Effects of Multimode Analgesic Protocol on Analgesia, Hemodynamic and Inflammatory Factors in Patients with Gynecological Laparoscopy

Objective
】 To investigate the effect of multi-mode analgesia on analgesia, hemodynamics and inflammatory factors in patients undergoing gynecological laparoscopic surgery.【
Methods
】 A total of 136 patients who underwent gynecological laparoscopic surgery in our hospital were selected and divided into two groups by random. The control group was given the traditional single analgesic regimen, while the observation group was given the multi-mode analgesic regimen. The analgesic effect, hemodynamics and inflammatory factors between the two groups were compared. 【
Results
】 The dosage of fentanyl in the observation group was lower than that in the control group (P>0.05).However, no significant differences were shown in the dosage of opioid drugs and the days of hospitalization after operation between the two groups (P>0.05). The heart rate (HR) and level of mean arterial pressure (MAP) between the two groups before induction (T

) after anesthesia induction did not show significant difference (P>0.05).The levels of MAP and HR in the observation group were lower than those in the control group (P<0 05) immediately after operation (T

) and immediately after extubation (T

). There were no statistically significant differences in the levels of TNF-α and IL-6 between the two groups before surgery(P>0.05). While the levels of TNF-α and IL-6 in the observation group were lower than those in the control group at 30 minutes, 2 hours and 24 hours after operation (P<0 05). There was no significant difference in VAS scores between the two groups at 10 min after extubation (P>0 05). The scores of VAS in the observation group were lower than those in the control group at 6 h, 12 h and 24 h after operation (P<0.05). 【
Conclusion
】 The application of multi-mode analgesia in gynecological laparoscopic surgery can stabilize the level of MAP and HR, inhibit the expression of inflammatory factors, and alleviate the pain degree of patients.

Case Report of 4 Patients with Disorders of Sex Development (46, XY)

Objective
】 To investigate the etiology, diagnosis and treatment options of 46, XY, disorders of sex development (DSD). 【
Methods
】 Retrospective analysis of general information of physical examination, auxiliary examination and treatment information of the four pathologically confirmed 46, XY,DSD patients, who were admitted to Hunan Maternal and Child Health Hospital in 2018. 【
Results
】 Of the four patients with 46,XY,DSD, two patients were diagnosed with 46,XY, simple gonadal hypoplasia, and two were diagnosed with 46,XY,complete androgen insensitivity syndrome (CAIS). In general patients have normal growth and intelligence, however, they had primary amenorrhea, no female secondary sexual characteristics in adolescence, lack or sparse genitalia and axillary hair, underdeveloped breasts, and immature internal and external genitalia. For 46, XY, and DSD patients, it was recommended to perform whole exome sequencing test and SRY gene test.【
Conclusion
】 After the diagnosis of 46, XY, DSD is clear, gender assignment is one of the most important and difficult issues. Based on the diagnosis and the comprehensive assessment of a multidisciplinary teams, respect of the parents' participation and decision making, and protection of patient privacy, the experienced surgeons should clearly explain to patients and their families with possible surgical options and outcomes, patient's sex psychology, risk of gonadal tumors, possible reproductive potential, and hormone replacement therapy during adolescence.

Effects of Different Sedation Methods on Myocardial Injury Markers and Prognosis in Patients with Septic Mechanical Ventilation

Objective
】 To compare the effects of different sedation strategies on the levels of myocardial injury markers and prognosis in patients with sepsis myocardial injury. 【
Methods
】 A total of 72 patients with sepsis who met the criteria were randomly divided into two groups, i.e. the light sedation group and the deep sedation group, with 36 cases in each group. Patients in the light sedation group had RASS score (0~-2 points),whilepatients in the deep sedation group had RASS score (-3~-4 points). All patients were sedated with propofol. The values of brain natriuretic peptide (BNP), cardiac troponin I (cTnI) and troponin T (cTnT) in the two groups were compared between the first day to the third day after sedation.【
Results
】 The levels of BNP (pg/mL) gradually decreased from day 1 to day 3 in both groups.The differences between the groups were statistically significant (P<0.05).The levels of cTnT and cTnI decreased slowly from day 1 to day 3 in both groups, however, the difference was not statistically significant (P>0.05).The mechanical ventilation time and ICU stay time were compared between the two groups. The differences between the two groups were statistically significant (P<0.05).There was no significant difference in terms of 28-day case-fatality rates between the groups (P>0.05). 【
Conclusion
】 The Deep sedation has a stronger myocardial protection effect in patients with sepsis myocardial injury, while the light sedation can shorten the patient's mechanical ventilation time and reduce the patient's ICU hospitalization time. However, both can't change patients' 28-day case-fatality rates.

Effect of Medical Cooperation Team Intervention on Self-efficacy and Quality of Life of Patients with Chronic Diseases in Medical and Nursing Institutions

Objective
】 To explore the effect of medical cooperation team intervention on self-efficacy and quality of life of patients with chronic diseases in medical and nursing institutions.【
Methods
】 A total of 120 patients with chronic diseases admitted to our hospital were randomly divided into observation group and control group, 60 cases in each group. The control group was given routine nursing intervention for patients with chronic diseases, and the experimental group was intervened by medical cooperation team. After 3 months of intervention, the self-efficacy, medication compliance and quality of life were compared between the two groups.【
Results
】 Both groups completed the study with 58 cases in each group. After 3 months of follow-up, the symptom management scores of the two groups were significantly higher than those before the intervention, and the scores of symptom management, emotional control and communication function in the observation group were significantly higher than those in the control group (P<0.05). After 3 months of follow-up, the medication compliance of the two groups was significantly higher than that before the intervention. The medication compliance of the observation group was 96.55% (56/58), which was higher than 86.21% (50/58) of the control group, and the differences were statistically significant (P<0.05). After 3 months of follow-up, the scores of physical function, emotional function and cognitive function in the observation group were significantly higher than those in the control group (P<0.05).【
Conclusion
】 Medical cooperation intervention can promote the development of self-efficacy of patients with chronic diseases, improve medication compliance, and improve the quality of life of patients.

