Comparison of Curative Effect of Different Surgical Methods on Elderly Patients with Lumbar Disc Herniation

【Objective】 To compare the therapeutic effect of Lumbar endoscopic surgery and percutaneous puncture surgery in elderly patients with Lumbar disc herniation (LDH). 【Methods】A total of 100 LDH patients who received surgical treatment in our hospital from January 2023 to January 2024 were selected and divided into control group (percutaneous puncture surgery) and observation group (lumbar endoscopic surgery) according to different treatment methods, with 50 cases in each group. The efficacy rate, visual analogue scale (VAS), Oswestry disability index (ODI), quality of life and psychological status were compared between the two groups. 【Results】3 months after treatment and at the last follow-up, the VAS score of lower back, VAS score of lower limb and ODI score of the observation group were lower than those of the control group (P<0.05). After treatment, the curative effect rate of observation group was 92.00% (46/50), which was significantly higher than that of the control group [72.00% (36/50)], the difference was statistically significant (P<0.05). After treatment, the life quality score of the observation group was higher than that of the control group, and the scores of the self-rating Anxiety scale (SAS) and self-rating Depression scale (SDS) were lower than that of the control group, with statistical significance (P<0.05).【Conclusion】Compared with percutaneous puncture surgery, endoscopic spinal surgery is effective in the treatment of LDH patients, which can significantly improve the pain symptoms and adverse emotions of elderly patients, and improve the quality of life of patients. However, in the selection of surgical methods, the best operation method suitable for patients should be selected according to the specific conditions of patients.

Clinical Efficacy of GS Regimen and PD1 Immunotherapy Combined with Probiotics in Patients after Pancreatic Cancer Resection

【Objective】 To explore the clinical efficacy of the GS regimen (gemcitabine combined with tegafur) and PD1 immunotherapy combined with probiotics in patients after pancreatic cancer resection.【Methods】 Eighty patients who underwent pancreatic cancer resection were randomly divided into two groups, A and B, with 40 cases in each group. Group B received the GS regimen combined with PD1 immunotherapy, while Group A received the same treatment as Group B, but with the addition of probiotics. After treatment, NLR, SII, and PLR decreased in both groups (P< 0.05), with Group A having significantly lower values than Group B (P< 0.05). The total incidence of adverse reactions in Group A was 30.00%, which was lower than Group B's 55.00% (P< 0.05).【Conclusion】 The GS regimen combined with PD1 immunotherapy and probiotics can improve immune function, lower tumor marker levels, and reduce the incidence of adverse reactions in patients after pancreatic cancer resection.

Clinical Effect of Musculoskeletal Ultrasound-Guided Precise Intervention Combined with Massage Therapy in Post-Stroke Patients with Shoulder Pain

【Objective】To investigate the clinical effect of musculoskeletal ultrasound-guided precision intervention combined with massage in the treatment of post-stroke patients with shoulder pain. 【Methods】A total of 120 stroke patients with post-stroke shoulder pain admitted to our hospital from March 2021 to February 2024 were selected. According to random number table method, patients were divided into the observation group (receiving musculoskeletal ultrasound-guided precision interventional therapy combined with massage therapy) and the control group (receiving massage therapy), with 60 cases in each group. The degree of shoulder pain, musculoskeletal color ultrasound index, shoulder joint range of motion, shoulder joint function and upper limb function were compared between the two groups. 【Results】After treatment, the pain analog scale (VAS) scores of both groups were lower than before treatment (P<0.05), and the VAS in the observation group was lower than that in the control group (P<0.05). After treatment, the distance from acromion to greater tubercle and the thickness of joint capsule below glenohumeral joint in the two groups were lower than before treatment (P<0.05), and these parameters in the observation group was lower than those in the control group (P<0.05). After treatment, the flexion, extension and extension range of shoulder, the Chinese version of Constant-Murley Shoulder Joint Rating Scale (CMS) score and the simplified Fugl-Meyer Upper Limb Function Rating Scale (FMA-UE) score of the two groups were higher than before treatment (P<0.05), and these scores in the observation group was higher than those in the control group (P<0.05). 【Conclusion】Musculoskeletal ultrasound-guided precise intervention combined with massage in the treatment of post-stroke shoulder pain patients can reduce shoulder pain, reduce musculoskeletal color ultrasound indicators, improve shoulder joint motion, and improve shoulder joint function and upper limb function.

Clinical Effect of Unilateral Biportal Endoscopic Discectomy Nucleus for Pulposus Extraction on Patients with Lumbar Disc Herniation

【Objective】To investigate the clinical efficacy of unilateral biportal endoscopic discectomy (UBED) in the treatment of Lumbar disc herniation (LDH). 【Methods】A total of 120 patients diagnosed and treated in our hospital from March 2023 to March 2024 were divided into UBED group and traditional fenestration discectomy (FD) group according to different treatment methods, with 60 cases in each group. Surgery related indexes, postoperative complications, dysfunction index (ODI) scores and visual analogue scale (VAS) and Japanese Orthopaedic Association scores (JOA) scores before and after treatment were compared between the two groups.【Results】The operation time of the UBED group was significantly longer than that of the FD group, and the hospital stay was significantly shorter than the FD group (P<0.05). After 1 month of treatment, ODI and VAS scores in the UBED group were significantly lower than those in the FD group (P<0.05), JOA scores were significantly higher than those in the FD group (P<0.05). The total incidence of postoperative complications in the UBED group was significantly lower than that in the FD group (P<0.05).【Conclusion】UBED can significantly shorten the length of hospital stay in LDH patients, improve dysfunction, relieve pain, reduce the incidence of postoperative complications, and improve lumbar function, but the operation time is relatively long.

