324 Effect of β-Caroteneon the Autophagy Mechanism of Intestinal Recess Epithelial Cells IEC-6 Induced by Lipopolysaccharide

【Objective】To explore the effect of β-Carotene(βC) on the autophagy mechanism of intestinal recess epithelial cells IEC-6induced by lipopolysaccharide.【 Methods 】 The intestinal epithelial cell line IEC-6 was induced by lipopolysaccharide (LPS) to construct the in vitro model of neonatal necrotizing enterocolitis (NEC).The PI3K inhibitor voxtalisib (VOX) was used to analyze the potential mechanism of βC regulating autophagy. IEC-6 cells were divided into control group, LPS group and LPS+βC group and LPS+βC+Vox group after routine culture in vitro. CCC-8 was used to detect cell activity. mRFPGFPLC3 autophagy double labeled adenovirus was transfected to detect intracellular autophagy.The expressions of autophagy related proteins and PI3K/Akt/mTOR signal pathway related proteins were detected by Western blotting.【Rresults】Compared with the control group, the cell activity of LPS group decreased significantly (P<0.05), LC3 Ⅱ/Ⅰ increased significantly (P<0.05), the expression of P62 protein decreased significantly (P<0.05), and the number of autophagy spots increased significantly (P<0.05). Compared with the control group, the cell activity in the LPS+βC group increased significantly (P<0.05), LC3 Ⅱ/Ⅰ decreased significantly (P<0.05), the protein expression of P62 increased significantly (P<0.05), and the number of autophagy spots decreased significantly (P<0.05). Compared with the LPS group, the cell activity in the LPS+βC group increased significantly (P<0.05), LC3 Ⅱ/Ⅰ decreased significantly (P<0.05). The protein expression of P62, p-PI3K, p-AKT and p-mTOR increased significantly (P<0.05), and the number of autophagy spots decreased significantly (P<0.05). 【Conclusion】 βC can reduce the autophagy of IEC-6 cells induced by LPS, and its mechanism is related to the activation of PI3K/AKT/mTOR signaling pathway.

328 Changes and Significance of Intraoperative Neurophysiology in Patients with Cervical Spondylotic Myelopathy with Abnormal Intramedullary MRI Signal

【Objective】To investigate the changes of intraoperative neuroelectrophysiological monitoring (IONM) indexes, motor evoked potentials (MEPs) and somatosensory evoked potentials (SEPs) in patients with cervical spondylotic myelopathy (CSM) with abnormal MRI signals, and to analyze the relationship between abnormal MRI signals and IONM changes.【Methods】The clinical and IONM data of 150 CSM patients with abnormal MRI signals from January 2019 to January 2021 were collected and divided into two groups according to their signal characteristics. Group A: T2WIs high signal (120 cases); Group B: T2WIs high signal and T1WIs low signal (30 cases). The IONM data of important surgical nodes were recorded. 【Results】 Group A: 9 cases had IONM changes, 1 case had temporary nerve injury, and there was no new permanent nerve injury; Group B: 18 cases of IONM changes, 5 cases of temporary nerve injury and 2 cases of new permanent nerve injury. There was significant difference in IONM between the two groups (MEPs, 5/120 vs 12/30,P<0.05; SEPs, 4/120 vs 6/30,P<0.05). In addition, there was significant difference in IONM amplitude before and after decompression in the group B [MEPs,(134±30.2) μV vs (65±26.2) μV, P<0.05;SEPs, (2.4±0.9) μV vs (1.3±0.7) μV, P<0.05].【Conclusion】In CSM patients, high T2WIs signal and low T1WIs signal are more prone to IONM changes than high T2WIs signal alone, resulting in nerve injury. In the process of CSM spinal cord decompression, we should pay attention to the changes of IONM, and take appropriate surgical intervention measures when necessary.

332 Treg and Treg-related Cytokines in Patients with Femoral Neck Fracture and PMOP

【Objective】To investigate the frequency of Treg and levels of Treg-related cytokines (IL-10 and TGF-b) in patients with femoral neck fracture and post-menopausal osteoporosis(PMOP).【Methods】From October 2016 to September 2020, 30 patients with femoral neck fracture and PMOP(the PMOP fracture group),30 patients with femoral neck fracture and normal BMD (in the non-PMOP fracture group,PMOP fracture group),and 30 healthy control subjects (the control group) were enrolled for the study. Chemiluminescence method was used to detect serum osteocalcin (BGP), type Ⅰ collagen carboxy-terminal peptide (b-CTX) and type Ⅰ collagen N-terminal propeptide (PINP). Flow cytometry was used to detect the frequency of Treg cells in each group. The levels of interleukin 10 (IL-10) and transforming growth factor beta (TGF-β) were detected by ELISA.Venous blood was drawn to separate PBMCs and serum, then the frequency of Treg and level of IL-10 and TGF-β was examined and compared among the three groups. The serum level of BGP,PINP and b-CTX were tested and compared as well. 【Results】The frequency of Treg cells and the levels of serum IL-10 and TGF-β in the patients with PMOP fracture were lower than those in the non-PMOP fracture group and the control group (P<0.05). There was no statistical difference between the control group and the non-PMOP fracture group.The serum levels of b-CTX and PINP in the PMOP fracture group were higher than those in the non-PMOP fracture group and the control group, and the BGP level was lower than that in the non-PMOP fracture group and the control group, and the differences were statistically significant (P<0.05). Pearson correlation analysis showed that serum b-CTX and PINP levels were negatively correlated with Treg cell frequency (r=-0.262 and r=-0.255,respectively, both P<0.05).【Conclusion】The frequency of Treg and Treg-related cytokine levels were decreased in patients with femoral neck fracture and PMOP, which might participate in the pathogenesis of osteoporosis.

335 Relationship of Levels of EPO and Hepcidin in serum with Prognosis of Allogeneic Hematopoietic Stem Cell Transplantation in Patients with Myelodysplastic Syndrome

【Objective】To investigate the correlation of serum erythropoietin (EPO) and ferromodulin (Hepcidin) levels with the prognosis of myelodysplastic syndrome (MDS) after allogeneic hemopoietic stem cell transplantation (myelodysplastic syndrome-HSCT).【Methods】A total of 175 patients with MDS treated with allo-HSCT in our hospital from January 2018 to January 2020 were selected for the study. They were divided into the survival group (n=113) and the death group (n=62) based on the 36-month follow-up results. Patients' gender, age, the International Prognostic Integration System Revision (IPSS-R), pre-HSC cell transplant complications index (HCT-CI), HLA compatibility, chromosomal karyotype, bone marrow primitive cell ratio, neutrophil count, platelet count, hemoglobin, EPO, Hepcidin were collected and compared between groups. Spearman analysis was applied to evaluate the relationship between serum EPO and Hepcidin in MDS patients. Survival analysis of MDS patients was performed using the product limit (K-M) method and the Log-rank test was compared the significance.Multivariate Logistic regression model was used to analyze the factors affecting the prognostic survival status of patients with MDS who received allo-HSCT.【Results】The proportion of IPSS-R, HCT-CI, chromosome karyotype, and bone marrow primitive cells in the survival group was compared with the death group, and the difference was statistical significant (P<0.05). In contrast to the survival group, serum EPO level was lower in the death group (P<0.05), while the level of Hepcid in was higher (P<0.05).Compared to patients with high level of EPO, those with low level of EPO had a lower survival rate (P<0.05) and a shorter survival period (P<0.05); Compared to patients with low level of Hepcidin, those with high level of Hepcidin had a lower survival rate (P<0.05) and the survival time was shorter(P<0.05).Spearman analysis showed that serum EPO and Hepcidin levels in MDS patients were negatively correlated (P<0.05)Multivariate Logistic regression analysis showed that the IPSS-R was poor to extremely poor, HCT-CI≥3, low EPO level, and high Hepcidin level were independent risk factors affecting the prognostic survival status of MDS patients (P<0.05).【Conclusion】The serum EPO and Hepcidin levels of MDS patients are closely associated with the prognostic survival of allo-HSCT, which may be used for prognostic death risk assessment.

