1121 Expression of MMP-2 and ADAM17 in Osteosarcoma and its Clinical Significance

【Objective】To investigate the expression of matrix metalloproteinase 2 (MMP-2) and a disintegrin and metalloproteinase 17 (ADAM17) in osteosarcoma and its clinical significance.【Methods】The osteosarcoma tissues and normal adjacent tissues (at least 2 cm from the tumor edge) were collected from 53 cases of orthopaedic open surgery in our hospital. The expression levels of MMP-2 and ADAM17 in 53 osteosarcoma tissues were detected and compared, and to analyze the correlation between them and the clinicopathological characteristics of osteosarcoma patients.【Results】The positive expression rate of MMP-2 in osteosarcoma was 73.58%, which was significantly higher than 18.87% in normal tissues adjacent to cancer. The positive expression rate of ADAM17 in osteosarcoma was 79.25%, which was significantly higher than 5.66% in normal tissues adjacent to cancer (P<0.05); The expression level of MMP-2 and ADAM17 protein in osteosarcoma was significantly higher than that in normal tissues (P<0.05); The positive expression rates of MMP-2 and ADAM17 were correlated with distant metastasis and ennecking stage (P<0.05), but not with age, sex and tumor diameter (P>0.05). Spearman correlation showed that MMP-2 was positively correlated with ADAM17 expression in osteosarcoma (P<0.05). 【Conclusion】The positive expression rate of MMP-2 and ADAM17 in osteosarcoma group is significantly higher than that in adjacent normal tissues. The expression of MMP-2 and ADAM17 is related to the distant metastasis and ennecking stage of the patients. The expression of MMP-2 and ADAM17 in osteosarcoma is positively correlated, which is of great significance for the diagnosis, prognosis and treatment of osteosarcoma.

1124 Effects of Ticagrelor on Perioperative Sialic Acid and Heart Functiion of Diabetic Patients Complicated with Non-ST-elevation Acute Coronary Syndrome Receiving Perioperative-early Percutaneous Coronary Intervention

【Objective】To investigate the effects of ticagrelor on the level of sialic acid (SA) and heart function of non-ST-elevation acute coronary syndrome (NSTE-ACS) complicated with type 2 diabetic mellitus (T2DM) patients while receiving perioperative-early percutaneous coronary intervention (PCI). 【Methods】A total of 200 T2DM diabetic patients complicated with non-ST-elevation acute coronary syndrome who were hospitalized and underwent early PCI in Hunan Province's Brain Hospital and People's Hospital of Leiyang from January 2019 to July 2021 were recruited in the study. They were divided into two groups in random,the observation group(with ticagrelor,n=100)and the control group(with clopidogrel,n=100).The serum SA levels were compared between the two groups before operation and postoperation (6 h,24 h,3 d and 7 d). The left ventricular ejection fraction (LVEF),left ventricular end-diastolic diameter(LVDd) and left ventricular end-diastolic volume(LVEDV)were compared between the two groups before operation and 1 week after surgery. 【Results】The levels of SA in two groups were not significantly different before operation(P>0.05),while postoperative 6 h,the levels were higher than those before operation(P<0.05),then SA levels at 24 h,3 d and 7 d after operation decreased in both groups. The difference was statistically significant (P<0.05). The levels of serum SA in the observation group at 6 h,24 h,3 d,and 7 d after operation were lower than those in the control group,the differences were statistically significant (P<0.05). On the 7th day after operation,the LVDd and LVEDV of the two groups were lower than those before operation,while the LVEF was higher than that before operation. The difference between the observation group and the control group was statistically significant (P<0.05).【Conclusion】Both ticagrelor and clopidogrel can effectively down-regulate SA and improve heart function of NSTE-TCS patients complicated with T2DM after PCI.The efficacy of ticagrelor was better than clopidogrel.

1128 Effect of Whole Course Nursing Management of Cancer Pain on Pain Control and Quality of Life of Patients with Advanced Lung Cancer

【Objective】 To explore the effect of whole course nursing of cancer pain in pain control and quality of life of patients with advanced lung cancer. 【Methods】 A total of 108 patients with cancer pain of advanced lung cancer admitted to the department of hematology and oncology of the 921st Hospital of the Joint Logistic Support Force of the Chinese People's Liberation Army from January 2019 to December 2020 were selected and divided into control group (January to December 2019) and observation group (January to December 2020) according to the admission time, with 54 patients in each group. The control group received routine and traditional cancer pain nursing, and the intervention group received the whole process nursing management of pain on this basis. The pain control, quality of life, medication compliance and nursing satisfaction of the two groups were compared. 【Results】The scores of body, role, emotion, society, cognitive function and general health of the patients in the observation group were higher than those in the control group, and the scores of pain, fatigue and insomnia were lower than those in the control group (P<0.05). The NRS score and the incidence of explosive pain in the observation group were lower than that in the control group (P<0.05). The excellent rate of medication compliance and nursing satisfaction in the observation group were higher than those in the control group (P<0.05). 【Conclusion】 The whole course nursing of cancer pain can effectively alleviate the pain degree of patients with advanced lung cancer combined with cancer pain, reduce the occurrence of explosive pain, improve the quality of life of patients, and improve the compliance and satisfaction of patients with medication.

1131 Effect of SOCS6 on Biological Behavior of Prostatitis Epithelial Cells

【Objective】To investigate the effect of cytokine signal transduction inhibitor 6 (SOCS6) on the injury of prostatitis epithelial cells. 【Methods】Normal prostate tissue cells were divided into the normal control group,the model group and the SOCS6 model group. The cells were transfected with PCDNA3.1-SOCS6 and stimulated with lentivirus infection. The expression level of SOCS6 mRNA in the cells was detected by qRT-PCR.Expression of SOCS6 protein,cysteine-containing caspase-3 (C-caspase-3) and cysteine-containing caspase-9(C-caspase-9) was detected by Western blot. MTT was used to detect cell proliferation and flow cytometry was used to detect cell apoptosis.【Results】The expression levels of SOCS6 protein and SOCS6 mRNA in the SOCS6 up-regulated group were significantly higher than those in the model group and the normal control group,and the differences were statistically significant (all P<0.05). The survival rate of cells in the SOCS6 up-regulation group was higher than that in the model group,and the apoptosis rate was lower than that in the model group (P<0.05). Compared to the normal control group,the survival rate and apoptosis rate of SOCS6 up-regulated group were not significantly different (P>0.05). The results of Western blot assay showed that the protein levels of C-caspase-3 and C-caspase-9 in the SOCS6 up-regulated group were lower than those in the model group and higher than those in the normal control group,and the differences were statistically significant (all P<0.05).【Conclusion】The low expression of SOCS6 protein in prostatitis tissue can increase the proliferation and apoptosis of epithelial cells,which is of great value for clinical diagnosis and treatment and prognosis prediction.

1135 Construction and Evaluation of Diabetes Retinopathy Model

【Objective】 To establish the model of streptozotocin (STZ)-induced diabetes retinopathy and its evaluation method. 【Methods】 Thirty male SD rats were randomly divided into normal group (10 rats) and model group (20 rats). In the model group, fasting blood glucose was measured 3 days after intraperitoneal injection of STZ (65 mg/kg). Ten diabetes rats with blood glucose >14 mmol/L were selected as the model group. Fasting blood glucose was measured and weighed every 4 weeks. Retinal optical coherence tomography (OCT) was used to observe the changes of retinal blood vessels in diabetes rats, and the morphological changes of retinal tissues in diabetes rats were analyzed by HE staining.【Results】Compared with the normal group, the fasting blood glucose level in the model group increased from 3 days to 20 weeks; OCT imaging results showed that at the 12th week of modeling, the blood vessels in the fundus of the model group were bent and arranged in disorder; At the 20th week of modeling, obvious abnormal neovascularization was observed; The pathological results showed that the retinal nuclear layer of diabetes rats was disorganized from 12 to 20 weeks, the nerve fiber layer and ganglion cell layer were edema, the lumen was dilated, some nuclei were pyknosis, and the number of retinal ganglion cells was significantly reduced.【Conclusion】OCT can be used to dynamically monitor the development of retinopathy and detect fasting blood glucose, which can successfully evaluate the model of diabetes retinopathy.

