1249 Analysis of Correlation between Mismatch Repair Protein Expression and Clinicopathological Features and Prognosis in Endometrial Cancer

【Objective】To investigate the correlation between the expression of mismatch repair (MMR) proteins (MLH1, PMS2, MSH2 and MSH6) and the clinicopathological features and prognosis of endometrial cancer.【Methods】The expression of MMR protein (MLH1, PMS2, MSH2, MSH6) in endometrial cancer tissues was analyzed by immunohistochemistry. The correlation between the expression of MMR protein and the clinicopathological characteristics of patients and the recurrence rate of disease 2 years after operation were compared .【Results】Among 143 cases of endometrial cancer, 39 cases (27.3%) had at least one expression deletion in MLH1, PMS2, MSH2 and MSH6. The expression of MMR protein was significantly correlated with family history, histological grade and vascular invasion of endometrioid adenocarcinoma (P<0.05), but not with age, histological type, FIGO stage, depth of myometrial invasion and lymph node metastasis (P>0.05).The recurrence rate of endometrial cancer patients with loss of MMR protein expression was 17.9% in 2 years after operation, which was not significantly different from that of patients with normal MMR protein expression (19.2%) (P>0.05).【Conclusion】The loss of MMR protein expression in endometrial cancer is significantly correlated with family history, high grade of endometrioid adenocarcinoma and vascular invasion. There is no significant difference in recurrence rate between patients with normal MMR protein expression and patients with deficient MMR protein expression in 2 years. The long-term prognosis needs further follow-up study.

1252 Therapeutic Effect of Voglibose Capsule Combined with Sitagliptin Phosphate Tablets in Elderly Patients with Type 2 Diabetes

【Objective】To investigate the efficacy of voglibose capsule combined with sitagliptin phosphate tablets in the treatment of elderly patients with type 2 diabetes mellitus (T2DM). 【Methods】A total of 186 patients with T2DM admitted to our hospital from February 2016 to May 2018 were enrolled. Patients were divided into the observation group and the control group by the random number table method, with 93 cases in each group. The control group received oral sitagliptin phosphate tablets and the observation group received oral voglibose capsules and sitagliptin phosphate tablets. The levels of fasting blood glucose (FPG) and 2 hours postprandial blood glucose (2hPG) were measured by glucose oxidase method before and after treatment, and the levels of HbA1c were analyzed by high performance liquid chromatography. The average daily blood glucose fluctuation (MAGE) and blood glucose standard deviation (SD) were calculated according to the blood glucose map in the Accu-Chek Performa Roche blood glucose meter, and the incidence of hypoglycemia was recorded. The incidence of adverse reactions in the two groups during treatment was recorded. 【Results】 Before treatment, there were no significant differences in the levels of FPG, 2 h PG and HbA1c between the two groups (P>0.05). After treatment, the levels of FPG, 2 h PG and HbA1c were significantly lower in the two groups (P<0.05). The values in the observation group were remarkably lower than those in the control group (P<0.05). The incidence of daily mean MAGE, SD and hypoglycemia in the observation group were significantly lower than those in the control group (P<0.05). The incidence of adverse reactions in the observation group and the control group were 9.68% and 10.75%, respectively. There was no significant difference between the two groups (P>0.05). 【Conclusion】 Voglibose capsule combined with sitagliptin phosphate tablets in the treatment of T2DM is better than single sitagliptin phosphate tablets. It shows that the blood sugar fluctuation is smaller, the incidence of hypoglycemia is lower, and it is safe and reliable.

1255 Clinical Analysis of Three Cases of Pulmonary Disease with Non-tuberculous Mycobacterium Tuberculosis Complicated with Talaromyces Marneffei

【Objective】To analyze the clinical features,characteristics of imaging ,pathogenic manifestations and related treatment of the non-tuberculous mycobaeteria(NTM)pulmonary disease with Talaromyces marneffei. 【Methods】The clinical data of 3 patients with NTM with Talaromyces marneffei admitted to Guangzhou Check Hospital from January 2017to June 2018 were retrospectively analyzed. 【Results】Of 3 patients,1 was male and 2 were female. All were 50~60 years old. All 3 patients had cough, sputum, and lymph node enlargement with ulceration. Among 3 patients, 2 had repeated bloody sputum,2 had skin and soft tissue abscess with ulceration,and 2 had severe lung infection and respiratory failure which needed transferred to ICU treatment. Three patients with sputum or bronchial lavage fluid were diagnosed as NTM by mycobacterium culture of sputum(2 cases of Mycobacterium avium,1 case of Mycobaeterium intracelhlare).In addition, 2 cases with skin soft tissue abscess and ulceration had pus culture with Marneffei blue-like bacteria growth, and 1 case had blood culture with Marneffei blue-like bacteria growth. All patients had no AIDS with normal T lymphocytes. All were diagnosed with NTM lung disease at the beginning, however, the symptomatic improvement was not obvious after anti-infection treatment. Finally, the diagnosis as Talaromyces marneffei was confirmed. And symptoms were much improved after anti-fungus infection treatment(1 by voriconazole,2 by amphotericin B). 【Conclusion】NTM lung disease combined with Talaromyces marneffei was rare in clinic practice. For the cases of conventional treatment failure NTM lung disease with recurrent skin abscess ulceration and lymph node enlargement plus ulceration, consideration should be given to the possibility of complicating Talaromyces marneffei.

