913 Clinical Effect of IL-6 Receptor Antagonist in the Treatment of Patients with Ventricular Arrhythmia after Myocardial Infarction

【Objective】To investigate the clinical effect of interleukin-6 (IL-6) receptor antagonist in the treatment of patients with ventricular arrhythmia after myocardial infarction.【Methods】A total of 100 patients with ventricular arrhythmia after myocardial infarction treated in our hospital from June 2022 to June 2024 were selected and divided into the observation group (treated with IL-6 receptor antagonist tocilizumab injection) and the control group (treated with conventional succinate metoprolol sustained-release tablets) according to different treatment methods, with 50 cases in each group. The total treatment efficiency, total satisfaction, quality of life, cardiac function indexes, and inflammatory factor levels were compared between the two groups. 【Results】The total treatment efficiency, satisfaction, and quality of life scores of the observation group were significantly higher than those of the control group (P<0.05); the cardiac function indexes of the observation group were better than those of the control group (P<0.05); the level of matrix metalloproteinase-9 (MMP-9) in the observation group was higher than that in the control group, while the levels of tumor necrosis factor-α (TNF-α), IL-6, and high-sensitivity C-reactive protein (hs-CRP) were lower than those in the control group, and the differences were statistically significant (P<0.05). 【Conclusion】The treatment of patients with ventricular arrhythmia after myocardial infarction with the IL-6 receptor antagonist tocilizumab injection has a significant effect, which can improve patient satisfaction, quality of life and cardiac function, and reduce the levels of inflammatory factors.

916 The Clinical Efficacy of the Xiaoyu Zhitong Formula Combined with the Diaoshang Ointment in Treating Patients with Traumatic Synovitis of the Knee Joint Caused by Qizhi Xueyu both Internally and Externally

【Objective】To explore the clinical efficacy of the Xiaoyu Zhitong Formula combined with the Diaoshang Ointment (plaster external application) in the internal and external treatment of patients with traumatic synovitis of the knee joint caused by qizhi xueyu.【Methods】A total of 74 patients with traumatic synovitis of the knee joint with qizhi xueyu type who were diagnosed and treated in our hospital from June 2023 to July 2024 were selected and divided into the observation group (treated with Xiaoyu Zhitong Formula combined with Diaoshang ointment) and the control group (treated with loxoprofen sodium patch combined with ibuprofen sustained-release capsules) according to the random number table method, with 37 cases in each group. The synovial thickness score, Lysholms score, knee joint range of motion and knee joint circumference of the two groups were compared before treatment and 2 weeks after treatment. 【Results】After 2 weeks of treatment, the synovial thickness scores and knee joint circumference in both groups were lower than those before treatment; And these measurements in the observation group were significantly lower than those in the control group, with statistically significant differences (P<0.05). After 2 weeks of treatment, the Lysholms scores and the range of knee flexion in both groups were higher than those before treatment; And the parameters in the observation group were significantly higher than those in the control group (P<0.05). 【Conclusion】The combination of Xiaoyu Zhitong Formula and Diaoshang Ointment for both internal and external treatment of patients with traumatic synovitis of the knee joint caused by qizhi xueyu can effectively alleviate synovitis of the knee joint, effectively reduce the thickness of the knee joint synovium and the circumference of the knee joint, improve the function of the knee joint of patients, and improve the range of the knee flexion.

920 Efficacy of Anlotinib in the Treatment of Advanced Non-Small Cell Lung Cancer and its Influence on Quality of Life

【Objective】To investigate the efficacy of Anlotinib in the treatment of patients with advanced non-small cell lung cancer (NSCLC) and its influence on quality of life. 【Methods】A total of 98 patients with advanced NSCLC treated in our hospital from August 2022 to August 2024 were selected and divided into control group (given conventional chemotherapy) and observation group (given Anlotinib on the basis of conventional chemotherapy) according to different treatment methods, with 49 cases in each group. The clinical efficacy, serum indexes, quality of life and incidence of adverse reactions were compared between the two groups. 【Results】The disease control rate and overall response rate of the observation group were higher than those of the control group (P<0.05). After treatment, the levels of vascular endothelial growth factor (VEGF), carbohydrate antigen 125 (CA125) and matrix metalloproteinase-9 (MMP-9) in the observation group were lower than those in the control group (P<0.05). After treatment, the score of Generic Quality of Life Inventory (GQOLI-74) in the observation group was higher than that in the control group (P<0.05). There was no significant difference in the total incidence of adverse reactions between the observation group and the control group (P>0.05). 【Conclusion】Anlotinib combined with conventional chemotherapy has significant efficacy in the treatment of patients with advanced NSCLC, which can significantly improve the quality of life of patients, significantly down-regulate the levels of biomarkers related to tumor angiogenesis, invasion and metastasis, and has good safety.

923 Clinical Efficacy and Safety of Entecavir Combined with Antitubercular Drugs in Different Treatment Regimens for Pulmonary Tuberculosis Complicated with Chronic Hepatitis B

【Objective】 To explore the clinical efficacy and safety of entecavir combined with antitubercular drugs in different treatment regimens for pulmonary tuberculosis complicated with chronic hepatitis B.【Methods】 A retrospective analysis was performed on the clinical data of 120 newly diagnosed patients with pulmonary tuberculosis complicated with chronic hepatitis B admitted to Jiangxi Chest hospital from January 2021 to June 2023. According to different treatment regimens, the patients were divided into the control group (n=32),the observation group A (n=47), and the observation group B (n=41). The treatment efficacy and incidence of adverse reactions were compared among the three groups. The levels of serum total bilirubin (TBIL), alanine aminotransferase (ALT), and gamma-glutamyl transpeptidase (GGT) were compared before and after treatment. The HBV-DNA levels were compared before treatment, at 6 months, 12 months of treatment, and 3 months after drug withdrawal. 【Results】 The total clinical effective rates of observation group A and observation group B were higher than that of the control group, with statistically significant differences (P<0.05). After treatment, the serum TBIL and ALT levels in observation group A and observation group B were higher than those before treatment, the GGT levels were lower than those before treatment, and the serum TBIL, ALT, and GGT levels were all lower than those in the control group, with statistically significant differences (P<0.05). At 6 months, 12 months of treatment, and 3 months after drug withdrawal, the HBV-DNA levels in observation group A and observation group B were lower than those in the control group, with statistically significant differences (P<0.05). The incidences of adverse reactions in the three group were no significant differences (P>0.05). 【Conclusion】 For patients with pulmonary tuberculosis complicated with chronic hepatitis B taking antitubercular drugs, continuing antiviral therapy after treatment according to the principles of "early, regular, combined, appropriate, and full-course" can achieve better clinical efficacy and better safety.

927 Related Risk Factors for Subcutaneous Hematoma in Patients with Arteriovenous Fistula during Uremic Maintenance Hemodialysis

【Objective】To explore the related risk factors of subcutaneous hematoma in patients with arteriovenous fistula during uremic maintenance hemodialysis.【Methods】A retrospective analysis was conducted on the clinical data of 120 patients with arteriovenous fistula in uremic maintenance hemodialysis admitted to our hospital from July 2018 to December 2023. The patients were divided into the occurrence group (n=34) and the non-occurrence group (n=86) based on whether subcutaneous hematoma occurred. The clinical data of the patients were collected, and Logistic regression analysis was used to analyze the related risk factors of subcutaneous hematoma in patients with arteriovenous fistula during uremic maintenance hemodialysis.【Results】The proportions of age >65 years old, puncture times ≥3 times, puncture site infection, and compression hemostasis time <30 min after dialysis in the occurrence group were all higher than those in the non-occurrence group, and the differences were statistically significant (P<0.05). The results of Logistic regression analysis showed that age >65 years old, puncture times ≥3 times, puncture site infection, and compression hemostasis time <30 minutes after dialysis were all risk factors for subcutaneous hematoma in patients with arteriovenous fistula during uremic maintenance hemodialysis (P<0.05). 【Conclusion】Puncture site infection, puncture frequency, hemostasis compression time, age, etc. are all related factors affecting the subcutaneous hematoma of arteriovenous fistula in patients undergoing maintenance hemodialysis for uremia. Medical staff should strengthen the prevention and control nursing of puncture site infection, appropriately reduce the puncture frequency, prolong the hemostasis compression time, and enhance the condition monitoring and symptom assessment of elderly patients.

