564 Effect of Calcitonin Combined with Tenghuang Jiangu Capsule on Bone Mineral Density and Bone Metabolism in Treatment of Patients with Osteoporosis

【Objective】To investigate the efficacy of calcitonin combined with Tenghuang Jiangu capsule in the treatment of osteoporosis (OP) patients and its influence on bone mineral density and bone metabolism. 【Methods】A total of 100 OP patients admitted to our hospital from January 2022 to January 2023 were divided into the control group (treated with calcitonin) and the observation group (treated with calcitonin combined with tenghuang jiangu capsule) according to random number table, with 50 cases in each group. The therapeutic effect, bone mineral density, bone metabolic biochemical indexes and inflammatory indexes were compared between the two groups. 【Results】The total effective rate of the observation group was 90.00%, which was higher than that of control group (74.00%), the difference was statistically significant (P<0.05). After treatment, bone mineral density of femoral neck, lumbar vertebra bone density, bone mineral content of femoral neck, lumbar vertebra bone mineral content, bone-specific alkaline phosphatase (BALP), osteocalcin and calcitonin in two groups were higher than those before treatment, and these measurement levels in the observation group were higher than those in the control group (P<0.05). After treatment, the β-typeⅠcollagen carboxy-terminal peptide (β-CTX), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) in the two groups were lower than those before treatment, and the levels of the observation group were lower than those of the control group (P<0.05). 【Conclusion】In the treatment of OP patients, calcitonin combined with Tenghuang Jiangu capsule can significantly improve clinical efficacy, bone mineral content, bone mineral density, biochemical indexes of bone metabolism and inflammatory indexes.

567 Clinical Effect of Liver-Spleen-Kidney Coordinated Treatment of Spleen Deficiency Dampness-Toxicity Syndrome of Cervical HR-HPV Infection

【Objective】To investigate the clinical effect of liver-spleen-kidney coordinated treatment in patients with spleen deficiency and dampness-toxicity syndrome infected with high-risk cervical human papillomavirus (HR-HPV). 【Methods】A total of 60 patients with spleen deficiency and dampness-toxicity syndrome of cervical HR-HPV infection treated in our hospital from March 2023 to September 2024 were selected and divided into the observation group and the control group according to random number table method, with 30 cases in each group. The observation group was treated with the liver-spleen-kidney-coordinated Wuwei Xiaoduyin decoction, while the control group was treated with Baofukang Suppository vaginal medicine. Both groups were treated for 3 courses of treatment. TCM syndrome score and efficacy, total effective rate of HR-HPV negative conversion, vaginal microecological functional index and occurrence of adverse reactions were compared between the two groups. 【Results】After treatment, the TCM syndrome score of the observation group was (1.86±2.01) points, which was significantly lower than that of control group (5.70±3.10) points, and the difference was statistically significant (P<0.05). The total effective rate of the observation group was 92.86% (26/28), which was significantly higher than that of the control group (66.67%,18/27), and the difference was statistically significant (P<0.05). The total effective rate of HR-HPV negative conversion in the observation group was 32.14% (9/28), compared to 33.33% (9/27) in the control group, there was no significant difference (P>0.05). There were no significant differences in pH abnormality rate, H2O2 positive rate and SNA positive rate between the two groups before and after treatment (P>0.05). The positive rate of LE in the observation group after treatment was significantly lower than before treatment, and the difference was statistically significant (P<0.05). There was no significant difference between the positive rate of LE after treatment and before treatment in control group (P>0.05). There were no obvious adverse reactions after treatment in both groups. 【Conclusion】The total effective rate of negative conversion of HR-HPV in two groups is comparable, and the safety is good. However, the improvement of TCM symptom scores and clinical efficacy in the observation group are better than those in the control group, and the positive rate of LE decreases. The observation group is administered orally, which can be extended to male HR-HPV infection.

571 Application Effect of Evidence-Based Specialist Nurse Guidance on Early Respiratory Muscle Training in Patients with Mechanical Ventilation in ICU

【Objective】To investigate the application effect of evidence-based specialist nurse guidance on early respiratory muscle training in patients with mechanical ventilation in intensive care unit (ICU). 【Methods】A total of 160 patients with mechanical ventilation in ICU who were treated in Henan Provincial People's Hospital from April 2023 to June 2024 were selected. Among them, 80 patients with mechanical ventilation in ICU who underwent routine rehabilitation training from April 2023 to October 2023 were included in Group B. From February 2024 to June 2024, 80 patients with mechanical ventilation in ICU who implemented early respiratory muscle training under the guidance of evidence-based specialist nurses were included in group A. The recovery status, respiratory muscle function [maximum inspiratory pressure (MIP), maximum expiratory pressure (MEP)], grip strength, muscle strength classification score, disease severity, the occurrence of adverse events, and mechanical ventilation withdrawal were compared in the two groups.【Results】The mechanical ventilation time, ICU stay and total hospital stay in group A were shorter than those in group B (P<0.05). After the intervention, MEP, MIP, grip strength and MRC muscle strength rating in group A were higher than those in group B (P<0.05), acute physiological and chronic health status Ⅱ (APACHEⅡ) scores were lower than those in group B (P<0.05). The incidence of acquired asthenia, respiratory related pneumonia and delirium in ICU in group A was lower than that in group B, and the success rate of mechanical ventilation withdrawal was higher than that in group B (P<0.05). 【Conclusion】Early respiratory muscle training guided by evidence-based specialist nurses can improve the respiratory muscle function, enhance the muscle strength of patients with mechanical ventilation in ICU, reduce the risk of adverse events of mechanical ventilation, and promote the recovery of patients.

575 The Application Effect of the Teaching Mode of the Situational Clinical Nursing Skills Competition in Undergraduate Clinical Nursing Teaching

【Objective】To explore the application effect of the teaching mode of the situational clinical nursing skills competition in undergraduate clinical nursing teaching. 【Methods】Two hundred female undergraduate nursing students who did internships in our hospital from May to December 2022 were selected and divided into the observation group (based on the teaching mode of the situational clinical nursing skills competition) and the control group (using the traditional nursing teaching mode) by the random number table method, with 100 cases in each group. The scores of theoretical knowledge and practical operation, teaching effects, and students' satisfaction with the teaching methods were compared between the two groups. 【Results】The scores of theoretical knowledge and practical operation in the observation group were both higher than those in the control group, and the difference was statistically significant (P<0.05). The excellent and good rates of theoretical knowledge, basic nursing knowledge, nursing operation norms, coping ability with common clinical problems, the latest development of the nursing discipline, basic nursing operation skills, special nursing skills, clinical emergency handling ability, nursing communication ability and comprehensive ability assessment in the observation group were all higher than those in the control group, and the difference was statistically significant (P<0.05). The satisfaction of students in the observation group with the teaching method was significantly higher than that in the control group, and the difference was statistically significant (χ2=31.512, P<0.05). 【Conclusion】The teaching mode of the situational clinical nursing skills competition is conducive to improving students' nursing operation skills, enhancing their comprehensive qualities. Students have a high satisfaction with the teaching method, which is helpful to improve the effect of practical teaching.