1138 Effect of 4-Methylumbelliferone on Dendritic Cells Derived from Peripheral Blood Mononuclear Cells

【Objective】To investigate the effect of 4-methylumbelliferone (4MU) on monocyte derived dendritic cells (Mo-DCs) in human peripheral blood.【Methods】Human monocytes were obtained by density gradient centrifugation and cultured for 5 days by human granulocyte monocyte colony stimulating factor and interleukin-4, Mo-DCs were treated with different concentrations of 4mu, the activity of Mo-DCs in each group was detected by CCK-8 method, the immunophenotypic marker level of Mo-DCs in each group was detected by flow cytometry, and the IL-6 level of Mo-DCs in each group was detected by electrochemiluminescence method.【Results】 When the concentration of 4mu was 0.1~1 mmol/L, it has little effect on the activity of Mo-DCs. 4MU inhibited the expression of immunophenotypic markers CD40, CD83, CD80, HLA-DR and Mo-DCs secreting IL-6.【Conclusion】 4MU inhibits the maturation of Mo-DCs and reduces the secretion of IL-6, which provides a new idea for the prevention and treatment of infectious inflammation. 4MU may become a new drug for immune regulation.

1141 Correlation Analysis of Serum Uric Acid and Blood Lipid Levels with Parkinson's Disease

【Objective】To explore the relationship between serum uric acid (UA), triglyceride (TG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C) and the onset of Parkinson's disease (PD). 【Methods】A total of 136 PD patients (PD group) and 91 healthy persons (control group) treated in our hospital from January 2019 to August 2020 were collected, and the levels of UA, TG, TC, LDL-C and HDL-C were compared between the two groups; According to Hoehn Yahr (H-Y) stage, PD patients were divided into three subgroups:early stage, middle stage and late stage, and the UA levels of different subgroups were compared; The influencing factors of PD incidence were analyzed by binary logistic regression. 【Results】The levels of UA, TG, TC, LDL-C and HDL-C in the PD group were significantly lower than those in the control group (P<0.05). The UA level in the PD group was significantly lower than that in the control group (P<0.05); There was no significant difference in UA level between women in the PD group and those in the control group (P>0.05). The UA level of PD patients in different stages was significantly different (P<0.05). Binary logistic regression analysis showed that UA and HDL-C levels were correlated with the onset of PD, and high blood UA and HDL-C levels were protective factors for the onset of PD. 【Conclusion】 The levels of UA and HDL-C are related to the pathogenesis of PD.

1144 Effect of HGF-induced Umbilical Cord Mesenchymal Stem Cells on Improving Steatohepatitis Complicated with Hepatic Fibrosis in Rats via TGF- β/Smad Pathway

【Objective】 To discuss the mechanism of umbilical cord mesenchymal stem cells (hUC-MSCs) induced by hepatocyte growth factor (HGF) in ameliorating steatohepatitis complicated with liver fibrosis in rats through transforming growth factors β (TGF-β)/ Smad pathway.【Methods】SPF Wistar rats were randomly divided into normal control group and experimental model group (model blank group, model treatment group 1 and model treatment group 2). The rats in the experimental model group were fed with high-fat diet and subcutaneously injected with 40% carbon tetrachloride (CCl4) olive oil solution; The normal control rats were fed with ordinary diet and subcutaneously injected with olive oil solution. After 6 weeks of modeling, the normal control group and the model blank group were injected with PBS buffer through tail vein; The model treatment group 1 was injected with hUC-MSCs; In the model treatment group 2, hUC-MSCs containing HGF were injected. All rats were killed after 3 weeks of observation. The body weight and liver wet weight were weighed and the liver index was calculated;The serum AST, ALT and ALB levels were detected; The liver tissue was taken for HE and Masson staining to evaluate the pathological condition of liver tissue and observe the liver injury and fibrosis; By immunohistochemical method, the expression level of TGF-β1 and Smad3 in liver tissue was detected. 【Results】 Compared with the model blank group, the liver wet weight, liver index, ALT and AST levels of rats in the normal control group, model treatment group 1 and model treatment group 2 were significantly decreased; The level of ALB increased significantly (P<0.05). In the model blank group, there were obvious bullous steatosis, accompanied by obvious liver fibrosis, and even the formation of false lobules without normal structure in the model blank group; The degree of liver fibrosis and the number of adipocytes in the two model treatment groups were significantly reduced compared with the model blank group, and the degree of steatosis and fibrosis in the model treatment group 2 was slightly reduced compared with the model treatment group 1. Compared with the model blank group, the expression of the TGF-β1 and Smad3 of rats in the normal control group, the model treatment group 1 and the model treatment group 2 was significantly down regulated (P<0.05).【Conclusion】hUC-MSCs have a certain effect on the treatment of steatohepatitis and hepatic fibrosis in rats. The effect of hUC-MSCs induced by HGF is better than that of hUC-MSCs, and the mechanism may be related to the inhibition of TGF-β/Smad signaling pathway.

1149 Study on the Characteristics of Heart Rate and its Influencing Factors in Patients with Primary Aldosteronism Diagnosed for the First Time

【Objective 】 To explore the characteristics of heart rate and its influencing factors in patients with primary aldosteronism (PA) diagnosed for the first time.【Methods】A total of 196 patients with PA (observation group) admitted to the first people's Hospital of Changde from January 2015 to December 2019 were selected, and 196 patients with essential hypertension were included as the control group. The resting heart rate, 24 h average heart rate, day average heart rate, night average heart rate and blood potassium were compared between the two groups. The correlation between renin, aldosterone and aldosterone/renin activity ratio (ARR) and heart rate was studied by correlation analysis, and the effects of renin, aldosterone and ARR on heart rate were analyzed by multiple linear regression.【Results】The renin, blood potassium (K) and renin in the observation group were lower than those in the control group, and the fasting blood glucose (FPG), triglyceride (TG), blood sodium (Na), aldosterone and ARR in the observation group were higher than those in the control group (P<0.05). The resting heart rate, 24 h average heart rate, day average heart rate and night average heart rate of the patients in the observation group were lower than those in the control group, while the systolic blood pressure (SBP) and diastolic blood pressure (DBP) were higher than those in the control group (P<0.05). According to the results of adrenal CT examination, PA patients were divided into hyperplasia group (n=81) and adenoma group (n=115). The resting heart rate, 24 h average heart rate, daytime average heart rate and nighttime average heart rate of hyperplasia group were higher than those of adenoma group, but the difference was not statistically significant (P>0.05). Spearman correlation analysis showed that renin was positively correlated with resting heart rate, 24 h average heart rate, daytime average heart rate and nighttime average heart rate (P<0.05); Aldosterone was negatively correlated with resting heart rate, 24 h mean heart rate and daytime mean heart rate (P<0.05); ARR was negatively correlated with resting heart rate, 24 h mean heart rate, daytime mean heart rate and nighttime mean heart rate (P<0.05). Multiple linear regression analysis showed that lnARR was an independent correlation factor of resting heart rate, 24 h average heart rate, daytime average heart rate and nighttime average heart rate (all P<0.05), and DBP was also a correlation factor of nighttime average heart rate (P<0.05). 【Conclusion】The resting heart rate, 24 hour average heart rate, day average heart rate and night average heart rate of PA patients are slower than those of essential hypertension patients. Renin is positively correlated with heart rate, while aldosterone and ARR are negatively correlated with heart rate.