Effect of Combined Warming Measures-assisted Anesthesia Recovery Intervention on Patients Undergoing Laparoscopic Total Hysterectomy

Correlation Study on Severity of Childhood Depression with Levels of Serum Adrenaline and miR-202 Expression

【Objective】To investigate the correlation between levels of serum adrenaline and miR-202 expression and the severity of childhood depression.【Methods】A total of 107 children with depression were included in the observation group. Based on the Children's Depression Inventory (CDI), they were divided into the mild group(34 cases), the moderate group(43 cases) and the severe group(30 cases). An additional 100 healthy children were selected as the control group. Serum adrenaline level and miR-202 expression level were compared between the two groups. Spearman correlation analysis was used to assess the relationship between these levels and the severity of depression. Receiver Operating Characteristic (ROC) curve analysis was used to evaluate the diagnostic value of these markers for childhood depression.【Results】Serum adrenaline level and miR-202 expression level in the observation group were significantly lower than those in the control group (P<0.05). Among the mild, moderate, and severe groups, serum adrenaline level and miR-202 expression level showed a gradual decreasing trend, with significant differences among the groups (P<0.05). Serum adrenaline level and miR-202 expression level were negatively correlated with the severity of depression (P<0.05). The diagnostic efficacy of the combined detection of both serum adrenaline and miR-202 was higher than that of either test alone.【Conclusion】Serum adrenaline level and miR-202 expression are significantly correlated with the severity of childhood depression. Combined detection of both markers has important value for diagnosing and assessing the condition of childhood depression.

Effect of Plain Film Hernia Repair on Inflammatory Factors and Postoperative Pain in Patients with Inguinal Hernia

【Objective】To investigate the effects of transabdominal preperitoneal inguinal hernia repair (TAPP) and Lichtenstein flat film hernia repair on inflammatory factors and postoperative pain in patients with inguinal hernia. 【Methods】The clinical data of 76 patients with inguinal hernia admitted to our hospital from March 2021 to March 2024 were retrospectively analyzed, and the patients were divided into observation group and control group according to treatment methods, with 38 cases in each group. The control group received Lichtenstein plain film hernia repair, and the observation group received TAPP. Perioperative indexes, visual analogue scale (VAS) scores and serum inflammatory factors [tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), C-reactive protein (CRP)] were compared between the two groups. The incidence of complications was compared between the two groups. 【Results】 The time of operation, the time of getting out of bed, the time of eating and the time of hospitalization in the observation group were shorter than those in the control group, and the hospitalization cost was higher than that in the control group, the difference was statistically significant (P<0.05). The VAS scores of the two groups at 3 and 7 days after surgery were lower than those of the same group at 1 day after surgery, and the VAS scores at 7 days after surgery were lower than those of the same group at 3 days after surgery, the difference was statistically significant (P<0.05). The VAS scores of the observation group at 3 and 7 days after operation were lower than those of the control group, and the difference was statistically significant (P<0.05). After operation, the serum levels of TNF-α, IL-6 and CRP in the two groups were higher than before operation, and the observation group was lower than control group, with statistical significance (P<0.05). The incidence of complications in the observation group was lower than that in the control group, the difference was statistically significant (P<0.05).【Conclusion】Compared with Lichtenstein surgery, TAPP surgery in the treatment of inguinal hernia patients has faster postoperative recovery, less pain, lower incidence of complications and higher safety.

Effect of PCI and Optimized Drug Therapy on Long-Term Quality of Life and Prognosis in Elderly Patients with Coronary Heart Disease

【Objective】To investigate the effects of percutaneous coronary intervention (PCI) and optimal drug therapy (ODT) on long-term quality of life and prognosis in elderly patients with coronary heart disease. 【Methods】The clinical data of 178 elderly patients with coronary heart disease admitted to our hospital from June 2018 to June 2023 were retrospectively analyzed, and the patients were divided into percutaneous coronary intervention group (PCI group, n=88) and optimized drug therapy group (ODT group, n=90) according to different treatment methods. The Value of Left Ventricular Work Index Combined with Serum Creatinine in Assessing Cardiac Function in Chronic Renal Failure Patients

【Objective】 To investigate the value of the left ventricular work index (Tei index) combined with serum creatinine (Scr) in assessing cardiac function in chronic renal failure (CRF) patients.【Methods】 A total of 141 CRF patients were included, with 109 patients in functional class Ⅰ-Ⅱ (Group A) and 32 patients in functional class Ⅲ-Ⅳ (Group B). Tei index and Scr level were compared between the two groups, and factors affecting the cardiac function classification of CRF patients were analyzed.【Results】 The Tei index and Scr level in Group B were significantly higher than those in Group A (P<0.05). Group A had higher left ventricular ejection fraction (LVEF) level than Group B, while uric acid, cardiac troponin I (cTnI), and N-terminal pro B-type natriuretic peptide (NT-proBNP) levels in Group A were significantly lower compared to Group B (P<0.05). Logistic regression analysis showed that Tei index, Scr, LVEF, and NT-proBNP levels were significant factors influencing the cardiac function classification of CRF patients (P<0.05). The combined assessment of Tei index and Scr showed even higher sensitivity, specificity, and area under the curve (AUC) for evaluating the cardiac function classification of CRF patients compared to each test alone (P<0.05).【Conclusion】 The combined assessment of Tei index and Scr is effective in evaluating the cardiac function classification of CRF patients and holds significant clinical value.