578 Clinical Characteristics and Short-Term Prognosis of Pediatric Patients with Autoimmune GFAP Astrocytopathy

【Objective】 To investigate the clinical characteristics and short-term prognosis of pediatric patients with autoimmune glial fibrillary acidic protein astrocytopathy (GFAP-A).【Methods】 A retrospective analysis was conducted on the clinical data of 15 pediatric patients with cerebrospinal fluid positivity for glial fibrillary acidic protein (GFAP).【Results】 The median age of onset was 8 years. The main clinical symptoms included fever, limb weakness, headache, and loss of appetite. The predominant clinical phenotypes were encephalitis, meningitis, and myelitis. Magnetic resonance imaging (MRI) revealed lesions involving brain parenchyma, meninges, optic nerves, and spinal cord, presenting as patchy, linear, punctate, and strip-like T2 hyperintensities. Among 15 patients,13 cases had elevated cerebrospinal fluid white blood cells, mainly lymphocytes, 8 with elevated protein, and 4 with decreased sugar. Fourteen patients responded well to first-line immunotherapy, including corticosteroids and immunoglobulins, while one patient with overlapping aquaporin(AQP-4) antibody experienced relapse and had a poor response to first-line therapy.【Conclusion】Pediatric GFAP-A patients have diverse clinical symptoms and MRI findings, lacking distinct specificity. First-line immunotherapy is generally effective in treatment, although patients with overlapping AQP-4 antibody tend to have a poor prognosis.

583 Application of Low-Frequency Repetitive Transcranial Magnetic Stimulation Combined with Repetitive Facilitation Technique in the Recovery Period of Stroke Patients

【Objective】To explore the effect of low-frequency repetitive transcranial magnetic stimulation (rTMS) combined with repetitive facilitation technique on motor function recovery in stroke patients. 【Methods】 Eighty-two stroke patients were randomly divided into two groups, with 41 cases each group. The control group received repetitive facilitation technique alone, while the observation group received rTMS combined with repetitive facilitation technique. Both groups were treated for 6 weeks. Neurological function, balance ability, hand function, walking ability, upper and lower limb motor function and quality of life were compared before and after treatment in the two groups. 【Results】 After treatment, the observation group showed greater improvements in neurological function, balance ability, hand function, walking ability, upper and lower limb motor function and quality of life were superior to the control group (all P<0.05).【Conclusion】 The combination of rTMS and repetitive facilitation technique can enhance neurological and motor functions, thus improve the quality of life in stroke patients.

586 Related Risk Factors Affecting Medication Compliance of Patients with Pulmonary Tuberculosis

【Objective】To explore the risk factors related to medication compliance in patients with pulmonary tuberculosis. 【Methods】A retrospective analysis was conducted on the clinical data of 95 patients with pulmonary tuberculosis admitted to our hospital from January 2021 to December 2022. The medication compliance of patients with pulmonary tuberculosis was evaluated using the 8-item Morisky Medication Compliance Scale (MMAS-8). The patients were divided into the good compliance group (MMAS-8 score ≥6 points) and the poor compliance group (MMAS-8 score < 6 points) according to the MMAS-8 score. Univariate analysis was conducted on the clinical data of the two groups of patients, and multivariate Logistic regression analysis was used to analyze the related factors affecting the medication compliance of patients with pulmonary tuberculosis. 【Results】Among the 95 pulmonary tuberculosis patients, a total of 58 patients had a MMAS-8 score of ≥6 points, and the good rate of medication compliance was 61.05%. A total of 37 patients had a MMAS-8 score of <6 points, and the proportion of those with poor medication compliance was 38.95%. There were statistically significant differences between the good compliance group and the poor compliance group in terms of age, living status, per capita family income, cognitive level of tuberculosis knowledge, and psychological distress (P<0.05). Multivariate Logistic regression analysis showed that age ≥60 years old, living alone, low per capita family income, low cognitive level of tuberculosis knowledge, and psychological distress were all independent risk factors affecting the medication compliance of tuberculosis patients (P<0.05).【Conclusion】Age ≥60 years old, living alone, low per capita family income, low cognitive level of tuberculosis knowledge, and psychological distress are all independent risk factors affecting the medication compliance of tuberculosis patients. Clinicians need to pay close attention to the above-mentioned tuberculosis patients, strengthen health education for tuberculosis patients, and improve patients' cognitive level of tuberculosis knowledge, which is helpful to improve the medication compliance of tuberculosis patients.

590 The Application Value of 3D Virtual Fiberoptic Bronchoscopy Combined with VR Staining in Airway Foreign Bodies in Children

【Objective】To explore the application value of 3D virtual fiberoptic bronchoscopy combined with VR staining in airway foreign bodies in children. 【Methods】 A retrospective analysis was conducted on the CT and clinical data of 103 children with bronchial foreign bodies. To analysis the 3D Virtual fiberoptic bronchoscopy combined with VR staining technology and fiberoptic bronchoscopy in the exploration of airway foreign bodies after post-processing of the images.【Results】Among the 103 children, airway foreign bodies were located in the trachea and the left bronchus in 1 case, the left bronchus in 38 cases, the right bronchus in 63 cases, and bilateral in 1 case. 3D Virtual fiberoptic bronchoscopy combined with VR staining technology was used and 103 cases were diagnosed correctly. 3D Virtual fiberoptic bronchoscopy combined with VR technology has a good consistency with fiberoptic bronchoscopy in the exploration of airway foreign bodies (Kappa=0.934). Indirect signs: 98 cases of emphysema, 43 cases of obstructive pneumonia, 6 cases of atelectasis, 2 cases of subcutaneous emphysema and mediastinal emphysema, and 1 case of pleural effusion. 【Conclusion】 For airway foreign bodies, 3D Virtual fiberoptic bronchoscopy combined with VR staining is a safe and effective method, and can be used as the preferred examination method for patients with suspected airway foreign bodies.

593 Clinical Efficacy of Transoral Vestibular Approach in the Treatment of Differentiated Thyroid Cancer and its Impact on Recurrent Laryngeal Nerve Function

【Objective】To investigate the clinical efficacy of the transoral vestibular approach in the treatment of differentiated thyroid cancer and its impact on recurrent laryngeal nerve function in patients. 【Methods】A retrospective analysis was conducted on the clinical data of 106 patients with differentiated thyroid cancer treated in our hospital from January 2022 to December 2023.The patients were divided into a control group(underwent traditional open surgery)and an observation group(underwent transoral vestibular approach endoscopic surgery),with 53 cases in each group. The swallowing function, pain level, and incision aesthetic satisfaction of the patients were assessed using the Swallowing Impairment Score(SIS),Visual Analog Scale(VAS),and Patient and Observer Scar Assessment Scale.The clinical efficacy, perioperative indicators, recurrent laryngeal nerve injury, and incidence of complications were compared between the two groups. 【Results】There was no significant difference in the clinical efficacy rate and the total incidence of postoperative complications between the two groups(both P>0.05).The observation group had a longer operation time than the control group,but the intraoperative blood loss and postoperative drainage volume were lower than those in the control group, with significant differences(P<0.05).There was no significant difference in the number of lymph nodes dissected between the two groups(P>0.05).The SIS, VAS, Observer Scar Assessment Scale(OSAS) and Patient Scar Assessment Scale(PSAS) scores in the observation group were lower than those in the control group, and the serum C-reactive protein(CRP) and interleukin-6(IL-6) levels were also lower than those in the control group, with significant differences(P<0.05).The incidence of temporary hoarseness in the observation group was lower than that in the control group, with a significant difference(P<0.05).No permanent hoarseness occurred in either group. 【Conclusion】The transoral vestibular approach endoscopic surgery shows good clinical efficacy in the treatment of differentiated thyroid cancer. It has significant advantages in improving patients' swallowing function, reducing postoperative pain, enhancing incision aesthetic satisfaction, and protecting the recurrent laryngeal nerve, with good safety.