340 Clinical Observation of Minimally Invasive Plate Internal Fixation with Three-point Positioning of Kirschner Wire and Preservation of Pronator Muscle in the Treatment of Distal Radius Fractures

【Objective】To investigate the clinical effect of minimally invasive plate internal fixation with three-point Kirschner wire and preservation of pronator muscle in the treatment of distal radius fractures.【Methods】A total of 118 patients with distal radius fractures treated in Qishan County Hospital from July 2018 to July 2021 were selected and divided into control group and observation group according to different treatment methods, with 59 cases in each group. The control group was treated with conventional minimally invasive plate internal fixation with preservation of pronator muscle, and the observation group was treated with minimally invasive plate internal fixation with preservation of pronator muscle by three-point positioning of Kirschner wire. The perioperative indexes (operation time, intraoperative bleeding, number of times of anterior pronator submuscular plate adjustment, incision length) and the clinical effect of 3 months after operation were observed. The wrist range of motion, wrist function (Gartland Werley score), upper limb function (DASH score) and complications were compared and observed between the two groups at 3 months after operation. 【Results】There was no significant difference between the two groups in operation time, intraoperative bleeding, incision length and excellent rate of treatment effect (P>0.05); The number of adjustment and release of pronator submuscular plate in the observation group was less than that in the control group (P<0.05); Three months after operation, the pronation angle and radial deviation angle in the observation group were greater than those in the control group (P<0.05); At 1 and 3 months after operation, the Gartland Werley and DASH scores in the observation group were lower than those in the control group (P<0.05); There was no significant difference in the incidence of complications between the two groups (P>0.05). 【Conclusion】Kirschner wire three-point positioning and minimally invasive plate internal fixation with preservation of pronator muscle in the treatment of distal radius fractures can reduce the number of adjustment and placement of pronator muscle plate, improve wrist range of motion, improve wrist function and upper limb function, and has high safety, which is worthy of clinical application.

344 Effects of Cluster Nursing on Self-acceptance, Chemotherapy Tolerance and Quality of Life in Patients with Malignant Bonetumors Undergoing Chemotherapy

【Objective】 To explore the effect of cluster nursing on self acceptance, chemotherapy tolerance and quality of life of chemotherapy patients with malignant bone tumors.【Methods】86 patients with malignant bone tumors diagnosed and treated with chemotherapy in our hospital from March 2019 to March 2020 were randomly divided into experimental group and control group, with 43 cases in each group. The experimental group received cluster nursing, and the control group received routine nursing. The intervention began at the first hospitalization until the end of the chemotherapy cycle. General information questionnaire, self acceptance questionnaire, chemotherapy toxicity and side effects and quality of life scale were used to investigate. 【Results】 There was no significant difference in general information, self acceptance scores and quality of life scores between the two groups before intervention (P>0.05); After the intervention, the self acceptance score of the experimental group was (38.23±4.32)score, which was significantly higher than that of the control group (32.23±4.28)score (t=3.628, P<0.05); After the intervention, the incidence of bone marrow suppression, oral mucositis, nausea and vomiting, gastrointestinal reaction and early termination of chemotherapy in the experimental group were lower than those in the control group (P<0.05); After the intervention, the overall health function, physical function, role function and social function of the experimental group were significantly higher than those of the control group (P<0.05). The scores of all single scales in the experimental group were lower than those of the control group. The differences in the scores of shortness of breath, insomnia, anorexia and constipation between the two groups were statistically significant (P<0.05). 【Conclusion】Cluster nursing can help patients with malignant bone tumors better accept themselves, improve the toxic and side effects of chemotherapy, and improve their chemotherapy tolerance and quality of life.

349 Relationship between Serum Nesfatin-1 Level and Mild Cognitive Impairment in Patients with Type 2 Diabetes Mellitus Complicated with Hypertension

【Objective】To investigate the correlation between serum Nesfatin-1 level and mild cognitive impairment in patients with Type 2 diabetes mellitus complicated with hypertension. 【Methods】A total of 786 elderly patients with T2DM treated in our hospital from December 2018 to January 2020 were divided into simple T2DM group (group A), T2DM + hypertension subgroup (subgroup B1) and T2DM + hypertension + MCI subgroup (subgroup B2). The levels of fasting blood glucose (FPG), fasting insulin, homocysteine (Hcy), total cholesterol (TC), low density lipoprotein (LDL-C), triglyceride (TG), high density lipoprotein (HDL-C), high sensitivity C-reactive protein (hs-CRP), neuronal enolase (NSE), glycosylated hemoglobin (HbA1c) and Nesfatin-1 were compared, and the insulin sensitivity index (ISI) was calculated. Logistic multivariate regression analysis was used to analyze the related risk factors of MCI in elderly T2DM patients with hypertension. 【Results】 The levels of FPG, Nesfatin-1, LDL-C, ISI and Hcy in the B1 subgroup and the B2 subgroup were significantly higher than those in the group A (P<0.05); The levels of BMI, FPG, Nesfatin-1, ISI, LDL-C and NSE in the subgroup B2 were significantly higher than those in the subgroup B1 (P<0.05), and the level of NSE was significantly higher than that in the group A (P<0.05). Pearson correlation analysis showed that serum Nesfatin-1 was positively correlated with BMI, FPG, HbA1c, NSE and Hcy (P<0.05), and negatively correlated with ISI (P<0.05). Logistic multivariate regression analysis showed that FPG, BMI and Nesfatin-1 were the risk factors of MCI in T2DM patients with hypertension. 【Conclusion】 Serum Nesfatin-1 level, FPG and BMI are the risk factors of MCI in T2DM patients with hypertension. It is of great significance to effectively control body weight, improve fasting blood glucose level and increase insulin sensitivity.

