Application of Unsupported Upper Limb Exercise Combined with Breathing Training in Stable COPD Patients

【Objective】To investigate the effect of unsupported upper limb exercises combined with respiratory training on chronic obstructive pulmonary disease (COPD). 【Methods】A total of 90 patients with stable COPD, who were diagnosed and treated in the Second Affiliated Hospital of Henan University of Science and Technology from July to December 2021, were randomly divided into the observation group and the control group, with 45 cases in each group. The control group was treated with routine nursing intervention. While the observation group was treated with unsupported upper limb exercise combined with breathing training in addition to routine nursing. The total effective rate, 6-min walking distance, COPD patient Assessment Test (CAT) score, St. George's Breathing Questionnaire (SGRQ) score, self-rating Depression Scale (SDS) score and self-rating Anxiety Scale (SAS) score were compared between the two groups. 【Results】After intervention, the total effective rate of the observation group was higher than that of the control group, the difference was statistically significant (P<0.05). The 6-min walking distance of the observation group was much longer than that of the control group, and the difference was statistically significant (P<0.05). CAT scores, SGRQ scores, SDS scores and SAS scores in observation group were lower than those in control group, and the differences were statistically significant (P<0.05). 【Conclusion】Unsupported upper limb exercise combined with breathing training can enhance exercise endurance, improve quality of life,alleviate anxiety and depression of patients with stable COPD, which is worthy of clinical application.

【Objective】To investigate the optimal method of calcium carbonate D

chewable tablet formulation based on reducing side effects of constipation. 【Methods】The effects of different excipients on small intestine movement of mice were investigated by literature, and 5 commercially available products composed of different excipients were selected. According to the excipients composition of commercially available products, 2 kinds of finished products were made in the laboratory, and the optimal excipients composition was selected by mouse small intestine movement test. ICR mice were randomly divided into 8 groups according to body weight, namely blank control group, 401 self-made group, 601 self-made group, marketed product 1, 2, 3, 4 group and Jinbei commercial product group, with 20 mice (half male and half female) in each group. Single administration test (once administration) and repeated administration test (30 days administration) were conducted respectively. The dose of mice in each administration group was 130 mg/kg (measured by calcium), and the corresponding volume of pure water was given to the blank control group. The general clinical manifestations and fecal characteristics of mice were observed every day during the experiment. The fecal properties of mice in each experimental group were observed and photographed every week in the repeated administration test groups, and the fecal weight was measured within 6 hours. Intestinal propulsion test was performed after the last administration in both single administration test and repeated administration test to calculate the intestinal ink propulsion rate of mice. 【Results】Single administration of Calcium carbonate and vitamin D

chewable tablets composed of different excipients had no significant effect on small intestinal propulsion rate in mice. After repeated administration for 30 days, compared with the blank control group, fecal dryness was observed in mice in 601 self-made group, marketed product 1, 2, 3, 4 group, and the ink advancing rate was significantly reduced(P<0.05). The fecal weight of mice treated with 3 weeks and 4 weeks in 601 self-made group, marketed product 3, 4 group, and the fecal weight of mice treated with 4 weeks in marketed product 1, 2 group were significantly reduced(P<0.05); There were no significant effects on fecal traits, intestinal propulsion rate and fecal weight in 401 self-made group and Jinbei brand market group. 【Conclusion】Calcium carbonate and vitamin D
3
chewable tablets containing defecation promoting components can reduce constipation side effects caused by repeated administration, and excipients with different defecation promoting mechanisms can play a synergistic role. Through comparison in this study, Jinbei brand Calcium carbonate and vitamin D3 chewable tablets (Ⅱ) (patent No.ZL 2021 1 0357997.6), which is owned by Zhejiang Huanling Pharmaceutical Technology Co., Ltd., has the best prescription process. It can effectively reduce the side effect of constipation caused by repeated administration.

Investigation on the Influencing Factors and Drug Resistance of Klebsiella Pneumoniae Infection in Hospital

【Objective】To explore the influencing factors and drug resistance of Klebsiella pneumoniae in hospital acquired infections. 【Methods】A total of 87 patients with hospital acquired infections admitted to our hospital from March 2018 to January 2023 were selected as the study subjects. Among them, 28 patients with hospital acquired infections of Klebsiella pneumoniae were included in the observation group, and 59 patients with hospital acquired infections of Acinetobacter baumannii were included in the control group. Two sets of general information were compared; Strain source, drug resistance, and virulence genes; The influencing factors and drug resistance of Klebsiella pneumoniae infection in the hospital were analyzed. 【Results】In the observation group, 21 cases (75.00%) were positive for mixed infections, including 5 cases (23.81%) of type A (Streptococcus pneumoniae), 4 cases (19.05%) of type B (Staphylococcus aureus), 2 cases (9.52%) of type C (Haemophilus influenzae), 4 cases (19.05%) of type A+B, 2 cases (9.52%) of type A+C, 3 cases (14.29%) of type B+C, and 1 case (4.76%) of type A+B+C. In the control group, 26 cases (44.07%) were positive for mixed infections, including 6 cases of type A (23.08%), 5 cases of type B (19.23%), 4 cases of type C (15.38%), 3 cases of type A+B (8.69%), 3 cases of type A+C (11.54%), 4 cases of type B+C (15.38%), and 1 case of type A+B+C (3.85%). There was no statistically significant difference (P>0.05) in age, gender, length of hospital stay, albumin levels, department distribution, white blood cell count (WBC) and indwelling catheter count, invasive procedures, and the proportion of infected patients staying in the same ward for ≥1 week or with incomplete final disinfection between the two groups; The observation group had a higher proportion of long-term use of broad-spectrum antibiotics, combined with at least 2 other bacterial infections, and early colonization of Klebsiella pneumoniae than the control group (P<0.05). The results of logistic regression analysis showed that long-term use of broad-spectrum antibiotics, concurrent infection with ≥2 other bacteria, and early colonization of Klebsiella pneumoniae were risk factors for hospital acquired Klebsiella pneumoniae infection in patients (P<0.05). 28 strains of bacteria mainly come from sputum and blood; Klebsiella pneumoniae has the highest resistance to piperacillin/tazobactam and ceftriaxone (100%), while it has the lowest resistance to tigecycline (3.57%) and tetracycline (7.14%); Among the 28 strains, 6 strains (21.43%) were high mucinous and 22 strains (78.57%) were non high mucinous. The positive rate of high mucinous strains in the wire drawing test was higher than that of non mucinous strains (P<0.05); The positive rates of virulence genes iroN, rmpA, and ybtS in high mucinous strains were higher than those in non mucinous strains (P<0.05). 【Conclusion】Long term use of broad-spectrum antibiotics, combined with at least 2 other bacterial infections, and early colonization of Klebsiella pneumoniae are risk factors for patients to be infected with Klebsiella pneumoniae. Klebsiella pneumoniae is more resistant to common antibiotics.