Establishment and Evaluation of a Rapid Detection Method for Neisseria Gonorrhoeae Nucleic Acid

Objective
】 To establish a Neisseria gonorrhoeae (NG) nucleic acid rapid test method and assess the analytic sensitivity and specificity and detection consistency with quantitative PCR method. 【
Methods
】 The rapid detection method of Neisseria gonorrhoeae nucleic acid was established by RPA-LFA; the sensitivity and specificity of the method were evaluated by using the national reference for Neisseria gonorrhoeae nucleic acid detection; sixty-one samples of clinical genital secretions were detected by fluorescence quantitative PCR kit and RPA-LFA respectively, and the consistency of the detection results was evaluated.【
Results
】 The sensitivity of RPA-LFA method was 10
4
cells/mL, the specificity was good, 10 different Neisseria gonorrhoeae strains could be detected, 10 non Neisseria gonorrhoeae strains could not be detected. The consistency between RPA-LFA method and fluorescent quantitative PCR method was high (Kappa=0.97), the positive detection rate of RPA-LFA method was 52.5% (32/61) and that of fluorescent quantitative PCR method was 54.1% (33/61). The sensitivity and specificity of rpa-lfa were 96.97% and 100.00% respectively, and the flow time of real-time PCR for single sample was 120±20min, significantly longer than 35±5min of RPA-LFA (P<0.05).【
Conclusion
】 The RPA-LFA method for rapid detection of Neisseria gonorrhoeae nucleic acid is successfully established. The method has high consistency with the fluorescent quantitative PCR method, and does not need complex and precise temperature control equipment. The interpretation of detection results is simple, which shortens the detection time of Neisseria gonorrhoeae nucleic acid and reduces the cost.

Relationship between Serum Vaspin Concentration and Coronary Heart Disease

Objective
】 To investigate the correlation between serum Vaspin level and severity of coronary heart disease (CHD). 【
Methods
】 A total of 120 patients with CHD in our hospital from January 2015 to December 2017 were selected as the observation group and 60 healthy volunteers in the same period as the control group. Serum Vaspin, triglyceride (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C) and fasting blood glucose (FPG) were detected in the two groups. The correlation between serum vaspin and the severity of coronary artery disease was analyzed. The high-risk factors of CHD were analyzed by logistic regression.【
Results
】 The serum TG, TC, HDL-C, LDL-C and FPG of the observation group were significantly higher than those of the control group, and the serum Vaspin level was lower than that of the control group (P<0.05). The serum Vaspin level of moderate stenosis group and severe stenosis group was lower than that of the mild stenosis group, and that of the severe stenosis group was lower than that of the moderate stenosis group (F=22.198, P<0.05). The Gensini score of the observation group was (60.55 ± 13.91), and there was a significant negative correlation between serum vaspin and Gensini score (r=-0.692, P<0.05). Logistic regression analysis showed that the decrease of serum Vaspin and HDL-C levels, and the increase of TC, TG and LDL-C levels were the independent risk factors for CHD (P<0.05).【
Conclusion
】 The serum Vaspin level was significantly lower in CHD patients, and the serum vaspin level is negatively correlated with the severity of coronary artery disease in CHD patients.

Investigation and Analysis of Novel Coronavirus Pneumonia Prevention and Control Training and Cognition among Radiological Technology Workers

Objective
】 To investigate the training and cognition of radiation science and technology nursing staff in the prevention and control of new coronavirus pneumonia.【
Methods
】 The novel coronavirus pneumonia treatment and protection plan, policy and regulations, protection requirements and standards, and protective operation theory were trained in two forms of radiology technology personnel in Xiangya Hospital of Central South University, including self-learning method (method 1), network self-study combined with on-site teaching method (method 2). Covid-19 prevention and control knowledge was investigated and assessed in the form of network questionnaire and field operation assessment.【
Results
】 The score of technical staff on the content of diagnosis and treatment plan was better than that of nursing staff, and the score of nursing staff's protective skill operation was better than that of technician, the difference was statistically significant (P<0.05). The results showed that the scores of diagnosis and treatment plan content of male technical nurses were better than that of women, and the score of female protective skill operation was better than that of male, the difference was statistically significant (P<0.05). The protective requirements and standards, protective skill operation and overall score of the technical nurses in training method 2 were higher than those in method 1, and the difference was statistically significant (P<0.05). There was no significant difference in the results of training and assessment among junior, intermediate and associate senior titles (P>0.05). There were statistically significant differences in diagnosis and treatment scheme category and overall score of technical nursing staff with different education background (P<0.05); the total score of postgraduate examination was significantly different from that of undergraduate and specialty (P<0.05). 【
Conclusion
】 Radiation science and technology nurses have a good grasp of the theoretical knowledge of prevention and control. Network training combined with on-site teaching can improve the mastery of prevention and control operation practice.

Clinical Effect of Thoracoscopic Non Cutting Obturator in the Treatment of Giant Pulmonary Bullae

Objective
】 To investigate the effect and safety of thoracoscopic non-cutting closure on the clinical outcomes of patients with giant bullous bullae. 【
Methods
】 Eighty-eight patients with pulmonary bullae in our hospital from February 2017 to February 2019 were selected and randomly divided into control group and observation group, with 44 cases in each group. The control group was given endoscopic linear cutting obturator operation, and the observation group was given thoracoscopic non cutting obturator operation. The operation related conditions (total operation cost, chest tube drainage, postoperative hospitalization, intraoperative blood loss, postoperative air leakage rate), postoperative blood gas level, clinical efficacy and incidence of dyspnea were compared between the two groups.【
Results
】 The operation related conditions (total operation cost, chest tube drainage, postoperative hospitalization and intraoperative blood loss) of the observation group were better than those of the control group (P<0.05). After treatment, there was no significant difference in blood gas level between the two groups (P>0.05); the incidence of dyspnea in the observation group was significantly lower than that in the control group (P<0.05).【
Conclusion
】 Thoracoscopic non cutting obturator surgery for giant pulmonary bullae can improve the operation conditions of patients, reduce the symptoms of dyspnea, and promote the recovery of patients, which is worthy of clinical application.