597 Clinical Effects of High-Flow Nasal Cannula Oxygen Therapy and Non-Invasive Positive Pressure Ventilation in Patients with Pulmonary Edema

【Objective】Objective To investigate the clinical effects of high-flow nasal cannula oxygen therapy(HFNC)and non-invasive positive pressure ventilation(NIPPV)in patients with pulmonary edema.【Methods】The clinical data of 166 patients with pulmonary edema treated in our hospital from September 2021 to September 2024 were retrospectively analyzed. Among them,86 patients received NIPPV(the control group)and 80 patients received HFNC(the observation group).Further,65 cases were selected from each group using propensity score matching as the study subjects. The improvement of symptoms and arterial oxygen partial pressure(PaO2),arterial carbon dioxide partial pressure(PaCO2),oxygen saturation(SpO2),mean arterial pressure(MAP),heart rate(HR),respiratory rate(RR),peak expiratory flow(PEF),forced expiratory volume in the first second(FEV1),and the ratio of FEV1 to forced vital capacity(FEV1/FVC)before and 2 days after treatment were compared between the two groups. The incidence of complications during treatment and 28-day mortality were also statistically analyzed. 【Results】The tracheal intubation rate within 7 days in the observation group was lower than that in the control group. The duration of non-invasive ventilation, the time for disappearance of bilateral lung rales, the time for complete disappearance of pulmonary edema, and the hospital stay were all significantly shorter in the observation group than in the control group(P<0.05).After treatment,PaO2,SpO2,MAP,PEF,FEV1,and FEV1/FVC in the observation group were significantly higher than those in the control group(P<0.05),while PaCO2, HR, and RR were significantly lower(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05);however,the 28-day mortality in the observation group was significantly lower than that in the control group(P<0.05). 【Conclusion】 HFNC can significantly improve clinical symptoms in patients with pulmonary edema, reduce the tracheal intubation rate and 28-day mortality, and has high safety.

601 The Clinical Efficacy of Modified Nice Knot Combined with Bone Plate Internal Fixation in the Treatment of Patients with Metacarpophalangeal Oblique Fractures

【Objective】 To explore the efficacy of modified Nice knot combined with bone plate internal fixation in the treatment of patients with metacarpophalangeal oblique fractures. 【Methods】A retrospective analysis was conducted on 68 patients with metacarpal oblique fractures admitted to our hospital from January 2023 to July 2024. They were divided into the control group (treated with Kirschner needle internal fixation) and the observation group (treated with modified Nice knot combined with bone plate internal fixation) according to different treatment methods, with 34 cases in each group. The perioperative indicators, Dargan functional scores and complication rates of the two groups were compared. 【Results】The operation time, hospital stay and fracture healing time of the observation group were shorter than those of the control group. The intraoperative blood loss was less than that of the control group. The Visual Analogue Scale (VAS) score 24 hours after the operation was lower than that of the control group (P<0.05). The excellent and good rate of Dargan functional score was higher than that of the control group (P<0.05). The total incidence of complications was lower than that of the control group (P<0.05). 【Conclusion】Compared with Kirschner's needle internal fixation, the modified Nice knot combined with bone plate internal fixation in the treatment of patients with metacarpophalangeal oblique fractures has obvious clinical advantages. It can significantly shorten the operation time, hospital stay and fracture healing time, reduce intraoperative blood loss, alleviate postoperative pain and promote the recovery of finger function. Moreover, it has high safety and is worthy of promotion and application.

604 Clinical Efficacy of Bevacizumab Combined with TC Regimen in the Treatment of Ovarian Cancer Patients and its Impact on Serum HE4 and CA125 Levels

【Objective】To investigate the clinical efficacy of bevacizumab combined with carboplatin and paclitaxel (TC) regimen in the treatment of ovarian cancer patients and its effect on serum levels of human epididymal protein 4 (HE4) and carbohydrate antigen 125 (CA125). 【Methods】A total of 126 ovarian cancer patients admitted to our hospital from February 2016 to July 2022 were randomly divided into an observation group (receiving bevacizumab combined with TC regimen treatment) and a control group (receiving TC regimen treatment) using a random number table method, with 63 patients in each group. The clinical efficacy, incidence of adverse reactions, immune function indicators (CD3+, CD4+, CD8+T lymphocyte levels), and serum HE4 and CA125 levels were compared between two groups. After a one-year follow-up, the survival status of the two groups was compared. 【Results】The objective remission rate and disease control rate of the observation group were significantly higher than those of the control group, and the difference was statistically significant (P<0.05). After treatment, the levels of CD3+, CD4+, and CD8+in both groups decreased (P<0.05), but there was no statistically significant difference in the above indicators between the two groups (P>0.05). After treatment, the levels of serum HE4 and CA125 in both groups decreased (P<0.05); And the serum levels of HE4 and CA125 in the observation group were lower than those in the control group (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two treatment groups within 4 cycles. After a one-year follow-up, the survival rate of the observation group was 65.08% (41/63), which was significantly different from the control group's 44.44% (28/63) (P<0.05). 【Conclusion】The combination of bevacizumab and TC regimen has significant therapeutic effects on ovarian cancer patients, significantly reducing serum HE4 and CA125 levels without increasing the impact on immune function. It is safe and reliable, and worthy of clinical promotion and application.

608 Development and Validation of a Risk Model for Post-Stroke Depression after Acute Ischemic Stroke Based on Cystatin C Level

【Objective】 To construct and validate a risk model for post-stroke depression after acute ischemic stroke (AIS) based on cystatin C (cys C) level.【Methods】 Clinical data of 102 AIS patients were collected, and the occurrence of depression during hospitalization was recorded. Factors influencing the development of depression in these patients were analyzed, and a risk nomogram for depression in AIS patients was developed and validated.【Results】 Among the 102 patients, 30 developed depression. Logistic multivariate regression analysis identified left-sided lesions, NIHSS score > 4, homocysteine (Hcy) ≥ 15 μmol/L, and cys C ≥ 1.09 mg/L as factors influencing post-AIS depression (P<0.05). Using these risk factors as predictive variables, a nomogram model was established with a total score range (including each factor) of 87 to 372 points, which were corresponding to a risk rate of 0.05 to 0.81. The validation of the nomogram model showed that a C-index was 0.811 (95%CI: 0.774-0.849), and the calibration curve for predicting depression after AIS was close to the ideal curve (P>0.05).【Conclusion】 Lesion side, NIHSS score, Hcy levels, and cys C levels are associated with post-stroke depression after AIS. The nomogram model developed based on these factors has good predictive efficacy for assessing post-stroke depression in AIS patients.