Expression of MTA1 and VEGF-C in Lung Cancer Tissue and Correlation with Clinical Staging and Prognosis of Patients

【Objective】To analyze the expression levels of metastasis related gene 1 (MTA1) and vascular endothelial growth factor C (VEGF-C) in lung cancer tissue and their correlation with clinical staging and prognosis of patients.【Methods】Retrospective analysis of clinical data from 85 patients diagnosed with lung cancer in our hospital. All lung cancer tissues and adjacent tissues of the patients were collected as pathological specimens, and the positive expression rates of MTA1 and VEGF-C were detected using immunohistochemistry. Meanwhile, the expression levels of MTA1 and VEGF-C was detected by PCR fluorescence quantification method. The positive expression rates of MTA1 and VEGF-C were compared among patients with different clinical pathological parameters and prognostic outcomes. 【Results】The positive rates of MTA1 and VEGF-C in lung cancer tissues were higher than those in adjacent tissues (P<0.05). And the expression levels of both proteins of cancer tissues were also higher than those of the adjacent tissues (P<0.05). The positive rates and expression levels of MTA1 and VEGF-C in patients with TNM stages, low differentiation, tumor diameter>5 cm, lymph node metastasis, vascular invasion, and poor prognosis showed an upward trend (P<0.05). The cumulative progression free survival rate at 24 months in the MTA1 positive expression group was lower than the negative expression group (P<0.05). The survival rate of the VEGF-C positive expression group was lower than the negative expression group, and the difference was significant (P<0.05). The expression of MTA1 and VEGF-C in lung cancer tissue is closely related to tumor TNM staging, differentiation degree, tumor diameter, lymph node metastasis, vascular invasion, and prognosis (P<0.05). 【Conclusion】MTA1 and VEGF-C are often positively expressed in lung cancer tissues, and their expression levels can serve as important factors for evaluating pathological features such as tumor staging, diameter, and prognosis to a certain extent. It is worth further exploration in clinical practice.

Investigation and Analysis on Willingness, Cognition and Training Status of Nurses in Different Levels of Hospitals Performing Internet plus Nursing Services

【Objective】To investigate and analyze the willingness, cognition and training status of nurses to carry out Internet + nursing services. 【Methods】 A cross-sectional survey was conducted among 16 299 nursing staff in 32 first, second and third level hospitals in China by using self-designed questionnaires. 【Result】Among 16 299 nursing staff, 86.3% thought it was necessary to carry out Internet plus nursing services, and 87.3% were willing to carry out Internet plus nursing services. There was a statistically significant difference (P<0.05) in the willingness of nursing staff in different levels of hospitals to carry out Internet plus nursing services. There was a statistically significant difference (P<0.05) between the different levels of hospital nursing staff in terms of which model they most needed to adopt to carry out Internet plus nursing services, the way of formulating service transportation fees and the standards of charging fees. There was no statistically significant difference (P>0.05) in the necessity of conducting relevant training and whether nursing staff from different levels of hospitals had received such training. 【Conclusion】Nurses in hospitals of different levels are willing to carry out Internet plus nursing services, but Internet plus nursing service development mode, fee collection, home service training and other aspects need to be strengthened.

The Impact of Risk Prevention Nursing Based on Orthopedic Damage Control on Postoperative Recovery of Patients with Scaphoid Bone Fractures

【Objective】To explore the effect of risk prevention nursing based on orthopedic damage control on postoperative recovery of patients with scaphoid bone fractures. 【Methods】A total of 137 patients with scaphoid fractures admitted to our hospital from June 2019 to June 2023 were selected and randomly divided into an observation group (receiving routine nursing combined with risk prevention nursing based on orthopedic damage control, n=68) and a control group (receiving routine nursing, n=69) using a stratified random sampling method. The Gartland-Weirey wrist joint function score, visual analog scale (VAS) score, wrist joint range of motion, quality of life comprehensive assessment questionnaire (GQOLI-74) score, and incidence of complications were compared between the two groups. 【Results】After intervention, the Gartland-Weirey wrist joint function score and VAS score in the observation group were lower than those in the control group (P<0.05), while the wrist joint range of motion and GQOLI-74 score were higher than those in the control group (P<0.05); The total incidence of complications in the observation group was lower than that in the control group (P<0.05). 【Conclusion】 Risk prevention nursing based on orthopedic damage control can significantly improve wrist joint function, alleviate postoperative pain, improve patients' quality of life, and reduce the overall incidence of complications in patients with scaphoid fractures. It has a positive effect on postoperative recovery of patients with scaphoid fractures.

