Abstract 【Objective】 To investigate the bone flap absorption after autologous skull repair. 【Methods】 A total of 14 patients who underwent autologous skull repair in our hospital from January 2020 to December 2023 were enrolled in this study. Two patients were excluded due to missing imaging data, and a total of 12 patients were finally included. The skull volume on CT images was delineated and automatically calculated by computer software, and the Oulu Resorption Scale (ORS) was adopted to evaluate the degree of bone flap absorption. 【Results】 All patients received complete large integral bone flaps. The skull defect area after craniectomy was (114.23±27.19) cm2, and the bone flap volume was (56.09±14.67) cm3. The operation duration of decompressive craniectomy was (162.83±52.72) min, the duration of skull repair surgery was (189.67±53.19) min, and the interval between the two operations was (130.75±67.11) days. Bone flap absorption occurred in all patients after surgery with varying degrees. The average bone flap volume within 3 months after surgery decreased to 84.12% (45.05%-99.89%) of the original volume, while the average volume decreased to 74.75% (35.45%-96.88%) of the original volume after 3 months postoperatively. The ORS scores were 2.08 (0-7) and 3.83 (0-8) within 3 months and after 3 months after surgery, respectively. 【Conclusion】 Bone flap absorption can occur in some patients after autologous skull repair, and the absorption degree is more severe after 3 months postoperatively than that within 3 months after surgery.