930 Factors Influencing Postoperative Incision Infection in Natural Delivery Parturients Undergoing Episiotomy and Intervention Measures

【Objective】To explore the influencing factors and intervention measures for postoperative incision infection in natural delivery parturients undergoing episiotomy.【Methods】A total of 200 natural delivery parturients who underwent episiotomy in our hospital from January 2021 to January 2022 were selected and divided into an infection group (n=89) and a non-infection group (n=111) based on whether incision infection occurred postoperatively. Multivariate logistic regression analysis was used to identify the risk factors for postoperative incision infection in natural delivery parturients after episiotomy.【Results】Multivariate logistic regression analysis showed that body mass index (BMI) >28 kg/m2, history of diabetes, reproductive tract infection, vaginal examinations >3 times, operative time >30 minutes, and premature rupture of membranes were independent risk factors for postoperative incision infection in natural delivery parturients undergoing episiotomy. 【Conclusion】BMI >28 kg/m2, history of diabetes, reproductive tract infection, vaginal examinations >3 times, operative time >30 minutes, and premature rupture of membranes are independent risk factors for postoperative incision infection in natural delivery parturients after episiotomy. Targeted nursing interventions for these risk factors can help reduce the risk of incision infection.

933 The Correlation between Inter-Nurse Horizontal Violence and Turnover Intention in the Operating Room

【Objective】To investigate and analyze the current situation of inter-nurse horizontal violence in the operating room, and to explore the correlation between inter-nurse horizontal violence and turnover intention in the operating room. 【Methods】Fifty-three nurses who worked in the operating room of our hospital from September 2023 to September 2024 were selected and investigated through a self-made demographic questionnaire, a horizontal violence questionnaire and a turnover intention scale. Pearson correlation analysis was used to analyze the relationship between inter-nurse horizontal violence and turnover intention, and multiple Logistic regression analysis was used to analyze the correlation factors influencing the turnover of operating room nurses. 【Results】Among the 53 nursing staff in the operating room, 31 had suffered both overt and covert lateral violence simultaneously, and 12 had never suffered lateral violence. There were 6 cases of simple explicit horizontal violence and 14 cases of simple implicit horizontal violence. In the horizontal violence survey scale, the proportions of opinions being ignored, gossip or rumors being spread, distrust, information being blocked/concealed, humiliation or ridicule are relatively high. The average score of turnover intention of 53 nursing staff in the tested operating room was (3.01±0.28) points. The inter-nurse horizontal violence in the operating room was positively correlated with turnover intention (r=0.895, P<0.05). Multivariate Logistic regression analysis showed that 1-5 years of working time in the main position, secondary vocational school education level, nurse title level, contract system, ignored opinions, being gossiped or rumored, not being trusted, being blocked or concealing information, and being humiliated or ridiculed were risk factors leading to the turnover of operating room nurses (P<0.05). 【Conclusion】Inter-nurse horizontal violence in the operating room is closely related to the intention to resign. Nursing managers should attach importance to the adverse effects of horizontal violence on nursing staff, actively improve management strategies and working systems, and reduce the turnover of nursing staff.

936 Effect of Functional Carbohydrates on Liver Function, Insulin Resistance and Quality of Life in Patients with Non-Alcoholic Fatty Liver Disease

【Objective】To explore the effect of functional carbohydrates (FCs) on liver function, insulin resistance and quality of life in patients with non-alcoholic fatty liver disease (NAFLD). 【Methods】A total of 128 NAFLD patients admitted to our hospital from June 2022 to June 2023 were selected and divided into the observation group (n=66) and the control group (n=62) by random number table method. The control group was treated with conventional NAFLD treatment regimen, and the observation group was given FCs dietary intervention on the basis of the control group. Both groups were intervened for 6 months. The changes of insulin resistance (HOMA-IR), blood lipid levels, quality of life and liver function indexes were compared between the two groups. 【Results】After intervention, HOMA-IR, fasting plasma glucose (FPG) and glycosylated hemoglobin (HbA1c) in both groups were lower than those before intervention, and the observation group was lower than the control group, with statistically significant differences (P<0.05). After intervention, triglyceride (TG) and total cholesterol (TC) levels in both groups were lower than those before intervention, and the observation group was lower than the control group, with statistically significant differences (P<0.05). After intervention, aspartate aminotransferase (AST), alanine aminotransferase (ALT) and gamma-glutamyl transferase (GGT) in the observation group were lower than those before intervention, and GGT in the control group was lower than that before intervention (P<0.05); AST, ALT and GGT in the observation group were lower than those in the control group (P<0.05). After intervention, the scores of physical function, general health and physical function in the observation group were higher than those before intervention and higher than those in the control group, with statistically significant differences (P<0.05). After intervention, there was no significant difference in liver stiffness measurement (LSM) and controlled attenuation parameter (CAP) between the control group and those before intervention (P>0.05); LSM and CAP in the observation group were lower than those before intervention and lower than those in the control group, with statistically significant differences (P<0.05). 【Conclusion】FCs can effectively improve insulin resistance and liver function in NAFLD patients, improve the quality of life of patients, and help alleviate the accumulation of liver fat, which is worthy of clinical reference.

940 Value Analysis of HDL-C Combined with RDW in Evaluating the Severity and Prognosis of Sepsis

【Objective】To explore the clinical value of high-density lipoprotein cholesterol (HDL-C) combined with red blood cell distribution width (RDW) in evaluating the severity and prognosis of sepsis. 【Methods】 A total of 103 sepsis patients admitted to our hospital from January 2020 to January 2024 were selected and divided into sepsis group (n=42), severe sepsis group (n=36), and septic shock group (n=25) according to the severity of illness, and into survival group (n=60) and death group (n=43) according to prognosis. HDL-C and RDW levels were compared among patients with different severities, the correlation between HDL-C, RDW and sepsis severity was analyzed, clinical data and HDL-C, RDW levels were compared between survival and death groups, and related factors affecting death in sepsis patients were analyzed. 【Results】HDL-C levels gradually decreased and RDW levels gradually increased in the sepsis group, severe sepsis group, and septic shock group, with statistically significant differences (P<0.05). HDL-C was negatively correlated with sepsis severity (P<0.05), while RDW was positively correlated with sepsis severity (P<0.05). HDL-C level in the death group was lower than that in the survival group, and RDW level was higher than that in the survival group, with statistically significant differences (P<0.05). The death group had higher C-reactive protein (CRP), sequential organ failure assessment(SOFA) score, Interleukin-35(IL-35), acute physiology and chronic health evaluation Ⅱ score(APACHEⅡ score), white blood cell count, aspartate aminotransferase (AST), serum creatinine (Scr), and procalcitonin (PCT) than the survival group, with statistically significant differences (P<0.05). Multivariate Logistic regression analysis showed that APACHEⅡ, HDL-C, RDW, and SOFA score were related factors affecting the prognosis of sepsis patients (P<0.05). ROC curve analysis showed that the AUC values of RDW, HDL-C, and their combination for predicting death in sepsis patients were 0.824, 0.805, and 0.899 (P<0.05), respectively, and the combined AUC value was higher (P<0.05). 【Conclusion】HDL-C and RDW are closely related to the prognosis and severity of sepsis patients, and their combination has a higher efficacy in predicting patient prognosis.