352 Effect of Early PFMT on Pelvic Floor Function Rehabilitation of Primipara after Vaginal Delivery

【Objective】To investigate the effect of early pelvic floor muscles training (PFMT) on the rehabilitation of pelvic floor function after vaginal delivery. 【Methods】 A total of 223 primiparas who delivered vaginally in Guigang People's Hospital from January to October 2020 were randomly divided into observation group and control group. The control group implemented natural rehabilitation of postpartum pelvic floor, and the observation group began PFMT intervention on the second day postpartum on the basis of the control group. Three months after the intervention, the pelvic floor muscle strength, the score of the pelvic floor dysfunction inventory-20 (PFDI-20), urinary incontinence and anal incontinence were compared between the two groups before and after the intervention. 【Results】 42 days after delivery, the muscle strength of Class Ⅱ muscle fibers in the observation group recovered better than that in the control group. The incidence of urinary incontinence and PFDI-20 score in the observation group were significantly lower than those in the control group (P<0.05); Three months after delivery, the recovery effect of Class Ⅰ and class Ⅱ muscle fibers in the observation group was better than that in the control group. The incidence of urinary incontinence and PFDI-20 score in the observation group were significantly lower than those in the control group (P<0.05). 【Conclusion】PFMT intervention can improve the muscle strength of postpartum pelvic floor muscle, reduce the incidence of postpartum urinary incontinence and improve the quality of life of pregnant women.

356 Study on Relationship of Severity of Illness in Patients Bitten by Agkistrodon with Serum NLR and PLR Levels

【Objective】To investigate the relationship the severity of agkistrodon bite with neutrophil-to-lymphocyte ratio (NLR)and platelet-lymphocyte ratio (PLR) levels in patient serum.【Methods】There are 143 cases of patients bitten by agkistrodon in total who were admitted to our Hospital. Of which, 115 patients were selected based on the inclusion and exclusion criteria. According to the Snake Bite Severity Scale (SSS) score, they were divided into 45 cases in the mild group,(0~3 points), 40 cases in the moderate group (4~7 points), and 30 cases in the severe group (8~20 points). During the same time period, 20 healthy volunteers were randomly selected from Health Management Center and divided into the healthy control group. The levels of NLR, PLR, high-sensitivity C-reactive protein(hs-CRP), procalcitonin (PCT), interleukin-1 (IL-1) and interleukin-6 (IL-6) in each group were compared. After that, the relationship of severity of the disease in patients bitten by agkistrodon with levels of NLR and PLR was analyzed.【Results】Serum NLR and PLR were positively correlated with the severity of snake bite (P<0.05). Compared with the control group, the levels of NLR and PLR in the snake poisoning groups were significantly increased (P<0.05). Compared to patients in the mild and moderate groups, the levels of NLR and PLR in the severe group were significantly higher (P<0.05). Compared with the patients in the mild group, the levels of NLR and PLR in the moderate group were Increased (P>0.05). Compared to the control group, the levels of hs-CRP, PCT, IL-1, and IL-6 were significantly higher in the agkistrodon poisoning groups (all P<0.05). Among the poisoning groups, levels of hs-CRP, PCT, IL-1, and IL-6 in the mild group were lower than those in the moderate group and the severe group, however, the difference between the mild group and the moderate group was not statistically significant (All P>0.05) and the difference between the mild group and the severe group was statistically significant (P<0.05).【Conclusion】Both NLR and PLR are positively correlated with the severity of patients bitten by agkistrodon.

359 Clinical Comparison of Distal Tibial Posterolateral Approach and Anteromedial Approach in the Treatment of Pilon Fracture

【Objective】To investigate the effects of distal tibial posterolateral approach and anteromedial approach on Pilon fracture and perioperative complications. 【Methods】A total of 78 patients with Pilon fracture treated in the Department of orthopedics, Zhabei Central Hospital, Jing'an District, Shanghai from January 2018 to January 2021 were randomly divided into observation group and control group. The control group adopted the anteromedial approach of distal tibia, and the observation group adopted the posterolateral approach of distal tibia. The basic data, surgical indexes, recovery and perioperative complications of the two groups were compared. 【Results】The operation time of the observation group was longer and the fracture healing time was shorter than that of the control group (P<0.05). One month after operation, the visual analogue pain score (VAS) of the observation group was lower than that of the control group, and the ankle score of the American foot and ankle surgery association (AOFAS) was higher than that of the control group (P<0.05). At 6 months after operation, the VAS of the two groups was lower than that of 1 month after operation, while the AOFAS ankle score of the control group was higher than that of 1 month after operation (P<0.05); The total excellent rate of the observation group was higher than that of the control group(87.18% vs 66.67%,χ2=4.622,P<0.05);There was no significant difference in the incidence of perioperative complications between the two groups (10.26% vs 17.95%,χ2=0.953,P>0.05).【Conclusion】Compared with the anteromedial approach of the distal tibia, the posterolateral approach in the treatment of Pilon fracture can shorten the fracture healing time, reduce postoperative pain, improve AOFAS ankle score, and have a good effect on the recovery of joint function, which is safe and reliable.

363 Clinical Efficacy of Omeprazole, Octreotide and Thrombin in the Treatment of Liver Cirrhosis Complicated with Esophageal and Gastric Variceal Bleeding

【Objective】To investigate the clinical efficacy of omeprazole, octreotide and thrombin in the treatment of liver cirrhosis complicated with esophageal and gastric variceal bleeding. 【Methods】A total of 88 patients with liver cirrhosis complicated with esophageal and gastric variceal bleeding treated in our hospital from April 2018 to April 2021 were prospectively selected. They were randomly divided into observation group and control group, with 44 cases in each group. The control group was treated with omeprazole and octreotide, and the observation group was treated with thrombin on the basis of the control group. The therapeutic effects of the two groups were compared, and the hemostasis time, blood transfusion volume, symptom relief time, hospital stay and adverse reactions were observed.【Results】The total effective rate of the control group was 77.27% (34/44), which was significantly lower than 93.18% (41/44) in the observation group (χ2=4.423,P=0.035 < 0.05). The hemostasis time, symptom improvement time and hospitalization time of the observation group were shorter than those of the control group, and the amount of blood transfusion was lower than that of the control group (P<0.05). The incidence of adverse reactions in the control group was 9.09% (4/44), which was higher than 4.55% (2/44) in the observation group, but the difference was not statistically significant (χ2=4.062, P=0.443>0.05). 【Conclusion】The combination of omeprazole, octreotide and thrombin in the treatment of liver cirrhosis complicated with esophageal and gastric variceal bleeding has good clinical efficacy, good hemostatic effect, good safety and good prognosis.

366 Effects of Salvianolic Acid B on Renal Interstitial Injury in Rats with Chronic Pyelonephritis and its Mechanism

【Objective】 To investigate the effect of salvianolic acid B on renal interstitial injury in rats with chronic pyelonephritis and its mechanism. 【Methods】 Forty SD rats were divided into control group (normal saline gavage), model group (normal saline gavage), salvianolic acid B group (12.5 mg/kg salvianolic acid B gavage) and activator group (12.5 mg/kg salvianolic acid B gavage + 2.5 mg/kg Smad3 activator SRI-011381 intraperitoneal injection), with 10 rats in each group. The rat model of chronic pyelonephritis was established in the model group,the salvianolic acid B group and the activator group. The drug was administered once a day for 6 weeks from the next day. Expression of the contents of serum creatinine, 24-hour urinary protein and renal tissue,αSmooth muscle actin(α-SMA), Smad3, transforming growth factor-β1(TGF-β1) were measured. 【Results】Compared with the control group, the 24-hour urinary protein content in the model group increased (P<0.05); Compared with the control group, the expression of α-SMA of the renal tissue in the salvianolic acid group B and the activator group decreased(P<0.05); Compared with the control group, the expression of α-SMA in the salvianolic acid group B was lower than that in the activator group(P<0.05). 370 Clinical Effect of TURP Combined with Superselective Prostate Artery Embolization in the Treatment of Severe BPH