Clinical Efficacy of Qingao Xiaojitang in Treating Pediatric Digestive Disorders and its Impact on Gastrointestinal Function Indicators

【Objective】To explore the clinical efficacy of Qingao Xiaojitang in treating pediatric digestive disorders and its impact on gastrointestinal function indicators. 【Methods】A total of 124 children with digestive disorders admitted to two hospitals from January 2021 to March 2022 were selected and randomly divided into an observation group (treated with domperidone combined with Qingao Xiaojitang) and a control group (treated with domperidone) using a random number table method, with 62 cases in each group. Two groups were compared in terms of clinical efficacy, TCM syndrome scores, gastrointestinal hormone levels, and changes in gastric motility before and after treatment.【Results】The total effective rate of the observation group was 90.32% (56/62), which was higher than the control group's 75.81% (47/62), and the difference was statistically significant (χ
2
=4.644, P<0.05). After treatment, the scores of early satiety, nausea, abdominal distension, belching, etc. in both groups were lower than before treatment, and the observation group was lower than the control group, with a statistically significant difference (P<0.05). After treatment, the levels of MTL, GAS, and GHR in both groups of children were higher than before treatment, and the observation group was higher than the control group, with statistical significance (P<0.05). After treatment, the frequency and amplitude of gastric contraction waves in both groups were significantly higher than before treatment, and the observation group was higher than the control group, with a statistically significant difference (P<0.05).【Conclusion】 Qingao Xiaojitang has significant clinical efficacy in treating pediatric digestive disorders, effectively alleviating gastrointestinal hormone levels and improving gastric motility indicators.

Application Effect of Intermittent Load Training on Exercise Endurance Recovery in Stable COPD Patients

【Objective】To investigate the effect of intermittent load training on exercise endurance recovery in patients with stable chronic obstructive pulmonary disease (COPD). 【Methods】A total of 80 patients with COPD from June 2021 to September 2023 were selected and divided into the control group and the observation group according to random number table method, with 40 cases in each group. The control group received routine pulmonary rehabilitation training, while the observation group received intermittent load training on the basis of control group. The degree of dyspnea, exercise endurance and quality of life scores were compared between the two groups on first day of admission and 12 weeks of follow-up after discharge. 【Results】At 12 weeks follow-up, the dyspnea questionnaire (mMRC) in the observation group was better than that in the control group (P<0.05). There was no significant difference in 6 minute walking distance (6MWD) between the two groups on the first day of admission (P>0.05). At 12 weeks follow-up, the values of 6MWD in both groups was significantly higher than those on the first day of admission, and the 6MWD in the observation group was significantly higher than that in the control group (P<0.05). There was no significant difference in the scores of Chronic respiratory disease questionnaire (CRQ) between the two groups on the first day of admission (P>0.05). At 12 weeks of follow-up, CRQ scores in both groups were higher than those on the first day of admission, and the CRQ scores in the observation group were higher than that in the control group (P<0.05). 【Conclusion】Intermittent load training can improve respiratory function, exercise endurance and quality of life in patients with COPD.

Clinical Value of Platelet Distribution Width in Predicting the Prognosis of Patients with Radical Resection of Lung Adenocarcinoma

【Objective】 To explore the clinical value of platelet distribution width (PDW) in predicting the prognosis of patients with radical resection of lung adenocarcinoma. 【Methods】The PDW levels of 186 patients undergoing radical resection of lung adenocarcinoma admitted to two hospitals were measured. The critical value of PDW was determined based on the receiver operating characteristic (ROC) curve of the subjects, and the patients were divided into high PDW group and low PDW group. The relationship between PDW levels and clinical characteristics of cancer patients was analyzed. After discharge, continuous follow-up for 3 years was conducted to explore the influencing factors of patient prognosis using multivariate Cox regression analysis. 【Results】The optimal cutoff value for PDW was 2.57, which had the highest predictive value. The high expression rate of PDW in patients with TNM stage Ⅱ-Ⅲ and tumor diameter>3 cm was higher than that in patients with TNM stage I and tumor diameter ≤ 3 cm (P<0.05). After a 3-year follow-up, 119 out of 186 patients survived, with an overall survival rate of 63.98% (119/186); The 3-year overall survival rate of patients was related to TNM staging, tumor diameter, and PDW (mean P<0.05); The 3-year overall survival rate of patients in the low PDW group was 79.00% (79/100), which was higher than the 46.51% (40/86) in the high PDW group (P<0.05). Multivariate Cox regression analysis showed that TNM stage Ⅱ-Ⅲ, tumor diameter ≥3 cm, and PDW ≥ 2.57 were risk factors for poor prognosis in patients (P<0.05). 【Conclusion】The level of PDW is closely related to the prognosis of patients with radical resection of lung adenocarcinoma, and it also has high predictive value for prognosis evaluation. PDW, TNM staging, and tumor diameter are independent risk factors affecting patient prognosis.

Value of Lumen Enhanced CT Density of Dual Source CT Coronary Angiography in Evaluating Degree of Coronary Artery Stenosis and Influencing Factors

【Objective】To explore the value of lumen enhanced CT density of dual source CT coronary angiography in evaluating the degree of coronary artery stenosis, and analyze the relevant factors affecting the evaluation of coronary artery stenosis by dual source CT coronary angiography. 【Methods】The medical records of 102 patients with coronary artery stenosis admitted to our hospital from March 2019 to March 2022 were retrospectively analyzed. According to the degree of coronary artery stenosis, they were divided into mild stenosis group, moderate stenosis group and severe stenosis group. The CT enhanced density values of proximal and distal lumens were calculated by dual source CT coronary angiography. Spearman correlation analysis was used to analyze the correlation between intraluminal CT enhancement value and the degree of lumen stenosis. The area under the receiver operating characteristic (ROC) curve (AUC) was used to evaluate the value of enhanced CT density in evaluating the degree of intraluminal stenosis. In addition, logistic regression analysis was used to analyze the related factors affecting the evaluation of coronary stenosis by dual source CT coronary angiography. 【Results】Lumen enhanced CT density in the moderate stenosis and severe stenosis group were higher than that in the mild stenosis group (P<0.05). Spearman correlation analysis showed that the enhancement value of intraluminal CT was positively correlated with the degree of lumen stenosis (P<0.05). ROC analysis showed that the AUC of enhanced CT density value for ROC diagnosis of intraluminal stenosis was 0.842 (95%CI:0.756-0.929). Analysis of the Application Effects and Prognostic Factors of Surgery and Conservative Treatment in Elderly Patients with Malignant Tumor-Related Acute Intestinal Obstruction