944 Predictive Value of PLR Combined with CHA2DS2-VASc Score for No-Reflow Phenomenon after PCI in STEMI Patients

【Objective】 To explore the predictive value of the platelet-to-lymphocyte ratio (PLR) combined with the CHA2DS2-VASc score for the no-reflow phenomenon after percutaneous coronary intervention (PCI) in patients with ST-segment elevation myocardial infarction (STEMI).【Methods】A total of 136 STEMI patients who underwent PCI were analyzed. According to the occurrence of the no-reflow phenomenon postoperatively, the patients were divided into groups. Binary logistic regression analysis was used to identify the influencing factors of reflow or no-reflow after PCI. Receiver operating characteristic (ROC) curve analysis was performed to assess the predictive value of PLR and CHA2DS2-VASc score for postoperative no-reflow.【Results】 Among the 136 patients, 31 cases (21.23%) developed the no-reflow phenomenon after PCI (no-reflow group), while the remaining 105 cases did not (reflow group). Preoperative systolic and diastolic blood pressure, interleukin-6, total stent length, PLR, and CHA2DS2-VASc score were all significantly higher in the no-reflow group compared to the reflow group (P<0.05). Logistic regression analysis showed that total stent length, PLR, and CHA2DS2-VASc score were risk factors for postoperative no-reflow. ROC curve analysis indicated that the combination of PLR and CHA2DS2-VASc score had a significantly higher area under the curve and specificity for predicting no-reflow after PCI compared to either parameter alone (P<0.05).【Conclusion】 The combination of PLR and CHA2DS2-VASc score has a high predictive value for the occurrence of no-reflow in STEMI patients after PCI and can serve as an important reference for evaluating postoperative no-reflow risk.

947 Effect of Multidisciplinary Collaboration Combined with the 5P Transfer System on the Efficiency of Long-Distance Transfers in Critically Ill Emergency Patients

【Objective】 To explore the effectiveness of applying a multidisciplinary collaboration model combined with the 5P transfer system in the long-distance transport of critically ill emergency patients.【Methods】 A total of 322 critically ill patients requiring long-distance transfer between January and June,2024 were included as the control group, using the conventional transfer protocol. Another 359 critically ill patients transferred between July and December,2024 constituted the observation group, where the multidisciplinary collaboration combined with the 5P transfer system was implemented. Transfer time, incidence of adverse events during transport, occurrence of information omissions during transfer and handover, and nursing cooperation level were compared between the two groups.【Results】 There was no significant difference in transfer distance between the two groups (P>0.05). The observation group had significantly shorter transfer times, a lower overall incidence of adverse events during transport, fewer handover information omissions, and a higher level of nursing cooperation compared to the control group (all P<0.05).【Conclusion】 The application of a multidisciplinary collaboration model combined with the 5P transfer system in the long-distance transport of critically ill emergency patients can significantly improve transfer efficiency, reduce transfer risks, and contribute to the stabilization of patients' conditions transfer.

951 Application Value of Microscope and Foraminoscope in Lumbar Canal Decompression and Interbody Fusion for Patients with Degenerative Lumbar Spinal Stenosis

【Objective】 To explore the application value of microscope and foraminoscope in transforaminal lumbar interbody fusion (TLIF) for patients with degenerative lumbar spinal stenosis (DLSS). 【Methods】 The clinical data of 82 DLSS patients admitted to our hospital from March 2021 to March 2023 were retrospectively analyzed. According to different surgical methods, they were divided into control group (TLIF under microscope) and observation group [TLIF under foraminoscope (E-TLIF)], with 41 cases in each group. Perioperative indicators, pain mediator levels, inflammatory stress levels, total incidence of postoperative complications, and lumbar function recovery were compared between the two groups. 【Results】 The incision length, operation time, and hospital stay in the observation group were shorter than those in the control group, and the blood loss was less than that in the control group (P<0.05). At 12 h, 24 h, and 48 h after surgery, the visual analogue scale (VAS) scores of the two groups were lower than those before surgery (P<0.05), and the observation group was lower than the control group (P<0.05). After surgery, the levels of substance P (SP), prostaglandin E2 (PGE2), neuropeptide Y (NPY), C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), monocyte chemoattractant protein (MCP-1), and malondialdehyde (MDA) in both groups were higher than those before surgery, but the observation group was lower than the control group (P<0.05). There was no significant difference in the total incidence of complications between the two groups (P>0.05). At 6 months and 12 months after surgery, the Oswestry Disability Index (ODI) scores of the two groups were lower than those before surgery, and the Japanese Orthopedic Association (JOA) scores were higher than those before surgery (P<0.05), but there was no significant difference in ODI scores and JOA scores between the two groups at 6 months and 12 months after surgery (P>0.05). 【Conclusion】 Both E-TLIF and microscope-assisted TLIF can promote the recovery of lumbar function in DLSS patients, but E-TLIF can reduce operation time and blood loss, alleviate pain and inflammatory stress, and cause less trauma to the body.

955 Diagnostic Value of MRI and Different Postural MSCT 3D Imaging for Complex Ankle Joint Injury

【Objective】To investigate the diagnostic and prognostic value of magnetic resonance imaging (MRI) and multi-slice spiral CT (MSCT) 3D examination in different body positions in the diagnosis and classification of complex ankle joint injuries. 【Methods】The clinical data of 215 patients with complex ankle injuries admitted to the hospital from April 2021 to March 2023 were retrospectively analyzed. All patients received conventional position MSCT 3D examination, optimized position MSCT 3D examination and MRI examination. Taking pathological diagnosis as the gold standard, the diagnostic value of MRI and different body positions' MSCT 3D in complex ankle joint injury, classification, type of cartilage injury and prognosis were analyzed. 【Results】Of 215 cases of complex ankle injuries, 46 cases of pronation-pronation type (PE), 38 cases of supination-external pronation type (SE), 49 cases of supination-adduction type (SA), 32 cases of vertical compression type (VC), 50 cases of pronation-abduction type (PA). There were 40 cases of stage Ⅰ, 59 cases of stage Ⅱ, 64 cases of stage Ⅲ, and 52 cases of stage Ⅳ. Anatomical reduction occurred in 186 cases and non-anatomical reduction in 29 cases. The diagnostic accuracy of optimized postural MSCT 3D and VC was higher than that of conventional position MSCT 3D and MRI (P<0.05), and the diagnostic rate of MSCT 3D in conventional position was higher than that of MRI (P<0.05). There was no difference in the accuracy of PE, SE, SA and PA among the three methods in the diagnosis of complex ankle injury classification (P>0.05). The accuracy of MRI in diagnosing anatomic reduction of different types of cartilage injury and complex ankle injury was higher than that of MSCT 3D in optimal position and MSCT 3D in conventional position (P<0.05), and the diagnostic accuracy of MSCT 3D in optimal position was higher than that of conventional position (P<0.05). 【Conclusion】Optimized postural MSCT 3D examination is more valuable than MRI and conventional postural MSCT 3D examination in diagnosing complex ankle injuries. The value of MRI in the diagnosis of cartilage injury type and prognosis is higher than that of MSCT 3D in different positions.

959 Predictive Value of Serum LDH and its Isoenzymes, FAR for Prognosis of Patients with Acute Exacerbation of COPD

【Objective】To explore the predictive value of serum lactate dehydrogenase (LDH) and its isoenzymes, fibrinogen (FIB)/albumin (ALB) ratio (FAR) for the prognosis of patients with acute exacerbation of chronic obstructive pulmonary disease (COPD). 【Methods】 The clinical data of 81 patients with acute exacerbation of COPD treated in our hospital from January 2022 to June 2024 were retrospectively analyzed. According to the prognosis within 1 year after treatment, the patients were divided into poor prognosis group (n=32) and good prognosis group (n=49). The general data, serum LDH and its isoenzymes, and FAR were compared between the two groups. Logistic multivariate regression was used to analyze the risk factors for poor prognosis in patients with acute exacerbation of COPD, and receiver operating characteristic (ROC) curve was used to analyze the predictive value of serum LDH and its isoenzymes, FAR for the prognosis of such patients. 【Results】The FAR and serum LDH1, LDH2, LDH4, LDH5, total LDH levels in the poor prognosis group were higher than those in the good prognosis group, with statistically significant differences (P<0.05). High levels of FAR, serum LDH1, LDH4, LDH5, and total LDH were risk factors for poor prognosis in patients with acute exacerbation of COPD (P<0.05). ROC curve analysis showed that FAR, serum LDH1, LDH4, LDH5, and total LDH levels all had certain predictive value for the prognosis of patients with acute exacerbation of COPD, and the combined prediction value of these indicators was better than that of single indicator. 【Conclusion】Serum LDH and its isoenzymes, FAR have certain predictive value for the prognosis of patients with acute exacerbation of COPD, and the combined detection of these indicators has a better predictive value than single indicator.