1153 Protection of Shenqi Dihuang Decoction on Mesangial Proliferative Glomerulonephritis Rats and Its Effect on p38MAPK/NF-κB Signal Pathway

【Objective】To investigate protective effect of Shenqi Dihuang decoction on mesangial proliferative glomerulonephritis (MsPGN) rats and its effect on 38 mitogen-activated protein kinase/nuclear transcription factor Kappa B (p38MAPK/NF-κB) pathway. 【Methods】Forty rats were randomly divided into the sham operation group,MsPGN model group and low and high dose groups of shenqi dihuang decoction,with 10 rats in each group. The rats in the sham-operated group were exposed to the left kidney only,and the rest were the same as those in the model group. The rats in the MsPGN group were exposed to the left kidney only after anesthesia,the rats in the low and high dose groups were given 10 mL/(kg·d) of shenqi dihuang decoction (1.4 mg/mL and 2.8 mg/mL solution) for 4 weeks after the MsPGN model was established,rats in the sham operation group and model group were given the same amount of normal saline at the same time. The 24 h urine protein,urea nitrogen and blood creatinine were measured. The levels of IL-8 and TNF-α in renal tissues were detected by ELISA,and p38MAPK/NF-κB pathway proteins (p38MAPK,p-p38MAPK,NF-κB p65) were detected by Western blotting.【Results】Compared with the sham operation group,the levels of 24 h urinary protein,urea nitrogen,serum creatinine,IL-8,TNF-α,p-p38MAPK/p38MAPK and NF-κB p65 in the model group and Shenqi Dihuang decoction group were all increased (P<0.05). Compared with the model group,the levels of 24 h urinary protein,urea nitrogen,serum creatinine,IL-8,TNF-α,p-p38MAPK/p38MAPK and NF-κB p65 in the low and high dose groups were decreased (P<0.05). Compared with the low-dose group,the levels of all indexes in the high dose group were decreased (P<0.05). 【Conclusion】The protective effect of Shenqi Dihuang decoction on MsPGN rats may be realized by inhibiting the activation of p38MAPK/NF-κB pathway and thus inhibiting the inflammatory reaction of renal tissue.

1157 Relationship of Serum CRP,PCT,LDH and cys C Levels with Kidney Injury after Bee Sting

【Objective】To investigate the relationship of serum C-reactive protein (CRP),procalcitonin (PCT),lactate dehydrogenase (LDH) and cystatin C (cys C) levels with kidney injury after bee sting.【Methods】The clinical data of 130 patients with bee stings were retrospectively analyzed. According to whether acute kidney injury (AKI),patients were divided into the AKI group (n=38) and the non-AKI group (n=92).At admission,baseline information was collected and laboratory indicators [platelet count (PLT),CRP,PCT,LDH,cys C] were measured. The value of serum CRP,PCT,LDH and cys C in predicting AKI in patients with bee sting was analyzed by ROC. The difference information between the two groups was included in multivariate Logistic regression analysis and quantitative assignment was performed to identify the influencing factors of AKI in patients with bee sting.【Results】There were no significant differences in gender,age,bee sting site,associated diseases,bee sting time and PLT level between the two groups (P>0.05).The proportion of bee stings and hospital admission time≥6 h after injury in the AKI group was significantly higher than that in the non-AKI group,and the levels of serum CRP,PCT,LDH and cys C were significantly higher in the AKI group than those in the non-AKI group,with statistical significance (P<0.05).ROC analysis showed that CRP≥6.285 mg/L,PCT≥1.570 ng/mL,LDH≥1482.175 U/L and cys C≥1.430 mg/L could be used to predict AKI in bee sting patients (P<0.05).Multiple Logistic regression analysis confirmed that wasp species of sting,hospital admission time and serum levels of CRP,PCT,LDH and cys C were significant influencing factors of AKI in patients with bee sting injury (P<0.05).【Conclusion】Serum levels of CRP,PCT,LDH and cys C were significant influencing factors of AKI in bee sting patients. The occurrence of AKI can be predicted when CRP≥6.285 mg/L,PCT≥1.570 ng/mL,LDH≥1482.175 U/L and cys C≥1.430 mg/L showed up.

1161 Relationship between HR-HPV Negative Cervical Lesions and Cervical Microenvironment

【Objective】To explore the relationship between high risk human papillomavirus (HR-HPV)-negative cervical lesions and cervical microenvironment. 【Methods】A total of 90 patients with HR-HPV negative cervical lesions were selected as the study subjects, including 26 patients with low-grade squamous intraepithelial lesion (LSIL group), 31 patients with high-grade squamous intraepithelial lesion (HSIL group), and 33 patients with cervical squamous cell carcinoma (CSCC group). The positive expression levels of CD4, CD8, Foxp3 and CD163 in the cervical lesions and the levels of IL-2 and IL-10 in the vaginal lavage fluid were detected and compared. 【Results】 Lactobacillus, Gardnerella and Bacillus were the most common vaginal flora in HPV negative cervical lesions; The detection rate of Lactobacillus in the CSCC group was lower than that in the LSIL group and the HSIL group, while the detection rate of Gardnerella and bacillus was higher than that in the LSIL group and the HSIL group. Compared with patients in LSIL and HSIL groups, the number of CD4+ T cells in cervical lesions of the CSCC group decreased, while the number of CD8+ T cells, Foxp3+ T cells and CD163+ tumor associated macrophages increased (P<0.05). Compared with the patients in the LSIL and HSIL groups, the level of IL-2 in vaginal lavage fluid in the CSCC group decreased, but the level of IL-10 increased (P<0.05). 【Conclusion】Vaginal micro ecology and immune imbalance are closely related to the progression of HR-HPV negative cervical lesions. Targeted treatment of vaginal infection and improvement of immunity can prevent the progression of cervical lesions.

1165 Correlation and Clinical Significance of miR-130a and RUNX3 Expression in Esophageal Squamous Cell Carcinoma

【Objective】To investigate the correlation and clinical significance of the expression of miR-130a and recombinant human Runt-related transcription factor 3 (RUNX3) in esophageal squamous cell carcinoma. 【Methods】A total of 84 patients with esophageal squamous cell carcinoma who were treated in our hospital were collected. The cancer tissues and the corresponding adjacent tissues of patients at the time of surgery were taken for exam. The qRT-PCR was performed to detect the expression level of miR-130a in tissues; Western blot and immunohistochemical methods were performed to detect the expression of RUNX3 protein in tissues. All postoperative patients were followed up to investigate the effects of miR-130a and RUNX3 expression on the prognosis of patients with esophageal squamous cell carcinoma. 【Results】Compared with adjacent tissues,the expression level of miR-130a in esophageal squamous cell carcinoma tissues was higher (P<0.05),while the expression level of RUNX3 protein was lower (P<0.05). The expression levels of miR-130a and RUNX3 protein in esophageal squamous cell carcinoma were negatively correlated (P<0.05). The protein expression levels of miR-130a and RUNX3 were obviously correlated with T stage,cancer differentiation and lymph node metastasis (P<0.05). Kaplan-Meier survival curve analysis showed that the median survival time of patients with esophageal squamous cell carcinoma of high miR-130a expression was obviously shorter than that of patients with low miR-130a expression (P<0.05).Also the result showed that the median survival time of patients with esophageal squamous cell carcinoma with low RUNX3 expression was obviously shorter than that of patients with high RUNX3 expression (P<0.05). Multivariate Cox analysis showed that T staging (T3-T4),lymph node metastasis,high expression of miR-130a,and low expression of RUNX3 were independent risk factors for poor prognosis in patients with esophageal squamous cell carcinoma (P<0.05). 【Conclusion】The expression level of miR-130a is up-regulated and the expression level of RUNX3 protein is down-regulated in esophageal squamous cell carcinoma. The two are closely related to the poor prognosis of patients with esophageal squamous cell carcinoma. Patients with esophageal squamous cell carcinoma with high expression of miR-130a and low expression of RUNX3 may have a higher risk of poor prognosis.