1259 Effect of Laryngeal Mask Ventilation Combined with Intercostal Nerve Block Anesthesia in Modified Radical Mastectomy for Breast Cancer

【Objective】To investigate the anesthetic effect of laryngeal mask ventilation combined with intercostal nerve block anesthesia in patients undergoing modified radical mastectomy for breast cancer.【Methods】Seventy-four patients with radical mastectomy were randomly divided into two groups, 37 in each group. The control group was given laryngeal mask general anesthesia, while the observation group was given laryngeal mask ventilation combined with intercostal nerve block anesthesia. The changes of hemodynamics, stress response index and anesthesia safety before induction of anesthesia (T1), during skin incision (T2), 1 hour after skin incision (T3) and 5 minutes after removal of laryngeal mask (T4) were compared between the two groups.【Results】Systolic blood pressure (SBP) and heart rate (HR) at T3 and T4 in the observation group were lower than those in the control group (P<0.05); there was no significant difference in SBP and HR at four time points in the observation group (P>0.05); SBP and HR at T3 and T4 time points in the control group were higher than those at T1 and T2 time points (P<0.05). Cor and Ang-Ⅱ levels at T2, T3 and T4 in the observation group were lower than those in the control group (P<0.05); Ang-Ⅱ levels at T4 in the observation group were higher than those at T1 (P<0.05); Cor and Ang-Ⅱ levels at T2, T3 and T4 in the control group were higher than those at T1 (P<0.05). There was no significant difference in the incidence of perioperative adverse reactions between the two groups (P>0.05).【Conclusion】Laryngeal mask ventilation combined with intercostal nerve block anesthesia for modified radical mastectomy of breast cancer has less influence on intraoperative hemodynamics, which is helpful to reduce perioperative stress response of patients, and has high anesthesia safety. It is worth popularizing and applying.

1262 Pelvic Floor Ultrasound and Pelvic Floor Muscle Strength Assessment

【Objective】To explore the effect of different delivery methods on pelvic floor function by pelvic floor ultrasound and pelvic floor muscle strength evaluation. 【Methods】Sixty-three cases of primipara admitted to our hospital from June 2017 to June 2018 were selected as the research object. According to different delivery methods, the puerperas were divided into cesarean section group (n=33) and natural delivery group (n=30). Another 28 women who came to our hospital for health examination in the same period were selected as the control group. The changes of pelvic floor ultrasonography and pelvic floor muscle strength evaluation indexes were compared between the two groups, and the data were compared with those of the control group. 【Results】In resting state, pelvic floor ultrasonography showed that the bladder neck angle, bladder neck mobility and bladder rotation angle of natural delivery group and cesarean section group were significantly higher than those of control group, while those of natural delivery group were higher than those of cesarean section group, the difference was significant (P<0.05). Pelvic floor muscle strength evaluation results showed that natural delivery group and cesarean section group were significantly higher than those of natural delivery group (P<0.05). The persistent contraction force and maximum contraction force of pelvic floor muscle in the uterine delivery group were lower than those in the control group, while those in the natural delivery group were lower than those in the cesarean section group, and the difference was also statistically significant (P<0.05).【Conclusion】Pelvic floor ultrasonography and pelvic floor muscle strength assessment have certain predictive value for postpartum stress urinary incontinence and organ prolapse.

1264 Effect of Stent-assisted Coil Embolization in the Treatment of Large or Giant Intracranial Aneurysms:36 Case Report

【Objective】To investigate the effect of stent-assisted coil embolization in the treatment of large or giant intracranial aneurysms. 【Methods】The clinical data of 65 patients with large or giant intracranial aneurysms treated in our hospital were retrospectively studied. They were divided into observation group (stent-assisted coil embolization,n=36) and control group (simple coil embolization, n=29). The degree of embolism, prognosis and intraoperative complications were compared between the two groups.【Results】The degree of immediate embolization in the observation group was 61.11%, which was better than 34.48% in the control group (Z=2.212,P=0.027<0.05). There were 23 cases (63.89%) with mRS score of 0-1 in the observation group and 18 cases (62.07%) with good prognosis in the control group. There was no significant difference between the two groups (χ2=0.023,P=0.88>0.05). Fisher test analysis showed that there was no significant difference in aneurysm re-rupture, coil displacement, detachment and rotation, aneurysm ischemia and cerebral vasospasm between the observation group and the control group (P>0.05).【Conclusion】Stent-assisted coil embolization has good clinical effect and safety in the treatment of large or giant intracranial aneurysms, which is worthy of clinical application.

1267 Diagnostic Values of DNA Ploidy Detection in Pancreatic Space-occupying Lesions

【Objective】To explore the diagnostic values of DNA ploidy analysis in pancreatic space-occupying lesions. 【Methods】 A total of 148 patients with pancreatic space-occupying lesions from the Third Xiangya Hospital, Central South University were collected. EUS-FNA (ultrasound endoscopically guided final needle aspiration) were performed in all cases. DNA ploidy analysis system was used to analyze DNA content in nucleus. 【Results】EUS-FNA was successfully performed and adequate specimen for cytological diagnosis were obtained in 148 patients.Results showed that 64 patients with non-neoplastic lesion of the pancreas had no DNA ploidy abnormal cells and out of the 84 patients with pancreatic neoplastic lesions, 79 cases were found with DNA ploidy abnormal cells.The aneuploidy positive rate was (94.0%(79/84))in 84 pancreatic neoplastic lesion, which was significantly higher than non-neoplastic lesion of pancreas 0.0% (0/64). The difference was statistically significant between the two groups(P<0.05). 【Conclusion】DNA ploidy analysis can improve the positive detection rate of pancreatic space-occupying lesions. It has clinical guidance for the diagnosis of patients with pancreatic space-occupying lesions.

1270 Application of Three Internal Fixation Methods (Single, Double Steel Plate and Intramedullary Nailing) in Patients with Comminuted Distal Medial Femur Fractures

【Objective】To compare the application effect of three internal fixation methods in patients with comminuted distal medial femoral fracture: single steel plate, double steel plate and intramedullary nail.【Methods】A total of 82 patients with distal medial comminuted femoral fractures in our hospital from January 2013 to December 2017 were selected as the research subjects. According to the internal fixation materials, patients were divided into the single steel plate group (27 cases), the double steel plate group (34 cases), and the intramedullary nailing group (21 cases). , The femur angle variation were compared by the X-ray radiography examination of femoral angle at immediately after surgery and the last follow-up. The operation time, intraoperative blood loss, fracture healing status and time were recorded. 【Results】 The average operation time and the average blood loss in the double-plate group were significantly higher than those in the single-plate group and the intramedullary nail group, with statistically significant differences (P<0.05). The rate of malformation in the single plate group was significantly higher than that in the double plate group and the intramedullary nail group, and the difference was statistically significant (P<0.05). There were no significant differences in fracture healing time among groups (P>0.05). X-ray radiography examinations of femoral angle at immediately after surgery and the last follow-up showed that the difference of femoral angle changes in single plate group was significantly higher than that in double plate group and the intramedullary nail group, and the double plate group was significantly higher than that in the intramedullary nailing group. The differences were statistically significant (P<0.05). 【Conclusion】According to the comprehensive evaluation of surgical trauma and reduction ability maintenance, the therapeutic effect of the three types of internal fixation for the treatment of comminuted fracture of the distal femur were followed by intramedullary nail, double plate and single plate, except for those involving the articular surface.