【Objective】To explore the clinical efficacy of a combination treatment of metformin, empagliflozin, pegylated liraglutide and exercise therapy in the treatment of obese type 2 diabetes. 【Methods】A total of 112 obese type 2 diabetes patients admitted to our hospital from October 2020 to October 2023 were selected and divided into groups by the ball-picking method (odd numbers assigned to the control group and even numbers to the observation group), with 56 cases in each group. The control group was treated with metformin, empagliflozin combined with exercise therapy, while the observation group was treated with pegylated liraglutide in addition to treatment of the control group. The glycolipid metabolism indicators, pancreatic β-cell function, and carotid atherosclerosis indicators were compared before and after treatment in both groups. 【Results】After treatment, the levels of fasting blood glucose (FBG), postprandial blood glucose (PBG), glycosylated hemoglobin (HbA1c), total cholesterol (TC), triglycerides (TG), and low-density lipoprotein cholesterol (LDL-C) in the observation group were lower than those in the control group, while the level of high-density lipoprotein cholesterol (HDL-C) was higher than that in the control group, and the differences were statistically significant (P<0.05). The pancreatic β-cell function index (HOMA-β) in the observation group was higher than that in the control group, and the insulin resistance index (HOMA-IR) was lower than that in the control group, and the differences were statistically significant (P<0.05). The carotid intima-media thickness (CIMT) and pulse wave velocity (PWV) in the observation group were lower than those in the control group, and the differences were statistically significant (P<0.05). 【Conclusion】The combination of metformin, empagliflozin, pegylated liraglutide and exercise therapy in the treatment of obese type 2 diabetes patients can significantly improve glycolipid metabolism, enhance pancreatic β-cell function, and reduce atherosclerosis indicators.

966 Effect of Endoscopic Dacryocystorhinostomy Combined with Cross-Linked Hyaluronic Acid Gel on the Treatment of Dacryocystitis and Dry Eye Symptoms

【Objective】To explore the clinical effect of endoscopic dacryocystorhinostomy combined with self-cross-linked sodium hyaluronate gel in the treatment of dacryocystitis and its influence on dry eye symptoms in patients. 【Methods】A total of 92 patients with dacryocystitis admitted to our hospital from January 2019 to December 2022 were selected. The patients were divided into the observation group and the control group according to different treatment methods, with 46 cases in each group. The control group was treated with the traditional external nasal dacryocystorhinostomy, while the observation group was treated with endoscopic dacryocystorhinostomy combined with self-crosslinking sodium hyaluronate gel. The lacrimal gland secretion function (Schirmer Ⅰ test filter paper tear adhesion length), tear discharge degree score, tear film rupture time, ocular surface Disease Index Scale (OSDI) score, and inflammatory indicators [tumor necrosis factor-α(TNF-α), C-reactive protein (CRP), interleukin-6(IL-6), and interleukin-10 (IL-10)],bleeding conditions, and the occurrence and recurrence of postoperative complications were compared between the two groups. 【Results】One month after the operation, the tear staining length of the filter paper in the Schirmer Ⅰ test in the observation group was longer than that in the control group (P<0.05); Two months after the operation, the score of tear overflow degree in the observation group was lower than that in the control group (P<0.05). One month after the operation, The tear film break-up time in the observation group was longer than that in the control group (P<0.05). Two months after the operation, the OSDI score of the observation group was lower than that of the control group (P<0.05). One month after the operation, the levels of TNF-α, CRP and IL-6 in the observation group were lower than those in the control group, while the level of IL-10 was higher than that in the control group (P<0.05). The bleeding rate, the incidence of postoperative complications and the recurrence rate in the observation group were all lower than those in the control group (P<0.05). 【Conclusion】Endoscopic dacryocystorhinostomy combined with self-cross-linked sodium hyaluronate gel in the treatment of patients with dacryocystitis can significantly improve the secretory function of the lacrimal gland, effectively alleviate dry eye symptoms, reduce inflammatory responses, and lower the incidence and recurrence rate of postoperative complications. It is a safe and effective treatment method.

970 Diagnostic Value of Multimodal Ultrasound for Benign and Malignant Thyroid Nodules in Patients with Diffuse Thyroid Diseases

【Objective】To evaluate the diagnostic value of multimodal ultrasound for benign and malignant thyroid nodules in patients with diffuse thyroid diseases. 【Methods】A total of 85 patients with diffuse thyroid diseases complicated with thyroid nodules treated in two hospitals from January 2021 to January 2023 were selected. Taking pathological results as the gold standard, all patients were divided into benign group (n=45) and malignant group (n=40). All patients underwent multimodal ultrasound examination. The nature, boundary, morphology, echo, aspect ratio, and calcification of thyroid nodules, as well as elastic parameters, blood flow typing, and hemodynamic parameters [peak systolic velocity (PSV), end-diastolic velocity (EDV), resistance index (RI)] were compared between the two groups. Multivariate analysis was performed based on the ultrasonic characteristics of thyroid nodules in the two groups, and a nomogram model was constructed. The receiver operating characteristic (ROC) curve was used to evaluate the diagnostic efficacy of multimodal ultrasound for benign and malignant thyroid nodules in patients with diffuse thyroid diseases. 【Results】Compared with the benign group, the malignant group had more solid nodules, blurred boundaries, irregular morphology, low or extremely low echo, larger aspect ratio, and obvious microcalcification. The proportions of blood flow types Ⅲ and Ⅳ in the malignant group were higher than those in the benign group, PSV and RI were higher than those in the benign group, and EDV was lower than that in the benign group, with statistically significant differences (all P<0.05). Multivariate analysis showed that the ultrasonic characteristics and blood flow parameters of thyroid nodules were significantly correlated with the benignancy and malignancy of thyroid nodules (P<0.05). The accuracy, sensitivity, and specificity of multimodal ultrasound combined examination in diagnosing benign and malignant thyroid nodules in patients with diffuse thyroid diseases were 95.3%, 95.0%, and 95.6%, respectively, with high diagnostic efficacy. 【Conclusion】 Multimodal ultrasound has high diagnostic value for benign and malignant thyroid nodules in patients with diffuse thyroid diseases.

974 Influencing Factors of Intestinal Obstruction Following Chemotherapy after Laparoscopic Radical Surgery in Patients with Gastrointestinal Tumors

【Objective】To investigate the factors influencing the occurrence of intestinal obstruction in patients with gastrointestinal tumors following laparoscopic radical surgery and subsequent chemotherapy.【Methods】A total of 162 patients who underwent laparoscopic radical surgery and postoperative chemotherapy for gastrointestinal tumors were followed for 30 days postoperatively. The incidence of intestinal obstruction was recorded. Univariate and multivariate Logistic regression analyses were performed to identify factors associated with the development of intestinal obstruction.【Results】Among the 162 patients, 13 cases developed intestinal obstruction within 30 days postoperatively, yielding an incidence rate of 8.02%. Factors showing significantly more prevalent in the obstruction group compared to the non-obstruction group included: male gender, age ≥70 years, tumor stage Ⅲ-Ⅳ, preoperative hypoproteinemia, history of preoperative intestinal obstruction, history of abdominal surgery, operative time ≥2 hours, combined organ resection, use of antiemetic drugs post-chemotherapy, time to first flatus ≥48 hours, time to first defecation ≥72 hours, time to recovery of bowel sounds ≥72 hours, and time to postoperative feeding ≥96 hours (P<0.05). Multivariate logistic regression analysis identified gender, age, tumor stage, preoperative hypoproteinemia, history of abdominal surgery, combined organ resection, use of antiemetic drugs post-chemotherapy, time to first flatus, time to first defecation, time to recovery of bowel sounds, and time to postoperative feeding as significant factors influencing the occurrence of intestinal obstruction (P<0.05).【Conclusion】The development of intestinal obstruction following chemotherapy after laparoscopic radical surgery in patients with gastrointestinal tumors is influenced by multiple factors, including gender, age, tumor stage, preoperative hypoproteinemia, history of abdominal surgery, combined organ resection, and use of antiemetic drugs post-chemotherapy. Clinicians should proactively implement preventive measures to minimize the risk of intestinal obstruction.

