SUN Hui, HUANG Han, DING Xiaofang, PENG Li, YU Jingwen
DOI: 10.3969/j.issn.1671-7171.2026.06.001
【Objective】 To analyze the clinical characteristics of drug-resistant severe Mycoplasma pneumoniae pneumonia (MPP) in children of different age groups. 【Methods】 A retrospective analysis was performed on the clinical data of 207 children diagnosed with drug-resistant severe MPP admitted to The First Affiliated Hospital of Hunan Normal University (Hunan Provincial People's Hospital) from August 2023 to April 2024. The enrolled children were divided into three groups according to age: infant group (age<3 years old, n=40), preschool group (age 3-6 years old, n=73) and school-age group (age>6 years old, n=94). The clinical characteristics were compared among the three groups. 【Results】 Fever and cough were the main clinical manifestations in all children. The infant group had a significantly higher incidence of wheezing (32.50%) and single viral or bacterial co-infection rate (60.00%) than the other two groups, with single viral infection (40.00%) as the predominant complication. The levels of serum C-reactive protein, neutrophil-to-lymphocyte ratio and lactate dehydrogenase in the preschool and school-age groups were significantly higher than those in the infant group (P<0.05). The median D-dimer level of the three groups showed no obvious elevation compared with the normal range (P>0.05), while significantly increased D-dimer level (median: 25.72 mg/L) was observed in 5 children complicated with pulmonary embolism. Massive pulmonary consolidation was the main imaging feature in all groups, especially in the preschool and school-age groups, which also had a higher proportion of pleural effusion than the infant group (P<0.05). All 5 cases of pulmonary embolism occurred in preschool and school-age children. Extrapulmonary complications occurred in 99 children (47.83%), mainly including liver dysfunction and myocardial injury, with a higher incidence in the infant group. One school-age child developed multi-site vascular embolism and achieved a good prognosis after treatment. The school-age group had higher proportions of children receiving tetracycline antibiotics and undergoing bronchoscopic lavage for no less than twice (P<0.05). A total of 28 children (13.53%) received immune supportive therapy, most of whom were from the infant and school-age groups (23/28). All children improved and were discharged after treatment. 【Conclusion】 There are significant differences in clinical characteristics of children with drug-resistant severe MPP across different age groups. Clinically, individualized diagnosis and treatment strategies should be optimized based on age stratification characteristics to improve the efficacy of clinical treatment.

914 Analysis of Parenting Stress and its Influencing Factors among Mothers of Preterm Infants at 1 Year Corrected Age
SHI Junxia, LIN Lin
DOI: 10.3969/j.issn.1671-7171.2026.06.010
【Objective】 To explore the current status and influencing factors of parenting stress among mothers of preterm infants at 1 year corrected age. 【Methods】 A total of 500 preterm infants at 1 year corrected age and their mothers who received follow-up in the high-risk infant clinic of our hospital from January 2024 to December 2025 were enrolled by random sampling method. The Parenting Stress Index-Short Form (PSI-SF), Maternal and Child Feeding Difficulty Scale (MCH-FS), Brief Infant Sleep Questionnaire (BISQ), and Neuropsychological Development Scale for Children Aged 0-6 Years were adopted for investigation. Parenting stress scores of mothers with different basic characteristics were compared, and multiple linear regression analysis was performed to analyze the influencing factors of parenting stress among mothers of preterm infants at 1 year corrected age. 【Results】 Significant differences were found in parental distress (PD), parent-child dysfunctional interaction (P-CDI), difficult child (DC) scores and total PSI-SF scores among mothers with different educational levels, annual family incomes, numbers of children, previous adverse pregnancy histories and quantities of gestational complications (P<0.05). Similarly, the above scores differed significantly in groups stratified by infants' small for gestational age status, birth weight, neonatal complications, twin pregnancy/multiple pregnancy status and in vitro fertilization conception status (P<0.05). In addition, statistically significant differences existed in PD, P-CDI, DC and total PSI-SF scores among mothers of infants with different developmental quotient (DQ) at 1 year corrected age, sleep regularity, falling-asleep difficulty, sleep disorders and feeding difficulty degrees (P<0.05). Multiple linear regression analysis showed that infant sleep difficulty, irregular sleep, feeding difficulty, developmental quotient, number of neonatal complications, number of children in the family and twin pregnancy were independent influencing factors for maternal parenting stress (P<0.05). 【Conclusion】 Parenting stress among mothers of preterm infants at 1 year corrected age is affected by multiple factors. In high-risk infant management, medical staff should attach great importance to the screening and intervention of infant sleep and feeding problems, and provide targeted psychosocial support for families with high-risk characteristics, so as to promote maternal and infant health.