1273 Value of Serum Cys-C , Scr and BNP in Predicting Cardiovascular Events in Chronic Kidney Disease Patients Treated with Maintenance Hemodialysis

【Objective】To investigate the value of serum cystatin C (Cys-C), serum creatinine (Scr) and brain natriuretic peptide (BNP) in predicting cardiovascular events in chronic kidney disease (CDK) patients treated with maintenance hemodialysis (MHD).【Methods】A total of 153 CDK patients who were treated with MHD in our hospital were followed up for 24 months. According to the cardiovascular events at the last follow-up, patients were divided into the cardiovascular event group (group A,n=51) and the non-cardiovascular event group (group B,n=102). Patients' clinical data, blood urea nitrogen (BUN), parathyroid hormone (iPTH), serum Cys-C, Scr and BNP were collected. Regression analysis was performed to identify independent risk factors for cardiovascular events. Receiver operating curve (ROC) was used to analyze the value of Cys-C, Scr and BNP in predicting cardiovascular events.【Results】Patients' age, dialysis time period, serum calcium (Ca), iPTH, uric acid, BUN, Cys-C, Scr and BNP in group A were significantly higher than those in group B (P<0.05). Cys-C, Scr and BNP were positively correlated with dialysis time period, serum Ca, iPTH, uric acid and BUN (P<0.05). Multivariate logistic stepwise regression analysis showed that iPTH, uric acid, Cys-C, Scr and BNP were independent risk factors for cardiovascular events in MHD patients (P<0.05). The areas under ROC curve of Cys-C, Scr and BNP in predicting cardiovascular events were 0.705, 0.614 and 0.739, respectively. The sensitivity was 79.2% for Cyc-C, 71.4% for Scr and 84.3% for BNP;And the specificity was 73.5%,67.3% and 86.5%, respectively.【Conclusion】Cys-C, Scr and BNP are independent risk factors for cardiovascular events in CDK patients with MHD and can predict the occurrence of cardiovascular events.

1276 Comparison of Diagnostic Value of DES and Low-dose MSCT in the Diagnosis of Traumatic Concealed Rib Fracture

【Objective】To compare the diagnostic value of the first dual energy subtraction (DES) and low-dose multi-slice spiral CT (MSCT) in the diagnosis of concealed/occult rib fractures in a short period of time. 【Methods】The imaging data of 118 highly suspected rib fracture patients admitted to our hospital from October 2017 to October 2018 were retrospectively analyzed. All patients underwent DES and low-dose MSCT within 3 days after trauma. The results were compared with gold standard image data which were collected with routine dose of MSCT after 4 ~ 8 weeks. 【Results】Of the 118 patients diagnosed with conventional MSCT, 147 had rib fractures, of which 88 were diagnosed as occult rib fractures. The sensitivity, specificity and coincidence rate of low dose MSCT in the diagnosis of occult rib fracture were 97. 40% (75/77), 81. 43% (57/70) and 89. 80% (132/147), respectively. The sensitivity of DES in the diagnosis of occult rib fracture was 96.25% (77/80), the specificity was 83.58% (56/67), and the coincidence rate was 90.48% (133/147).There was no significant difference in sensitivity, specificity and coincidence rate between the two methods (P > 0.05). 【Conclusion】 DES and low-dose MSCT have comparable diagnostic value in the diagnosis of occult rib fracture. We can choose different examination methods according to the patient's specific situation and the indication of the disease.

1279 Application of Annular Incision and Disconnection of Hernia Sac Neck in Laparoscopic Transperitoneal Hernioplasty

【Objective】To analyze the value of laparoscopic transperitoneal herniorrhaphy (LHR) with annular incision and disconnection of the neck of hernia sac. 【Methods】One hundred cases of inguinal hernia treated with LHR in our hospital from January 2017 to March 2019 were selected as the study subjects. The patients were randomly divided into the control group and the observation group with 50 cases in each group. The patients in the control group were treated with transperitoneal hernioplasty (TAPP) and the patients in the observation group were treated with " annular incision and disconnection of hernia sac neck ". The perioperative condition, pain score, complications and recurrence were compared between the two groups. 【Results】The operation time of the observation group was significantly shorter than that of the control group (P<0.05), the amount of bleeding during the operation was significantly less than that of the control group (P<0.05), and the damage of spermatic cord vessels and vas deferens was significantly less than that of the control group (P<0.05). There was no significant difference in nerve injury, time of getting out of bed and hospital stay between the two groups (P > 0.05). The VAS pain scores of the observation group at 6, 12 and 24 hours after operation were significantly lower than those of the control group (P<0.05). There was no significant difference in subcutaneous hematoma, foreign body sensation, recurrence rate and chronic pain between the two groups (P>0.05). The incidence of scrotal effusion in the observation group was significantly lower than that in the control group (P<0.05). 【Conclusion】"Annular incision and disconnection of hernia sac neck" can shorten the operation time, reduce the amount of bleeding during operation, reduce the injury of spermatic cord and vas deferens, and relieve the pain of patients after operation. Compared to conventional TAPP, the operation is simpler and thus it is particularly suitable for grass-roots hospitals.