Predictive Value of Thrombelastography on Pregnancy Rate of Patients with Early Recurrent Spontaneous Abortion

Objective
】 To explore the application of thromboelastography to predict the pregnancy rate of patients with early recurrent spontaneous abortion (RSA). 【
Methods
】 A total of 72 RSA patients from January 2016 to December 2017 in our hospital were selected and randomly divided into group A and group B with 36 cases in each group. In addition, 40 normal pregnant women in the same period were selected as group C (the control group). Patients in the group A received both aspirin and low molecular weight heparin calcium, while patients in the group B only received aspirin treatment. RSA patients of both groups started taking the medication two months before the planned pregnancy and continued the medication until 1 month after pregnancy. Thromboelastogram in both group A and B was detected before therapy and after drug withdrawal, while in control group thromboelastogram was detected before pregnancy and 1 month after pregnancy. Thromboelastogram was followed up for 12 months. Thromboelastogram index and pregnancy loss rate were compared among groups before and after pregnancy. The correlation between thromboelastogram parameters and pregnancy loss rate were analyzed. 【
Results
】 Before pregnancy, the R value and alpha (α) angle of group A and B were significantly lower than those of Group C, while the MA value and K value of group A and B were significantly higher than Group C. The differences were statistically significant (P<0.05).The R value and α angle in group A were lower than those in group B, while K value in group A was higher than in group B. The differences were statistically significant (P<0.05), however, the MA value in group A and group B dii not show significant difference (P>0.05). After pregnancy, the R value and α angle of group A and group C were significantly higher than that of group B, and the K value and MA value were significantly lower than group B. The differences were statistically significant (P<0.05). The K value, alpha angle and MA value in group A and C had no significant difference (P>0.05). The pregnancy loss rates in group A,B and C were 16.7% (6/36), 38.9% (14/36) and 2.5% (1/40), respectively. The pregnancy loss rate showing group C < group A < group B, so there were significant differences among groups (P<0.05). According to the analysis of binary Logistic regression model, pregnancy loss rate was significantly correlated with MA value (
r
=0.614, P<0.05), but was not significantly correlated with the R value, K value and α angle. 【
Conclusion
】 Thromboelastography can detect the pre-thrombotic state of patients with early recurrent spontaneous abortion and predict the pregnancy loss rate . It is worth popularizing and applying in clinic.

Effect of Simvastatin Combined with Thrombolytic Enzyme on Cerebral Thrombosis Patients

Objective
】 To explore the effect of simvastatin combined with thromboplastin on cerebral thrombosis patients and its influence on coagulation factor level. 【
Methods
】 A total of 108 cerebral thrombosis patients admitted to our hospital from July 2016 to July 2018 were selected. According to simple random method, patients were divided into the control group (n=54) and the research group (n=54),The control group was treated with Agkistrodon halys antithrombotic enzyme. The research group was treated with simvastatin in addition to the drug of the control group. The clinical efficacy, changes of carotid intima-media thickness (CIMT), plaque area, coagulation factor, coagulation time, blood lipid index, stroke scale of the National Institutes of Health (NIHSS) and the occurrence of adverse reactions in both groups were compared before and after treatment.【
Results
】 The total effective rate of the research group was higher than that of the control group (92.59% vs 68.52%,

=18.494; P<0.05). Before treatment, there was no significant difference in CIMT, plaque area, coagulation factor, coagulation time, blood lipid index between the two groups (P>0.05). After treatment, the CIMT, plaque area, coagulation factors, coagulation time, low density lipoprotein cholesterol (LDL-C) and total cholesterol (TC) of the two groups were all lower than those before treatment (P<0.05), except that the high density lipoprotein cholesterol (HDL-C) levels of the two groups were higher than before treatment (P<0.05). Of which, the above indicators of the research group were better than those of the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).【
Conclusion
】 Simvastatin combined with antithrombotic enzyme can improve the therapeutic effect of cerebral thrombosis, reduce the level of coagulation factors and improve blood lipid metabolism and neurological function without significantly increasing adverse reactions.

Effectiveness of Trastuzumab in Combination with Total Thyroidectomy in the Surgical Treatment of Thyroid Cancer and Influence on Inflammatory Cytokines

Objective
】 To explore the effectiveness of trastuzumab in combination with total thyroidectomy in the surgical treatment of thyroid cancer an influence on serum inflammatory cytokines. 【
Methods
】 A total of 120 patients with thyroid cancer in our hospital were selected and divided into two groups. Patients in the control group received total thyroidectomy, while patients in the observation group received both thyroidectomy and trastuzumab treatment. The clinical effectiveness, hospital stay time after the operation, complications and serum level of inflammatory cytokines (CCL-18 and IL-6) were compared between the two groups. 【
Results
】 The rate of clinical effectiveness in the observation group was 98.3%(59/60),which was significantly higher than that in the control group [75.0%(45/60)](P<0.05). The average postoperative analgesia and hospital stay, VAS score at 12h and 24h after the operation in the observation group were all significantly lower than those in the control group(P<0.05). The levels of serum CCL-18 and IL-6 were significantly lower in the two groups after the treatment, while the observation group had significantly better results compared to the control group(P<0.05). There was no significant difference in the rate of complications between the two groups(P>0.05). 【
Conclusion
】 Trastuzumab in combination with total thyroidectomy in the surgical treatment of thyroid cancer has good clinical effect, which can lower serum level of CCL-18 and IL-6, shorten postoperative analgesia and hospital stay, decrease the risk of complications, It should be widely used in clinic.