【Objective】To investigate the clinical effect of transurethral prostatectomy (TURP) combined with superselective prostate arterial embolization (PAE) in the treatment of severe benign prostatic hyperplasia (BPH). 【Methods】 The clinical data of 113 patients with severe BPH treated in our hospital from January 2018 to December 2020 were analyzed retrospectively. According to different surgical schemes, they were divided into observation group (treated with selective PAE combined with TURP, n=57) and control group (treated with TURP, n=56). The perioperative indexes and complications were compared between the two groups; The indexes of oxidative stress, International Prostate Symptom Score (IPSS), urodynamic indexes [maximum urinary flow rate (Qmax), residual urine volume (RUV)], quality of life (QOL score) and erectile function (IIFE-5 score) were compared between the two groups.【Results】The operation time of TURP, the indwelling time of urinary catheter, the continuous washing time of bladder, the length of hospital stay and the amount of bleeding during TURP in the observation group were lower than those in the control group (P<0.05). There was no significant difference in oxidative stress indexes between the two groups before operation and 1 and 5 days after TURP (P>0.05); One day after TURP, the level of serum cortisol, norepinephrine, interleukin-6 and tumor necrosis factor-αin the two groups was higher than that before operation (P<0.05). One month after TURP, QOL,IIFE-5 scores and Qmax of the two groups were higher than those before TURP, and RUV was lower than that before TURP (P<0.05). The incidence of complications in the observation group was lower than that in the control group (P<0.05).【Conclusion】TURP combined with superselective PAE has significant clinical effect in the treatment of severe BPH. It has the advantages of less intraoperative bleeding, high resection efficiency and less complications. The patients have good postoperative quality of life and erectile function.

374 Efficacy and Safety of Dexibuprofen in Children with Juvenile Idiopathic Arthritis

【Objective】To investigate the efficacy and safety of dexibuprofen in children with juvenile idiopathic arthritis (JIA) and the expression of serum pain mediators and inflammatory factors. 【Methods】A total of 84 patients with JRA in our hospital were selected as the research objects and they were randomly divided into two groups, with 42 cases in the control group and 42 cases in the observation group. The control group was treated with naproxen, while the study group was treated with dexibuprofen for 2 weeks. The juvenile arthritis disease activity scale (JADAS27) was used to assess the improvement of clinical symptoms and physical signs before and after treatment. The serum pain mediators [serotonin (5-HT) and prostaglandin E2 (PGE2), slow excitation peptide bradykinin (BK), neuropeptide Y (NPY)] and inflammatory factor [tumor necrosis factor alpha (TNF-α), interleukin 1 (IL-1), interleukin 17 (IL-17), matrix metalloproteinases-3(MMP-3)] expression levels were compared between the two groups as well. The incidence of adverse reactions were also compared. 【Results】The total effective rate of the observation group was 85.71%, which was significantly higher than 64.29% of the control group (P<0.05). The scores of each dimension and total score of JADAS27 in the two groups after treatment were significantly lower than those before treatment (all P<0.05), and the above indexes in the observation group after treatment were significantly lower than those in the control group after treatment (all P<0.05). The serum levels of 5-HT, PGE2, BK, NPY, TNF-α, IL-1, IL-17 and MMP-3 in the two groups after treatment were significantly lower than those before treatment (all P<0.05), and the above indexes in the observation group were significantly lower than those in the control group (all P<0.05). The incidence of adverse reactions in the observation group was 4.76% (2/42), which was significantly lower than 19.05% (8/42) in the control group (P<0.05). 【Conclusion】The efficacy of dextral ibuprofen (dexibuprofen) in the treatment of JIA is remarkable, which can significantly improve the clinical symptoms and signs of children, inhibit the expression of inflammatory factors and pain-causing mediators, and has good safety.

378 Clinical Efficacy of Modified TiaomianxiaozhengDecoction Combined with Conventional Western Medicinein the Treatment of Cervical High Risk HPV Persistent Infection

【Objective】To explore the effect of modified Tiaomianxiaozheng decoction on cervical high risk human papilloma virus (HR-HPV) persistent infection (spleen deficiency and dampness excess syndrome).【Methods】A total of 94 patients with persistent cervical HR-HPV infection (spleen deficiency and dampness syndrome) were randomly divided into 2 groups with 47 cases in each group. The control group received western medicine treatment, and the observation group was treated with Tiaomianxiaozheng decoction in addition to conventional basis. The traditional Chinese medical(TCM) symptom score, the curative effect of TCM, HPV viral loads and the changes of T lymphocyte subsets of the two groups before and after treatment were compared. The negative HPV conversion rate, clinical efficacy and the incidence of adverse reactions during treatment were compared.【Results】After treatment, the TCM symptom score, HPV viral load and Th17/Treg of the two groups were decreased (P<0.05), CD3+, CD4+ and CD4+/CD8+ were all increased (P<0.05). Compared with the control group, the TCM symptom score, HPV viral load and Th17/Treg of the observation group after treatment were all lower (P<0.05), and CD3+, CD4+ and CD4+/CD8+ were all higher (P<0.05). The total effective rate of TCM, HPV negative conversion rate and total clinical effective rate in the observation group were higher than those in the control group (P<0.05).The incidence of adverse reactions between the two groups during treatment was not different (P>0.05).【Conclusion】Modified Tiaomianxiaozheng decoction combined with conventional western medicine is used to treat HR-HPV persistent infection patients with spleen deficiency and dampness excess syndrome, which can significantly promote the remission of symptoms, reduce the load of HPV virus, regulate the immune function and improve the clinical efficacy. It has obvious curative effect and high safety.

382 Expression, Mutation and Clinical Significance of MUC16 in Non-small Cell Lung Cancer

【Objective】To detect the expression level and mutation of MUC16 in non-small cell lung cancer (NSCLC), and to explore the relationship of expression and mutation of MUC16 with prognosis. 【Methods】A total of 113 patients with NSCLC in our hospital were selected as the research objects. The cancer tissues were in the observation group (A group) and the adjacent "normal" tissues were selected as the control group (B group). Real-time quantitative PCR (qRT-PCR) was used to detect the level of MUC16 mRNA in lung adenocarcinoma and the expression level of MUC16 in lung adenocarcinoma was retrieved from the Ualcan database. The immunohistochemistry was applied to detect the expression level of MUC16 protein. The high throughput sequencing was performed to identify the mutation of MUC16 gene in NSCLC. The diagnostic value of MUC16 mRNA in NSCLC was analyzed and the relationship of prognosis with MUC16 expression level and mutation in patients with NSCLC cancer was analyzed as well. 【Results】In the Ualcan database, the level of MUC16 mRNA in lung adenocarcinoma tissue was shown higher than that in normal lung tissue (P<0.05). The level of MUC16 mRNA and protein positive rate in the A group were higher than those in the B group (P<0.05). The area under the curve (AUC) of MUC16 mRNA level in the diagnosis of NSCLC was 0.882, the cutoff value was 1.213, the specificity was 84.1%, and the sensitivity was 83.2%. The 5-year survival rate of MUC16 positive non-small cell lung cancer patients was 30.14%, which was lower than that of MUC16 negative patients (65.00%, P<0.05). The 5-year survival rate of 22.22% in patients with non MUC16 mutation was lower than that in patients with MUC16 mutation (55.88%, P<0.05). 【Conclusion】MUC16 plays an important role in the occurrence and development of NSCLC cancer, and it has certain diagnostic value. Its expression level and mutation are closely related to the prognosis of NSCLC, which may provide a new potential target for the treatment of NSCLC.