The Effect of Feedforward-Control-Based Intervention Model on the Recovery Quality of Patients Under General Anesthesia

【Objective】To investigate the application of a feedforward-control-based intervention model in patients under general anesthesia.【Methods】A total of 110 patients undergoing general anesthesia were randomly divided into the control group (n=55) and the observation group (n=55). The control group received conventional anesthesia, while the observation group received anesthesia intervention based on feedforward control. Both groups were intervened for 24 hours. The anesthesia conditions, vital signs at 3 minutes and 30 minutes after recovery, and adverse reactions during the intervention were compared between the two groups.【Results】After the intervention, the total incidence of agitation during the recovery period in the observation group was 7.28%, which was lower than the 21.81% in the control group (P<0.05). The observation group showed shorter times for extubation, recovery of orientation, staying in the post-anesthesia care unit (PACU), restoration of spontaneous breathing, and eye-opening compared to the control group (P<0.05). The levels of consciousness, limb activity, airway patency, and total recovery quality score in the observation group were significantly higher in the control group (P<0.05). At 30 minutes after recovery, the systolic blood pressure (SBP), diastolic blood pressure (DBP), and heart rate (HR) in both groups were lower than those at 3 minutes after recovery (P<0.05), with the observation group showing lower values compared to the control group (P<0.05). The total incidence of adverse reactions during the intervention was 10.91% in the observation group, which was lower than the 34.55% in the control group (P<0.05).【Conclusion】The feedforward-control-based intervention model used in general anesthesia improves the recovery quality of patients, promotes the restoration of various physiological functions and vital signs after anesthesia, reduces the incidence of adverse reactions and recovery period agitation, and has higher safety.

Diagnostic Value of Contrast-Enhanced Ultrasound Combined with Fine Needle Aspiration Cytology in C-TIRADS 4 Thyroid Nodules

【Objective】To evaluate the value of contrast-enhanced ultrasound(CEUS) combined with fine needle aspiration cytology in the diagnosis of Category 4 of thyroid nodules in Chinese Thyroid Image Report and Data System (C-TIRADS). 【Methods】The clinical data of 106 patients with C-TIRADS 4 thyroid nodules admitted to our hospital from April 2021 to April 2023 were retrospectively analyzed, with a total of 132 thyroid nodules. With postoperative histopathological examination as the gold standard, the preoperative CEUS and ultrasound-guided fine needle puncture cytology data were complete, and the sensitivity, specificity and accuracy of CEUS and ultrasound-guided fine needle puncture cytology in the diagnosis of benign and malignant C-TIRADS 4 of thyroid nodules were analyzed. 【Results】Among 132 thyroid nodules of C-TIRADS 4, the results of histopathological examination were as follows: 96 malignant nodules (72.73%) were thyroid papillary carcinoma. There were 36 benign thyroid nodules (27.27%), 28 nodular goiter and 8 nodular cysts. The positive predictive value and negative predictive value of CEUS in the diagnosis of C-TIRADS 4 malignant thyroid nodules were 91.76% and 61.70%, respectively. The diagnostic accuracy was 81.06%, and the consistency Kappa value was 0.564. The positive predictive value and negative predictive value of ultrasound guided fine needle puncture cytology in the diagnosis of C-TIRADS 4 thyroid malignant nodules were 100.00% and 75.00%, respectively, the diagnostic accuracy was 90.91%, and the consistency Kappa value was 0.792. The positive predictive value and negative predictive value of CEUS combined with fine needle puncture in the diagnosis of C-TIRADS 4 thyroid malignant nodules were 93.07%, 93.55%, 93.18%, and the consistency Kappa value was 0.820. Receiver operating characteristics (ROC) curve analysis showed that the sensitivity of CEUS combined with ultrasound -guided fine needle aspiration cytology in the diagnosis of C-TIRADS 4 thyroid nodules was higher than that of single diagnosis (P<0.05), and there was no significant difference in specificity and area under the curve (P>0.05). 【Conclusion】 Contrast-enhanced ultrasound combined with fine needle aspiration cytology has high accuracy in the diagnosis of C-TIRADS 4 thyroid nodules, and can be used to assist in the diagnosis of C-TIRADS 4 thyroid nodules.

Effect of Functional Electrical Stimulation Combined with New Bobath Technique on Rehabilitation of Shoulder Pain in Stroke Patients with Shoulder Pain

【Objective】 To investigate the effect of functional electrical stimulation (FES) combined with new Bobath technique on the rehabilitation of stroke patients with shoulder pain.【Methods】 A total of 90 stroke patients with shoulder pain were randomly divided into a control group (treated with new Bobath technique) and an observation group (treated with FES combined with new Bobath technique), with 45 patients in each group. The clinical efficacy of the two groups was compared.【Results】 The total effective rate in the observation group was higher than that in the control group (P<0.05). After treatment, the visual analog scale (VAS) scores in both groups were lower than before treatment (P<0.05), with the observation group showing lower scores than the control group (P<0.05). The Fugl-Meyer motor function scores in both groups were higher than before treatment (P<0.05), with the observation group scoring higher than the control group (P<0.05). The range of motion scores for shoulder flexion, abduction, and external rotation in both groups improved compared to before treatment (P<0.05), with the observation group showing greater improvements than the control group (P<0.05). Quality of life scores in both groups were higher than before treatment (P<0.05), with the observation group showing higher scores than the control group (P<0.05).【Conclusion】 FES combined with new Bobath technique is more effective in the rehabilitation of stroke patients with shoulder pain and is worthy of clinical promotion.