Clinical Characteristics and Survival Status of Patients with Pulmonary Sarcomatoid Carcinoma

Objective
】 To investigate the clinical characteristics and survival status of patients with pulmonary sarcomatoid carcinoma (PSC). 【
Methods
】 The clinical data of 75 PSC patients diagnosed and treated in our hospital from January 2016 to January 2019 were retrospectively analyzed, and the clinical characteristics and survival status of PSC patients were analyzed. 【
Results
】 The main clinical symptoms were cough, hemoptysis, chest pain and chest tightness. TNM was divided into stage Ⅰ, stage Ⅱ, stage Ⅲ and stage IV. chest CT plain scan and enhanced scan showed that the tumor morphology was a single round or quasi round solid nodule or mass with an average diameter of (4.71±2.29) cm; there was no significant difference in the cumulative survival rate between patients with gender, age, smoking history, tumor location, surgery plus postoperative adjuvant chemotherapy, surgery alone, advanced radiotherapy and chemotherapy and no treatment; tumor size, lymph node metastasis, distant metastasis, TNM stage and early surgical treatment affected the cumulative survival rate.【
Conclusion
】 PSC has characteristic manifestations, the patient's survival status is poor, and the effect of radiotherapy and chemotherapy is not ideal. Early surgical treatment can be implemented to benefit the patient's survival status.

Effects of Mifepristone Combined with Guizhi Fuling Capsule on Serum Estrogen, Progesterone and Oxidative Stress Factors in Patients with Uterine Fibroids

Objective
】 To investigate the effect of mifepristone combined with Guizhi Fuling Capsule on serum estrogen, progesterone and oxidative stress factors in patients with uterine fibroids.【
Methods
】 A total of 66 patients with uterine fibroids in our hospital from January 2017 to February 2018 were selected and divided into observation group and control group according to different treatment methods. The control group was treated with mifepristone, and the observation group was treated with Guizhi Fuling Capsule on the basis of the control group. The clinical efficacy, serum levels of estradiol (E

), progesterone (P), matrix metalloproteinase-2 (MMP-2), MMP-9, malondialdehyde (MDA), superoxide dismutase (SOD), human epididymal protein 4 (HE4) and hemorheological indexes were observed before and after treatment. The recurrence rate of uterine fibroids was observed after 1 year follow-up.【
Results
】 Before treatment, there was no significant difference in uterine fibroid volume, MMP-2, MMP-9, P, E

, HE4, MDA, SOD levels between the two groups before treatment (P>0.05); after treatment, the uterine fibroid volume of the observation group was smaller than that of the control group, the levels of MMP-2, MMP-9, P, E
2
, HE4 and MDA in the observation group were significantly lower than those in the control group, and the SOD level was significantly higher than that in the control group (P<0.05); there was no difference in the hemorheological indexes between the two groups before treatment. After treatment, the whole blood high shear viscosity, low shear viscosity and plasma viscosity of the observation group were significantly lower than those of the control group (P<0.05). There was no significant difference in adverse reactions and recurrence rate after 3 months between the two groups (P>0.05). 【
Conclusion
】 Mifepristone combined with Guizhi Fuling Capsule in the treatment of uterine leiomyoma can reduce the volume of leiomyoma, reduce the expression levels of estrogen, progesterone and HE4, improve hemorheological indexes, reduce oxidative stress injury, and has high safety, which is worthy of clinical application.

Correlation Analysis of Anticardiolipin Antibodies with Severity of Hypertensive Disorder Complicating Pregnancy and Outcomes of Maternal and Infant

Objective
】 To investigate the effects of anticardiolipin antibodies (ACA) on the severity of hypertensive disorder complicating pregnancy (HDCP) and outcomes in maternal and fetal. 【
Methods
】 A total of 133 pregnant women with gestational hypertension who received childbirth in our hospital from January to February 2018 were selected as the observation group. According to the severity of the disease, the patients were divided into the hypertension group (n=56) and the preeclampsia group (n=77). During the same period, 50 normal pregnant women who gave birth in our hospital were enrolled in the control group. The levels of anticardiolipin antibodies (ACA), blood pressure, urine protein, maternal and infant outcomes premature delivery, fetal growth restriction, placental abruption, perinatal death were compared among groups. The relationship of anticardiolipin antibody levels (ACA-IgG, ACA-IgM and ACA-IgA) with blood pressure, urine protein, and maternal and fetal outcomes was analyzed. 【
Results
】 The ACA-IgG, ACA-IgM, diastolic blood pressure, systolic blood pressure and urine protein levels in the preeclampsia group and hypertension group were higher than those in the control group (P<0.05). The ACA-IgG, ACA-IgM, diastolic blood pressure, systolic blood pressure and urine protein levels in the preeclampsia group were higher than those in the hypertension group (P<0.05). There was no significant difference in ACA-IgA between the three groups (P>0.05). The incidence of premature delivery, fetal growth restriction, placental abruption, and perinatal death in the preeclampsia group was significantly higher than that in the hypertension group (P<0.05). The incidence of premature delivery, fetal growth restriction, placental abruption, and perinatal death in the hypertension group was significantly higher than that in the control group (P<0.05). ACA-IgG was positively correlated with systolic blood pressure, diastolic blood pressure and premature birth (r=0.317, 0.453, 0.521, P<0.05). ACA-IgM was positively correlated with premature delivery and placental abruption (r=0.650, 0.524, P<0.05). 【
Conclusion
】 The serum ACA level of pregnant women with hypertensive disorder complicating pregnancy is closely related to the severity of the disease and outcomes of maternal and infant. The detection of serum ACA level has certain reference value for predicting the outcome of poor maternal and child.