978 Relationship between Brain Structural Changes and Cognitive Function in Subcortical Ischemic Cerebrovascular Disease

【Objective】To investigate the relationship between brain structural changes and cognitive function in patients with subcortical ischemic vascular disease (SIVD).【Methods】 A total of 134 SIVD patients were divided into a non-cognitive impairment group (MoCA scores≥26, 38 cases, Group A) and a cognitive impairment group (MoCA scores<26, 96 cases, Group B) based on the Montreal Cognitive Assessment (MoCA) scores. The brain structural changes and cognitive functions between the two groups were compared, including differences in cerebrospinal fluid volume, white matter volume, and gray matter volume. Brain regions with significant differences in gray matter volume were compared, and the correlation between the gray matter volume of these regions and cognitive function scores in Group B was analyzed.【Results】 The Hamilton Depression Scale (HAMD) and Hamilton Anxiety Scale (HAMA) scores in Group B were higher than those in Group A (P<0.05). The Rey Auditory Verbal Learning Test (RAVLT) delayed recall and immediate recall scores, MoCA scores, Mini-Mental State Examination (MMSE) scores, and Verbal Fluency Test (VFT) scores in Group B were lower than those in Group A (P<0.05). The gray matter volume of the left medial superior frontal gyrus and other regions in Group B was lower than that in Group A (P<0.05). In Group B, the gray matter volume of the left insula was negatively correlated with the time taken in part A of the Stroop Color-Word Test (CWT-A) (P<0.05); the gray matter volume of the left anterior insula was positively correlated with MMSE scores and Symbol Digit Modalities Test (SDMT) scores (P<0.05); the gray matter volume of the left anterior cingulate gyrus was positively correlated with MMSE, VFT, and SDMT scores (P<0.05).【Conclusion】 Patients with SIVD and cognitive impairment exhibits gray matter atrophy in certain brain regions. The gray matter volume of the left insula, left anterior insula, and left anterior cingulate gyrus is mainly associated with language, attention, cognition, and executive function.

982 Application Value of Small Incision with External Stripping and Internal Ligation Resection in Postoperative Pain Management of Patients with Mixed Hemorrhoids

【Objective】To explore the application value of small incision with external stripping and internal ligation resection in postoperative pain management of patients with mixed hemorrhoids. 【Methods】A retrospective analysis was conducted on the clinical data of 156 patients with mixed hemorrhoids admitted to our hospital from March 2022 to March 2024. According to different surgical methods, the patients were divided into a control group (with traditional external excision and internal ligation resection, n=75) and an observation group (with small incision external excision and internal ligation resection, n=81). Efficacy, clinical indicators (intraoperative blood loss, surgery time, hospital stay, wound healing time, duration of pain), pre-operative and post-operative pain scores [Visual Analog Scale (VAS) scores], and incidence of complications were compared between two groups. 【Results】The total effective rate of the observation group was 96.30% (78/81), which was higher than the control group's 86.67% (65/75), and the difference was statistically significant (χ2=4.727, P<0.05). The intraoperative blood output of the observation group was lower than that of the control group, and the operation time, hospitalization time, wound healing time, and duration of pain were all shorter than those of the control group, with statistical significance (P<0.05). On postoperative day 1, both groups had higher VAS scores than before surgery, but the difference was not statistically significant (P>0.05). One week after surgery, the VAS scores of both groups were lower than those before surgery and one day after surgery; And the VAS scores of the observation group were lower than those of the control group, with a statistically significant difference (P<0.05).【Conclusion】The clinical efficacy of small incision with external stripping and internal ligation resection for the treatment of mixed hemorrhoids is good, which can effectively reduce intraoperative bleeding, shorten surgical time and hospital stay, promote wound healing, alleviate pain, and have a low incidence of postoperative complications. It is worthy of clinical reference.

985 Influence of Different Embolic Materials on Tumor Volume and Postoperative Complications in Interventional Embolization of Giant Hepatic Hemangioma

【Objective】 To investigate the influence of different embolic materials on tumor volume and postoperative complications in interventional embolization of giant hepatic hemangioma. 【Methods】Clinical data of 94 patients with giant hepatic hemangioma admitted to our hospital from January 2020 to January 2022 were retrospectively analyzed. Patients were divided into the pingyangmycin group and the bleomycin group according to the embolic materials used during surgery, with 47 cases in each group. After 6 months of follow-up, clinical efficacy, blood supply typing, intraoperative lipiodol dosage, hospitalization status, and serum levels of alpha-fetoprotein (AFP), tumor-specific growth factor (TSGF), and tumor necrosis factor-α (TNF-α) were compared between the two groups. Postoperative complications were recorded. 【Results】There were no significant differences in blood supply typing, intraoperative lipiodol dosage, hospitalization time, or hospitalization costs between the two groups (P>0.05). The total effective rate of the pingyangmycin group was higher than that of the bleomycin group (P<0.05). At 3 and 6 months postoperatively, the tumor diameter in the pingyangmycin group was smaller, and the tumor shrinkage rate was higher than those in the bleomycin group (P<0.05). At 6 months postoperatively, serum levels of AFP, TSGF, and TNF-α in both groups were lower than those before surgery, and the levels in the pingyangmycin group were lower than those in the bleomycin group (P<0.05). There was no significant difference in the incidence of complications between the two groups (P>0.05). 【Conclusion】 The application of pingyangmycin-lipiodol mixture in interventional embolization of giant hepatic hemangioma is beneficial to improving clinical efficacy, with no serious complications and high safety.

989 Construction and Validation of a Nomogram Model for Predicting Symptomatic Intracranial Hemorrhage after Intravenous Thrombolysis in Acute Ischemic Stroke

【Objective】To construct and validate a nomogram model based on clinical data to predict symptomatic intracranial hemorrhage (sICH) following intravenous thrombolysis in patients with acute ischemic stroke (AIS).【Methods】 A total of 213 AIS patients who received intravenous thrombolysis were randomly divided into a training set and a validation set in a 7∶3 ratio using stratified sampling. Multivariate logistic regression analysis was used to identify independent predictors of sICH, which were then used to build a nomogram model. The model was evaluated using receiver operating characteristic (ROC) curves, calibration curves, and decision curve analysis (DCA).【Results】 The training set included 149 AIS patients, among whom 25 cases (16.78%) developed sICH. Multivariate logistic regression analysis identified age, baseline National Institutes of Health Stroke Scale (NIHSS) score, onset-to-treatment time (OTT), neutrophil-to-lymphocyte ratio (NLR), and hyperdense middle cerebral artery sign (HMCAS) as independent predictors of sICH (P<0.05). ROC curve analysis showed that the area under the curve (AUC) of the nomogram model was 0.972 in the training set and 0.940 in the validation set, indicating good discrimination. The calibration curve demonstrated good agreement between predicted and observed sICH risk (P=0.196). DCA indicated that the nomogram had good clinical utility.【Conclusion】 Age, NIHSS score, OTT, NLR, and HMCAS are independent predictors of sICH after intravenous thrombolysis in AIS patients. The developed nomogram model can effectively predict the risk of sICH.