1282 Clinical effect of Hepatic Artery Injection of Cinobufotalin Combined with Hyperthermic Perfusion Chemotherapy in the Treatment of Primary Hepatocellular Carcinoma

【Objective】To investigate the clinical effect of cinobufotalin injection through hepatic artery combined with hyperthermic perfusion chemotherapy in the treatment of primary hepatocellular carcinoma. 【Methods】Seventy-six patients with primary liver cancer were randomly divided into two groups, 38 cases each. The control group received transcatheter arterial chemoembolization (TACE) after transcatheter arterial infusion chemotherapy, while the observation group received hepatic artery injection of cinobufotalin combined with transcatheter arterial infusion chemotherapy (TACE) three times, one month apart. The efficacy, incidence of adverse reactions, PFS and OS of the two groups were compared.【Results】The disease control rate of the observation group was 63.16% (24/38), higher than that of the control group 41.67% (20/38), but there was no significant difference between the two groups (P>0.05). The incidence of pain in the observation group was 36.84% (14/38), significantly lower than that in the control group 63.16% (24/38), and there was a significant difference between the two groups (P<0.05). The PFS and OS of the observation group were (12.01±2.89) months and (19.32±5.48) months, which were significantly higher than those of the control group (10.03±3.00) months and (15.43 ±4.82) months, and there were significant differences between the two groups (all P<0.05). 【Conclusion】The anti-tumor effect of cinobufotalin intrahepatic artery injection combined with hyperthermic perfusion chemotherapy for primary hepatocellular carcinoma is better, the survival time of patients is prolonged and the pain is alleviated, which is worthy of clinical promotion.

1285 Effect of Acupuncture and Moxibustion on Rheumatoid Arthritis and Changes of Blood Lipid Level before and after Treatment

【Objective】To explore the clinical efficacy of acupuncture and moxibustion in the treatment of rheumatoid arthritis and its influence on blood lipid level.【Methods】One hundred and fifty patients with rheumatoid arthritis were randomly divided into acupuncture group and control group. The therapeutic effect was compared between the two groups. The changes of anti-streptococcal hemolysin O antibody (ASO) and rheumatoid factor (RF) levels, triglyceride (TG) and cholesterol (TC) levels before and after treatment were compared. 【Results】The total effective rate of acupuncture group was 90.67% (68/75), which was significantly higher than 76.0 %(57/75) of the control group, and there was a significant difference between the two groups (P<0.05). There was no significant difference in VAS score, ASO, RF index and blood lipid level between the two groups before treatment (P>0.05); after treatment, the VAS score, ASO, RF level in the acupuncture group were significantly lower than those in the control group (P<0.05); the TC and TG levels in the acupuncture group were also significantly lower than those in the control group (P<0.05).【Conclusion】Acupuncture and moxibustion treatment of rheumatoid arthritis has a significant therapeutic effect. It can improve the clinical symptoms of patients, at the same time, it can promote the improvement of blood lipid level. It is suitable for clinical treatment.

1288 Microsurgical Treatment of Parasagittal and Parafalx Meningiomas

【Objective】To analyze the efficacy and safety of microsurgery for parasagittal and parafalx meningiomas, and to summarize the clinical experience.【Methods】The clinical data of 95 patients with parasagittal and parafalx meningiomas admitted to our department from April 2010 to April 2018 were analyzed retrospectively. The analysis included baseline data, imaging examination, surgical method, resection effect, follow-up outcome, etc. Based on this, the effect and safety of microsurgery were summarized, and the clinical experience was discussed.【Results】The total resection rate of 95 cases (grade Ⅰ~Ⅲ) was 92.63%. Postoperative intracranial hemorrhage occurred in 2 cases and recovered after craniotomy. There was no recurrence in grade Ⅰ resection, 6 cases (10.71%) in grade Ⅱ resection, 5 cases (83.33%) in grade Ⅲ resection and 6 cases (100.00%) in grade Ⅳ resection.【Conclusion】The total resection rate of parasagittal sinus and parafalx meningiomas should be confirmed. Grade I resection should be achieved to reduce the risk of recurrence and improve the quality of prognosis.

1291 Value of Cardiac Ultrasound-related Parameters in the Evaluation of Postoperative Recurrence of Atrial Fibrillation Radiofrequency Ablation

【Objective】To study the value of cardiac ultrasonography-related parameters in the evaluation of recurrence after radiofrequency ablation of atrial fibrillation (RAOAF). 【Methods】A total of 66 patients with RAOAF who were treated in our hospital from February 2016 to January 2018 were enrolled in the recurrence group (observation group) and the non-recurrent group (control group). Cox regression analysis were performed to reveal the influencing factors of recurrent atrial fibrillation after RAOAF.【Results】 Of the 66 patients, 32 had recurrence, accounting for 48.48%. The left atrial size and systolic peak velocity of the mitral annulus (s') in the observation group were significantly greater than those of the control group, while the peak velocity of the mitral annulus at late diastolic phase (A'), and the peak of early diastolic velocity (E') were significantly smaller than those of the control group. The differences were statistically significant (P<0.05). There were no significant differences in left ventricular ejection fraction (LVEF), left ventricular size, left ventricular posterior wall thickness, and right ventricular anteroposterior diameter between the two groups (P>0.05). Multivariate Cox regression analysis showed that A′peak value was an independent risk factor of recurrence after RAOAF. 【Conclusion】Among the parameters related to cardiac ultrasound, A′peak value has certain reference value for the evaluation of postoperative recurrence of RAOAF, and is worthy of special attention.

1293 Association of Clinical Efficacy of Decitabine with Immune Lymphocyte Counts in Elderly Patients with Myelodysplastic Syndrome

【Objective】To investigate the association of decitabine efficacy with immune lymphocyte counts in elderly patients with myelodysplastic syndrome. 【Methods】The clinical data of 63 elderly patients with myelodysplastic syndrome treated in our hospital from March 2014 to May 2016 were retrospectively analyzed. According to the absolute value of lymphocyte counts on the 28th day after the first treatment of decitabine (DAC), patients were divided into the high lymphocyte group (H-Lym group, 27 cases, absolute lymphocyte count≥1.2×109/L) and the low lymphocyte group (L-Lym group, 36 cases, absolute lymphocyte count<1.2×109/L). The differences in baseline data, total response rate (ORR), and progression-free survival (PFS) between the two groups were compared, and the risk factors for differences in PFS between the two groups were analyzed. 【Results】 (1) There were no significant differences in gender, age, IPSS classification, complete blood count, karyotype and treatment-related death between the two groups (P>0.05). Compared to the L-Lym group, the proportion of patients with ≥10% of the original cells in the H-Lym group was significantly higher (P<0.05). (2) At the time of diagnosis, there was no significant difference in lymphocyte count between the two groups (P>0.05). After receiving the first course of DAC, the lymphocyte counts in the H-Lym group were not significantly different from those in the diagnosis (P>0.05). However, the lymphocyte counts in the L-Lym group after the first course of DAC treatment were significantly lower than those in the diagnosis (P<0.05). (3) The ORR of the H-Lym group was 81.48%, which was significantly higher than that of the L-Lym group (38.89%) (P<0.05). Patients in the relatively high-risk group were more likely to respond to DAC therapy than patients with a relatively low-risk group of IPSS (P<0.05). (4) The median PFS value of patients in the H-Lym group was 10.7 months (95% CI, 7.9~13.5 months), which was significantly higher than that of the L-Lym group 8.1 months (4.3 to 11.9 months) (P<0.05). Correlation analysis showed that high lymphocyte count was a risk factor for difference in PFS between the two groups. 【Conclusion】Patients with absolute lymphocyte count≥1.2×109/L on the 28th day after the first treatment of DAC have higher total response rate (ORR) and progression-free survival (PFS), which can be used for early prediction of DAC response.

