Effect of Tirofiban Combined with Adenosine on Microcirculation and Cardiac Function in Patients with Coronary Heart Disease after PCI

Abstract  【Objective】To investigate the effects of Tirofiban combined with adenosine on microcirculation and cardiac function in patients with coronary heart disease after percutaneous coronary intervention (PCI). 【Objective】A total of 120 patients with coronary heart disease who underwent PCI in our hospital from January 2020 to May 2022 were selected and divided into observation group and control group by random number table, with 60 cases in each group. The control group was given tirofiban before surgery, the observation group was given Tirofiban combined with adenosine before surgery. The serum levels of cardiac troponin I (cTnI) and creatine kinase isoenzyme (CK-MB) were compared between the two groups before and after operation. The indexes of myocardial microcirculation immediately after operation[thrombolysis in myocardial infarction(TIMI) blood flow grading and correcting TIMI frame count (CTFC)] were compared between the two groups. The indexes of cardiac function before and after operation [left ventricular end-systolic diameter (LVESD), left ventricular end-diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF)]were compared between the two groups. The incidence of major adverse cardiovascular events(MACE) was also compared between the two groups. 【Results】Before operation, there was no significant difference in serum cTnI and CK-MB levels between 2 groups (P>0.05). After operation, serum cTnI and CK-MB levels in 2 groups were lower than before operation, and the observation group was lower than the control group, the difference was statistically significant (P<0.05). There was no significant difference in immediate TIMI blood flow classification between the two groups (P>0.05). The CTFC immediately after operation in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). The level of LVEF after operation was higher than before operation, and the level of LVESD and LVEDD were lower than before operation, the difference was statistically significant (P<0.05). The LVEF level in the observation group was higher than that in the control group, and the LVESD and LVEDD levels were lower than that in the control group, with statistical significance (P<0.05). The incidence of MACE in the observation group was 1.67% (1/60), significantly lower than that in the control group 15.00% (9/60), and the difference was statistically significant (P<0.05). 【Conclusion】Tirofiban combined with adenosine can effectively improve myocardial microcirculation and cardiac function in patients with coronary heart disease before PCI.