612 Prenatal Ultrasonic Features of Fetal Congenital Cystic Adenomatoid Malformation and their Impact on Fetal Prognosis

【Objective】To explore the prenatal ultrasound characteristics of Congenital Cystic Adenomatoid Malformation (CCAM) in the fetus and its influence on the prognosis of the fetus. 【Methods】A retrospective analysis was conducted on 52 fetuses with CCAM who underwent prenatal ultrasound examinations in our hospital from January 2021 to December 2023. The classification of CCAM, ultrasound characteristics, occurrence of complications and prognosis were evaluated. 【Results】Among the 52 cases of CCAM, there were 10 cases of large cystic type (type Ⅰ), 24 cases of medium cystic type (type Ⅱ), and 18 cases of microcystic type (type Ⅲ). Prenatal ultrasound features: Lesions were located on one side in 48 cases and on both sides in 4 cases. The volume ratio of type Ⅰ to type Ⅲ CCAM showed an increasing trend (P<0.05), and the ratio of lung volume to thoracic volume showed a decreasing trend (P<0.05). There was a statistically significant difference in the occurrence of polyhydramnios and fetal edema and effusion among different types of CCAM (P<0.05). The induction rate of labor in type Ⅰ - Ⅲ CCAM gradually increased. The comparison of the induction rates of labor in different subtypes of CCAM showed statistically significant differences (P<0.05).The follow-up of CCAM fetuses revealed that there was no statistically significant difference in the proportion of lesion volume reduction among different subtypes (P>0.05). The proportion of pulmonary consolidation and the proportion of those with respiratory symptoms in type Ⅰ- Ⅲ CCAM increased successively (P<0.05). 【Conclusion】Prenatal ultrasound is an effective tool for differentiating fetal CCAM and is of great value in predicting disease prognosis and improving the clinical outcome of neonates.

615 Comparison of the Therapeutic Effects of Anterior and Posterior Decompression and Fusion in the Treatment of Patients with Ossification of the Posterior Longitudinal Ligament of the Cervical Spine

【Objective】To compare the efficacy of anterior and posterior decompression and fusion in the treatment of patients with ossification of the posterior longitudinal ligament of the cervical spine (OPLL). 【Methods】A total of 97 patients with ossification of the posterior longitudinal ligament of the cervical spine admitted to the orthopedics department of our hospital from January 2021 to December 2023 were selected and randomly divided into the control group (receiving posterior decompression and fusion surgery, n=48) and the observation group (receiving anterior decompression and fusion surgery, n=49) according to the envelope lottery method. The surgery-related indicators (blood loss, operation time, postoperative drainage volume, hospital stay, hospitalization cost), functional recovery [Cervical Disability Index (NDI), Japanese Orthopaedic Association (JOA) score], C2-7 Angle, C2-7 sagittal plane vertical distance, and the occurrence of postoperative complications were compared between the two groups. 【Results】The amount of bleeding, postoperative drainage volume and hospitalization cost in the observation group were all less than those in the control group, while the operation time and hospital stay were shorter than those in the control group. One week and three months after the operation, the NDI scores of both groups were lower than those before the operation (P<0.05), and the NDI scores of the observation group was lower than those of the control group (P<0.05). One week and three months after the operation, the JOA scores of both groups were higher than those before the operation (P<0.05), and the scores of the observation group was higher than those of the control group (P<0.05). Three months after the operation, the C2-7 angles in both groups improved significantly, and the angle in the observation group was greater than that in the control group. Three months after the operation, the sagittal plane vertical distance of C2-7 in both groups was significantly shorter than that before the operation, and the distance in the observation group was shorter than that in the control group. The difference was statistically significant (P<0.05). The total incidence of complications in the observation group was significantly lower than that in the control group, and the difference was statistically significant (P<0.05). 【Conclusion】Anterior decompression-fusion is superior to posterior decompression and fusion in improving the function and structural adjustment of patients with ossification of the posterior longitudinal ligament of the cervical spine. And anterior decompression and fusion has better safety.

619 Observation of the Effect of Incremental Positive End-Expiratory Pressure Ventilation for Lung Recruitment in Patients Undergoing Thoracoscopic Lung Resection under General Anesthesia

【Objective】 This study aims to explore the effectiveness of lung recruitment using the incremental positive end-expiratory pressure (PEEP) in patients undergoing thoracoscopic lung resection under general anesthesia. 【Methods】 Seventy-eight lung cancer patients were randomly divided into an observation group and a control group, with 39 patients in each group. All patients underwent thoracoscopic lung resection under general anesthesia. The control group received controlled lung expansion for lung recruitment, whereas the observation group used the PEEP incremental method. The comparison between the two groups were based on lung atelectasis score (LUS), oxygenation index (OI), dynamic lung compliance (Cdyn), mean arterial pressure (MAP), and lung function indicators [vital capacity (VC), forced vital capacity (FVC), forced expiratory volume in one second (FEV1), FEV1/FVC and maximum voluntary ventilation (MVV)], along with the incidence of complications. 【Results】 After lung recruitment, there was no significant statistical difference in OI, Cdyn, and MAP in the observation group compared to before recruitment (P>0.05). In contrast, these measures after lung recruitment significantly decreased in the control group (P<0.05). After recruitment, the levels of OI, Cdyn, and MAP in the observation group were higher than those in the control group (P<0.05). The LUS scores of both groups at the end of the surgery (T1) were significantly lower than those at one minute after the induction of mechanical ventilation (T0) and further decreased 30 minutes after entering the anesthesia recovery room (T2) (P<0.05), with the observation group showing lower LUS scores at T1 and T2 than the control group (P<0.05). Postoperatively, VC, FVC, FEV1, FEV1/FVC, and MVV decreased in both groups compared to preoperative levels (P<0.05), with the observation group having higher values than the control group (P<0.05). The total incidence of postoperative complications in the observation group was lower than that in the control group (P<0.05). 【Conclusion】 The PEEP incremental method in thoracoscopic lung resection under general anesthesia can more rapidly restore OI, Cdyn, and MAP, increase lung aeration, speed up lung function recovery, and result in fewer postoperative complications. It is worthy of clinical promotion and application.

623 Correlation Analysis of Macrosomia with Gestational Diabetes Mellitus with Cord Blood Leptin and IGFBP-3 Levels

【Objective】To explore the correlation between the levels of leptin and insulin-like growth factor binding protein 3 (IGFBP-3) in umbilical cord blood and the delivery of macrosomia in pregnant women with gestational diabetes mellitus (GDM). 【Methods】 A total of 312 pregnant women with GDM who delivered in our hospital from April 2020 to June 2023 were selected. The pregnant women were divided into Group A (macrosomia, neonatal birth weight ≥4 kg) and Group B (non-macrosomia, neonatal birth weight normal or lower than normal weight) according to the birth weight of the newborns. The levels of leptin and IGFBP-3 in umbilical cord blood before delivery were compared between the two groups. We analyzed the influencing factors of macrosomia delivery in pregnant women with GDM to explore the correlation of macrosomia delivery in pregnant women with GDM with umbilical cord blood levels of leptin and IGFBP-3. The predictive value of umbilical cord blood leptin and IGFBP-3 levels for macrosomia delivery in pregnant women with GDM was evaluated. 【Results】The levels of IGFBP-3, umbilical cord blood leptin and fasting blood glucose in group A were all higher than those in group B (P<0.05). The results of binary Logistic regression analysis showed that higher levels of fasting blood glucose, umbilical cord blood leptin, and IGFBP-3 were risk factors for pregnant women with GDM to deliver macrosomia (P<0.05). The results of the receiver operating characteristic (ROC) curve analysis showed that the area under the curve (AUC) of the combined prediction of umbilical cord blood leptin, IGFBP-3, and fasting blood glucose for the delivery of macrosomia in pregnant women with GDM was higher than that of the single index detection (P<0.05). The results of the correlation analysis showed that umbilical cord blood leptin and IGFBP-3 were positively correlated with the delivery of macrosomia in pregnant women with GDM(P<0.05).【Conclusion】The combined detection of fasting blood glucose, umbilical cord blood leptin, and IGFBP-3 has a high predictive value for the delivery of macrosomia in pregnant women with GDM.