386 Detection Levels and Significance of Serum IFN-γ, IL-10, E2 and β-hCG in Pregnant Syphilis Patients

【Objective】To detect the expression levels of interferon-γ (IFN-γ), interleukin-10 (IL-10), estradiol (E2) and β-human chorionic gonadotropin (β-hCG) in serum of pregnant syphilis (PS) patients, and to analyze their clinical significance. 【Methods】 A total of 92 patients with pregnancy complicated with PS treated in Xi'an Ninth Hospital from July 2018 to July 2020 were selected as PS group. They were divided into good pregnancy outcome group and bad pregnancy outcome group according to pregnancy outcome; In addition, 85 normal pregnant women who underwent prenatal examination in Xi'an Ninth Hospital in the same period were selected as the control group.The expression level o serum IFN-γ, IL-10, E2 and β-hCG of patients in different groups were compared; The expression level serum IFN-γ, IL-10, E2 and β-hCG was analyzed by receiver operating characteristic curve (ROC)in the value of the diagnosis of adverse pregnancy outcomes in patients with PS; Logistic regression was used to analyze the risk factors of adverse pregnancy outcomes in patients with PS.【Results】 The level of IL-10, E2 and β-hCG of the PS patients was significantly lower than that of the control group (P<0.05), and decreased with the increase of syphilis severity, and the expression level of IFN-γwas significantly higher than that in the control group (P<0.05), and increased with the increase of the severity of syphilis; Compared with the good pregnancy outcome group, the expression level of the serum levels of IL-10, E2 and β-hCG in PS patients of the adverse pregnancy outcome group decreased significantly (P<0.05), and the expression level of IFN-γwas significantly increased (P<0.05); ROC curve showed that the area under the curve (AUC) of serum IFN-γ, IL-10, E2 and β-hCG expression level in the diagnosis of adverse pregnancy outcome in patients with PS were 0.865, 0.821, 0.717 and 0.816 respectively. The AUC of adverse pregnancy outcome in PS patients diagnosed by the combination of the four was 0.936, which was significantly higher than that diagnosed by serum IFN-γ, IL-10, E2 and β-hCG expression alone (P<0.05). 【Conclusion】The expression level in serum IFN-γ, IL-10, E2 and β-hCG in PS patients is closely related to the condition of PS and the adverse pregnancy outcome of PS patients, which has a certain diagnostic value for the occurrence of adverse pregnancy outcome.

390 Clinical Efficacy of Ivabradine Combined with Sodium Aescinate in the Treatment of Senile Heart Failure Complicated with Chronic Obstructive Pulmonary Disease

【Objective】 To investigate the clinical efficacy of ivabradine combined with sodium aescinate in elderly patients with heart failure complicated with chronic obstructive pulmonary disease (COPD). 【Methods】 A total of 92 elderly patients with heart failure complicated with COPD treated in the First Affiliated Hospital of Xi'an Jiaotong University from March 2019 to June 2020 were randomly divided into observation group (treated with ivabradine combined with sodium aescinate) and control group (treated with ivabradine), with 46 cases in each group. The cardiac function, pulmonary function and exercise endurance of the two groups were compared before and after treatment, and the adverse reactions of the two groups were recorded. 【Results】After treatment, the left ventricular ejection fraction, left ventricular end diastolic diameter, left ventricular end diastolic volume and ventricular septal diastolic thickness in the two groups were higher than those before treatment, and the difference between the observation group and the control group was statistically significant (P<0.05); The forced vital capacity, forced expiratory volume in the first second and 6-minute walking distance of the two groups were higher than those before treatment (P<0.05), and the observation group was higher than that of the control group (P<0.05). The incidence of adverse reactions in the observation group was 13.04% (6/46), higher than 8.70% (4/46) in the control group, but the difference was not statistically significant (χ2=0.45=0.45, P=0.50>0.05). 【Conclusion 】Ivabradine combined with sodium aescinate can significantly improve the heart and lung function and exercise endurance of elderly patients with heart failure and COPD, with good safety and certain clinical value.

393 Efficacy Comparison of Ceritinib and Crizotinib in the Treatment of Advanced Non-small Cell Lung Cancer with Positive ALK Fusion in Elderly Patients

【Objective】Comparison of the efficacy of ceritinib and crizotinib in the treatment of elderly patients of advanced non-small cell lung cancer (NSCLC) with positive anaplastic lymphoma kinase (ALK) fusion gene.【Methods】A total of 74 patients of advanced NSCLC with positive ALK fusion gene, who admitted to our hospital, were selected as the research objects. According to the treatment plan, patients were divided into A group with ceritinib treatment a(n=44) and B group with crizotinib treatment (n=30). The clinical efficacy, quality of life (EORTC QLQ-C30) and adverse reactions after one course of treatment were compared between the two groups of patients. Survival within 12 months in the two groups of patients was recorded. 【Results】The disease control rate of A group was 84.09%, which was higher than that of B group (63.33%), and the difference was statistically significant (P<0.05). After treatment, the EORTC QLQ-C30 scores of the two groups were both lower than before treatment, and the score of A group was lower than that of B group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05), the 1-year survival rate of A group (68.18%) was higher than that of B group (43.33%), the difference was statistically significant (P<0.05). 【Conclusion】Compared to crizotinib, the treatment of ceritinib in NSCLC elderly patients with positive ALK fusion gene can more effectively improve the patient's disease control rate, improve the quality of life, extend the life cycle and have good safety.

397 Effects of Arthroscopic Meningoplasty Combined with Sodium Hyaluronate Injection on Curative Efficacy, Pain Degree and Knee Range of Motion in Patients with Meniscus Injury

【Objective】To explore the effect of arthroscopic meningoplasty combined with sodium hyaluronate injection on the curative efficacy, degree of pain and range of motion of knee joint in patients with meniscus injury. 【Methods】 A total of 94 patients with meniscus injury treated in the Second Affiliated Hospital of Xi'an Medical College from May 2017 to June 2020 were randomly divided into observation group (treated with arthroscopic meningoplasty combined with sodium hyaluronate injection, 2.5 ml sodium hyaluronate intra-articular injection immediately after surgery, once a week for 4 weeks) and control group (treated with arthroscopic meningoplasty), with 47 cases in each group. The clinical efficacy, degree of pain, range of motion of knee joint and incidence of adverse reactions were compared between the two groups. 【Results】The excellent and good rate of the observation group was higher than that of the control group (93.62% vs 78.72%). Four weeks after operation, the levels of inflammatory factors in knee joint fluid of the two groups decreased significantly compared with those before operation, and the levels of interleukin-6 (IL-6) and interleukin-1β (IL-1 β),tumor necrosis factor-α(TNF-α) and VAS score in the observation group were lower than those in the control group, and the scores of knee activity and knee function were higher than those in the control group (P<0.05). There was no significant difference in the incidence of complications between the two groups (4.26% vs 2.13%) (P>0.05). 【Conclusion】 The combination of arthroscopic meningoplasty and sodium hyaluronate injection is effective in the treatment of meniscus injury, which can effectively reduce inflammation and pain and promote the recovery of knee function.