Clinical Significance of Serum Tumor Marker Detection in Lymphoma Patients

Objective
】 To investigate the application of serum tumor markers combined with bone marrow morphometry in patients with lymphoma. 【
Methods
】 A total of 52 patients with lymphoma in our hospital were selected as the observation group (including 22 cases with tumor infiltration and 30 cases with non-infiltration). At the same time, 52 healthy volunteers were selected as the control group. The levels of serum ferritin (SF), carcinoembryonic antigen 15-3 (CA15-3), carcinoembryonic antigen 125 (CA125), carcinoembryonic antigen (CEA), clinical data and cell morphology were compared between the two groups. 【
Results
】 (1) The levels of SF, CA15-3, CA125 and CEA in the observation group were higher than those in the control group (P<0.05). The levels of SF, CA15-3, CA125 and CEA in patients with tumor infiltration were higher than those with non-invasive infiltration (P<0.05). (2) Serum SF, CA15-3, CA125, CEA had no correlation with age, gender, WBC, HGB, PLT, lymphocyte proportion (P>0.05), while serum CA15-3, CA125 had correlation with infant lymphocyte proportion (P<0.05), serum SF, CA15-3, CA125, CEA had correlation with lymphoma B symptoms (P<0.05), serum SF and CA15-3 had correlation with lymphoma stage (P<0.05), serum SF, CA15- 3 and CA125 were correlated with international prognostic index (IPI) (P<0.05). 【
Conclusion
】 Serum tumor markers combined with bone marrow morphometry can be helpful to the diagnosis, staging, and prognosis of lymphoma, and provide a reliable reference for the diagnosis of bone marrow infiltration. It has important clinical significance.

Survery on Influencing Factors of Physical Activity in Elderly Patients with Chronic Obstructive Pulmonary Disease

Objective
】 To investigate the influencing factors of physical activities in elderly patients with chronic obstructive pulmonary disease (COPD). 【
Methods
】 A total of 180 COPD patients in our hospital from September 2016 to July 2018 were enrolled in this study. The physical activities of patients were recorded and evaluated. The factors affecting physical activity were analyzed by Logisitic regression analysis. 【
Results
】 The score of the physical activity scale (PACE) in 180 COPD patients were (72.02±23.59) points. There were no significant differences in PACE scores among patients with different gender, age, marital status, payment method of medical expenses and place of residence (P>0.05). However, there were significant differences in physical activity levels of patients with different oxygen therapy status, modified dyspnea index (mMRC) score and GOLD lung function grading (P<0.05). Logisitic regression analysis showed that oxygen therapy status, mMRC score, GOLD pulmonary function classification, duration of illness, SES6 score, MFI-20 score, and SF-36 score were the influencing factors of physical activity in COPD patients. Person correlation analysis showed that the patient's physical activity level was negatively correlated with the patient's illness time and MFI-20 score (r=-0.284, P<0.01; r=-0.492, P<0.01); it was positively correlated with SES6 score and SF-36 score (r=0.675, P<0.01; r=0.443,P<0.01).【
Conclusion
】 Oxygen therapy status, mMRC score, GOLD grading, sick time, SES6 score, MFI-20 score and SF-36 score are the influencing factors of physical activity in COPD patients. Therefore, in the process of pulmonary rehabilitation treatment, it is necessary to maintain a good oxygen therapy condition, reduce the patient's illness time and fatigue, enhance physical activity, and improve self-efficacy and quality of life.

Correlation between Serum Alkaline Phosphatase and Coronary Artery Calcification

Objective
】 To explore the relationship between serum alkaline phosphatase and coronary artery calcification. 【
Methods
】 A total of 154 hospitalized patients underwent 128-layer coronary artery CT scan in Suzhou traditional Chinese Medicine Hospital. According to the coronary artery CT calcification score, the severity of coronary artery calcification was evaluated, and the serum alkaline phosphatase level was measured. The patients were divided into calcified group and non-calcified group according to the calcification score. The clinical data and ALP level of the patients in the two groups were compared, and the correlation between coronary artery calcification score and serum ALP level, age and blood lipid index in the calcified group was further analyzed.【
Results
】 There were significant differences in age, smoking history, diabetes mellitus history, hypertension history and serum high density lipoprotein cholesterol (HDL-C) and ALP levels between calcification group and non calcification group (P<0.05); there were no significant differences in gender, drinking history, serum total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), triglyceride (TG) (P>0.05).In calcification group, calcification score was significantly positively correlated with age and ALP (P<0.01), and negatively correlated with HDL-C(P=0.016<0.05), but there was no correlation between calcification score and TC, TG, LDL-C (P>0.01).【
Conclusion
】 Serum alkaline phosphatase level is closely related to coronary artery calcification, which can be used as a biological index for the diagnosis of coronary artery calcification.

Analysis of Pathogenic Bacteria, Drug Resistance and Influencing Factors of CRBSI in MHD Patients

Objective
】 To investigate the pathogens, drug resistance and influencing factors of catheter-related bloodstream infections (CRBSI) in patients with maintenance hemodialysis (MHD). 【
Methods
】 A total of 96 MHD patients with CRBSI were enrolled in the study. Univariate analysis of risk factors for CRBSI, and multivariate analysis of independent risk factors for CRBSI in patients with MHD. Pathogens causing CRBSI and drug resistance to CRBSI pathogens were analyzed as well. 【
Results
】 Age, urea nitrogen (BUN), creatinine (SCr), globulin (GIO), albumin (ALB), hematocrit (HCT), hemoglobin level (HB), neutrophil percentage (NE), RBC and WBC were risk factors for CRBSI in patients with MHD (P<0.05). SCr, ALB, HB, NE, and RBC were independent risk factors for CRBSI in patients (P<0.05). A total of 82 pathogenic bacteria were isolated from 96 blood samples, with the isolation rate of 85.4%, of which Gram-negative bacteria (G
-
) accounted for 42.7% (35/82) and Gram-positive bacteria (G
+
) accounted for 57.3% (47/87). CRBSI pathogen resistance in Gram-positive bacteria did not show linezolid- and vancomycin-resistant strains. Staphylococcus epidermidis was highly resistant to erythromycin, tetracycline and oxacillin (≥40%); The methicillin-resistant Staphylococcus aureus showed very resistance to clindamycin, tetracycline, erythromycin and penicillin G. In Gram-negative bacteria, the resistance of Escherichia coli to sulfamethoxazole/trimethoprim, aztreonam, ceftriaxone, cefepime, and ceftazidime was 33.3%, while Pseudomonas aeruginosa was high resistance to sulfamethoxazole/trimethoprim, aztreonam and ceftriaxone, which was 100%.【
Conclusion
】 SCr, ALB, HB, NE and RBC are independent risk factors for CRBSI in patients with MHD. The drug resistance of various CRBSI pathogens is also high. Thus, it provides a basic knowledge for clinical prevention of CRBSI infections.