1170 Genotype and Phenotype Analysis of RET in Different Types of HSCR Patients in Southern Hunan

【Objective】To explore the relationship of RET genotypes with different phenotypes of (Hirschsprung's disease,HSCR) patients in Southern Hunan. 【Methods】A total of 188 patients with HSCR in our hospital from January 2015 to January 2021 were collected as the HSCR group,and 206 healthy children matched with gender and age in our hospital in the same period were selected as the control group. Using literature references and bioinformatics database for searching the polymorphic sites of the target gene,five polymorphic sites of RET gene (rs1799939,rs1800858,rs1800862,rs2435357 and rs2506030) were selected in this study. The target gene was amplified by PCR,and the above five sites were genotyped by TaqMan genotyping technology. The correlation of RET genotypes with the genetic susceptibility of HSCR patients was analyzed. 【Results】There was no significant difference in allele frequencies of rs1799939,rs1800858,rs1800862,rs2435357 and rs2506030 in patients with different types of HSCR (P>0.05). The rs1799939,rs1800858,rs2435357 and rs2506030 significantly increased the risk of HSCR,with OR values of 3.777,1.328,7.614 and 3.184,respectively. Compared to individuals without the above risk alleles,individuals with risk alleles at the above four loci had a significantly increased risk of HSCR. The OR values of rs1799939,rs1800858,rs2435357 and rs2506030 were 1.718,2.059,2.678 and 1.636,respectively. 【Conclusion】The SNP loci rs1799939,rs1800858,rs2435357 and rs2506030 of RET gene are significantly associated with patients with HSCR in southern Hunan,which can an important role in early warning of HSCR in Southern Hunan.

1174 Expression and Clinical Significance of Semaphorin 3A and CXCL12 in Vitreous Humor and Serum of Patients with Diabetic Retinopathy

【Objective】To investigate the expression and clinical significance of semaphorin 3A (Sema 3A) and CXC chemokine ligand 12 (CXCL12) in vitreous humor and serum of patients with diabetic retinopathy (DR). 【Methods】A total of 113 DR patients who were diagnosed and treated in our hospital from July 2019 to July 2021 were collected as the research objects. The DR patients were divided into proliferative diabetic retinopathy (PDR) group (n=44) and non-proliferative diabetic retinopathy (NPDR) group (n=69) according to the international clinical classification of DR. In addition,52 healthy volunteers who underwent physical examination in our hospital were as the control group. The general information and biochemical indicators of the research subjects were collected,the levels of Sema 3A and CXCL12 in serum and vitreous humor were detected by enzyme-linked immunosorbent assay; the influencing factors of DR were analyzed by logistic regression; and the clinical value of Sema 3A and CXCL12 levels in predicting the occurrence of PDR was analyzed by receiver operating characteristic (ROC) curve. 【Results】The levels of Sema 3A and CXCL12 in the vitreous humor and serum of the patients in the PDR group were higher than those in the NPDR group (P<0.05). The levels of Sema 3A and CXCL12 in the serum of the patients in the PDR and NPDR groups were higher than those in the control group (P<0.05). Levels of HbA1c,TC,TG,LDL-C,HDL-C,and FPG in the PDR group and NPDR group were higher than those in the control group,and these indicators in the PDR group were higher than that in the NPDR group,with statistical significance (P<0.05). the expression of Sema 3A and CXCL12 in serum of DR patients was positively correlated (r=0.664,P<0.05),and the expression of Sema 3A and CXCL12 in vitreous humor was positively correlated (r=0.457,P<0.05). Logistic regression analysis showed that Sema 3A,CXCL12,FPG,HbA1c,HDL-C,TC,TG and LDL-C were all risk factors for DR (P<0.05). ROC curve showed that the areas under the curve (AUC) of serum Sema 3A and CXCL12 levels for predicting PDR were 0.951 (95%CI:0.906-0.979) and 0.953 (95%CI:0.909-0.980),respectively; and the corresponding sensitivities were 93.18 % and 95.45%,respectively,the specificities were 91.74% and 82.64%,respectively. The combined predictive AUC of serum Sema 3A combined with CXCL12 levels was 0.981 (95%CI:0.947-0.996),with a sensitivity of 95.45% and a specificity of 92.56%. 【Conclusion】The levels of Sema 3A and CXCL12 in serum and vitreous humor of patients with PDR are up-regulated,which are closely related to the occurrence and development of DR,and can be used as potential indicators to evaluate the occurrence of PDR.

【Objective】To investigate the expression of protein tyrosine phosphatase non-receptor type 23 (PTPN23) in hepatocellular carcinoma (HCC) tissues and its relationship with the efficacy of transcatheter arterial chemoembolization (TACE). 【Methods】A total of 102 patients with HCC who were treated in the two hospitals were selected as the research subjects. Western blot was used to detect the expression of PTPN23 protein in HCC tissues; the modified response evaluation criteria in solid tumor (mRECIST) was used to evaluate the efficacy of TACE treatment. The relationship between the PTPN23 level in HCC tissues and the therapeutic effect of TACE was analyzed. 【Results】After TACE treatment in 102 HCC patients,18 patients (17.65%) CR,33 (32.35%) PR,21 (20.59%) SD and 30 (29.41%) PD. The PTPN23 protein level in the treatment ineffective group (n=30) was 0.80±0.18,which was lower than that in the treatment effective group 1.07±0.23,and the difference was statistically significant (P<0.05).The area under the ROC curve,sensitivity and specificity of PTPN23 to evaluate the efficacy of TACE treatment were 0.819,70.00% and 79.17%,respectively. Multivariate Logistic regression analysis showed that tumor diameter≥5 cm and PTPN23<0.90 were independent risk factors for TACE treatment (P<0.05). 【Conclusion】The detection of PTPN23 expression in HCC tissues is helpful to judge the efficacy of TACE treatment. Low expression of PTPN23 in HCC tissues is an independent risk factor for the efficacy of TACE therapy.

1182 Expression of miR-200b-5p and ATAD2 in Cervical Cancer Tissues and Their Relationship with Prognosis

【Objective】To explore the expression of miR-200b-5p and Adenosine triphosphate family protein 2 (ATAD2) in cervical cancer tissues and their relationship with prognosis. 【Methods】A total of 124 patients with cervical cancer who underwent surgical treatment in our hospital from January 2019 to March 2020 were selected in the study. All cervical cancer tissue specimens and para-cancerous tissues (≥5 cm from the cancerous tissue) were collected. The expression levels of miR-200b-5p mRNA and ATAD2 mRNA in cancer tissues and adjacent tissues were compared. After 18 months follow-up, the prognosis of cervical cancer patients was counted and divided into the death group and the survival group according to the prognosis. Compare the clinical data of patients in the death group and survival group. Logistic multivariate regression was used to analyze the risk factors affecting the prognosis of cervical cancer patients. A receiver operating characteristic curve (ROC) was made, and the area under the curve (AUC) was used to analyze the predictive power of the expression of miR-200b-5p and ATAD2 in cervical cancer tissues on the prognosis of cervical cancer patients. 【Results】The miR-200b-5p mRNA in cervical cancer tissue was lower than that in adjacent tissues (P<0.05), while ATAD2 mRNA was higher than that in adjacent tissues (P<0.05). After 18 months of follow-up, the death rate of cervical cancer patients was 17.74%. In the death group, the proportion of stage Ⅲ/Ⅳ, the proportion of lymph node metastasis, the proportion of poorly differentiated degree, the proportion of muscle invasion depth>1/2, the proportion of ATAD2 mRNA in cervical cancer tissue was higher than those in the survival group (P<0.05), while cervical cancer tissue miR-200b-5p level was lower than in the survival group (P<0.05). Logistic regression analysis showed that the clinical stage Ⅲ/Ⅳ, poorly differentiated, low expression level of miR-200b-5p mRNA and high level ATAD2 mRNA in cervical cancer tissues were all risk factors affecting the prognosis of cervical cancer patients (OR=2.835, 2.793 , 3.080, 3.364, respectively, P<0.05). ROC analysis showed that the AUCs of miR-200b-5p mRNA and ATAD2 mRNA in cervical cancer tissues to predict the prognosis of cervical cancer death were 0.793 and 0.860, respectively. 【Conclusion】The level of miR-200b-5p mRNA is low in cervical cancer tissues, while ATAD2 mRNA level is high. Both miR-200b-5p mRNA and ATAD2 mRNA in cervical cancer tissue have a high predictive power for the prognosis of cervical cancer patients. Thus they can be used as important reference indicators for evaluating the cervical cancer prognosis.