Clinical Efficacy of Infliximab Combined with Escitalopram Oxalate in the Treatment of Patients with Depression and Rheumatoid Arthritis

【Objective】To investigate the clinical efficacy of infliximab combined with escitalopram oxalate in the treatment of patients with depression and rheumatoid arthritis. 【Methods】A total of 180 patients with depression combined with rheumatoid arthritis admitted to Wuxi Mental Health Center from January 2018 to March 2022 were selected and divided into the control group and the observation group according to random number table method, with 90 cases in each group. The control group was treated with escitalopram oxalate tablets on the basis of basic treatment; The observation group was additionally treated with infliximab for 30 weeks. The clinical efficacy, occurrence of adverse reactions, pain score, depression score, erythrocyte subsidence rate (ESR), tumor necrosis factor-α (TNF-α), C-reactive protein (CRP), rheumatoid factor (RF), homocysteine (Hcy), brain-derived neurotrophic factor (BDNF), 5-hydroxytryptophan (5-HT) and dopamine were compared between the two groups cortisol levels.【Results】 6 cases fell off in the observation group, and 84 cases were included in the analysis. In the control group, 4 cases fell off, and 86 cases were included in the analysis. After treatment, the total effective rate of the observation group was 88.10%, which was significantly higher than that of the control group (72.09%), and the difference was statistically significant (P<0.05). After treatment, the pain score, depression score, ESR, TNF-α, CRP, RF, Hcy and cortisol levels in the observation group were lower than those in the control group, while the levels of BDNF, 5-HT and dopamine were higher than those in the control group, with statistical significance (P<0.05). There was no significant difference in the incidence of adverse drug reactions between the two groups (P>0.05). 【Conclusion】Infliximab combined with escitalopram oxalate in the treatment of patients with depression and rheumatoid arthritis can reduce joint inflammation and improve the effect of anti-depression.

Clinical Effect of Tongluo Zhitong Decoction with Low Frequency Ultrasound on Postpartum Patients with Low Back Pain

【Objective】To investigate the clinical effect of Tongluo Zhitong Decoction with low frequency ultrasound on postpartum patients with low back pain. 【Methods】A total of 78 patients with postpartum low back pain treated in our hospital from December 2023 to September 2024 were selected and divided into the control group (receiving conventional rehabilitation therapy) and the observation group (receiving Tongluo Zhitong Decoction with low-frequency ultrasound on the basis of control group) according to random number table method, with 39 cases in each group. The clinical efficacy, pain score, lumbar and back function score, quality of life score and lumbar forward flexion, back flexion, left and right rotation motion were compared between the two groups. 【Results】 The total effective rate of observation group was higher than that of the control group (P<0.05). After treatment, the scores of Global Pain Scale (GPS), Oswestry Disability Index (SCODI) and quality of life in the observation group were lower than those in the control group (P<0.05). Lumbar forward flexion, back flexion, left and right rotation movement were higher than those in the control group (P<0.05). 【Conclusion】In addition to the conventional rehabilitation therapy, the application of low-frequency ultrasound to Tongluo Zhitong Decoction in the treatment of postpartum patients with low back pain has significant effect, which can reduce pain, improve quality of life and improve low back function.

Effect of Local Injection of Platelet-Rich Plasma in the Treatment of Diabetic Foot Refractory Ulcer and its Influence on Serum VEGF and HA Levels

【Objective】To investigate the efficacy of local injection of platelet-rich plasma (PRP) in the treatment of patients with diabetic foot refractory ulcer and its effect on serum vascular endothelial growth factor (VEGF) and hyaluronic acid (HA) levels. 【Methods】 A total of 80 patients with diabetic foot refractory ulcer admitted to our hospital from August 2021 to November 2023 were selected and divided into control group (conventional treatment) and observation group (PRP local injection treatment on the basis of control group) according to different treatment methods, with 40 cases in each group. The clinical efficacy, blood perfusion volume, bacterial quantification, ulcer healing (ulcer reduction rate, granulation tissue growth thickness, granulation tissue coverage, ulcer skin grafting time and ulcer healing time), total complication rate, skin temperature of affected foot before and after treatment, ankle brachial index and serum VEGF and HA levels were compared between the two groups. 【Results】The total effective rate, ulcer blood perfusion volume, ulcer shrinkage rate and granulation tissue coverage rate in observation group were higher than those in control group (P<0.05), and the bacterial quantity in observation group was lower than that in control group (P<0.05).The growth thickness of granulation tissue was higher than that of control group (P<0.05).The time of skin grafting and healing of ulcer were shorter than that of control group (P<0.05).After treatment, the skin temperature, ankle-brachial index, VEGF and HA levels in the observation group were higher than those in the control group (P<0.05). There was no significant difference in the total incidence of complications between the two groups (P>0.05). 【Conclusion】 Local injection of PRP in the treatment of patients with diabetic foot refractory ulcer can significantly improve the total effective rate, shorten the healing time of ulcer, and enhance the levels of VEGF and HA in serum, and has a high safety, which is worthy of further clinical application.

