Value Analysis of HDL-C Combined with RDW in Evaluating the Severity and Prognosis of Sepsis |

Abstract 【Objective】To explore the clinical value of high-density lipoprotein cholesterol (HDL-C) combined with red blood cell distribution width (RDW) in evaluating the severity and prognosis of sepsis. 【Methods】 A total of 103 sepsis patients admitted to our hospital from January 2020 to January 2024 were selected and divided into sepsis group (n=42), severe sepsis group (n=36), and septic shock group (n=25) according to the severity of illness, and into survival group (n=60) and death group (n=43) according to prognosis. HDL-C and RDW levels were compared among patients with different severities, the correlation between HDL-C, RDW and sepsis severity was analyzed, clinical data and HDL-C, RDW levels were compared between survival and death groups, and related factors affecting death in sepsis patients were analyzed. 【Results】HDL-C levels gradually decreased and RDW levels gradually increased in the sepsis group, severe sepsis group, and septic shock group, with statistically significant differences (P<0.05). HDL-C was negatively correlated with sepsis severity (P<0.05), while RDW was positively correlated with sepsis severity (P<0.05). HDL-C level in the death group was lower than that in the survival group, and RDW level was higher than that in the survival group, with statistically significant differences (P<0.05). The death group had higher C-reactive protein (CRP), sequential organ failure assessment(SOFA) score, Interleukin-35(IL-35), acute physiology and chronic health evaluation Ⅱ score(APACHEⅡ score), white blood cell count, aspartate aminotransferase (AST), serum creatinine (Scr), and procalcitonin (PCT) than the survival group, with statistically significant differences (P<0.05). Multivariate Logistic regression analysis showed that APACHEⅡ, HDL-C, RDW, and SOFA score were related factors affecting the prognosis of sepsis patients (P<0.05). ROC curve analysis showed that the AUC values of RDW, HDL-C, and their combination for predicting death in sepsis patients were 0.824, 0.805, and 0.899 (P<0.05), respectively, and the combined AUC value was higher (P<0.05). 【Conclusion】HDL-C and RDW are closely related to the prognosis and severity of sepsis patients, and their combination has a higher efficacy in predicting patient prognosis.