1186 Effect of 3D and 2D Thoracoscopic Radical Resection of Lung Cancer on Perioperative Immune Function of Patients with Lung Cancer

【Objective】 To to explore the effect of 3D and 2D thoracoscopic radical resection on perioperative immune function of patients with lung cancer. 【Methods】 The clinical data of 59 patients with lung cancer who underwent thoracoscopic radical resection of lung cancer in Suqian Hospital of Nanjing Drum Tower Hospital Group and the Nanjing Gulou Hospital from March 2019 to March 2022 were retrospectively analyzed. According to the different surgical methods, they were divided into observation group (n=30) and control group (n=29). Patients in the observation group underwent radical lung cancer surgery with 3D video-assisted thoracoscopy system, while patients in the control group underwent radical lung cancer surgery with 2D video-assisted thoracoscopy system. The perioperative operation time, intraoperative bleeding volume, number of lymph node dissections, retention time of drainage tube and hospitalization time were compared between the two groups. The levels of carcinoembryonic antigen (CEA), carbohydrate antigen 50 (CA50), cytokeratin 19 fragment antigen 21-1(CYFRA21-1) and T lymphocyte subsets CD4+, CD8+, CD4+/CD8+ before and after operation were compared between the two groups. The postoperative complications of the two groups were compared. 【Results】 The operation time, intraoperative bleeding volume, retention time of drainage tube and hospitalization time of the observation group were shorter than those of the control group, and the number of lymph node dissection was greater than that of the control group (P<0.05). There was no significant difference in CEA, CA50 and CYFRA21-1 between the two groups before and after operation (P>0.05); The levels of CEA, CA50 and CYFRA21-1 in both groups were significantly lower than those before operation (P<0.05). CD4+, CD8+, CD4+/CD8+ in both groups were significantly better than that before operation, and the observation group was significantly better than the control group (P<0.05). The total incidence of postoperative complications in the observation group was 13.33% (4/30), which was lower than that in the control group [20.69%(6/29)], but the difference was not statistically significant (P>0.05). 【Conclusion】Radical resection of lung cancer under 3D and 2D thoracoscopy has no significant effect on tumor marker level and complication rate of patients, but 3D thoracoscopy can shorten the operation time, drainage tube retention time, hospital stay, reduce intraoperative bleeding, and have little impact on the immune function of patients.

1190 Analysis of Prognostic Factors of Patients with Lung Cancer after Radical Resection

【Objective】 To explore the prognostic factors of patients with lung cancer after radical resection. 【Methods】A total of 160 patients who underwent radical resection of lung cancer in our hospital from June 2019 to June 2021 were selected and followed up for 30 months. According to the prognosis, they were divided into poor prognosis group (death or metastasis at other sites during follow-up, n=55) and good prognosis group (no death or metastasis at other sites during follow-up, n=105). The clinical baseline data such as preoperative albumin (ALB), red blood cell volume distribution width (RDW) and the levels of serum proliferating cell nuclear antigen (PCNA), excision repair cross complementary gene 1 (ERCC1) and cytokeratin 19 fragment antigen 21-1 (CYFRA21-1) were collected and compared between the two groups. Multivariate logistic regression was used to analyze the risk factors of prognosis of patients with lung cancer after radical resection.The predictive efficacy of serum PCNA, ERCC1 and CYFRA21-1 combined with clinical characteristics on poor prognosis of patients with lung cancer after radical resection was analyzed by receiver operating characteristic (ROC) curve.【Results】 The age≥60 years, TNM stage Ⅲ ~ Ⅳ, preoperative ALB<35 g/L, preoperative RDW>12.95%, preoperative PCNA and CYFRA21-1 levels in the poor prognosis group were significantly higher than those in the good prognosis group (P<0.05), and the preoperative ERCC1 levels were significantly lower than those in the good prognosis group (P<0.05). Age≥60 years, TNM stage Ⅲ~Ⅳ, preoperative ALB<35 g/L, preoperative RDW > 12.95%, preoperative PCNA≥396.20 pg/mL, ERCC1 ≤74.85 ng/mL, CYFRA21-1≥9.00 ng/mL were independent risk factors for poor prognosis of patients with lung cancer after radical resection (P<0.05). Serum PCNA, ERCC1, CYFRA21-1 combined with age, TNM stage, ALB and RDW have certain predictive value for poor prognosis of patients with lung cancer after radical resection. The area under the ROC curve is 0.945, the predictive sensitivity is 0.909, and the specificity is 0.981. 【Conclusion】 The prognosis of patients with lung cancer after radical resection is affected by many factors, including age, TNM stage, ALB, RDW, PCNA, ERCC1 and CYFRA21-1. Among them, serum PCNA, ERCC1 and CYFRA21-1 can be used as reliable biomarkers to predict the prognosis of patients with lung cancer after radical resection, and their combination with the above other indicators can improve the prediction efficiency.

1194 Diagnostic Value of Quantitative Parameters of Strain and Shear Wave Elastography in Benign and Malignant Breast Nodules

【Objective】To investigate the diagnostic value of quantitative parameters of strain elastography (SE) and shear wave elastography (SWE) in benign and malignant breast nodules. 【Methods】 A total of 132 patients with breast diseases diagnosed and treated in our hospital from January 2018 to October 2020 were retrospectively analyzed. All patients underwent ultrasound elastography, and the pathological examination results were the gold standard. The SE parameter index strain rate ratio (SR), elastic imaging score, maximum elastic modulus value (Emax), mean value (Emean), elastic standard deviation (Esd), elastic ratio between lesion and surrounding tissue (Eratio), and elastic ratio between lesion and surrounding tissue (A/shell) of benign and malignant patients were compared; The consistency of SE, SWE and their combined diagnosis of benign and malignant breast nodules with the gold standard was analyzed and the diagnostic efficiency was calculated; The diagnostic value of SE, SWE and their combined application in benign and malignant breast nodules was analyzed by ROC curve.【Results】 Among 136 lesions in 132 patients, 91 were benign and 45 were malignant. There were significant differences in SE parameters, elastic image score, SR and SWE parameters (Emax, Emean, Esd, Eratio, A/shell) between benign nodules and malignant nodules (P<0.05); Among SWE parameters, Emax has a moderate value in the diagnosis of benign and malignant breast nodules and was higher than other parameters (P<0.05), and other parameters had a lower value in the diagnosis of benign and malignant breast nodules (P<0.05); SE, SWE and their combined diagnosis were consistent with the gold standard (P<0.05). The combined diagnosis had the highest efficacy, with sensitivity of 82.22%, specificity of 90.11% and accuracy of 87.50%; SE, SWE and their combination in the diagnosis of benign and malignant breast nodules had moderate diagnostic value, which was higher than that of SE and SWE alone (P<0.05).【Conclusion】There are clear differences in SE and SWE parameters between benign and malignant breast nodules. SE and SWE have their own advantages in judging the benign and malignant breast nodules. The combined application of these parameters can improve the diagnostic value and provide reference for the evaluation of clinical breast nodules.

1197 Correlation Analysis of Serum Levels of miR-21, cys C, sCD40L and sICAM-1 with Diabetes Nephropathy

【Objective】To investigate the relationship between serum levels of microRNA-21 (miR-21), Cystatin C (cys C), soluble leukocyte differentiation antigen CD40 ligand (sCD40L) and soluble intercellular adhesion factor-1 (sICAM-1) and diabetes nephropathy (DN). 【Methods】A total of 162 patients with type 2 diabetes admitted to our hospital from October 2019 to October 2021 were selected. According to urinary microalbumin excretion rate (UACR), they were divided into group A (UACR<30 mg/24 h, simple type 2 diabetes) 70 cases, group B (UACR 30-300 mg/24 h, early DN) 52 cases, and group C (UACR >300 mg/24 h,DN) 40 cases; In addition, 60 healthy volunteers who underwent physical examination in our hospital in the same period were selected as Group D. The serum levels of miR-21, cys C, sCD40L, sICAM-1 and their correlations with various biochemical indexes were compared in each group. The diagnostic efficacy of the above indexes for early DN was analyzed by using the receiver operating characteristic (ROC) curve. 【Results】 The levels of serum miR-21 in group A, B and C were significantly lower than those in group D (P<0.05), and the levels of serum cys C, sCD40L and sICAM-1 were significantly higher than those in group D (P<0.05); The levels of serum miR-21 in group A, group B and group C gradually decreased (P<0.05), and the levels of serum cys C, sCD40L and sICAM-1 gradually increased (P<0.05). Serum miR-21, cys C, sCD40L and sICAM-1 were positively correlated with HbA1c, TG, LDL-C and UACR (P<0.05), and negatively correlated with estimated glomerular filtration rate (EGFR) (P<0.05). Serum miR-21,cys C,sCD40L and sICAM-1 had certain diagnostic efficacy for early DN, and the area under the curve was 0.801, 0.774, 0.873 and 0.789, respectively.【Conclusion】miR-21, cys C, sCD40L and sICAM-1 are all involved in the pathogenesis of DN and are related to the severity of the disease. They can be used as reliable markers for early diagnosis of DN.