930 Construction of an Invasiveness Assessment System for Lung Adenocarcinoma Based on Triple Markers of CD34, α-SMA, and β-Tubulin-Ⅲ and its Clinical Significance
CAI Lin, LIN Yong
DOI: 10.3969/j.issn.1671-7171.2026.06.014
【Objective】 To investigate the value of the combined detection of CD34, α-smooth muscle actin (α-SMA), and β-Ⅲ tubulin (β-Tubulin-Ⅲ) immunostaining for assessing invasiveness in lung adenocarcinoma. 【Methods】 We retrospectively analyzed 100 resected lung adenocarcinomas, including 15 adenocarcinoma in situ (AIS), 35 minimally invasive adenocarcinomas (MIA), and 50 invasive adenocarcinomas (IA). The CD34, α-SMA, and β-Tubulin-Ⅲ expressions were evaluated by immunohistochemistry and correlated with clinicopathological findings and follow-up data. 【Results】 AIS, MIA, and lepidic-predominant IA mainly showed a "double-track" pattern, which was characterized by reticular CD34 positivity and linear α-SMA staining along the basement membrane. In contrast, acinar- and solid-predominant IA showed a "loss-and-gain" pattern, which exhibited the loss of CD34 expression and strong stromal α-SMA positivity, whereas papillary-predominant IA showed an irregular linear pattern. β-Tubulin-Ⅲ was negative in AIS and most lepidic areas, but its expression was significantly upregulated in IA and correlated with invasive extent (all P<0.05). Follow-up data demonstrated favorable outcomes in AIS/MIA, most of whom underwent sublobar resection. Recurrence/metastasis-free survival did not have statistical significance between lepidic-predominant IA and AIS/MIA groups (P>0.05), whereas the risk of recurrence or metastasis in other types of IA was significantly higher than that in the AIS/MIA group and the lepidic-predominant IA group (P<0.05). 【Conclusion】 The combined detection of CD34, α-SMA, and β-Tubulin-Ⅲ aids in improving the accuracy of determining true invasion in lung adenocarcinoma, thereby providing a pathological basis for sublobar resection strategies in AIS/MIA and for individualized surgical decision-making in invasive adenocarcinomas.

DOI: 10.3969/j.issn.1671-7171.2026.06.015
【Objective】 To explore the imaging features of urinary system calculi by color Doppler ultrasound and analyze its clinical diagnostic value. 【Methods】 A retrospective analysis was conducted on the clinical data of 80 patients with urinary system calculi admitted to two hospitals from September 2021 to September 2025. All patients underwent preoperative two-dimensional ultrasound and color Doppler ultrasound examination. With surgical exploration or stone discharge after extracorporeal lithotripsy as the gold standard, the stone detection rate and segmental ureteral stone detection results of the two examination methods were compared, and the imaging characteristics of urinary calculi were summarized. 【Results】 Among the enrolled patients, 12 cases were diagnosed with bladder calculi, 31 cases with renal calculi, and 37 cases with ureteral calculi. The detection rates of bladder calculi, renal calculi and ureteral calculi by color Doppler ultrasound were significantly higher than those of two-dimensional ultrasound (all P<0.05). For upper and middle ureteral calculi, color Doppler ultrasound presented superior detection efficiency with statistically significant differences (all P<0.05). There was no significant difference in the detection rate of lower ureteral calculi between two-dimensional ultrasound and color Doppler ultrasound (P>0.05). 【Conclusion】 Color Doppler ultrasound can clearly display the ultrasonic imaging characteristics of different types of urinary system calculi. It has a significantly higher detection rate for bladder calculi, renal calculi, and upper and middle ureteral calculi than two-dimensional ultrasound, possessing important clinical application value.

YUAN Shuai, LI Yun
DOI: 10.3969/j.issn.1671-7171.2026.06.025
【Objective】 To explore the diagnostic efficacy of cardiac magnetic resonance (CMR)-based left atrial strain parameters for left ventricular fibrosis in patients with dilated cardiomyopathy (DCM). 【Methods】 A total of 42 patients diagnosed with DCM complicated with left ventricular fibrosis in our hospital from January 2022 to January 2025 were prospectively enrolled as the observation group. Meanwhile, 42 healthy volunteers who underwent CMR examination during the same period were selected as the control group. Left ventricular and left atrial functional parameters were compared between the two groups, including left ventricular end-diastolic volume (LVEDV), left ventricular end-systolic volume (LVESV), stroke volume (SV), left ventricular ejection fraction (LVEF), cardiac output (CO), cardiac index (CI), left ventricular end-diastolic myocardial mass (LVEDM), as well as left atrial strain parameters: left atrial reservoir strain (LARS), left atrial conduit strain (LACS), left atrial booster strain (LABS), late negative peak strain rate (SRa), early negative peak strain rate (SRe), and systolic peak strain rate (SRs). Receiver operating characteristic (ROC) curves were plotted to evaluate the diagnostic value of left atrial strain parameters for left ventricular fibrosis. 【Results】 The observation group had higher LVEDV, LVESV, heart rate (HR) and LVEDM, and lower SV, LVEF, CO and CI than the control group, with statistically significant differences (P<0.05). All left atrial strain parameters including LARS, LACS, LABS, SRa, SRe and SRs in the observation group were significantly lower than those in the control group (P<0.05). Multivariate Logistic regression analysis showed that LARS, LACS, LABS and SRs were independent correlated factors for the occurrence of DCM fibrosis (P<0.05). ROC curve analysis indicated that LARS yielded the highest single-test sensitivity (100.0%), while LABS presented identical and favorable specificity (92.86%). The area under the curve (AUC) and Youden index of combined detection were significantly higher than those of single parameter detection (P<0.05). 【Conclusion】 CMR-based left atrial strain parameters are closely correlated with left ventricular fibrosis in DCM patients. The combined detection achieves the optimal diagnostic efficacy, and single LARS detection presents the best sensitivity, which can serve as potential non-invasive indicators for clinical evaluation of left ventricular fibrosis in DCM patients.