993 Risk Factors for Erectile Dysfunction after Laparoscopic Radical Prostatectomy

【Objective】To explore the high-risk factors affecting erectile dysfunction (ED) after laparoscopic radical prostatectomy. 【Methods】A retrospective analysis was performed on 160 prostate cancer patients admitted from June 2019 to June 2023, all of whom underwent laparoscopic radical prostatectomy. Patients were divided into the ED group and normal group based on postoperative ED status. Medical records were collected, and multivariate Logistic regression was used to analyze factors associated with postoperative ED. The receiver operating characteristic (ROC) curve was drawn to evaluate the efficacy of the nomogram model in predicting ED after surgery. 【Results】At 6 months postoperatively, the International Index of Erectile Function-5 (IIEF-5) score was lower than the preoperative score, with a statistically significant difference (P<0.05). Among 160 patients, 46 (28.75%) developed postoperative ED, and 114 (71.25%) had normal erectile function. The ED group had higher age, prostate-specific antigen (PSA), Gleason score, and higher proportions of smoking history, alcohol abuse history, and clinical stage T2 compared with the normal group, with statistically significant differences (P<0.05). Logistic regression analysis showed that advanced age, elevated PSA, higher Gleason score, smoking history, alcohol abuse history, and clinical stage T2 were risk factors for ED after laparoscopic radical prostatectomy (P<0.05). ROC curve analysis showed that the nomogram model had a sensitivity of 0.870 (95%CI: 0.613-0.972), specificity of 0.772 (95%CI: 0.563-0.871), and area under the curve (AUC) of 0.893 (95%CI:0.851-0.935) for predicting postoperative ED. 【Conclusion】Advanced age, elevated PSA, higher Gleason score, smoking history, alcohol abuse history, and rapid pathological progression are high-risk factors for ED after laparoscopic radical prostatectomy. Clinicians should provide relevant interventions in a timely manner based on specific patient conditions.

【Objective】To explore the clinical efficacy of perospirone combined with olanzapine in the treatment of schizophrenia and its influence on glucose and lipid metabolism.【Methods】A total of 102 schizophrenia patients admitted to our hospital from November 2020 to December 2023 were selected and divided into the observation group (treated with perospirone combined with olanzapine) and the control group (treated with olanzapine alone) by random number table method, with 51 cases in each group. The clinical efficacy after two months of treatment, symptom scores [Positive and Negative Symptom Scale (PANSS) scores] before and after two months of treatment, glucose and lipid metabolism levels [fasting blood glucose (FBG), glycosylated hemoglobin (HbA1c), total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C)], and cognitive function [Wisconsin Card Sorting Test (WCST) scores] were compared between the two groups. 【Results】 The total clinical effective rate of the observation group was 94.12% (48/51), which was higher than 78.43% (40/51) of the control group, and the difference was statistically significant (χ2=5.299, P<0.05). After treatment, the positive symptoms, negative symptoms, and general pathological scores of the two groups were lower than those before treatment, and the observation group was lower than the control group, with statistically significant differences (P<0.05). After treatment, the serum FBG, HbA1c, TC, TG, and LDL-C levels of the two groups were lower than those before treatment, and the observation group was lower than the control group, with statistically significant differences (P<0.05). After treatment, the number of completed classifications and total correct scores of the two groups were higher than those before treatment, and the observation group was higher than the control group, with statistically significant differences (P<0.05); the total response number, persistent error number, and total error scores of the two groups were lower than those before treatment, and the observation group was lower than the control group, with statistically significant differences (P<0.05). 【Conclusion】Perospirone combined with olanzapine can effectively improve the clinical efficacy of schizophrenia patients, ameliorate their psychiatric symptoms, effectively regulate glucose and lipid metabolism levels, and help improve patients' cognitive function.

1000 Clinical Observation of Pegylated Interferon α-2a Combined with Tenofovir Disoproxil Fumarate in the Treatment of HBeAg-Positive Chronic Hepatitis B Patients

【Objective】To investigate the clinical efficacy of pegylated interferon α-2a combined with tenofovir disoproxil fumarate in the treatment of hepatitis B e antigen (HBeAg)-positive chronic hepatitis B (CHB) patients. 【Methods】A total of 76 newly diagnosed CHB patients with HBeAg positivity admitted to our hospital from June 2021 to May 2024 were randomly divided into the observation group and the control group using a random number table method, with 38 patients in each group. The control group was treated with tenofovir disoproxil fumarate, while the observation group was treated with pegylated interferon alpha-2a in addition to the control group. Both groups were treated continuously for 48 weeks. The clinical efficacy [negative conversion rate of hepatitis B virus load (HBV-DNA), virological response rate, negative conversion rate of HBeAg, serological conversion rate of HBsAg] and adverse drug reactions of the two groups were compared, and the levels of HBV-DNA, HBeAg, ALT, liver fibrosis indicators [hyaluronic acid (HA), laminin (LN), type Ⅲ procollagen (PC Ⅲ), type Ⅳ collagen (C-Ⅳ)], inflammatory factors [interleukin-17 (IL-17), interleukin-4 (IL-4), interferon-α(IFN-α)] before and after treatment were compared in the two groups. 【Results】The HBV-DNA negative conversion rate, HBeAg negative conversion rate, virological response rate, and HBsAg seroconversion rate in the observation group were significantly higher than those in the control group, and the differences were statistically significant (P<0.05). After treatment, the levels of HBV-DNA, HBeAg, and ALT in both groups were lower than those before treatment, and the levels of HBV-DNA, HBeAg, and ALT of the observation group was even lower than those of the control group, with statistical significance (P<0.05). After treatment, the levels of HA, LN, PC Ⅲ, C-Ⅳ in both groups were lower than those before treatment, and the levels of HA, LN, PC Ⅲ, C-Ⅳof the observation group was lower than those of the control group, with statistical significance (P<0.05). After treatment, IL-17 and IL-4 levels in both groups were lower than those before treatment, while IFN-α levels were higher than that before treatment, with statistically significant differences (P<0.05); The levels of IL-17 and IL-4 in the observation group were lower than those in the control group, while IFN-α was higher than that in the control group, and the differences were statistically significant (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】The clinical effect of pegylated interferon alpha-2a combined with tenofovir disoproxil fumarate in the treatment of HBeAg positive initial CHB is good. It inhibits the progression of liver fibrosis and inflammatory response, and has high safety.

1004 Application Value of Combined Detection of Chemiluminescent Microparticle Immunoassay and Treponema Pallidum Particle Agglutination Test in the Diagnosis of Syphilis

【Objective】 To explore the diagnostic value of combining chemiluminescent microparticle immunoassay (CMIA) and Treponema pallidum particle agglutination assay (TPPA) in the diagnosis of syphilis.【Methods】 Venous blood samples were collected from 40 clinically confirmed syphilis patients and 40 healthy individuals. All samples were tested using both CMIA and TPPA. The results of the two methods were compared. Subjects were also grouped based on CMIA derived S/CO values, and TPPA results were compared across these groups.【Results】Among the 80 subjects, CMIA detected 41 positives and 39 negatives; TPPA detected 38 positives and 42 negatives. There was no statistically significant difference between the results of CMIA and TPPA (P>0.05). The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of CMIA were 100.00%, 97.50%, 97.56%, 100.00%, and 98.75%, respectively, with a Kappa value of 0.975. Compared with clinical diagnosis, there was no significant difference (P>0.05). The corresponding values for TPPA were 95.00%, 100.00%, 100.00%, 95.24%, and 97.50%, respectively, with a Kappa value of 0.950, also showing no significant difference from clinical diagnosis (P>0.05). The concordance rate between CMIA and TPPA was 96.25%, with a Kappa value of 0.925 (P>0.05). TPPA positivity increased with higher S/CO values. When S/CO > 7, TPPA positivity reached 100.00%.【Conclusion】 Both CMIA and TPPA exhibit high sensitivity, specificity, and accuracy in syphilis diagnosis. TPPA positivity increases significantly with higher S/CO values. A stratified diagnostic approach based on an S/CO threshold suggests that high S/CO(>7) cases can be directly diagnosed following CMIA screening without TPPA confirmation, thereby reducing detection time and cost. TPPA verification remains necessary for cases with S/CO ≤ 7. This optimized diagnostic workflow can improve efficiency and is well-suited for large-scale screening programs.