401 Clinical Effects of Different Intrauterine Devices on Women of Childbearing Age with Scar Uterus

【Objective】To investigate the clinical effect of different intrauterine devices (IUD) on women of childbearing age with scar uterus. 【Methods】 A total of 240 women of childbearing age with IUD placed in scar uterus after cesarean section in our hospital from June 2015 to June 2018 were selected. According to different IUD placement, they were divided into four groups: Uterine-ring IUD group (n=60), T-type IUD group (n=60), Mycu IUD group (n=60) and Gyneflex IN/S IUD group (n=60), with 60 cases in each group. The failure rate after IUD placement, the incidence of adverse reactions and the continuation of IUD for 1 ~ 2 years were compared among the four groups. 【Results】 The postoperative failure rate of the Gyneflex group (1.67%) was significantly lower than that of the Uterine-ring group (8.33%), the T-type group (16.67%) and the Mycu group (8.33%) (P<0.05); The total incidence of adverse reactions of the Geneflex IN/S group (18.33%) was significantly lower than that of the Uterine-ring group (35.00%), the T-type group (41.67%) and the Mycu group (30.00%). The difference was statistically significant (P<0.05); The IUD continuation rate after 2 years of operation in the four groups was lower than that after 1 year of operation. The IUD continuation rate after 2 years of operation in the Genyflex group (88.33%) was significantly higher than that in the Uterine-ring group (61.67%), the T-type group (51.67%) and the Mycu group (70.00%) (P<0.05). 【Conclusion】Compared with Uterine-ring IUD, T-type IUD and Mycu IUD,Gyneflex IN/S IUD has lower failure rate and lower total incidence of adverse reactions after IUD placement with higher continuation rate after 2 years of operation, which is worthy of clinical application.

【Objective】 To investigate the expression level of microRNA-155 host gene (miR-155 HG) in peripheral blood mononuclear cells (PBMC) of patients with psoriasis vulgaris and its relationship with the balance of helper T cell 17/regulatory T lymphocyte (Th17/treg) cells.【Methods】 A total of 92 blood samples from patients with psoriasis vulgaris treated in the Yulin Second Hospital from October 2020 to October 2021 was selected as disease group, and 92 blood samples from healthy people in the same period were selected as control group. The expression levels of miR-155, Hg and miR-155 in PBMC were detected by real-time quantitative PCR (RT-PCR), and the proportion of Th17 and treg cells was detected by flow cytometry, Th17/treg valu was calculated; Serum interleukin-6 (IL-6) and transforming growth factorβ1(TGF-1) were detected by enzyme-linked immunosorbent assay (ELISA) ; The levels of miR-155 and miR-155 HG in PBMC and their relationship with Th17/treg value, IL-6 and TGF-β1 were analyzed by Pearson method.【Results】Compared with the control group, the levels of miR-155, miR-155 HG, IL-6 and Th17/treg in PMBC in the disease group were significantly increased (P<0.05), and serum TGF-β1 decreased (P<0.05). Of patients in the disease group, the level of miR-155 HG in PBMC was positively correlated with that of miR-155 (r=0.573,P=0.000); The level of miR-155 HG was positively correlated with Th17/treg value and IL-6 level (r=0.482, 0.512,P=0.000), and was negatively correlated TGF-β1 (r=-0.596,P=0.000); The level of miR-155 was positively correlated with Th17/treg value and IL-6 level (r=0.694, 0.488,P=0.000), and was negatively correlated with TGF-β1 level (r=-0.509, P=0.000). 【Conclusion】The level of miR-155 HG in PBMC of patients with psoriasis vulgaris is up-regulated, which may be related to the imbalance of secreting miR-155 and Th17/treg cells.

408 Relationship between CYP2D6*10 gene polymorphism and Analgesic Effect of Tramadol During Knee Arthroplasty

【Objective】To investigate the relationship between cytochrome P450 2D6*10 (CYP2D6*10) gene polymorphism and the analgesic effect of tramadol in knee arthroplasty. 【Methods】A total of 345 patients who underwent total knee arthroplasty for the first time in our hospital from June 2016 to June 2021 were selected. CYP2D6*10 genotyping was performed by polymerase chain reaction (PCR) sequencing technology. The clinical data of patients were collected. The knee range of motion and visual analog pain score (VAS) at different time points after total knee arthroplasty were measured, and the adverse reactions were recorded to evaluate the analgesic effect of tramadol. The relationship between CYP2D6*10 gene polymorphism and intraoperative tramadol analgesic effect was evaluated by 2test or Fisher exact probability 【Results】PCR sequencing showed that 83 cases (24.06%) of CYP2D6*10 gene wild type (CC type), 165 cases (47.82%) of heterozygous mutation (CT type) and 97 cases (28.12%) of homozygous mutation (TT type). There was no significant difference in gender, age, height, BMI, operation time, tramadol use and adverse reactions among CC, CT and TT patients (P>0.05). Compared with CC type, CT and TT type patients had lower joint activity and higher VAS score at the same time, and the difference was statistically significant (P<0.05). 【Conclusion】CYP2D6*10 gene polymorphism is closely related to the analgesic effect of tramadol in knee arthroplasty, which may have certain guiding significance for its clinical application.

【Objective】To explore the risk factors of low molecular weight heparin anticoagulant bleeding in patients with end-stage renal disease (ESRD) with atrial fibrillation after hemodialysis treatment. 【Methods】A retrospective analysis of the clinical data of 400 ESRD patients with atrial fibrillation admitted to our hospital was performed. The incidence of low molecular weight heparin anticoagulant bleeding after hemodialysis in ESRD patients with atrial fibrillation was counted. Logistic regression analysis was used to analyze the multiple factors affecting low molecular weight heparin anticoagulant bleeding in patients with ESRD with atrial fibrillation after hemodialysis treatment. 【Results】The incidence of low-molecular-weight heparin anticoagulant bleeding events after hemodialysis in ESRD patients with atrial fibrillation was 7.40% (37/400). There were statistically significant differences between the bleeding group and the non-bleeding group in terms of age, drinking history, comorbid digestive tract disease, weekly dialysis time, platelet count, hypocalcemia, and spicy food eating habit(P<0.05). Logistic multivariate analysis showed that gastrointestinal diseases, weekly dialysis time, and hypocalcemia were independent influencing factors of low molecular weight heparin anticoagulant bleeding after hemodialysis in ESRD patients with atrial fibrillation (P<0.05). 【Conclusion】Gastrointestinal diseases, weekly dialysis time and hypocalcemia are risk factors for low-molecular-weight heparin anticoagulant bleeding in ESRD patients with atrial fibrillation after hemodialysis treatment.

