Abstract:【Objective】By improving the classical roll test, to explore its diagnostic value in benign paroxysmal positional vertigo of horizontal semicircular canal. 【Methods】 The clinical data of 57 patients with horizontal canal benign paroxysmal positional vertigo (HC-BPPV) were collected from October 1, 2017 to December 31, 2019, At the same time, the classic roll test and the improved roll test (head roll improved from 90°to 180°) were given to compare their effects on the duration of nystagmus and the diagnosis of the affected side of HC-BPPV. Finally, the success of manual reduction was used as the final diagnostic verification and correction, and all diagnostic processes were observed and completed under the naked eye.【Results】Comparing the duration and duration difference of nystagmus between the affected side and the healthy side, there was significant difference between the modified roll test and the classical roll test, whether it was free otolith or adhesive otolith (P<0.05); There was no significant difference between the modified roll test and the classical roll test in the diagnosis rate of the affected side of free otolith (P=0.242 > 0.05), but there was significant difference in the diagnosis rate of adhesive otolith (P<0.05). 【Conclusion】 Compared with the classical roll test, the improved roll test makes the difference of nystagmus on both sides more obvious, helps to better judge the affected side of HC-BPPV, and effectively improves the diagnostic rate of HC-BPPV with adhesive otolith, but has little effect on the diagnostic rate of HC-BPPV with free otolith.