1007 Clinical Effect of Shakubactri Valsartan in the Treatment of Chronic Heart Failure with Hypertension

【Objective】To investigate the clinical efficacy of sacubitril valsartan in the treatment of hypertension with chronic heart failure. 【Methods】A total of 98 patients with hypertension and chronic heart failure admitted to the two hospitals from February 2019 to March 2023 were selected. The patients were randomly divided into the observation group and the control group using a random number table, with 49 patients in each group. Blood pressure, serum N-terminal pro brain natriuretic peptide (NT-proBNP), angiotensin Ⅱ (Ang Ⅱ) levels, cardiac function indicators [left ventricular end systolic diameter (LVESD), left ventricular end diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF)], and medication safety were compared between two groups before and after treatment. 【Results】The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). After treatment, the systolic and diastolic blood pressure in both groups were lower than those before treatment, and the value of the observation group was lower than that of the control group, with a statistically significant difference (P<0.05). The serum levels of NT-proBNP and AngⅡ in both groups were lower than those before treatment, and those in the observation group were lower than those in the control group with statistically significant difference (P<0.05). LVESD and LVEDD after treatment in both groups were lower than those before treatment, while LVEF was higher than that before treatment (P<0.05). After treatment, both LVESD and LVEDD of the observation group were lower than those of the control group, while LVEF was higher than that of the control group (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】Sacubitril valsartan can reduce the levels of serum AngⅡ and NT-proBNP in patients with hypertension and chronic heart failure, lower blood pressure, improve cardiac function. And it is safe and reliable.

1011 Effects and Mechanism of Finasteride on Lower Urinary Tract Symptoms Caused by Benign Prostatic Hyperplasia in Rats

【Objective】To investigate the effect and mechanism of finasteride on lower urinary tract symptoms (LUTS) caused by benign prostatic hyperplasia (BPH) in rats. 【Methods】 Thirty rats were divided into sham operation group, model group, and finasteride group, with 10 rats in each group. The model group and finasteride group were used to establish BPH models by castration plus injection of testosterone propionate, while the sham operation group was not castrated and injected with an equal amount of sesame oil. After successful modeling, the finasteride group was intragastrically administered with finasteride at 0.8 mg/kg, and the model group and sham operation group were intragastrically administered with normal saline for 28 consecutive days. After the last gavage, the urination status of each group was detected; the rats were sacrificed to calculate the prostate index and bladder index. HE staining was used to observe the histopathological changes of prostate and bladder tissues, Masson staining to observe the collagen fiber deposition in bladder tissues, and TUNEL method to detect the apoptosis rate of bladder detrusor cells. Western blot was used to detect the relative expression levels of nucleotide-binding oligomerization domain-like receptor protein 3 (NLRP3), interleukin-1β (IL-1β), and cysteine-aspartic acid protease-1 (Caspase-1) in bladder detrusor. 【Results】Compared with the sham operation group, the prostate index, bladder index, micturition frequency, and residual urine volume in the model group were increased (P<0.05), while the total urine volume had no change (P>0.05). Compared with the model group, the prostate index, bladder index, micturition frequency, and residual urine volume in the finasteride group were decreased (P<0.05), and the total urine volume had no change (P>0.05). HE staining showed that the prostate tissue in the model group had obvious hyperplasia, the lamina propria connective tissue of the bladder tissue was hyperplastic, the mucosa was thickened, and a large number of bladder epithelial cells showed papillary hyperplasia; the histopathology of the prostate and bladder tissues in the finasteride group was significantly improved. Compared with the sham operation group, the apoptosis rate of bladder detrusor cells, the positive area ratio of collagen fibers, and the relative expression levels of NLRP3, IL-1β, and Caspase-1 proteins in bladder tissues in the model group were increased (P<0.05). Compared with the model group, the apoptosis rate of bladder detrusor cells, the positive area ratio of collagen fibers, and the relative expression levels of NLRP3, IL-1β, and Caspase-1 proteins in bladder tissues in the finasteride group were decreased (P<0.05). 【Conclusion】Finasteride can effectively improve LUTS caused by BPH in rats and inhibit the apoptosis of bladder detrusor cells, and its mechanism may be related to the NLRP3/Caspase-1 signaling pathway.

1015 Efficacy of Hemoperfusion Combined with Hemodialysis in the Treatment of Acute Antidepressant Poisoning

【Objective】To investigate the efficacy of hemoperfusion combined with hemodialysis in the treatment of patients with acute antidepressant poisoning.【Methods】A total of 82 patients with acute antidepressant poisoning were randomly divided into the control group and the observation group, with 41 patients in each group. Patients in the control group received hemodialysis treatment, while those in the observation group received treatment of hemoperfusion combined with hemodialysis. Both groups were treated continuously for three days. Changes in cardiac function, myocardial enzymes, inflammatory factors, gastrointestinal and renal function indicators before and after treatment, as well as the incidence of complications, were compared between the two groups.【Results】After treatment, the observation group showed a higher left ventricular ejection fraction and lower left ventricular end-diastolic diameter and left ventricular end-systolic diameter compared to the control group (P<0.05). Levels of creatine kinase, lactate dehydrogenase, creatine kinase-MB, C-reactive protein, interleukin-6, tumor necrosis factor-α, alanine aminotransferase, aspartate aminotransferase, and blood urea nitrogen were all significantly lower in the observation group than in the control group (P<0.05). There was no statistically significant difference in the overall incidence of complications between the two groups (P>0.05).【Conclusion】Hemoperfusion combined with hemodialysis is highly effective in treating patients with acute antidepressant poisoning. It can significantly improve cardiac function, reduce myocardial damage, enhance gastrointestinal and renal function, and inhibit the synthesis of inflammatory factors, with good safety.

1019 Construction of a Prediction Model for Recurrence after Transnasal Endoscopic Surgery in Patients with Recurrent Nasopharyngeal Carcinoma

【Objective】To construct a prediction model for recurrence after transnasal endoscopic surgery in patients with recurrent nasopharyngeal carcinoma (NPC). 【Methods】The clinical data of 133 patients with recurrent NPC who underwent transnasal endoscopic surgery in our hospital from April 2020 to April 2023 were retrospectively analyzed. All patients were followed up for 2 years and divided into recurrence group and non-recurrence group according to postoperative recurrence status. Univariate and multivariate analyses were used to identify influencing factors of postoperative recurrence, and a Nomogram model was constructed to predict recurrence risk. The predictive efficacy was evaluated by the area under the receiver operating characteristic (ROC) curve (AUC). 【Results】The 2-year postoperative recurrence rate was 32.33% (43/133). Univariate analysis showed that the recurrence group had higher proportions of clinical stage Ⅲ-Ⅳ, systemic immune-inflammation index (SII) ≥445.3, poor differentiation, lymph node metastasis, and positive EB virus DNA load, and lower proportion of albumin level ≥35 g/L (P<0.05). Binary Logistic regression showed that high clinical stage, high SII, and positive EB virus DNA load were independent risk factors for postoperative recurrence (P<0.05), while high albumin level was an independent protective factor (P<0.05). The Nomogram model constructed based on these factors had a C-index of 0.852 by Bootstrap internal validation, with the calibration curve approaching the ideal curve (P>0.05). ROC curve showed the model had a sensitivity of 88.40%, specificity of 87.80%, and AUC of 0.883 for predicting recurrence. 【Conclusion】High clinical stage, high SII, and positive EB virus DNA load are independent risk factors for postoperative recurrence, while high albumin level is an independent protective factor. The Nomogram model based on these factors can effectively assess the recurrence risk of NPC patients after surgery.

