Abstract:【Objective】To investigate the changes and value of homocysteine (Hcy), lipoprotein a and liver fatty acid binding protein (L-FABP) in early renal injury of diabetic nephropathy (DN).【Methods】The study contained patients with type 2 diabetes mellitus (T2DM group,n=60), type 2 diabetes mellitus with microalbuminuria (MA group,n=60), type 2 diabetic kidney injury (CN group,n=60) and healthy volunteers as the control group (n=60). The levels of Hcy, lipoprotein a, L-FABP and renal function were compared in the four groups. The relationship of levels of Hcy, lipoprotein a and L-FABP with changes in renal function of patients was investigated by a linear correlation method.【Results】The levels of Hcy, lipoprotein a and L-FABP in the T2DM group, MA group and CN group were significantly higher than those in the control group (P<0.05). Hcy, lipoprotein a and L-FABP levels in the MA group and CN group were higher than those in the T2DM group. The levels of Hcy, lipoprotein a and L-FABP in the CN group was significantly higher than those in the MA group (P<0.05). The levels of BUN and SCr in the CN group were significantly higher than those in the T2DM group, the MA group and the control group (P<0.05). The UAER values of the MA group and the CN group were higher than those of the T2DM group (P<0.05); and the UAER value of the CN group was higher than that of the MA group (P<0.05); The levels of Hcy, lipoprotein a and L-FABP in the T2DM patients with >20μg/min were significantly positively correlated with SCr and UAER (P<0.05).【Conclusion】The levels of Hcy, lipoprotein a, and L-FABP in patients with DN were significantly elevated and closely correlated with renal dysfunction in patients.