1297 Effect of Dexmedetomidine Anesthesia on Postoperative Cognitive Function and Hemodynamic Parameters in Elderly Patients undergoing Radical Resection of Colorectal Cancer

【Objective】To investigate the effect of dexmedetomidine(Dex) anesthesia on postoperative cognitive function and hemodynamic parameters in elderly patients undergoing radical resection of colorectal cancer. 【Methods】A total of 108 elderly patients with colorectal cancer were divided into two groups, the observation group (n=55) and the control group (n=53). Dex was infused at a rate of 0.6μg/(kg?h) until 30 min before the end of surgery after a loading dose of Dex 1.0 μg/kg was infused at 15 min after anesthesia induction in the observation group, while the control group received equal volume of normal saline. The stress response and hemodynamic changes of the two groups were compared before, after, during and after extubation . The incidence of cognitive function and cognitive impairment (POCD) were compared between groups before anesthesia as well as 6h, 12h, 24h and 72h after anesthesia. 【Results】After anesthesia, the stress level of the observation group was lower than that before anesthesia (P<0.05). The index of the two groups was significantly different (P<0.05).The heart rate, blood pressure, and left ventricular ejection fraction index of the observation group were significantly decreased after anesthesia. The hemodynamic index of the observation group was significantly better than that of the control group (P<0.05). The cognitive function MMSE scores of the observation group was significantly better than those of the control group at 6h, 12h, and 24h after anesthesia (P<0.05). There was no significant difference in the MMSE scores between the two groups at the 72h after anesthesia (P>0.05); The incidence of COPD in the observation group was significantly lower than that in the control group at 6h and 12h after anesthesia (P<0.05). 【Conclusion】The Dex anesthesia is beneficial to stabilize the stress response and hemodynamic changes during anesthesia induction period in elderly patients undergoing rectal cancer resection. It also promotes postoperative cognitive function recovery, which is worthy of clinical promotion.

1300 Analysis of Correlation between Shortening of Electrocardiogram QT Interval and Ventricular Arrhythmia in Variant Angina Pectoris

【Objective】To explore the correlation between electrocardiogram QT interval and ventricular arrhythmia in patients with variant angina pectoris. 【Methods】The clinical data of 74 patients with variant angina pectoris in our hospital were retrospectively analyzed. According to the corrected QT interval between attack and non-attack, the patients were divided into observation group (shortened QT interval, n=34) and control group (normal QT interval, n=40). The ventricular premature systolic load, the proportion of ventricular tachycardia, the value of QT and the therapeutic effect were compared between the two groups.【Results】The proportion of premature ventricular systolic load and ventricular tachycardia in the observation group was higher than that in the control group, while the value of QT and LVEF were lower than those in the control group (P<0.05). The effective rate of the observation group was 52.94% (18/34), which was significantly lower than that of the control group 80.00% (32/40). There was a significant difference between the two groups (χ2=6.141,P=0.013<0.05). Pearson linear correlation analysis showed that shortened QT interval was significantly correlated with ventricular arrhythmia in patients with variant angina pectoris (r=0.46,P=0.035<0.05). 【Conclusion】The proportion of premature ventricular systolic load and ventricular tachycardia increase significantly in patients with variant angina with shortened QT interval, and the effect of antiarrhythmic therapy is poor. Clinicians should give timely and effective treatment as soon as possible according to the results of relevant electrocardiogram examination.

【Objective】To investigate the correlation between the expression of bone morphogenetic protein-2 (BMP-2) in serum and synovial fluid and KL grade and clinical evaluation of osteoarthritis disease activity index (WOMAC) score in patients with osteoarthritis (OA).【Methods】Fifty patients with OA were selected as the observation group and 50 healthy people were randomly selected as the control group. The expression of BMP-2 protein in serum and synovial fluid of OA patients and serum of healthy people was detected so as to analyze the correlation between BMP-2 protein expression in serum and synovial fluid of patients with OA and KL grade and WOMAC score.【Results】The expression level of BMP-2 protein in serum of OA patients was significantly higher than that of healthy people (P<0.05). The expression of BMP-2 protein in serum and synovial fluid of OA patients increased with the increase of KL classification among different KL classification subgroups (P<0.05). At the same time, BMP-2 expression in synovial fluid was significantly higher than that in serum in patients with OA and the same KL grade (P<0.05). The expression of BMP-2 protein in serum and synovial fluid of patients with OA was positively correlated with OA KL grade and WOMAC score.【Conclusion】The expression level of BMP-2 in serum and synovial fluid of OA patients is significantly higher than that of healthy people, and with the increase of KL classification, the expression level of BMP-2 in serum and synovial fluid increase gradually.The expression level of BMP-2 in serum and synovial fluid of patients with OA was positively correlated with KL grade and WOMAC, which may be valuable in evaluating the severity of OA.