【Objective】To explore the correlation between serum anti-β 2 glycoprotein 1 antibody (aβ2GP1) and the stability of carotid atherosclerotic plaques in elderly patients with H-type hypertension. 【Methods】 A total of 124 elderly patients with H-type hypertension admitted to our hospital from August 2019 to October 2023 were selected. According to the results of carotid ultrasound examination, the patients were divided into the unstable plaque group (n=69) and the stable plaque group (n=55). Logistic regression analysis was used to explore the risk factors influencing the stability of carotid atherosclerotic plaques in elderly patients with H-type hypertension. The receiver operating characteristic (ROC) curve was fabricated to analyze the predictive value of serum aβ2GP1 level for unstable carotid atherosclerotic plaques in elderly patients with H-type hypertension. 【Results】 The levels of homocysteine, C-reactive protein, tumor necrosis factor-α, interleukin-1β, aβ2GP1 and IMT in the unstable plaque group were higher than those in the stable plaque group, and the difference was statistically significant (P<0.05). Logistic multifactor regression analysis showed that Hcy, aβ2GP1 all affected the elderly H-type hypertensive patients with carotid atherosclerotic plaque stability related factors (P<0.05). ROC curve analysis showed that the optimal cutoff point of serum aβ2GP1 for predicting carotid atherosclerotic unstable plaques in elderly patients with H-type hypertension was 15.24 RU/mL, with sensitivity and specificity of 72.46% and 83.64% respectively, and the area under the curve was 0.831 (95%CI: 0.753-0.892). 【Conclusion】The level of serum aβ2GP1 is related to the stability of carotid atherosclerotic plaques in elderly patients with H-type hypertension, and the level of serum aβ2GP1 can be used as an important reference indicator for predicting the stability of carotid atherosclerotic plaques in elderly patients with H-type hypertension.

630 The Clinical Effect of Lithium Carbonate Sustained-Release Tablets Combined with Magnesium Valproate Sustained-release Tablets in the Treatment of Manic Episodes of Bipolar Disorder and its Influence on the Cognitive Function of Patients

【Objective】To explore the clinical effect of lithium carbonate sustained-release tablets combined with magnesium valproate sustained-release tablets in the treatment of manic episodes of bipolar disorder and its influence on the cognitive function of patients.【Methods】One hundred patients with manic episodes of bipolar disorder admitted to our hospital from March 2022 to March 2024 were selected and divided into the control group and the observation group according to the random number table method, with 50 cases in each group. The control group was treated with lithium carbonate sustained-release tablets, while the observation group was treated with lithium carbonate sustained-release tablets combined with magnesium valproate sustained-release tablets. The clinical efficacy, cognitive function [Montreal Cognitive Assessment Scale (MoCA) score], degree of mania [Beck and Raffenson Mania Scale (BRMS) score], laboratory indicators [thyroid hormone (TH), thyroid stimulating hormone (TSH), adrenocorticotropic hormone (ACTH), cortisol (Cor)], and incidence of adverse reactions were compared between the two groups. 【Results】After treatment, the total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). After treatment, the MoCA scores of both groups were higher than those before treatment, and the BRMS scores were lower than those before treatment. Moreover, the MoCA score of the observation group was higher than that of the control group, and the BRMS score was lower than that of the control group. The differences were statistically significant (P<0.05). After treatment, the levels of serum TH, ACTH and Cor in both groups were lower than those before treatment, and the level of TSH was higher than that before treatment. The levels of serum TH, ACTH and Cor in the observation group were lower than those in the control group, and the level of TSH was higher than that in the control group. The differences were statistically significant (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the observation group and the control group (P>0.05). 【Conclusion】The combination of lithium carbonate sustained-release tablets and magnesium valproate sustained-release tablets has a good clinical efficacy in the treatment of manic episodes of bipolar disorder. It can improve cognitive function and manic symptoms, regulate neuroendocrine function, and has good safety.

633 Construction and Validation of Prediction Model for Gestational Diabetes Mellitus in Early Pregnancy

【Objective】This study aims to develop a predictive model based on clinical factors and selected biochemical markers for the early risk assessment of gestational diabetes mellitus (GDM). 【Methods】The data of 1008 pregnant women admitted to the South Campus of Shanghai Sixth People's Hospital, which is affiliated to Shanghai Jiaotong University, from March 2021 to June 2022 were retrospectively analyzed, including 426 cases of gestational diabetes mellitus (the GDM group) and 582 cases of healthy pregnant women (the NGT group). The clinical data and related indicators in early pregnancy of all pregnant women were collected, multivariate Logistic regression analysis was used to explore the risk factors associated with GDM, and the prediction model was constructed and verified. 【Results】Multivariate logistic regression analysis showed that the age, fasting blood glucose, glycosylated hemoglobin, triglyceride, white blood cell count, hemoglobin, and neutrophil lymphocyte ratio of pregnant women were the risk factors for diabetes in pregnancy (P<0.05). Based on these information, the column chart model was constructed;The area under the ROC curve (AUC) of this model was 0.882 (95%CI:0.857-0.907), which indicated that the model had good predictive performance. The Hosmer-Lemeshow test results showed χ2=7.147 and P=0.521, indicating good consistency between the predicted probability of GDM occurrence by the predictive model and the actual diagnosis probability. 【Conclusion】The model constructed in this study contains 7 independent variables to predict the occurrence of diabetes in the early pregnancy, which is helpful for clinical screening of high-risk GMD pregnant women, early intervention to reduce the occurrence of diabetes in pregnancy and reducing adverse complications.