Prenatal Ultrasound Diagnosis of Congenital Intestinal Atresia or Stenosis

Objective
】 To summarize the clinical data of prenatal ultrasound diagnosis of small intestinal atresia (SBA) or small intestinal stenosis (SBS), so as to provide reference for prenatal diagnosis of fetal congenital SBA.【
Methods
】 A retrospective analysis of 208 cases of prenatal color Doppler ultrasound screening in our hospital from January 2013 to December 2017 was conducted. Fetal intestinal dilatation was found and autopsy was performed after operation or induction of labor. The clinical data of fetuses confirmed as SBA or SBS were analyzed statistically.【
Results
】 Among 208 fetuses with dilated bowel, 58 were confirmed as SBA or SBS by autopsy after operation or induction of labor. Among them, 18 cases (31.03%) were located in duodenum, 15 cases (25.9%) in jejunum and 25 cases (43.1%) in ileum. Among the 58 cases, 13 cases were complicated with other malformations and 4 cases (22.22%) were complicated with trisomy 21 syndrome. The induced labor rate of duodenal atresia or stenosis was 33.33%, the postpartum operation rate was 66.67%, and the cure rate was 91.67%; the induced labor rate of jejunal atresia was 46.67%, the postpartum operation rate was 53.33%, and the cure rate was 87.5%; the induced labor rate of ileal atresia was 48%, the postpartum operation rate was 52%, and the cure rate was 76.92%. There was no significant difference in the cure rate among the groups (P>0.05).【
Conclusion
】 Prenatal ultrasound plays an important role in screening congenital SBA or SBS. Because of its good surgical effect and high cure rate, prenatal diagnosis can reduce unnecessary induction of labor.

Effect of Different Time Enteral Nutrition on Severe Acute Pancreatitis

Objective
】 To compare the effect of enteral nutrition at different time on severe acute pancreatitis (SAP).【
Methods
】 One hundred and three patients with SAP in our hospital were randomly divided into early enteral nutrition group (EEN group,n=53) and delayed enteral nutrition group (DEN group,n=50). Enteral nutrition was started at 48 hours in the EEN group and 96 hours in the DEN group. The hospitalization time, abdominal pain relief time and spontaneous defecation time, mortality and complications of the EEN group and the DEN group were compared. The changes of nutritional indexes (albumin, prealbumin) and inflammatory indexes (C-reactive protein and procalcitonin) in peripheral blood of the two groups on the day 14th were compared.【
Results
】 The time of abdominal pain relief, the time of spontaneous defecation and the mortality rate during hospitalization in the EEN group were lower than those in the DEN group (P<0.05). The incidence of systemic inflammatory response syndrome (SIRS), intra-abdominal hypertension and pancreatic abscess complications in the EEN group was lower than that in the DEN group (P<0.05). Ten days after admission, the levels of albumin and prealbumin in the EEN group were lower than those in the DEN group, the difference was statistically significant (P<0.05); there was no significant difference in peripheral blood inflammatory indexes (C-reactive protein, procalcitonin) between the two groups (P>0.05).【
Conclusion
】 Early enteral nutrition support is conducive to the recovery of SAP patients, and can reduce the mortality and some complications.

Effects of Remifentanil on Anesthesia, Ramsay score and VAS score in Pregnant Women undergoing Caesarean Section

Objective
】 To explore effects of remifentanil on anesthesia, sedation score (Ramsay score) and visual analogue score (VAS score) in pregnant women undergoing caesarean section.【
Methods
】 A total of 100 cases of caesarean section pregnant women were randomly divided into two groups. Remifentanil anesthesia was used in the observation group and the conventional anesthesia was used in the control group. The anesthesia effect, Ramsay score, VAS score and postoperative recovery of the two methods were compared between the two groups. 【
Results
】 The excellent and good rate of the observation group was 98% (49/50), which was higher than that of the control group 86% (43/50).The difference was statistically significant (P<0.05). There was no significant difference between the two groups in the immediate postoperative Ramsay score and VAS score (P>0.05). However, both Ramsay score and VAS score at 24 hours after operation in the observation group were significantly better than those in the control group (P<0.05). The neonatal arterial blood gas indicators did not show any significant difference between the two groups. Three women in the observation group had nausea and vomiting, the incidence rate was 6% (3/50), and two patients in the control group had nausea and vomiting, the incidence rate was 4% (2/50). There was no significant difference between the two groups (P>0.05).【
Conclusion
】 Remifentanil has better anesthetic effect on caesarean section patients, which can effectively reduce the pain of patients and has no significant effect on the blood gas of newborns. Thus it is worthy of further clinical promotion.