1305 OCT technique posterior pole asymmetry examination for early diagnosis of glaucoma

【Objective】To explore the value of post-polar asymmetry examination of optical coherence tomography (OCT) in the early diagnosis of glaucoma. 【Methods】From January 2015 to January 2018, 21 patients (31 eyes) with suspected open angle glaucoma (SOAG) and 23 patients (33 eyes) with primary open angle glaucoma (POAG) were selected as the study subjects. At the same time, 30 patients (49 eyes) with visual acuity and healthy subjects were selected as the control group (CON group). The parameters of visual field index, retinal nerve fiber layer thickness (RNFL) and retinal thickness at the posterior pole were compared among the three groups. The correlation between mean visual field defect (MD) and asymmetry of retinal thickness at the posterior pole in POAG group and SOAG group was analyzed.【Results】The mean thickness of RNFL in temporal side, RNFL in nasal side, RNFL in upper part and RNFL in lower part of CON group were significantly higher than those in POAG group (P<0.05). The mean thickness of RNFL in lower part of the CON group was significantly higher than that in the SOAG group (P<0.05). The posterior retina thickness of the CON group was significantly higher than that of the POAG group (P<0.05). There was no significant difference between the SOAG group and the CON group (P>0.05). The visual field MD of the CON group, the POAG group and the SOAG group were (1.43±1.03) dB, (-13.18±1.12) dB, (-1.69 ±1.04) dB, respectively. Spearman correlation analysis showed that D2, D3 and D5 in the SOAG group were correlated with visual field MD, while parameters in the POAG group were not correlated with visual field MD.【Conclusion】OCT can detect the asymmetric changes of retinal thickness in the posterior pole of patients with SOAG, which is helpful for the diagnosis of early open-angle glaucoma.

【Objective】 To investigate the effects of general anesthesia combined with thoracic paravertebral nerve block (TPVB) on postoperative recovery, inflammatory response and cognitive function in patients undergoing radical thoracoscopic lung cancer mastectomy. 【Methods】A total of 80 patients with NSCLC who underwent thoracoscopic radical resection of lung cancer in our hospital from April 2014 to April 2019 were randomly assigned to the observation group and the control group according to the patient's admission time. The observation group received general anesthesia combined with TPVB while the control group underwent general anesthesia. The operation time, single lung ventilation time, surgical bleeding volume, recovery time, extubation time, propofol dosage, remifentanil dosage, serum interleukin-6 (IL-6),interleukin-10 (IL-10), tumor necrosis factor-α (TNF-α), cortisol (Cor), adiponectin (ADP), S100 protein, and cognitive dysfunction (PCOD) were compared between the two groups. 【Results】There were no significant differences in the operation time, single lung ventilation time and surgical bleeding volume between the observation group and the control group (P>0.05). The recovery time of the observation group was longer than that of the control group (P<0.05). The extubation time, the dosage of propofol and remifentanil of the observation group were significantly lower than those of the control group (P<0.05). There were no significant differences in serum levels of IL-6, IL-10, TNF-α, Cor, ADp and S100 protein between the two groups before surgery (P>0.05). At 24 hours after operation, the serum levels of IL-6, IL-10, TNF-α, Cor and S100 protein in the observation group were lower than those in the control group (P<0.05). While the ADP level the observation group was higher than in the control group (P<0.05) at 24 hoours after surgery, PCOD incidence rate in the observation group was 10.00%, which was lower than that in the control group (31.11% ),and the difference was statistically significant (P<0.05).【Conclusion】General anesthesia combined with TPVB can prolong the recovery time, reduce the time of extubation and the amount of anesthetic drugs, reduce the inflammatory response and the incidence of PCOD in patients undergoing thoracoscopic radical resection of lung cancer.

1311 Effects of Dex Combined with Etomidate on Cognitive and Cerebral Oxygen Metabolism in Elderly Patients Undergoing Cardiac Surgery

【Objective】To investigate the effects of dexmedetomidine (Dex) combined with etomidate on cognitive function and cerebral oxygen metabolism in elderly patients undergoing open heart surgery. 【Methods】One hundred and twenty elderly patients undergoing open heart surgery in our hospital were randomly divided into group A and group B with 60 cases in each group. After tracheal intubation, the group A was given a loading dose of Dex 0.5μg/kg for 10 minutes, then Dex 0.3μg/kg/h was infused to the chest, while the group B was given the same amount of saline. The cerebral arteriovenous oxygen content difference (Da-jvO2), cerebral oxygen uptake rate (CERO2), S-100βprotein, neuron-specific enolase (NSE) and the score of Mini-Mental State Scale (MMSE) and incidence of adverse reactions were compared between the two groups before anesthesia induction (T0), after sternotomy (T1), 30 minutes after cardiopulmonary bypass (T3) and at the end of operation(T4).【Results】There was no significant difference between the two groups at T0 (P>0.05); the values of da-jvO2 and CERO2 in the group A at T1, T2 and T3 were significantly lower than those in the group B (P<0.05); the levels of S-100βprotein and NSE in the group A at 24h and 72h after operation were significantly lower than those in the group B (P<0.05); and the scores of MMSE in the group A at 24h after operation were significantly higher than those in the group B (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】The application of Dex combined with etomidate in elderly patients undergoing open heart surgery can reduce cerebral oxygen metabolism and reduce the occurrence of postoperative cognitive dysfunction.

1314 Effect of Acceptance and Commitment Therapy on the Negative Emotions and Coping Styles of Patients with Depression

【Objective】To investigate the effect of acceptance and commitment therapy on the negative emotions and coping styles of patients with depression. 【Methods】A total of 2659 patients with depression from January 2015 to July 2017 were randomly divided into the observation group (n=1350) and the control group (n=1309). Patients in both groups were given antidepressant and anti-anxiety drugs. The control group received conventional nursing interventions including health education, psychological nursing and cognitive interventions. The observation group received acceptance and commitment therapy in addition to treatments in the control group. After 6 months of follow-up, negative emotion and coping styles were assessed using the Self-rating Anxiety Scale (SAS), the Self-rating Depression Scale (SDS) and the Coping Style Questionnaire (CSQ). 【Results】The SAS and SDS scores in the observation group were significantly lower than those in the control group (P<0.01). The ability of seeking help, problem solving and rationalization scores in the observation group were significantly higher than those in the control group (P<0.01); and self-reproach, withdrawal, and illusion were significantly lower than in the control group (P<0.05). 【Conclusion】Acceptance and commitment therapy can help relieve anxiety and depression, avoid negative coping behaviors, and promote the development of positive coping behaviors in patients with depression.

