DOI: 10.3969/j.issn.1671-7171.2020.06.003
【Objective】To analyze the risk factors of bronchopulmonary dysplasia (BPD) in very (extremely) low birth weight premature infants. 【Methods】A total of 139 premature infants with birth weight less than 1500g were collected from the NICU of Changsha Hospital for Maternal&Child Health Care, from January 2017 to December 2019. According to whether be diagnosed as BPD, they were divided into the non-BPD group (n=81) and the BPD group (n=58). The differences between the two groups of premature infants in terms of maternal age, chorioamnionitis, gender, gestational age, and birth weight of the children were compared, and independent risk factors for BPD occurrence were analyzed by multivariate logistic regression.【Results】The proportion of mothers with chorioamnionitis and assisted conception in premature infants in the BPD group was higher than that in the non-BPD group, and the difference was statistically significant (P<0.05). The hospital stay of premature infants in the BPD group was higher than that in the non-BPD group. The proportion of males, invasive ventilator use, neonatal respiratory distress syndrome (NRDS), pulmonary surfactant (PS), neonatal pneumonia, and pharyngeal swab UU (ureaplasma urealyticum) positive tests in the BPD group were higher than those in non-BPD groups (P<0.05). Multivariate unconditional logistic regression analysis showed that gestational age and NRDS were the independent high-risk factors of very (extremely) low birth weight preterm infants.【Conclusion】The risk factors related to the development of bronchopulmonary dysplasia (BPD) in very (extremely) low birth weight premature infants were chorioamnionitis, birth weight, gestational age, NRDS, use of invasive ventilation and pharyngeal swab UU positive test. Of which, the gestational age and NRDS were independent risk factors.

LIU Dong-yan, YANG Jing
DOI: 10.3969/j.issn.1671-7171.2020.06.018
【Objective】To investigate the ultrastructural changes and pathogenesis of gastrointestinal tract in diabetic gastrointestinal disorders. 【Methods】A total of 50 SD rats were randomly divided into the control group (n=25) and the observation group (n=25). Animal model of diabetes combined with gastrointestinal dysfunction in the observation group were established with streptozotocin infection and diet-controlled method. Left ventricular end-diastolic diameter (LVEDD), end-systolic diameter (LVESD), systolic ventricular septal thickness (IVSs), left ventricular posterior wall thickness (LVPWs), and left ventricular ejection fraction(LVEF) were measured by echocardiography.Meanwhile, fasting blood glucose (FPG) levels were determined by blood glucose meter and fasting insulin (FINS) was measured by enzyme-linked immunosorbent assay (ELISA), while insulin resistance index (HOMA-IR) and insulin sensitivity index (ISI) were calculated by steady state model. Finally, rat gastrointestinal tissues of the two groups were taken and the ultrastructural changes were observed by transmission electron microscopy. 【Results】The levels of LVEDD, LVESD,FPG,FINS and HOMA-IR in the observation group were higher than those in the control group (P<0.05). The levels of IVSs, LVPWs, ISI and LVEF in the observation group were lower than those in the control group (P<0.05). The results of transmission electron microscopy showed that the ultrastructure of the gastric antrum and colon in the gastrointestinal tract tissue of laboratory diabetic rats was seen dramatic changes. While the ultrastructure of the small intestine changed relatively strongly. The lesions were mainly focused on smooth muscles and microvessels. No ultrastructural changes occurred in the nerve endings. At the same time, the arrangement of muscle cells was disordered, with larger cytosolic vacuoles within the muscle cells. Overall, no significant abnormalities were observed in the control rats. 【Conclusion】Diabetic rats are often accompanied by blood glucose level disorder and gastrointestinal dysfunction, which may be related to the ultrastructural changes of the gastrointestinal tract. Our research may provide some ideas for the treatment of diabetes.

DOI: 10.3969/j.issn.1671-7171.2020.06.019
【Objective】To investigate the value of tumor markers in preoperative evaluation of gastric cancer and indications for endoscopic treatment. 【Methods】A total of 79 patients with gastric cancer admitted to our hospital from March 2016 to March 2019 was divided into two groups according to the indication of endoscopic submucosal dissection (ESD) (n=38) and not according to the indication of ESD (n=51). Enzyme linked immunosorbent assay (ELISA) was used to detect the levels of serum carbohydrate antigen 19-9 (CA19-9), carbohydrate antigen 724 (CA724), alpha fetoprotein (AFP) and carcinoembryonic antigen (CEA), and to analyze the relationship between them and the clinical characteristics of patients, and to analyze the value of each index in ESD.【Results】 The positive rate of CA19-9 was related to TNM stage and lymph metastasis (P<0.05); the positive rate of CA724 was related to TNM stage, depth of invasion and distant metastasis (P<0.05); the positive rate of AFP was related to TNM stage and distant metastasis (P<0.05); the positive rate of CEA was related to TNM stage, depth of invasion and distant metastasis (P<0.05). The levels of CA19-9, CA724 and CEA in the non ESD group were significantly higher than those in the ESD group (P<0.05). ROC curve analysis showed that the difference was statistically significant (
Z=4.153, P=0.001;Z=3.375, P
=0.029); the difference was not statistically significant (P>0.05) when CA19-9 and CA724 were detected alone, and the sensitivity and specificity of combined detection were better than that of single detection (P<0.05). 【Conclusion】CA19-9, CA724, CEA and AFP detection have certain clinical application value in preoperative evaluation of gastric cancer patients and indications of endoscopic treatment. Combined detection can improve sensitivity and specificity, which is worthy of clinical reference.

