| Clinical Effect of Nasal Intermittent Positive Pressure Ventilation in the Treatment of Neonatal Respiratory Distress Syndrome and its Influence on Blood Gas Indexes and Neurodevelopment of Neonates |

Abstract 【Objective】 To investigate the clinical effect of nasal intermittent positive pressure ventilation (NIPPV) in the treatment of neonatal respiratory distress syndrome (NRDS) and its influence on blood gas indexes and neurodevelopment of neonates. 【Methods】 A total of 148 neonates with NRDS admitted to our hospital from August 2019 to August 2024 were selected and divided into observation group and control group according to different ventilation regimens, with 74 cases in each group. The observation group was treated with NIPPV mode, while the control group received nasal continuous positive airway pressure (N-CPAP) mode. The treatment outcomes, arterial partial pressure of carbon dioxide (PaCO2), arterial partial pressure of oxygen (PaO2), pH value, oxygenation index (OI) and neurodevelopment status were compared between the two groups. 【Results】 The hospitalization stay and oxygen therapy duration of the observation group were shorter, and the number of apnea episodes was less than those of the control group, with statistically significant differences (P<0.05). After treatment, PaCO2 and OI decreased, while pH value and PaO2 increased in both groups compared with baseline; the observation group had lower PaCO2 and OI, as well as higher pH value and PaO2 than the control group, and all differences were statistically significant (P<0.05). At 12 months after treatment, the proportions of neonates with intelligence scale score ≥85, motor scale score ≥85 and developmental quotient (DQ) ≥85 in the observation group were all higher than those in the control group, with statistically significant differences (P<0.05). The total incidence of adverse reactions in the observation group and control group was 13.51% (10/74) and 14.86% (11/74) respectively, and there was no statistically significant difference between the two groups (χ2=0.055, P>0.05). 【Conclusion】 NIPPV can rapidly improve respiratory function and clinical symptoms, significantly ameliorate oxygenation function and correct hypoxemia in neonates with NRDS, help maintain the body's acid-base balance, and possesses favorable safety.
