Clinical Effect of Bronchoalveolar Lavage on Children with Severe Pneumonia

【Objective】To investigate the clinical effect of bronchoalveolar lavage in the treatment of children with severe pneumonia. 【Methods】 A total of 77 children with severe pneumonia treated and treated in our hospital from December 2019 to March 2022 were selected and divided into observation group (receiving bronchoalveolar lavage, n=40) and control group (not receiving bronchoalveolar lavage, n=37) according to whether the guardians of the children agreed to receive bronchoalveolar lavage. The clinical efficacy, the time of symptom disappearance and the intensity of antibiotic use were compared between the two groups after treatment, and the blood gas indexes before and after bronchoalveolar lavage in the observation group were compared. 【Results】The total effective rate of observation group was 92.50%, which was significantly higher than that of the control group 72.97%(P<0.05). ) were significantly increased and partial pressure of carbon dioxide in arterial blood(PaCO

) was significantly decreased in the observation group, with statistical significance (P<0.05). The time of cough and sputum improvement, fever remission and hospital stay in the observation group were shorter than those in the control group (P<0.05). After 7 and 10 days of treatment, the antibiotic use intensity in the observation group was significantly lower than that in the control group (P<0.05). 【Conclusion】Bronchoalveolar lavage in children with severe pneumonia in the early stage of clinical treatment, the clinical effect is remarkable, the symptom improvement time is shortened, the blood gas index is significantly improved, and the intensity of antibiotic use is reduced. It is recommended to treat children with severe pneumonia in the early stage of bronchoalveolar lavage.

Study on the Relationship of Quality of Life with Sense of Life Meaning and Attitudes toward Death in Elderly Patients with Liver Cancer

【Objective】 To explore the impact of the sense of life meaning and attitudes toward death on the quality of life of elderly patients with liver cancer. 【Methods】 A total of 97 elderly liver cancer patients were surveyed using a general information questionnaire, the Quality of Life Measurement Scale for Liver Cancer Patients (QOL-LC), the Chinese Version of the Meaning of Life Questionnaire (C-MLQ), and the Death Attitude Profile-Revised (DAP-R). Correlation analyses were performed on the patients' quality of life, sense of life meaning, and attitudes toward death. 【Results】 The total quality of life score for the 97 patients was (121.14±24.87), with a sense of life meaning score of (40.76±8.04). The scores for acceptance of death in the natural dimension were (15.68±3.49), avoidance dimension (15.47±3.74), escape acceptance dimension (14.49±2.87), fear dimension (13.29±3.61), and approach acceptance dimension (12.31±3.02). The assessment of life meaning in elderly liver cancer patients showed a significant positive correlation with quality of life (P<0.05). The scores of the four dimensions (avoidance, escape, fear, and approach acceptance) regarding attitudes toward death were significantly negatively correlated with quality of life (P<0.05). Conversely, the score for natural acceptance of death was significantly positively correlated with quality of life (P<0.05).【Conclusion】 The quality of life in elderly liver cancer patients is positively correlated with their sense of life meaning and natural acceptance of death, while it is negatively correlated with negative attitudes toward death, such as avoidance and fear. Clinical treatment should strengthen attention to and intervention in patients' psychological states to improve their quality of life.

The Effect of Repetitive Transcranial Magnetic Stimulation Combined with Mirror Therapy on Hand Function Rehabilitation in Stroke Patients

【Objective】To investigate the effect of repetitive transcranial magnetic stimulation (rTMS) combined with mirror therapy (MT) on hand function rehabilitation in stroke patients.【Methods】Eighty stroke patients in the recovery period were randomly divided into a control group and an observation group, with 40 patients in each group. The control group received conventional hand function rehabilitation training, while the observation group received rTMS combined with MT mirror therapy. After one month of treatment, the electroencephalogram (EEG) indices [δ/α value (DAR), brain symmetry index (BSI)], upper limb motor function scores (Fugl-Meyer, FMA), hand function scores (using the Brunnstrom evaluation scale and the Modified Ashworth Spasticity Scale), daily living abilities (Modified Barthel Index), and quality of life were compared between the two groups.【Results】The levels of DAR and BSI in the observation group were significantly lower than those in the control group after treatment (P<0.05). The differences in Brunnstrom evaluation stages and Ashworth spasticity grades between the two groups after treatment were statistically significant (P<0.05). The total FMA scores for the wrist, hand, and upper limbs, as well as the Barthel scores and various quality of life scores after treatment, were higher in the observation group than in the control group (P<0.05).【Conclusion】The rTMS combined with MT can significantly improve EEG indices in stroke patients, promote hand function rehabilitation, and enhance daily living abilities and quality of life.

Correlation Analysis of Serum HMGB1 and ICAM-1 Levels with APACHE Ⅱ and BISAP Scores in Patients with Acute Pancreatitis

【Objective】To investigate the correlation between serum levels of high mobility group 1 protein (HMGB1) and intercellular adhesion molecule-1 (ICAM-1) in patients with acute pancreatitis with acute physiology and chronic health status evaluation Ⅱ (APACHE Ⅱ) and bedside index of acute pancreatitis severity (BISAP) scores. 【Methods】Eighty patients with acute pancreatitis admitted to our hospital from April 2020 to April 2022 were selected and divided into the mild group (n=39), moderate group (n=23), and severe group (n=18) according to the severity of their condition. Another 80 healthy volunteers who underwent physical examinations in our hospital were selected as the healthy group. Levels of serum HMGB1, ICAM-1, C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), as well as APACHE Ⅱ and BISAP scores were compared among the four groups. The correlation of serum HMGB1 and ICAM-1 levels with CRP, TNF-α, IL-6, APACHE Ⅱ score, and BISAP score was analyzed among the four groups. 【Results】The APACHE Ⅱ scores of the four groups were shown in the order of the healthy group<the mild group<the moderate group<the severe group; And the difference was statistically significant (P<0.05). The BISAP scores of the three groups of patients showed an increasing trend with the mild group<the moderate group<the severe group, and the difference was statistically significant (P<0.05). The comparison of serum levels of HMGB1, ICAM-1, CRP, TNF-α, and IL-6 in the four groups showed that the healthy group was least followed by the order of the mild group<the moderate group < the severe group, with statistically significant differences (P<0.05). Pearson correlation analysis showed that serum levels of HMGB1 and ICAM-1 were positively correlated with serum levels of CRP, TNF-α, IL-6, as well as APACHE Ⅱ score and BISAP score (P<0.05). 【Conclusion】The levels of serum HMGB1 and ICAM-1 are correlated with the severity of acute pancreatitis in patients. Clinical physicians can evaluate the severity of patients' condition by detecting the levels of serum HMGB1 and ICAM-1.