YANG Hua, HOU Jing-fang
DOI: 10.3969/j.issn.1671-7171.2020.06.020
【Objective】To investigate the expression of matrix metalloproteinase-9 (MMP-9), matrix metalloproteinase 2 (MMP-2), filagin A (FLNa) and migration-inducible protein-7 (MIG-7) in colon cancer and reveal their correlation with clinicopathological factors and prognosis of colon cancer. 【Methods】A total of 120 patients with colon cancer who underwent surgical resection in our hospital were enrolled in the study. The postoperative cancer tissues and adjacent tissues were taken to measure the expression of MMP-9, MMP-2, FLNa and MIG-7 in colon cancer by immunohistochemical examination. Meanwhile their relationship with clinical pathological factors and prognosis were analyzed as well. 【Results】 The positive rates of MMP-9, MMP-2 and MIG-7 in cancer tissues were higher than those in adjacent tissues, while the positive rate of FLNa in cancer tissues was lower than that in adjacent tissues (P<0.05). The lymph node metastasis rate, phase Ⅲ ratio, vascular infiltration rate, and middle and low differentiation rate of MMP-9, MMP-2, and MIG-7 positive patients were higher than those with negative expression patients(P<0.05), however, the lymph node metastasis rate, phase Ⅲ ratio, vascular infiltration rate, and middle and low differentiation rate of FLNa positive patients were lower than those with negative expression patients (P<0.05). The median follow-up time was 26 months and 22 patients died. COX multivariate analysis showed that TNM stage, FLNa, MIG-7, MMP-2, MMP-9, tumor differentiation, and lymph node metastasis were independent factors of survival (P<0.05). 【Conclusion】MMP-9, MMP-2 and MIG-7 are highly expressed in colon cancer tissues, and FLNa is lowly expressed in colon cancer tissues. The expressions of FLNa, MIG-7, MMP-2 and MMP-9 are associated with tumor invasion and prognosis.

ZHANG Hong, CHEN Ying-ping, CHEN Sheng-wen
DOI: 10.3969/j.issn.1671-7171.2020.06.021
【Objective】To explore the effect of radial axial incision on corneal astigmatism after ultrasonic phacoemulsification. 【Methods】A total of 60 cases (with 62 eyes) of cataract patients were selected from July 2017 to November 2015 in our hospital for the study. Patients were divided into the group A and group B randomly, with 30 cases in each group. Both groups received a corneal incision in the certain part of cornea. The incision of the group A was at the maximum corneal refractive radial axial and the incision of the group B was at the traditional temporal side (right eye) or nasal side (left eye). At the same time, patients in the two groups had ultrasonic phacoemulsification and foldable intraocular lens implantation. The astigmatism and axial direction of patients were measured by CT200 corneal topography before the surgery and after the surgery (at 1 day, 1 week and 1 month). The surgically derived astigmatism was calculated according to the corneal astigmatism and axial direction. Then, the data of corneal astigmatism, surgically induced astigmatism and visual acuity were compared between the two groups. 【Results】Postoperative visual acuity was significantly improved in the two groups (P<0.05), while there was no statistically significant difference in visual acuity between the two groups at 1 day, 1 week and 1 month after the operation (P>0.05). The corneal astigmatism of the two groups was not statistically significant at 1 day after surgery (P>0.05), while the corneal astigmatism in group A at 1 week and 1 month after surgery was significantly less than that in group B, and the difference was statistically significant (P<0.05). At 1 week and 1 month after surgery, the surgically derived astigmatism in group A was significantly lower than in group B(P<0.05). 【Conclusion】The incision at the maximum curvature of the cornea i.e. radial axial incision in cataract surgery can significantly reduce the corneal astigmatism and surgically derived astigmatism, which has good clinical application value.