638 Comparison of the Effects of Sertraline and Paroxetine on Neurotransmitter Power and Depressive Mood in Patients with Post-Stroke Depression

【Objective】To compare the effects of sertraline and paroxetine on neurotransmitter power and depression in patients with post-stroke depression.【Methods】A total of 140 patients with post-stroke depression were divided into two groups according to different treatment methods, with 70 cases in each group. Both groups were given conventional symptomatic treatment, while group A was also given sertraline treatment, while group B was given paroxetine treatment. The changes in Hamilton Depression Rating Scale-24 (HAMD) score, National Institutes of Health Stroke Scale (NIHSS) score, Barthel Index (BI) score, Asberg Antidepressant Side Effects Rating Scale (SERS) score before and after treatment were compared between the two groups. The changes in gamma-aminobutyric acid (GABA), acetylcholine (Ach), glutamate (Glu), norepinephrine (NE), interleukin-2 (IL-2), tumor necrosis factor-α (TNF-α) levels before and after treatment, as well as the efficacy of the two groups, were also compared.【Results】The total effective rate of group A was 92.86% (65/70), which was higher than that of group B (80.00%,56/70) (P<0.05). After 4 weeks and 8 weeks of treatment, the HAMD scores of the two groups gradually decreased (P<0.05), and the HAMD score of group A was lower than that of group B (P<0.05). After 4 weeks and 8 weeks of treatment, the levels of GABA, Ach, Glu, NE, and BI scores gradually increased in both groups (P<0.05), while the GABA, Ach, Glu, NE, and BI scores in group A were higher than those in group B (P<0.05). After 4 weeks and 8 weeks of treatment, the IL-2, TNF-α, and NIHSS scores gradually decreased in both groups (P<0.05), and the IL-2, TNF-α, and NIHSS scores in group A were lower than those in group B (P<0.05). The SERS scores of group A were lower than those of group B after 4 weeks and 8 weeks of treatment (P<0.05).【Conclusion】Sertraline treatment for post-stroke depression can effectively improve patients' negative emotions, increase neurotransmitter power, reduce neural functional damage, and have high safety.

642 Efficacy of Laparoscopic Roux-en-Y Distal Anastomosis for Radical Gastrectomy of T1-4aN0-3M0 Gastric Cancer

【Objective】To explore the therapeutic efficacy and impact on the quality of life of laparoscopic non-disruptive Roux-en-Y distal anastomosis for radical gastrectomy in treating patients with T14aN03M0 gastric cancer. 【Methods】Eighty patients with T1-4aN0-3M0 gastric cancer were divided into the non-disruptive group (43 patients) and the traditional group (37 patients) based on the surgical approach. The non-disruptive group underwent laparoscopic non-disruptive Roux-en-Y distal anastomosis for radical gastrectomy, while the traditional group underwent traditional laparoscopic Roux-en-Y distal anastomosis for radical gastrectomy. The surgical indicators, pre- and post-operative gastric fluid pH values, self-rated quality of life scores, and incidence of complications were compared between the two groups. 【Results】There was no significant difference in postoperative hospital stay, time to postoperative anal exhaust, preoperative gastric fluid pH value, and time to liquid food intake after surgery, postoperative day 1 gastric fluid pH value, and total complication rate between the two groups (P>0.05). The non-disruptive group had shorter time to semi-liquid abnormal food intake and shorter operative time if compared to the traditional group (P<0.05). The non-disruptive group had less intraoperative blood loss than the traditional group (P<0.05). Both groups showed a significant increase in postoperative day 1 and day 5 gastric fluid pH values compared to preoperative level, while the non-disruptive group had even higher pH value on postoperative day 5 than the traditional group (P<0.05). Both groups showed a significant improvement in postoperative appetite, mental state, family understanding and cooperation, sleep quality, daily life, and cancer awareness compared to before surgery (P<0.05), and the non-disruptive group had higher postoperative scores than the traditional group (P<0.05). 【Conclusion】 Compared to traditional laparoscopic Roux-en-Y anastomosis for radical distal gastrectomy, the use of non-disruptive Roux-en-Y anastomosis for radical distal gastrectomy in treating patients with T14aN03M0 gastric cancer significantly shortens the operative time, reduces intraoperative blood loss, and significantly improves patients' quality of life, warranting its clinical promotion and use.

645 Expression of HER2 and WT-1 in Epithelial Ovarian Cancer and their Impact on Disease Prognosis

【Objective】This study aims to explore the expression of Human Epidermal Growth Factor Receptor 2 (HER2) and Wilms' Tumor 1 (WT-1) in epithelial ovarian cancer (EOC) and their impact on patient prognosis.【Methods】 Clinical and pathological data of 86 patients diagnosed with primary EOC, who underwent surgical treatment at Shenmu City Hospital, were collected. The expression of HER2 and WT-1 in cancerous and adjacent non-cancerous tissues was analyzed, along with their relationship to clinical pathological characteristics and patient prognosis. 【Results】 The positive expression rates of HER2 and WT-1 in cancerous tissues were significantly higher than those in adjacent non-cancerous tissues (P<0.05). There were no statistically significant differences in the positive expression rates of HER2 and WT-1 across different ages, histological types, and lymph node metastasis status (P>0.05). However, differences in their expression rates were statistically significant when comparing FIGO stages and differentiation degrees (P<0.05). Kaplan-Meier survival analysis showed a statistically significant difference in survival rates between the two groups (P<0.05).【Conclusion】 There are significant differences in the positive expression rates of HER2 and WT-1 among EOC patients with different tumor stages and degrees of differentiation in both cancerous and adjacent non-cancerous tissues. Moreover, patients with positive expression of HER2 and WT-1 have a worse prognosis.

649 Clinical Efficacy of Periareolar Incision Surgery for Benign Breast Tumors and its Effect on Postoperative Pain Level and Sensory Nerve Function

【Objective】To investigate the effects of periareolar incision surgery on postoperative therapeutic efficacy, pain levels in patients with benign breast tumors.【Methods】A total of 120 patients with benign breast tumors, admitted to Anyang Traditional Chinese Medicine Hospital between May 2021 and July 2024, were randomly assigned into two groups using a random number table. The control group (57 patients) underwent conventional open surgery, while the observation group (63 patients) received periareolar incision surgery. Postoperative therapeutic efficacy, pain level (visual analog scale, VAS), areolar sensory nerve function, and complication rates were compared between the two groups.【Results】The observation group achieved a therapeutic efficacy, markedly surpassing the control group's efficacy(P<0.05). The VAS scores for both groups at 24 hours and 48 hours postoperatively were lower than those at 12 hours, with the 48-hour VAS score being significantly lower than at 24 hours (P<0.05). The observation group had lower VAS scores at all time points postoperatively compared to the control group (P<0.05).Compared to the control group, the observation group demonstrated better recovery of areolar sensory nerve function at 1 day and 3 months postoperatively (P<0.05). The observation group experienced notably fewer complications compared to the control group (P<0.05).【Conclusion】Periareolar incision surgery significantly improves therapeutic efficacy, reduces postoperative pain, promotes recovery of areolar sensory nerve function, and has a lower complication rate compared to conventional open surgery in patients with benign breast tumors.