CT Imaging Features of Spontaneous Intracerebral Hemorrhage and its Value in Predicting Early Hematoma Enlargement

【Objective】To investigate the CT imaging features of hematoma enlargement in early stage of spontaneous intracerebral hemorrhage and its value in predicting early hematoma enlargement. 【Methods】Forty-two patients with spontaneous intracerebral hemorrhage diagnosed by head CT in two hospitals from February 2023 to February 2024 were selected and divided into expanded hematoma group (n=28) and unexpanded hematoma group (n=14) according to the changes of early hematoma. The imaging features of early expanded hematoma in patients with spontaneous intracerebral hemorrhage and the influencing factors of early expanded hematoma in patients with spontaneous intracerebral hemorrhage were analyzed. 【Results】The detection rate of island sign and mixed sign in patients with enlarged hematoma was significantly higher than that in the group without enlarged hematoma (P<0.05). Logistic regression analysis showed that island sign, mixed sign, island sign and mixed sign were independent predictors of early hematoma enlargement in patients with spontaneous intracerebral hemorrhage (P<0.05). The sensitivity and specificity of island sign to predict early hematoma enlargement were 53.57% and 85.71% respectively. The sensitivity and specificity of mixed signs in predicting early hematoma enlargement were 85.71%. The sensitivity and specificity of island sign and mixed sign were 89.29% and 92.86% respectively. The sensitivity and specificity of both island sign and mixed sign were higher than those of island sign and mixed sign in predicting early hematoma enlargement. 【Conclusion】The prediction value of island sign and mixed sign is higher for early hematoma enlargement in patients with spontaneous intracerebral hemorrhage, but the prediction value of island sign and mixed sign is higher, which can provide important reference for clinicians.

Correlation of Serum YKL-40, Hcy, UA, and BDNF Levels with Cognitive Function in Patients with Parkinson's Disease

【Objective】 To investigate the relationship of serum levels of human cartilage glycoprotein 39 (YKL-40), homocysteine (Hcy), uric acid (UA), and brain-derived neurotrophic factor (BDNF) with cognitive function in patients with Parkinson's disease (PD).【Methods】 A total of 113 PD patients were enrolled. According to the presence or absence of cognitive impairment, they were divided into a cognitive impairment group (Group A, n=62) and a non-cognitive impairment group (Group B, n=51). Patients in Group A were further classified into a mild group (n=34), a moderate group (n=18), and a severe group (n=10) based on the severity of cognitive impairment. The serum levels of YKL-40, Hcy, UA, and BDNF were compared between Groups A and B and analyzed among sub-groups of patients with different degrees of cognitive impairment as well. The predictive value for cognitive impairment was analyzed, and the correlations between these serum indicators and the degree of cognitive impairment were further examined. 【Results】Serum YKL-40 and Hcy levels in Group A were significantly higher than those in Group B, whereas serum UA and BDNF levels were significantly lower than those in Group B (P<0.05). Receiver operating characteristic curve analysis showed that the optimal cutoff values YKL-40,Hcy,UA,BDNF for predicting cognitive impairment were 4.015 mg/mL, 24.930 μmol/L, 296.235 μmol/L, and 11.195 ng/mL (all P<0.05). In the severe subgroup, serum YKL-40 and Hcy levels were higher and UA and BDNF levels were lower than those in both mild and moderate subgroups (P<0.05). In the moderate subgroup, serum YKL-40 and Hcy levels were higher and UA and BDNF levels were lower than those in the mild subgroup (P<0.05). Correlation analysis showed that serum YKL-40 and Hcy levels were positively correlated with the degree of cognitive impairment (P<0.05), whereas serum UA and BDNF levels were negatively correlated with the degree of cognitive impairment (P<0.05). 【Conclusion】 PD patients with cognitive impairment exhibit notably elevated serum YKL-40 and Hcy levels as well as decreased serum UA and BDNF levels, all of which are closely related to the severity of cognitive impairment.

Clinical Significance of Serum Cytokine Level Measurement in Children Infected with Influenza A and B Viruses

strain (Group B), and 30 with influenza B (Group C). Another 40 healthy children were selected as the control group. Based on the severity of their condition, the patients were divided into a mild group (78 cases) and a severe group (24 cases). Serum levels of interferon-γ(IFN-γ), tumor necrosis factor-alpha (TNF-α), interleukin-17 (IL-17), IL-10, IL-8, IL-6, IL-4, and IL-2 were compared among the groups. 【Results】 Compared to the healthy control group, serum levels of IL-6, IL-8, IL-17, TNF-α, and IFN-γ were significantly elevated in both mild and severe groups (P<0.05), with the severe group showing higher levels than the mild group (P<0.05). There were no statistically significant differences in IL-4 and IL-10 levels among the three groups (P>0.05). IL-2 levels in the severe group were significantly lower than those in the mild group and the control group (P<0.05), and the mild group had lower IL-2 level than the control group (P<0.05). Serum levels of IL-6 and TNF-α in Groups B and C were higher than in Group A (P<0.05), but there was no significant difference between Groups B and C (P>0.05). No statistically significant differences were observed in the levels of other indicators among the three groups (P>0.05). 【Conclusion】 After infection with influenza A and B viruses, children exhibited significantly elevated levels of IL-6, IL-8, IL-17, TNF-α, and IFN-γ, especially in severe cases. Measurement of cytokine levels can serve as an important indicator for assessing the severity and prognosis of influenza in children.