3
Level with Acute Asthma Severity and Related Indicators
LIU Yi-Jun, LIU Xiang
DOI: 10.3969/j.issn.1671-7171.2020.06.023
【Objective】To explore the relationship of serum 25(OH)D
3
level with disease severity and related indexes in patients acute asthma. 【Methods】A total of 72 patients with acute asthma diagnosed and treated in the Department of Respiratory Medicine in our hospital from March 2016 to February 2018 were enrolled as the observation group. According to the severity of asthma, patients in the observation group were further divided into three sub-groups, including 18 cases in the mild group, 28 cases in the moderate group, and 26 cases in the severe group. At the same time, 53 healthy volunteers in our physical examination center were selected as the control group. Serum 25-hydroxyvitamin D
3
(25(OH)D
3
) levels, high-sensitivity C-reactive protein (Hs-CRP) levels, serum calcium, serum inorganic phosphorus, and serum alkaline phosphatase were measured. Pulmonary function tester was used to measure the forced expiratory volume in one second of asthma patients as a percentage of the predicted value (FEV1%). The correlation of serum 25(OH)D
3
level with disease severity and related indexes in acute asthma patients was explored. 【Results】The 25(OH)D
3
level in the observation group was significantly lower than that in the control group (P<0.05), and the Hs-CRP level in the observation group was significantly higher than that in the control group (P<0.05). The levels of both FEV1% and 25(OH)D
3
in the mild group, moderate group and severe group decreased in turn (P<0.05), while the Hs-CRP levels in the mild group, moderate group and severe group increased in sequence(P<0.05). The differences were statistically significant (P<0.05). Pearson correlation analysis showed that 25(OH)D
3
in the observation group was positively correlated with FEV1% (r=0.755, P<0.05) and negatively correlated with Hs-CRP (r=-0.804, P<0.05). 【Conclusion】The serum 25(OH)D
3
level in patients with acute asthma is inversely proportional to the severity of the disease, namely the more severe the condition, the lower the 25(OH)D
3
level. At the same time, it is positively correlated with FEV1% value and negatively correlated with serum hs-CRP. The lower the serum 25(OH)D
3
level, the worse the lung function in patients with acute asthma and the more likely to induce inflammation.

DOI: 10.3969/j.issn.1671-7171.2020.06.030
【Objective】To observe the efficacy of meglumine adenosine cyclophosphat combined with musk Baoxin pill on the treatment of chronic pulmonary heart disease with heart failure. 【Methods】A total of 110 patients with chronic pulmonary heart disease and heart failure were enrolled. According to the random number table method, the patients were divided into the observation group (55 cases, with treatment of meglumine adenosine cyclophosphat combined with musk Baoxin pill) and the control group (55 cases, with meglumine adenosine cyclophosphat treatment only). Lung function indexes (FEV1, FEV1/FVC), blood gas analysis indexes (SaO
2
, PaO
2
), cardiac function indicators (LVEF, LVD, CO, LVS) before and after treatment were analyzed. The efficacy, occurrence of adverse drug reactions, and the mortality within 14 days were observed. 【Results】The total effective rate of the observation group at the end of treatment was 96.36%, which was higher than that of the control group (83.64%), the difference was statistically significant (χ
2
=4.946,P=0.026). After treatment, the parameters of SaO
2
, PaO
2
, FEV1 and FEV1/FVC in the two groups were higher than those before treatment. Of which, the SaO
2
, PaO
2
in the control group were higher than those in the observation group; and the difference was statistically significant (P<0.05). However, there were no significant differences in FEV1 and FEV1/FVC between the two groups after treatment (P>0.05). The LVEF of the control group after the treatment was higher than that before treatment, while both LVD and LVS of the observation group were lower than those of the control group, the differences were statistically significant (P<0.05). There was no significant difference in the incidence of adverse reactions and mortality rate between the control group and the observation group (P>0.05). 【Conclusion】Meglumine adenosine cyclophosphat combined with musk Baoxin pill has good clinical effect on the treatment of chronic pulmonary heart disease with heart failure, which improves the cardiac function and blood gas analysis indicators and reduces pulmonary artery pressure safely.

DOI: 10.3969/j.issn.1671-7171.2020.06.031
【Objective】To investigate the relationship between the expression of annexin A2 and HMGB1 and the pathological grade of gliomas and seizures. 【Methods】Eighty glioma tissue samples (observation group) and 40 surgical tissues (control group) were collected from the pathology center of our hospital. The expression of annexin A2 and HMGB1 protein in the two groups were detected by immunohistochemical staining, and the differences of annexin A2 and HMGB1 protein expression in patients with different histological grades, seizure frequency and seizure time were analyzed.【Results】The positive rates of annexin A2 protein and HMGB1 protein in the observation group were significantly higher than those in the control group (χ
2
=22.097,P<0.05;χ
2
=26.167,P<0.05). There was no significant difference in the positive expression rate of annexin A2 protein among the patients with different pathological grades in the observation group (P<0.05). The positive expression rate of HMGB1 protein in different pathological grades, seizure frequency and duration of epilepsy in the observation group was statistically significant (P<0.05).【Conclusion】The positive expression rate of annexin A2 and HMGB1 protein in the lesions of glioma patients increased significantly; overexpression of annexin A2 protein and HMGB1 protein is related to the increase of malignant degree of glioma and seizure.