WANG Xiaolong, ZHANG Jianbao, ZHANG Xiyang
DOI: 10.3969/j.issn.1671-7171.2026.06.028
【Objective】 To evaluate the diagnostic value of combining magnetic resonance imaging (MRI) parameters with surface electromyography (sEMG) of the lumbar paraspinal muscles in assessing postoperative lumbar function in patients with lumbar spinal stenosis (LSS). 【Methods】 A total of 63 LSS patients who underwent endoscopic decompression surgery at our hospital between February 2023 and June 2024 were enrolled. Based on their lumbar functional outcomes at 3 months postoperatively, they were divided into an improvement group and a non-improvement group. Additionally, 31 healthy individuals of similar age who underwent routine physical examinations at the same period were selected as the control group. MRI parameters and sEMG parameters of the paraspinal muscles were compared among the three groups: the improvement group, non-improvement group at 3 months post-surgery and the control group during physical examination. The diagnostic value of MRI parameters and sEMG parameters for postoperative lumbar function was assessed using receiver operating characteristic (ROC) curve analysis. 【Results】 Among the 63 LSS patients, 30.16% (19/63) showed no improvement after surgery. The spinal canal area, sagittal diameter, transverse diameter, and dural sac area in both the control group and the improvement group were significantly larger than those in the non-improvement group. Similarly, the integrated electromyography (IEMG) and mean power frequency (MPF) values were higher in the improvement and control groups compared to the non-improvement group, with all differences being statistically significant (P<0.05). ROC analysis revealed that the combination of MRI parameters and sEMG parameters yielded higher accuracy, sensitivity, and specificity in diagnosing postoperative lumbar function in LSS patients compared to either MRI or sEMG alone. 【Conclusion】 The combined use of MRI parameters and sEMG of the lumbar paraspinal muscles has high diagnostic value for assessing postoperative lumbar function in LSS patients.

DOI: 10.3969/j.issn.1671-7171.2026.06.029
【Objective】 To explore the clinical value of 18-lead electrocardiogram (ECG) combined with echocardiography in diagnosing coronary heart disease (CHD) complicated with myocardial ischemia and localizing culprit vessels. 【Methods】 A total of 82 patients with suspected CHD scheduled for coronary angiography admitted to our hospital from June 2023 to January 2025 were prospectively enrolled. According to coronary angiography findings, patients were divided into observation group (hemodynamically significant stenosis, n=56) and control group (no significant stenosis, n=26). The diagnostic performance of 18-lead ECG, echocardiography and their combination (defined as positive if either test showed positive result) was compared. Hierarchical Logistic regression models were established: Model 1 contained clinical baseline data; Model 2 added 12-lead ST-T changes based on Model 1; Model 3 supplemented ST-T abnormalities from additional leads of 18-lead ECG on the basis of Model 2; Model 4 incorporated echocardiographic wall motion abnormalities into Model 3. The area under the receiver operating characteristic curve (AUC) and incremental diagnostic value of each model were analyzed. 【Results】 Fifty-six patients were confirmed to have hemodynamically significant coronary stenosis. For the diagnosis of myocardial ischemia, 18-lead ECG yielded a sensitivity of 76.79%, specificity of 73.08% and AUC of 0.749; echocardiography showed a sensitivity of 73.21%, specificity of 76.92% and AUC of 0.751. Combined detection achieved a higher sensitivity of 91.07%, specificity of 69.23% and AUC of 0.802, which were all superior to single examination (P<0.05). The AUC values of Model 1 to Model 4 were 0.702, 0.752, 0.781 and 0.832 respectively. Compared with Model 4, Model 1, Model 2 and Model 3 had significantly lower AUCs with DeLong test P values of 0.004, 0.021 and 0.047 separately. The variance inflation factor (VIF) of all models was less than 5 without obvious multicollinearity, and the Hosmer-Lemeshow test showed P>0.05 for all models. 【Conclusion】 In patients admitted to chest pain center, 18-lead ECG and echocardiography have respective advantages in identifying hemodynamically significant myocardial ischemia in CHD patients. Their combination presents favorable overall diagnostic efficiency, which can be applied as a comprehensive evaluation protocol for patients with moderate-to-high risk suspected coronary heart disease.