Observation of Therapeutic Effect of Fushen Lingjiao Decoction in the Treatment of Tanqi Yujie Schizophrenia

【Objective】To investigate the efficacy and clinical value of Fushen Lingqiao decoction in the treatment of Tanqi Yujie schizophrenia. 【Methods】A total of 80 patients with Tanqi Yujie schizophrenia who were admitted to our hospital from September 2022 to March 2023 were randomly divided into the control group (treated with conventional western medicine) and the observation group (treated with Fushen Lingqiao decoction on the top of the control group), with 40 cases in each group. The clinical efficacy, positive and negative symptom scale (PANSS) scores, Lowingston Occupational Therapy Cognitive Rating Scale (LOTCA) score, neurological function [brain-derived neurotrophic factor (BDNF), neuron-specific enolase (NSE), serotonin (5-HT)], endocrine function [melatonin (MT), thyroid-stimulating hormone (TSH), dopamine (DA)] and adverse reactions were compared between the two groups. 【Results】 The clinical effective rate of the observation group was higher than that of the control group, the difference was statistically significant (P<0.05). After treatment, the PANSS scores of the two groups were lower than those before treatment, their LOTCA scores were higher than those before treatment. The PANSS score of the observation group was lower than that of the control group, and the LOTCA score of the observation group was higher than that of the control group. The differences were statistically significant(P<0.05). After treatment, the serum levels of BDNF and 5-HT in the two groups were higher than those before treatment, and the NSE level was lower than before treatment. The levels of NSE in the observation group were higher than those in the control group, while the level of NSE in the observation group was lower than that in the control group. The differences were statistically significant (P<0.05). After treatment, the serum levels of MT, TSH and DA in the two groups were higher than those before treatment, and levels of MT, TSH and DA in the observation group was higher than those in the control group. The differences were statistically significant (P<0.05). The incidence of adverse reactions was 15.00% (6/40) in the observation group and 7.50% (3/40) in the control group, and the difference was not statistically significant (P>0.05). 【Conclusion】Fushen Lingqiao decoction combined with conventional western medicine has significant efficacy in the treatment of phlegm-qi schizophrenia, which can significantly improve clinical symptoms and cognitive function, regulate neurological function and endocrine function. It is safe treatment and is worthy of clinical promotion.

Effects of Levothyroxine Sodium on Thyroid Hormone, anti-TG and TSHR-Ab Levels in Children with Hypothyroidism

【Objective】To investigate the efficacy of levothyroxine sodium in the treatment of children with hypothyroidism (hypothyroidism) and its influence on the levels of thyroid stimulating hormone (TSH), anti-thyroglobulin antibody (anti-TG) and thyroid stimulating hormone receptor antibody (TSHR-Ab). 【Methods】A total of 160 children with hypothyroidism admitted to our hospital from April 2023 to April 2024 were randomly divided into the control group (treated with thyroid tablets) and the observation group (treated with levothyroxine sodium) according to the random drawing method, with 80 cases in each group. Application of Diffusion Weighted Imaging Imagomics in the Grading Diagnosis of Intracranial Tumors in Children

【Objective】To explore the value of diffusion weighted imaging (DWI) based imaging-omics in the differential diagnosis of low-grade gliomas (LGG) and high-grade gliomas (HGG) in children. 【Methods】A total of 92 children with intracranial tumors who underwent surgical treatment in our hospital from January 2015 to April 2021 were retrospectively analyzed and divided into low-grade gliomas group (LGG group) (grade Ⅰ-Ⅱ, n=53) and high-grade gliomas group (HGG group) (grade Ⅲ-Ⅳ, n=39) according to tumor grade. DWI examination was performed on both groups to observe their image features. MRMicro GL software was used to extract lesion features, and ITK-SNAP software was used to extract and evaluate the image omics features, which had diagnostic value and were significantly correlated with tumor grade. Receiver operating characteristic (ROC) curve was used to evaluate the value of ADC, DWI alone and ADC+DWI in the diagnosis of intracranial tumors. 【Results】108 image features were selected from ADC and DWI images for dimensionality reduction screening and model evaluation. The ADC model area under the curve (AUC) was 0.7865, and the diagnostic sensitivity and specificity were 1.0000 and 0.5000, respectively. The AUC of DWI model was 0.7135, the diagnostic sensitivity and specificity were 0.4375 and 1.0000, respectively, and the correlation coefficients were generally consistent. The AUC of ADC+DWI model was 0.9479, the correlation coefficient was consistent well, and the diagnostic efficiency was the best (P<0.05). 【Conclusion】ADC, DWI and ADC+DWI models can identify intracranial tumors in children, and ADC+DWI models have high efficiency in differentiating high and low grade gliomas, which can improve the diagnostic performance of the models and help to guide clinicians to make surgery and treatment plans.

Application Effect of Virtual Reality Technology Combined with Birth Dance Intervention in Late Pregnancy and Delivery of Primipara

【Objective】To explore the effect of virtual reality (VR) technology combined with birth dance intervention in the late pregnancy and delivery of primipara. 【Methods】A total of 80 cases of primipara who underwent antenatal examination in our hospital from June 2021 to June 2024 were selected and divided into the control group (with routine nursing intervention) and the observation group (with VR technology combined with fertility dance intervention) according to random number table method, with 40 cases in each group. The psychological status [Hospital Anxiety and Depression Scale (HADS), Pregnancy Stress Scale (PPS), Childbirth Expectation Scale] and self-efficacy [General self-efficacy Perception Scale (GSES)] scores of the two groups were compared before and after the intervention. Intrapartum conditions (delivery situation) [simplified McGill Pain Questionnaire (SF-MPQ), first stage of labor time, delivery mode, neonatal Apgar score] and adverse delivery outcomes (such as neonatal asphyxia, postpartum hemorrhage, lateral perineal incision, severe perineal laceration) were compared as well.【Results】Before intervention, there was no significant difference in psychological status and self-efficacy scores between the two groups (P>0.05). After intervention, HADS and PPS scores in the observation group were significantly lower than those in the control group, while childbirth expectation scale and GSES scores in the observation group were significantly higher than those in the control group, with statistical significance (P<0.05). During delivery, the SF-MPQ scores and the incidence of adverse delivery outcomes in the observation group were lower than those in the control group, the first stage of labor was shorter than that in the control group, and the neonatal Apgar score and vaginal delivery rate were significantly higher than those in the control group, with statistical significance (P<0.05). 【Conclusion】VR technology combined with birth dance intervention can effectively relieve the psychological pressure of first-time mothers in the third trimester of pregnancy, improve the delivery expectation and self-efficacy of pregnant women, reduce the pain of pregnant women during childbirth, shorten the labor time, improve the delivery outcome and newborn situation, and reduce the risk of adverse pregnancy outcomes, which can be applied in clinical promotion.