Prognostic Evaluation of Microinflammatory Markers Combined with NLR in Patients with End-Stage Renal Disease Undergoing Maintenance Hemodialysis

【Objective】To explore the prognostic evaluation of microinflammatory markers combined with the neutrophil to lymphocyte ratio (NLR) in patients with end-stage renal disease (ESRD) undergoing maintenance hemodialysis (MHD).【Methods】A total of 76 ESRD patients undergoing MHD were followed up for one year, and their prognosis was recorded. Based on the outcomes, patients were divided into the poor prognosis group (Group A, 18 cases) and the good prognosis group (Group B, 58 cases). Factors influencing prognosis and the efficacy of microinflammatory markers combined with NLR in prognostic assessment were analyzed.【Results】Among the 76 patients, the incidence of poor prognosis was 23.68%(18/76). Group A had higher proportions of malnutrition and infection, longer dialysis duration, and significantly elevated levels of high-sensitivity C-reactive protein (hs-CRP), tumor necrosis factor-α (TNF-α), and NLR compared to Group B (P<0.05). Additionally, the residual renal filtration rate in Group A was significantly lower than that in Group B (P<0.05). Independent prognostic factors for ESRD patients included residual renal filtration rate, hs-CRP, and NLR (all P<0.05). The sensitivity of hs-CRP, NLR, and their combination in predicting the prognosis of ESRD patients undergoing MHD was 72.22%, 83.33%, and 66.67%, respectively, while the specificity was 72.41%, 65.52%, and 98.28%, respectively. The specificity of the combined diagnosis of both hs-CRP and NLR was significantly higher than that of single marker's diagnosis (P<0.05).【Conclusion】The hs-CRP and NLR have good predictive value for the prognosis of ESRD patients undergoing MHD.

Multivariate analysis was used to identify the factors influencing the development of PICS in sepsis patients, and receiver operating characteristic (ROC) curves were used to evaluate the predictive value of T lymphocyte subsets in the occurrence of PICS.【Results】 The PICS group had lower blood lactate and inflammatory factor levels than the non-PICS group, while T lymphocyte subsets and nutritional indicators were higher in the PICS group (all P<0.05). Multivariate analysis showed that blood lactate, inflammatory levels, immune function, and nutritional status were important factors influencing the development of PICS in sepsis patients (P<0.05). The combined detection of T lymphocyte subsets showed an area under the curve (AUC) of 0.965 for predicting PICS in sepsis patients, which was higher than the individual tests of T lymphocyte subsets (P<0.05).【Conclusion】 T lymphocyte subset levels have high predictive value for the occurrence of PICS in sepsis patients.

Correlation Analysis of Early Blood Inflammation Indicators and Severity of Disease in Patients with Community-Acquired Pneumonia

【Objective】To investigate the association between early blood inflammatory markers and the severity of disease in patients with community-acquired pneumonia (CAP). 【Methods】A total of 99 patients with CAP treated in our hospital from November 2022 to May 2024 were selected and divided into mild group, moderate group and severe group according to the pneumonia Severity index (PSI) score. Laboratory detection indicators and early blood inflammation indicators of patients were collected, and Pearson was used to analyze the correlation between early blood inflammation indicators and the severity of disease in CAP patients. 【Results】There were 33 cases (33.33%) in the mild group, 35 cases (35.36%) in the moderate group, and 31 cases (31.31%) in the severe group. There was no significant difference in total bilirubin (TBIL), direct bilirubin (DBIL), indirect bilirubin (IBIL), platelet count, red blood cell count and fibrinogen (Fib) between mild group, moderate group and severe group (P>0.05). The serum sodium concentration of severe group was lower than that of mild group and moderate group. Comparing within the Alanine aminotransferase (ALT), aspartate aminotransferase (AST), blood urea nitrogen (BUN), blood lactate dehydrogenase (LDH), procalcitonin (PCT), C-reactive protein (CRP), white blood cell count, γ-glutamyl transpeptidase (GGT), thrombin time (TT), prothrombin time (PT) and D-dimer in three groups (DD), the differences were statistically significant (P<0.05).Pearson correlation analysis showed that blood sodium concentration was negatively correlated with PSI score in CAP patients (P<0.05), while ALT, AST, BUN, LDH, PCT, CRP, white blood cell count, GGT, TT, PT, DD were positively correlated with PSI score (P<0.05). 【Conclusion】The severity of CAP patients is closely related to many factors in the blood, which is helpful to evaluate the severity of CAP patients.

Multimodal Ultrasonographic Analysis of HER-2 Positive Breast Cancer Patients with Different Expression States of Estrogen Receptor and Progesterone Receptor