1202 Prognosis and Tumor Marker Changes of Concurrent Chemoradiotherapy Combined with DC-CIK Cell Immunotherapy in NSCLC

【Objective】To investigate the effect of concurrent chemoradiotherapy combined with dendritic cell-cytokine-induced natural killer (DC-CIK) cell immunotherapy on the prognosis and tumor marker levels of non-small cell lung cancer (NSCLC). 【Methods】Seventy-two NSCLC patients were randomly divided into the control group and the observation group,with 36 cases in each group. The control group was treated with TP regimen (paclitaxel + cisplatin) combined with intensity-modulated radiotherapy (IMRT),while the observation group was treated with DC-CIK cellular immunotherapy on the basis of treatment in the control group. The curative effect,peripheral blood T lymphocyte subsets (CD3+,CD4+,CD8+,CD4+/CD8+),serum microRNA-137 (miR-137),microRNA-29a (miR-29a) and serum tumor markers were compared between the two groups before and after treatment. Changes in the levels of substances [carbohydrate antigen 125 (CA-125),carcinoembryonic antigen (CEA),cytokeratin 19 fragment antigen 21-1(CYFRA21-1)] were compared as well. The incidence of adverse reactions in the two groups was observed. 【Results】The objective remission rate (RR) of the observation group was 94.44% (34/36),which was higher than that of the control group of 77.78% (28/36) (P<0.05). After treatment,CD3+,CD4+,CD4+/CD8+ in the two groups were higher than those before treatment (P<0.05),and CD8+ was lower than those before treatment (P<0.05). And after treatment,CD3+,CD4+,CD4+/CD8+ in the observation group were higher than those in the control group (P<0.05),there was no significant difference in CD8+ compared with the control group (P>0.05). After treatment,the levels of miR-137,miR-29a,CA-125,CEA,and CYFRA21-1 in the two groups were lower than those before treatment (P<0.05); and the levels of each index in the observation group were lower than those in the control group after treatment (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】Concurrent chemoradiotherapy combined with DC-CIK cellular immunotherapy can improve the curative effect of NSCLC patients,down-regulate the expression of miR-137 and miR-29a,improve the immune function and serum tumor markers of patients. And the risk of adverse reactions is low,which is worthy of clinical promotion.

【Objective】 To investigate the effect of artificial plasma exchange on laboratory indexes and pregnancy outcome of patients with acute fatty liver during pregnancy. 【Methods】A total of 46 patients with gestational acute fatty liver diagnosed and treated in the First Affiliated Hospital of Naval Medical University from January 2017 to April 2019 were selected and divided into control group (comprehensive treatment, n=23) and observation group (comprehensive treatment combined with artificial plasma exchange, n=23) according to different treatment methods. The liver function index, coagulation index, curative effect and pregnancy outcome were compared between the two groups before and after treatment. 【Results】 Before treatment, the expression levels of direct bilirubin (DBIL), total bilirubin (TBIL), aspartate aminotransferase (AST), alanine aminotransferase (ALT), prothrombin time (PT), platelet count (PLT) and fibrinogen (FIB) were not significantly different between the two groups (P>0.05). Compared with that before treatment, the levels of DBIL, TBIL, AST, ALT and PT decreased and the levels of PLT and FIB increased in the two groups after treatment; The improvement effect and total effective rate of the above indexes in the observation group were better than those in the control group (P<0.05). The incidence of neonatal mild asphyxia and severe asphyxia in the observation group was significantly lower than that in the control group (P<0.05). 【Conclusion】 Artificial plasma exchange therapy can effectively improve the liver function, coagulation function and pregnancy outcome of patients with acute fatty liver during pregnancy, and has certain safety, which is worthy of clinical application.

1209 Expression and Clinical Significance of GATA Binding Protein 3 in Cervical Cancer and Precancerous Lesions

【Objective】To explore the expression and clinical significance of GATA binding protein 3 (GATA3) in cervical cancer and precancerous lesions. 【Methods】A total of 162 cases of cervical surgery specimens admitted to our hospital were collected,including 35 cases of cervical intraepithelial neoplasia (CIN) Ⅰ (the CIN Ⅰ group),43 cases of CIN Ⅱ~Ⅲ (the CIN Ⅱ~Ⅲ group),42 cases of cervical cancer (the cervical cancer group) and 42 cases of normal cervix (the control group). The clinical data and GATA3 mRNA expression levels of the four groups were compared. The clinical data of patients with cervical cancer group and precancerous lesion group (CINⅠ and CINⅡ~Ⅲ) were compared with the other groups. Logistic multivariate regression was analyzed the risk factors affecting cervical cancer. Receiver operating characteristic curve (ROC) and the area under the curve (AUC) were drawn to evaluate the diagnostic efficacy of GATA3 mRNA levels for cervical cancer. 【Results】There were statistically significant differences in GATA3 mRNA among the four groups(P<0.05). GATA3 mRNA in the CINⅠ,CINⅡ~Ⅲ,and cervical cancer groups were all lower than those in the control group (P<0.05). GATA3 mRNA level in the cervical cancer group was lower in that in the CIN Ⅰ group and CIN Ⅱ~Ⅲ group (P<0.05). GATA3 mRNA level in the CIN Ⅱ~Ⅲ group was lower than that in the CIN Ⅰ group (P<0.05). The number of births,the percentage of smoking history,proportion of family history of cervical cancer,and the percentage of sexual partners≥2 in the cervical cancer group were higher than those in the precancerous lesion group (P<0.05),while the age of first sex,age of first childbearing and GATA3 mRNA were lower (P<0.05) . Logistic multivariate analysis showed that the number of births,sex partners≥2 and GATA3 mRNA were all risk factors affecting cervical cancer ( P<0.05). ROC analysis showed that the best cut-off point of GATA3 mRNA expression in cervical tissues for predicting the occurrence of cervical cancer was 1.12,and the sensitivity,specificity and AUC were 80.95%,83.33%,and 0.869,respectively. 【Conclusion】The expression of GATA3 mRNA in cervical cancer and precancerous lesions is abnormally low,and its diagnostic efficiency for cervical cancer is relatively high. It can be used as an important reference index for clinical diagnosis of cervical cancer.