Efficacy of Nail-Rod Reverse Arch Folding Technique in Elderly Thoracolumbar Burst Fractures

【Objective】To investigate the effect of nail-rod reverse arch folding technique in elderly patients with thoracolumbar burst fractures. 【Methods】A total of 102 elderly patients with thoracolumbar burst fracture, who diagnosed and treated in our hospital from May 2020 to May 2023, were divided into the observation group and the control group by random number table method, with 51 cases in each group. The control group was treated with posterior decompression and internal fixation with bone grafting, while the observation group was treated with posterior decompression and internal fixation with bone grafting plus internal fixation with nail-rod reverse arch folding top. Follow-up was performed up to 6 months after surgery. The operation, healing time, pain, lumbar function, imaging parameters, spinal canal invasion and complications were compared between the two groups. 【Results】The operation time of the observation group was longer than that of the control group, the amount of intraoperative blood loss was more than that of the control group, but the fracture healing time was shorter than that of the control group, and the differences were statistically significant (P<0.05). The visual analogue scale (VAS) scores of the observation group were lower than those of the control group at 1 month and 3 months after surgery (P<0.05). The differences of Japanese Orthopaedic Association (JOA) score and Oswestry Disability Index (ODI) before and after treatment in the observation group were higher than those in the control group (P<0.05). The ratio of anterior vertebrae edge height, the ratio of posterior vertebrae edge height and the Cobb Angle difference of the injured vertebra before and after treatment in the observation group were higher than those in the control group (P<0.05). Compared to pre-operation, the invasion rates of vertebral canal in both groups decreased at 1 month and 6 months after operation. Compared with 1 month after operation, the invasion rates of vertebral canal in both groups at 6 months after operation were lower than those at 1 month after operation. At 1 month and 6 months after operation, the invasion rates of vertebral canal in observation group were lower than those in control group (P<0.05). There was no significant difference in the total incidence of complications between the two groups (P>0.05). 【Conclusion】The application of napping technique in elderly thoracolumbar burst fractures is remarkable, which can promote fracture healing, improve the function of lumbar spine and obtain satisfactory correction results.

The Influencing Factors of Lower Limb DVT Occurrence in Patients with Traumatic Intracranial Hemorrhage and the Clinical Value of Caprini Score in Predicting Lower Limb DVT Occurrence

【Objective】To investigate the influencing factors of lower limb deep venous thrombosis(DVT) in patients with traumatic intracranial hemorrhage and the clinical value of Caprini score in predicting the occurrence of lower limb DVT. 【Methods】A total of 126 patients with traumatic intracranial hemorrhage admitted to our hospital from January 2020 to May 2023 were selected and divided into DVT group (n=35) and non-DVT group (n=91) according to the incidence of DVT in lower limbs. The influence factors of lower limb DVT were analyzed by univariate and multivariate Logistic regression equations, and the clinical value of Caprini score in predicting the occurrence of lower limb DVT was analyzed by receiver operating characteristic (ROC) curve. 【Results】The proportion of Caprini score, age≥60 and use of dehydrating drugs in the DVT group was higher than that in the non-DVT group, and the hospital stay was longer than that in non-DVT group, and the GCS score was lower than that in the non-DVT group,with statistical significance (P<0.05). Logistic multivariate regression analysis showed that GCS score, Caprini score, age, use of dehydrating drugs, and length of stay were all influencing factors for DVT of lower limbs in patients with traumatic intracranial hemorrhage (P<0.05). It is of great significance to take effective measures to prevent the occurrence of DVT in the lower extremity according to the influencing factors.

Application of Early Nourishing Enteral Nutrition in Patients with Severe Cerebral Infarction

Effect of Phased Nutritional Balance Target Intervention in Critically Ill Patients with Mechanical Ventilation-Assisted Therapy

【Objective】To explore the effect of phased nutritional balance target intervention in critically ill patients receiving mechanical ventilation-assisted therapy.【Methods】A total of 86 patients with severe mechanical ventilation-assisted therapy admitted to our hospital from January 2018 to December 2020 were divided into the control group and the experimental group according to the ratio of 1∶1, with 43 cases in each group. The control group was given clinical routine nutritional support intervention, while the experimental group was given phased nutritional balance target intervention. The nutritional indexes, blood gas analysis results, inflammatory factors and clinical prognostic indicators of the two groups were compared. 【Results】After intervention, the nutritional index and blood gas analysis results of the experimental group were significantly higher than those of the control group(P<0.05). Inflammation factors, mid-intestinal nutritional intolerance rate, Acute Physiology and Chronic Health Evaluation (APACHE Ⅱ) score, lung symptom disappear time, mechanical ventilation time, hospital stay and prognostic indicators in the experimental group were significantly lower than those in the control group. The differences were statistically significant (P<0.05). 【Conclusion】The use of phased nutritional balance target intervention for critically ill patients receiving mechanical ventilation-assisted therapy can significantly improve their nutritional status, reduce the level of inflammation and improve the prognosis of patients.

Efficacy of Transcranial Magnetic Stimulation Combined with Whole-Body Vibration Training in the Treatment of Swallowing, Cognitive, and Lower Limb Function in Stroke Patients

【Objective】 To explore the effects of transcranial magnetic stimulation (TMS) combined with whole-body vibration training (WBVT) on swallowing, cognitive, and lower limb function in stroke patients.【Methods】 A total of 126 stroke patients were randomly divided into two groups: the control group (63 patients) and the observation group (63 patients). Patients in the control group received conventional treatment, while those in the observation group received TMS combined with WBVT on top of conventional therapy. After 4 weeks of treatment, the differences in swallowing function [Standardized Swallowing Assessment (SSA), Penetration-Aspiration Scale (PAS)], cognitive function [Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA)], and lower limb function [Berg Balance Scale (BBS), Holden Ambulation Index (FAC)] and levels of serum albumin (ALB), hemoglobin (Hb), homocysteine (Hcy), and VILIP-1 were compared between the two groups.【Results】After treatment, swallowing function, cognitive function, and lower limb function significantly improved in both groups (P<0.05). The observation group showed significantly lower SSA and PAS scores and significantly higher scores of MMSE, MoCA, BBS, and FAC compared to the control group (P<0.05). Serum ALB and Hb levels in the observation group were significantly higher than those in the control group, while Hcy and VILIP-1 levels in the observation group were significantly lower than those in the control group (P<0.05).【Conclusion】TMS combined with WBVT can effectively improve swallowing function, cognitive function, and lower limb function in stroke patients, thereby enhancing their quality of life.