【Objective】To investigate the effects of surgery and conservative treatment in elderly patients with malignant tumor-related acute intestinal obstruction and to analyze the factors influencing prognosis. 【Methods】A retrospective analysis was conducted on the medical records of 96 elderly patients with acute intestinal obstruction admitted to our hospital. Among them, 71 patients were treated with bowel resection and anastomosis, stoma creation, or bowel exteriorization based on their condition, while 25 patients received conservative treatment such as gastrointestinal decompression. The patients were divided into a good prognosis group (79 cases) and a poor prognosis group (17 cases) based on the prognosis two months after treatment. The factors influencing prognosis were analyzed. 【Results】The treatment success rate was 94.37%(67/71) in the surgery group, which was higher than the 48.00%(12/25) in the non-surgery group (P<0.05). There was no statistically significant difference in the overall incidence of postoperative complications between the two groups (P>0.05). The poor prognosis group had higher proportions of pathological stage Ⅲ, low differentiation of tumors, and older age compared to the good prognosis group (P<0.05). Logistic multivariate regression analysis showed that age, pathological stage, and tumor differentiation degree were factors influencing prognosis (P<0.05). 【Conclusion】Compared to non-surgical treatment, surgical treatment is more effective in alleviating symptoms of intestinal obstruction in elderly patients with malignant tumor-related acute intestinal obstruction. Factors such as patient age, pathological stage, and tumor differentiation degree can influence prognosis.

The Effectiveness of Combined Staged Laparoscopic Surgery in the Treatment of Acute Common Bile Duct and Gallbladder Stones

【Objective】 To investigate the effectiveness of combined staged laparoscopic surgery in treating patients with acute common bile duct and gallbladder stones.【Methods】 Sixty patients admitted to two hospitals with acute common bile duct and gallbladder stones were randomly divided into an observation group and a control group, with 30 patients in each. The control group underwent simultaneous total laparoscopic surgery [laparoscopic common bile duct exploration (LCBDE) + laparoscopic cholecystectomy (LC)], while the observation group underwent combined staged laparoscopic surgery [endoscopic sphincterotomy (EST) + endoscopic retrograde cholangiopancreatography (ERCP) + LC]. Surgical related indicators, pre- and post-operative intestinal flora levels, post-operative complications and prognosis were compared between the two groups.【Results】The operation time, post-operative time to first flatus, and post-operative hospital stay in the observation group were shorter than those in the control group (P<0.05). The intraoperative blood loss was also less in the observation group (P<0.05). Compared to the control group, the levels of E. coli and Enterococcus were lower, while the level of Bifidobacterium was higher in the observation group post-operatively (P<0.05). The rates of post-operative fever and acute pancreatitis were 3.33% and 6.67%, respectively, in the observation group, which were lower than 26.67% and 23.33% in the control group (P<0.05). There were no significant differences in the rates of post-operative bleeding, wound infection, and peritoneal infection between the groups (P>0.05).【Conclusion】 Combined staged laparoscopic surgery not only shortens the surgery time, post-operative time to first flatus, and post-operative hospital stay, reduces intraoperative blood loss, but also improves intestinal flora levels in patients with acute common bile duct and gallbladder stones. The Effect of 15 ° "Hook Plate Angle" Clavicle Hook Steel Plate Internal Fixation on Postoperative Pain and Treatment Outcomes in Patients with Distal Clavicle Fractures

【Objective】To investigate the effect of 15 ° "hook plate angle" clavicle hook plate internal fixation on postoperative pain and treatment outcomes in patients with distal clavicle fractures. 【Methods】A retrospective analysis was conducted on the clinical data of 93 patients with distal clavicle fractures admitted to our hospital from January 2016 to February 2021. Among them, 46 patients were treated with 0 ° "hook plate angle" clavicle hook plate internal fixation (control group), and 47 patients were treated with 15 ° "hook plate angle" clavicle hook plate internal fixation (observation group). The surgical conditions, fracture healing, postoperative complications, visual analogue scale (VAS) scores before and after surgery, shoulder joint function, reduction effect, and anatomical structure recovery of the acromioclavicular joint were compared between the two groups. 【Results】There was no statistically significant difference (P>0.05) in the surgical time, intraoperative blood loss, and fracture healing time between the two groups. The VAS scores of the observation group were lower than those of the control group at 1, 3, and 6 months after surgery (P<0.05). The Constant Murley score, Neer score, and UCLA shoulder joint scoring system score of the observation group were all higher than those of the control group at 12 months after surgery (P<0.05). There was no statistically significant difference (P>0.05) in the postoperative reduction effect between the two groups, as well as in the beak lock distance, flexion range of motion, and external rotation range of motion 12 months after surgery. There was no statistically significant difference in the total incidence of complications between the two groups (P>0.05). 【Conclusion】Compared with traditional 0 ° "hook plate angle" clavicle hook plate internal fixation, the use of 15 ° "hook plate angle" clavicle hook plate internal fixation for patients with distal clavicle fractures during internal fixation surgery can reduce postoperative pain and promote shoulder joint function recovery.

Changes in Serum Levels of Perilipin-5, RBP-4, FGF-23 in Patients with Coronary Heart Disease and their Correlation with the Degree of Coronary Artery Stenosis