415 Expression and Clinical Significance of Serum MiR-98 and IL-13 in Elderly Patients with Acute Attack of Asthma

【Objective】To investigate the expression level and clinical significance of serum microrna-98(miR-98) and interleukin-13(IL-13) in elderly patients with acute attack of asthma. 【Methods】A total of 360 elderly patients with asthma diagnosed in the Ankang Central Hospital from March 2017 to January 2020 was selected, including 200 cases in the acute attack group (90 cases in the mild group, 80 cases in the moderate group and 30 cases in the severe group) and 160 cases in the remission group; In addition, 90 elderly people who underwent physical examination in the Ankang Central Hospital in the same period were selected as the health group. The relative expression of serum miR-98 was detected by real-time fluorescence quantitative PCR (Quantitative Real-time PCR,qRT-PCR), the level of serum IL-13 was detected by enzyme-linked immunosorbent assay, and the first second forced expiratory volume (FEV1) and maximum peak expiratory flow rate (PEF) in acute attack group were detected and compared by pulmonary function instrument. 【Results】 The expression of serum miR-98 in the acute attack group was significantly lower than that in the remission group and the healthy group, and decreased successively with the aggravation of acute attack (P<0.05); The expression of serum IL-13 was higher than that in the remission group and the healthy group, and increased successively with the aggravation of acute attack (P<0.05). FEV1 and PEF in the mild group, the moderate group and the severe group decreased successively with the aggravation of the disease(P<0.05). The relative expression of miR-98 in serum of patients with the acute attack group was positively correlated with FEV1 and PEF(P<0.05), and negatively correlated with IL-13(P<0.05); The expression level of IL-13 in serum was negatively correlated with FEV1 and PEF (P<0.05).【Conclusion】The abnormal expression of serum miR-98 and IL-13 in patients with acute attack of asthma is related to the degree of attack and the indexes of pulmonary function FEV1 and PEF. It can be used as a potential index to evaluate the elderly asthma.

419 Effect of Intelligent Knee Rehabilitation Instrument Combined with Continuous Passive Activity on Postoperative Rehabilitation of Tibial Plateau Fracture

【Objective】To analyze the effect of intelligent CPM rehabilitation instrument combined with continuous passive activity on postoperative rehabilitation of tibial plateau fracture. 【Methods】 A total of 84 patients with tibial plateau fractures treated in our hospital from May 2019 to may 2021 were randomly divided into observation group and control group, with 42 cases in each group. The control group was given continuous passive activity rehabilitation training, while the observation group was treated with intelligent CPM rehabilitation instrument on the basis of the control group. The degree of pain, knee swelling and improvement of knee function were compared between the two groups after intervention with different rehabilitation training programs. 【Results】 After 2, 4, 6 and 8 weeks of treatment, the swelling degree of knee joint in the two groups was lower than that before operation (P<0.05); The swelling degree of knee joint in the observation group was lower than that in the control group, and there was significant difference between the two groups (P<0.05). The postoperative visual analogue pain score (VAS) of the observation group was significantly lower than that of the control group, while the score of HSS knee scale was significantly higher than that of the control group (P<0.05). 【Conclusion】 Intelligent CPM rehabilitation instrument combined with continuous passive activity training can promote the postoperative rehabilitation of tibial plateau fracture, effectively remove knee edema, reduce the degree of pain and improve knee function.

422 Analysis of Retcam Ⅲ Fundus Screening Results of 3987 Neonates

【Objective】Based on data from the utilization of wide-area digital fundus imaging system (Retcam Ⅲ) for full-term neonates fundus screening, this thesis aims to explore its application value by systematic analyzing clinical data arrays.【Methods】Clinical data of 3987 full-term newborns in Hunan Provincial Maternal and Child Health Care Hospital from January 2017 to December 2019 were retrospectivelyanalyzed to link Retcam Ⅲ fundus screening results with birth weight and delivery methods. 【Results】Among the 3987 full-term newborns who completed the Retcam Ⅲ examination, a total of 1020 of them had fundus lesions, therefore the abnormality rate was 25.58%.Among those fundus neonates, there were 736 cases of retinal hemorrhage(18.46%), 233 cases of retinal exudative changes (5.84%), 19 cases of suspected familial exudative vitreoretinopathy(0.48%), 13 cases of retinal pigmented nevus(0.33%), 8 cases of albinism with fundus change(0.20%), 6 cases of loss of retinal pigment(0.15%), 6 cases of persistent fetal vasculature (0.15%),3 cases of choroidal coloboma(0.08%),2 cases of retinoschisis (0.05%) and 1 case of retinoblastoma(0.03%). Neonatal retinal hemorrhage was associated with mode of delivery and birth weight (P<0.05).The incidence of blinding eye diseases accounted for 0.78% (31/39870), including 19 cases of familial exudative vitreoretinopathy, 6 cases of persistent fetal vasculature, 3 cases of choroidal coloboma, 2 cases of retinoschisis and 1 case of retinoblastoma.【Conclusion】Various types of fundus lesions can appear in full-term neonates with a rather high rate of abnormal screening incidence. The utilization of Retcam Ⅲ is a very efficient method for fundus defects early detection and diagnosis to give patients a better chance to avoid blindness and low vision.

425 Effect of Hemodialysis with Different Fluxes on Cardiac Function, Myocardial Enzymes and prognosis in Elderly Patients with Diabetic Nephropathy

【Objective】To investigate the effects of hemodialysis with different fluxes on cardiac function, myocardial enzymes and prognosis in elderly patients with diabetic nephropathy (DN). 【Methods】 The clinical data of 88 elderly patients with DN admitted to the hospital were retrospectively analyzed. There were 46 patients treated with high-throughput hemodialysis in group A and in the same period 42 patients were treated with low-throughput hemodialysis in group B. Cardiac function indicators: left ventricular ejection fraction (LVEF), left ventricular end diastolic diameter (LVEDD), interventricular septal diastolic thickness (IVSDT), Myocardial enzyme indicators such as creatine phosphokinase (CK), lactate dehydrogenase (LDH), hydroxybutyrate dehydrogenase (HBDH) and blood pressure were compared before and after treatment in two groups. The incidence of cardiovascular events in the 12 months of follow-up were statistically analyzed. The survival at 6 months and 12 months during the follow-up period were counted. 【Results】There was no significant difference in each indicator between the two groups before treatment (P>0.05). LVEF level was increased after treatment in both groups (P<0.05), and LVEF level in group A was significantly higher than that in group B (P<0.05). After treatment, LVEDD and IVSDT in both groups were reduced (P<0.05), and levels of LVEDD and IVSDT in group A were significantly lower than those in group B (P<0.05). CK, LDH and HBDH levels in the two groups were all decreased after treatment (P<0.05), while levels of CK, LDH and HBDH in group A were significantly lower than those in group B (P<0.05). There was no significant difference in systolic and diastolic blood pressure between the two groups before and after treatment (P>0.05). The total incidence rate of cardiovascular events in group A was lower than that in group B (P<0.05). There was no significant difference in 6-month survival rate between the two groups(P>0.05), while the 12-month survival rate in group A was higher than that in group B (P<0.05). 【Conclusion】High-throughput hemodialysis for elderly patients with DN can effectively improve their cardiac function, reduce the level of myocardial enzymes and the incidence of cardiovascular events, and improve the survival rate.