Effect of Nicodil on Cardiac Function and Renal Function in Patients with Acute ST Segment Elevation Myocardial Infarction Undergoing Emergency Percutaneous Coronary Intervention

【Objective】To investigate the effects of nicodil on cardiac function and renal function in patients with acute ST elevation myocardial infarction (STEMI) after emergency percutaneous coronary intervention (PCI). 【Methods】The clinical data of 160 patients with acute STEMI who underwent emergency PCI in Zhenjiang First People's Hospital from November 2020 to August 2022 were retrospectively analyzed, and they were divided into observation group (conventional treatment + nicodil, n=89) and control group (conventional treatment, n=71) according to treatment methods. ST segment regression, echocardiographic parameters [left ventricular ejection fraction (LVEF), left ventricular end-diastolic diameter (LVDD), left ventricular end-systolic diameter (LVDS)] and renal function indicators (serum creatinine, urea nitrogen, uric acid) were compared between the two groups. 【Results】The proportion of ST segment regression rate ≥ 50% in the control group 2 h after PCI was lower than that in the observation group, and the difference was statistically significant (P<0.05). At 1 week and 1 month after surgery, LVDD and LVEF in 2 groups were higher than 1 day after surgery, the difference was statistically significant (P<0.05), while LVDS in 2 groups was not statistically significant compared with 1 day after surgery (P>0.05). There was no significant difference in LVDD, LVEF and LVDS between the observation group at 1 week and 1 month after surgery (P>0.05). The level of urea nitrogen 2 days after operation was lower than that before operation, the difference was statistically significant (P<0.05), but there was no statistically significant difference in serum creatinine and uric acid levels between the observation group and before operation (P>0.05).There were no significant differences in the levels of urea nitrogen, creatinine and uric acid in patients with anterior infarction in the observation group (P>0.05). The levels of urea nitrogen and uric acid 2 days after operation in patients with inferior infarction were lower than those before operation, and the differences were statistically significant (P<0.05). There were significant differences in the levels of creatinine and uric acid in patients with anterior infarction in the control group (P<0.05). The levels of urea nitrogen 2 days after operation in patients with inferior infarction were lower than those before operation, and the differences were statistically significant (P<0.05). 【Conclusion】Nicodil can improve myocardial perfusion level and protect renal function in acute STEMI patients after emergency PCI, especially in patients with anterior infarction.

The Impact of Suspension Therapy Combined with Acupuncture at the Paravertebral Points on Balance Function in Patients with Post-Stroke Hemiplegia

【Objective】 To explore the effects of combining suspension therapy with acupuncture at the paravertebral points on the balance function of patients with post-stroke hemiplegia.【Methods】 A total of 98 patients with post-stroke hemiplegia admitted to our hospital were randomly divided into the control group and the observation group, with 49 patients in each group. The control group received conventional rehabilitation care, while the observation group received suspension therapy combined with acupuncture at the paravertebral points. After 3 months of rehabilitation care, the balance function and walking ability of both groups were compared.【Results】 After rehabilitation, the Berg Balance Scale (BBS), lower limb Fugl-Meyer Assessment (FMA-L), and Modified Barthel Index (MBI) scores of both groups were higher than those before treatment, with the observation group showing significantly better results (P<0.05). The gait analysis, 10-meter walking speed (10MWS), and Functional Ambulation Classification (FAC) scores of both groups were also higher than those before treatment (P<0.05), with the observation group outperforming the control group (P<0.05).【Conclusion】 Suspension therapy combined with acupuncture at the paravertebral points significantly improves the balance function and activities of daily living in patients with post-stroke hemiplegia, making it a valuable clinical approach for rehabilitation.

Effect of Tirofiban Combined with Adenosine on Microcirculation and Cardiac Function in Patients with Coronary Heart Disease after PCI

【Objective】To investigate the effects of Tirofiban combined with adenosine on microcirculation and cardiac function in patients with coronary heart disease after percutaneous coronary intervention (PCI). 【Objective】A total of 120 patients with coronary heart disease who underwent PCI in our hospital from January 2020 to May 2022 were selected and divided into observation group and control group by random number table, with 60 cases in each group. The control group was given tirofiban before surgery, the observation group was given Tirofiban combined with adenosine before surgery. The serum levels of cardiac troponin I (cTnI) and creatine kinase isoenzyme (CK-MB) were compared between the two groups before and after operation. The indexes of myocardial microcirculation immediately after operation[thrombolysis in myocardial infarction(TIMI) blood flow grading and correcting TIMI frame count (CTFC)] were compared between the two groups. The indexes of cardiac function before and after operation [left ventricular end-systolic diameter (LVESD), left ventricular end-diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF)]were compared between the two groups. The incidence of major adverse cardiovascular events(MACE) was also compared between the two groups. 【Results】Before operation, there was no significant difference in serum cTnI and CK-MB levels between 2 groups (P>0.05). After operation, serum cTnI and CK-MB levels in 2 groups were lower than before operation, and the observation group was lower than the control group, the difference was statistically significant (P<0.05). There was no significant difference in immediate TIMI blood flow classification between the two groups (P>0.05). The CTFC immediately after operation in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). The level of LVEF after operation was higher than before operation, and the level of LVESD and LVEDD were lower than before operation, the difference was statistically significant (P<0.05). The LVEF level in the observation group was higher than that in the control group, and the LVESD and LVEDD levels were lower than that in the control group, with statistical significance (P<0.05). The incidence of MACE in the observation group was 1.67% (1/60), significantly lower than that in the control group 15.00% (9/60), and the difference was statistically significant (P<0.05). 【Conclusion】Tirofiban combined with adenosine can effectively improve myocardial microcirculation and cardiac function in patients with coronary heart disease before PCI.