【Objective】To investigate the changes in serum levels of perilipin-5, retinol binding protein-4 (RBP-4), and fibroblast growth factor-23 (FGF-23) in patients with coronary heart disease and their correlation with the degree of coronary artery stenosis. 【Methods】A total of 142 patients with coronary heart disease admitted to two hospitals from May 2020 to January 2023 (observation group) were selected, and 100 healthy volunteers who underwent physical examinations during the same period were selected as the control group. We compared the changes in serum levels of perilipin-5, RBP-4, and FGF-23 between two groups, and compared the levels of perilipin-5, RBP-4, and FGF-23 in patients with different types of coronary heart disease, number of lesion branches, and degree of stenosis. We analyzed the correlation between serum levels of perilipin-5, RBP-4, and FGF-23 and the number of lesion branches and degree of stenosis, as well as their diagnostic value for the degree of stenosis. 【Results】The serum levels of RBP-4 and FGF-23 in the observation group were higher than those in the control group, and the serum level of perilipin-5 was lower than that in the control group, with statistical significance (P<0.05). The serum levels of perilipin-5 in patients with stable angina are higher than those in patients with unstable angina and acute myocardial infarction, while the serum levels of RBP-4 and FGF-23 are lower than those in patients with unstable angina and acute myocardial infarction, and the differences are statistically significant (P<0.05). The serum levels of perilipin-5 in patients with mild stenosis were higher than those in patients with moderate and severe stenosis, while the serum levels of RBP-4 and FGF-23 were lower than those in patients with moderate and severe stenosis, and the differences were statistically significant (P<0.05). The serum level of perilipin-5 in patients with single vessel disease is higher than that in patients with double vessel disease and multi vessel disease, while the serum levels of RBP-4 and FGF-23 are lower than those in patients with double vessel disease and multi vessel disease, and the differences are statistically significant (P<0.05). Spearman correlation analysis showed that serum perilipin-5 is negatively correlated with the number and degree of stenosis of the lesion, while RBP-4 and FGF-23 are positively correlated with the number and degree of stenosis of the lesion. The analysis of the working characteristic curve of the subjects showed that the AUC of the combined diagnosis of perilipin-5, RBP-4, and FGF-23 for the degree of coronary heart disease stenosis was 0.819, which was better than the diagnosis of each single indicator (P<0.05). 【Conclusion】The changes in serum levels of perilipin-5, RBP-4, and FGF-23 are significantly correlated with the condition of coronary heart disease patients. The combined diagnosis of the three can help achieve disease risk stratification.

Clinical Efficacy of Osimertinib Combined with Gemcitabine in the Treatment of Elderly Patients with Advanced NSCLC and Renal Metastasis

【Objective】 To investigate the clinical efficacy and survival status of osimertinib combined with gemcitabine in the treatment of elderly patients with advanced non-small cell lung cancer (NSCLC) and renal metastasis. 【Methods】 A total of 238 elderly patients with advanced NSCLC admitted to our hospital were randomly divided into the control group and the observation group, with 119 cases in each group. The control group was treated with osimertinib, while the observation group was additionally treated with cisplatin and gemcitabine. The efficacy after 3 treatment cycles, changes in tumor markers and renal function indicators before and after treatment, drug safety, and 1-year survival status were compared between the two groups. 【Results】 The clinical control rate in the observation group was higher than that in the control group (P<0.05). After treatment, the levels of carcinoembryonic antigen (CEA), pro-gastrin-releasing peptide (ProGRP), and squamous cell carcinoma antigen (SCCA) were lower in both groups compared to before treatment (P<0.05), with the observation group showing even lower levels than the control group (P<0.05). There were no significant differences in blood urea nitrogen and serum creatinine levels between the two groups before and after treatment (P>0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). The 1-year overall survival curve of the observation group was better than that of the control group (P<0.05). 【Conclusion】 The treatment regimen of osimertinib combined with gemcitabine for elderly patients with advanced NSCLC and renal metastasis is significantly effective,which can reduce tumor markers, provide short-term survival benefits, and has good safety.

Relationship of GTPase-Activating Protein SH3 Domain-Binding Protein 1 Expression in Non-Small Cell Lung Cancer with Radiotherapy Efficacy

【Objective】To investigate the expression of GTPase-activating protein SH3 domain-binding protein 1 (G3BP1) in tissues of patients with non-small cell lung cancer (NSCLC) and its relationship with the efficacy of radiotherapy. 【Methods】The expression levels of G3BP1 protein were measured in the tissues of 133 NSCLC patients. The relationship between G3BP1 expression and NSCLC clinicopathological parameters as well as radiotherapy efficacy was analyzed. 【Results】There were statistically significant differences in G3BP1 protein levels among NSCLC tissues with different tumor diameters, TNM stages, and lymph node metastasis (P<0.05). The G3BP1 protein level in the radiotherapy effective group was lower than that in the ineffective group, with a statistically significant difference (P<0.05). The area under the receiver operating characteristic (ROC) curve (AUC) for evaluating the efficacy of G3BP1, optimal cutoff point, sensitivity, and specificity were 0.802, 2.33, 86.54%, and 61.73%, respectively. Logistic multivariate regression analysis showed that tumor diameter, TNM stage, lymph node metastasis, acute radiation pneumonitis, and G3BP1 level were independent risk factors affecting the efficacy of radiotherapy in NSCLC (P<0.05). 【Conclusion】G3BP1 is related to tumor diameter, TNM stage, and lymph node metastasis in NSCLC. Its level can provide a reference for evaluating the efficacy of radiotherapy in patients.

The Influence of Lesion Location and Age on Postoperative Complications of ESD in Patients with Early Gastrointestinal Tumors

【Objective】 To investigate the influence of lesion location and age on postoperative complications of endoscopic submucosal dissection (ESD) in patients with early gastrointestinal tumors. 【Methods】A total of 89 patients with early gastrointestinal tumors admitted to our hospital from February 2018 to March 2021 were selected, and all patients underwent ESD treatment. We collected statistics on the incidence of complications one month after ESD surgery, analyzed the factors that affect the occurrence of complications in patients with early gastrointestinal tumors after ESD surgery, and analyzed the incidence of complications in patients of different age groups and different lesion sites. 【Results】Among 89 patients with early gastrointestinal tumors, 15 cases (16.85%) developed complications one month after surgery. Logistic multiple regression analysis showed that age ≥ 60 years, location of lesions in the colon, and long-term use of antithrombotic drugs were independent factors affecting the occurrence of complications after ESD surgery in patients with early gastrointestinal tumors (P<0.05). The incidence of complications in patients aged ≥ 60 years is higher than that in patients aged < 60 years (P<0.05). The incidence of complications in patients with colorectal lesions was higher than that in patients with esophageal lesions (P<0.05). 【Conclusion】Early digestive tract tumor patients have a high risk of postoperative complications after ESD surgery. The postoperative complications of ESD surgery in early digestive tract tumor patients are related to the site of the lesion and age. Clinical attention should focus on elderly patients and patients with colon lesions.