【Objective】To investigate the multimodal ultrasound characteristics of human epidermal growth factor receptor 2 (HER-2) positive breast cancer patients with different expression states of estrogen receptor (ER) and progesterone receptor (PR). 【Methods】A total of 149 patients with HER-2 positive breast cancer admitted to our hospital from May 2020 to March 2024 were selected and divided into group A (n=71, positive HER-2, negative ER and PR), group B (n=27, positive HER-2, positive ER or PR) and group C (n=51, positive ER or PR) according to immunohistochemical results. HER-2, ER, PR positive).Ultrasonic characteristics of HER-2 positive breast cancer patients with different expression states of ER and PR were analyzed. 【Results】The proportion of the group A with unclear boundary, enhanced posterior echo, calcification, mixed internal echo, aspect ratio > 0.7, Adler grade 2-3, and BI-RADS grade 5 was higher than that of the group B and the group C (P<0.05). The above indexes in the group B were higher than those in the group C (P<0.05). The ratio of elasticity score ≥3 and hardness ratio of lesion tissue to adipose tissue in the group A were higher than those in the groups B and C (P<0.05).The above indexes in group B were higher than those in the group C (P<0.05). The peak time (PT) in the group A was shorter than that in the groups B and C (P<0.05), and the PT in group B was shorter than that the in group C (P<0.05). The proportion of enhancement intensity, internal perfusion defect and clear boundary after enhancement in the group A was higher than that in the group B and the group C (P<0.05).The above indexes in group B were higher than those in group C (P<0.05). 【Conclusion】The multimodal ultrasound features of patients with HER-2 positive breast cancer are closely related to the expression of ER and PR.

Comparison of the Clinical Efficacy and Impact on Cognitive Function of Lorazepine and Olanzapine in the Treatment of Schizophrenia

【Objective】To compare the clinical efficacy and their impact on patients' cognitive function of lurasidone and olanzapine in the treatment of schizophrenia.【Methods】A total of 116 patients with schizophrenia admitted to Baoshan District Mental Health Center in Shanghai from August 2020 to December 2023 were selected and randomly divided into two groups using a random number table: the lurasidone group and the olanzapine group, with 58 patients in each group. Both groups of patients were treated for 12 weeks, and their clinical symptom improvement, cognitive function, biochemical indicators of neurological function [nerve growth factor (NGF), gamma aminobutyric acid (GABA), specific enolase (NSE)], inflammatory and metabolic indicators [interleukin-4 (IL-4), interleukin-8 (IL-8), triglycerides (TG), body mass index (BMI)], and incidence of adverse reactions were compared. 【Results】The scores of the Negative and Positive Symptom Scale (PANSS) in both groups were lower than those before treatment, and the PANSS scores in the lurasidone group were lower than those in the olanzapine group, with statistically significant differences (P<0.05). At the 4th and 12th week of treatment, the MCCB scores of both groups were higher than those before treatment, and the MCCB score of the lurasidone group was higher than that of the olanzapine group, with statistical significance (P<0.05). After treatment, the levels of serum NGF and GABA in both groups were higher than those before treatment, and the levels of serum NGF and GABA in the lurasidone group were higher than those in the olanzapine group, with statistical significance (P<0.05). The serum NSE levels in both groups were lower than those before treatment, and the level in the lurasidone group was lower than that in the olanzapine group, with statistically significant difference (P<0.05). After treatment, the average levels of IL-4 and IL-8 in the serum of both groups were lower than those before treatment. The levels of IL-4 and IL-8 in the lurasidone group were lower than those in the olanzapine group. The BMI and TG in the lurasidone group were lower than before treatment and both BMI and TG were lower than those in the olanzapine group, with statistical significance (P<0.05); After treatment, there was no statistically significant difference in BMI and TG in the olanzapine group if compared to before treatment (P>0.05). The total incidence of adverse reactions in the lurasidone group was 8.62% (5/58), which was lower than the 24.14% (14/58) in the olanzapine group, and the difference was statistically significant (P=0.024). 【Conclusion】Lurasidone can improve cognitive function, reduce levels of inflammatory factors, and not increase the risk of metabolic disorders in patients with schizophrenia. It has a low incidence of adverse reactions and good safety.

The Efficacy of Mesalazine Combined with Compound Glutamine in the Treatment of Ulcerative Colitis

【Objective】To explore the efficacy of mesalazine combined with compound glutamine in the treatment of ulcerative colitis (UC).【Methods】 A total of 102 UC patients were randomly divided into a control group and an observation group, with 51 cases in each group. Patients in the control group received mesalazine treatment, while those in the observation group received mesalazine combined with compound glutamine. The outcomes, colonic function (improvement time in bloating, diarrhea, and complete recovery time of stool frequency and form/properties), intestinal mucosal barrier function [levels of diamine oxidase (DAO) and D-lactic acid, inflammatory markers (tumor necrosis factor-alpha (TNF-α), C-reactive protein (CRP), interleukin-8 (IL-8), interleukin-(IL-10)], and disease activity (intestinal mucosal lesion score, Mayo activity index score) were compared between the two groups before and after treatment.【Results】 Before treatment, there were no statistically significant differences in the indices between the two groups (P>0.05). After treatment, the observation group showed significantly shorter improvement times for bloating and diarrhea, as well as faster complete recovery of stool frequency and form compared to the control group (P<0.05). Additionally, the DAO, D-lactic acid, TNF-α, CRP, IL-8 levels, intestinal mucosal lesion score, and Mayo activity index score in the observation group were significantly lower than those in the control group, while IL-10 level in the observation group was higher than that in the control group (P<0.05).【Conclusion】 Mesalazine combined with compound glutamine significantly optimizes colonic function and disease activity in UC patients, improves intestinal mucosal barrier function, and reduces inflammation markers.

ROC curve analysis results showed that the sensitivity and specificity of maternal intestinal flora combined with neonatal immune indexes in predicting neonatal infectious pneumonia were 97.5% and 95.5%, respectively, and the area under ROC curve was 0.987, both of which were better than single detection. 【Conclusion】The imbalance of maternal intestinal flora and abnormal neonatal immune function indexes are the risk factors for neonatal infectious pneumonia, and the combination of maternal intestinal flora and neonatal immune indexes has high predictive value for neonatal infectious pneumonia.