Effect of Propofol and Sevoflurane Combined with Remifentanil on Hemodynamics and Nerve Function in Patients Undergoing Craniotomy

【Objective】To investigate the effects of propofol and sevoflurane combined with remifentanil on hemodynamics and nerve function in patients undergoing craniotomy.【Methods】A total of 100 patients undergoing craniotomy were randomly divided into the observation group (propofol combined with remifentanil) and the control group (sevoflurane combined with remifentanil), with 50 cases in each group. The operation conditions were compared between the two groups in terms of hemodynamics at each time point [Heart rate (HR), mean arterial pressure (MAP)],stress responses before and after surgery [cortisol (COR), blood glucose (GLU), adrenocorticotropin (ACTH)],neural function [Specific enolase (NSE), S100β protein] indicators and incidence of postoperative adverse events. 【Results】Postoperative wakefulness and extubating time in the observation group were shorter than those in the control group (P<0.05). There was no significant difference in MAP and HR between the two groups at each time point (P>0.05). During operation, COR, GLU and ACTH in both groups were higher than before operation (P<0.05), and the observation group was lower than the control group (P<0.05). During operation, NSE and S100β protein in both groups were higher than before operation (P<0.05), and the observation group was lower than the control group (P<0.05). There was no significant difference in the incidence of adverse events between the two groups (P>0.05).【Conclusion】 The use of propofol and sevoflurane combined with remifentanil in craniotomy can achieve good anesthetic effects and maintain the hemodynamic stability of patients, but propofol combined with remifentanil is more conducive to reducing the stress response of patients and protecting nerve function.

Relationship of Disease Severity with the Characteristics and Morphological Changes of IVBA Plaques in Patients with Posterior Circulation Ischemia Evaluated by MRI and MSCT

【Objective】To explore the relationship of disease severity with the characteristics and morphological changes of intracranial vertebrobasilar artery (IVBA) plaques in patients with posterior circulation ischemia evaluated by magnetic resonance imaging (MRI) and multi-slice spiral CT (MSCT). 【Methods】A retrospective analysis was conducted on the clinical data of 90 patients with posterior circulation ischemia admitted to our hospital from February 2019 to December 2022. Based on the MRI examination results, patients with infarction lesions were included in the infarction group, while those without infarction lesions but with corresponding symptoms were included in the TIA group. MRI and MSCT examinations were performed within one week to compare the distribution, characteristics, and geometric morphology of intracranial IVBA plaques between the two groups of patients. 【Results】The MSCT examination results showed a total of 102 plaques in 90 IVBA patients, including 32 plaques (31.37%) detected in the initial segment of vertebral artery, 26 plaques (25.49%) detected in the basilar artery segment of vertebral artery, and 23 plaques (22.55%) detected in the intracranial segment of the vertebral artery. The detection rate of soft plaques in the cerebral infarction group was higher than that in the TIA group, while the detection rate of hard plaques was lower than that in the TIA group, and the differences were statistically significant (P<0.05). The true length, tortuosity rate, plaque area, and plaque burden of the basal artery in the cerebral infarction group were greater than those in the TIA group, and the differences were statistically significant (P<0.05). The lesion vascular lumen area in the cerebral infarction group was lower than that in the TIA group, and the difference was statistically significant (P<0.05). The proportion of unilateral vertebral artery dominance in the cerebral infarction group was higher than that in the TIA group, and the proportion of equally thick vertebral arteries on both sides was lower than that in the TIA group, with statistical significance (P<0.05). 【Conclusion】The characteristics and morphological abnormalities of intracranial IVBA plaques in patients with posterior circulation ischemia are closely related to the occurrence of cerebral infarction. MRI and MSCT examinations can detect and evaluate IVBA lesions in patients.

Diagnostic Value of Transvaginal Ultrasonography and Serum CA199 and CA125 Levels in Adenomyosis

【Objective】To evaluate the diagnostic value of transvaginal ultrasonography and serum levels of carbohydrate antigen (CA) 199 and CA125 in adenomyosis. 【Methods】A total of 124 suspected patients with adenomyosis admitted to our hospital from January 2021 to December 2022 were selected, the pathological diagnosis results were statistically analyzed, the serum CA199 and CA125 levels were detected, and the diagnostic efficacy of single and combined transvaginal ultrasound and serum CA199 and CA125 levels in adenomyosis was compared. 【Results】Among the 124 suspected cases of adenomyosis, 70 cases were diagnosed as adenomyosis and 54 cases were diagnosed as non-adenomyosis. Serum levels of CA199 and CA125 in patients with adenomyosis were higher than those in patients without adenomyosis (P<0.05). The results of CA125 in the diagnosis of adenomyosis were moderately consistent with the pathological diagnosis, and the results of the combined diagnosis of adenomyosis were highly consistent with the pathological diagnosis. The accuracy of the combined diagnosis of adenomyosis by transvaginal ultrasonography and serum CA199 and CA125 levels was higher than that of single diagnosis of each index, and the difference was statistically significant (P<0.05).【Conclusion】Transvaginal ultrasonography and serum CA199 and CA125 levels are of high diagnostic value in adenomyosis, but the combined value of all indexes is higher, which can provide reference for clinical diagnosis and treatment of adenomyosis.

Changes of Trace Elements and Th1/Th2 in Children with Recurrent Upper Respiratory Tract Infection and their Significance

【Objective】To investigate the changes and significance of trace elements and helper T cell 1 (Th1)/Helper T cell 2 (Th2) in children with recurrent upper respiratory tract infection (RRTI). 【Methods】Sixty-two children with RRTI admitted to our hospital from October 2019 to October 2021 (the observation group) were selected, and 60 healthy children (the control group) who underwent physical examination in our hospital during the same period were selected. General information, trace elements [iron (Fe), lead (Pb), magnesium (Mg), zinc (Zn), calcium (Ca)] and Th1/Th2 levels of the two groups were compared. The correlation between trace elements and Th1/Th2 was analyzed by Pearson, and the influencing factors of RRTI were analyzed by Logistic regression. 【Results】There were no significant differences in gender, age, body mass index (BMI), preterm birth, cesarean section, malnutrition and outdoor activities between the two groups (P>0.05). The proportion of family member's smoking and using antibacterial drugs ≥3 times/year in the observation group was higher than that in the control group, and the proportion of breastfeeding was lower than that in the control group (P<0.05). There was no significant difference in Pb and Mg levels between the two groups (P>0.05). Fe, Zn, Ca and Th1/Th2 in the observation group were lower than those in the control group (P<0.05). Pearson correlation analysis showed that Fe, Zn, Ca were positively correlated with Th1/Th2 (P<0.05). Logistic regression analysis showed that family members smoking, antimicrobial use times ≥3 times/year and low Fe, Zn, Ca, Th1/Th2 were all risk factors for RRTI development, and breastfeeding was a protective factor for RRTI development (P<0.05). 【Conclusion】 Trace element deficiency in children with RRTI is closely related to Th1/Th2 immune balance, and is an important factor affecting the development of RRTI.