【Objective】To explore the application value of serum 25-hydroxyvitamin D [25(OH)D] combined with clinical data in the prognostic assessment of patients with Cerebral Ischemic Stroke (CIS). 【Methods】 Clinical data of 1000 CIS patients treated in our hospital from January 2018 to December 2019 were collected. Based on their prognosis, patients were divided into the good prognosis group (survival time ≥ 3 months, n=510) and the poor prognosis group (survival time < 3 months, n=490). The basic data of the two groups were compared. The value of 25(OH)D level in predicting poor prognosis in CIS patients was analyzed using the Receiver Operating Characteristic (ROC) curve. The risk factors for poor prognosis in CIS patients were identified through a Logistic regression analysis model. 【Results】 The proportion of patients with a Body Mass Index (BMI) > 23.99 kg/m2, onset to hospital admission time > 24 h, smoking history, infarct area ≥ 1.5 cm2, hyperglycemia, high body temperature, National Institutes of Health Stroke Scale (NIHSS) score ≥ 21, and Combined with hypertension was higher in the poor prognosis group than in the good prognosis group (P<0.05). Serum 25(OH)D levels were lower in the poor prognosis group (P<0.05). ROC analysis showed that the area under the curve (AUC) of 25(OH)D for predicting poor prognosis was 0.680, with 95%CI of 0.586-0.775, the sensitivity of 0.450, and the specificity of 0.700 (P<0.05). Multifactorial Logistic results indicated that BMI > 23.99 kg/m2, onset to admission time > 24 h, smoking history, infarct area ≥ 1.5 cm2, hyperglycemia, NIHSS score ≥ 21, high body temperature, hypertension, and 25(OH)D < 24.500 ng/mL were risk factors for poor prognosis (P<0.05). 【Conclusion】 The poor prognosis of CIS patients is closely related to BMI, onset to admission time, smoking history, infarct area, hyperglycemia, high body temperature, hypertension, and 25(OH)D level. The level of 25(OH)D can predict the prognosis of this disease.

656 Correlation between T-Wave Changes in Lead aVL on Electrocardiogram and Degree of Coronary Artery Stenosis in Patients with Stable Angina Pectoris

【Objective】To investigate the correlation between T-wave changes in lead aVL on electrocardiogram (ECG) and the degree of coronary artery stenosis in patients with stable angina pectoris. 【Methods】 The clinical data of 90 patients with stable angina pectoris admitted to our hospital from May 2018 to May 2021 were retrospectively analyzed. Based on prognosis, the patients were divided into a good prognosis group (n=60) and a poor prognosis group (n=30). General data and serum markers [cardiac troponin I (cTnI), high-sensitivity C-reactive protein (hs-CRP), brain natriuretic peptide (BNP), and creatine kinase-MB (CK-MB)] were compared between the two groups. The degree of coronary artery stenosis was compared among patients with different T-wave changes in lead aVL. Multivariate logistic regression analysis was used to identify independent risk factors for poor prognosis. 【Results】The proportions of T-wave changes in lead aVL, ST-segment elevation, pathological Q waves, and coronary artery stenosis were significantly higher in the poor prognosis group than in the good prognosis group (P<0.05). Logistic regression analysis showed that T-wave changes in lead aVL, ST-segment elevation, pathological Q waves, and coronary artery stenosis were all risk factors for poor prognosis in patients with stable angina pectoris (P<0.05). Based on T-wave changes in lead aVL, patients were further divided into a T-wave change group and a no T-wave change group. The degree of mid-segment stenosis in the left anterior descending artery was significantly higher in the T-wave change group than in the no T-wave change group (P<0.05).【Conclusion】 T-wave changes in lead aVL, ST-segment elevation, pathological Q waves, and coronary artery stenosis are risk factors for poor prognosis in patients with stable angina pectoris. Moreover, T-wave changes in lead aVL can predict the degree of mid-segment stenosis in the left anterior descending artery.

659 Effect of Tofacitinib on Peripheral Lymphocyte Subpopulations in Patients with Rheumatoid Arthritis Complicated with Interstitial Lung Disease

【Objective】To explore the clinical efficacy of tofacitinib in the treatment of patients with rheumatoid arthritis (RA) complicated with interstitial lung disease (ILD) and its influence on peripheral lymphocyte subsets of these patients. 【Methods】A total of 120 patients with RA complicated with ILD diagnosed and treated in our hospital from March 2020 to March 2023 were selected and divided into the control group (treated with methylprednisolone tablets) and the observation group (treated with tofacitinib combined with methylprednisolone tablets) according to different treatment regimens, with 60 cases in each group. Improvement of clinical symptoms (morning stiffness of the joints, joint swelling and joint pain), laboratory indicators, pulmonary function indicators and peripheral lymphocyte subsets were compared between the two groups. 【Results】After the treatment, the improvement of clinical symptoms in the observation group was better than those in the control group. After treatment, the levels of tumor necrosis factor-α (TNF-α), C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), D-dimer (D-D), rheumatoid factor (RF), and salivary liquid sugar chain antigen (KL-6) in the observation group were significantly lower than those in the control group. After treatment, the vital capacity (VC), total lung capacity (TLC), forced vital capacity (FVC), and the lung's diffusion capacity for carbon monoxide (DLCO) in the observation group were significantly higher than those in the control group. After treatment, CD4+ and CD4+/CD8+ in both groups significantly increased, while CD8+ significantly decreased. Moreover, CD4+ and CD4+/CD8+ in the observation group were higher than those in the control group, while CD8+ was lower than that in the control group. The differences were statistically significant (P<0.05). 【Conclusion】Tofacitinib in the treatment of patients with RA complicated with ILD can effectively improve the clinical symptoms, laboratory indicators and pulmonary function indicators of patients, and enhance the immune function.

663 Efficacy of Huangqi Guizhi Wuwu Decoction Combined with Indirect Moxibustion and its Effect on T Lymphocyte Function in Treatment of Patients with Non-Small Cell Lung Cancer in Patients

【Objective】To investigate the efficacy of Huangqi Guizhi Wuwu Decoction combined with indirect moxibustion and its impact on T lymphocyte function in the treatment of patients non-small cell lung cancer (NSCLC).【Methods】A total of 106 patients with NSCLC were randomly divided into the control group and the observation group, with 53 patients in each group. The control group received conventional treatment, while the observation group was treated with Huangqi Guizhi Wuwu Decoction combined with indirect moxibustion on top of the conventional treatment. The efficacy, T lymphocyte function before and after treatment, traditional Chinese medicine syndrome scores, KPS scores, vascular endothelial growth factor (VEGF) levels, and the incidence of adverse reactions were compared between the two groups.【Results】 After treatment, the total effective rate in the observation group was higher than that in the control group (P<0.05). The observation group had significantly higher levels of CD3+, CD4+, and CD4+/CD8+ ratios, and lower CD8+ level compared to the control group (P<0.05). The TCM syndrome scores and VEGF level in the observation group were lower than those in the control group, while the KPS score in the observation group was higher than that in the control group (P<0.05). The total incidence of adverse reactions in the observation group was lower than that in the control group (P<0.05).【Conclusion】Huangqi Guizhi Wuwu Decoction combined with indirect moxibustion significantly improves the efficacy of treatment and T lymphocyte function in NSCLC patients, alleviates traditional Chinese medicine syndrome scores, enhances KPS score, reduces VEGF level, and lowers the incidence of adverse reactions.

