Abstract:【Objective】To explore influences of different postures on the curative effect and functional recovery in treatment of lumbar disc herniation (LDH)with percutaneous transforaminal endoscopic discectomy (PTED) . 【Methods】A total of 80 LDH patients who were treated in Yulin Rehabilitation Hospital of Neck-Shoulder and Lumbocrural Pain from January 2017 to December 2018 were enrolled. They were divided into the control group and the observation group by random number table method, with 40 cases in each group. The control group underwent PTED in prone position, while the observation group underwent PTED in lateral position. The clinical curative effect after 1 year of surgery, intraoperative puncture time, times of fluoroscopy, number of cases with extreme discomfort, scores of visual analogue scale (VAS), Oswestry disability Index (ODI) and Japanese Orthopaedic Association (JOA) after 3 months and 1 year of surgery, and incidence of complications within 3 months after surgery were compared between the two groups. 【Results】After 12 months of surgery, there was no significant difference in good rate of treatment between the observation group and the control group (92.50% vs 87.50%) (P>0.05). During surgery, puncture time, times of fluoroscopy and number of cases with extreme discomfort in the observation group were lower than those in control group (P<0.05). There were no significant differences in scores of VAS of lumbar and leg pain, ODI and JOA before surgery between the two groups (P>0.05). At 3 months and 1 year after surgery, scores of lumbar and leg pain, and ODI in both groups were lower than those before surgery (P<0.05), while JOA scores were higher than those before surgery (P<0.05). Within 3 months after surgery, there was no significant difference in incidence of complications between the observation group and the control group (7.50% vs 10.00%) (P>0.05). 【Conclusion】The curative effect of PTED in both postures is good on LDH patients, which both can effectively improve lumbar function and alleviate lumbar and leg pain. The lateral position can reduce intraoperative puncture time and times of fluoroscopy, which showed more comfortable.