Value of ASPECTS in Evaluating Hemorrhagic Transformation after Alteplase Thrombolysis in Patients with Acute Cerebral Infarction

【Objective】 To investigate the value of the Alberta Stroke Program Early CT Score (ASPECTS) in evaluating hemorrhagic transformation (HT) after alteplase thrombolysis in patients with acute cerebral infarction (ACI). 【Methods】 A total of 92 ACI patients treated with alteplase thrombolysis underwent cranial CT scans at initial diagnosis. The occurrence of HT after thrombolysis was observed, and patients were divided into the HT group and non-HT group accordingly. ASPECTS scores and related clinical data of the two groups were compared. The value of ASPECTS in assessing the risk of HT after thrombolysis was analyzed using the receiver operating characteristic (ROC) curve, and multivariate logistic regression was used to analyze the influencing factors of HT. 【Results】 Among the 92 ACI patients treated with thrombolysis, 11 had type 1 and 3 had type 2 of hemorrhagic infarction, while 10 had parenchymal hematoma type 1 and 2 had parenchymal hematoma type 2. So there were a total of 26 cases (28.26%) in the HT group. The rest 66 cases (71.74%) without HT were included in the non-HT group. The proportion of patients with atrial fibrillation and National Institutes of Health Stroke Scale (NIHSS) scores in the HT group were higher than that in the non-HT group (P<0.05), and the ASPECTS scores were lower in the HT group than in the non-HT group (P<0.05). ROC analysis confirmed that ASPECTS could be used to assess the risk of HT after thrombolysis in ACI patients, with a sensitivity of 80.32% and a specificity of 79.02%. Multivariate logistic regression analysis showed that atrial fibrillation, NIHSS score, and ASPECTS score were independent influencing factors for HT after thrombolysis (P<0.05). 【Conclusion】 The risk of HT after alteplase thrombolysis is high in ACI patients. ASPECTS can be used as an effective early predictor of HT occurrence: the lower the score, the higher the risk of HT.

The Efficacy of Levocarnitine Combined with Trimetazidine in the Treatment of Dilated Cardiomyopathy and its Impact on Patients' Cardiac Function

【Objective】To investigate the clinical efficacy of levocarnitine combined with trimetazidine in the treatment of dilated cardiomyopathy (DCM) and its impact on patients' cardiac function. 【Methods】A total of 102 DCM patients admitted to our hospital from January 2022 to February 2023 were selected and randomly divided into a control group (treated with trimetazidine in addition to conventional medication) and an observation group (treated with levocarnitine in addition to the control group) using a random number table method, with 51 patients in each group. Two groups of therapeutic effects were compared, including pre-and post-treatment cardiac function indicators such as cardiac index (CI), left ventricular ejection fraction (LVEF), left ventricular end diastolic diameter (LVEDD), laboratory indicators such as periostin protein, transforming growth factor-β1 (TGF-β1), soluble growth stimulating gene 2 protein (sST2), cardiac troponin T (cTnT), creatine kinase isoenzyme (CK-MB), and myoglobin (Mb). 【Results】The total effective rate of the observation group was significantly higher than that of the control group, and the difference was statistically significant (P<0.05). After treatment, the CI and LVEF of both groups were higher than before treatment, and the observation group was higher than the control group; The LVEDD of both groups was lower than before treatment, and the observation group was lower than the control group, with a statistically significant difference (P<0.05). After treatment, cTnT, CK-MB, Mb, Periostin protein, TGF-β1, and sST2 in both groups were lower than before treatment, and the observation group was lower than the control group, with statistical significance (P<0.05). 【Conclusion】The combination of levocarnitine and trimetazidine is effective in treating DCM patients, improving their cardiac function and inhibiting ventricular remodeling.

Clinical Efficacy of Liangxue Huoxuetang Combined with UVB Phototherapy in the Treatment of Blood Heat Psoriasis Patients

【Objective】To explore the clinical efficacy of Liangxue Huoxuetang combined with narrow-band ultraviolet B (UVB) phototherapy in the treatment of blood heat psoriasis patients.【Methods】A total of 80 patients with blood heat psoriasis treated in our hospital from January 2021 to May 2022 were selected and divided into a control group (treated with triamcinolone acetonide urea cream combined with Liangxue Huoxuetang) and an observation group (treated with UVB phototherapy on the basis of the control group) according to admission order, with 40 cases in each group. Both groups were treated for 4 weeks. The clinical efficacy, psoriasis area and severity index (PASI) score, stratum corneum water content, sebum content, levels of related factors [interleukin-17 (IL-17), vascular endothelial growth factor (VEGF), monocyte chemoattractant protein-1 (MCP-1)], and incidence of adverse reactions were compared between the two groups. 【Results】The clinical efficacy of the observation group was significantly better than that of the control group (P<0.05). After treatment, the PASI scores, IL-17, VEGF, and MCP-1 in both groups decreased compared to before treatment, and the above indicators in the observation group were lower than those in the control group (P<0.05); After treatment, the water content and sebum content of the stratum corneum in both groups significantly increased compared to before treatment (P<0.05); And the water content and sebum content of the stratum corneum in observation group were higher than those in the control group (P<0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups of patients (P>0.05). 【Conclusion】 The combination of Liangxue Huoxuetang and UVB phototherapy has good clinical efficacy in treating patients with blood heat psoriasis, significantly improving the degree of skin lesions and related factors, and has good safety.