【Objective】To explore the value of transvaginal color Doppler ultrasound and cervical cerclage in the diagnosis and treatment of cervical incompetence.【Methods】 From June 2014 to July 2016, 127 high-risk patients with cervical insufficiency in our hospital were examined by transvaginal ultrasonography from 12 weeks of pregnancy. The length of cervical canal was measured and evaluated once every two weeks. The length of cervical canal was 25-30 mm, and the dilatation of cervical internal orifice showed as U-type or V-type was treated accordingly. The rest were excluded from the study. 86 patients with cervical insufficiency were divided into observation group and control group according to random number table method, 43 cases in each group. The control group was treated with didrogesterone orally, while the observation group was treated with cervical cerclage. Pregnancy outcomes (abortion, premature delivery, full-term pregnancy), pregnancy cycle, basic condition of newborn (body weight, postnatal monitoring time, Apgar score) and safety during treatment were compared between the two groups.【Results】The abortion rate and premature delivery rate in the observation group were significantly lower than those in the control group; the full-term pregnancy rate was significantly higher than those in the control group (P<0.05); the pregnancy cycle in the observation group was longer than that in the control group (P<0.05); the neonatal weight and Apgar score in the observation group were higher than those in the control group, and the monitoring time after birth was shorter than that in the control group (P<0.05). There were no complications in the observation group. There were few cases of nausea/vomiting and abdominal pain in the control group. There was no significant difference in the incidence of adverse events between the two groups (P>0.05).【Conclusion】Transvaginal color Doppler ultrasound examination is conducive to early detection of cervical insufficiency and timely treatment of patients. Cervical cerclage can effectively prolong pregnancy cycle, reduce the incidence of abortion and premature delivery, improve pregnancy outcomes. At the same time, the treatment program is safe and worthy of clinical application.

【Objective】To investigate the changes and significance of T lymphocyte subsets and immunoglobulins and inflammatory factors in children with bronchitis after glucocorticoid therapy. 【Methods】A total of 80 children with bronchitis admitted to our hospital from January 2015 to August 2018 were selected as subjects. They were divided into two groups with 40 cases each according to different treatment methods. Patients in the control group received symptotherapy like anti-infective and expectorant, while patients in the observation group received symptomatic treatments based on budesonide atomization inhalation therapy.). Clinical efficacy and changes of immune system and inflammatory factors were compared between the two groups.. 【Results】The disappearing time of cough, dyspnea, dry/wet sound of lung and hospitalization time in the observation group were shorter than those in the control group (P<0.05). There was no significant difference in T lymphocyte subsets and immunoglobulin levels between the two groups before treatment (P>0.05). After treatment, the levels of CD3+, CD4+, CD4+/CD8+, immunoglobulin A (IgA) and immunoglobulin G (IgG) in the observation group were significantly higher than those in the control group, and the levels of CD8+ and immunoglobulin E (IgE) were significantly lower than those in the control group(P<0.05). Before treatment, there were no significant differences between the two groups in the levels of interleukin-4 (IL-4), IL-23, and eosinophil cationic protein (ECP) (P>0.05). After treatment, the levels of IL-4, IL-23 and ECP in the observation group were significantly lower in the group than in the control group (P<0.05).【Conclusion】Glucocorticoid therapy in children with bronchitis can promote the disappearance of clinical symptoms and signs, improve the immune function of children, inhibit the expression levels of serum IL-4,IL-23,ECP, and shorten hospitalization time.

1323 Correlation between Serum Glucose and Lactate Levels and Prognosis in Patients with Aneurysmal Subarachnoid Hemorrhage

【Objective】 To investigate correlation between serum glucose and lactate levels and prognosis in patients with aneurysmal subarachnoid hemorrhage. 【Methods】The clinical data of 71 patients with aneurysmal subarachnoid hemorrhage admitted to our hospital from February 2015 to January 2018 were retrospectively analyzed. According to the prognosis of patients with hemorrhagic subarachnoid hemorrhage, patients were classified into the poor prognosis group (n=27) and the good prognosis group (n=44). Serum glucose and blood lactate levels were measured in both groups. Logistic regression was used to analyze the independent influencing factors of patients with poor prognosis. The receiver operating characteristic curve (ROC) was used to analyze the efficacy of serum glucose (Glu), blood lactate (Lac) and combined detection in the diagnosis of poor prognosis in patients with aneurysmal subarachnoid hemorrhage. 【Results】The levels of bleeding, WBC, SCr, cTnI, Lac, and Glu in patients with poor prognosis were significantly higher than those in patients with good prognosis. The proportion of Hunt-Hess grade and hypertension in patients with poor prognosis was higher than that in patients with good prognosis. The difference was statistically significant (P<0.05). Logistic regression analysis showed that the amount of bleeding, levels of Lac, Glu, and WBC, and Hunt-Hess grading were risk factors for poor prognosis (P<0.05). Serum Glu level was positively correlated with mRS score and blood loss (r=0.678, 0.734, P<0.05). Serum Lac level was also positively correlated with mRS score and blood loss (r=0.681, 0.705, both P<0.05) The efficacy of serum glucose diagnosis in patients with poor diagnosis is higher than that of blood lactate. The combined detection of serum glucose and blood lactate is more effective than single indicator in predicting prognosis of aneurysmal subarachnoid hemorrhage. 【Conclusion】Serum glucose and blood lactate levels are closely related to poor prognosis in patients with aneurysmal subarachnoid hemorrhage, which can accurately assess the prognosis of patients. The detection of serum Lac and Glu levels is convenient and rapid, and has certain efficacy for diagnosis. The combined detection and diagnosis of patients with poor prognosis is more effective, which is of great significance for clinical diagnosis and treatment.