429 Diagnostic Value of Transabdominal and Transperineal Ultrasonography in Cervical Insufficiency

【Objective】To explore the value of transabdominal and transperineal ultrasonography in the diagnosis of cervical insufficiency.【Methods】A total of 84 pregnant women with suspected cervical insufficiency treated in our hospital from January 2017 to June 2019 were randomly divided into transabdominal ultrasound group (transabdominal ultrasound, n=42) and transperineal ultrasound group (transperineal ultrasound group, n=42).Two examination methods were used to compare the projection of amniotic sac into the cervix and the degree of opening of cervical canal, and the diagnostic value of transabdominal ultrasonography and transperineal ultrasonography in cervical insufficiency was analyzed. 【Results】The detection rate of amniotic sac protruding into the cervix in transabdominal ultrasound group was 9.52% (4/42), and there was no significant difference compared with 14.29% (6/42) in the transperineal ultrasound group(χ2=0.454,P=0.500). There was no significant difference between transabdominal ultrasonography and transperineal ultrasonography in the diagnosis of the degree of cervical opening (P>0.05). Compared with the transabdominal ultrasound group, the sensitivity, specificity and diagnostic rate of the transperineal ultrasound group increased, and the missed diagnosis rate and misdiagnosis rate decreased. 【Conclusion】Compared with transabdominal ultrasonography, transperineal ultrasonography has a higher diagnostic rate of cervical insufficiency, and the missed diagnosis rate and misdiagnosis rate are reduced. It is worthy of clinical application.

432 Expression of MLF1IP in Bladder Urothelial Carcinoma and its Clinical Significance

【Objective】To investigate the expression of human myeloid leukemia factor 1-interacting protein gene (MLF1IP) in patients with bladder urothelial carcinoma. 【Methods】A total of 52 patients with bladder urothelial carcinoma who underwent surgery in our hospital from February 2015 to February 2017 were selected. The resected cancer tissues and adjacent normal tissues were taken and divided into bladder urothelial carcinoma group (observation group) and adjacent group (control group). The expression levels of MLF1IP mRNA and MLF1IP in adjacent tissues and cancer tissues of all subjects were detected, the relationship between MLF1IP mRNA expression level and clinicopathological features of bladder urothelial carcinoma was analyzed, the effect of MLF1IP mRNA expression level on the prognosis of patients with bladder urothelial carcinoma was evaluated, and the related factors affecting the prognosis of patients with bladder urothelial carcinoma were analyzed. 【Results】The positive expression rate of MLF1IP, MLF1IP mRNA and MLF1IP level in the observation group were significantly higher than those in the control group (P<0.05). The expression of MLF1IP mRNA in patients with bladder urothelial carcinoma was related to the pathological grade and clinical stage of bladder urothelial carcinoma (P<0.05), but not to the patient's gender, age and tumor diameter (P>0.05). The 3-year overall survival rate of patients with high expression of MLF1IP mRNA was lower than that of patients with low expression of MLF1IP mRNA (P<0.05). The high expression of MLF1IP mRNA and pathological grade were independent risk factors affecting the prognosis of patients with bladder urothelial carcinoma (P<0.05). 【Conclusion】The high expression of MLF1IP in patients with bladder urothelial carcinoma is related to the pathological grade, clinical stage and prognosis. It is of great significance to evaluate the prognosis of patients.

435 Correlation Between Serum Levels of BMP6, TGF-β1 and Severity of Coronary Artery Disease in Patients with Coronary Heart Disease

【Objective】To investigate the relationship between serum levels of bone morphogenetic protein 6 (BMP6) and transforming growth factor beta-1 (TGF-β1) in patients with coronary heart disease (CHD). 【Methods】90 patients with CHD diagnosed and treated in our hospital from December 2017 to October 2020 were selected as CHD group, and 90 healthy persons in physical examination in the same period were selected as healthy group. Serum BMP6, creatine kinase MB (CK-MB), TGF-β1, cardiac troponin I (cTnI) were compared between the two groups; the relationship between serum BMP6, TGF-β1 levels and CK-MB, cTnI in CHD patients, and the correlation between serum BMP6 levels and TGF-β1 were analyzed; the serum BMP6 and TGF-β1 levels in patients with CHD with different diseased branches were compared; the value of serum BMP6 and TGF-β1 levels in the diagnosis of CHD were evaluated. 【Results】Serum CK-MB, BMP6 and cTnI levels in CHD group were higher than those in healthy group (P<0.05), and TGF-β1 levels were lower than healthy group (P<0.05); Serum BMP6 levels in CHD patients were positively correlated with CK-MB and cTnI (P<0.05), and negatively correlated with TGF-β1 (P<0.05); serum TGF-β1 levels in CHD patients were negatively correlated with CK-MB and cTnI (P<0.05); with the increase of the number of coronary artery lesions (aggravation), the level of serum BMP6 increased gradually (P<0.05), and the level of TGF-β1 gradually decreased (P<0.05); there was a negative correlation between serum BMP6 level and TGF-β1 in CHD patients (P<0.05); the area under the curve (AUC) of serum BMP6 and TGF-β1 in the diagnosis of CHD was 0.899 and 0.845, the cut-off value was 35.36 pg/mL and 104.99 pg/mL, respectively, the both sensitivity was 88.9%, and specificity was 77.8% and 72.2% respectively; the AUC of the combined diagnosis was 0.939, the corresponding sensitivity was 86.7%, and the specificity was 90.0%.【Conclusion】The level of serum BMP6 in CHD patients is increased, and the level of TGF-β1 is decreased, they are related to the severity of coronary artery disease. The combination of BMP6 and TGF-β1 can improve the diagnostic value of CHD and contribute to the clinical diagnosis of CHD.

439 Clinical Efficacy of Different Doses Febuxostat in the Treatment of Uric Acid End-stage Nephropathy

【Objective】To investigate the clinical effects of febuxostat at different doses in the treatment of uric acid end-stage nephropathy (UAESN). 【Methods】A total of 98 UAESN patients treated in our hospital were selected and randomly divided into the observation group (n=49) and the control group (n=49).On the basis of conventional treatment, the observation group was given 20 mg febuxostat, and the control group was given 40mg febuxostat. Serum creatinine (SCr), glomerular filtration rate (eGFR), urinary microalbumin to creatinine ratio (ACR) and serum uric acid (SUA) were observed before and after treatment. Plasma viscosity (PV), whole blood low shear viscosity (ηL), whole blood high shear viscosity (ηH), malondialdehyde (MDA), advanced oxide protein product (AOPP), superoxide dismutase (SOD) and other oxidative stress indicators were recorded. The efficacy and adverse reactions of the two groups were compared. 【Results】Compared with before treatment, SCr and ACR were decreased, eGFR and SOD were increased, SUA, PV, ηL, ηH, MDA and AOPP were decreased after treatment (P<0.05). After treatment, there were no significant differences in SCr, ACR, eGFR, SUA, PV, ηL, ηH, MDA, AOPP and SOD between the two groups (P>0.05). There was no significant difference between the clinical efficacy of the observation group (91.84%) and the control group (95.92%) (P>0.05). The incidence of adverse reactions in the observation group was lower than that in the control group (P<0.05). 【Conclusion】Based on conventional treatment, 20 mg febuxostat has the same effect as 40 mg febuxostat in aspect of improving renal function, hemorheology, relieving oxidative stress and clinical efficacy of UAESN, however, 20 mg febuxostat has better safety.