Effect of Radiofrequency Ablation on Cardiac Function and Quality of Life in Patients with Atrial Fibrillation

Objective
】 To investigate the effect of radiofrequency ablation (RFA) on cardiac function and quality of life in patients with atrial fibrillation. 【
Methods
】 A total of 112 patients with atrial fibrillation in our hospital from December 2016 to March 2018 was selected and divided into drug group and RFA group according to different treatment methods. Patients in the control group were treated with drugs, while patients in the observation group were treated with RFA. The cardiac function indexes before and 3 months after treatment were compared between the two groups, and the quality of life of the two groups before and after 3 months of treatment was analyzed. 【
Results
】 After treatment, the maximum value of left atrial volume index (LAVI

) and minimum left atrial volume index(LAVI

)of the observation group were lower than those of the control group, and the left ventricular ejection fraction (LVEF) was higher than that of the control group, the difference was statistically significant (P<0.05); but there was no significant difference in the level of left ventricular diameter (LVD) between the observation group and the control group (P>0.05). Before treatment, there was no significant difference in quality of life scores between the two groups (P>0.05). After treatment, the scores of general health and quality of life in the observation group were higher than those in the control group, the difference was statistically significant (P<0.05), but there was no significant difference in the scores of mental health, social function and independent living ability between the two groups (P>0.05).【
Conclusion
】 RFA in the treatment of atrial fibrillation can improve the cardiac function, improve the general health and quality of life of patients, which is worthy of clinical reference.

Effect of Hypertensive Disorder Complicating Pregnancy on Thyroid Function during Pregnancy

】 To investigate the changes of thyroid function and the occurrence of thyroid dysfunction during pregnancy with hypertensive disorder complicating pregnancy (HDCP). 【
Methods
】 A total of 120 pregnant women with hypertensive disorder complicating pregnancy were selected as the observation group, including 48 cases of pregnancy-induced hypertension, 40 cases of pre-eclampsia, and 32 cases of eclampsia. Conclusion
】 The incidence of thyroid dysfunction in pregnant women with hypertensive disorder complicating pregnancy is high, especially in pregnant women with eclampsia. The screening of thyroid function in these patients should be paid attention to in clinical practice.

Clinical Effect of Percutaneous Coronary Intervention on Coronary Heart Disease and its Influence on Serum VEGF Level

Objective
】 To investigate the clinical effect of percutaneous coronary intervention (PCI) in the treatment of coronary heart disease (CHD) and its effect on serum vascular endothelial growth factor (VEGF).【
Methods
】 The clinical data of 92 patients with CHD admitted to our hospital from January 2017 to January 2019 were retrospectively analyzed, and they were divided into observation group and control group according to different treatment methods, with 46 cases in each group. The control group received conservative treatment, while the observation group received PCI on the basis of the control group. The treatment efficiency, cardiac function indexes (LVESD, LVEDD, LVEF, BNP), VEGF levels before and after treatment and the incidence of adverse cardiovascular events were compared between the two groups.【
Results
】 The effective rate of the observation group was 95.65% (44/46), which was significantly higher than 80.43% (37/46) of the control group (χ
2
=5.061,P=0.023<0.05). After treatment, LVESD, LVEDd and serum BNP levels in the two groups were lower than those before treatment, and LVEF was higher than that before treatment, and the differences between the two groups were statistically significant (P<0.05). Immediately after operation, the serum VEGF level of the observation group was higher than that of the control group, and the serum VEGF level of the observation group was lower than that of the control group 48 h and 1 week after treatment, the difference was statistically significant (P<0.05). After 3 months of follow-up, the incidence of adverse cardiovascular events in the observation group was 0, which was lower than 15.22% (7/46) of the control group (χ

Conclusion
】 CHD patients with PCI treatment has a good clinical effect, and can reduce the level of VEGF, can better improve the cardiac function of patients, worthy of clinical reference.

Surgical Treatment of ACI Patients with Forceps Clamp and its Effect on Neurological Rehabilitation

Objective
】 To investigate the surgical method of clamping thrombectomy for acute cerebral infarction (ACI) and the effect of nerve function recovery after operation. 【
Methods
】 The clinical data of 117 patients with ACI treated in our hospital from June 2016 to June 2018 were retrospectively analyzed. According to the different surgical methods, they were divided into observation group (treated with clamp thrombectomy,n=50) and control group (treated with traditional stent thrombectomy,n=67). The times of thrombectomy, success rate of single thrombectomy, operation time, recanalization rate of infarcted vessels were compared between the two groups. The NIHSS score before and after operation, the prognosis of 3 months after operation and the operation complications were compared between the two groups.【
Results
】 After 1 month and 3 months, NIHSS scores of the two groups were lower than those before operation (P<0.05). 3 months after operation, the good prognosis rate of the observation group was 68.00% (34/50), which was higher than 59.70% (40/67) of the control group, but the difference was not statistically significant (P>0.05). The incidence of postoperative complications in the observation group was 6.00% (3/50), including 2 cases of recurrent cerebral infarction and 1 case of intracranial hemorrhage. The incidence of postoperative complications in the control group was 8.96% (6/67), including 4 cases of recurrent cerebral infarction and 2 cases of intracranial hemorrhage. There was no significant difference between the two groups (χ

Conclusion
】 It is safe and feasible to remove thrombus in single time in the treatment of ACI. It can better restore the neurological function of patients and has important significance to improve the prognosis of patients.

Effect of Glucocorticoid Combined with Different Doses of Human Immunoglobulin on Serum TNF-α, IL-6 and CRP in Patients with Severe Drug Eruption