Diagnostic Value of TLR4 and NSE Levels in Children with Febrile Seizures

【Objective】 To explore the diagnostic value of Toll-like receptor 4 (TLR4) and neuron-specific enolase (NSE) levels in children with febrile seizures.【Methods】 A total of 72 children with febrile seizures were included in group C, 86 children with fever and infection but no seizures in group B, and 83 healthy children during physical exams in group A. The levels of TLR4 and NSE were measured in all groups. Spearman correlation analysis was used to assess the correlation of febrile seizures with levels of TLR4 and NSE . The diagnostic efficacy of TLR4 and NSE in febrile seizures was analyzed using receiver operating characteristic (ROC) curves.【Results】 The TLR4 and NSE levels in group B and group C were significantly higher than that in group A (P<0.05), with group C showing higher level than group B (P<0.05). Both TLR4 and NSE levels were positively correlated with febrile seizures (P<0.05). The sensitivity of TLR4 and NSE alone and combined detection was 86.1%, 79.2%, and 80.6%, respectively; their specificities were 55.8%, 74.4%, and 88.4%, respectively. And the area under the curve (AUC) was 0.742, 0.832, and 0.911, respectively. Combined detection had better diagnostic value than individual detection (P<0.05).【Conclusion】 Elevated TLR4 and NSE levels in children with febrile seizures are closely associated with the severity. Combined detection of TLR4 and NSE can improve the diagnostic efficiency of febrile seizures and has important clinical significance.

Effect of Rabeprazole Combined with Mosapride on Gastric Motility Indicators and Gastrointestinal Hormones in Patients with Functional Dyspepsia

【Objective】 To explore the effects of rabeprazole combined with mosapride on gastric emptying rate, intestinal emptying rate, gastric motility indicators, and gastrointestinal hormones in patients with functional dyspepsia (FD).【Methods】 A total of 102 patients with FD were randomly divided into two groups: the observation group and the control group, with 51 patients in each group. Patients in the control group received only rabeprazole treatment, while the observation group was treated with rabeprazole combined with mosapride. The gastric emptying rate, intestinal emptying rate, gastric motility indicators [plasma leptin (LEP), motilin (MTL), corticotropin-releasing hormone (CRH)], and gastrointestinal hormones [gastrin (GAS), substance P (SP), vasoactive intestinal peptide (VIP)] levels were compared between the two groups before and after treatment. The incidence of adverse reactions was also recorded and compared.【Results】 After treatment, the observation group showed significantly better gastric emptying rate, intestinal emptying rate, gastric motility indicators, and gastrointestinal hormone levels compared to the control group (P<0.05). The total incidence of adverse reactions in the observation group was 5.88% (3/51), significantly lower than that of the control group at 19.61% (10/51), with a statistically significant difference (P<0.05).【Conclusion】 Rabeprazole combined with mosapride can significantly improve gastric and intestinal emptying rates, gastric motility, and gastrointestinal hormone levels in FD patients, while reducing the incidence of adverse reactions. It is worthy of clinical promotion and application.

The total incidence of complications in the observation group was 11.76%, compared with 7.84% in the control group, there was no significant difference (P>0.05). 【Conclusion】Prednisone and cyclosporin A can effectively regulate sex hormone levels and immune function in RSA patients, improve pregnancy outcomes, and have certain safety.

Serum CRP and NLR Combination Detection for Assessing the Severity and Prognosis of Pediatric Lobular Pneumonia

【Objective】To explore the value of combined detection of serum C-reactive protein (CRP) and neutrophil/lymphocyte ratio (NLR) in assessing the severity and prognosis of pediatric lobular pneumonia. 【Methods】A total of 83 children with lobular pneumonia admitted to our hospital from August 2020 to April 2023 were included in the study. According to the severity of the disease condition, patients were divided into the mild (31 cases), moderate (45 cases), and severe (19 cases) groups. The CRP and NLR levels of the three groups were measured, and Spearman correlation analysis was conducted to assess the relationship of disease severity with CRP and NLR levels. Based on prognosis, the patients were divided into a good prognosis group (70 cases) and a poor prognosis group (25 cases). Logistic regression was used to analyze the risk factors for poor prognosis, and a receiver operating characteristic (ROC) curve was constructed to evaluate the predictive value of combined serum CRP and NLR levels for prognosis. 【Results】The levels of CRP and NLR in the severe group were significantly higher than those in the moderate and mild groups, and the level of CRP and NLR in the moderate group showed higher level than that in the mild group (P<0.05). Correlation analysis indicated a positive correlation between the severity of the disease and serum levels of CRP and NLR (

There were significant differences in the proportion of pleural effusion, white blood cell count, NLR, and CRP between the poor prognosis group and the good prognosis group (P<0.05). Logistic multivariate analysis revealed that the presence of pleural effusion, elevated CRP level and high NLR level were independent risk factors for poor prognosis (P<0.05). ROC curve analysis showed that the areas under the curve (AUC) of NLR and CRP levels for predicting the poor prognosis of children with lobar pneumonia were 0.785 and 0.808, respectively, and the specificity and AUC of the combination of the two were 93.44% and 0.873, respectively. 【Conclusion】 Serum levels of CRP and NLR are effective in assessing the severity and prognosis of pediatric lobular pneumonia. The combined detection of both markers provides high sensitivity and specificity for predicting poor prognosis.