1326 Effect of Autophagy on the Expression of MEK,ERK,NRF1 and Atg7 in Human Nucleus Pulposus Cells during Degeneration of Intervertebral Disc

【Objective】To investigate the autophagy of mitogen-activated protein kinase (MEK), cell signal-regulated kinase (ERK), transcription factor CNC-bZIP protein (NRF1) and autophagy-associated protein 7 (Atg7) during intervertebral disc degeneration to explore the basic research and clinical treatment of lumbar degenerative changes. 【Methods】 A total of 18 patients with severe degeneration of lumbar intervertebral disc (Pfirrmann grade 5) in the observation group and 18 patients with young lumbar vertebrae fractures in the control group were selected as the study subjects. The nucleus pulposus cells were isolated from lumbar intervertebral disc tissue and CD24 positive cells were sorted by flow cytometry. The cells were cultured for 96 h under simulated physiological pressure. The expressions of MEK, ERK, NRF1 and Atg7 mRNA in nucleus pulposus cells were detected by RT-PCR. The levels of MEK, ERK, NRF1 and Atg7 protein in nucleus pulposus cells were detected by Western blot. 【Results】 Results of RT-PCR showed that the expression levels of MEK, ERK, NRF1 and Atg7 mRNA in the observation group were significantly higher than those in the control group t (P<0.05).Western blot results showed that the expression levels of MEK, ERK, NRF1 and Atg7 proteins in the observation group were higher than those in the control group. The differences between the two groups were statistically significant (P<0.05). 【Conclusion】Autophagy upregulates the expression of MEK, ERK, NRF1 and Atg7 proteins in nucleus pulposus cells during degeneration of intervertebral disc. The MEK/ERK/NRF1/Atg7 signaling pathway is the main signaling pathway regulating nucleus pulposus cell apoptosis and may be involved in the pathogenesis of lumbar degenerative changes.

1329 Changes of Serum Levels of IL-6, S100β , IL-1β and IL-2 in Patients with Schizophrenia

【Objective】To explore the changes and significance of serum interleukin-6 (IL-6), calcium binding protein β (S100β), interleukin-1β (IL-1β) and interleukin-2 (IL-2) in patients with first-episode schizophrenia. 【Methods】Ninety patients with first-episode schizophrenia admitted to our hospital from March 2015 to May 2018 were divided into positive symptoms group (n=48) and negative symptoms group (n=42) according to their symptoms. Sixty healthy volunteers in the same period were selected as the control group. The serum levels of IL-6, S100β, IL-1β and IL-2 were compared among the three groups, and the relationship between serum levels of IL-6, S100beta, IL-1β and IL-2 and PANSS score was analyzed. 【Results】The serum levels of IL-6, S100β and IL-1β in the positive and negative symptoms groups were significantly higher than those in the control group, and the serum levels of IL-2 were significantly lower than those in the control group (P<0.05); the serum levels of IL-6 in the positive symptoms group and the negative symptoms group were significantly higher than those in the control group (P<0.05). The scores of positive symptoms, negative symptoms and PANSS in the positive symptoms group and negative symptoms group were significantly higher than those in the control group, and the scores of positive symptoms in the positive symptoms group were significantly higher than those in the negative symptoms group (P<0.05). There was a significant positive correlation between serum IL-1beta and PANSS total score (P<0.05). There was no significant correlation between serum IL-6, S100β and IL-2 levels and PANSS total score (P>0.05).【Conclusion】The serum levels of IL-6, S100β and IL-1β in schizophrenia patients increased significantly, while the expression of IL-2 decreased significantly. The expression of IL-1beta is closely related to the PANSS score of schizophrenia patients.

1332 Clinical Efficacy and Mechanism of Different Neoadjuvant Chemotherapy Administrations in the Treatment of Advanced Cervical Cancer

【Objective】To investigate the expression of PI3K/AKT/mTOR signaling pathway and clinical efficacy of different Neoadjuvant Chemotherapy administration (Paclitaxel+Cisplatin) for advanced cervical cancer. 【Methods】From March 2016 to March 2018, 68 patients with advanced cervical cancer who received neoadjuvant chemotherapy in Boai Hospital of Zhongshan City were selected. According to the random table method, they were divided into the interventional group (neoadjuvant interventional arterial chemotherapy) and the intravenous group (neoadjuvant intravenous chemotherapy), with 34 cases in each group. The levels of p-PI3K, p-Akt, p-mTOR, matrix metalloprotease 2 (MMP2) and vascular endothelial growth factor (VEGF) were compared between the two groups. The short-term efficacy and adverse reactions were recorded and compared as well. 【Results】The effective rate of the intervention group was higher than that of the intravenous group, and the incidence of adverse reactions was lower than that of the intravenous group (P<0.05). The levels of p-PI3K, p-Akt, p-mTOR, MMP2 and VEGF protein in cervical cancer tissues of intervention group were significantly lower than those of venous group (P<0.05). 【Conclusion】The short-term efficacy of neoadjuvant interventional arterial chemotherapy for advanced cervical cancer treatment is better than neoadjuvant intravenous chemotherapy. It can effectively inhibit the PI3K/AKT/mTOR signaling pathway in cervical cancer cells, which may be related to the significant inhibition of its phosphorylation.

1335 Observation on the Curative Effect of Extraventricular Drainage Combined with Intracerebroventricular Fibrolysis in the Treatment of Hypertensive Intraventricular Hemorrhage

【Objective】To observe the incidence of poor prognosis after hypertensive intraventricular hemorrhage.【Methods】The clinical data of 195 patients with hypertensive intraventricular hemorrhage treated in our hospital were analyzed retrospectively. According to the different treatment methods, they were divided into observation group (extraventricular drainage combined with intraventricular fibrinolysis,n=98) and control group (extraventricular drainage treatment group,n=97). The surgical conditions and the therapeutic effects of the two groups were compared,and the incidence of poor prognosis in patients with hypertensive intraventricular hemorrhage was observed.【Results】The incidence of poor prognosis in the observation group was lower than that in the control group (χ2=13.387,P<0.001). The ADL grade of 76 patients (77.55%) in the observation group was higher than that of 66 patients (68.04%) in the control group (χ2=-2.375,P=0.018 < 0.05). The hospitalization time, the extraction time of ventricular drainage and the amount of residual hematoma in the observation group were significantly shorter than those in the control group (P<0.05). The incidence of delayed hydrocephalus and intracranial infection in the observation group was significantly lower than that in the control group (χ2=3.327,P=0.072;χ2=0.423,P=0.515).【Conclusion】Extraventricular drainage combined with fibrinolysis can improve the daily living ability of patients with hypertensive intraventricular hemorrhage, shorten hospitalization time, and improve the prognosis of patients with important significance.
