Therapeutic Effect of Digital Subtraction Angiography Guided Neurointervention Combined with Alteplase in Patients with Ischemic Cerebrovascular Disease

Abstract:【Objective】 To investigate the therapeutic effect of digital subtraction angiography(DSA)-guided neurointervention combined with alteplase in patients with ischemic cerebrovascular disease(ISCV).【Methods】 A total of 102 patients with ISCV were randomly assigned into the control group and the observation group, with 51 cases in each. The control group received intravenous thrombolysis with alteplase, while the observation group received DSA-guided neurointervention combined with alteplase. Clinical efficacy, vascular endothelial function, neurological function [National Institutes of Health Stroke Scale(NIHSS) score], quality of life [Activities of Daily Living(ADL) score], cerebral hemodynamic parameters, and incidence of complications were compared between the two groups before treatment and 12 weeks after treatment.【Results】 The total effective rate in the observation group was higher than that in the control group(P<0.05). After treatment, brachial-ankle pulse wave velocity(ba-PWV) and ankle-brachial index(ABI) levels in the observation group were lower, while flow-mediated dilation(FMD) was higher compared with the control group(P<0.05). The NIHSS score in the observation group was lower than that in the control group, and the ADL score in the observation group was higher than that in the control group(P<0.05). Peak systolic velocity(Vs) and end-diastolic velocity(Vd) in the observation group were lower than those in the control group(P<0.05). The overall incidence of complications was also lower in the observation group compared to the control group(P<0.05).【Conclusion】 DSA-guided neurointervention combined with alteplase has a superior therapeutic effect in ISCV patients. It can improve vascular endothelial function and neurological function, enhance quality of life, and reduce the incidence of complications.