1023 Clinical Efficacy of Zhishen Tiaosui Acupuncture Combined with Fuyang Moxibustion in the Treatment of Post-Stroke Cognitive Impairment

【Objective】 To investigate the clinical efficacy of Zhishen Tiaosui acupuncture combined with Fuyang moxibustion in treating post-stroke cognitive impairment (PSCI).【Methods】 A total of 106 patients with PSCI were randomly divided into the control group (treated with conventional acupuncture) and the observation group (treated with Zhishen Tiaosui acupuncture combined with Fuyang moxibustion), with 53 patients in each group. Neurological function, cognitive function, daily living ability, oxidative stress and metabolic indicators, and neuroprotective markers were compared before and after treatment.【Results】 After treatment, the observation group had significantly lower National Institutes of Health Stroke Scale (NIHSS) scores and significantly higher Neurobehavioral Cognitive Status Examination (NCSE), Montreal Cognitive Assessment (MoCA), Activities of Daily Living (ADL), and Barthel Index scores compared to the control group (P<0.05). The levels of homocysteine (Hcy), cystatin C (cys C), malondialdehyde (MDA), and neuron-specific enolase (NSE) were significantly lower in the observation group, while glutathione (GSH) and brain-derived neurotrophic factor (BDNF) levels were significantly higher than in the control group (P<0.05).【Conclusion】 Zhishen Tiaosui acupuncture combined with Fuyang moxibustion can significantly improve cognitive function, enhance daily living abilities, and optimize oxidative stress response and metabolic function in patients with PSCI.

1027 Relationship of Serum S100β and NSE Levels with the Severity and Prognosis in Traumatic Brain Injury Patients

【Objective】To investigate the relationship of serum levels of S100 calcium-binding protein β (S100β) and neuron-specific enolase (NSE) with the severity and prognosis in traumatic brain injury (TBI) patients.【Methods】A total of 116 TBI patients were categorized into three groups based on the Glasgow Coma Scale (GCS) score: mild group(n=53), moderate group(n=38), and severe group(n=25). They were also grouped by prognosis into the favorable prognosis (n=95) and the poor prognosis (n=21) groups. Serum levels of S100β and NSE were compared across these groups. Receiver operating characteristic (ROC) curves were constructed to assess the predictive value of serum S100β and NSE levels for patient prognosis.【Results】 Serum S100β and NSE levels in the mild group were significantly lower than those in the moderate group, while the levels of serum S100β and NSE in the moderate group were lower than those in the severe group (P<0.05). Patients in the poor prognosis group had significantly higher S100β and NSE levels than those in the favorable prognosis group (P<0.05). ROC analysis showed that the area under the curve (AUC) for the combined prediction using both S100β and NSE was significantly higher than that of either marker alone.【Conclusion】 Serum levels of S100β and NSE in TBI patients increase with the severity of the condition. They both have good predictive value for patient prognosis.

1030 Efficacy of Warm Acupuncture Combined with Tadalafil in the Treatment of Erectile Dysfunction of Middle-Aged and Elderly Patients

【Objective】To investigate the efficacy of warm acupuncture combined with tadalafil in the treatment of erectile dysfunction (ED) of middle-aged and elderly patients. 【Methods】A total of 98 middle-aged and elderly ED patients admitted to Tianshui Hospital of Integrated Traditional Chinese and Western Medicine from June 2022 to May 2024 were randomly divided into two groups, with 49 cases in each group. The control group received conventional tadalafil treatment, while the observation group was additionally treated with warm acupuncture. The total treatment duration was 6 weeks. Clinical efficacy was compared between the two groups by evaluating the International Index of Erectile Function (IEF-5), Erection Hardness Score (EHS), total serum testosterone (tT) levels, Sexual Satisfaction Scale (SS), The measurement of slow-twitch muscle (Type Ⅰ) and fast-twitch muscle (Type Ⅱ),and the endurance test values. Adverse events during treatment were also recorded. 【Results】The total effective rate in the observation group was higher than that in the control group (P<0.05). After treatment, the IEF-5 scores, EHS, SS scores and serum total testosterone levels of the two groups were higher than those before treatment. After 6 weeks of treatment, the levels of IEF-5, EHS, SS scores, and serum tT in the observation group were higher than those in the control group (P<0.05). The measurement of slow-twitch muscle (Type Ⅰ) and fast-twitch muscle (Type Ⅱ), as well as their endurance test values of the two groups of patients were higher than those before treatment. And these test values in the observation group were also higher than those in the control group (P<0.05). 【Conclusion】Warm acupuncture combined with tadalafil has a significant clinical efficacy in treating erectile dysfunction in middle-aged and elderly patients, significantly improving erectile function and sexual satisfaction without increasing the incidence of adverse events.

1033 Analysis of Factors Influencing the Efficacy of Azithromycin Sequential Therapy in Children with Mycoplasma Pneumoniae Pneumonia

【Objective】To explore the clinical efficacy of sequential azithromycin therapy in the treatment of Mycoplasma pneumoniae pneumonia (MPP) in children. 【Methods】A total of 126 children with MPP diagnosed and treated in our hospital from June 2022 to July 2024 were selected and divided into the effective group (n=98) and the ineffective group (n=28) according to the therapeutic effect of the children. The general information, laboratory tests and imaging examination results of the two groups of children were compared. 【Result】The duration of fever in the effective group was shorter than that in the ineffective group. The hypersensitive C-reactive protein (hs-CRP), clinical pulmonary infection score (CPIS), and pediatric critical illness score (PCIS) in the effective group were lower than those in the ineffective group. The incidence of circulatory system complications and the infection rate of multiple bacteria in the effective group were lower than those in the ineffective group. The differences were statistically significant (P<0.05). The results of multivariate Logistic regression analysis showed that the duration of fever, circulatory system complications, hs-CRP, and PCIS were the influencing factors of the therapeutic effect of sequential azithromycin therapy in children with MPP (P<0.05). The area under the receiver operating characteristic (ROC) curve (AUC) of the combined prediction of fever duration, circulatory system complications, hs-CRP and PCIS for children who were ineffective azithromycin sequential therapy was 0.956, with a sensitivity of 85.7% and a specificity of 99.0%. 【Conclusion】The duration of fever, circulatory system complications, hs-CRP, and PCIS are the influencing factors of the therapeutic effect of sequential azithromycin therapy in children with MPP. Clinically, these factors should be comprehensively considered to formulate individualized treatment plans to improve the therapeutic effect of patients.

1037 Effect of Single Injection of Dexmedetomidine on Hemodynamic Parameters of Patients Undergoing Laparoscopic Myomectomy

【Objective】To investigate the effect of single injection of dexmedetomidine (Dex) on hemodynamics in patients undergoing laparoscopic myomectomy (LM). 【Methods 】A total of 90 patients with LM were randomly divided into two groups, 45 cases in each group. Dex was injected once in the observation group before anesthesia induction, while Dex was injected continuously in the control group before anesthesia induction. The clinical indexes, hemodynamic parameters, serological indexes, adverse reactions and sleep quality were compared between the two groups. 【Results】There was no significant difference in operation time between the two groups (P>0.05). The extubation time, anesthesia recovery time and postoperative recovery time in the observation group were shorter than those in the control group (P<0.05). There were no significant differences in mean arterial pressure (MAP), heart rate (HR) and blood oxygen saturation (SPO2) between the two groups before anesthesia induction (P>0.05), and there were no significant differences in HR and SPO2 between the two groups after anesthesia induction (P>0.05). After anesthesia induction, MAP and HR levels in both groups were higher than before induction (P<0.05), and MAP in observation group was lower than control group (P<0.05). There was no significant difference in the levels of epinephrine (E), aldosterone (ALD) and cortisol (Cor) between the two groups before surgery (P>0.05), and the three indexes of the two groups were lower than those of the two groups before surgery (P<0.05), and the three indexes of the observation group were higher than those of the control group (P<0.05). There was no significant difference in Pittsburgh sleep quality index (PSQI) scores between the two groups 1 week after surgery (P>0.05), and PSQI scores in the observation group were lower than those in the control group 2 weeks and 3 weeks after surgery (P<0.05). 【Conclusion】Single injection of Dex has a good anesthetic effect in patients with LM, which is conducive to the stability of hemodynamic indexes of the body.