Effects of Fufang Xueshuantong Dripping Pills on Serum VEGF and Ocular Hemodynamic Indexes in Patients with Retinal Vein Obstruction

【Objective】To investigate the effects of fufang xueshuantong dripping pills on serum vascular endothelial growth factor (VEGF) and ocular hemodynamics in patients with retinal vein obstruction. 【Methods】 A total of 80 patients with retinal vein obstruction admitted to Chenggu County Hospital of Traditional Chinese Medicine in Shaanxi Province from January 2019 to March 2022 were selected and divided into the control group (receiving conventional drug treatment) and the observation group (adding fufang xueshuantong dripping pill to the control group) by random number table method, with 40 cases in each group. After treatment, Chinese medicine symptom score, VEGF level, endothelin-1 (ET-1) level, nitric oxide (NO) level, peak systolic blood velocity (PSV), end-diastolic blood velocity (EDV) and resistance index (RI) were compared between the two groups.【Results】The effective rate of the observation group was 95.00%, which was significantly higher than that of the control group 80.00% (P<0.05). After treatment, the visual acuity drop, chest and hypochthonic distension and eye distension scores in the observation group were lower than those before and after treatment in the control group (P<0.05). After treatment, the levels of VEGF and ET-1 in the observation group were significantly lower than before treatment and after treatment in the control group, and NO was higher than before treatment and after treatment in the control group (P<0.05). After treatment, RI in the observation group was significantly lower than before treatment and after treatment in the control group, PSV and EDV were significantly higher than before treatment and after treatment in the control group (P<0.05).【Conclusion】The Xueshuantong dripping pill has a significant effect in the treatment of patients with retinal vein obstruction, which can significantly improve the TCM symptom score and eye hemodynamic index, and reduce the levels of VEGF and ET-1.

【Objective】 To investigate the pulmonary function, inflammatory factors and clinical outcomes of children with refractory mycoplasma pneumoniae pneumonia (RMPP) and Mycoplasma pneumoniae pneumonia (MPP). 【Methods】A total of 110 children with MPP treated in our hospital from January 2019 to January 2022 were selected, of which 48 children with RMPP were included in the RMPP group and 62 children with common MPP were included in the MPP group. and FVC in the recovery stage were higher than those in the acute stage (P<0.05), but there was no significant difference in FEV
1
/FVC in the acute stage and the recovery stage between the two groups (P>0.05). The FEV
1
and FVC of the RMPP group in acute stage and recovery stage were lower than those of the MPP group (P<0.05). Serum levels of IL-2, IL-6, TNF-α and INF-γ in the recovery stage were lower than those in the acute stage (P<0.05). The serum levels of IL-2, IL-6, TNF-α and INF-γ in RMPP group were higher than those in the MPP group (P<0.05). There was no significant difference in the incidence of abnormal lung function between the two groups (P>0.05). The proportion of children with normal lung function in the RMPP group was lower than that in the MPP group, and the proportion of children with restrictive ventilation dysfunction was higher than that in the MPP group (P<0.05). 【Conclusion】Compared with children with MPP, children with RMPP have more severe lung function injury and inflammation. From the acute stage to the convalescent stage, both children with RMPP and MPP have improved lung function and reduced inflammation. However, children with RMPP have poor recovery and clinical outcomes, and a higher proportion of restricted ventilation dysfunction in convalescent stage.

Efficacy and Safety of PKEP in the Treatment of Massive Benign Prostatic Hyperplasia

【Objective】To investigate the efficacy and safety of plasmakinetic enucleation of prostate (PKEP) in the treatment of massive benign prostatic hyperplasia.【Methods】A total of 112 patients with massive prostatic hyperplasia who were treated in our hospital from January 2020 to January 2021 were selected and divided into the observation group (n=56) and the control group (n=56).The patients in the observation group underwent PKEP, while the patients in the control group underwent transurethral plasma kinetic resection of prostate (TUPKP).The perioperative indexes and postoperative complications of the two groups of patients were compared. The patients were followed up for 3 months after the operation, then the follow-up related indexes, renal function, and sexual function of the two groups of patients before and after the operation were compared.【Results】The operation time, intraoperative blood loss, indwelling catheter time and hospital stay in the observation group were significantly shorter than those in the control group, and the weight of gland removal was significantly higher than that in the control group (P<0.05).The postoperative prostate symptom score (IPSS), quality of life score (QOL) and residual urine volume of the two groups were significantly lower than those before operation, and the maximum urinary flow rate was significantly higher than that before operation (P<0.05), but there were no significant differences of those parameters between the two groups (P>0.05).The postoperative blood urea nitrogen (BUN) and creatinine (Scr) of the two groups were significantly lower than those before the operation (P<0.05), however, there were no significantly different levels of BUN and Scr between the two groups (P>0.05).The scores of the International Index of Erectile Function (IIEF-5) at 6 months and 12 months after operation in the observation group were significantly lower than those before operation of the observation and those in the control group (P<0.05).The total incidence of complications in the observation group significantly lower than the control group (P<0.05).【Conclusion】The efficacy and safety of PKEP in the treatment of massive benign prostatic hyperplasia are good.