Changes and Clinical Significance of Coagulation Function in Severe Preeclamps Complicated with HELLP Syndrome

【Objective】To analyze the changes of coagulation function and its clinical significance in patients with severe preeclampsia (SPE) complicated with HELLP syndrome. 【Methods】Ninety-eight SPE patients admitted to our hospital from January 2018 to December 2022 were selected and divided into group A (with HELLP syndrome, n=30) and group B (without HELLP syndrome, n=68) according to whether the patients would develop HELLP syndrome during hospitalization. Coagulation indexes [fibrinogen (FIB), prothrombin time (PT), activated partial thromboplastin time (APTT), fibrinogen degradation product (FDP), D-dimer (D-D)], interleukin (IL-6) and C-reactive protein (CRP) were compared between the two groups at admission. Multiple Logistic regression was used to analyze the influencing factors of SPE complicated with HELLP syndrome.【Results】There were no significant differences in pregnant type, age, gestational week, body weight, respiratory rate (RR), systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR), mean arterial pressure (MAP), creatinine (Scr) and blood urea nitrogen (BUN) between the two groups (P>0.05). The PT and APTT measurements in group A were longer than those in group B, the levels of D-D, FDP, IL-6 and CRP were higher than those in group B, and the levels of FIB and red blood cell (RBC) count were lower than those in group B, with statistical significance (P<0.05). Multivariate Logistic regression analysis showed that PT, APTT, D-D, FDP, FIB and RBC were the influencing factors of HELLP syndrome in SPE patients (P<0.05). 【Conclusion】Abnormal RBC, PT, APTT, FIB, D-D and FDP are risk factors for HELLP syndrome in SPE patients. Monitoring the changes of the above indexes can provide references for clinicians to evaluate the condition, efficacy and prognosis of SPE patients with HELLP syndrome.

Expression and Diagnostic Value of Endocan and Galectin-3 in Serum of Patients with Microvascular Angina

【Objective】To investigate the expression and diagnostic value of endothelial cell-specific molecule-1 (Endocan) and Galectin-3 in the serum of patients with microvascular angina (CMSA). 【Methods】A total of 87 CMSA patients treated at Yanji County People's Hospital were selected as the observation group, while 87 healthy individuals during the same period were selected as the control group. The relevant data and serum Endocan and Galectin-3 expression levels of the two groups were compared. Logistic multivariate regression analysis was used to analyze the influencing factors of CMSA. The diagnostic value of serum Endocan and Galectin-3 levels for CMSA was evaluated by the area under the receiver operating characteristic (ROC) curve (AUC). 【Results】The proportions of diabetes, hypertension, and high levels of fasting blood glucose, serum Endocan and Galectin-3 in the observation group were higher than those in the control group (P<0.05). Logistic multivariate regression analysis showed that increased levels of diabetes, hypertension, fasting blood glucose, serum Endocan, and Galectin-3 were risk factors for CMSA (P<0.05). ROC analysis showed that the AUC of serum Endocan, Galectin-3, and their combined detection for diagnosing CMSA were 0.731, 0.785, and 0.891, respectively. 【Conclusion】There is a high expression of serum Endocan and Galectin-3 in CMSA patients, and the diagnostic valueefficacy of serum Endocan and Galectin-3 in CMSA is high.

【Objective】To explore the effect of nutritional risk grading intervention on the nutritional status of patients with cirrhosis. 【Methods】A total of 86 patients with liver cirrhosis admitted to our hospital from August 2021 to August 2022 were randomly divided into the control group (receiving routine nutritional intervention measures) and the observation group (receiving nutritional risk grading intervention measures on the basis of the control group) using a random number table method, with 43 patients in each group. The levels of serum albumin (ALB), total protein (TP), aspartate aminotransferase (AST), and alanine aminotransferase (ALT) were compared between two groups before and after intervention, and the incidence of complications was compared between the two groups of patients. 【Results】After intervention, the serum levels of ALB and TP in both groups of patients were higher than before intervention, and the observation group was higher than the control group, with statistical significance (P<0.05). After intervention, the serum levels of AST and ALT in both groups of patients were lower than before intervention, and the observation group was lower than the control group, with a statistically significant difference (P<0.05). The incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). 【Conclusion】Nutritional risk grading intervention measures can help improve the nutritional status and liver function of patients with cirrhosis, and reduce the incidence of complications.

Clinical Effect of Electromagnetic Navigation Assisted Neuroendoscopic Hematoma Clearance Surgery in the Treatment of Hypertensive Intracerebral Hemorrhage

【Objective】To investigate the efficacy of electromagnetic navigation assisted neuroendoscopic hematoma evacuation in the treatment of hypertensive intracerebral hemorrhage (HICH) patients and its impact on short-term prognosis. 【Methods】 A retrospective analysis was conducted on the clinical data of 102 HICH patients admitted to our hospital. According to different surgical plans, they were divided into the observation group (treated with electromagnetic navigation assisted neuroendoscopic hematoma evacuation) and the control group (treated with conventional neuroendoscopic hematoma evacuation), with 51 cases in each group. Two groups of perioperative indicators, National Institutes of Health Stroke Scale (NIHSS) scores before and after surgery, Modified Rankin Scale (mRS) scores, oxidative stress indicators (SOD, GSH-Px, MDA), vascular endothelial function indicators (AVP, Ang-1, ET), and incidence of complications were compared. 【Results】The residual hematoma volume and hematoma clearance rate in the observation group were lower than those in the control group after surgery, and the hospitalization time was shorter than that in the control group, with statistical significance (P<0.05). At 1 and 3 months after surgery, the NIHSS and mRS scores in both groups were lower than before surgery, and the observation group was lower than the control group, with statistical significance (P<0.05). One month after surgery, the levels of SOD and GSH-Px in the serum of both groups were higher than before surgery, and the level of MDA in the serum was lower than before surgery. The differences in these indicators between the two groups were statistically significant (P<0.05). One month after surgery, the levels of serum AVP, Ang-1, and ET in both groups were lower than before surgery, and the observation group was lower than the control group, with statistical significance (P<0.05). The incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). 【Conclusion】Electromagnetic navigation assisted neuroendoscopic hematoma removal surgery can improve neurological function and prognosis in HICH patients, optimize perioperative indicators, alleviate oxidative stress, improve vascular endothelial function, and reduce the risk of complications.

