of adverse reactions, incidence of adverse pregnancy outcomes, and iron metabolism indicators [hepcidin (Hepcidin), serum iron (SI), ferritin (SF), transferrin saturation (TSAT), transferrin (TRF), soluble transferrin receptor (sTfR), ferroportin (FPN), ferritin-like protein (FJV)] and red blood cell parameters [hemoglobin (HGB), mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), mean corpuscular volume (MCV), hematocrit (HCT)] were compared between the two groups after 1 month of treatment. 【Results】After treatment, the observation group showed a higher total effective rate and higher levels of SI, SF, TSAT, and FJV compared to the control group. The levels of Hepcidin, TRF, sTfR, FPN, and the incidence of adverse pregnancy outcomes in the observation group were lower than those in the control group (P<0.05). The levels of HGB, MCH, MCHC, MCV, and HCT in the observation group were higher than those in control group (P<0.05).The incidence of adverse reactions between the two groups was not significantly different (P>0.05). 【Conclusion】The combined therapy is highly effective in treating pregnant women with iron deficiency anemia, significantly improving iron metabolism and red blood cell parameters, alleviating anemia, and reducing adverse pregnancy outcomes. It is safe treatment.

Evaluation of Volumetric CT Digital Subtraction Angiography and CT Angiography on the Restenosis Efficacy of Stent Placement at Vertebral Artery Ostium

【Objective】To evaluate the efficacy of volumetric CT digital subtraction angiography (VCTDSA) and CT angiography (CTA) in the assessment of restenosis in patients with posterior circulating cerebral infarction after stent placement in the vertebral artery ostium.【Methods】 From January 2020 to July 2022, 82 patients with posterior circulation cerebral infarction who underwent vertebral artery initiation stenting were prospectively selected. Using DSA as the gold standard, CTA, VCTDSA and DSA imaging examinations were performed sequentially. CTA, VCTDSA and DSA were recorded to evaluate the incidence and morphology of arterial stenosis, respectively. Kappa test was used to analyze the consistency between the results of CTA and VCTDSA in detecting arterial stenosis and DSA, and the diagnostic value was evaluated by receiver operating characteristic (ROC) curve.【Results】DSA examination results showed that 22 of 82 patients who underwent vertebral artery initiation stenting had postoperative stenosis, and the stenosis incidence was 26.83%. The Kappa values of CTA, VCTDSA in diagnosing postoperative arterial stenosis were 0.766 and 0.936, respectively. The area under the curve (AUC) of CTA and VCTDSA were 0.839 and 0.938, respectively. DSA examination results showed that there were 22 stenosis vessels, 14 regular stenosis vessels and 8 irregular stenosis vessels in 82 patients who underwent the treatment of vertebral artery initiation stenting. The Kappa values of stenosis artery morphology after diagnosis by CTA, VCTDSA were 0.585 and 0.697, respectively. The AUC of CTA and VCTDSA were 0.777 and 0.839, respectively. 【Conclusion】Both CTA and VCTDSA have higher diagnostic efficacy in restenosis and vascular morphology after stenting at the beginning of vertebral artery, while VCTDSA is superior.

Analysis of Risk Factors and Preventive Measures of Relapse in Children with Epilepsy after Drug Treatment

【Objective】To investigate the risk factors of relapse in children with epilepsy after drug treatment. 【Methods】The clinical data of 90 children with epilepsy admitted to our hospital from May 2020 to May 2021 were retrospectively analyzed, and they were divided into the observation group (children with relapse) and the control group (children without relapse) according to the recurrence of epilepsy after drug treatment. Logistic multivariate regression analysis was used to investigate the independent risk factors of relapse after drug treatment in children with epilepsy. 【Results】There were 24 cases of recurrence in 90 cases, the recurrence rate was 26.67%. Univariate analysis showed that there were statistically significant differences in age, frequency of onset, type of disease, duration of epilepsy, intracranial lesions and seizure free time before drug withdrawal between the two groups (P<0.05). Multivariate Logistic regression analysis showed that age (< 2 years), duration of disease before treatment ( >1 year), duration of drug withdrawal (< 6 months), and ECG after drug withdrawal showed that epileptic discharge were independent risk factors for relapse after drug treatment in children with epilepsy (P<0.05). 【Conclusion】 Age, course of disease before treatment, duration of drug withdrawal, eclamptic discharge shown by ECG after drug withdrawal are important risk factors for relapse in children with epilepsy after drug treatment. Clinical preventive measures can be taken according to risk factors to reduce the recurrence rate of epilepsy after drug treatment and improve the quality of life of children with epilepsy.