Objective
】 To investigate the effect of glucocorticoid combined with different doses of human immunoglobulin on serum tumor necrosis factor-α(TNF-α), interleukin-6 (IL-6) and C-reactive protein (CRP) in patients with severe drug eruption.【
Methods
】 Eighty-six patients with severe drug eruption were randomly divided into group A (n=30), group B (n=30) and group C (n=26). Group A was treated with glucocorticoid and medium dose human immunoglobulin, group B with glucocorticoid and high dose human immunoglobulin, and group C with glucocorticoid. Five days after the treatment, the clinical efficacy, serum TNF -α, IL-6, CRP levels of the two groups were compared. 【
Results
】 After treatment, the total effective rate of the group A was 93.33% (28/30), significantly higher than that of the group B and group C [76.67% (23/30) and 57.69% (15/26)], and the difference was significant (P<0.05); the total effective rate of the group B was higher than that of the group C, and the difference was significant (P<0.05). Before treatment, there was no significant difference in serum TNF-α, IL-6 and CRP levels among the three groups (P>0.05); after treatment, TNF-α, IL-6 and CRP levels were significantly lower than before treatment (P<0.05), and TNF-α, IL-6 and CRP levels in the group A were lower than those in the group B and C, while those in the group B were lower than those in the group C, and the difference was significant (P<0.05). After treatment, the antipyretic time, rash regression time and hospitalization time in the group A were significantly lower than those in the group B and C (P<0.05), while those in the group B were lower than those in the group C (P<0.05).【
Conclusion
】 The medium dose of human immunoglobulin combined with glucocorticoid can improve the levels of TNF-α, IL-6 and CRP in patients with severe drug eruption.

Expression and Significance of mir-99b-5p, hs CRP and UA in Patients with Schizophrenia

Objective
】 To study the changes and significance of serum miR-99b-5p, hs-CRP and UA levels in schizophrenic patients.【
Methods
】 From February 2015 to January 2018, 85 patients with schizophrenia (schizophrenia group), 76 patients with bipolar disorder (bipolar disorder group) and 78 healthy volunteers (control group) were prospectively selected. The serum levels of mir-99b-5p, hs CRP and UA were detected by RT-PCR, latex enhanced immunoturbidimetry and uric acid oxidase method. The value of serum mir-99b-5p, hs CRP and UA levels in the diagnosis of schizophrenia was analyzed. 【
Results
】 The serum levels of mir-99b-5p, hs CRP and UA in patients with schizophrenia were lower than those in patients with bipolar disorder (P<0.05). Serum mir-99b-5p level was negatively correlated with PANSS positive symptom score, general pathological score and total score (P<0.05) and positively correlated with negative symptom score (P<0.05); serum hs CRP level was positively correlated with PANSS positive symptom score, general pathological score and total score; serum UA level was positively correlated with PANSS positive symptom score and total score. Mir-99b-5p, hs CRP and UA were independent risk factors of schizophrenia. Serum mir-99b-5p and hs CRP had high value and specificity in the diagnosis of schizophrenia (P<0.05). 【
Conclusion
】 Mir-99b-5p and hs CRP can be used as markers of schizophrenia. The detection of serum mir-99b-5p, hs CRP and UA levels in patients with schizophrenia will be helpful for disease diagnosis and clinical treatment.

Application of Microcatheter Ipsilateral Double Contrast Angiography in Anterior Interventional Therapy of CTO Lesions

Objective
】 To investigate the application value of microcatheter ipsilateral double angiography in patients with chronic total occlusion (CTO) undergoing anterior percutaneous coronary intervention (PCI). 【
Methods
】 Retrospective analysis was made on the patients diagnosed as J-CTO by coronary angiography in our hospital from January 2018 to December 2019. All 30 patients were treated with forward technique, including 13 cases of ipsilateral double angiography with microcatheter (observation group), and 17 cases with routine bilateral angiography (control group). Coronary angiography, J-CTO score, X-ray exposure time, total PCI time, dosage of contrast agent, incidence of surgical complications, complications of arterial puncture, incidence of major adverse cardiovascular events (MACE) and secondary end-point events and ejection fraction (LVEF) were compared between the two groups.【
Results
】 There was no significant difference in J-CTO score, the number of coronary artery lesions, the proportion of left anterior descending artery (LAD), left circumflex artery (LCX), and right coronary artery (RCA) between the two groups (P>0.05). There was no significant difference in X-ray exposure time, total PCI time, operation complications and artery puncture complications between the two groups (P>0.05). The dosage of contrast medium in the observation group was lower than that in the control group (P<0.05). There was no significant difference in MACE events, secondary end-point events and LVEF between the two groups 30 days after operation (P>0.05). 【
Conclusion
】 In the process of antegrade PCI Treatment of CTO lesions, the application of microcatheter ipsilateral dual angiography can better guide the process of PCI operation, and does not increase the difficulty of operation. At the same time, it can also reduce the use of contrast media and reduce the risk of complications of arterial puncture.

Objective
】 To investigate the effect of coaxial micro incision phacoemulsification on corneal endothelial cell injury and repair.【
Methods
】 A total of 423 cataract patients who were treated in our hospital from January 2017 to December 2017 were selected and divided into two groups according to the size of incision, including 221 cases in standard incision group (3.0 mm) and 202 cases in micro incision group (1.8-2.2 mm);corneal endothelial cell density, coefficient of variation and morphology of corneal endothelial cells were examined by corneal endothelial microscope and compared before operation, 1 day, 1 week, 1 month and 3 months after operation.【
Results
】 Before operation, there were no significant differences in corneal endothelial cell density, coefficient of variation, proportion of hexagonal cells and central corneal thickness between the two groups (P>0.05). The corneal endothelial cell density at 7 days and 3 months after operation in the two groups were significantly higher than that before operation, the variation coefficient of corneal endothelial cells was significantly higher than that before operation, and the proportion of hexagonal cells was significantly lower than that before operation (P<0.05). At 7 days after operation, the density of corneal endothelial cells in the micro incision group was significantly higher than that in the standard incision group; the proportion of hexagonal cells in the micro incision group was higher than that in the standard incision group, and the difference was statistically significant (P<0.05); at 3 months after operation, there was no statistical significance in the corneal endothelial cell density and hexagonal cell (P>0.05). There was no significant difference in the coefficient of variation of corneal endothelial cells and central corneal thickness at 7 days and 3 months after operation (P>0.05).【
Conclusion
】 Compared with conventional incision, coaxial micro incision phacoemulsification has less damage to corneal endothelial cells and faster postoperative recovery, which is worthy of clinical application.