1213 Effects of Different Nutritional Support Programs on Adverse Reactions during Concurrent Chemoradiotherapy in Patients with Esophageal Cancer

【Objective】To investigate the effects of different nutritional support programs on adverse reactions during concurrent chemoradiotherapy in patients with esophageal cancer.【Methods】A total of 96 patients with esophageal cancer treated in our hospital were randomly divided into the observation group and the control group,with 48 cases in each group. The patients in the control group were given oral nutrients in addition to regular diet,and the patients in the observation group were given enteral nutrition support in addition to routine diet. The treatment effect,adverse reactions during treatment,body mass index (BMI),subjective global assessment scale (PG-SGA) score,serum albumin (ALB),hemoglobin (Hb),serum transferrin (TF) and T lymphocyte (CD4+,CD8+,CD4+/CD8+) levels before and after treatment were compared between the two groups.【Results】There was no significant difference in the effective rate of treatment between the two groups (P>0.05). After treatment,the BMI and PG-SGA score in the observation group were higher than those before treatment. The BMI in the control group was lower than that before treatment,and the PG-SGA score in the control group was higher than that before treatment (P<0.05). After treatment,the BMI of the observation group was higher than that of the control group,and the PG-SGA score was lower than that of the control group (P<0.05).The levels of ALB,Hb and TF in the observation group after treatment were higher than those before treatment,and those in the control group were lower than those before treatment (P<0.05). After treatment,there were no significant differences in CD4+,CD8+,CD4+/CD8+ in the observation group compared with those before treatment (P>0.05). Both CD4+ and CD4+/CD8+ in the control group were lower than those before treatment and the observation group (P<0.05),and there was no significant difference in CD8+ among those comparisons (P>0.05). The incidences of infection and myelosuppression≥grade 3 in the observation group were significantly lower than those in the control group (P<0.05). There was no significant difference in the incidence of radiation pneumonitis≥grade 3 and radiation esophagitis≥grade 3 between the two groups (P<0.05).【Conclusion】Enteral nutrition support program for patients with esophageal cancer during concurrent radiotherapy and chemotherapy can improve nutritional status,enhance immune function,reduce the incidence of infection and myelosuppression,and improve radiotherapy and chemotherapy tolerance.

1217 MACC1 and c-Met Expression and Clinical Value in Different Tissues of Patients with Gastric Cancer

【Objective】To observe the expression and clinical application value of MACC1 and c-Met in different tissues of gastric cancer patients. 【Methods】The clinical data of 133 patients with gastric cancer who underwent radical surgical treatment were retrospectively analyzed. The expression of MACC1 and hepatocyte growth factor receptor (c-Met) were detected in cancerous and adjacent tissues after surgery. The positive protein detection rates of MACC1 and c-Met,and the relative mRNA expression levels of MACC1 and c-Met in different tissues were compared. The positive detection rates of MACC1 and c-Met in gastric cancer patients with different clinicopathological features were compared as well. Finally,the difference in postoperative survival rate of patients with positive and negative MACC1 and c-Met were follow-up.【Results】The positive detection rates of MACC1 and c-Met in cancerous tissues were significantly higher than those in adjacent tissues (P<0.05).The relative expression levels of MACC1 mRNA and c-Met mRNA in cancer tissues were significantly higher than those in adjacent tissues (P<0.05).There were no significant differences in the positive detection rates of MACC1 and c-Met in cancer tissues of patients with different gender,age,tumor diameter,tumor location and tissue type (P>0.05).The positive rates of MACC1 and c-Met were significantly increased in patients with TNM stage Ⅲ or Ⅳ,infiltration depth T3 or T4,Lauren's intestinal type,low differentiation or undifferentiation,lymph node metastasis (P<0.05). The 3-year mortality was 33.08%(44/133). Kaplan-meier survival analysis showed that the higher the positive rate of MACC1and c-Met in cancer tissue,the lower the survival rate of patients (P<0.05).【Conclusion】High expression of MACC1 and c-Met in cancer tissues of gastric cancer patients is closely related to the progression,invasion,metastasis prognosis of the disease.

1221 Application Value of CT Perfusion Imaging Combined with Serum T3 and FT4 in Diagnosis of Early Severe Traumatic Brain Injury

【Objective】To explore the value of CT perfusion imaging (CTPI) combined with serum triiodothyronine (T3) and free thyroxine (FT4) in the diagnosis of early severe traumatic brain injury (sTBI). 【Methods】 The clinical data of 80 patients with early sTBI admitted to our hospital from January 2018 to December 2021 were retrospectively analyzed. According to the Glasgow Coma Scale (GCS) after admission, they were included in the severe group (n=43) and the mild to moderate group (n=37). Cerebral CTPI was performed in both groups to compare the regional cerebral blood flow parameters and serum T3, FT4 levels in the two groups, and the regional cerebral blood flow parameters and serum T3, FT4 levels and GCS in the severe group. The correlation between GCS and regional cerebral blood flow parameters, serum T3 and FT4 levels was analyzed. 【Results】 The levels of rCBV, rCBF, serum T3 and FT4 in the severe group were lower than those in moderate group, and MTT was higher than that in the mild and moderate group (P<0.05). On the 7th day after operation, the rCBV, rCBF, GCS scores, serum T3 and FT4 levels in the severe group were higher than those on the next day after admission, and the MTT was higher than that on the next day after admission. The difference was statistically significant (P<0.05). Pearson correlation analysis showed that GCS was negatively correlated with rCBV and serum T3 and FT4 levels (r=-0.4003,-0.6841,-0.7136,all P<0.05), but not with rCBF and MTT (P>0.05). 【Conclusion】 The combination of brain CTPI and serum T3, FT4 in the diagnosis of early sTBI is helpful to evaluate the degree of brain injury and to improve the diagnostic accuracy.

1224 Relationship between RDW/ESR, Chemerin, IL-13 and Coronary Artery Dilatation in Acute Kawasaki Disease

【Objective】 To investigate the relationship between the level of red blood cells distribution width/erythrocyte sedimentation rate (RDW/ESR), Chemerin , interleukin-13 (IL-13) and coronary dilatation in acute Kawasaki disease.【Methods】A total of 135 children with acute Kawasaki disease treated in our hospital from August 2016 to August 2020 were selected and divided into dilation group (n=28) and non dilation group (n=107) according to whether coronary artery dilation occurred. The baseline data and RDW/ESR, Chemerin and IL-13 of the two groups were compared. The multivariate logistic regression equation was used to analyze the relevant influencing factors of coronary artery dilatation, and the levels of RDW/ESR, Chemerin and IL-13 of children with different therapeutic responsiveness to intravenous immunoglobulin were compared. The receiver operating characteristic curve (ROC curve) was used to analyze the value of each index and combined prediction of therapeutic responsiveness.【Results】RDW/ESR, Chemerin and IL-13 in the expansion group were higher than those in the non expansion group (P<0.05); Multiple logistic regression analysis showed that RDW/ESR, Chemerin and IL-13 were all related to coronary dilatation (P<0.05). The levels of Chemerin and IL-13 in treatment sensitive children were lower than those in non sensitive children (P<0.05). ROC curve showed that the AUC of the combination of Chemerin and IL-13 in predicting IVIG treatment response was higher than that predicted alone, and the AUC was 0.891.【Conclusion】RDW/ESR, Chemerin and IL-13 are all related to coronary artery dilatation in children with acute Kawasaki disease. Chemerin and IL-13 can effectively predict the therapeutic response of intravenous immunoglobulin, and provide basis for timely and effective individualized treatment.

1228 Level and Significance of SIRT7 in Peripheral Blood Mononuclear Cells of Patients with Non-alcoholic Fatty Liver Disease

【Objective】To investigate the level and significance of silent information regulator 7 (SIRT7) in peripheral blood mononuclear cells of patients with non-alcoholic fatty liver disease (NAFLD). 【Methods】A total of 205 patients with NAFLD (the NAFLD group) were selected as the research subjects from three hospitals.Healthy volunteers who participated in the health examination at the same time were selected as the control group (n=100). Ficoll density gradient centrifugation was used to isolate mononuclear cells in peripheral blood; Western blot was used to detect the expression level of SIRT7 in mononuclear cells. The relationship between SIRT7 and the severity of NAFLD was analyzed. 【Results】The expression level of SIRT7 in the NAFLD group was lower than that in control group,the difference was statistically significant (P<0.05). With the aggravation of NAFLD,the expression level of SIRT7 gradually decreased. The area under ROC curve,sensitivity and specificity of SIRT7 in the diagnosis of moderate and severe NAFLD were 0.890,75.61% and 87.80%,respectively. Logistic regression analysis showed that FPG,TC,TG,ALT,AST,and SIRT7 were all independent risk factors for progression to moderate and severe NAFLD (P<0.05). The predicted curve constructed by FPG,TC,TG,ALT,AST,and SIRT7 had a high degree of coincidence between the fit curve of the nomogram model and the ideal curve with a C-index of 0.963. 【Conclusion】The low expression level of SIRT7 in peripheral blood mononuclear cells of NAFLD patients indicates that the disease is serious. The nomogram models constructed by FPG,TC,TG,ALT,AST,and SIRT7 have high discrimination and precision,and can effectively judge moderate and severe NAFLD.