Factors Influencing Hypoglycemia in Obese Patients after Spinal Anesthesia

【Objective】To investigate the occurrence and influencing factors of hypoglycemia in obese patients following spinal anesthesia.【Methods】 Eighty obese patients who underwent spinal anesthesia were enrolled in the study. They were divided into two groups based on whether they developed hypoglycemia after anesthesia: the hypoglycemia group (32 cases) and the non-hypoglycemia group (48 cases). Relevant clinical data of all patients were collected and analyzed. A preliminary screening of risk factors for hypoglycemia was conducted using univariate analysis of variance, followed by a multivariate logistic regression analysis to identify independent risk factors for hypoglycemia in obese patients after anesthesia.【Results】Univariate analysis showed that a high body mass index (BMI), the presence of diabetes, hypertension, prolonged surgery duration, and extended preoperative fasting time were all associated with the occurrence of hypoglycemia in obese patients following spinal anesthesia (P<0.05). Multivariate Logistic regression analysis revealed that a high BMI, the presence of diabetes, and prolonged surgery duration were independent risk factors for hypoglycemia after anesthesia in obese patients (P<0.05).【Conclusion】 For obese patients, especially those with complications such as diabetes, strict weight control and careful management of surgery duration before undergoing spinal anesthesia are essential to effectively reduce the risk of postoperative hypoglycemia.

Value of Dynamic Enhanced MRI Based on FIGO 2018 Staging in Preoperative Staging Diagnosis of Cervical Cancer

【Objective】To investigate the diagnostic value of dynamic enhanced magnetic resonance imaging (MRI) in the preoperative staging of cervical cancer based on the staging of International Federation of Obstetrics and Gynecology (FIGO) 2018. 【Methods】Clinical data of 78 patients with cervical cancer admitted to our hospital from January 2021 to August 2022 were retrospectively analyzed, all of which were confirmed by surgical pathological examination. All patients underwent dynamic enhanced magnetic MRI before surgery. Based on FIGO 2018 staging, the consistency of preoperative staging diagnosis with pathological diagnosis was compared. The diagnostic value of dynamic enhanced magnetic MRI in preoperative staging of cervical cancer was analyzed. 【Results】Pathological examination showed that the pathological types of 78 cervical cancer patients were adenocarcinoma in 7 cases, squamous cell carcinoma in 67 cases, adenosquamous cell carcinoma in 2 cases, poorly differentiated cervical neuroendocrine carcinoma in 1 case and small cell carcinoma in 1 case. Kappa consistency test analysis showed that dynamic enhanced MRI based on FIGO 2018 staging was in good agreement with pathological diagnosis of cervical cancer in preoperative staging (Kappa=0.766, P<0.05). The accuracy of dynamic enhanced MRI based on FIGO 2018 staging in the diagnosis of preoperative stage ⅠB1, ⅠB2, ⅠB3, ⅡA1, ⅡA2 and ⅢC1 was 97.44%, 89.74%, 93.59%, 92.31%, 94.87% and 94.87%, respectively. 【Conclusion】The diagnosis of cervical cancer by dynamic enhanced MRI based on FIGO 2018 staging has good consistency with pathological diagnosis and high diagnostic accuracy.

Clinical Efficacy of Warm Acupuncture Combined with Medical Chitosan in the Treatment of Elderly Knee Osteoarthritis

【Objective】To explore the clinical efficacy of warm acupuncture combined with medical chitosan in the treatment of elderly patients with knee osteoarthritis. 【Methods】From January 2020 to June 2024, a total of 300 elderly patients with knee osteoarthritis were selected for treatment. They were randomly divided into the observation group and the control group, with 150 patients in each group. Patients in the control group received conventional treatment, namely intra-articular injection of medical chitosan, while those in the observation group received warm acupuncture in addition to conventional treatment. Clinical efficacy, knee joint range of motion (ROM), pain level [Visual Analogue Scale (VAS)], inflammatory factors [Interleukin-1β (IL-1β), Tumor Necrosis Factor-α (TNF-α), Matrix Metalloproteinase-9 (MMP-9), Hyaluronic Acid (HA), Rheumatoid Factor (RF)], and incidence of adverse reactions were compared between the two groups. 【Results】After treatment, the clinical efficacy and ROM in the observation group were higher than those in the control group (P<0.05), while VAS scores, IL-1β, TNF-α, MMP-9, HA, RF levels and the incidence of adverse reactions were lower than those in the control group (P<0.05). 【Conclusion】Warm acupuncture combined with medical chitosan can significantly improve knee joint mobility, relieve pain, reduce inflammatory factor levels, and has fewer adverse reactions. It showes high clinical value.

Clinical Effect of Valproic Acid Oral Liquid Combined with Levetiracetam Oral Liquid in the Treatment of Children with Epilepsy and its Influence on Cognitive Function

【Objective】To investigate the clinical effect of valproic acid oral liquid combined with levetiracetam oral liquid in the treatment of children with epilepsy and its influence on cognitive function. 【Methods】A total of 36 children with epilepsy admitted to our hospital from February 2021 to February 2023 were divided into combination group and routine group according to treatment methods, with 18 cases in each group. The conventional group was given valproic acid oral liquid, and the combined group was combined with levetiracetam oral liquid on the basis of the conventional group. Both groups were treated for 6 months. The two groups were compared in terms of the efficacy, clinical symptom improvement (seizure frequency, seizure duration) and cognitive function [verbal intelligence quotient (VIQ), operational intelligence quotient (PIQ), full scale intelligence quotient (FIQ)], neurotransmitter [S-100β protein, high mobility group protein-1 (HMGB-1), neuropeptide Y (NPY), galanin (GAL)] and incidence of adverse reactions. 【Results】The total effective rate of the combined group was higher than that of the conventional group, the difference was statistically significant (P<0.05). After treatment, the seizure frequency of the two groups was lower than before treatment, and the seizure time was shorter than before treatment, and the seizure frequency of the combined group was lower than that of the conventional group, and the seizure time was shorter than that of the conventional group, with statistical significance (P<0.05). After treatment, VIQ, PIQ and FIQ scores in 2 groups were higher than before treatment, and the combined group was higher than the conventional group (P<0.05). After treatment, the levels of serum S-100β protein and HMGB-1 were lower than before treatment, and the levels of serum NPY and GAL were higher than before treatment, and the difference was statistically significant between the combined group and the conventional group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】Valproic acid oral liquid combined with levetiracetam oral liquid is effective in the treatment of children with epilepsy, which can effectively reduce the frequency and duration of seizures and reduce the damage to cognitive function of children.