666 The Clinical Application Value of DTI and DCE-MRI Parameters in the Diagnosis of Prostate Cancer

【Objective】To explore the clinical application value of diffusion tensor imaging (DTI) and dynamic contrast-enhanced resonance scanning (DCE-MRI) parameters alone and in combination in the diagnosis of prostate cancer. 【Methods】A total of 89 patients with suspected prostate cancer who received urological treatment in our hospital from January 2019 to January 2023 were selected as the research subjects. According to the pathological examination results, the patients were divided into the benign prostatic hyperplasia group (BPH group) (n=57) and the prostate cancer group (n=32). The DTI parameters [manifestation diffusion coefficient (ADC), partial anisotropy value (FA)] and DCE-MRI parameters [volume transport constant (Ktrans), rate constant (Kep), and extracellular volume fraction outside the blood vessels (Ve)] of the two groups were compared. The receiver operating characteristic (ROC) curve was used to analyze the value of DTI and DCE-MRI parameters alone in predicting prostate cancer, and the Kappa test was used for consistency analysis. 【Results】 The pathological examination results of 89 patients showed that 57 cases had BPH, among which 27 cases had mild benign prostatic hyperplasia, 20 cases had moderate hyperplasia, and 10 cases had severe hyperplasia. There were 32 cases of prostate cancer, including 27 cases of well-differentiated adenocarcinoma, 3 cases of poorly differentiated adenocarcinoma and 2 cases of undifferentiated carcinoma. The ADC in the prostate cancer group was lower than that in the BPH group, and the FA was higher than that in the BPH group. The difference was statistically significant (P<0.05). Both Ktrans and Kep in the prostate cancer group were higher than those in the BPH group, and the difference was statistically significant (P<0.05). There was no statistically significant difference in Ve between the two groups (P>0.05). ROC curve analysis showed that ADC、FA、Ktrans、Kep both DTI and DCE-MRI parameters could be used in the prediction of prostate cancer, and the areas under the curves were 0.875, 0.916, 0.878, and 0.899 respectively (P<0.05). Consistency analysis showed that the combined differentiation of prostate cancer by DTI and DCE-MRI parameters had an accuracy rate of 93.26%, a sensitivity of 92.98%, a specificity of 93.75%, a positive predictive value of 96.36%, a negative predictive value of 88.24%, and Kappa =0.856.【Conclusion】Both DTI and DCE-MRI parameters can be used to distinguish prostate cancer. The sensitivity is higher when used in combination. The combined diagnosis of multiple parameters can further improve the objectivity and accuracy of the diagnostic results and reduce the occurrence of missed diagnoses and misdiagnoses, which is worthy of clinical reference.

670 Correlation Analysis of Blood Biochemical Indicators and Kidney Injury in Patients with Diabetes and Hypertension

【Objective】 To explore the correlation between blood biochemical indicators and kidney injury in patients with diabetes combined with hypertension.【Methods】 A total of 320 patients with diabetes combined with hypertension were divided into a non-injury group (152 cases) and a kidney injury group (168 cases) based on the presence or absence of kidney injury. All patients underwent relevant blood biochemical tests, and the correlation between various indicators and the occurrence of kidney injury was analyzed. Multi-factor logistic regression was used to identify the influencing factors.【Results】 There was no significant difference in fasting blood glucose (FBG), 2-hour postprandial blood glucose (2hPG), total cholesterol (TC), triglycerides (TG), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) between the two groups (P>0.05). The levels of glycated hemoglobin (HbA1c), cystatin C (cys C), serum creatinine (Scr), and serum β2-microglobulin (β2-MG) in the kidney injury group were higher than those in the non-injury group (P<0.05). Serum HbA1c, cys C, Scr, and β2-MG levels were positively correlated with the occurrence of kidney injury in patients with diabetes combined with hypertension (P<0.05). Logistic regression analysis showed that HbA1c, cys C, Scr, and β2-MG levels were important influencing factors for the occurrence of kidney injury (OR>1, P<0.05).【Conclusion】 Serum levels of HbA1c, cys C, Scr, and β2-MG are significantly positively correlated with the occurrence of kidney injury in patients with diabetes combined with hypertension, and these indicators are important influencing factors for occurrence of kidney injury, which can provide reference for clinical practice.

673 Analysis of the Efficacy and Safety of Perampanone Tablets in the Treatment of Refractory Epilepsy in Adults

【Objective】To explore the clinical efficacy and safety of Perampanone in the treatment of refractory epilepsy in adults. 【Methods】A retrospective analysis was conducted on the clinical data of 90 patients with refractory epilepsy admitted to our hospital from January 2018 to March 2024. The patients were divided into the observation group (combined with Perampanone on the basis of conventional antiepileptic drugs, n=49) and the control group (combined with gabapentin on the basis of conventional antiepileptic drugs, n=41) according to different treatment methods. The clinical efficacy, cognitive function, neurological function and changes of cranial nerve-related factors before and after treatment of the two groups were compared, and the occurrence of adverse drug reactions during the treatment period of the two groups was statistically analyzed. 【Results】 The total effective rate of the observation group was 69.39% (34/49), which was higher than that of the control group (48.78%, 20/41), and the difference was statistically significant (P<0.05). After treatment, the MMSE scores of both groups were higher than those before treatment, and the score of the observation group was higher than that of the control group. The difference was statistically significant (P<0.05). The differences in latency and amplitude before and after treatment in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.05). The differences in serum BDNF, NSE and S100β before and after treatment in the observation group were all higher than those in the control group, and the differences were statistically significant (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】 Perampanone has a good clinical efficacy in the treatment of refractory epilepsy in adults. It can improve the cognitive function and neurological function of patients, regulate the levels of cranial nerve-related factors, and has good safety.

676 Observation of Blood Glucose, TNF-α, CRP and PCT Levels in Patients with Acute Craniocerebral Injury of Different Severity

【Objective】To investigate the levels of fasting blood glucose (FPG), glycosylated hemoglobin a1c (HbA1c), tumor necrosis factor-α (TNF-α), C-reactive protein (CRP) and procalcitonin (PCT) in patients with acute craniocerebral injury (ABI) of different severity. 【Methods】A total of 139 ABI patients admitted to our hospital from January 2020 to June 2022 were selected. According to the Glasgow coma score (GCS) within 24 hours of admission, patients were divided into the mild group (GCS of 13-15,n=43), the moderate group (GCS of 9-12, n=9) and the severe group (GCS of 3-8,n=50). At the same time period, 40 healthy volunteers were selected as the control group. The differences of FPG, HbA1c, TNF-α, CRP, PCT among the four groups were compared. Spearman rank correlation analysis was used to explore the relationship of the severity of the disease with levels of FPG, HbA1c, TNF-α, CRP, and PCT in ABI patients. 【Results】The levels of FPG and HbA1c in the control group were lower than those in the mild, moderate and severe groups (P<0.05); FPG and HbA1c levels in the mild group were lower than those in the moderate and severe groups (P<0.05); And FPG and HbA1c levels in the moderate group were lower than those in the severe group (P<0.05). TNF-α, CRP and PCT levels in the control group were lower than those in mild, moderate and severe groups (P<0.05); The levels of TNF-α, CRP and PCT in the mild group were lower than those in the moderate and severe group (P<0.05); And TNF-α, CRP and PCT levels in the moderate group were lower than those in the severe group (P<0.05). Spearman correlation showed that FPG, HbA1c, TNF-α, CRP, and PCT were positively correlated with the severity of ABI (P<0.05). 【Conclusion】Compared to healthy people, the levels of FPG, HbA1c, TNF-α, CRP and PCT in ABI patients are elevated. And FPG, HbA1c, TNF-α, CRP and PCT levels are positively correlated with the severity of the disease, which is worthy of clinical attention.