Clinical Efficacy of Laparoscopic Ovariostomy Combined with Estradiol Valerate in the Treatment of Infertility Patients with Polycystic Ovary Syndrome

【Objective】To investigate the clinical effect of laparoscopic ovariectomy (LOD) combined with estradiol valerate in the treatment of polycystic ovary syndrome (PCOS) patients with infertility. 【Methods】A total of 117 infertility patients with PCOS admitted to our hospital from June 2019 to April 2022 were recruited. According to different treatment methods, patients were divided into two groups. The control group (n=56) was treated with LOD combined with clomiphene, and the observation group (n=61) was treated with LOD combined with estradiol valerate. Patients in both groups were treated for 3 menstrual cycles, and the efficacy of the two groups was compared. 【Results】After treatment, follicle stimulating hormone (FSH), estradiol (E

), follicle number and endometrial thickness in the observation group were higher than those in control group, while luteinizing hormone (LH) in the obseation group was lower than that in control group (P<0.05). The total treatment effective rate, high-quality follicle rate and 1-year follow-up pregnancy rate in the observation group were higher than those in the control group (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】LOD combined with estradiol valerate for treating PCOS patients with infertility can significantly improve the quality follicle rate and pregnancy rate, improve ovarian function and endometrial thickness, and not increase the incidence of adverse reactions.

Risk Factors of Secondary Infection in Patients with Severe Acute Pancreatitis Treated by Ultrasound-Guided Percutaneous Catheter Drainage

【Objective】To investigate the risk factors of secondary infection in patients with severe acute pancreatitis treated by ultrasound-guided percutaneous catheter drainage (PCD). 【Methods】The clinical data of 70 patients with severe acute pancreatitis treated with ultrasound-guided PCD from January 2021 to January 2024 were retrospectively analyzed, and the patients were divided into infected group (n=28) and uninfected group (n=42) according to whether they had secondary infection. Clinical data of all patients were collected, and clinical data and related laboratory indicators [albumin (ALB), white blood cell (WBC) count, hypersensitive C-reactive protein (hs-CRP), procalcitonin (PCT), platelet count (PLT), blood urea nitrogen (BUN)] of the two groups were compared. Multivariate Logistic regression was used to analyze the risk factors of secondary infection in patients with severe acute pancreatitis. 【Results】 The age of the infected group was ≥60 years old, the APACHE Ⅱ score at admission was > 11 points, the mechanical ventilation time was ≥6 days, the proportion of hypoxemia and multiple organ failure was higher than that of the uninfected group, and the WBC, PCT and hs-CRP were higher than that of the uninfected group, with statistical significance (P<0.05). Multivariate Logistic regression analysis showed that increased WBC count, increased hs-CRP, increased PCT, age ≥60 years, APACHE Ⅱ score > 11 points upon admission, mechanical ventilation duration ≥6 days, hypoxemia and multiple organ failure were risk factors for secondary infection in patients with severe acute pancreatitis (P<0.05).【Conclusion】Secondary infection after treatment of severe acute pancreatitis with ultrasound-guided PCD is related to age, APACHE Ⅱ score at admission, mechanical ventilation time, hypoxemia, multiple organ failure and other factors. WBC count, hs-CRP and PCT can be used as predictors of secondary infection of severe acute pancreatitis.

Serum HMGB1 and PTX3 Levels and their Correlation with Disease Severity and Prognosis in Children with Mycoplasma Pneumoniae Pneumonia Complicated with Pleural Effusion

【Objective】To investigate the serum levels of high mobility group protein B1 (HMGB1) and pentaprotein 3 (PTX3) and their correlation with disease severity and prognosis in children with mycoplasma pneumoniae pneumonia (MPP) complicated with pleural effusion. 【Methods】A total of 88 children with MPP admitted to our hospital from January 2020 to December 2023 were selected and divided into MPP children with pleural effusion group (combined group, n=58) and MPP children without pleural effusion group (uncombined group, n=30) according to whether the children had pleural effusion. Healthy children who came to our hospital for physical examination at the same period were selected as the control group (n=30). According to the severity of MPP, the combined group was further divided into mild group (n=23), moderate group (n=18) and severe group (n=17), and the combined group was divided into good group (n=27) and adverse group (n=31) according to the prognosis of the children. Serum HMGB1 and PTX3 levels in children with different disease severity were compared, and serum HMGB1 and PTX3 levels in children with different prognosis were compared. Spearman correlation coefficient was used to analyze the correlation between serum HMGB1 and PTX3 and symptoms, disease severity and prognosis of children. 【Results】The levels of serum HMGB1 and PTX3 in the non-combined group and the combined group were significantly higher than those in the control group, and the levels in the combined group were significantly higher than those in the non-combined group, with statistical significance (P<0.05). Serum HMGB1 and PTX3 levels in the moderate group and the severe group were significantly higher than those in the mild group, and the severe group was significantly higher than the moderate group, with statistical significance (P<0.05). The levels of serum HMGB1 and PTX3 in the good group were lower than those in the bad group, and the difference was statistically significant (P<0.05). Spearman correlation analysis showed that serum HMGB1 and PTX3 levels were positively correlated with pleural effusion, disease severity and prognosis (P<0.05). 【Conclusion】 The serum levels of HMGB1 and PTX3 in children with MPP complicated with pleural effusion are significantly correlated with disease severity and prognosis.