Diagnostic Value of High Frequency Ultrasound Combined with Ultrasonographic Microvascular Imaging in Diagnosis of Neck Lymph Node Metastasis in Papillary Thyroid Carcinoma

【Objective】To investigate the diagnostic value of high frequency ultrasound combined with ultrasonographic microvascular imaging in neck lymph node metastasis of papillary thyroid carcinoma. 【Methods】A retrospective analysis was performed on 60 patients with a total of 90 lymph nodes who underwent thyroid papillary carcinoma surgery and neck lymph node dissection in our hospital from January 2020 to June 2023. High-frequency ultrasound, Ultrasonographic microvascular imaging, and high-frequency ultrasound combined with Ultrasonographic microvascular imaging were performed before surgery to compare the accuracy of diagnosing metastatic lymph nodes.【Results】A total of 90 cervical lymph nodes were metastatic in 60 patients, 65 of which were in the central neck and 25 in the lateral neck.The accuracy of high-frequency ultrasound combined with Ultrasonographic microvascular imaging in diagnosing cervical lymph node metastasis was significantly higher than that of high-frequency ultrasound and Ultrasonographic microvascular imaging alone (P<0.05). High-frequency ultrasonography showed that 44 patients with thyroid papillary carcinoma had short rod-shaped and dot-shaped blood flow signals inside or around the lesions , and 5 patients had branch-shaped blood flow signals inside the lesions. Ultrasonographic microvascular imaging showed that short rod-shaped and dot-shaped blood flow signals inside or around the lesions were seen in 40 patients, and branch-shaped blood flow signals inside the lesions were seen in 14 patients. 【Conclusion】High frequency ultrasound combined with ultra-micro angiography is of high diagnostic value in neck lymph node metastasis of papillary thyroid carcinoma.

Relationship of Fibrinogen and D-Dimer Levels with Cognitive Dysfunction in Patients with Cerebral Infarction

【Objective】 To investigate the relationship between serum fibrinogen (FIB) and D-dimer (D-D) levels and cognitive dysfunction in patients with cerebral infarction.【Methods】 Eighty-two patients with cerebral infarction were divided into mild, moderate, and severe groups according to the degree of cognitive dysfunction. Additionally, 30 healthy individuals undergoing physical examination during the same period were selected as the control group. Serum FDP and D-D levels were compared among all groups, and cognitive function was assessed as well. Pearson analysis was used to evaluate the correlation between FIB and D-D levels and the degree of cognitive dysfunction. Spearman analysis was performed to determine the correlation of MoCA scores with FIB and D-D levels.【Results】 Compared to the control group, FIB and D-D levels were significantly higher in the mild, moderate, and severe groups (P<0.05). FIB and D-D levels in the severe group were higher than those in the mild and moderate groups (P<0.05). Levels of FIB and D-D were positively correlated with the degree of cognitive dysfunction in patients with cerebral infarction (both P<0.05). =0.531 and 0.478, respectively).【Conclusion】 The degree of cognitive dysfunction in patients with cerebral infarction is positively correlated with both serum FIB and D-D levels, which can be served as independent predictors of the severity of cognitive dysfunction in these patients.

Application Effect and Influencing Factors of Statins in Secondary Prevention of Ischemic Cerebrovascular Disease

【Objective】To explore the application effect and influencing factors of statins in the secondary prevention of ischemic cerebrovascular disease (ICVD).【Methods】A total of 109 ICVD patients treated in our hospital were followed up for one year. One patient died, and two were lost to follow-up. Patients were divided into a recurrence group and a non-recurrence group based on whether ICVD recurred. Univariate analysis was used to compare the clinical data of the two groups and to explore the factors affecting the preventive effect of statins. 【Results】Among the 106 patients, 15 cases (14.15%) had ICVD recurrence, while 91 cases (85.85%) did not have recurrence of ICVD. There were no statistically significant differences in age, gender, family history, smoking history, alcohol consumption history, white blood cell count, and blood glucose levels between the recurrence and non-recurrence groups (P>0.05). However, significant differences were found in body mass index (BMI), educational level, medical payment method, number of comorbidities, types of medications taken, and compliance (P<0.05). Multivariate regression analysis showed that high BMI, low educational level, self-payment for medical expenses, a high number of comorbidities, taking multiple types of medications, and low compliance were independent risk factors for recurrence in the secondary prevention of ICVD with statins (P<0.05). 【Conclusion】 The effect of statins in the secondary prevention of ICVD is influenced by factors such as patients' BMI, educational level, medical payment method, comorbidities, types of medications, and compliance. Optimizing secondary prevention requires attention to these related factors.

Clinical Efficacy of Ultrasound-guided Intervention Therapy with Lauromacrogol Combined with Postoperative Oral Gestrinonein the Treatment of Patients with Ovarian Endometriotic Cysts

【Objective】To explore the clinical effect of ultrasound-guided intervention with Lauromacrogol combined with postoperative oral Gestrinonein in the treatment of patients with ovarian endometriotic cysts. 【Methods】A total of 82 patients with ovarian endometriotic cysts treated at the People's Hospital of Fufeng County, Shaanxi Province from January 2021 to December 2023 were selected. They were divided into a control group and an observation group based on different treatment methods, with 41 cases in each group. The control group received ultrasound-guided intervention therapy with Lauromacrogol, while the observation group received ultrasound-guided intervention therapy with Lauromacrogol combined with postoperative oral Gestrinonein treatment. Two groups of surgical indicators (surgery time, hospital stay), incidence of postoperative complications, and levels of ovarian function indicators [follicle stimulating hormone (FSH) and luteinizing hormone (LH)] before and 5 days after treatment were compared.【Results】The hospitalization time of the observation group was shorter than that of the control group (P<0.05). The total effective rate of the observation group was 95.12%, significantly higher than 78.05% of the control group (P<0.05). Compared with preoperative treatment, FSH and LH levels increased in both groups 5 days after treatment, but the FSH and LH levels in the observation group were lower than those in the control group 5 days after treatment(P<0.05). The total incidence of complications in the observation group was 7.32%, significantly lower than the control group's 24.39%(P<0.05). 【Conclusion】The combination of ultrasound-guided Lauromacrogol intervention with oral Gestrinonein is effective in treating patients with ovarian endometriotic cysts. It can promote postoperative recovery, improve ovarian function, and reduce the incidence of complications.