1232 Comparison of the Clinical Effects of Small Suprapubic Incision Cystolithotomy and Transurethral Holmium Laser Lithotripsy in the Treatment of Large Bladder Stones

【Objective】To compare the clinical effects of small suprapubic incision cystolithotomy and transurethral holmium laser lithotripsy in the treatment of large bladder stones. 【Methods】A total of 94 male patients with large bladder stones admitted to our hospital from October 2018 to October 2020 were retrospectively analyzed. According to different treatment methods, they were divided into observation group (small suprapubic incision cystolithotomy) and control group (transurethral holmium laser lithotripsy). The operation condition, VAS score, VRS score, maximum urine flow rate, residual urine volume and postoperative complication rate were compared between the two groups. 【Results】The lithotripsy time and operation time in the observation group were shorter than those in the control group (P<0.05); There was no significant difference between the two groups (P>0.05). The VAS score and VRS score of the two groups were lower than that before operation, and that of the observation group was lower than that of the control group (P<0.05). The maximum postoperative urine flow rate of the two groups was higher than that before operation, and the residual urine volume was lower than that before operation. The difference between the observation group and the control group was statistically significant (P<0.05). The incidence of postoperative stone residue and hypothermia in the observation group was lower than that in the control group (P<0.05). 【Conclusion】Compared with transurethral holmium laser lithotripsy, small suprapubic incision cystolithotomy has more advantages in the treatment of large bladder calculi. The operation time is shorter, the pain of patients can be effectively reduced, and the incidence of complications can be reduced. It is worth popularizing.

1235 Expression and Significance of Serum NGAL,cys C and Urinary KIM-1 in Patients with Liver Cirrhosis Complicated with Acute Renal Injury

【Objective】To investigate the levels of serum neutrophil gelatinase associated lipid carrier protein (NGAL),cysteine protease inhibitor C (cys C) and urinary renal injury factor 1 (KIM-1) in patients with cirrhosis complicated with acute renal injury (AKI) and analyze the risk factors of AKI. 【Methods】A total of 184 patients with liver cirrhosis were divided into the AKI group (n=27) and the non AKI group (n=157) according to whether they were complicated with AKI. The difference between the two groups of patients' basic information and the detection level of relevant laboratory indicators [including hemoglobin (Hb),serum albumin (ALB),white blood cells (WBC),platelet count (PLT),serum alanine aminotransferase (ALT),aspartate aminotransferase (AST),prothrombin activity (PTA),NGAL,cys C,and urinary KIM-1] were collected and compared. The serum NGAL cys C and urinary KIM-1 levels were used to predict the value of AKI in patients with liver cirrhosis. Finally,the different information between the two groups was included in the multivariate logistic regression analysis,and the quantitative assignment was performed to identify the risk factors of AKI in patients with liver cirrhosis. 【Results】There was no significant difference between the two groups in terms of age,sex,body mass index,hypertension,smoking history,classification of liver cirrhosis,etiology of liver cirrhosis,ascites,Hb,ALB,WBC,PLT,PTA,ALT and AST levels (P>0.05).The proportion of patients with AKI combined with diabetes and infection,and the levels of serum NGAL,serum cys C and urinary KIM-1 were significantly higher than those in non AKI group (P<0.05). ROC analysis showed that serum NGAL,cys C and urinary KIM-1 were used to predict AKI in patients with liver cirrhosis. The areas under the curve were 0.854,0.835 and 0.834,respectively,(all P<0.05). Multivariate logistic regression analysis showed that diabetes,infection,serum NGAL,cys C and urinary KIM-1 were independent influencing factors of AKI in patients with liver cirrhosis (P<0.05). 【Conclusion】Serum NGAL,cys C and urinary KIM-1 levels were significantly increased in patients with cirrhosis complicated with AKI,which can be used as sensitive indicators for early prediction of cirrhosis complicated with AKI.

1239 Effect of Simultaneous Heart Valve Replacement and Coronary Artery Bypass Grafting on Cardiac Function in Patients with Moderate Coronary Artery Disease and Heart Valve Disease

【Objective】 To investigate the effect of simultaneous heart valve replacement and coronary artery bypass grafting on cardiac function in patients with moderate coronary artery disease and heart valve disease. 【Methods】The clinical data of 114 patients with moderate coronary artery disease and heart valve disease admitted to our hospital from January 2019 to June 2020 were retrospectively analyzed. According to the different treatment schemes, they were divided into observation group and control group, with 57 patients in each group. The control group received non simultaneous heart valve replacement + coronary artery bypass grafting, and the observation group received simultaneous heart valve replacement + coronary artery bypass grafting. The related indexes of the two groups, the incidence of major cardiovascular adverse events at 6 months after the operation, and the cardiac function [left ventricular ejection fraction (LVEF), left ventricular end diastolic volume, cardiothoracic ratio] at 6 months before and after the operation were compared, The scores of Seattle Angina Questionnaire (SAQ) were compared before and after treatment.【Results】The aortic occlusion time in the observation group was longer than that in the control group, and the ventilator circulation time, the hospitalization days in the general ward, and the ICU monitoring time in the cardiac surgery department were shorter than that in the control group (P<0.05). At 6 months after operation, the cardiothoracic ratio and left ventricular end diastolic volume in the observation group were lower than those in the control group, and the LVEF was higher than that in the control group (P<0.05). The incidence of malignant arrhythmia, acute myocardial infarction, intractable myocardial ischemia, atrial fibrillation and acute heart failure in the observation group was lower than that in the control group (P<0.05). At 3 and 6 months after operation, the SAQ score of the observation group was higher than that of the control group (P<0.05). 【Conclusion】Simultaneous cardiac valve replacement and coronary artery bypass grafting can improve cardiac function, reduce the incidence of major cardiovascular adverse events and improve the quality of life of patients with moderate coronary artery disease and heart valve disease.

1242 Efficacy of Aidi Injection-assisted 3D-CRT in Treatment of Patients with Advanced Esophageal Cancer

【Objective】To explore the effect of Aidi injection-assisted three-dimensional conformal radiotherapy (3D-CRT) in the treatment of patients with advanced esophageal cancer. 【Methods】A total of 96 patients with advanced esophageal cancer in our hospital from May 2017 to August 2021 were selected and divided into two groups according to the random number table method, with 48 cases in each group. The control group was treated with 3D-CRT, and the observation group was treated with Aidi injection in addition to 3D-CRT. The clinical effects, factors in peripheral blood mononuclear cells (PBMC) such as Th1 type transcription factor T-bet mRNA, Th2 type cytokine transcription factor GATA-3 mRNA and T-bet/GATA-3 before and after treatment, angiogenesis indexes [tumor vascular microdensity (MVD), serum vascular endothelial cell growth factor (VEGF), and angiopoietin 2 (Ang-2)] levels, quality of life (WHOQOL-BREF) score and the occurrence of acute radiation injury were compared between the two groups. 【Results】The total clinical effective rate of 70.83% (34/48) in the observation group, which was not statistically significant higher than that in the control group 60.42% (29/48) (P>0.05). After 3 weeks and 6 weeks of treatment, T-bet mRNA and T-bet/GATA-3 in PBMCs of the two groups were lower than before treatment, and GATA-3 mRNA was higher than before treatment. Compared with the control group, the difference was statistically significant (P<0.05). The levels of MVD, serum VEGF and Ang-2 in the observation group were lower than those in the control group after 3 and 6 weeks of treatment (P<0.05). The scores of physiological status, psychological status, social relationship, and physical environment in the WHOQOL-BREF scale in the observation group were higher than those in the control group after 6 weeks of treatment (P<0.05). 【Conclusion】The combination of Aidi injection and 3D-CRT in the treatment of advanced esophageal cancer has a good clinical effect and can reduce acute radiation injury. It can effectively improve cellular immune function in patients, and inhibit angiogenesis, which is of positive significance for improving the quality of life of patients.