Clinical Study on Glucocorticoid-Induced Osteoporosis Treated with Desumab Combined with Calcitonin Injection

【Objective】To investigate the clinical efficacy of disumab combined with salmon calcitonin injection in the treatment of glucocorticoid-induced osteoporosis (GIOP). 【Methods】A total of 204 patients with GIOP admitted to our hospital from July 2021 to January 2023 were selected and divided into the control group (n=103) and the observation group (n=101) according to random number table method. The control group was treated with calcitonin injection on the basis of conventional treatment, while the observation group was treated with sulizumab injection on the basis of control group. Both groups were treated for 12 months. The clinical efficacy, total incidence of adverse drug reactions, bone mineral density [osteocalcin (N-MID), procollagen type Ⅰ N-terminal peptide (PⅠNP), β-collagen special sequence (β-CTX), alkaline phosphatase (BAP), calcitonin (CT)], and dysfunction [assessed by Oswestry Disability Index (ODI) score] and pain [visual analogue pain scale (VAS)] before and after treatment were compared between the two groups.【Results】After treatment, L

, bone mineral density and BAP levels of femoral neck in observation group were higher than those in control group, while β-CTX, N-MID, PⅠNP and CT were lower than those in control group, with statistical significance (P<0.05). After 6 months and 12 months of treatment, VAS and ODI scores in observation group were lower than those in control group (P<0.05). The total effective rate of observation group (91.09%) was higher than that of control group (80.58%) (P<0.05). There was no significant difference in the total incidence of adverse drug reactions between the two groups (P>0.05). 【Conclusion】Disumab combined with calcitonin injection is effective in the treatment of GIOP patients, which can increase bone mineral density, relieve pain and dysfunction, improve bone metabolism indexes, and is safe and reliable.

Effect of Intracranial Vascular Intervention on Elderly Patients with Ischemic Cerebrovascular Disease and its Influence on Neurological Function

【Objective】 To investigate the effect of intracranial vascular intervention on elderly patients with ischemic cerebrovascular disease (ICVD) and its influence on the neurological function. 【Methods】A total of 70 elderly patients with ICVD who received intracranial vascular interventional therapy were randomly divided into the control group and the observation group, with 35 cases in each group. The control group received traditional drug therapy, and the observation group received intracranial vascular interventional therapy on the basis of the control group. The hemodynamics, hemorheology, neurological deficit score (NDS), National Institutes of Health Stroke Scale (NIHSS) score and the occurrence of adverse reactions were compared between the two groups before and after treatment. 【Results】After treatment, the changes of hemodynamics and hemorheology indexes in the observation group were significantly better than those in the control group (P<0.05), and the scores of NDS and NIHSS were significantly lower than those in the control group (P<0.05). After treatment, there was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05).【Conclusion】Intracranial vascular interventional therapy can effectively improve the efficacy and neurological function of elderly patients with ICVD without increasing the risk of adverse reactions.

The Expression Level and Significance of Renal Function Index in Peripheral Blood of Patients with Chronic Obstructive Pulmonary Disease

【Objective】To investigate the expression levels and significance of renal function indicators such as uric acid (UA), creatinine (Cr), cystatin C(cys C) and B-type brain natriuretic peptide (BNP) in peripheral blood of patients with chronic obstructive pulmonary disease (COPD). 【Methods】 A total of 96 patients with COPD admitted to our hospital from May 2020 to June 2022 were selected, including 45 patients in the acute stage (acute group) and 51 patients in the stable stage (stable group). In addition, 90 healthy volunteers who came to our hospital for physical examination during the same period were selected as the control group. The basic data and UA, Cr, cys C and BNP levels of the three groups were compared. Patients in the acute stage were divided into mild group (n=16), moderate group (n=16) and severe group (n=13) according to the degree of airflow limitation. The levels of UA, Cr, cys C and BNP in the three groups were compared, and the correlation between the levels of UA, Cr, cys C and BNP and the degree of airflow limitation in the acute stage was analyzed. 【Results】Compared with the control group, the smoking history ratio of the acute group and stable group was significantly increased, and the difference was statistically significant (P<0.05). The levels of UA, Cr, cys C and BNP in the acute group, the stable group and the control group were decreased successively, and the differences among groups were statistically significant (P<0.05). The levels of UA, Cr, cys C and BNP in severe group, moderate group and mild group were decreased successively, and the differences among groups were statistically significant (P<0.05). Spearman correlation analysis showed that the levels of UA, Cr, cys C and BNP were positively correlated with the degree of airflow restriction in acute patients (P<0.05). 【Conclusion】The abnormal expression levels of UA, Cr, cys C and BNP in peripheral blood of patients with COPD are significantly correlated with the severity of COPD.
